The deep inspiration breath hold (DIBH) technique is effective for heart dose reduction in patients with left-sided breast cancer. In deep breathing, some women breathe in thoracic respiration; and others, in abdominal respiration. This study evaluated differences in dose reduction in organs at risk (OAR) and reproducibilities of the target and OAR between thoracic DIBH (T-DIBH) and abdominal DIBH (A-DIBH). Fourteen patients with left-sided breast cancer who had planned to receive whole-breast irradiation were included. Computed tomography (CT) was performed in free breathing (FB), T-DIBH, and A-DIBH, and the dosimetric indexes of the target and OAR for three treatment plans were compared. Purpose To evaluate the effect of setup uncertainties including uncertainties between different breath holds (BH) and inter-fractional anatomical changes under CT-guided BH with intensity-modulated proton therapy (IMPT) in patients with liver cancer. Methods and materials This retrospective study considered 17 patients with liver tumors who underwent feedback-guided BH (FGBH) IMRT treatment with daily CT-on-rail imaging. Planning CT images were acquired at simulation using FGBH, and FGBH CT-on-rail images were also acquired prior to each treatment. Selective robust IMPT plans were generated using planning CT and re-calculated on each daily CT-on-rail image. Subsequently, the fractional doses were deformed and accumulated onto the planning CT according to the deformable image registration between daily and planning CTs. The doses to the target and organs at risk (OARs) were compared between IMRT, planned IMPT, and accumulated IMPT doses. Results For IMPT plans, the mean of D98% of CTV for all 17 patients was slightly reduced from the planned dose of 68.90 ± 1.61 Gy to 66.48 ± 1.67 Gy for the accumulated dose. The target coverage could be further improved by adjusting planning techniques. The dose–volume histograms of both planned and accumulated IMPT doses showed better sparing of OARs than that of the IMRT. Conclusions IMPT with FGBH and CT-on-rail guidance is a robust treatment approach for liver tumor cases.

Background Clinical guidelines recommend the use of established T2 mapping sequences to detect and quantify myocarditis and edema, but T2 mapping is performed in two dimensions with limited coverage and repetitive breath holds. Purpose To assess the reproducibility of an accelerated free-breathing three-dimensional (3D) whole-heart T2 MRI mapping sequence in phantoms and participants without a history of cardiac disease and to investigate its clinical performance in participants with suspected myocarditis. Materials and Methods Eight participants (three women, mean age, 31 years ± 4 [standard deviation]; cohort 1) without a history of cardiac disease and 25 participants (nine women, mean age, 45 years ± 17; cohort 2) with clinically suspected myocarditis underwent accelerated free-breathing 3D whole-heart T2 mapping with 100% respiratory scanning efficiency at 1.5 T. The participants were enrolled from November 2018 to August 2020. Three repeated scans were performed on 2 separate days in cohort 1. Segmental variations in T2 relaxation times of the left ventricular myocardium were assessed, and intrasession and intersession reproducibility were measured. In cohort 2, segmental myocardial T2 values, detection of focal inflammation, and map quality were compared with those obtained from clinical breath-hold two-dimensional (2D) T2 mapping. Statistical differences were assessed using the nonparametric Mann-Whitney and Kruskal-Wallis tests, whereas the paired Wilcoxon signed-rank test was used to assess subjective scores. Results Whole-heart T2 maps were acquired in a mean time of 6 minutes 53 seconds ± 1 minute 5 seconds at 1.5 mm3 resolution. Breath-hold 2D and free-breathing 3D T2 mapping had similar intrasession (mean T2 change of 3.2% and 2.3% for 2D and 3D, respectively) and intersession (4.8% and 4.9%, respectively) reproducibility. The two T2 mapping sequences showed similar map quality (P = .23, cohort 2). Abnormal myocardial segments were identified with confidence (score 3) in 14 of 25 participants (56%) with 3D T2 mapping and only in 10 of 25 participants (40%) with 2D T2 mapping. Conclusion High-spatial-resolution three-dimensional (3D) whole-heart T2 mapping shows high intrasession and intersession reproducibility and helps provide T2 myocardial characterization in agreement with clinical two-dimensional reference, while enabling 3D assessment of focal disease with higher confidence. © RSNA, 2021 Online supplemental material is available for this article. Purpose: Current cardiovascular magnetic resonance (CMR) examinations require expert planning, multiple breath holds, and 2D imaging. To address this, we sought to develop and validate a comprehensive free -breathing 3D cine function and flow CMR examination using a steady-state free precession (SSFP) sequence to depict anatomy fused with a spatially registered phase contrast (PC) sequence for blood flow analysis.Methods: In a prospective study, 25 patients underwent a CMR examination which included a 3D cine SSFP sequence and a 3D cine PC (also known as 4D flow) sequence acquired during free-breathing and after the administration of a gadolinium-based contrast agent. Both 3D sequences covered the heart and mediastinum, and used retrospective vectorcardiogram gating (20 phases/beat interpolated to 30 phases/beat) and prospective respiratory motion compensation confining data acquisition to end-expiration. Cardiovascular measurements derived from the 3D cine SSFP and PC images were then compared with those from standard 2D imaging.Results: All 3D cine SSFP and PC acquisitions were completed successfully. The mean time for the 3D cine sequences including prescription was shorter than that for the corresponding 2D sequences (21 min vs. 36 min, P-value < 0.001). Background Respiratory motion management with breath hold for patients with hepatobiliary cancers remain a challenge in the precise positioning for radiotherapy. We compared different image-guided alignment markers for estimating positional errors, and investigated the factors associated with positional errors under breath-hold control. Methods Spirometric motion management system (SDX) for breath holds was used in 44 patients with hepatobiliary tumor. Among them, 28 patients had a stent or embolized materials (lipiodol) as alignment markers. Cone-beam computed tomography (CBCT) and kV-orthogonal images were compared for accuracy between different alignment references. Breath-hold level (BHL) was practiced, and BHL variation (ΔBHL) was defined as the standard deviation in differences between actual BHLs and baseline BHL. Mean BHL, ΔBHL, and body-related factors were analyzed for the association with positional errors. Results Using the reference CBCT, the correlations of positional errors were significantly higher in those with stent/lipiodol than when the vertebral bone was used for alignment in three dimensions. Patients with mean BHL > 1.4 L were significantly taller (167.6 cm vs. 161.6 cm, p = 0.03) and heavier (67.1 kg vs. 57.4 kg, p = 0.02), and had different positional error in the craniocaudal direction (− 0.26 cm [caudally] vs. + 0.09 cm [cranially], p = 0.01) than those with mean BHL 0.03 L. Conclusion Under rigorous breath-hold respiratory control, BHL correlated with body weight and height. With more accurate alignment reference by stent/lipiodol, actual BHL but not breath-hold variation was associated with craniocaudal positional errors.

PURPOSE To demonstrate the feasibility of compressed sensing (CS) to accelerate the acquisition of hyperpolarized (HP) 129 Xe multi-b diffusion MRI for quantitative assessments of lung microstructural morphometry. METHODS Six healthy subjects and six chronic obstructive pulmonary disease (COPD) subjects underwent HP 129 Xe multi-b diffusion MRI (b = 0, 10, 20, 30, and 40 s/cm2 ). First, a fully sampled (FS) acquisition of HP 129 Xe multi-b diffusion MRI was conducted in one healthy subject. The acquired FS dataset was retrospectively undersampled in the phase encoding direction, and an optimal twofold undersampled pattern was then obtained by minimizing mean absolute error (MAE) between retrospective CS (rCS) and FS MR images. Next, the FS and CS acquisitions during separate breath holds were performed on five healthy subjects (including the above one). Additionally, the FS and CS synchronous acquisitions during a single breath hold were performed on the sixth healthy subject and one COPD subject. However, only CS acquisitions were conducted in the rest of the five COPD subjects. Finally, all the acquired FS, rCS and CS MR images were used to obtain morphometric parameters, including acinar duct radius (R), acinar lumen radius (r), alveolar sleeve depth (h), mean linear intercept (Lm ), and surface-to-volume ratio (SVR). The Wilcoxon signed-rank test and the Bland-Altman plot were employed to assess the fidelity of the CS reconstruction. Moreover, the t-test was used to demonstrate the effectiveness of the multi-b diffusion MRI with CS in clinical applications. RESULTS The retrospective results demonstrated that there was no statistically significant difference between rCS and FS measurements using the Wilcoxon signed-rank test (P > 0.05). Good agreement between measurements obtained with the CS and FS acquisitions during separate breath holds was demonstrated in Bland-Altman plots of slice differences. Specifically, the mean biases of the R, r, h, Lm , and SVR between the CS and FS acquisitions were 1.0%, 2.6%, -0.03%, 1.5%, and -5.5%, respectively. Good agreement between measurements with the CS and FS acquisitions was also observed during the single breath-hold experiments. Purpose Contrary to what is commonly assumed, organs continue to move during breath-holding. We investigated the influence of lung volume on motion magnitude during breath-holding and changes in velocity over the duration of breath-holding. Materials and methods Sixteen healthy subjects performed 60-second inhalation breath-holds in room-air, with lung volumes of ∼100% and ∼70% of the inspiratory capacity, and exhalation breath-holds, with lung volumes of ∼30% and ∼0% of the inspiratory capacity. During breath-holding, we obtained dynamic single-slice magnetic-resonance images with a time-resolution of 0.6s. We used 2-dimensional image correlation to obtain the diaphragmatic and pancreatic velocity and displacement during breath-holding. Results Organ velocity was largest in the inferior–superior direction and was greatest during the first 10s of breath-holding, with diaphragm velocities of 0.41mm/s, 0.29mm/s, 0.16mm/s and 0.15mm/s during BH 100% , BH 70% , BH 30% and BH 0% , respectively. Organ motion magnitudes were larger during inhalation breath-holds (diaphragm moved 9.8 and 9.0mm during BH 100% and BH 70% , respectively) than during exhalation breath-holds (5.6 and 4.3mm during BH 30% and BH 0% , respectively). Conclusion Using exhalation breath-holds rather than inhalation breath-holds and delaying irradiation until after the first 10s of breath-holding may be advantageous for irradiation of abdominal tumors.

PURPOSE To present an image registration method for renal blood oxygen level-dependent (BOLD) measurements that enables semiautomatic assessment of parenchymal and medullary R2* changes under a functional challenge. METHODS In a series of breath-hold acquisitions, three-dimensional data were acquired initially for prospective image registration of subsequent BOLD measurements. An algorithm for kidney alignment for BOLD renal imaging (KALIBRI) was implemented to detect the positions of the left and right kidney so that the kidneys were acquired in the subsequent BOLD measurement at consistent anatomical locations. Residual in-plane distortions were corrected retrospectively so that semiautomatic dynamic R2* measurements of the renal cortex and medulla become feasible. KALIBRI was tested in six healthy volunteers during a series of BOLD experiments, which included a 600- to 1000-mL water challenge. RESULTS Prospective image registration and BOLD imaging of each kidney was achieved within a total measurement time of about 17 s, enabling its execution within a single breath-hold. KALIBRI improved the registration by up to 35% as found with mutual information measures. In four volunteers, a medullary R2* decrease of up to 40% was observed after water ingestion. CONCLUSION KALIBRI improves the quality of two-dimensional time-resolved renal BOLD MRI by aligning local renal anatomy, which allows for consistent R2* measurements over many breath-holds. To determine the limits of agreement of hepatic fat fraction and R2* relaxation rate quantified with accelerated magnetic resonance (MR) imaging reconstructed with combined compressed sensing and parallel imaging compared with conventional fully sampled acquisitions.Eleven subjects with type 2 diabetes and a healthy control subject were recruited with the approval of the Newcastle and North Tyneside 2 ethics committee and written consent. Undersampled data at ratios of 2.6×, 2.9×, 3.8×, and 4.8× were prospectively acquired in addition to fully sampled data by using five gradient echoes per repetition time at 3.0 T. Fat fraction maps were calculated by using combined compressed sensing and parallel imaging (CS-PI) reconstruction and Bland-Altman analysis performed to assess bias and 95% limits of agreement. Inter- and intrarater analysis was performed for quantitative fat fraction and R2* relaxation rate, and image quality was assessed with a four-point scale by two independent observers.The fat fractions from the accelerated acquisitions had 95% limits of agreement of 1.2%, 1.2%, 1.1%, and 1.5%, respectively, with no bias. When compared with the intra- and interrater 95% limits of agreement (0.7% and 0.8%), acceleration of up to 3.8× did not greatly degrade the fat fraction measurements. No or minimal artifact was detected at 2.6× and 2.9× accelerations, moderate artifact was detected at 3.8× acceleration, and substantial artifact was detected at 4.8× acceleration.Prospective undersampling and CS-PI reconstruction of liver fat fractions can be used to accelerate liver fat fraction measurements. The fat fractions and image quality produced were acceptable up to a factor of 3.8×, thereby shortening the required breath-hold duration from 17.7 seconds to 4.7 seconds.

Background and purpose To report preliminary results of passive breath gating (PBG) equipment for cone-beam CT image-guided gated RapidArc gastric cancer treatments. Material and methods Home-developed PBG equipment integrated with the real-time position management system (RPM) for passive patient breath hold was used in CT simulation, online partial breath hold (PBH) CBCT acquisition, and breath-hold gating (BHG) RapidArc delivery. The treatment was discontinuously delivered with beam on during BH and beam off for free breathing (FB). Pretreatment verification PBH CBCT was obtained with the PBG-RPM system. Additionally, the reproducibility of the gating accuracy was evaluated. Results A total of 375 fractions of breath-hold gating RapidArc treatments were successfully delivered and 233 PBH CBCTs were available for analysis. The PBH CBCT images were acquired with 2–3 breath holds and 1–2 FB breaks. The imaging time was the same for PBH CBCT and conventional FB CBCT (60 s). Compared to FB CBCT, the motion artifacts seen in PBH CBCT images were remarkably reduced. The average BHG RapidArc delivery time was 103 s for one 270-degree arc and 269 s for two full arcs. Conclusions The PBG-RPM based PBH CBCT verification and BHG RapidArc delivery was successfully implemented clinically. The BHG RapidArc treatment was accomplished using a conventional RapidArc machine with high delivery efficiency.

We present an evaluation of an MRI-guided radiotherapy system's gating performance using an MRI-compatible respiratory motion phantom and radiochromic film. Our evaluation is geared toward validation of our institution's clinical gating protocol which involves planning to a target volume formed by expanding 5 mm about the gross tumor volume (GTV) and gating based on a 3 mm window about the GTV.MethodsThe motion phantom consisted of a target rod containing high-contrast target inserts which moved in the superior-inferior direction inside a body structure containing background contrast material. The target rod was equipped with a radiochromic film insert. Treatment plans were generated for a 3 cm diameter spherical planning target volume, and delivered to the phantom at rest and in motion with and without gating. Both sinusoidal trajectories and tumor trajectories measured during MRI-guided treatments were used. Similarity of the gated dose distribution to the planned, motion-frozen, distribution was quantified using the gamma technique.ResultsWithout gating, gamma pass rates using 4%/3 mm criteria were 22-59% depending on motion trajectory. Using our clinical standard of repeated breath holds and a gating window of 3 mm with 10% target allowed outside the gating boundary, the gamma pass rate was 97.8% with 3%/3 mm gamma criteria. Using a 3 mm window and 10% allowed excursion, all of the patient tumor motion trajectories at actual speed resulting in at least 95% gamma pass rate at 4%/3 mm.ConclusionsOur results suggest that the device can be used to compensate respiratory motion using a 3 mm gating margin and 10% allowed excursion results in conjunction with repeated breath holds. Full clinical validation requires a comprehensive evaluation of tracking performance in actual patient images, outside the scope of this study.

Purpose: Motion must be accounted for with focal treatments such as liver directed stereotactic body radiotherapy otherwise there is a risk of marginal miss and/or increased toxicity to surrounding structure. There have been many proposed methods for motion management including respiratory gating, inhibition and tracking. The purpose of this study is to evaluate a pressure-based respiratory motion management system with biofeedback to the patient for MR-guided radiotherapy treatments. Methods: A respiratory motion management belt was modified in house to provide visual biofeedback of subject respiratory trace using pressure readings from an air bladder. Three groups participated in the study: Group I included two investigators who were imaged twice, with imaging sessions separated by one week, while wearing the motion management belt. Group II included four healthy volunteers, imaged using the belt for CINE and anatomical imaging. Group III included two cancer patients with liver lesions imaged while wearing the device, imaged pre-radiotherapy treatment, and post-treatment. During image acquisition, a display showed participants their respiratory traces and guidelines to indicate the desired respiratory amplitude. Results: The average peak-to-peak motion of the liver dome was decreased from 30.55 ± 7.83 mm to 5.58 ± 0.85 mm for free-breathing and utilizing a small window guiding box with visual guidance CINE image sets respectively in the investigator study. Volunteers and cancer patients were able to consistently manage their breath traces, including sequential breath holds, over the course of 30–60 min imaging sessions. Conclusions: The proposed motion management belt with visual biofeedback design is able to provide greatly reduced respiratory motion of the liver, and repeatable breath hold in MR simulation. The components used in the device also allow it to be in place during all steps of the radiotherapy planning process, including MR anatomical imaging, CT simulation, and treatment delivery due to the absence of high z or metallic material.

The development of hybrid medical imaging scanners has allowed imaging with different detection modalities at the same time, providing different anatomical and functional information within the same physiological time course with the patient in the same position. Until now, the acquisition of proton MRI of lung anatomy and hyperpolarised gas MRI of lung function required separate breath-hold examinations, meaning that the images were not spatially registered or temporally synchronised. We demonstrate the spatially registered concurrent acquisition of lung images from two different nuclei in vivo. The temporal and spatial registration of these images is demonstrated by a high degree of mutual consistency that is impossible to achieve in separate scans and breath holds. This paper describes an algorithm for extracting pulmonary vascular trees (arteries plus veins) from three-dimensional (3D) thoracic computed tomographic (CT) images. The algorithm integrates tube enhancement filter and traversal approaches which are based on eigenvalues and eigenvectors of a Hessian matrix to extract thin peripheral segments as well as thick vessels close to the lung hilum. The resultant algorithm was applied to a simulation data set and 44 scans from 22 human subjects imaged via multidetector-row CT (MDCT) during breath holds at 85% and 20% of their vital capacity. A quantitative validation was performed with more than 1000 manually identified points selected from inside the vessel segments to assess true positives (TPs) and 1000 points randomly placed outside of the vessels to evaluate false positives (FPs) in each case. On average, for both the high and low volume lung images, 99% of the points was properly marked as vessel and 1% of the points were assessed as FPs. Our hybrid segmentation algorithm provides a highly reliable method of segmenting the combined pulmonary venous and arterial trees which in turn will serve as a critical starting point for further quantitative analysis tasks and aid in our overall goal of establishing a normative atlas of the human lung.

Abdominal imaging with multi-detector row computed tomography (CT) can be performed during short breath holds. On 16-channel multi-detector row CT scanners, the effective detector row thickness, depending on the manufacturer, is typically 1.0-1.5 mm, which results in a beam collimation of 16-24 mm. At a gantry rotation speed of 0.5 second and a pitch of 1, the table travel speed will be 32-48 mm/sec. At a smaller effective detector row thickness and a narrower beam collimation, a slightly higher pitch may be needed to obtain short-breath-hold CT scans. Typically, transverse scans are viewed at a reconstructed section thickness of 3-5 mm, with thinner sections used for CT angiography and whenever off-axial reformations are obtained. The radiologic technique should be optimized according to the transverse section thickness used for interpretation, and the contrast material administration protocol should be optimized according to the clinical problem, with the scanning triggered for enhancement of a target organ.

A new magnetic resonance (MR) sequence was developed to acquire real-time images in a multi-slice dynamic imaging mode to cover the complete heart in 15 seconds without the need for electrocardiogram (ECG) triggering and multiple breath holds. In 34 patients, left ventricular function was assessed with the new technique and a standard technique. The new technique proved to be feasible and accurate for functional cardiac examinations. J. Background Respiratory gating in positron emission tomography (PET) is used to improve detection of small tumors in the lower lung regions and in the liver, and to obtain a better estimate of the standardized uptake value (SUV). Purpose To develop a time-efficient method for acquisition of respiratory-gated PET/CT that would produce one single high quality image volume corresponding to a breath-hold state. Material and Methods An instrument was developed that displayed to the patient either red or green numbers, counting down from a chosen maximum to one at a rate of one dial per second. The patient was instructed to repeatedly hold the breath in moderate inspiration when red numbers were displayed and to breathe freely during display of green numbers. PET data were acquired in list mode and trigger signals were sent to the scanner and inserted into the list file each time the color of the countdown numbers switched from green to red. Data acquired during breath-holds were used to create one single image volume. Results High quality breath-hold images were obtained from 10 min data acquisition at one bed position. Improved image quality compared to standard whole-body PET was demonstrated by a significant reduction of noise (standard deviation) in regions of normal liver tissues. Conclusion The instruction to perform repeated breath-holds was well understood by patients and they cooperated satisfactorily. When the new procedure is used the duration of the data acquisition may typically be reduced by a factor of 4 compared to conventional respiratory-gated protocols where the patient breathes freely.

Purpose: ABC is a breath-hold technique for SBRT treatments. At our institution, three CTs with ABC are acquired at simulation to check the breath-hold consistency because tumor position may deviate among three scans. The purpose of this study was to determine whether RPM can predict tumor position deviation in SBRT breath-hold treatments with ABC. Methods: Five SBRT lung and five SBRT liver patients were selected. An RPM block was placed on the patients between the xiphoid and the umbilicus. RPM recorded patients’ anterior-posterior skin surface positions during breath holds with ABC. For each patient, three CTs (ABC1, ABC2, ABC3) were acquired at the same breath-hold level. ABC2 and ABC3 CTs were rigidly registered to ABC1 CT. Tumor position deviations were measured on the fused CTs (ABC2 with ABC1, ABC3 with ABC1) and compared to the corresponding RPM position shifts. Linear regression, k-means clustering, and decision tree classifier were used to determine the relationship between RPM position shifts and tumor position deviations. Results: Tumor position deviations were 0.4–8.7 mm for lung patients and 1.6–9.1 mm for liver patients. There were no significant linear correlations between RPM position shifts and tumor position deviations for both lung (R2=0.20) and liver (R2=0.45) patients. In contrast, decision tree classification showed that the RPM position shift of 3.45 mm or more indicated tumor position deviation >5 mm for lower lobe lung tumors and liver tumors that are close to the diaphragm. Conclusion: RPM position shifts are not linearly correlated with tumor position deviations. However, RPM can be used to predict large tumor position deviations (>5 mm) among three ABC CTs in SBRT for lower lobe lung tumors and liver tumors and to validate the breath-hold consistency with the ABC technique. Therefore, RPM has the potential to replace the additional verification CT scans.

Purpose: For African-American patients receiving breast radiotherapy with a bolus, skin darkening can affect the surface visualization when using optical imaging for daily positioning and gating at deep-inspiration breath holds (DIBH). Our goal is to identify a region-of-interest (ROI) that is robust against deteriorating surface image quality due to skin darkening. Methods: We study four patients whose post-mastectomy surfaces are imaged daily with AlignRT (VisionRT, UK) for DIBH radiotherapy and whose surface image quality is degraded toward the end of treatment. To simulate the effects of skin darkening, surfaces from the first ten fractions of each patient are systematically degraded by 25–35%, 40–50% and 65–75% of the total area of the clinically used ROI_ipsilateral-chestwall. The degraded surfaces are registered to the reference surface in six degrees-of-freedom. To identify a robust ROI, three additional reference ROIs — ROI_chest+abdomen, ROI_bilateral-chest and ROI_extended-ipsilateral-chestwall are created and registered to the degraded surfaces. Differences in registration using these ROIs are compared to that using ROI_ipsilateral-chestwall. Results: For three patients, the deviations in the registrations to ROI_ipsilateral-chestwall are > 2.0, 3.1 and 7.9mm on average for 25–35%, 40–50% and 65–75% degraded surfaces, respectively. Rotational deviations reach 11.1° in pitch. For the last patient, registration is consistent to within 2.6mm even on the 65–75% degraded surfaces, possibly because the surface topography has more distinct features. For ROI_bilateral-chest and ROI_extended-ipsilateral-chest registrations deviate in a similar pattern. However, registration on ROI_chest+abdomen is robust to deteriorating image qualities to within 4.2mm for all four patients. Conclusion: Registration deviations using ROI_ipsilateral-chestwall can reach 9.8mm on the 40–50% degraded surfaces. Caution is required when using AlignRT for patients experiencing skin darkening since the accuracy of AlignRT registration deteriorates. To avoid this inaccuracy, we recommend use of ROI_chest+abdomen, on which registration is consistent within 4.2mm even for highly degraded surfaces.

A real-time Late Gadolinium Enhancement (LGE) MRI technique (free breathing and non-gated) is presented for detection of myocardial scars. Conventional LGE imaging methods currently in use are applied in conjunction with breath-hold and, thus, are difficult to use in patients with cardiac disease and may lead to motion artifacts. Additionally, conventional techniques involve ECG gating, which is problematic in patients with arrhythmias requiring multiple breath holds and use of arrhythmia rejection techniques. Finally, conventional LGE techniques require accurate estimates for the inversion time in order to null the normal myocardium, revealing the location of the scar with high contrast. Real-time LGE imaging obviates these difficulties and can, in principle, acquire cine images to assess wall motion over several heart phases as part of the same scan. To date, the main limitation of real-time LGE imaging has been long acquisition window and low temporal resolution. These limitations lead to temporal blurring of wall motion and possible overestimation of infarct size. The goal of this study was to increase the temporal resolution of real-time, cine LGE imaging, providing the possibility for better visualization of the wall motion and more accurate assessment of myocardial viability.

Spiral (helical) computed tomography (CT) involves continuous patient translation during x-ray source rotation and data acquisition. As a result, a volume data set is obtained in a relatively short period of time. For chest or abdominal scanning, an entire examination can be completed in a single breath hold of the patient or in several successive short breath holds. The data volume may be viewed as conventional transaxial images or with multiplanar and three-dimensional methods. The authors review the technologic aspects of spiral CT, as well as its advantages, limitations, and current clinical applications.

Background and purpose To reduce the dose to the heart during left breast irradiation, a moderate deep breath hold technique (MDIBH) was introduced. Originally, verification of the MDIBH was performed with portal images acquired in movie loop during the treatment delivery. However, this verification method is not compatible with the use of dynamic MLC compensation, recently introduced because of its often superior dose distribution. Magnetic sensors were evaluated as an additional/alternative method to monitor the breath hold. Material and methods In a first phase, the reproducibility of MDIBH for breast patients was evaluated by investigating for 19 patients the set-up errors derived from portal images in cine loop acquisition during MDIBH. In a second phase, for 10 patients, the breathing curves recorded by magnetic sensors were used to monitor beam-on and beam-off while portal images were simultaneously recorded in movie loop. In a third phase, breast patients treated with dynamic MLC compensation were trained for MDIBH and monitored with magnetic sensors. Results The interfraction reproducibility of MDIBH for the initial 19 patients was recorded: the mean set-up error, the systematic and the random deviations are all smaller than 4 mm in the anterior–posterior direction and in the cranio-caudal direction and smaller than 2° along the rotation axis. Magnetic sensors provided a reproducible breathing curve: while the mean amplitude recorded for 10 patients varied substantially between patients, the individual standard deviation of the amplitude for each session was smaller than 3 mm. For these 10 patients, the intrafraction set-up variation between the first portal image of two consecutive breath holds and the intra-breath hold set-up variation between the first and last portal image of each breath hold is smaller than 2 mm in the anterior–posterior direction, smaller than 3 mm in the cranio-caudal direction and smaller than 1.5° along the rotation axis. Conclusion Using magnetic sensors to record the breathing curve of left breast patients in MDIBH, a verification method was developed, suitable for combining MDIBH with dynamic MLC compensation.

Purpose: A six-fraction, high-precision radiotherapy protocol for unresectable liver cancer has been developed in which active breathing control (ABC) is used to immobilize the liver and daily megavoltage (MV) imaging and repositioning is used to decrease geometric uncertainties. We report the accuracy of setup in the first 20 patients consecutively treated using this approach. Methods and materials: After setup using conventional skin marks and lasers, orthogonal MV images were acquired with the liver immobilized using ABC. The images were aligned to reference digitally reconstructed radiographs using the diaphragm for craniocaudal (CC) alignment and the vertebral bodies for anterior-posterior (AP) and mediolateral (ML) alignment. Adjustments were made for positioning errors >3 mm. Verification imaging was repeated after repositioning to assess for residual positioning error. Offline image matching was conducted to determine the setup accuracy using this approach compared with the initial setup error before repositioning. Real-time beam’s-eye-view MV movies containing an air-diaphragm interface were also evaluated. Results: A total of 405 images were evaluated from 20 patients. Repositioning occurred in 109 of 120 fractions because of offsets >3 mm. Three to eight beam angles, with up to four segments per field, were used for each isocenter. Breath holds of up to 27 s were used for imaging and treatment. The average time from the initial verification image to the last treatment beam was 21 min. Image guidance and repositioning reduced the population random setup errors (σ) from 6.5 mm (CC), 4.2 mm (ML), and 4.7 mm (AP) to 2.5 mm (CC), 2.8 mm (ML), and 2.9 mm (AP). The average individual random setup errors (σ) were reduced from 4.5 mm (CC), 3.2 mm (AP), and 2.5 mm (ML) to 2.2 mm (CC), 2.0 mm (AP), and 2.0 mm (ML). The standard deviation of the distribution of systematic deviations (Σ) was also reduced from 5.1 mm (CC), 3.4 mm (ML), and 3.1 mm (AP) to 1.4 mm (CC), 2.0 mm (ML), and 1.9 mm (AP) with image guidance and repositioning. The average absolute systematic errors were reduced from 4.1 mm (CC), 2.4 mm (AP), and 3.1 (ML) to 1.1 mm (CC), 1.3 mm (AP), and 1.6 mm (ML). Analysis of 52 real-time beam’s-eye-view MV movies revealed an average absolute CC offset in diaphragm position of 1.9 mm. Conclusion: Image guidance with orthogonal MV imaging and ABC for stereotactic body radiotherapy for liver cancer is feasible, improving setup accuracy compared with ABC without daily imaging and repositioning.

Purpose To improve the signal-to-noise ratio (SNR) of breath-held coronary magnetic resonance angiography (CMRA) without increasing the number or duration of breath holds. Materials and Methods In this BACSPIN (Breathing AutoCorrection with SPiral INterleaves) technique, a single breath-held electrocardiogram (ECG)-gated multi-slice interleaved-spiral data set is acquired, followed by repeated imaging of the same slices during free breathing. Each spiral interleaf from the breath-held data set is used as a standard for comparison with corresponding acquisitions at the same interleaf angle during free breathing. The most closely matched acquisitions are incorporated into a multi-slice, multi-average data set with increasing SNR over time. In-plane translations of the coronary artery can be measured and compensated for each accepted acquisition before combination with the other acquisitions. Results CMRA was performed on six volunteers, with improved SNR and minimal motional blurring. In some cases, breath holding could be dispensed with completely and the average respiratory position used as a reference. Purpose: The current SBRT treatment techniques include free breathing (FB) SBRT and gated FB SBRT. Gated FB SBRT has smaller target and less lung toxicity with longer treatment time. The recent development of direct connectivity between the ABC and linac allowing for automated beam gating. In this study, we have examined the feasibility of using ABC system to gate the lung SBRT treatment. Methods: A CIRS lung phantom with a 3cm sphere-insert and a moving chest plate was used in this study. Sinusoidal motion was used for the FB pattern. An ABC signal was imported to simulate breath holds. 4D-CT was taken in FB mode and average-intensity-projection (AIP) was used to create FB and 50% gated FB SBRT planning CT. A manually gated 3D CT scan was acquired for ABC gated SBRT planning.An SBRT plan was created for each treatment option. A surface-mapping system was used for 50% gating and ABC system was used for ABC gating. A manually gated CBCT scan was also performed to verify setup. Results: Among three options, the ABC gated plan has the smallest PTV of 35.94cc, which is 35% smaller comparing to that of the FB plan. Consequently, the V20 of the left lungmore » reduced by 15% and 23% comparing to the 50% gated FB and FB plans, respectively. The FB plan took 4.7 minutes to deliver, while the 50% gated FB plan took 18.5 minutes. The ABC gated plan delivery took only 10.6 minutes. A stationary target with 3cm diameter was also obtained from the manually gated CBCT scan. Conclusion: A strategy for ABC gated lung SBRT was developed. ABC gating can significantly reduce the lung toxicity while maintaining the target coverage. Comparing to the 50% gated FB SBRT, ABC gated treatment can also provide less lung toxicity as well as improved delivery efficiency. This research is funded by Elekta.« less

To illustrate a new concept for fast coronary artery screening with breath-hold volume targeted magnetic resonance (MR) imaging.Ten volunteers and 25 patients were imaged at a field strength of 1.5 T with an MR system with phased-array-coil reception and capable of echo-planar imaging. End-expiration breath-hold volume localization of the entire heart was performed with three-dimensional (3D) multishot segmented echo-planar imaging in 16-22 heartbeats. Interaction with a multiplaner reformation platform provided the optimal double-oblique volumes necessary to target seven coronary artery segments. Each segment was evaluated with 24-mm-thick volumes and breath holds at end expiration and magnetization transfer-enhanced 3D turbo fast low-angle shot imaging in 21 heartbeats. An intravascular contrast agent was used in eight patients to improve blood-myocardium contrast for the heart volume localizer acquisitions.The entire coronary tree was consistently covered in fewer than 13 breath holds. The scheme was successful in all volunteers and in 22 patients who could achieve adequate breath hold. With end-expiration acquisitions, the prescribed 24-mm-thick volumes were reproducible for all coronary segments in all cooperative subjects.Despite its status as the indisputable standard of reference for the detection of coronary artery disease, conventional coronary angiography remains costly and highly invasive, with associated risks of major complications, including stroke and death. Breath-hold volume targeted acquisitions permit rapid localization and coverage of the entire coronary tree with adequate resolution for evaluating the coronary arteries.

Objectives The aim of this study was to evaluate the feasibility and accuracy of free-breathing three-dimensional (3D) phase-sensitive inversion-recovery (PSIR) Turbo FLASH sequence for noninvasive assessment of left ventricular myocardial scar in swine. Methods Nine Chinese minipigs with experimentally induced acute myocardial infarction (MI) were studied. At 1 week and the study endpoint 4 weeks after MI surgery, the 3D and 2D LGE imaging were performed randomly by using a 1.5T clinical MR imaging system. Comparisons of myocardial scar volume (in cubic centimetres), scar transmurality (on a 5 points scale) and image quality (on a 4 points Likert scale) were performed by using the Pearson correlation and Bland-Altman analysis (for myocardial scar volume) or κ statistics (for transmurality) or Wilcoxon signed rank test (for image quality). Results In 6 of the 9 pigs, all procedures were successfully completed. In these pigs, a total of 26 segments with myocardial scars were detected by both 3D and 2D sequences, and there was good agreement for classification of scar transmurality (κ = 0.930). The scar volume determined by TTC staining (3.52 ± 1.40 cm 3 ) showed a good correlation with both 3D (3.54 ± 1.36 cm 3 , r = 0.957, p = 0.003) and 2D sequence (3.53 ± 1.26 cm 3 , r = 0.942, p = 0.005) at 4 weeks. And there were good correlation between scar volumes obtained from 3D and 2D techniques (r = 0.859, p Conclusions This animal study suggests that the new free-breathing 3D PSIR Turbo FLASH sequence is feasible for accurate assessment of left ventricular myocardial scar volume and transmurality, without the need of breath holds during the acquisition required for the 2D PSIR Turbo FLASH sequence.

Purpose: Respiration‐gated intensity modulated radiation therapy (IMRT) and volumetric modulated radiation therapy (VMAT) deliver the treatment beam at a single phase to reduce dose uncertainties resulting from respiratory‐induced organ motion. In some cases, gating can significantly prolong treatment times, leading to increased patient discomfort, treatment duration, and dose uncertainty due to patient postural shifts. To decrease treatment duration without increasing dose uncertainty, we propose dual‐gating in conjunction with with alternating coached five to ten second breath holds at inhale and exhale. In this study, we examine the feasibility of maintaining reproducible inhale and exhale RPM marker positions in a healthy volunteer and compare the expected dual‐gated delivery speedup over the free breathing case.Materials/Methods: Varian Real‐Time Position Management (RPM) respiratory signals from a volunteer were recorded during free breathing and during alternating ten second inspiration and exhalation breath hold coaching. The signals were then processed to examine the volunteer's ability to consistently reproduce RPM marker position during subsequent breath holds. Conventionally‐gated and dual‐gated treatment times were simulated for a given treatment plan under both normal and alternating breath hold conditions and compared. Results: Dual‐gated delivery under free breathing increases the duty cycle from 30.3% to 58.3% as compared with gating at exhale only. Alternated breath hold coaching further improves the duty cycle to 85.1%. Treatment delivery requiring 29.05 minutes to complete under normal exhale gating conditions is reduced to 16.58 minutes for dual‐gating during free breathing and only 5.88 minutes with dual‐gated alternated breath holds. RPM marker reproducibility was within 8% over the course of the delivery time. Conclusion: While additional improvements and testing are necessary, dual‐gated treatment delivery during alternated breath hold coaching is a feasible method for increasing duty cycle and thus improving treatment efficiency for tumors undergoing respiratory‐motion.

Previous studies have demonstrated that the SNR of abdominal MR images can be increased by averaging images obtained in different breath-hold acquisitions. In this note, the authors present a simple new methodology for ensuring that images acquired in multiple breath-hold periods are accurately co-registered. Within each breath-hold, a quick coronal scout scan is followed by a longer axial scan. The scout is used to position the axial slices in a fixed position relative to the organ under examination. This MR technique can, in principle, be automated so as to add less than 1 s to the imaging time of the axial scan. The method can be used to increase SNR by signal averaging or to co-register images acquired during, for example, uptake of contrast agents. SNR improvement with negligible blurring is demonstrated in liver images acquired by this method from healthy volunteers.

To reduce respiratory blur and ghosts in 3D coronary imaging, a data acquisition scheme using consistent multiple breath-holds was implemented. A navigator echo was acquired and processed in real time to dynamically measure diaphragm position. This information was provided as a visual prompt to the patient to maintain consistency in breath-hold levels such that the variation range of diastolic heart position was less than 2 mm. Preliminary results indicate that this multiple breath-hold acquisition scheme, compared with acquisition under respiration, can significantly reduce blur and ghost artifacts in 3D coronary imaging.

Background Evaluation of cardiac function is clinically performed using multi-slice breath-hold (BH) acquisition, in which patient breathes for a period of 40-60 seconds between a BH of 10-12 seconds. This results in suboptimal data acquisition efficiency. In this study, we propose to take advantage of the time between multiple BHs to acquire additional free-breathing (FB) data that can be used in the reconstruction of undersampled BH acquisition without increasing the total scan time.

The act of breathing results in considerable bulk cardiac motion, thus breath holding is usually required to image the coronary arteries. Normally, breath hold periods of about 15 sec are required, which can be difficult to sustain over several minutes. We introduce a breath holding strategy that only requires a 1-sec breath hold in every 4 sec. A set of cross-sectional images is acquired in approximately 10 min, and 3-dimensional renderings of the coronary tree are produced from these.

Whole-heart (WH) magnetic resonance coronary angiography (MRCA) studies are usually performed during free breathing while monitoring the position of the diaphragm with real-time motion correction. However, this results in a long scan time and the patient’s breathing pattern may change, causing the study to be aborted. Alternatively, WH MRCA can be performed with multiple breath-holds (mBH). However, one problem in the mBH method is that patients cannot hold their breath at the same position every time, leading to image degradation. We have developed a new WH MRCA imaging method that employs both the mBH method and automatic breathing-level tracking to permit automatic tracking of the changes in breathing or breath-hold levels. Evaluation of its effects on WH MRCA image quality showed that this method can provide high-quality images within a shorter scan time. This proposed method is expected to be very useful in clinical WH MRCA studies.

Background CMR allows investigating cardiac contraction, rotation and torsion non-invasively by the use of tagging sequences. Three-dimensional tagging has been proposed to cover the whole-heart but data acquisition requires three consecutive breath holds and hence demands considerable patient cooperation. In this study we have implemented and studied k-t undersampled cine 3D tagging in conjunction with k-t PCA reconstruction to potentially permit for single breath-hold acquisitions. Methods The performance of undersampled cine 3D tagging was investigated using computer simulations and in-vivo measurements in 8 healthy subjects and 5 patients with myocardial infarction. Fully sampled data was obtained and compared to retrospectively and prospectively undersampled acquisitions. Fully sampled data was acquired in three consecutive breath holds. Prospectively undersampled data was obtained within a single breath hold. Based on harmonic phase (HARP) analysis, circumferential shortening, rotation and torsion were compared between fully sampled and undersampled data using Bland-Altman and linear regression analysis. Results In computer simulations, the error for circumferential shortening was 2.8 ± 2.3% and 2.7 ± 2.1% for undersampling rates of R = 3 and 4 respectively. Errors in ventricular rotation were 2.5 ± 1.9% and 3.0 ± 2.2% for R = 3 and 4. Comparison of results from fully sampled in-vivo data acquired with prospectively undersampled acquisitions showed a mean difference in circumferential shortening of −0.14 ± 5.18% and 0.71 ± 6.16% for R = 3 and 4. The mean differences in rotation were 0.44 ± 1.8° and 0.73 ± 1.67° for R = 3 and 4, respectively.
