We describe a scalp-recorded measure of tonotopic selectivity, the "cortical onset response" (COR) and compare the results between humans and cats. The COR results, in turn, were compared with psychophysical masked-detection thresholds obtained using similar stimuli and obtained from both species. The COR consisted of averaged responses elicited by 50-ms tone-burst probes presented at 1-s intervals against a continuous noise masker. The noise masker had a bandwidth of 1 or 1/8th octave, geometrically centred on 4000 Hz for humans and on 8000 Hz for cats. The probe frequency was either - 0.5, - 0.25, 0, 0.25 or 0.5 octaves re the masker centre frequency. The COR was larger for probe frequencies more distant from the centre frequency of the masker, and this effect was greater for the 1/8th-octave than for the 1-octave masker. This pattern broadly reflected the masked excitation patterns obtained psychophysically with similar stimuli in both species. However, the positive signal-to-noise ratio used to obtain reliable COR measures meant that some aspects of the data differed from those obtained psychophysically, in a way that could be partly explained by the upward spread of the probe's excitation pattern. Our psychophysical measurements also showed that the auditory filter width obtained at 8000 Hz using notched-noise maskers was slightly wider in cat than previous measures from humans. We argue that although conclusions from COR measures differ in some ways from conclusions based on psychophysics, the COR measures provide an objective, noninvasive, valid measure of tonotopic selectivity that does not require training and that may be applied to acoustic and cochlear-implant experiments in humans and laboratory animals., (© 2022. Objective: This review article presents an overview of all Digit Triplet Tests (DTT) and digits-in-noise tests (DIN) and their variations in language, speech material, masking noise, test procedures, and targeted population. The effects on aspects of validity, reliability, and feasibility are investigated., Design: Scoping review., Study Sample: All studies referring to the DTT and DIN were collected from Pubmed and Embase. Search terms "digit triplet test" and "digits in noise" were used. Citations of selected articles were scanned backwards in time (the bibliography of the already selected research article) and forward in time (articles that cited the already selected research article). The search terms yielded 95 results in total. Eventually, 39 papers were selected., Results: Analyses showed psychometric reference-curves with steep slopes and speech reception thresholds with high measurement precision which are strongly associated with pure tone audiometry. High sensitivity and specificity to detect elevated pure tone thresholds were noted for test variants. Certain procedural modifications of the DTT and DIN can further improve the test. Additionally, large-scale application of the DTT and DIN is feasible., Conclusion: The DTT and DIN are a very valuable tool for screening and diagnostics for a wide variety of populations.

Developmental dyslexia is most commonly associated with phonological processing difficulties. However, children with dyslexia may experience poor speech-in-noise perception as well. Although there is an ongoing debate whether a speech perception deficit is inherent to dyslexia or acts as an aggravating risk factor compromising learning to read indirectly, improving speech perception might boost reading-related skills and reading acquisition. In the current study, we evaluated advanced speech technology as applied in auditory prostheses, to promote and eventually normalize speech perception of school-aged children with dyslexia, i.e., envelope enhancement (EE). The EE strategy automatically detects and emphasizes onset cues and consequently reinforces the temporal structure of the speech envelope. Our results confirmed speech-in-noise perception difficulties by children with dyslexia. However, we found that exaggerating temporal "landmarks" of the speech envelope (i.e., amplitude rise time and modulations)-by using EE-passively and instantaneously improved speech perception in noise for children with dyslexia. Moreover, the benefit derived from EE was large enough to completely bridge the initial gap between children with dyslexia and their typical reading peers. Taken together, the beneficial outcome of EE suggests an important contribution of the temporal structure of the envelope to speech perception in noise difficulties in dyslexia, providing an interesting foundation for future intervention studies based on auditory and speech rhythm training., (© 2021. Envelope following responses (EFRs) can be evoked by a wide range of auditory stimuli, but for many stimulus parameters the effect on EFR strength is not fully understood. This complicates the comparison of earlier studies and the design of new studies. Furthermore, the most optimal stimulus parameters are unknown. To help resolve this issue, we investigated the effects of four important stimulus parameters and their interactions on the EFR. Responses were measured in 16 normal hearing subjects evoked by stimuli with four levels of stimulus complexity (amplitude modulated noise, artificial vowels, natural vowels and vowel-consonant-vowel combinations), three fundamental frequencies (105 Hz, 185 Hz and 245 Hz), three fundamental frequency contours (upward sweeping, downward sweeping and flat) and three vowel identities (Flemish /a:/, /u:/, and /i:/). We found that EFRs evoked by artificial vowels were on average 4-6 dB SNR larger than responses evoked by the other stimulus complexities, probably because of (unnaturally) strong higher harmonics. Moreover, response amplitude decreased with fundamental frequency but response SNR remained largely unaffected. Thirdly, fundamental frequency variation within the stimulus did not impact EFR strength, but only when rate of change remained low (e.g. not the case for sweeping natural vowels). Finally, the vowel /i:/ appeared to evoke larger response amplitudes compared to /a:/ and /u:/, but analysis power was too small to confirm this statistically. Vowel-dependent differences in response strength have been suggested to stem from destructive interference between response components. We show how a model of the auditory periphery can simulate these interference patterns and predict response strength. A language-independent automated self-test on tablet based on masked recognition of ecological sounds, the Sound Ear Check (SEC), was developed. In this test, 24 trials of eight different sounds are randomly presented in a noise that was spectrally shaped according to the average frequency spectra of the stimulus sounds, using a 1-up 2-down adaptive procedure. The test was evaluated in adults with normal hearing and hearing loss, and its feasibility was investigated in young children, who are the target population of this test. Following equalization of perceptual difficulty across sounds by applying level adjustments to the individual tokens, a reference curve with a steep slope of 18%/dB was obtained, resulting in a test with a high test-retest reliability of 1 dB. The SEC sound reception threshold was significantly associated with the averaged pure tone threshold ( r  = .70), as well as with the speech reception threshold for the Digit Triplet Test ( r  = .79), indicating that the SEC is susceptible to both audibility and signal-to-noise ratio loss. Sensitivity and specificity values on the order of magnitude of ∼70% and ∼80% to detect individuals with mild and moderate hearing loss, respectively, and ∼80% to detect individuals with slight speech-in-noise recognition difficulties were obtained. Homogeneity among sounds was verified in children. Psychometric functions fitted to the data indicated a steep slope of 16%/dB, and test-retest reliability of sound reception threshold estimates was 1.3 dB. A reference value of -9 dB signal-to-noise ratio was obtained. Test duration was around 6 minutes, including training and acclimatization.

Objectives: This study aims to investigate the large-scale applicability of the Digit Triplet test (DTT) for school-age hearing screening in fifth grade elementary (5E) (9 to 12 years old) and third grade secondary (3S) (13 to 16 years old) school children. The reliability of the test is investigated as well as whether pass/fail criteria need to be corrected for training and/or age, and whether these criteria have to be refined with respect to referral rates and pure-tone audiometry results., Design: Eleven school health service centers participated in the region of Flanders (the Northern part of Belgium). Pure-tone screening tests, which are commonly used for hearing screening in school children, were replaced by the DTT. Initial pass/fail criteria were determined. Children with speech reception thresholds (SRT) of -7.2 dB signal to noise ratio (SNR) (5E) and -8.3 dB SNR (3S) or worse were referred for an audiogram and follow-up. In total, n = 3412 (5E) and n = 3617 (3S) children participated., Results: Population SRTs (±2 SD) were -9.8 (±1.8) dB SNR (5E) and -10.5 (±1.6) dB SNR (3S), and do not need correction for training and/or age. Whereas grade-specific pass/fail criteria are more appropriate, a linear regression analysis showed an improvement of 0.2 dB per year of the SRT until late adolescence. SRTs could be estimated with a within-measurement reliability of 0.6 dB. Test duration was also grade-dependent, and was 6 min 50 sec (SD = 61 sec) (5E) and 5 min 45 sec (SD = 49 sec) (3S) on average for both ears. The SRT, test reliability, and test duration were comparable across centers. With initial cut-off values, 2.9% (5E) and 3.5% (3S) of children were referred. Based on audiograms of n = 39 (5E) and n = 59 (3S) children, the diagnostic accuracy of the DTT was assessed. A peripheral hearing loss was detected in 31% (5E) and 53% (3S) of the referred children. Hearing losses found were mild. Less strict pass/fail criteria increased the diagnostic accuracy. Optimal pass/fail criteria were determined at -6.5 dB SNR (5E) and -8.1 dB SNR (3S). With these criteria, referral rates dropped to 1.3% (5E) and 2.4% (3S)., Conclusions: The DTT has been implemented as the new hearing screening methodology in the Flemish school-age hearing screening program. Based on the results of this study, pass/fail criteria were determined and optimized to be used for systematic hearing screening of 5E and 3S school children. Furthermore, this study provides reference values for the DTT in children 9 to 16 years of age. Reliable SRTs can be obtained with the test, allowing accurate monitoring of hearing over time. This is important in the context of a screening guideline, which aims to identify children with noise-induced hearing loss. Validation of the screening result should go beyond taking an audiogram, as a peripheral hearing impairment cannot always be found in children with a failed test.

Objectives: This study investigated whether age affects Flemish Digit Triplet Test (DTT) scores, as well as sensitivity and specificity of the test to detect high-frequency hearing impairment in middle age., Design: DTT performance in broadband and low-pass filtered noise was assessed for 21 young and 61 middle-aged persons with normal-hearing sensitivity. Cutoff values based on their performance were used to evaluate sensitivity and specificity of the DTT as a screening tool in 145 middle-aged persons, with varying audiometric profiles., Results: DTT performance for normal-hearing persons depended on age in both noise conditions. For low-pass filtered noise, the trade-off between sensitivity and specificity also increased when age-adjusted cutoff values were applied for screening in middle age., Conclusions: The Flemish DTT with broadband noise had high sensitivity and specificity to detect high-frequency hearing impairment in middle age. Similar results were obtained for low-pass filtered noise when cutoff values for referral were age adjusted.

State-of-the-art hearing aids (HAs) try to overcome the deficit of poor speech intelligibility (SI) in noisy listening environments using digital noise reduction (NR) techniques. The application of time-frequency masks to the noisy sound input is a common NR technique to increase SI. The binary mask with its binary weights and the Wiener filter with continuous weights are representatives of a hard- and a soft-decision approach for time-frequency masking. In normal-hearing listeners, the ideal Wiener filter (IWF) outperforms the ideal binary mask (IBM) in terms of SI and speech quality with perfect SI even at very low signal-to-noise ratios. In this paper, both approaches were investigated for hearing-impaired (HI) listeners. Perceptual and auditory model-based measures were used for the evaluation. The IWF outperformed the IBM in terms of SI. Quality-wise, there was no overall difference between the NR algorithms perceived. Additionally, the processed signals were evaluated based on an auditory nerve model using the neurogram similarity metric (NSIM). The mean NSIM values were significantly different for intelligible and unintelligible sentences. The results suggest that a soft-mask seems to be promising for application in HAs.

Psychophysical procedures are used to balance loudness across the ears. However, they can be difficult and require active cooperation. We investigated whether 40-Hz auditory steady-state response (ASSR) amplitudes can be used to objectively estimate the balanced loudness across the ears for a group of young, normal-hearing participants. The 40-Hz ASSRs were recorded using monaural stimuli with carrier frequencies of 500, 1000, or 2000 Hz over a range of levels between 40 and 80 dB SPL. Behavioral loudness balancing was performed for at least one reference level of the left ear. ASSR amplitude growth functions were listener dependent, but median across-ear ratios in ASSR amplitudes were close to 1. The differences between the ASSR-predicted balanced levels and the behaviorally found balanced levels were smaller than 5 dB in 59% of cases and smaller than 10 dB in 85% of cases. The differences between the ASSR-predicted balanced levels and the reference levels were smaller than 5 dB in 54% of cases and smaller than 10 dB in 87% of cases. No clear hemispheric lateralization was found for 40-Hz ASSRs, with the exception of responses evoked by stimulus levels of 40 to 60 dB SPL at 2000 Hz.

As people grow older, speech perception difficulties become highly prevalent, especially in noisy listening situations. Moreover, it is assumed that speech intelligibility is more affected in the event of background noises that induce a higher cognitive load, i.e., noises that result in informational versus energetic masking. There is ample evidence showing that speech perception problems in aging persons are partly due to hearing impairment and partly due to age-related declines in cognition and suprathreshold auditory processing. In order to develop effective rehabilitation strategies, it is indispensable to know how these different degrading factors act upon speech perception. This implies disentangling effects of hearing impairment versus age and examining the interplay between both factors in different background noises of everyday settings. To that end, we investigated open-set sentence identification in six participant groups: a young (20-30 years), middle-aged (50-60 years), and older cohort (70-80 years), each including persons who had normal audiometric thresholds up to at least 4 kHz, on the one hand, and persons who were diagnosed with elevated audiometric thresholds, on the other hand. All participants were screened for (mild) cognitive impairment. We applied stationary and amplitude modulated speech-weighted noise, which are two types of energetic maskers, and unintelligible speech, which causes informational masking in addition to energetic masking. By means of these different background noises, we could look into speech perception performance in listening situations with a low and high cognitive load, respectively. Our results indicate that, even when audiometric thresholds are within normal limits up to 4 kHz, irrespective of threshold elevations at higher frequencies, and there is no indication of even mild cognitive impairment, masked speech perception declines by middle age and decreases further on to older age. The impact of hearing impairment is as detrimental for young and middle-aged as it is for older adults. When the background noise becomes cognitively more demanding, there is a larger decline in speech perception, due to age or hearing impairment. Hearing impairment seems to be the main factor underlying speech perception problems in background noises that cause energetic masking. However, in the event of informational masking, which induces a higher cognitive load, age appears to explain a significant part of the communicative impairment as well. We suggest that the degrading effect of age is mediated by deficiencies in temporal processing and central executive functions. This study may contribute to the improvement of auditory rehabilitation programs aiming to prevent aging persons from missing out on conversations, which, in turn, will improve their quality of life., (Copyright © 2016 Elsevier B.V. Objective: Currently, there is no up-to-date speech perception test available in the Estonian language that may be used to diagnose hearing loss and quantify speech intelligibility. Therefore, based on the example of the Nederlandse Vereniging voor Audiologie (NVA)-lists ( Bosman, 1989 ; Wouters et al, 1994 ) an Estonian words in noise (EWIN) test has been developed., Design: Two experimental steps were carried out: (1) selection and perceptual optimization of the monosyllables, and (2) construction of 14 lists and an evaluation in normal hearing (NH) subjects both in noise and in quiet., Study Sample: Thirty-six normal-hearing (NH) native speakers of Estonia (age range from 17 to 46 years)., Results: The reference psychometric curve for NH subjects was determined, with the slope and speech reception threshold being well in accordance with the respective values of the NVA lists. The 14 lists in noise yielded equivalent scores with high precision., Conclusion: The EWIN test is a reliable and valid speech intelligibility test, and is the first of its kind in the Estonian language.

Objective: Recently, the digit triplet test was shown to be a sensitive speech-in-noise test for early high-frequency hearing loss in noise-exposed workers. This study investigates if a further improvement is achieved when using a closed set of consonant-vowel-consonant (CVC) speech items with the same vowel, and/or a low-pass (LP) filtered version of the standard speech-shaped noise., Design: Speech reception thresholds in noise were gathered for the digit triplet, CVC, and CVC_LP test and compared to the high-frequency pure-tone average (PTA)., Study Sample: 118 noise-exposed workers showing a wide range of high-frequency hearing losses., Results: For the 84 Dutch-speaking participants, the CVC test showed an increased measurement error and a decreased between-subject variation, leading to a weaker correlation with the PTA2,3,4,6 (R = 0.64) and thus a lower sensitivity compared to the digit triplet test (R = 0.86). However, the use of LP-filtered noise resulted in a sensitivity improvement (R = 0.79 versus R = 0.64) due to the large increase in between-subject spread. Similar trends were found for the 34 French-speaking workers., Conclusions: Using CVC words with the same vowel could not increase the sensitivity to detect isolated high-frequency hearing loss. With LP-filtered noise, test sensitivity improved, but it did not surpass the original digit triplet test.

Objectives: Hearing screening in occupational medicine is generally based on pure-tone threshold audiometry. However, reliable and valid thresholds can only be obtained in a sound-proof room, using a high-quality, well-calibrated audiometer, and by a well-trained administrator. Thresholds also need to be determined for several audiometric frequencies. This makes the test time-consuming and expensive, which is not ideal for the screening of large populations. A Speech-In-Noise test (SPIN), by contrast, does not have the abovementioned requirements. Because it can be implemented as a quick automated self-test, possibly over the Internet, a SPIN test is highly advantageous for screening purposes. However, its sensitivity for (isolated) high-frequency hearing loss, as typically seen in noise-exposed listeners, was unclear up to present. In this study, the authors investigated the sensitivity and specificity of the Digit Triplet SPIN test for detecting and monitoring (early-stage) high-frequency hearing loss, and its similarity across two different language versions., Design: One-hundred eighteen noise-exposed workers, representing a wide range from no to severe high-frequency hearing loss, completed the French or Dutch version of the broadband Digit Triplet self-test in an office-like room. Pure-tone thresholds, collected by a professional audiologist in favorable settings, served as the reference., Results: The 84 Dutch-speaking participants showed a very strong linear relation between the reference and the Digit Triplet test, with the pure-tone average at 2, 3, 4, and 6 kHz as a strong predictor (R = 0.86) for the speech-reception threshold. The sensitivity and specificity to detect mild high-frequency hearing loss were 92% (61 of 66) and 89% (16 of 18), respectively. The area under the receiver operating characteristic (ROC) curve was very high (≥0.91) for several degrees of high-frequency hearing loss. With a within-subject standard deviation of only 0.8 dB, the Digit Triplet test also had a low measurement error. The results of the 34 French-speaking subjects showed a highly similar trend., Conclusions: The Digit Triplet test proves to have a high sensitivity and specificity for detecting different degrees of high-frequency hearing loss. Given its ease of use, this test is very suitable for screening purposes in occupational medicine, and potentially for the screening of adolescents at risk of recreational noise-induced hearing loss.

The present study investigates the most important factors that limit the intelligibility of the cochlear implant (CI) processed speech in noisy environments. The electrical stimulation sequences provided in CIs are affected by the noise in the following three manners. First of all, the natural gaps in the speech are filled, which distorts the low-frequency ON/OFF modulations of the speech signal. Secondly, speech envelopes are distorted to include modulations of both speech and noise. Lastly, the N-of-M type of speech coding strategies may select the noise dominated channels instead of the dominant speech channels at low signal-to-noise ratio's (SNRs). Different stimulation sequences are tested with CI subjects to study how these three noise effects individually limit the intelligibility of the CI processed speech. Tests are also conducted with normal hearing (NH) subjects using vocoded speech to identify any significant differences in the noise reduction requirements and speech distortion limitations between the two subject groups. Results indicate that compared to NH subjects CI subjects can tolerate significantly lower levels of steady state speech shaped noise in the speech gaps but at the same time can tolerate comparable levels of distortions in the speech segments. Furthermore, modulations in the stimulus current level have no effect on speech intelligibility as long as the channel selection remains ideal. Finally, wrong maxima selection together with the introduction of noise in the speech gaps significantly degrades the intelligibility. At low SNRs wrong maxima selection introduces interruptions in the speech and makes it difficult to fuse noisy and interrupted speech signals into a coherent speech stream., (Copyright © 2013 Elsevier B.V. Recent studies have shown that transient parts of a speech signal contribute most to speech intelligibility in normal-hearing listeners. In this study, the influence of enhancing the onsets of the envelope of the speech signal on speech intelligibility in noisy conditions using an eight channel cochlear implant vocoder simulation was investigated. The enhanced envelope (EE) strategy emphasizes the onsets of the speech envelope by deriving an additional peak signal at the onsets in each frequency band. A sentence recognition task in stationary speech shaped noise showed a significant speech reception threshold (SRT) improvement of 2.5 dB for the EE in comparison to the reference continuous interleaved sampling strategy and of 1.7 dB when an ideal Wiener filter was used for the onset extraction on the noisy signal. In a competitive talker condition, a significant SRT improvement of 2.6 dB was measured. A benefit was obtained in all experiments with the peak signal derived from the clean speech. Although the EE strategy is not effective in many real-life situations, the results suggest that there is potential for speech intelligibility improvement when an enhancement of the onsets of the speech envelope is included in the signal processing of auditory prostheses.

Electrically evoked compound action potentials (ECAPs) are widely used to study the excitability of the auditory nerve and stimulation properties in cochlear implant (CI) users. However, ECAP detection can be difficult and very subjective at near-threshold stimulation levels or in spread of excitation measurements. In this study, we evaluated the statistical properties of the background noise (BN) and the postaverage residual noise (RN) in ECAP measurements in order to determine an objective detection criterion. For the estimation of the BN and the RN, a method currently used in auditory brainstem response measurements was applied. The potential benefit of using weighted (Bayesian) averages was also examined. All estimations were performed with a set of approximately 360 ECAP measurements recorded from five human CI users of the CII or HiRes90K device (advanced bionics). Results demonstrated that the BN was normally distributed and the RN decreased according to the square root of the number of averages. No additional benefit was observed by using weighted averaging. The noise was not significantly different either at different stimulation intensities or across recording electrodes along the cochlea. The analysis of the statistical properties of the noise indicated that a signal-to-noise ratio of 1.7 dB as a detection criterion corresponds to a false positive detection rate of 1% with the used measurement setup.

Objective: To compare results on the everyday sentence test 'FIST', the new closed-set sentence test 'FrMatrix', and the digit triplet screening test 'FrDigit3'., Design: First, the FrMatrix was developed and normative values were obtained. Subsequently, speech reception thresholds (SRTs) for the three types of tests were gathered at four study centers representing different geographic regions in Belgium and France., Study Sample: Fifty-seven normal-hearing listeners took part in the normative study of the FrMatrix, and 118 subjects, with a wide range of hearing thresholds, participated in the comparative study., Results: Homogenizing the individual words of the FrMatrix with regard to their intelligibility resulted in a reference SRT of -6.0 (±0.6) dB SNR and slope at the SRT of 14.0 %/dB. The within-subject variability was only 0.4 dB. Comparison of the three tests showed high correlations between the SRTs mutually (>0.81). The FrMatrix had the highest discriminative power, both in stationary and in fluctuating noise. For all three tests, differences across the participating study centers were small and not significant., Conclusions: The FIST, the FrMatrix, and the FrDigit3 provide similar results and reliably evaluate speech recognition performance in noise both in normal-hearing and hearing-impaired listeners.

Objective: To investigate the extent to which temporal gaps, temporal fine structure, and comprehensibility of the masker affect masking strength in speech recognition experiments., Design: Seven different masker types with Dutch speech materials were evaluated. Amongst these maskers were the ICRA-5 fluctuating noise, the international speech test signal (ISTS), and competing talkers in Dutch and Swedish., Study Sample: Normal-hearing and hearing-impaired subjects., Results: The normal-hearing subjects benefited from both temporal gaps and temporal fine structure in the fluctuating maskers. When the competing talker was comprehensible, performance decreased. The ISTS masker appeared to cause a large informational masking component. The stationary maskers yielded the steepest slopes of the psychometric function, followed by the modulated noises, followed by the competing talkers. Although the hearing-impaired group was heterogeneous, their data showed similar tendencies, but sometimes to a lesser extent, depending on individuals' hearing impairment., Conclusions: If measurement time is of primary concern non-modulated maskers are advised. If it is useful to assess release of masking by the use of temporal gaps, a fluctuating noise is advised. If perception of temporal fine structure is being investigated, a foreign-language competing talker is advised.

A French speech intelligibility screening test in noise that applies digit triplets as stimuli has been developed and evaluated for both telephone and broadband headphone use. After optimizing the speech material based on the intelligibility of the individual digits, norms for normal-hearing subjects were established. speech reception thresholds (SRTs) of -6.4 +/- 0.4 and -10.5 +/- 0.3 dB SNR, and slopes of 17.1 and 27.1 %/dB were obtained for telephone and broadband headphone presentation, respectively. The French digit triplet test by telephone was then implemented as an automatic self-screening test by home telephone, and further evaluated in normal-hearing and hearing-impaired listeners. A test-retest variability of 0.7 dB was found and the correlation between SRT and pure-tone average (PTA(0.5,1,2,4)) was 0.77. One month after launching the test, 20,000 calls were registered. It can be concluded that both versions of the newly developed test have steep slopes and small SRT differences across normal-hearing listeners. The screening test by telephone is highly reliable and proves to fulfill the need for an easily accessible and objective hearing screening.

This paper evaluates the influence of three multimicrophone noise reduction algorithms on the ability to localize sound sources. Two recently developed noise reduction techniques for binaural hearing aids were evaluated, namely, the binaural multichannel Wiener filter (MWF) and the binaural multichannel Wiener filter with partial noise estimate (MWF-N), together with a dual-monaural adaptive directional microphone (ADM), which is a widely used noise reduction approach in commercial hearing aids. The influence of the different algorithms on perceived sound source localization and their noise reduction performance was evaluated. It is shown that noise reduction algorithms can have a large influence on localization and that (a) the ADM only preserves localization in the forward direction over azimuths where limited or no noise reduction is obtained; (b) the MWF preserves localization of the target speech component but may distort localization of the noise component. The latter is dependent on signal-to-noise ratio and masking effects; (c) the MWF-N enables correct localization of both the speech and the noise components; (d) the statistical Wiener filter approach introduces a better combination of sound source localization and noise reduction performance than the ADM approach.

Objective: This paper evaluates the benefit of the two-microphone adaptive beamformer BEAM in the Nucleus Freedom cochlear implant (CI) system for speech understanding in background noise by CI users., Design: A double-blind evaluation of the two-microphone adaptive beamformer BEAM and a hardware directional microphone was carried out with five adult Nucleus CI users. The test procedure consisted of a pre- and post-test in the lab and a 2-wk trial period at home. In the pre- and post-test, the speech reception threshold (SRT) with sentences and the percentage correct phoneme scores for CVC words were measured in quiet and background noise at different signal-to-noise ratios. Performance was assessed for two different noise configurations (with a single noise source and with three noise sources) and two different noise materials (stationary speech-weighted noise and multitalker babble). During the 2-wk trial period at home, the CI users evaluated the noise reduction performance in different listening conditions by means of the SSQ questionnaire. In addition to the perceptual evaluation, the noise reduction performance of the beamformer was measured physically as a function of the direction of the noise source., Results: Significant improvements of both the SRT in noise (average improvement of 5-16 dB) and the percentage correct phoneme scores (average improvement of 10-41%) were observed with BEAM compared to the standard hardware directional microphone. In addition, the SSQ questionnaire and subjective evaluation in controlled and real-life scenarios suggested a possible preference for the beamformer in noisy environments., Conclusions: The evaluation demonstrates that the adaptive noise reduction algorithm BEAM in the Nucleus Freedom CI-system may significantly increase the speech perception by cochlear implantees in noisy listening conditions. This is the first monolateral (adaptive) noise reduction strategy actually implemented in a mainstream commercial CI.

In this paper, the first real-time implementation and perceptual evaluation of a singular value decomposition (SVD)-based optimal filtering technique for noise reduction in a dual microphone behind-the-ear (BTE) hearing aid is presented. This evaluation was carried out for a speech weighted noise and multitalker babble, for single and multiple jammer sound source scenarios. Two basic microphone configurations in the hearing aid were used. The SVD-based optimal filtering technique was compared against an adaptive beamformer, which is known to give significant improvements in speech intelligibility in noisy environment. The optimal filtering technique works without assumptions about a speaker position, unlike the two-stage adaptive beamformer. However this strategy needs a robust voice activity detector (VAD). A method to improve the performance of the VAD was presented and evaluated physically. By connecting the VAD to the output of the noise reduction algorithms, a good discrimination between the speech-and-noise periods and the noise-only periods of the signals was obtained. The perceptual experiments demonstrated that the SVD-based optimal filtering technique could perform as well as the adaptive beamformer in a single noise source scenario, i.e., the ideal scenario for the latter technique, and could outperform the adaptive beamformer in multiple noise source scenarios.

Objective: To evaluate the improvement in speech intelligibility in noise obtained with an assistive real-time fixed endfire array of bidirectional microphones in comparison with an omnidirectional hearing aid microphone in a realistic environment., Design: The microphone array was evaluated physically in anechoic and reverberant conditions. Perceptual tests of speech intelligibility in noise were carried out in a reverberant room, with two types of noise and six different noise scenarios with single and multiple noise sources. Ten normal-hearing subjects and 10 hearing aid users participated. The speech reception threshold for sentences was measured in each test setting for the omnidirectional microphone of the hearing aid and for the hearing aid in combination with the array with one and three active microphones. In addition, the extra improvement of five active array microphones, relative to three, was determined in another group of 10 normal-hearing listeners., Results: Improvements in speech intelligibility in noise obtained with the array relative to an omnidirectional microphone depend on noise scenario and subject group. Improvements up to 12 dB for normal-hearing and 9 dB for hearing-impaired listeners were obtained with three active array microphones relative to an omnidirectional microphone for one noise source at 90 degrees . For three uncorrelated noise sources at 90 degrees, 180 degrees, and 270 degrees, improvements of approximately 9 dB and 6 dB were obtained for normal-hearing and hearing-impaired listeners, respectively. Even with a single noise source at 45 degrees, benefits of 4 dB were achieved in both subject groups. Five active microphones in the array can provide an additional improvement at 45 degrees of approximately 1 dB, relative to the three-microphone configuration for normal-hearing listeners., Conclusions: These improvements in signal-to-noise ratio can be of great benefit for hearing aid users, who have difficulties with speech understanding in noisy environments.

Objective: The performance of an adaptive beam-former in a 2-microphone, behind-the-ear hearing aid for speech understanding in noisy environments was evaluated. Physical and perceptual evaluations were carried out. This was the first large-scale test of a wearable real-time implementation of this algorithm. The main perceptual research questions of this study were related to the influence on the noise reduction performance of (1) the spectro-temporal character of the jammer sound, (2) the jammer sound scene, (3) hearing impairment, and (4) the basic microphone configuration in the hearing aid. Four different speech materials were used for the perceptual evaluations. All tests were carried out in an acoustical environment comparable to living room reverberation., Design: The adaptive beamformer was implemented in Audallion, a small, body-worn processor, linked to a Danasound 2-microphone behind-the-ear aid. The strategy was evaluated physically in different acoustical environments. Using speech reception threshold (SRT) measurements, the processing was evaluated perceptually and the different research questions addressed with three groups of subjects. Groups I, II, and III consisted of 10 normal-hearing, 5 hearing-impaired, and 7 normal-hearing persons, respectively. The tests were carried out in three spectro-temporally different jammer sounds (unmodulated and modulated speech weighted noise, multitalker babble) and in three different noise scenarios (single noise source at 90 degrees, noise sources at 90 degrees and 270 degrees relative to speaker position, diffuse noise scene). Two microphone configurations were compared: a device equipped with two omnidirectional microphones and a device equipped with one hardware directional and one omnidirectional microphone. In each of these conditions, the adaptive beamformer and the directional and omnidirectional microphone configurations were tested., Results: The improvement in signal-to-noise ratio from the use of the adaptive beamformer did not depend on the spectro-temporal character of the jammer sounds and the speech materials used, although the absolute levels of the SRTs varied appreciably for different speech-noise combinations. The performance of the adaptive noise reduction depended on the jammer sound scene., Conclusions: No difference in signal-to-noise ratio improvement was observed between hearing-impaired and normal-hearing listeners, although individual SRT levels may differ. On average, an SRT improvement of 7.7 and 3.9 dB for a single noise source at 90 degrees and 5.9 and 3.4 dB for two noise sources at 90 degrees and 270 degrees was obtained for both normal-hearing and hearing-impaired listeners, using the adaptive beamformer and the directional microphone, respectively, relative to the omnidirectional microphone signal. An adaptive beamformer for behind-the-ear dual-microphone hearing aids has been optimized for speech intelligibility enhancement in the presence of disturbing sounds or noise. The noise reduction approach is based on the scheme presented by Vanden Berghe and Wouters (1998). A real-time implementation of the signal processing is realized in Audallion, a wearable, small digital signal processing (DSP) platform. After physical evaluation, speech-in-noise intelligibility tests have been carried out on three normally-hearing and two hearing-impaired subjects. A significant speech reception threshold improvement of 11.3 dB was obtained in a moderately reverberant environment for one jammer sound source (steady speech-weighted noise or multi-talker babble) in a direction of 90 degrees relative to the direction of the speech.

Hearing aids (HA) are a fundamental component in restoring auditory function; however, they cannot completely alleviate all problems encountered by adults with hearing impairment. The aim of this study is twofold. Firstly, we assess the health-related quality of life and coping strategies of experienced HA users. Secondly, we assess whether HA users can benefit from auditory training. To this end, 40 participants who had worn HAs for more than 6 months participated in this study. Half of the participants received auditory training, while the other half served as a passive control. The training consisted of a personalized training scheme, with outcome measures including speech in noise perception in free-field and via direct streaming to the HA, phoneme identification, cognitive control, and health-related quality of life. Results showed that experienced HA users reported a relatively good quality of life. Health-related quality of life was correlated with aided speech perception in noise, but not with aided pure tone audiometry. Coping strategies were adaptive, leading to improved communication. Participants showed improvements in trained tasks, consonant identification, and speech in noise perception. While both groups yielded improved speech in noise perception at the end, post hoc analysis following a three-way interaction showed a significantly larger pre–post difference for the trained group in the streaming condition. Although training showed some improvements, the study suggests that the training paradigm was not sufficiently challenging for HA users. The aim of this study was to develop an open-set word recognition task in speech-weighted noise and in quiet for young children and examine age effects for open versus closed response formats. Dutch monosyllabic words were presented in quiet and in stationary speech-weighted noise to 4- and 5-year-old children as well as to young adults in an open-set response format. Additionally, performance in open and closed context was assessed, as well as in a picture-pointing paradigm More than 200 children and 50 adults with normal hearing participated in the various validation phases. Average fitted speech reception thresholds (50%) yielded an age effect between 4-year and 5-year olds (and adults), both in speech-weighted noise and in quiet. The closed-set format yielded lower (better) SNRs than the open-set format, and children benefitted to the same extent as adults from phonetically similar words in speech-weighted noise. Additionally, the 4 AFC picture-pointing paradigm can be used to assess word recognition in quiet from 3 years of age. The same materials reveal performance differences between 4 and 5 years of age (and adults), both in quiet and speech-weighted noise using an open-set response format. This relatively small yet significant difference in SRT for a gap of only 1 year shows a developmental change for word recognition in speech-weighted noise and in quiet in the first decade of life. The use of two types of speech-in-noise (SPIN) assessment, namely digits-in-noise self-tests and open-set, monosyllabic word tests, to assess the SPIN understanding performance of children with cochlear implants (CI) in mainstream and special education, was investigated. The tests' feasibility and reliability and the influence of specific cognitive abilities on their results were studied. The results of 30 children with CIs in mainstream and special education were compared to those of 60 normal-hearing children in elementary school. Results indicate that the digit triplet test (DTT) was feasible for all children tested in this study, as seen by the familiarity of all the digits, the high stability of the test results (<3 dB SNR), and a small measurement error (≤2 dB SNR). Remembering full triplets did not form a problem and results did not show systematic attention loss. For children with CIs, the performance on the DTT was strongly related to the performance on the open-set monosyllabic word-in-noise task. However, small but significant differences were observed in the performance of children with CIs in mainstream and special education on the monosyllabic word test. Both tests showed little influence of cognitive abilities, making them both useful in situations where the bottom-up auditory aspect of SPIN performance needs to be investigated or in situations where sentence-in-noise tests are too challenging. Speech perception in noise is an everyday occurrence for adults and children alike. The factors that influence how well individuals cope with noise during spoken communication are not well understood, particularly in the case of children. This article aims to review the available evidence on how working memory and attention play a role in children's speech perception in noise, how characteristics of measures affect results, and how this relationship differs in non-typical populations. This article is a scoping review of the literature available on PubMed. Forty articles were included for meeting the inclusion criteria of including children as participants, some measure of speech perception in noise, some measure of attention and/or working memory, and some attempt to establish relationships between the measures. Findings were charted and presented keeping in mind how they relate to the research questions. The majority of studies report that attention and especially working memory are involved in speech perception in noise by children. We provide an overview of the impact of certain task characteristics on findings across the literature, as well as how these affect non-typical populations. While most of the work reviewed here provides evidence suggesting that working memory and attention are important abilities employed by children in overcoming the difficulties imposed by noise during spoken communication, methodological variability still prevents a clearer picture from emerging. Objective: To investigate modifications of the Flemish Digit Triplet Test (DTT) to improve its efficiency, i.e. the use of a low-pass filtered noise and variable adaptive step sizes according to a digit scoring procedure, targeting different recognition probabilities. Design and study sample: Speech reception thresholds (SRT) were evaluated in terms of their test-retest reliability and correlation with pure tone averages (PTA) in a group of 68 adult participants with different degrees of hearing impairment. Results: The use of a low-pass filtered noise did not result in better test-retest reliability or an improved SRT-PTA correlation. Using digit scoring with adaptive step sizes parametrised to target a recognition probability of 79% (D79), corresponding to the recognition probability of the currently used DTT with triplet scoring and fixed adaptive step sizes of 2 dB, increased test-retest reliability. Lower recognition probabilities of 57 and 35% demonstrated worse reliability and worse SRT-PTA correlations. Conclusions: Given the increased test-retest reliability of D79, a similar reliability as for the currently used DTT could be obtained after considerably fewer trials, leading to a profit in test duration. Objective: The recent integration of automated real-ear measurements (REM) in the fitting software facilitates the hearing aid fitting process. Such a fitting strategy, TargetMatch (TM), was evaluated. Test-retest reliability and matching accuracy were quantified, and compared to a REM-based fitting with manual adjustment. Also, it was investigated whether TM leads to better perceptual outcomes compared to a FirstFit (FF) approach, using software predictions only. Design and study sample: Ten hearing impaired participants were enrolled in a counterbalanced single-blinded cross-over study comparing TM and FF. Aided audibility, speech intelligibility and real-life benefits were assessed. Repeated measurements of both TM and REMs with manual adjustment were performed. Results: Compared to a REM-based fitting with manual adjustment, TM had higher test-retest reliability. Also, TM outperformed the other fitting strategies in terms of matching accuracy. Compared to a FF, improved aided audibility and real-life benefits were found. Speech intelligibility did not improve. Conclusions: Preliminary data suggest that automated REMs increase the likelihood of meeting amplification targets compared with a FF. REMs integrated in the fitting software provide additional reliability and accuracy compared to traditional REMs. Findings need to be verified in a larger and more varied sample. Several filterbank-based metrics have been proposed to predict speech intelligibility (SI). However, these metrics incorporate little knowledge of the auditory periphery. Neurogram-based metrics provide an alternative, incorporating knowledge of the physiology of hearing by using a mathematical model of the auditory nerve response. In this work, SI was assessed utilizing different filterbankbased metrics (the speech intelligibility index and the speech-based envelope power spectrum model) and neurogram-based metrics, using the biologically inspired model of the auditory nerve proposed by Zilany, Bruce, Nelson, and Carney [(2009), J. Acoust. Soc. Am. 126(5), 2390–2412] as a front-end and the neurogram similarity metric and spectro temporal modulation index as a back-end. Then, the correlations with behavioural scores were computed. Results showed that neurogram-based metrics representing the speech envelope showed higher correlations with the behavioural scores at a word level. At a per-phoneme level, it was found that phoneme transitions contribute to higher correlations between objective measures that use speech envelope information at the auditory periphery level and behavioural data. The presented framework could function as a useful tool for the validation and tuning of speech materials, as well as a benchmark for the development of speech processing algorithms. A letter to the editor is presented in response to the article "International Collegium of Rehabilitative Audiology (ICRA) recommendations for the construction of multilingual speech tests" published on April 29, 2015, along with the response from the authors.

Objective:To provide guidelines for the development of two types of closed-set speech-perception tests that can be applied and interpreted in the same way across languages. The guidelines cover the digit triplet and the matrix sentence tests that are most commonly used to test speech recognition in noise. They were developed by a working group on Multilingual Speech Tests of the International Collegium of Rehabilitative Audiology (ICRA).Design:The recommendations are based on reviews of existing evaluations of the digit triplet and matrix tests as well as on the research experience of members of the ICRA Working Group. They represent the results of a consensus process.Results:The resulting recommendations deal with: Test design and word selection; Talker characteristics; Audio recording and stimulus preparation; Masking noise; Test administration; and Test validation.Conclusions:By following these guidelines for the development of any new test of this kind, clinicians and researchers working in any language will be able to perform tests whose results can be compared and combined in cross-language studies. Objective: The fundamental frequency modulation (F0mod) sound processing strategy was developed to improve pitch perception with cochlear implants. In previous work it has been shown to improve performance in a number of pitch-related tasks such as pitch ranking, familiar melody identification, and Mandarin Chinese tone identification. The objective of the current study was to compare speech perception with F0mod and the standard clinical advanced combination encoder (ACE) strategy. Study sample: Seven cochlear-implant listeners were recruited from the clinical population of the University Hospital Leuven. Design: F0mod was implemented on a real-time system. Speech recognition in quiet and noise was measured for seven cochlear-implant listeners, comparing F0mod with ACE, using three different Dutch-language speech materials. Additionally the F0 estimator used was evaluated physically, and pitch ranking performance was compared between F0mod and ACE. Results: Immediately after switch-on of the F0mod strategy, speech recognition in quiet and noise were similar for ACE and F0mod, for four out of seven listeners. The remaining three listeners were subjected to a short training protocol with F0mod, after which their performance was reassessed, and a significant improvement was found. Conclusions: As F0mod improves pitch perception, for the seven subjects tested it did not interfere with speech recognition in quiet and noise, and has a low computational complexity, it seems promising for implementation in a clinical sound processor. Bilateral cochlear implants (CIs) may offer deaf children a range of advantages compared to unilateral CIs. However, speech perception in noise is mainly facilitated by better-ear effects and much less by interaural comparisons or true ‘binaural’ hearing. Little is known about the development of the binaural auditory system with CIs provided at a young age. It is possible that, as with adults, binaural sensitivity exists but is not accessed due to technical limitations in electrical stimulation methods. In this paper, we present results on binaural hearing in children with bilateral CIs. Binaural masking level differences (BMLDs) were measured for a 180-degree phase shift in a 125-Hz sinusoid, presented in a 50-Hz-wide noise band and modulating a 1000-pps carrier pulse train. Stimuli were presented to a single electrode in the middle of the electrode array at both ears. Eight children between 6 and 15 years of age participated in this study. Six children had a significantly better detection threshold when the signal was out of phase (dichotic) between two ears than when it was in phase (diotic), with a mean difference (BMLD) of 6.4 dB. The present results show that children with bilateral CIs are sensitive to binaural cues in electrical stimuli, similar to adults, even when implants are provided at a later age and with a longer delay between implantations. On the basis of a statistical evaluation of the binaural sound source localization performance during listening tests by human subjects, it is shown that the convolution of a measured head-related transfer function (HRTF) with the room impulse response generated by a hybrid image source model with a stochastic scattering process using secondary sources provides an adequate model for the prediction of binaural room impulse responses (BRIR) for directional sound localization in the frontal horizontal plane. The source localization performance for sound stimuli presented to test subjects in a natural way was compared to that presented via headphones. Listening via headphones tends to decrease the localization performance only, and only slightly when localizing narrow-band high-frequency stimuli. Binaural sound presented to test subjects via headphones was generated on the basis of measurements and simulations. Two different headphone conditions were evaluated. The overall localization performance for simulated headphone sound obtained using a simulated BRIR was found to be equivalent to the one using a measured BRIR. The study also confirms expectations that the deterioration of sound source localization performance in reverberant rooms is closely linked to the direct-to-reverberant ratio for given loudspeaker and listener positions, rather than to the reverberation time of the room as such. A multi-channel noise reduction technique is presented based on a Speech Distortion-Weighted Multi-channel Wiener Filter (SDW-MWF) approach that incorporates the conditional Speech Presence Probability (SPP). A traditional SDW-MWF uses a fixed parameter to a trade-off between noise reduction and speech distortion without taking speech presence into account. Consequently, the improvement in noise reduction comes at the cost of a higher speech distortion since the speech dominant segments and the noise dominant segments are weighted equally. Incorporating the conditional SPP in SDW-MWF allows to exploit the fact that speech may not be present at all frequencies and at all times, while the noise can indeed be continuously present. In speech dominant segments it is then desirable to have less noise reduction to avoid speech distortion, while in noise dominant segments it is desirable to have as much noise reduction as possible. Experimental results with hearing aid scenarios demonstrate that the proposed SDW-MWF incorporating the conditional SPP improves the signal-to-noise ratio compared to a traditional SDW-MWF. A Dutch sentence test (LIST) and a Dutch number test (LINT) have been developed and validated for the accurate measurement of speech reception thresholds (SRT) in quiet and in noise with severely hearing-impaired individuals and cochlear implant recipients in Flanders and the Netherlands. The LIST consists of 35 lists of 10 sentences of equal known difficulty uttered by a female speaker; while the LINT consists of 400 numbers (1-100) by two male and two female speakers. Normative values were determined at fixed S/N ratios and using the adaptive method (Plomp & Mimpen, 1979), yielding identical results for SRT and slope. For the LIST, average fitted SRTs were 27.1 (0.9) dB SPL in quiet and -7.8 dB (0.2) SNR in noise. In addition, the LIST in noise displayed a steep discrimination function (17%/dB) and good reliability (within-subject standard deviation=1.2 dB). For the LINT average fitted SRTs in quiet were 20.7 (0.9) dB SPL and about -9.0 dB SNR in noise. Again, the slopes of the performance intensity functions were relatively steep, i.e. 8.5%/dB in quiet and 15.2%/dB in noise, suggesting that the LINT is accurate and efficient and thus capable of reflecting subtle changes in performance. First data with cochlear implanted subjects show that both LIST and LINT are feasible and are capable of mapping a large range of hearing disabilities. [ABSTRACT FROM AUTHOR]

For small-sized arrays such as hearing aids, noise reduction is obtained at the expense of an increased sensitivity to errors in the assumed signal model, i.e., microphone mismatch, variations in speaker and microphone positions, reverberation. However, the noise reduction algorithm should still be robust, i.e., insensitive to small signal model errors. In this paper, we evaluate the robustness of the Generalized Sidelobe Canceller (GSC) and a recently developed Multichannel Wiener Filtering (MWF) technique for hearing aid applications both analytically and experimentally. The analysis reveals that robustness of the GSC is especially crucial in complicated noise scenarios and that microphone mismatch is particularly harmful to the GSC, even when the adaptive noise canceller is adapted during noise only. Hence, a constraint on the noise sensitivity of the GSC is essential, at the expense of less noise reduction. The MWF on the other hand, is not affected by microphone mismatch and has a potential benefit over the robust GSC with noise sensitivity constraint. However, the MWF is sensitive to the estimation accuracy of the second order statistics of speech and noise so that its benefit may be lost in nonstationary noise scenarios. [ABSTRACT FROM AUTHOR]

Objectives: Binaural interaction can attenuate 30-50 Hz ASSRs when evoked dichotically and thereby reduce the measurement efficiency. The objective of the present study is to gain insight in this interaction effect and how it affects measurement efficiency. Material and methods: Monaurally and dichotically evoked ASSRs were recorded in twenty-three adults. Amplitude modulated octave-band noise bands were presented at 70 dB. Different modulation frequencies within the 30-50 Hz range were varied across ears and conditions. Results: Dichotio stimulation resulted in an overall attenuation of the 30-50 Hz ASSRs compared to monaural stimulation. This attenuation resulted in a -33% reduction in measurement efficiency for modulation frequencies >40 Hz. Conclusions: Our results show that multiple 40-50 Hz ASSRs are as efficient, or more efficient in -66% of the measurements compared to sequential monaural stimulation. [ABSTRACT FROM AUTHOR]

Developmental dyslexia has frequently been linked to deficits in auditory processing and speech perception. However, the presence and precise nature of these deficits and the direction of their relation with reading, remains debated. In this longitudinal study, 87 five-year-olds at high and low family risk for dyslexia were followed before and during different stages of reading acquisition. The processing of different auditory cues was investigated, together with performance on speech perception and phonology and reading. Results show no effect of family risk for dyslexia on prereading auditory processing and speech perception skills. However, a relation is present between the performance on these skills in kindergarten and later phonology and literacy. In particular, links are found with the auditory processing of cues characteristic for the temporal speech amplitude envelope, rather than with other auditory cues important for speech intelligibility. Hereby, cues embedded in the speech amplitude envelope show to be related to a broad range of phonological precursors for reading. In addition, speech-in-noise perception demonstrates to operate as the most contributing factor for later phonological awareness and to be a predictor for reading mediated by the association with phonology.
