Objective: Chronic fatigue syndrome (CFS) and fibromyalgia (FM) are disorders of unknown etiology and unclear pathophysiology, with overlapping symptoms of - especially muscular -fatigue and pain. Studies have shown increased muscle fiber conduction velocity (CV) in the non-painful muscles of FM patients. We investigated whether CFS patients also show CV abnormalities., Methods: Females with CFS (n = 25), with FM (n = 22), and healthy controls (n = 21) underwent surface electromyography of the biceps brachii, loaded up to 20% of maximum strength, during short static contractions. The mean CV and motor unit potential (MUP) velocities with their statistical distribution were measured., Results: The CV changes with force differed between CFS-group and both FM-group and controls (P = 0.01). The CV of the CFS-group increased excessively with force (P < 0.001), whereas that of the controls increased only slightly and non-significantly, and that of the FM-group did not increase at all. In the CFS-group, the number of MUPs conveying very high conduction velocities increased abundantly with force and the MUPs narrowed., Conclusion: Our results suggest disturbed muscle membrane function in CFS patients, in their motor units involved in low force generation. The objective of this study is to examine the effect of gravity compensation training on reaching and underlying changes in muscle activation. In this clinical trial, eight chronic stroke patients with limited arm function received 18 sessions (30 min) of gravity-compensated reach training (during 6 weeks) in combination with a rehabilitation game. Before and after training, unsupported reach (assessing maximal distance, joint angles and muscle activity of eight shoulder and elbow muscles) and the Fugl-Meyer assessment were compared. After training, the maximal reach distance improved significantly by 3.5% of arm length, together with increased elbow extension (+9.2°) and increased elbow extensor activity (+68%). In some patients, a reduced cocontraction of biceps and anterior deltoid was also involved, although this was not significant on group level. Improvements in unsupported reach after gravity compensation training in chronic stroke patients with mild to severe hemiparesis were mainly accompanied by increased activation of prime movers at the elbow, although in some patients, improved selective joint control may also have been involved. Gravity compensation seems to be a suitable way to provide active, task-specific treatment, without the need for high-tech devices. Further research on a larger scale, including control groups and combinations of arm support with functional hand training, is essential to enhance the potential of arm support to complement poststroke arm rehabilitation.

The aim of this study was to investigate motor unit (MU) characteristics of the biceps brachii during sinusoidal contractions in chronic stroke patients using high-density surface electromyography. Ten sinusoidal elbow flexion and extension movements were performed both passively and actively by 18 stroke patients and 20 healthy subjects. Motor unit action potentials (MUAPs) were extracted, and their root-mean-square value (RMS(MUAP)) was calculated. RMS(MUAP) was significantly larger in stroke than in healthy subjects. In both groups RMS(MUAP) was smaller during the stretch phase of passive movement than during active movement. The larger MUAPs indicate enlarged MUs, possibly as a result of reinnervation. The lower RMS(MUAP) values during passive stretch than during active movement indicates that the stretch reflex mainly activates smaller MUs, while a larger part of the MU pool can be recruited voluntarily. Background: Concerning chronic low back pain (CLBP), different cognitive-behavioral models have hypothesized that coping strategies play a role in the chronification of pain by changes in physical activity. Strategies such as avoidance - or persistent coping may be related to changes in (lumbar) muscle activity., Aim: Investigate the different coping strategies present in CLBP and whether these are differentially related to lumbar muscle activity during walking., Methods: In a cross sectional study, 63 subjects with CLBP walked on a treadmill at 3.8 km/h. Coping strategies were measured with the Dutch version of the Coping Strategies Questionnaire and three factors were identified with principal component analysis. Surface electromyography data of the erector spinae were obtained and smooth rectified electromyography (SRE) values were averaged per periods of swing and double support. The ratio of SRE values (swing/double support) was used as a measure of relaxation. The relation between SRE values and coping strategies was analyzed with random coefficient analysis., Results: Three coping strategies (i.e. "catastrophizing", "distraction" and "persistence and control") could be discerned. "Catastrophizing" was positively related to (natural logarithm) SRE values (beta=0.06, 95% CI=0.01-0.10; R(2)=7.7%). "Distraction" was negatively associated with SRE ratios (beta=-0.03, 95% CI=-0.05 to -0.01; R(2)=7.5%). No relation was found between "persistence and control" and SRE values or ratios., Conclusions: In CLBP, a maladaptive coping strategy like "catastrophizing" is related to increased lumbar muscle activity, and an adaptive strategy like "distraction" to increased lumbar muscle relaxation during walking., (Copyright (c) 2009. The effects of surface functional electrical stimulation (FES) of the tibial nerve of healthy subjects were evaluated. The FES was applied at three different times during gait: early, mid and late stances. The purpose of this work is to understand the effect of unilateral stimulation on the bilateral activation patterns of leg muscles, because FES is used in practice to improve gait, while associated neuromuscular change is not often measured. The experimental protocol presented here will be transferred to stroke subjects, who could benefit from improved push-off during gait. Results show that FES of the tibial nerve changes the offset timing of the tibialis anterior muscle on the stimulated side and the on- and offset timings of the tibialis anterior muscle of the leg contralateral to stimulation. Additionally, activity levels of the semitendinosus ipsilateral and tibialis anterior contralateral to the stimulated leg significantly decreased, with respect to the non-stimulated condition. For the semitendinosus, this was a difference of 6-7microV, with p<0.05. For the tibialis anterior, this was a difference of 7-15microV, with a significance of p=0.00, respectively. This information is important for future applications of stimulation as it means that stimulation not only affects the stimulated muscle but also the physiological motor control by the CNS.

Background: In many stroke patients arm function is limited, which can be related to an abnormal coupling between shoulder and elbow joints. The extent to which this can be translated to activities of daily life (ADL), in terms of muscle activation during ADL-like movements, is rather unknown. Therefore, the present study examined the occurrence of abnormal coupling on functional, ADL-like reaching movements of chronic stroke patients by comparison with healthy persons., Methods: Upward multi-joint reaching movements (20 repetitions at a self-selected speed to resemble ADL) were compared in two conditions: once facilitated by arm weight compensation and once resisted to provoke a potential abnormal coupling. Changes in movement performance (joint angles) and muscle activation (amplitude of activity and co-activation) between conditions were compared between healthy persons and stroke patients using a repeated measures ANOVA., Results: The present study showed slight changes in joint excursion and muscle activation of stroke patients due to shoulder elevation resistance during functional reach. Remarkably, in healthy persons similar changes were observed. Even the results of a sub-group of the more impaired stroke patients did not point to an abnormal coupling between shoulder elevation and elbow flexion during functional reach., Conclusions: The present findings suggest that in mildly and moderately affected chronic stroke patients ADL-like arm movements are not substantially affected by abnormal synergistic coupling. In this case, it is implied that other major contributors to limitations in functional use of the arm should be identified and targeted individually in rehabilitation, to improve use of the arm in activities of daily living.

It has been hypothesized that changes in trunk muscle activity in chronic low back pain (CLBP) reflect an underlying "guarding" mechanism, which will manifest itself as increased superficial abdominal - and lumbar muscle activity. During a functional task like walking, it may be further provoked at higher walking velocities. The purpose of this cross sectional study was to investigate whether subjects with CLBP show increased co-activation of superficial abdominal - and lumbar muscles during walking on a treadmill, when compared to asymptomatic controls. Sixty-three subjects with CLBP and 33 asymptomatic controls walked on a treadmill at different velocities. Surface electromyography data of the erector spinae, rectus abdominis and obliquus abdominis externus muscles were obtained and averaged per stride. Results show that, compared to asymptomatic controls, subjects with CLBP have increased muscle activity of the erector spinae and rectus abdominis, but not of the obliquus abdominis externus. These differences in trunk muscle activity between groups do not increase with higher walking velocities. In conclusion, the observed increased trunk muscle activity in subjects with CLBP during walking supports the guarding hypothesis.

Objectives: To investigate whether patients with chronic low back pain (CLBP) show "guarded" movements during walking. It is hypothesized that guarding will be reflected by increased lumbar muscle activity during all periods of stride and secondary, relatively lesser relaxation during periods of swing compared with double support. Furthermore, it is hypothesized that higher levels of perceived fear and disability are related to increased muscle activity and less relative relaxation., Materials and Methods: In a cross-sectional study 63 patients with CLBP and 33 healthy controls walked on a treadmill at 3.8 km/h. Surface electromyography (sEMG) data of the erector spinae were obtained and smoothed rectified sEMG (SRE) values were calculated per period of swing and double support. The ratio of SRE values in swing to double support was used as a measure of relative relaxation (SRE ratio). In addition, the relationship between SRE values, the Roland Morris Disability Questionnaire, and the Tampa Scale for Kinesiophobia was analyzed in patients with CLBP., Results: Mean SRE values were significantly higher in patients with CLBP than in controls both during periods of double support and swing. SRE ratios were not significantly different between groups. Results showed no influence of disability or fear of movement on either SRE values or ratios., Discussion: In patients with CLBP, increased lumbar muscle activity during all periods of stride, with comparable alteration between swing and double support, suggests difficulties with total muscle relaxation. On the basis of this evaluation, it is concluded that patients with CLBP show a guarding mechanism during walking. No relationship is found between perceived fear, disability, and muscle activity.

The use of array surface EMG recordings for detailed assessment of motor control and muscle properties is increasing. Motor unit action potentials (MUAPs) and their properties can be extracted from these recordings. The objective of this study was to determine the reproducibility of variables obtained from array surface EMG recordings of the shoulder and neck muscles during different functional tasks. Eight-channel linear arrays were placed on the upper trapezius (UT) and sternocleidomastoid (SCM) muscles of 12 healthy subjects. Subjects performed 3 tasks: shoulder abduction (90 degrees), ironing (repetitively touching two ends of a horizontal bar in front of the subject), and 90 degrees head turning. The protocol was performed twice while electrodes remained on and repeated a third time a week later. Three global and six MUAP-related variables were calculated. Intra-class correlation coefficients (ICC) were calculated to assess reliability and smallest detectable changes (SDC) were calculated to assess agreement. In general, the EMG variables showed high levels of reliability which suggests they may be effective for differentiating between-subjects. SDC was found to be considerably lower for the frequency-related (5-23%) than for the amplitude-related variables (15-78%), indicating that the frequency-related variables may be more suitable for investigating interventions which aim to modify motor control. There was no difference in reproducibility between global and MUAP-related variables, which justifies their complementary use.

The aim of this study was to describe the characteristics of spasticity, quantified as muscle activity during stretch, during passive and active movement. For this cross sectional study 19 stroke patients with spasticity in the lower limb were recruited. Reflex activity was studied with surface electromyography of knee flexor and extensor muscles during passive and active movement of the lower leg. On both the affected and unaffected side, root mean square values of the knee extensor muscles, while stretched, were higher during active than during passive movement (p<0.05). For the vastus lateralis (VL) the correlation was moderate (rho=0.54, p=0.022), for the rectus femoris (RF) high (rho=0.83, p<0.001). For the semitendinosus (ST) the correlation was low (rho=0.27) and not significant. During active movement the correlation between VL activity and activity of the antagonist ST, as an indicator for co-contraction of the affected muscles, was marked (rho=0.73, p=0.001). A moderate negative correlation was found between reflex activity of RF during passive stretch and the active range of motion (rho=-0.51, p=0.027). The results show that a passive stretch test alone is insufficient either as assessment method for spasticity during active motor tasks or as a measure for motor control.

Variations in surface electromyography (SEMG) amplitude have been shown to be dependent on the dislocation of recording electrodes. Yet no literature is available about the effect of electrode dislocation on SEMG amplitude of the lower back muscles. In this project, the aim was to determine this effect by investigating changes in the SEMG root mean square (RMS), induced by a well-defined dislocation of the recording electrodes. Bipolar SEMG of the longissimus dorsi (LD) muscles was measured in 16 healthy subjects undertaking five functional tasks (standing, forward flexion, re-extension, unsupported sitting and arm/leg lifting), and for eight of those subjects the experiment was repeated within two weeks. Intra-class correlation coefficients (ICCs) were used to show the reliability of the RMS in relation to electrode dislocation, the repeatability of the tasks, and the test-retest reliability. Results showed that: (1) lateral dislocation causes a significant decrease (18%, p<0.001) in RMS; (2) longitudinal dislocation does not change the RMS; and (3) the variability caused by electrode dislocation is comparable to the variability caused by repetitions of tasks or by electrode repositioning. Our conclusion is that positioning in the mediolateral direction should be exact to minimize changes in SEMG amplitude due to dislocation. However, precise longitudinal electrode positioning seems to be less critical in experimental setups which measure the SEMG of the lower back muscles.

Background: Arm support to help compensate for the effects of gravity may improve functional use of the shoulder and elbow during therapy after stroke, but gravity compensation may alter motor control., Objective: To obtain quantitative information on how gravity compensation influences muscle activation patterns during functional, 3-dimensional reaching movements., Methods: Eight patients with mild hemiparesis performed 2 sets of repeated reach and retrieval movements, with and without unloading the arm, using a device that acted at the elbow and forearm to compensate for gravity. Electromyographic (EMG) patterns of 6 upper extremity muscles were compared during elbow and shoulder joint excursions with and without gravity compensation., Results: Movement performance was similar with and without gravity compensation. Smooth rectified EMG (SRE) values were decreased from 25% to 50% during movements with gravity compensation in 5 out of 6 muscles. The variation of SRE values across movement phases did not differ across conditions., Conclusions: Gravity compensation did not affect general patterns of muscle activation in this sample of stroke patients, probably since they had adequate function to complete the task without arm support. Gravity compensation did facilitate active arm movement excursions without impairing motor control. Gravity compensation may be a valuable modality in conventional or robot-aided therapy to increase the intensity of training for mildly impaired patients.

Introduction: Arm support like gravity compensation may improve arm movements during stroke rehabilitation. It is unknown how gravity compensation affects muscle activation patterns during reach and retrieval movements. Since muscle activity during reach is represented by a component varying with movement velocity and a component supposedly counteracting gravity, we hypothesized that gravity compensation decreases the amplitude of muscle activity, but does not affect the pattern. To examine this, we compared muscle activity during well defined movements with and without gravity compensation in healthy elderly., Methods: Ten subjects performed reach and retrieval movements with and without gravity compensation. Muscle activity of biceps, triceps, anterior, middle and posterior parts of deltoid and upper trapezius was compared between the two conditions., Results: The level of muscle activity was lower with gravity compensation in all muscles, reaching significance in biceps, anterior deltoid and trapezius (p < or = 0.026). The muscle activation pattern did not differ between movements with and without gravity compensation (p > or = 0.662)., Discussion: Gravity compensation only influenced the level of muscle activity but not the muscle activation pattern in terms of timing. Future studies should examine if the influence of gravity compensation is comparable for stroke patients. This may stimulate early and intensive training during rehabilitation.

The aim of this study was to investigate motor unit (MU) characteristics of the biceps brachii in poststroke patients using high-density surface electromyography (sEMG). Eighteen chronic hemiparetic stroke patients took part. The Fugl-Meyer score for the upper extremity was assessed. Subjects performed an isometric step contraction consisting of force levels from 5%-50% maximal voluntary contraction while sEMG of the biceps brachii was recorded with a two-dimensional 16-channel electrode array. This was repeated for both sides. Motor unit action potentials (MUAPs) were extracted from the EMG signals, and their root-mean-square value (RMS(MUAP), reflecting MU size) and mean frequency of the power spectrum (FMEAN(MUAP), reflecting recruitment threshold) were calculated. FMEAN(MUAP) was smaller on the affected than on the unaffected side, indicating an increased contribution of low-threshold MUs, possibly related to degeneration of high-threshold MUs. The ratio of RMS(MUAP) on the affected side divided by that on the unaffected side correlated significantly with the Fugl-Meyer score. Surface electromyography parameters such as root-mean-square value (RMS) and median power frequency (FMED) are commonly used to assess the input of the central nervous system (CNS) to a muscle. However, RMS and FMED are influenced not only by CNS input, but also by peripheral muscle properties. The number of motor unit action potentials (MUAPs) per second, or MUAP Rate (MR), being the sum of the firing rates of the active motor units, would reflect CNS input solely. This study explored MR behaviour in relation to force and during a fatiguing contraction in comparison to RMS and FMED. In the first experiment (n=10) a step contraction of shoulder elevation force (20-100 N) was performed while multi-channel array EMG was recorded from the upper trapezius muscle. The sensitivity of MR for changes in force (1.8%/N) was almost twice as high as that of RMS (0.97%/N), indicating that MR may be more suitable for monitoring muscle force. The second experiment (n=6) consisted of a 15-min isometric contraction of the biceps brachii. MR increased considerably less than RMS (0.9% vs. 4.1%), suggesting that MR selectively reflects central motor control whereas RMS also reflects peripheral changes. These results support that, at relatively low force levels, MR is a suitable parameter for non-invasive assessment of the input of the CNS to the muscle.

This study aimed at investigating whether patients with neck-shoulder complaints from different aetiologies (work-related musculo-skeletal disorders, WMSD; whiplash associated disorders, WAD) show comparable muscle activation patterns, characterised by higher activation and lower relaxation levels of the trapezius muscles compared to healthy controls. Twenty healthy controls, 21 WMSD and 20 WAD patients with non-acute neck-shoulder pain were recruited for this cross-sectional study. Surface electromyography (sEMG) recordings were performed at the upper trapezius muscles during reference contractions, standardised computer tasks (typing and unilateral stress task), and rest measurements. sEMG was continuously recorded during these measurements. Outcome measures were root mean square (RMS) to study muscle activity, and relative rest time (RRT) to study muscle relaxation. Statistical analysis comprised the bootstrap technique and Kruskall-Wallis tests. Results showed no clear evidence for abnormal muscle activation patterns in WMSD and WAD patients compared to healthy controls. However, a tendency was observed for higher RMS levels during the reference contractions and computer tasks in both patient groups compared to healthy controls, and lower RRT levels at the non-dominant side during stress. Both patient groups also showed larger variability in RMS and RRT values. This variability has more often been reported in literature and may suggest the existence of subgroups of pain patients with corresponding different muscle activation patterns not related to aetiology. Future research may focus on identifying these subgroups of patients with neck-shoulder pain.

Work related musculoskeletal disorders (WMSDs) in the shoulder/neck area are a common and increasing problem among European computer workers, especially women. Long-term low-level workloads with low degree of muscle rest are a potential risk factor for developing WMSDs. The purpose of the present study of female computer users (age 45-65 years) in Denmark and Sweden was to investigate if subjects with self-reported neck/shoulder complaints (cases, N=35) show less trapezius muscle relative rest time (RRT) than controls (N=44) when performing standardised short-term computer work tasks in controlled laboratory conditions. Surface electromyography (EMG) signals were recorded bilaterally from the upper trapezius muscles during a type, edit, precision and colour word stress task. Besides RRT, 10th percentile RMS values were calculated. On the average, 15 of the cases and 18 of the controls showed analysable EMG files per task. For the colour word stress task, the results showed lower RRT values and higher 10th percentile RMS amplitude levels among cases compared to controls. No such signs could be found for the other tasks performed. The present results indicate an increased motor response to a psychological stressor among subjects with self-reported neck/shoulder complaints.

Objective: To investigate the influence of different positions on stretch reflex activity of knee flexors and extensors measured by electromyography in poststroke patients with spasticity and its expression in the Ashworth Scale., Design: Crossover trial with randomized order of positioning., Setting: Outpatient rehabilitation center in the Netherlands., Participants: Poststroke patients (N = 19) with lower-limb spasticity., Intervention: Changing position: sitting versus supine., Main Outcome Measures: Root mean square (RMS) values of muscle activity and goniometric parameters, obtained during the pendulum test and passive knee flexion and extension, and Ashworth scores., Results: RMS values of bursts of rectus femoris activity were significantly higher in the supine compared with the sitting position (P = .006). The first burst of vastus lateralis activity during the pendulum test (P = .049) and semitendinous activity during passive stretch (P = .017) were both significantly higher in the supine versus the sitting position. For both the pendulum test and passive movement test, the duration and amplitude of the cyclic movement of the lower leg changed significantly as well. In the supine position, we found significantly higher Ashworth scores for the extensors (P = .001) and lower scores for the flexors (P = .002)., Conclusions: The outcome of clinical and neurophysiologic assessment of spasticity is influenced considerably by subject positioning.

Studies using surface electromyography have demonstrated a reorganization of muscle activation patterns of the neck and shoulder muscles in patients with posttraumatic neck pain disability. The neurophysiologically oriented "pain adaptation" model explains this reorganization as a useful adaptation to prevent further pain and injury. The cognitive-behavioral-oriented "fear avoidance" model suggests that fear of movement, in addition to the effects of pain, modulates the muscle activation level. We analyzed the extent to which pain and fear of movement influenced the activation patterns of the upper trapezius muscle during the transition from acute to chronic posttraumatic neck pain. Ninety-two people with an acute traumatic neck injury after a motor vehicle accident were followed up for 24 weeks. Visual analog scale ratings of pain intensity, response on the Tampa Scale of Kinesophobia--fear of movement, and surface electromyography of the upper trapezius muscles during a submaximal isometric physical task were obtained at 1, 4, 8, 12, and 24 weeks after the motor vehicle accident. Multilevel analysis revealed that an increased level of both fear of movement (t value=-2.19, P=0.030) and pain intensity (t value=-2.94, P=0.004) were independently associated with a decreased level of muscle activation. Moreover, the results suggest that the association between fear of movement and lower muscle activity level is stronger in patients reporting high pain intensity (t value=2.15, P=0.033). The contribution of pain intensity to the muscle activation level appeared to decrease over time after the trauma (t value=2.58, P=0.011). The results support both the "pain adaptation" and the "fear avoidance" models. It is likely that the decrease in muscle activation level is aimed at "avoiding" the use of painful muscles.

Objective: To examine the combined effect of age and timing of augmented feedback on learning muscle relaxation. Performing a gross motor task, subjects had to lower their trapezius muscle activity using the electromyographic signal as visual myofeedback., Design: Healthy subjects (16 young adults: 20-35 yrs; and 16 older adults: 55-70 yrs) were randomly assigned to one of two timing conditions of myofeedback: concurrent (feedback was provided immediately during the trial) and terminal (feedback was provided delayed after the trial) condition., Results: The results indicated that young adults had a higher level of motor performance (i.e., lower muscle activity) compared with older adults when myofeedback was provided. These effects persisted during short- (after 10 mins) and long-term retention (after 1 wk) when no myofeedback was provided. In contrast to young adults, older adults did not improve their performance throughout the experiment. There were no interactions of age with the timing conditions of myofeedback during acquisition and retention., Conclusions: Either timing condition of augmented feedback was equally helpful to young adults, whereas neither was helpful for older adults in learning muscle relaxation.

The objective of this study was to investigate differences in motor control of the trapezius muscle in cases with work-related chronic pain, compared to healthy controls. Ten cases with chronic pain and 13 controls participated in the study. Electromyographic (EMG) signals were recorded from the upper trapezius during five computer work-related tasks. Motor control was assessed using global root-mean-square value (RMS(G)), motor unit action potential (MUAP) rate (number of MUAPs per second, MR) and two MUAP shape parameters, i.e. root-mean-square (RMS(MUAP)) and median frequency (FMED(MUAP)). MR and FMED(MUAP) were higher for the cases than for the controls (P < 0.05). RMS(MUAP) showed a trend for higher values in the chronic pain group (P < 0.13), whereas RMS(G) did not show a significant difference between the groups. The higher MR, FMED(MUAP) and the trend for higher RMS(MUAP) suggest that more high-threshold MUs contribute to low-level computer work-related tasks in chronic pain cases. Additionally, the results suggest that the input of the central nervous system to the muscle is higher in the cases with chronic pain.

The aim of the present study was to investigate the extent to which work-related muscular disorders of the upper trapezius affect the activity of other pain-free muscles, in particular in the biceps brachii. Two groups of female subjects (age >43 years) participated in the study: seven affected subjects with self-reported disorders in the shoulder/neck region (cases) and nine healthy subjects (control group). Multi-channel electromyography (EMG) and force were recorded during maximum voluntary contractions (MVC) and during 6 min sustained contractions (at 30% MVC) of the upper trapezius and biceps brachii on the dominant side. From the EMG signals, the root mean square (RMS), median frequency (MDF) and single motor unit (MU) conduction velocity (CV) were estimated. From the force signal, the coefficient of variation was calculated. All data are presented as mean values and standard deviation. Differences between the cases and controls were found in the MVC force of the upper trapezius, which was lower in cases [253 (70) N] than in controls [357 (75) N], while the coefficient of variation of force during the sustained contraction was increased [cases 5.5 (2.2); controls 4.1 (1.9)]. The RMS (normalized to the RMS at MVC) during the 6 min sustained contractions was significantly lower in the cases than in the controls for both the upper trapezius and the biceps brachii. A tendency towards a smaller increase in the RMS with fatigue was only found in the trapezius muscle [slope: cases 6.5 (14.1) %/min, controls 10.2 (12.9) %/min]. No differences were found between the two subject groups with respect to the MDF and single MU CV in both muscles. While the lower RMS for the trapezius muscles of the cases may reflect changes at the local level, as well as in motor control, the lower biceps activity indicates a change in the central control strategies of the primarily unaffected muscle. Indications for a changed fatigability of the muscle were only found in the trapezius.

The purpose of this study was to investigate changes in muscle activation patterns with respect to timing and amplitude that occur when subjects with stroke walk with and without a walking aid. This knowledge could help therapists in deciding whether or not patients should use a cane or quad stick while walking. Thirteen patients suffering from a first unilateral ischemic stroke participated in the study. Surface electromyography (SEMG) of the erector spinae, gluteus maximus, gluteus medius, vastus lateralis, semitendinosus, gastrocnemius and tibialis anterior of the affected side were measured during three different conditions: (1) walking without a walking aid, (2) walking with a cane and (3) walking with a quad stick. Timing and amplitude parameters of the activation patterns were quantified using an objective burst detection algorithm and statistically evaluated. Results showed a statistically significant and clinically relevant decrease in burst duration of both erector spinae and tibialis anterior when walking with a cane. The amplitude of the vastus lateralis and tibialis anterior dropped when patients walked with a cane and quad stick. The use of a cane should be considered when therapy is given to stroke patients to achieve normal muscle activation patterns.

The aim of this study was to investigate the influence of different intermittent myofeedback training schedules, as provided by a Cinderella-based myofeedback system, on learning relaxation and resistance to extinction of the trapezius muscle, in subjects performing a unilateral gross-motor task. Eighteen healthy subjects performed the task without and with feedback to study baseline and learning relaxation. Subsequently, resistance to extinction was investigated by performing the task without feedback. The gross-motor task consisted of continuously moving the dominant arm between three target areas at a constant pace. Subjects were randomly assigned into three groups, characterized by the sequence of feedback schedules with which the task was performed on 3 consecutive days. Auditory feedback was provided after a 5-, 10-, or 20-s interval when a pre-set level of 80% rest was not reached. Bipolar surface electromyography recordings performed at the dominant upper trapezius muscle were quantified using relative rest time (RRT) and root mean square (RMS) parameters. Learning relaxation was defined as an increase in RRT and a decrease in RMS values. Results showed the highest RRT levels as well as a decrease in RMS for the 10-s schedule. Additionally, the 10-s schedule was unique in its ability to elevate muscular rest above the 20% level, which may be considered relevant in preventing myalgia. None of the three schedules showed resistance to extinction. It was concluded that the 10-s interval was preferred over the 5- and 20-s schedules in learning trapezius relaxation in subjects performing a unilateral gross-motor task.

The purpose of this study was to describe the changes in muscle activation patterns using surface electromyography (sEMG) during walking in patients with cerebral palsy (CP), before and after hamstring lengthening. In the current clinical use of sEMG during walking in CP for pre-operative planning, various authors have observed that timing of muscle activation patterns hardly changes after surgical intervention. This observation is based on tendon transfer studies and visual interpretation of raw EMG signals. Little is known about the effect of muscle lengthening on muscle activation patterns of the lengthened muscles and their antagonists. Fifteen children with CP comprising a total of 23 hamstring lengthenings were included in this study. Surface EMG of semitendinosus and vastus lateralis was measured before and after surgery. Timing parameters of the sEMG patterns were quantified, using an objective burst detection algorithm and statistically evaluated. Results showed that hamstring lengthening causes statistically significant differences in timing of both the semitendinosus and vastus lateralis. It is concluded that timing parameters of operated muscles and their antagonists after surgery do change. The delayed off-time of the semitendinosus and the decreased burst duration of the antagonist (the vastus lateralis) after surgery were the most important changes., (Copyright 2003 Elsevier B.V.)

The purpose of this study was to investigate whether differences in the peripheral and control properties of the neuromuscular system due to long-term preferential use, related to side dominance, affect postural muscles, such as the upper trapezius. Therefore, fatigability properties of the upper trapezius muscles of the dominant and non-dominant side were assessed. Surface EMG signals were detected from the upper trapezius muscles of both sides of nine right- and five left-handed subjects with adhesive linear electrode arrays consisting of eight contact points. Static constant force contractions with the arms 90 degrees abducted were performed by the subjects while holding hand loads of 0 kg, 0.5 kg, and 1 kg. Surface EMG spectral and amplitude variables were computed from the recorded signals. EMG spectral variable rate of change (indicating fatigue) showed a statistically significant difference between the two sides, with the dominant side less fatigable than the non-dominant one. The observed differences held for both the right- and left-handed subject group. A possible explanation for the results is that long preferential use of one side with respect to the other leads to changes in muscle fiber membrane and control properties, in agreement with previous results on limb muscles.

The surface electromyographic (SEMG) signal obtained during gait is often presented as the SEMG profile, the average SEMG activation pattern during one gait cycle. A disadvantage of this method is that it omits the step-to-step variability of the timing of the muscle activation patterns that might be relevant information as a performance measure of motor control and balance. In this paper, a method was used in which every step in the gait cycle could be analysed with respect to the timing of the muscle activation. For this purpose, the approximated generalised likelihood (AGLR) algorithm was implemented and tested. Results of the simulations show that the AGLR was much more accurate than a standard threshold criterion. Timing parameters could be calculated from a SEMG recording during gait and a measure for symmetry and coordination could be extracted. The amplitude distribution within and outside defined bursts is also presented to avoid the less precise classification into on and off patterns.

Several theories about musculoskeletal pain syndromes such as whiplash-associated disorder (WAD) suggest that pain and muscle activity interact and may contribute to the chronicity of symptoms. Studies using surface electromyography (sEMG) have demonstrated abnormal muscle activation patterns of the upper trapezius muscles in the chronic stage of WAD (grade II). There are, however, no studies that confirm that these muscle reactions are initiated in the acute stage of WAD, nor that these muscle reactions persist in the transition from acute neck pain to chronic neck pain disability. We analyzed the muscle activation patterns of the upper trapezius muscles in a cohort of 92 subjects with acute neck pain due to a motor vehicle accident (MVA). This cohort was followed up in order to evaluate differences in muscular activation patterns between subjects who have recovered and those subjects who have not recovered following an acute WAD and developed chronic neck pain. sEMG parameters were obtained at 1, 4, 8, 12, and 24 weeks after an MVA. The level of muscle reactivity (the difference in pre- and post-exercise EMG levels) and the level of muscle activity during an isometric and a dynamic task were used as EMG parameters. The results revealed no elevated muscle reactivity either in the acute stage, or during the follow-up period. The results of both the isometric and dynamic task, showed statistically significant different EMG levels between four neck pain disability subgroups (analysis of variance reaching P-levels of 0.000), with an inverse relationship between the level of neck pain disability and EMG level. Furthermore, follow-up assessments of the EMG level during these two tasks, did not show a time related change. In conclusion, in subjects with future disability, the acute stage is characterized by a reorganization of the muscular activation of neck and shoulder muscles, possibly aimed at minimizing the use of painful muscles. This change of motor control, is in accordance with both the (neurophysiological) 'pain adaptation model' and (cognitive behavioral) 'fear avoidance model'.

Study Design: Surface electromyography measurements of the upper trapezius muscles were performed in patients with a chronic whiplash-associated disorder Grade 2 and those with nonspecific neck pain., Objective: To determine the etiologic relation between acceleration-deceleration trauma and the presence of cervical muscle dysfunction in the chronic stage of whiplash-associated disorder., Summary of Background Information: From a biopsychosocial perspective, the acceleration-deceleration trauma in patients with whiplash-associated disorder is not regarded as a cause of chronicity of neck pain, but rather as a risk factor triggering response systems that contribute to the maintenance of neck pain. One of the contributing factors is dysfunction of the cervical muscles. Considering the limited etiologic significance of the trauma, it is hypothesized that in patients with neck pain, there are no differences in muscle activation patterns between those with and those without a history of an acceleration-deceleration trauma., Methods: Muscle activation patterns, expressed in normalized smooth rectified electromyography levels of the upper trapezius muscles, in patients with whiplash-associated disorder Grade 2 were compared with those of patients with nonspecific neck pain. The outcome parameters were the mean level of muscle activity before and after a physical exercise, the muscle reactivity in response to the exercise, and the time-dependent behavior of muscle activity after the exercise., Results: There were no statistical significant differences in any of the outcome parameters between patients with whiplash-associated disorder Grade 2 and those with nonspecific neck pain. There was only a tendency of higher muscle reactivity in patients with whiplash-associated disorder Grade 2., Conclusions: It appears that the cervical muscle dysfunction in patients with chronic whiplash-associated disorder Grade 2 is not related to the specific trauma mechanism. Rather, cervical muscle dysfunction appears to be a general sign in diverse chronic neck pain syndromes.

The purpose of this study was to investigate which demographic parameters are most important in relation to lumbar dynamometry performance in patients with chronic low back pain (LBP). Forty-five chronic LBP patients participated in this study. Gender, age, weight and height were determined and a lumbar dynamometry measurement was carried out, using the Isostation B200. Student's t-test, ANOVA techniques and correlation coefficients were used to investigate the relationships between each demographic parameter and lumbar dynamometry performance. Stepwise multiple linear regression analyses were performed afterwards to determine which demographic parameters are most important in relation to lumbar dynamometry performance. Results indicate significant relationships (1) between gender, height, weight and all lumbar dynamometry parameters and (2) between age and three of the six isometric torque parameters. No significant relationship was found between age and maximum velocity parameters. Results of the stepwise multiple linear regression analyses show that the demographic parameters explain 27-47% of the variance in maximum isometric strength parameters and 19-25% of the variance in maximum velocity parameters. Gender is the most important demographic parameter, being related to nearly all maximum isometric torque parameters (percentage explained variance 6-37%) and height is the only important demographic parameter related to the velocity parameters (percentage explained variance 19-25%). Weight and age account for only a small amount of variance in lumbar dynamometry parameters (percentage explained variance 5-7%), meaning that these parameters are non-relevant predictors.

Lumbar dynamometry is a potentially useful method for assessing the state of trunk muscles in low back pain (LBP) patients. The purpose of this study was to assess the reliability of lumbar dynamometry measurements in chronic LBP patients by conducting test-retest measurements on different days. Thirty-one men and 14 women with chronic LBP participated in this study. The experiments consisted of three sets of lumbar dynamometry measurements (Isostation B200) carried out on three different days with a 2- to 3-day interval. A standard protocol was administered to all subjects, consisting of a range-of-motion measurement about each axis, a 5 s maximum isometric trial about each axis and five dynamic repetitions about each axis against a resistance set at 25% and at 50% of the maximum isometric torque. Correlation coefficients and regression analysis were used to detect possible learning effects. One-way anova and regression analysis were used to assess the reliability of the measurements. High coefficients were found for the correlation between the first and second lumbar dynamometry measurements. Regression analysis showed that the differences between those measurements were not significant. This means that there was no learning effect operating between the first and second lumbar dynamometry measurements. One-way anova showed a reliability higher than 0.90 for the torque and velocity parameters. Reliability for the range-of-motion parameters was somewhat lower: between 0.76 and 0.94. Regression analysis showed no significant differences between the second and third measurements for the torque and velocity parameters. For range-of-motion parameters significant differences were found. From this study it can be concluded that the Isostation B200 provides reliable measures of torque and velocity parameters, but measures of the range-of-motion parameters are unreliable. No learning effect operates between the first and second lumbar dynamometry measurements, which means that a single measurement, with prior warming up and practice, is sufficient to assess the performance of the LBP patient.
