Objective: B-mode ultrasound can be used to image musculoskeletal tissues, but one major bottleneck is analyses of muscle architectural parameters (i.e., muscle thickness, pennation angle and fascicle length), which are most often performed manually., Methods: In this study we trained two different neural networks (classic U-Net and U-Net with VGG16 pre-trained encoder) to detect muscle fascicles and aponeuroses using a set of labeled musculoskeletal ultrasound images. We determined the best-performing model based on intersection over union and loss metrics. We then compared neural network predictions on an unseen test set with those obtained via manual analysis and two existing semi/automated analysis approaches (simple muscle architecture analysis [SMA] and UltraTrack). DL_Track_US detects the locations of the superficial and deep aponeuroses, as well as multiple fascicle fragments per image., Results: For single images, DL_Track_US yielded results similar to those produced by a non-trainable automated method (SMA; mean difference in fascicle length: 5.1 mm) and human manual analysis (mean difference: -2.4 mm). Between-method differences in pennation angle were within 1.5°, and mean differences in muscle thickness were less than 1 mm. Similarly, for videos, there was overlap between the results produced with UltraTrack and DL_Track_US, with intraclass correlations ranging between 0.19 and 0.88., Conclusion: DL_Track_US is fully automated and open source and can estimate fascicle length, pennation angle and muscle thickness from single images or videos, as well as from multiple superficial muscles. Purpose: Incomplete maturation and aging-induced declines of the neuromuscular system affect postural control both in children and older adults and lead to high fall rates. Age-specific comparisons of the modulation of ankle muscle activation and behavioral center of pressure (COP) indices during upright stance have been rarely conducted. The objective of the present study was to quantify aging effects on a neuromuscular level. Thus, surface electromyography (SEMG) modulation and co-activity of ankle muscles during single-leg standing was compared in healthy children, young adults and seniors., Methods: Postural steadiness (velocity and mean sway frequency of COP), relative muscle activation (SEMG modulation) and co-activation of two ankle muscles (tibialis anterior, TA; soleus, SO) were examined during single-leg stance in 19 children [age, 9.7 (SD 0.5) years], 30 adults [23.3 (1.5) years] and 29 seniors [62.7 (6.1) years]., Results: Velocity of COP in medio-lateral and anterior-posterior directions, mean sway frequency in anterior-posterior direction, relative muscle activation (TA and SO) and co-activation revealed large age effects (P < 0.003, η p 2  > 0.14). Post-hoc comparisons indicated higher COP velocities, anterior-posterior frequencies, relative SO activation and co-activation in children and seniors when compared with adults. Relative TA activation was higher in children and adults compared with seniors (P < 0.001)., Conclusions: Increased postural sway in children and seniors seems to be counteracted with higher TA/SO co-activity and SO modulation. However, TA modulation is higher in children and adults, whereas seniors' TA modulation capacity is diminished. An aging-induced decline of TA motor units might account for deteriorations of TA modulation in seniors.

Objectives: The present study aimed to assess the between day reliability of isokinetic and isometric peak torque (PT) during trunk measurement on an isokinetic device (IsoMed 2000)., Design: Test-retest-protocol on five separate days., Participants: Fifteen healthy sport students (8 female and 7 male) aged 21 to 26., Main Outcome Measures: PT was assessed in isometric back extension and flexion as well as right and left rotation. Isokinetic strength was captured at a speed of 60°/s and 150°/s for all tasks., Results: For none of the assessed parameters a meaningful variation in PT during test days was observed. Relative reliability (ICC = 0.85-0.96) was excellent for all tasks. Estimates of absolute reliability as Coefficient of Variation (CoV) and Standard Error of Measurement (SEM in Nm/kg lean body mass) remained stable for isometric (6.9% < CoV < 9.4%; 0.15 < SEM < 0.23) and isokinetic mode (60°/s: 3.7% < CoV < 8.6%; 0.08 < SEM < 0.24; 150°/s: 6.9% < CoV < 12.4%; 0.10 < SEM < 0.31). In contrast, reliability between familiarization day and day 1 was lower (6.6% < CoV < 26.2%; 0.10 < SEM < 0.65)., Conclusions: Trunk strength measurement in flexion and extension or trunk rotation in either isometric or isokinetic condition is highly reliable. Background: Practice-induced effects of specific balance training on brain structure and activity in elderly people are largely unknown., Aim: In the present study, we investigated morphological and functional brain changes following slacking training (balancing over nylon ribbons) in a group of elderly people., Methods: Twenty-eight healthy volunteers were recruited and randomly assigned to the intervention (mean age: 62.3±5.4years) or control group (mean age: 61.8±5.3years). The intervention group completed six-weeks of slackline training. Brain morphological changes were investigated using voxel-based morphometry and functional connectivity changes were computed via independent component analysis and seed-based analyses. All analyses were applied to the whole sample and to a subgroup of participants who improved in slackline performance., Results: The repeated measures analysis of variance showed a significant interaction effect between groups and sessions. Specifically, the Tukey post-hoc analysis revealed a significantly improved slackline standing performance after training for the left leg stance time (pre: 4.5±3.6s vs. 26.0±30.0s, p<0.038) as well as for tandem stance time (pre: 1.4±0.6s vs. post: 4.5±4.0s, p=0.003) in the intervention group. No significant changes in balance performance were observed in the control group. The MRI analysis did not reveal morphological or functional connectivity differences before or after the training between the intervention and control groups (whole sample). However, subsequent analysis in subjects with improved slackline performance showed a decrease of connectivity between the striatum and other brain areas during the training period., Conclusion: These preliminary results suggest that improved balance performance with slackline training goes along with an increased efficiency of the striatal network., (Copyright © 2016 Elsevier B.V. Ageing impairs body balance and increases older adults' fall risk. Balance training can improve intrinsic fall risk factors. However, age comparisons of muscle activity responses during balance tasks are lacking. This study investigated relative muscle activity, muscle coordination and postural sway during various recommended static balance training tasks. Muscle activity (%MVC), amplitude ratios (AR) and co-activity (CAI) were determined during standing tasks for 30s (1: double limb stance on a foam surface, eyes open; 2: double limb stance on firm ground, eyes closed; 3: double limb stance, feet in step position on a foam surface, eyes open; 4: double limb stance, feet in step position on firm ground, eyes closed; 5: single limb stance on firm ground, eyes open) in 20 healthy young adults (24±2 y) and 20 older adults (73±6 y). Surface electromyography (SEMG) was applied (SENIAM guidelines) to ankle (tibialis anterior, soleus, medial gastrocnemius, peroneus longus) and thigh (vastus lateralis, vastus medialis, biceps femoris, semitendinosus) muscles (non-dominant leg). For performance and injury prevention in sport, core strength and endurance are focused prerequisites. Therefore we evaluated characteristics of trunk muscle activation and performance during strength-endurance related trunk field tests. Strength-endurance ability, as total time to failure, and activation of trunk muscles was measured in 39 football players of the highest German female football league (Bundesliga) (N = 18, age: 20.7 y [SD 4.4]) and the highest national male under-19 league (N = 21, age: 17.9 y [0.7]) in prone plank, side plank, and dorsal position. Maximal isometric force was assessed during trunk extension and flexion, rotation, and lateral flexion to normalize EMG and to compare with the results of strength-endurance tests. For all positions of endurance strength tests, a continuous increase in normalized EMG activation was observed (P < .001). Muscle activation of the rectus abdominis and external oblique in prone plank position exceeded the maximal voluntary isometric contraction activation, with a significantly higher activation in females (P = .02). We conclude, that in the applied strength-endurance testing, the activation of trunk muscles was high, especially in females. As high trunk muscle activation can infer fatigue, limb strength can limit performance in prone and side plank position, particularly during high trunk muscle activation.

Slackline training (balancing on nylon ribbons) has been shown to improve neuromuscular performance in children and adults. Comparable studies in seniors are lacking. Thus, 32 seniors were randomly assigned [strata: age, gender, physical activity (PA)] to an intervention [INT; n = 16, age: 65 ± 4 years, PA: 9 ± 5 h/week] or control [CON, n = 16, age: 63 ± 4 years, PA: 8 ± 4 h/week] group. Slackline training was given for 6 weeks (3 times per week, attendance 97%). Static and slackline standing balance performance, force development, and maximal strength of the ankle muscles were assessed before and after slackline training. Muscle activity (lower limb and trunk) was recorded during balance testing. Moderate to large group × time interactions (0.02 < P < 0.04, 0.11 < ηp (2)  < 0.17) in favor of INT were found for slackline standing times (INT: left, +278%, P = 0.02; right, +328%, P = 0.03; tandem, +94%, P = 0.007) and muscle activity during single-limb slackline standing [INT: right: rectus abdominis (RA), P = 0.003, -15%; multifidus (MF), P = 0.01, -15%; left: tibialis anterior (TIB), P = 0.03, -12%; soleus (SOL), P = 0.006, -18%; RA, P = 0.04, -11%; MF, P = 0.01, -16%; gastrocnemius medialis (GM), P = 0.02, -19%]. Static balance performance, ankle strength, and power were not affected. Slackline training induced large task-specific improvements of slackline standing performance accompanied with reductions of lower limb and trunk muscle activity. The study examined effects of traditional balance vs. mat-based Pilates training on balance and trunk strength in healthy community dwellers. Forty-eight seniors were either stratified to a balance training group (BAL, n=16, 69.1 (SD 5.8) y), Pilates training group (PIL, n=17, 70.8 (6.5) y) or control group (CON, n=15, 69.2 (6.1) y). BAL performed traditional balance training, while PIL conducted mat-based Pilates-exercises (8 weeks, 2 sessions/week 66 min each). Balance performance (single limb stance and perturbed kneeling, Y-Balance test), dynamic and isometric trunk flexion and extension were assessed during pre- and post-testing. According to the magnitude-based inference approach, substantial positive effects in favor of BAL compared to CON were found for the Y-balance score (right leg, effect size (d)=0.68; left leg, d=0.56), trunk extension (d=0.68) and single leg stance (right leg, d=0.61; left leg, d=0.38). Dynamic (d=0.32) and isometric (d=0.15) trunk flexion revealed unclear effects. For the Y-balance score (right leg, d=0.48, left leg, d=0.75) and single leg stance (right leg, +d=0.61%; left leg, d=0.67), interestingly, BAL substantially exceeded PIL. Aim: Physical fitness is related to injuries, attrition and military ability in military organisations. Therefore, all military organizations of the North Atlantic Treaty Organizations (NATO) test their employees' physical fitness at least once a year. The sit-up test is part of most of the fitness test batteries used. A possible alternative to the sit-up test is the global trunk muscle strength test (TMS). The aim of the present study was to compare the predictability of injuries, attrition and military ability between TMS and sit-up test performances., Methods: A total of 230 male recruits in a Swiss Army fusilier company completed TMS and sit-up tests in week 1 of military training school. During the following 13 weeks, injuries, attrition and military ability data were collected. Statistical analysis included backward binary regression and receiver operating characteristic (ROC) curve analysis to compare the discriminative power of TMS and the sit-up test to predict injuries, attrition and military ability., Results: ROC analysis revealed larger areas under the curve for total injuries, attrition and military ability for the TMS (areatotal injuries=0.58; areaattrition=0.60; areamilitary ability=0.59) than for the sit-up test (areatotal injuries=0.53; areaattrition=0.50; areamilitary ability=0.56). Binary logistic regression analysis revealed low body mass index, low TMS performance and cigarette smoking to be potential risk factors for injuries; while sit-up performance was extracted from the model., Conclusion: The TMS seems to be a valid alternative to the sit-up test in a military setting due to its appropriate results in predicting injuries in the present study.

Background: Available evidence suggests that young adults and seniors use different strategies to adjust for increasing body sway during quiet standing. Altered antagonist muscle co-activation and different ankle muscle coordination patterns may account for this finding. Consequently, we aimed at addressing whether aging leads to changes in neuromuscular coordination patterns as well as co-activation during quiet stance. We additionally investigated whether a bout of high intensity interval training additionally alters these patterns., Methods: Twenty healthy seniors (age: 70 ± 4 y) and twenty young adults (age: 27 ± 3 y) were enrolled in the present study. In between the testing procedures, four consecutive high-intensity intervals of 4 min duration at a target exercise intensity of 90 to 95% HRmax were completed on a treadmill. The total center of pressure (COP) path length displacement served as standing balance performance outcome. In order to assess ankle muscle coordination patterns, amplitude ratios (AR) were calculated for each muscle (e.g. tibialis anterior (TA) [%] = (TA × 100)/(gastrocnemius medialis (GM) + soleus (SOL) + peroneus longus (PL) + TA). The co-activation was calculated for the SOL and TA muscles computing the co-activation index (CAI = 2 × TA/TA + SOL)., Results: Seniors showed an inverted ankle muscle coordination pattern during single limb stance with eyes open (SLEO), compared to young adults (rest: GM, S: 15 ± 8% vs Y: 24 ± 9%; p = 0.03; SOL, S: 27 ± 14% vs Y: 37 ± 18%; p = 0.009; TA, S: 31 ± 13% vs Y: 13 ± 7%; p = 0.003). These patterns did not change after a high-intensity training session. A moderate correlation between amplitude ratios of the TA-contribution and postural sway was observed for seniors during SLEO (r = 0.61). Ankle co-activation was twofold elevated in seniors compared to young adults during SLEO (p < 0.001). These findings were also not affected by high intensity training., Conclusion: Increased ankle co-activation in the anterior-posterior plane and inverted ankle muscle coordination pattern merely occurred during single-leg stance. Seniors with decreased postural control showed higher TA contributions during SLEO. These neuromuscular changes are not affected by acute intermittent high intensity aerobic exercise.

Background: Balance and strength training can reduce seniors' fall risk up to 50%. Available evidence suggests that acute bouts of neuromuscular and endurance exercise deteriorate postural control. High-intensity endurance training has been successfully applied in different populations. Thus, it seemed valuable to examine the acute effects of high-intensity interval training (HIIT) on neuromuscular performance in seniors and young adults., Objective: The acute impact of a HIIT session on balance performance and muscle activity after exercise cessation and during post-exercise recovery was examined in young and old adults. We intended to investigate whether a transient exercise-induced fall-risk may occur in both groups., Methods: 20 healthy seniors (age 70 (SD 4) years) and young adults (age 27 (SD 3) years) were examined on 3 days. After exhaustive ramp-like treadmill testing in order to determine maximal heart rate (HRmax) on the first day, either a 4 × 4 min HIIT at 90% of HRmax or a control condition (CON) was randomly performed on the second and third day, respectively. Balance performance (postural sway) was assessed during single limb stance with open eyes (SLEO) and double limb stance with closed eyes (DLEC). EMG was recorded for the soleus (SOL), anterior tibialis (TIB), gastrocnemius (GM) and peroneus longus (PL) muscles at the dominant leg. All measures were collected before, immediately as well as 10, 30 and 45 min after HIIT and CON, respectively., Results: Compared to CON, HIIT induced significant increases of postural sway immediately after exercise cessation during SLEO in both groups (adults: p < 0.001, Δ = +25% sway; seniors: p = 0.007, Δ = +15% sway). Increased sway during DLEC was only found for seniors immediately and 10 min after HIIT (post: p = 0.003, Δ = +14% sway, 10 min post: p = 0.004, Δ = +18% sway). Muscle activity was increased during SLEO for TIB until 10 min post in seniors (0.008 < p < 0.03) and immediately after HIIT in adults (p < 0.001)., Conclusion: HIIT training may cause an acute 'open-fall-window' with a transient impairment of balance performance for at least 10 min after exercise cessation in both groups. Occluded vision in seniors seems to prolong this period up to 30 min. Purpose: To date, there has been limited research examining the influence of pacing pattern (PP) on middle-distance swimming performance. As such, the purpose of the current study was to examine the influence of PP manipulation on 400-m freestyle swimming performance., Methods: 15 front-crawl swimmers (5 female, 10 male; age 18 ± 2 y) performed 3 simulated 400-m swimming events. The initial trial was self-selected pacing (PPSS). The following 2 trials were performed in a counterbalanced order and required participants to complete the first 100 m more slowly (PPSLOW: 4.5% ± 2.2%) or quickly (PPFAST: 2.4% ± 1.6%) than the PPSS trial. 50-m split times were recorded during each trial., Results: Overall performance time was faster in PPSS (275.0 ± 15.9 s) than in PPFAST (278.5 ± 16.4 s, P = .05) but not significantly different from PPSLOW (277.5 ± 16.2 s, P = .22). However, analysis for practical relevance revealed that pacing manipulation resulted in a "likely" (>88.2%) decrease in performance compared with PPSS., Conclusion: Moderate manipulation of the starting speed during simulated 400-m freestyle races seems to affect overall performance. The observed results indicate that PPSS is optimal in most individuals, yet it seems to fail in some swimmers. Future research should focus on the identification of athletes possibly profiting from manipulations.

The study investigated the effects of slackline training (rope balancing) on balance, jump performance and muscle activity in children. Two primary-school classes (intervention, n=21, INT: age: 10.1 (SD 0.4) y, weight: 33.1 (4.5) kg; control, n=13, CON: age: 10.0 (SD 0.4) y, weight: 34.7 (7.4) kg) participated. Training was performed within 6 weeks, 5 times per week for 10 min each day. Balance (static and dynamic stance), countermovement jumps, reverse balancing on beams (3, 4.5 and 6 cm width), slackline standing (single- and double-limb) and electromyographic activity (soleus, gastrocnemius, tibialis anterior) were examined. INT significantly improved single- and double-limb slackline standing (double limb: 5.1 (3.4) s-17.2 (14.4) s; right leg: 8.2 (5.8) s-38.3 (36.0) s; left leg: 10.6 (5.8) s-49.0 (56.3) s; p<0.001; 0.17<ηp²<0.22). Reduced left-leg dynamic sway (-20.8%, p=0.06, ηp²=0.10), improved 4.5 cm balancing (+18.5%, p=0.08, ηp²=0.10) and decreased muscle activity during slackline standing for the mm. soleus (-23%, p=0.10, ηp²=0.18) and tibialis anterior (-26%, p=0.15, ηp²=0.14) was observed for INT. Jump performance remained unchanged (p=0.28, ηp²=0.04). In conclusion, daily slackline training results in large slackline-specific balance improvements. The present study aimed at comparing a high-volume, low-intensity vs. low-volume, high-intensity swim training. In a randomized cross-over design, 10 competitive swimmers performed two different 4-week training periods, each followed by an identical taper week. One training period was characterized by a high-training volume (HVT) whereas high-intensity training was prevalent during the other program (HIT). Before, after two and four weeks and after the taper week subjects performed psychometric and performance testing: profile of mood states (POMS), incremental swimming test (determination of individual anaerobic threshold, IAT), 100 m and 400 m. A small significant increase in IAT was observed after taper periods compared to pre-training (+ 0.01 m/s; p = 0.01). Maximal 100-m and 400-m times were not significantly affected by training. The POMS subscore of "vigor" decreased slightly after both training periods (p = 0.06). None of the investigated parameters showed a significant interaction between test-time and training type (p > 0.13). Nearly all (83 %) subjects swam personal best times during the 3 months after each training cycle. It is concluded that, for a period of 4 weeks, high-training volumes have no advantage compared to high-intensity training of lower volume.

Purpose: The work rate (WR) corresponding to ventilatory threshold (VT) is an appropriate intensity for regenerative and low-intensity training sessions. During incremental ramp exercise, VO(2) increase lags behind WR increase. Traditionally, a VO(2) time delay (t(d)) of 45 seconds is used to calculate the WR at VT from such tests. Considerable inaccuracies were observed when using this constant td. Therefore, this study aimed at reinvestigating the temporal relationship between VO(2) and WR at VT., Methods: 20 subjects (VO(2peak) 49.9 to 72.6 mL x min-1 x kg-1) performed a ramp test in order to determine VT and a subsequent steady-state test during which WR was adjusted to elicit the VO(2) corresponding to VT. The difference in WR and heart rate at VT was calculated between the ramp and the steady-state test (WR(diff), HR(diff)) as well as the time delay corresponding to WR(diff) during ramp exercise., Results: Mean values were td = 85 + or - 26 seconds (range 38 to 144), WR(diff) = 45 + or - 12 W (range 23 to 67), HR(diff) = 1 + or - 9 beats/min (range -21 to +15). The limits of agreement for the difference between WR at VT during ramp and steady-state exercise were + or - 24 W. No significant influence on td, WR(diff), or HR(diff) from differences in endurance capacity (VO(2peak) and VT; P > .10 for all correlations) or ramp increment (P = .26, .49, and .85, respectively) were observed., Conclusion: The wide ranges of td, WR(diff), and HR(diff) prevent the derivation of exact training guidelines from single-ramp tests. It is advisable to perform a steady-state test to exactly determine the WR corresponding to VT.
