The article describes the case of a 72-year-old white man with multiple myeloma and an 11-year history of monoclonal gammopathy of undetermined significance (MGUS). During a follow-up visit in 2009, his laboratory values, which include creatinine, calcium and albumin increased. His skeletal survey showed negative results for lystic lesions. Two months later, the patient complained of lower back pain located in the sacrum and left iliac bone areas. The patient started lenalidomide/dexamethasone treatment.

A letter to the editor is presented in response to an article which found that the prevalence of monoclonal gammopathy of undetermined significance (MGUS) among white patients 50 years older is twice than African American and obese individuals by Doctors Ola Landgren and Adam Justice Waxman.
