Background: Sarcomas are rarely seen in the head and neck region. They make up less than 1% of all malignant head and neck tumours. Not only the location, size and systemic manifestation, but also the histological differentiation plays an essential role in establishing the best treatment. If resectable, surgical removal of the tumour with clear margins is the preferred method. Adjuvant chemo- and/or radiotherapy might be considered in the case of high-grade lesions or narrow surgical margins., Patient: This paper reports a 23-year-old female patient who underwent routine surgical removal of unerupted wisdom teeth without clinical evidence of any follicular lesion and was diagnosed with a low-grade malignant fibrosarcoma of the dental follicle of the lower left unerupted third molar., Conclusion: This case report raises the question as to whether histological examination of all removed dental follicles should be carried out on a routine basis.

A questionnaire on third-molar problems was mailed to a systematic random sample of 200 Norwegian general dental practitioners in November 1991. The return rate was 88%. Similar questions were given to 59 5th-year dental students, with a return rate of 83%. Ten third-molar cases were presented by means of copies of radiographs and written information. The following conclusions were drawn: Apart from a terminology problem consisting of lack of discrimination between the terms retention and impaction, overall good diagnostic abilities were shown by both practitioners and students. Receiver operating characteristic analysis of treatment decisions indicated that students were more radical than practitioners. The students had an increased referral rate for third-molar surgery, whereas the practitioners indicated a more selective practice of referring advanced cases.

The purpose of this study was to describe cases of oral cancer in which the initial presenting symptoms were considered to be due to third molars and in which the underlying carcinoma therefore escaped early diagnosis. Between 1986 and 1990 a total of five cases with squamous cell carcinoma associated with a third molar (impacted, partially or totally erupted, or recently extracted) were treated at our unit. Because of acute or chronic symptoms related to disturbed eruption of third molars, the detection of a simultaneous malignant change and referral were delayed. In conclusion, all third molars, especially impacted, partially erupted, or overerupted ones, should be examined critically, especially if the patient has symptoms.

This study was examined the relationships between the condition of dentition of lower third molar and malocclusions and shape of mandibles, and to predict eruption for third molars by their relationships. Data were obtained from 90 degrees and 60 degrees rotated cephalometlic radiographs, orthopantomographic radiographs, mandibular occlusal films and gnathostatic models of 101 males showing a complete permanent dentition anterior to the second molars and who had not had extraction lower their molars and orthodontic treatments, were selected from 147 dental students at Asahi University. they were divided into 8 groups by condition of lower third molar dentitions. Bilateral presence of lower third molar occurred in 69.3% and unilateral in 20.8% on 60 degrees rotated cephalometlic radiographs. Bilateral eruption or half eruption occurred in 24.7% and impaction in 44.6% on gnathostatic models. Coronal arch length of erupted cases was longer than that of impacted cases. A apired t-test revealed the significant difference (p less than 0.05). Over bite of impacted cases were deeper than that of erupted cases (p less than 0.05). Arch length discrepancy of presented and impacted cases were larger. There were normal occlusions in each group. Most mal-occlusions were prognathia, second to deep bite, bi-maxillary. The most frequent lower arch conditions were crowding (65%). I. M. P. A. of bilateral impacted cases were langer. Mandiblar length (Cd-Pog) of erupted cases were longer. The distance from Xi-point to the distal surface of second molar was border at 25mm for lower third molar eruption.
