Background: The surgical removal of impacted third molars is considered a routine procedure for oral and maxillofacial surgeons. However, the distribution of impacted molars varies among different populations. A study into impaction patterns in the German population is currently not described. The aim of this retrospective study is to determine patterns of impacted third molars in a sample of German population, to identify gender and age specific differences as well as further risk factors for impacted molar extraction and to compare these results with other studies., Materials and Methods: The clinical findings and digital panoramic radiographs of 84 patients with a total of 243 impacted third molars that had been subjected for tooth extraction, from January to September 2023, were collected and analyzed. All third molars were analyzed according to Winter´s angulation, Pell and Gregory Score of depth and relationship to ramus. Furthermore, inferior alveolar nerve and maxillary sinus proximity were identified through a newly developed risk score and an analysis of patient´s gender and age regarding impaction patterns was performed., Results: No significant gender specific differences were found regarding the impaction patterns. Third molar angulations significantly differed regarding patient´s age (cut-off 26 years). Objective: The aim of this study is to determine if a deep learning (DL) model can predict the surgical difficulty for impacted maxillary third molar tooth using panoramic images before surgery., Materials and Methods: The dataset consists of 708 panoramic radiographs of the patients who applied to the Oral and Maxillofacial Surgery Clinic for various reasons. Each maxillary third molar difficulty was scored based on dept (V), angulation (H), relation with maxillary sinus (S), and relation with ramus (R) on panoramic images. The YoloV5x architecture was used to perform automatic segmentation and classification. To prevent re-testing of images, participate in the training, the data set was subdivided as: 80 % training, 10 % validation, and 10 % test group., Results: Impacted Upper Third Molar Segmentation model showed best success on sensitivity, precision and F1 score with 0,9705, 0,9428 and 0,9565, respectively. S-model had a lesser sensitivity, precision and F1 score than the other models with 0,8974, 0,6194, 0,7329, respectively., Conclusion: The results showed that the proposed DL model could be effective for predicting the surgical difficulty of an impacted maxillary third molar tooth using panoramic radiographs and this approach might help as a decision support mechanism for the clinicians in peri‑surgical period., Competing Interests: Declaration of competing interest The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (Copyright © 2024 Elsevier Masson SAS. Purpose: To compare the buccal alveolar bone thickness of mesioangulated mandibular impacted third molars (MITM) with buccal versus lingual inclination using cone-beam computed tomography (CBCT)., Methods: This retrospective cross-sectional study evaluated 69 individuals (39 women and 30 men) who presented a total of 101 mesioangulated MITM according to the Winter classification and in B position according to the Pell and Gregory classification. The average age was 22.99 ± 3.94 years. The superior, medial and inferior alveolar thickness regarding buccal (n = 44) or lingual (n = 57) mandibular third molar inclination were measured. T test or Mann-Whitney U test and finally, a multiple linear regression were applied (p < 0.05)., Results: The buccal alveolar bone thickness of mesioangulated mandibular impacted third molars was significantly greater in the group with lingual inclination compared to the group with buccal inclination, in the superior region (6.21 ± 3.27 vs. 4.85 ± 3.06; p = 0.036). The lingual inclination significantly influenced the buccal alveolar bone thickness in the middle region (p = 0.011)., Conclusions: The mesioangulated MITM in B position with lingual inclination have a greater thickness of the superior and medial buccal alveolar bone than the MITM with buccal inclination. These results should be considered during MITM diagnosis and surgical planning.

The approximity of the inferior alveolar nerve (IAN) to the roots of lower third molars (M3) is a risk factor for the occurrence of nerve damage and subsequent sensory disturbances of the lower lip and chin following the removal of third molars. To assess this risk, the identification of M3 and IAN on dental panoramic radiographs (OPG) is mandatory. In this study, we developed and validated an automated approach, based on deep-learning, to detect and segment the M3 and IAN on OPGs. As a reference, M3s and IAN were segmented manually on 81 OPGs. A deep-learning approach based on U-net was applied on the reference data to train the convolutional neural network (CNN) in the detection and segmentation of the M3 and IAN. Subsequently, the trained U-net was applied onto the original OPGs to detect and segment both structures. Dice-coefficients were calculated to quantify the degree of similarity between the manually and automatically segmented M3s and IAN. The mean dice-coefficients for M3s and IAN were 0.947 ± 0.033 and 0.847 ± 0.099, respectively. Deep-learning is an encouraging approach to segment anatomical structures and later on in clinical decision making, though further enhancement of the algorithm is advised to improve the accuracy.

Odontoma is considered to be the most common odontogenic tumor of the oral cavity. Most odontomas are asymptomatic and are discovered during routine radiographic investigations and can cause disturbances in the eruption of the teeth, most commonly delayed eruption or deflection. By a retrospective study design, demographic and clinical data regarding patients who presented odontomas from year 1995 to 2015 were obtained in order and the influence of active therapy on the dentition and on the treatment of impacted teeth was analyzed. Forty-five patients (mean age 14.2 years) with 29 complex and 16 compound odontomas were included in this retrospective study. Initial symptoms were delayed eruption of permanent teeth (n = 25), pain (n = 6), swellings (n = 4), and no symptoms (n = 10); 31 patients were discovered by incidence, all of them via panoramic radiographs. The mandible/maxilla ratio was about 2:1 (31/15). Thirty-two out of 45 odontomas were in close proximity of at least 1 tooth (n = 21 at incisive). A total of 12 teeth were extracted (complex: n = 8; compound: n = 4). Of the nonextracted teeth, 33 teeth were displaced and retained. Of those, 29 teeth were aligned through orthodontic-surgical approach and 4 teeth erupted spontaneously after surgery during the follow-up period. An early detection of odontoma is more likely an accidental radiological finding, hence the need for routine radiographic analysis should be emphasized. Early diagnosis of odontomas in primary dentition is crucial in order to prevent later complications, such as impaction or failure of eruption of teeth.

Objectives: The purpose of this study was to assess the pattern of third molar impaction and associated symptoms in a Central Anatolian Turkish population., Material and Methods: A total of 2,133 impacted third molar teeth of 705 panoramic radiographs were reviewed. The positions of impacted third molar teeth on the panoramic radiographs were documented according to the classifications of Pell and Gregory and of Winter. The presence of related symptoms including pain, pericoronitis, lymphadenopathy and trismus was noted for every patient. Distributions of obtained values were compared using the Pearson χ2 test. Nonparametric values were analyzed using the Mann-Whitney U test and Kruskal-Wallis test., Results: The mean age of the subjects was 30.58 ± 11.98 years (range: 19-73); in a review of the 2,133 impacted third molar teeth, the most common angulation of impaction in both maxillaries was vertical (1,177; 55%). Level B impaction was the most common in the maxilla (425/1,037; 39%), while level C impaction was the most common in the mandible (635/1,096; 61%). Pain (272/705; 39%) and pericoronitis (188/705; 27%) were found to be the most common complications of impaction. Among 705 patients (335 males, 370 females), pericoronitis was more prevalent in males (101; 30%) and usually related to lower third molars (236; 22%). The retromolar space was significantly smaller in females (p < 0.05). Moreover, there was a significant difference in retromolar space for the area of jaw (maxillary: 11.3 mm; mandibular: 14.2 mm) and impaction level (A: 14.7 mm; B: 11.1 mm; C: 10.3 mm; p < 0.05)., Conclusion: The pattern of third molar impaction in a Central Anatolian Turkish population was characterized by a high prevalence rate of level C impaction with vertical position. The objective of this study was to assess the association between the histopathological diagnoses of dentigerous cysts and pericoronal follicles and the positions of impacted third molars. The sample comprised 151 cases, diagnosed as dentigerous cysts and pericoronal follicles. Most patients were female (70.9%), white (90.1%), and age 20 years or older (63.6%). Ninety-eight cases (64.9%) were diagnosed as pericoronal follicles and 53 (35.1%) as dentigerous cysts. Teeth that developed dentigerous cysts were predominantly located in the mandible (84.9%), in a mesioangular position (49.1%), and in the 20- to 29-year age group (66.0%). The mandible seems to be the most frequent location of impacted teeth, probably because mandibular tooth eruption is more difficult, as well as dentigerous cysts, probably because the long duration of impaction increases the exposure to inflammatory stimuli and, thus, the potential for formation of dentigerous cysts. The findings also suggest that the incidence of dentigerous cysts tends to increase with age, especially for mandibular teeth in mesioangular positions.

This study evaluated the validity of panoramic radiography and cone beam computed tomography (CBCT) in the assessment of mandibular canal and impacted third molar. In this descriptive-analytical study, 58 mandibular third molars from 42 patients who showed a close relationship between impacted third molar and canal on panoramic radiographs were selected. They were then classified into seven radiographic markers in panoramic radiographs (superimposition, darkening of the root, interruption of the white lines, root narrowing, canal diversion, canal narrowing, and also closed distance in OPG <1mm). The groups of markers were further assessed with CBCT to see presence or absence of contact. The three most common markers seen in panoramic images are superimposition, interruption of white line and root darkening. In CBCT, superimposition marker always presented higher frequency of contact with canal compared to non-contact group. There are 31% of teeth presented with interruption of white lines and there are 29.3% of teeth presented with superimposition. About 55.6% and 35.3% of the impacted mandibular third molars which indicated interruption of white lines and superimposition also indicated contact in the CBCT respectively. Presence or absence of radiological sign in panoramic radiography was not properly predict a close relationship with third molar and it is suggested that in case of tooth-canal overlapping, the patient should be referred for CBCT assessment.

Background/aim: It is considered that the shortage of space is the major cause of the third molar impaction. The aim of this study was to establish the frequency of insufficient lower third molar eruption space in Serbian population, to question the differences in this frequency in the subjects of different age, to determine the influence of the lower third molar space (retromolar space) size on third molar eruption, and to investigate a possible correlation between the size of gonial angle and the space/third molar width ratio., Methods: Digital orthopantomograms were taken from 93 patients divided into two groups: early adult (16-18 years of age) and adult (18-26) patients. Retromolar space, mesiodistal third molar crown width, gonial angle and eruption levels were measured., Results: The space/third molar width in early adult subjects was smaller (p < 0.0001) and insufficient space was significantly more frequent (p = 0.0003) than in adult patients. Considerably more third molars erupted in case of enough space in both age groups (p < 0.0001). There was no difference between the means of gonial angle size in relations to the available space., Conclusions: The retromolar space/third molar width ratio is more favorable in adult subjects. Gonial angle is not in correlation with the retromolar space/third molar width ratio.

Spreading odontogenic infections are a common source of hospital admissions to Oral and Maxillofacial Surgery (OMFS) units. This report describes an unusual reaction to routine treatment for a spreading odontogenic infection in a healthy male with no known allergies, requiring the patient to be managed supportively in the resuscitation room. The patient deteriorated rapidly after the administration of paracetamol, intravenous fluids, steroids and antibiotics, demonstrating delusional behaviour, fever, rigors, tachycardia and hypoxia. Fever associated with sepsis can lead to confusional states, but similar symptoms have been described in the literature as a reaction to antibiotic therapy known as Jarisch-Herxheimer (J-H) reaction. This is potentially the first time a J-H like reaction has been described in the context of dental sepsis. The authors feel that the OMFS team should be aware of possible sequelae of medical therapy in patients with acute dental sepsis and be confident in their management of these complications.

We present a case of a 36-year-old female with a benign fibro-osseous lesion consistent with cemento-osseous dysplasia (COD) that was located coronally to the crown of an impacted mandibular third molar tooth and was associated with a complex odontoma. The pathogenesis of this unusual association is discussed and the odontogenic origin of COD is suggested.

Introduction: We studied the prognosis of upper and lower third molar evolution., Patients and Methods: Seventy-eight patients were included after a clinical examination, dental casts, panoramic and lateral cephalometric radiographs on inclusion (T1) and two years after (T2). Five parameters were studied: retromolar space (RMS) between the third molar (M3) and the anterior ridge of the ramus; RMS/M3 crown diameter ratio, distances (PTV-M(1)) and (X(i)-Mi(2)); and third molar angle with occlusal plane., Results: They were noted at T1 and T2. The gain of retromolar space from T1 to T2 reached 1.2 to 2.2mm without extraction, 2 to 2.7mm with PM extraction, and 4.5 to 6.8mm with M1 extraction. The "RMS/M3 diameter" ratio increased in the maxilla from 0.6 to 0.8 and from 0.5 to 0.8 in the mandible (P=0.01) between T1 and T2. PTV-M1 increased significantly from 17.1 to 19.9mm (P<0.001), and Xi-M2i increased significantly from 18.5 to 22.4mm (P<0.001) between T1 and T2., Discussion: The extraction of premolars has little influence on the variation of the retromolar space; conversely first molar extraction increases that space. Objective: The aim of this study was to evaluate and establish any similarity between the associated symptoms, pathologies, positions, and angulation types of bilateral occurring mandibular third molars among a group of young adult patients., Study Design: A total of 342 patients (167 females, 175 males), aged between 20 and 25 years (mean: 22.2, SD: 1.8) participated in the study. Clinical and radiographic examinations were performed. Eruption status, mucosal and bony coverage type, presence of pain, pericoronitis, suppuration, ulceration, caries in third molar, distal caries in second molar, bone loss, root resorption, or cyst or tumor formation was investigated in addition to the position and the angulation of each tooth. Patients having at least one completely or partially erupted mandibular third molar were classified as group 1 and patients having bilateral impacted mandibular third molars were classified as group 2., Results: No significant difference was found between the symptoms and pathologies related with the mandibular right third molar (RM) and the left third molar (LM) among both groups and genders (P > .05). In the total sample, no significant difference was found between the RM and the LM in terms of mucosal coverage type, bony coverage type, and position both in group 1 and group 2 (P > .05); but gender had an influence on the bony coverage type and ramus distance of the RM and the LM in group 2 (P < .05). In the total the sample, symmetry was present for horizontal or distoangular and vertical or distoangular angulations in group 1 and group 2, respectively. Gender was found to also have an impact on angulation symmetry., Conclusion: In most cases, a similarity was present between the symptoms and pathologies related with the bilateral mandibular third molars; but symmetry in position and angulation differed according to eruption status, angulation type, and gender.

Concrescence is an uncommon developmental anomaly that may influence exodontia as well as periodontal, endodontic, orthodontic and even prosthodontic diagnosis and treatment planning. Unexpected complications arising from this condition may lead to legal complications. To minimize risk and adverse outcome of treatment, consideration should be given to recognizing this condition and, possibly, modifying treatment planning. This article presents an unsuspected case of concrescence with a review of the literature.

Concrescence of teeth is a condition showing a union of adjacent teeth by only cementum. In all the previously reported cases, the union has been observed between the roots of the affected teeth. Here, we describe the first case that showed a concrescence of the crown of an impacted tooth and the roots of the erupted tooth. In addition, we discuss how this condition, especially the deposition of acellular cementum on the crown, occurred.

Aim: Surgical extraction of impacted Mandibular third molar is one of the commonest dentoalveolar surgeries. This study aims to investigate the pattern of presentation of impacted Mandibular third molars, the indications for extraction and the post operative complications after this procedure at the Lagos University Teaching Hospital., Patients and Methods: A prospective study of patients who required surgical extraction of impacted Mandibular third molars between October 2003 and May 2006 at the Lagos University Teaching Hospital (LUTH) was carried out. Data collected included Patients' age, sex, indication for extraction, tooth/teeth extracted. Also collected were the types of impactions and surgical morbidity (postoperative complications). The data collected were evaluated using the SPSS for windows (version 11.0: SPSS Inc, Chicago, IL) descriptive analysis was used as appropriate., Results: Three hundred and thirty one (331) Mandibular third molars were extracted from 329 patients. The ages ranged from 17 to 55 years with a mean of 26.63 (+/- 7.39). There were 153 males and 176 females; with male to female ratio was 1:1.15. Recurrent Pericoronitis was the most common indication for extraction (209 extractions; 63.1%), while the mesioangular impaction was the most common angulation (117 impactions; 53.4%). 47 (14.2%) of the extractions had postoperative complications and dry socket which occurred in 25 (53.2%) cases was most common., Conclusion: The pattern of presentation of impacted Mandibular third molars is similar to earlier reports. The morbidity is however higher than the average value in the literature, it however does not seem to increase with increasing age.

In numerous cases of head and facial pain, the underlying causes are found to be ENT problems. The most common cause of pain affecting the forehead and mid-facial region is acute sinusitis, in which the sphenoidal sinus is more commonly affected than previously assumed. Pain in the oral-facial region is largely due to disorders of the masticatory apparatus, but inflammatory diseases of the oral mucosa, abscesses or the so-called Eagle syndrome may also be involved. An accurate history and a clinical examination often points the physician in the right direction. To establish an accurate diagnosis, endoscopy of the upper airways and digestive system, together with ultrasonography, are usually required, followed, where necessary, by a radiological and NMR work-up, and puncture or biopsy. Provided that an accurate diagnosis has been established and multidisciplinary cooperation is available, the therapeutic options are usually effective.

This study was conducted to compare two different techniques used in odontectomy of impacted mandibular third molar; the modified lingual split technique and the conventional buccal technique as regarding the duration of surgery as well as the incidence of postoperative sequelae such as pain, edema, and trismus. This study was carried out on thirty adult male patients having class II, position B mesioangular impacted mandibular third molar scheduled for removal, the cases were divided into two equal groups. Group one using the conventional buccal technique and group two using the modified lingual split technique. Pain was categorized into a three grade scale according to the dose of the analgesic taken by the patients postoperatively. The degree of trismus was calculated by measuring the interincisal distance when the mouth was opened at a maximum using a graduated caliper both preoperatively and postoperatively. Edema was measured linearly using a tape between different predetermined facial anatomical landmarks in different planes. All measurements were carried out immediately before surgery and after one, two and seven days postoperatively. The collected data were tabulated and were statistically analyzed. There was a significant difference between the two techniques as regarding the duration of surgery in favour of the modified lingual split technique, with a mean of 36.3 minutes, in group two against a mean of 54.3 minutes in group I. As regarding pain there was no significant difference in the first and the second postoperative days, while there was a significant difference in favour of the modified lingual split bone technique in the seventh postoperative day. Trismus and edema were comparable in both groups.

In the modern population, few third molars erupt normally and have a good long-term prognosis. This paper reviews the etiology and potential pathology of impacted teeth, and discusses indications and contraindications for extraction.

A questionnaire on third-molar problems was mailed to a systematic random sample of 200 Norwegian general dental practitioners in November 1991. The return rate was 88%. Similar questions were given to 59 5th-year dental students, with a return rate of 83%. Ten third-molar cases were presented by means of copies of radiographs and written information. The following conclusions were drawn: Apart from a terminology problem consisting of lack of discrimination between the terms retention and impaction, overall good diagnostic abilities were shown by both practitioners and students. Receiver operating characteristic analysis of treatment decisions indicated that students were more radical than practitioners. The students had an increased referral rate for third-molar surgery, whereas the practitioners indicated a more selective practice of referring advanced cases.

The purpose of this study was to describe cases of oral cancer in which the initial presenting symptoms were considered to be due to third molars and in which the underlying carcinoma therefore escaped early diagnosis. Between 1986 and 1990 a total of five cases with squamous cell carcinoma associated with a third molar (impacted, partially or totally erupted, or recently extracted) were treated at our unit. Because of acute or chronic symptoms related to disturbed eruption of third molars, the detection of a simultaneous malignant change and referral were delayed. In conclusion, all third molars, especially impacted, partially erupted, or overerupted ones, should be examined critically, especially if the patient has symptoms.

Clinical observations for the past 30 years have stimulated a strong recommendation for removal of impacted third molars at an early age.
