Pigmented villonodular synovitis (PVS) is a rare proliferating process of the synovium, tendon sheaths and bursae usually affecting the bone and joints. Localized nodular synovitis (LNS) is a form of PVS and is characterized by solitary or aggregates of confluent nodules. The incidence of PVS in the United States of America is 1.8 patients per million people while 2.5% of the cases are referred to ankle and foot. The majority of them involve the ankle, metatarsals and toes [4]. The appearance of PVS in the mid-foot is extremely rare. There are only two cases previous reported in the literature to our knowledge. We present a case of localized nodular synovitis of the mid-foot with destructive lesions on the adjacent bones.
