Background: The analysis of mortality offers an important indicator for assessing the state of workers' occupational health. Workers involved in the extraction, refining, alloying and manufacturing of metals are frequently exposed to occupational risks that can lead to their death. The objective of this work was to synthesize the scientific evidence about factors associated with mortality among workers in the metallurgical industry., Methods: A bibliographic review was conducted using the PubMed database. Seventeen studies were included, where topics addressed specific problems that influence the mortality of workers in the metallurgical industry sector. Complete texts of the articles were reviewed., Results: Findings show the highest probabilities of death due to malignant neoplasms (48%), diseases of the circulatory system (28%), work accidents (15%), suicide and violence (9%)., Conclusions: Despite the research carried out, there are gaps and limitations in the study of mortality in workers in the metallurgical industry, mainly related to the relationship of the cause of death with occupational risk factors.

Objectives This report describes the extended follow-up (1941-2015) of a cohort of 38 549 automobile manufacturing workers with potential exposure to metalworking fluids (MWF). The outcomes of interest were mortality from cancers of the esophagus, stomach, intestine, rectum, bladder, liver, pancreas, larynx, lung, skin, prostate, brain, and female breast, as well as leukemia. This report includes 5472 deaths from cancer, more than ten times the numbers of deaths in our last summary report published 20 years ago. Methods Standardized mortality ratios were computed for the entire study period. Adjusted hazard ratios (HR) were estimated in Cox proportional hazard models with categorical variables for cumulative exposure to each type of MWF. Results Exposure-response patterns are consistent with prior mortality reports from this cohort. We found increased risk of skin and female breast cancer with straight fluids. For the first time, we found elevated risk of stomach cancer mortality. Overall, many of the exposure-response results did not suggest an association with MWF. Conclusions Mortality is a poor proxy for cancer diagnosis for treatable cancers and not the optimal outcome measure in etiological studies. Although the HR presented here handle bias from the healthy worker hire effect and left truncation, they do not handle bias from healthy worker survivor effect, which likely results in underestimates of the health impacts of MWF. Although this updated summary provides some information on the risk of cancer from MWF, targeted future analyses will help clarify associations.

Background: Badin, North Carolina, hosted an aluminum smelting plant from 1917 to 2007. The Concerned Citizens of West Badin reported suspected excess cancer mortality among former employees. This study aimed to investigate these concerns., Methods: The study cohort was enumerated from United Steel Workers' records of workers employed from 1980 to 2007. Cause-specific mortality rates in the cohort were compared with North Carolina population mortality rates using standardized mortality ratios (SMRs), standardized by age, sex, race, and calendar period. We estimated cause-specific adjusted standardized mortality ratios (aSMRs) using negative controls to mitigate healthy worker survivor bias (HWSB). Standardized rate ratios (SRRs) were calculated to compare mortality rates between workers ever employed vs never employed in the pot room., Results: All-cause mortality among Badin workers was lower than in the general population (SMR: 0.81, 95% confidence interval [CI]: 0.71-0.92). After adjusting for HWSB, excesses for all cancers (aSMR: 1.55, 95% CI: 1.10-2.21), bladder cancer (3.47, 95% CI: 1.25-9.62), mesothelioma (17.33, 95% CI: 5.40-55.59), and respiratory cancer (1.24, 95% CI: 0.77-1.99) were observed. Black males worked the highest proportion of their employed years in the pot room. Potroom workers experienced higher respiratory cancer (SRR: 2.99, 95% CI: 1.23-7.26), bladder cancer (SRR: 1.58, 95% CI: 0.15-15.28), and mesothelioma (SRR: 3.36, 95% CI: 0.21-53.78) mortality rates than never workers in the pot room., Conclusions: This study responds to concerns of a group of former aluminum workers. Objectives: Synthetic metalworking fluids (MWFs), widely used to cool and lubricate industrial machining and grinding operations, have been linked with increased risk of several cancers. Estimates of their relation with lung cancer, however, are inconsistent. Controlling for the healthy worker survivor effect, we examined the relations between lung cancer mortality and exposure to synthetic MWF, as well as to biocides added to water-based fluids to control microbial growth, in a cohort of autoworkers. Biocides served as a marker for endotoxin, which has reported antitumour effects, and were hypothesised to be the reason prior studies found reduced lung cancer risk associated with exposure to synthetic fluids., Methods: Using the parametric g-formula, we estimated risk ratios (RRs) comparing cumulative lung cancer mortality under no intervention with what would have occurred under hypothetical interventions reducing exposure to zero (ie, a ban) separately for two exposures: synthetic fluids and biocides. We also specified an intervention on synthetic MWF and biocides simultaneously to estimate joint effects., Results: Under a synthetic MWF ban, we observed decreased lung cancer mortality risk at age 86, RR=0.96 (0.91-1.01), but when we also intervened to ban biocides, the RR increased to 1.03 (0.95-1.11). A biocide-only ban increased lung cancer mortality (RR=1.07 (1.00-1.16)), with slightly larger RR in younger ages., Conclusions: Findings suggest a modest positive association for synthetic MWF with lung cancer mortality, contrary to the negative associations reported in earlier studies. Background: Metalworking fluids (MWF) are complex mixtures with dermal and inhalation exposure. Published reports reveal excess cancer risk., Methods: Using published findings exposure response was derived for each attributable cancer site. Aggregate excess lifetime risk was estimated by applying a lifetable calculation., Results: Cancer sites contributing the most attributable cases were larynx, esophagus, brain, female breast, and uterine cervix. With constant workplace MWF exposure of 0.1 mg/m 3 over a 45 years working life, the risk of attributable cancer was 3.7% or, excluding the less certain female cancers, 3.1%., Conclusion: Substantial cancer risks occurred at 0.1 mg/m 3 MWF, one fourth of the current NIOSH recommended exposure limit for MWF total particulate. Because ingredients in current MWF remain from earlier formulations, it is likely that some MWF carcinogenicity persists today. Purpose: Epidemiological studies in cohorts of uranium millers can be informative to improve knowledge of the health effects of uranium, but are very rare. The aim of this study was to analyze, for the first time, mortality in a French cohort of uranium millers., Methods: The F-Millers cohort includes permanent contract workers employed at least 6 months at French uranium milling plants. Vital status and causes of death were obtained from national registries between 1968 and 2013, in order to perform comparisons with French national and local mortality rates by computing standardized mortality ratios (SMRs) with 95% confidence intervals (95% CI)., Results: The cohort includes 1291 workers. The average duration of follow-up is 32.1 years. At the end of follow-up, 448 workers were deceased and 13 lost to follow-up. We observed a significant deficit of mortality for all causes combined when the national reference was considered (SMR 0.81; 95% CI [0.74;0.89]), but no significant difference when the local reference was considered (SMR 0.97; 95% CI [0.88;1.07]). Significant excesses were observed only in a subgroup of 552 workers hired at the manufacturing unit, mainly when the local reference was considered., Conclusion: No significant excess of mortality was observed at the scale of the full cohort. The cause-specific excesses of mortality observed in the subgroup of workers hired at the manufacturing unit were based on small number of cases, but would warrant further investigations. Undertaking analytical studies and combined analyses of cohorts of uranium millers would help to study the influence of potential risk factors and obtain more precise results.

Purpose: Long-term health risks of occupational exposures to uranium processing were examined to better understand potential differences with uranium underground miners and nuclear reactor workers., Methods: A cohort study of mortality of workers from Port Hope, Canada (1950-1999) and Wismut, Germany (1946-2008) employed in uranium milling, refining, and processing was conducted. Poisson regression was used to evaluate the association between cumulative exposures to radon decay products (RDP) and gamma-rays and causes of death potentially related to uranium processing., Results: The pooled cohort included 7431 workers (270,201 person-years of follow-up). Mean RDP exposures were lower than in miners while gamma-ray doses were higher than in reactor workers. Both exposures were highly correlated (weighted rho = 0.81). Radiation risks of lung cancer and cardiovascular diseases (CVD) in males were increased but not statistically significant and compatible with risks estimated for miners and reactor workers, respectively. Higher RDP-associated CVD risks were observed for exposures 5-14 years prior to diagnosis compared to later exposures and among those employed <5 years. Radiation risks of solid cancers excluding lung cancer were increased, but not statistically significant, both for males and females, while all other causes of death were not associated with exposures., Conclusions: In the largest study of uranium processing workers to systematically examine radiation risks of multiple outcomes from RDP exposures and gamma-rays, estimated radiation risks were compatible with risks reported for uranium miners and nuclear reactor workers. Continued follow-up and pooling with other cohorts of uranium processing workers are necessary for future comparisons with other workers of the nuclear fuel cycle.

Background: Prior studies have indicated associations between ingestion of inorganic arsenic and ischemic heart disease, nonmalignant respiratory disease, and lung, skin, bladder, and kidney cancers. In contrast, inhaled arsenic has been consistently associated only with lung cancer. Evidence for health effects of inhaled arsenic derives mainly from occupational studies that are subject to unique biases that may attenuate or obscure such associations., Objectives: We estimated the excess mortality from respiratory cancers, heart disease, and other causes resulting from occupational arsenic exposure while controlling for confounding using the parametric g-formula., Methods: Using a cohort of 8,014 male copper smelter workers who were hired between 1938 and 1955 and followed through 1990, we estimated the impacts of hypothetical workplace interventions on arsenic exposure on the risk of mortality from all causes, heart disease, and lung cancer using the parametric g-formula., Results: We estimate that eliminating arsenic exposure at work would have prevented 22 deaths by age 70 per 1,000 workers [95% confidence interval (CI): 10, 35]. Of those 22 excess deaths, we estimate that 7.2 (95% CI: -1.2, 15) would be due to heart disease, 4.0 (95% CI: -0.8, 8.2) due to respiratory cancers, and 11 (95% CI: 0.0, 23) due to other causes., Conclusions: Our analyses suggest that the excess deaths from causes other than respiratory cancers comprise the majority of the excess deaths caused by inhaled arsenic exposure. Healthy worker survivor bias may have masked such associations in previous analyses. These results emphasize the need for consideration of all exposure routes for upcoming risk assessment by the U.S. Environmental Protection Agency.

Objective: To examine the patterns of cause-specific mortality and relationship between internal exposure to uranium and specific causes in a pooled cohort of 29,303 workers employed at three former uranium enrichment facilities in the United States with follow-up through 2011., Methods: Cause-specific standardized mortality ratios (SMRs) for the full cohort were calculated with the U.S. population as referent. Internal comparison of the dose-response relation between selected outcomes and estimated organ doses was evaluated using regression models., Results: External comparison with the U.S. population showed significantly lower SMRs in most diseases in the pooled cohort. Internal comparison showed positive associations of absorbed organ doses with multiple myeloma, and to a lesser degree with kidney cancer., Conclusion: In general, these gaseous diffusion plant workers had significantly lower SMRs than the U.S., Population: The internal comparison however, showed associations between internal organ doses and diseases associated with uranium exposure in previous studies. Background: Lead exposure has been linked to impaired renal function and kidney failure. High lead exposures have been associated with increased mortality from certain cancers, hypertension, cardiovascular disease, and amyotrophic lateral sclerosis (ALS)., Methods: We extended vital status follow-up on a cohort of 1,990 lead smelter workers by 25 years and computed standardized mortality ratios and rate ratios (RR) stratified by cumulative lead exposure., Results: The update added 13,823 person-years at risk and 721 deaths. Increased risk of mortality was observed for the a priori outcomes of lung cancer, cardiovascular disease (including cerebrovascular disease), chronic kidney disease, and ALS. However, of these outcomes, only cardiovascular, cerebrovascular, and chronic kidney diseases were associated with a positive exposure-response in RR analyses., Conclusions: This study reaffirms the association of lead exposure with cardiovascular and kidney diseases; however, increased mortality observed for certain cancers is not likely to be due to lead exposure. Background: The current study evaluates the mortality of 2,354 workers first employed at a Baltimore chromate production plant between 1950 and 1974., Methods: The National Death Index (NDI Plus) was used to determine vital status and cause of death. Cumulative chromium (VI) exposure and nasal and skin irritation were evaluated as risk factors for lung cancer mortality., Results: There are 91,186 person-years of observation and 217 lung cancer deaths. Cumulative chromium (VI) exposure, nasal irritation, nasal perforation, nasal ulceration, and other forms of irritation (e.g., skin irritation) were associated with lung cancer mortality., Conclusion: Cumulative chromium (VI) exposure was a risk factor for lung cancer death. Background: The Sheet Metal Occupational Health Institute Trust (SMOHIT) established a screening program in 1985 to examine the health hazards of the sheet metal industry in the U.S. and Canada., Methods: 17,345 individuals with over 20 years in the trade and who participated in the program were followed for causes of death between 1986 and 2010. Both SMRs and Cox proportional hazards models investigated predictors of death due to lung cancer, mesothelioma, and chronic obstructive pulmonary disease (COPD)., Results: Significant excess mortality was seen for mesothelioma and asbestosis. Controlling for smoking, a strong trend for increasing lung cancer risk with increasing chest x-ray profusion >0/0 was observed. With an profusion score <1/0, FEV1 /FVC <80% was associated with lung cancer risk. COPD risk increased with increasing profusion score., Conclusions: This study demonstrates asbestos-related diseases among workers with largely indirect exposures and an increased lung cancer risk with low ILO scores., (© 2015 Wiley Periodicals, Inc.)

Objective: To determine the mortality and cancer incidence of aluminum smelter workers., Method: The experience of 17,089 workers between 1950 and 2004 was studied., Results: There were 25 deaths/47 female incident cancers and 6350 deaths/3984 male incident cancers. Mortality was significantly elevated for chronic obstructive pulmonary disease, Alzheimer disease (likely influenced by regional factors), all cancers, stomach, bladder, and lung cancers. Incidence was significantly elevated for bladder, lung, and laryngeal cancers. Digestive, lung, and bladder cancer mortality and lung, bladder, and buccal cancer incidence increased significantly with benzo(a)pyrene (B(a)P) exposure. Over time lung cancer risk has diminished but bladder cancer risk has not., Conclusions: Bladder cancer incidence was not increased at B(a)P exposures below 40 μg/m·yr. Occupational origins for other cancers occurring above expected rates but probably unrelated to B(a)P exposure cannot be excluded. In the prebake plant, nonsignificant increases of lung cancer were observed between 2000 and 2004.

Background: Current recommendations for limits on metalworking fluids may provide insufficient protection from workplace-related illness. Chronic obstructive pulmonary disease (COPD) is a challenging outcome in occupational cohorts because its long period of worsening pulmonary function allows sicker workers to reduce exposure, causing a healthy worker survivor bias. G-estimation is a statistical method that reduces this bias. We introduce a public health approach using g-estimation to compare a series of potential exposure-reducing interventions., Methods: Autoworkers at three General Motors plants in Michigan were followed for COPD mortality from 1 January 1941 to 31 December 1994. For each of the three fluid types (straight, soluble, synthetic), a series of binary variables indicated whether exposure exceeded a series of potential limits. Separate g-estimation analyses for each limit yielded results expressed as the total number of years of life that could have been saved among those who died from COPD had that exposure limit been enforced., Results: Lower limits would have had greater effect than higher limits. A ban on soluble fluids (the most common type) would have had the greatest effect, saving an estimated 1550 years of life. Corresponding estimates were 737 and 260 years for straight and synthetic fluids, respectively. Few workers were exposed to synthetic fluids, limiting analytic power., Conclusions: This application of g-estimation suggests that limiting exposure to metalworking fluids could have saved many years of life lost to COPD in this cohort. The approach permits comparison of different interventions. Separate limits should be considered for different types of fluids.

Comparative evaluation covered carcinogenic jeopardy at metallurgic copper production through studies of the workers' mortality with malignancies and calculation of individual carcinogenic risks. Findings are that the individual carcinogenic risks calucations correspond to the data obtained in epidemiologic study of the mortality with malignancies and could be used for evaluation of carcinogenic jeopardy.

Aim: to investigate general and cause-specific mortality of workers exposed to metals and other chemicals in the electroplating industry in Bologna Province., Materials and Methods: factory records of workers employed in 90 electroplating companies present in 1995 were used to build a cohort of subjects potentially exposed to carcinogenic and other substances in this industry, defined as "revised cohort", which was followed-up for mortality from 1960, or since first employment in an electroplating company if later, to 2008. Mortality risk was also examined separately in a subset of the cohort, composed of workers with at least one year of employment in electroplating, denominated "final cohort". Death rates of residents in Emilia-Romagna Region (Northern Italy) were used as a reference., Results: follow-up completeness was 99%. During the observation period, 533 deaths out of 2,983 subjects were observed in the revised cohort and 317 out of 1,739 in the final cohort. Significantly increased Standardized Mortality Ratios were estimated for overall mortality and for mortality from AIDS in the revised cohort and for bladder and rectal cancer in both cohorts., Conclusions: the present study is, to authors' knowledge, the largest mortality investigation conducted in Italy on electroplating workers, for both size and temporal extension. The presence of excess mortality from causes of death not consistently associated in the literature with exposure to agents in this industry suggests that further research is needed to confirm these associations.

Objective: To study the possible association between iron oxide exposures and lung cancer risk among workers in a French carbon steel-producing factory., Methods: 16 742 males and 959 females ever employed for at least 1 year between 1959 and 1997 were followed up for mortality from January 1968 to December 1998. Causes of death were ascertained from death certificates. Job histories and smoking habits were available for 99.7% and 72.3% of subjects, respectively. Occupational exposures were assessed by a factory-specific job-exposure matrix (JEM) validated with atmospheric measurements. Standardised mortality ratios (SMRs) were computed using local death rates (external references). Poisson regressions were used to estimate the relative risks (RRs) for occupational exposures (internal references), adjusted on potential confounding factors., Results: Among males, observed mortality was lower than expected for lung cancer compared to the local population (233 deaths, SMR 0.89, 95% CI 0.78 to 1.01) and higher than expected compared to the French population (SMR 1.30, 95% CI 1.15 to 1.48) No lung cancer excess was observed for exposure to iron oxides (RR 0.80, 95% CI 0.55 to 1.17) and no dose-response relationship with intensity, duration of exposure or cumulative index was found. A significant bladder cancer excess was observed among workers exposed to oil mist (RR 2.44, 95% CI 1.06 to 5.60), increasing significantly with intensity, duration of exposure and cumulative index., Conclusion: This study did not detect any relationship between exposure to iron oxides and lung cancer mortality. An excess of mortality from bladder cancer was found among workers exposed to oil mist.

In epidemiologic studies, researchers often need to establish a nonlinear exposure-response relation between a continuous risk factor and a health outcome. Furthermore, periodic interviews are often conducted to take repeated measurements from an individual. The authors proposed to use fractional polynomial models to jointly analyze the effects of 2 continuous risk factors on a health outcome. This method was applied to an analysis of the effects of age and cumulative fluoride exposure on forced vital capacity in a longitudinal study of lung function carried out among aluminum workers in Australia (1995-2003). Generalized estimating equations and the quasi-likelihood under the independence model criterion were used. The authors found that the second-degree fractional polynomial models for age and fluoride fitted the data best. The best model for age was robust across different models for fluoride, and the best model for fluoride was also robust. No evidence was found to suggest that the effects of smoking and cumulative fluoride exposure on change in forced vital capacity over time were significant. The trend 1 model, which included the unexposed persons in the analysis of trend in forced vital capacity over tertiles of fluoride exposure, did not fit the data well, and caution should be exercised when this method is used.

Objectives: Previous studies on mortality and the incidence of cancer among workers at primary aluminum smelters have produced conclusive results indicating an elevated risk of bladder cancer. An increased risk of lung cancer has also been reported several times. The objective of this study was to examine mortality and the incidence of cancer at a Swedish aluminum smelter when different neighboring reference populations were used to evaluate any relationships to the length of employment., Methods: A historical cohort--comprised of 2264 male nonoffice workers employed from 1942 on and tracked up to the year 2000--was examined. With the use of three reference populations for mortality and four for cancer incidence, standardized mortality and incidence ratios were calculated, together with hazard ratios derived from Cox regression models., Results: This study showed an excess risk of mortality due to chronic obstructive lung disease, mental disorders, and diseases of the digestive system among the short-term workers. An elevated risk of cancer was found for the lungs, central nervous system, and esophagus. The highest lung cancer risk was observed for the workers employed for > or = 10 years in the factory when they were compared with the reference group from northern Sweden (standardized incidence ratio 1.99, 95% confidence ratio 1.21-3.07)., Conclusions: The results support previous studies that demonstrated an excess risk of lung cancer, but, in contrast to the results of most studies, cancer of the central nervous system was also elevated. This study did not, however, verify an association between this type of exposure and cancer of the urinary organs.

Background: This study examined causes of deaths among unionized plumbers, pipefitters and allied trades., Methods: Deaths of union members from the years 1971, 1979, 1987, and 1995 were selected as a representative sample from a computer file provided by the union. These years provided 15,411 deaths for proportionate mortality ratio (PMR) analysis., Results: PMRs for lung cancer and asbestosis were significantly elevated compared to U.S. white males. PMRs for chronic disease of the endocardium and cardiomyopathy were also elevated. Elevations were not observed in other a priori causes: laryngeal cancer, lymphatic cancer, and neurological disorders. PMRs for transportation accidents for pipe/steam-fitters were elevated in 1971 and 1979, but not in 1987 or 1995., Conclusion: Despite the limitations of a PMR analysis, study results indicate mortality related to asbestos exposure is, and will continue to be, an area of concern for members of the union., (Copyright 2008 Wiley-Liss, Inc.)

Objective: To understand the major causes of death in automobile foundry workers and investigate casting manipulations hazards to health., Methods: A cohort study of 3529 foundry workers registered in one big automobile factory in Shiyan city of China was performed. Standardized mortality ratios (SMRs) were calculated for the main causes of death by using Chinese national mortality rates as reference., Results: The cohort mortality was traced from 1980 to the end of 2005 with an accumulation of 84 999 person-years, revealed 265 deaths. The results of this study showed that the standardized mortality ratio for all subjects was 0.96 (95% CI: 0.85 approximately 1.08), which was very close to that expected on the basis of the China national mortality rates. The SMR increased with age, the SMR became greater than 1 in all groups of age 50 and higher. The results showed that malignant neoplasm (3.43%), accidents (1.16%), cerebro-vascular diseases (1.08%), cardio-vascular diseases (0.79%) were the first four illnesses that threatened workers' life span. Statistically significant mortality of malignant neoplasm (SMR = 7.87), accidents (SMR = 2.70), cardio-vascular diseases (SMR = 2.68) and digestive diseases (SMR = 2.79) were found in the foundry workers. The relative risk of malignant neoplasm for first line workers to assistant workers was 1.99 (P < 0.05)., Conclusion: The occupational hazards in foundry factory have harmful impact on the workers' health and life span.

Bauxite is a reddish clay that is refined to produce alumina, which is then reduced to aluminium. There have been studies examining the health of workers in aluminium smelters, but not workers in bauxite mining and alumina refining. A cohort of employees of 1 large aluminium company since 1983 was assembled (n = 6,485, 5,828 men). Deaths and incident cancers to 2002 were ascertained by linkage to national and state cancer and death registries. SIRs and SMRs were calculated compared to national rates standardizing for calendar year, sex and 5-year age group. The mortality from all causes (SMR 0.68, 95% CI: 0.60-0.77), and from circulatory and respiratory diseases, all cancers combined and injury in the male cohort were lower than in the Australian male population and were similar across work groups and with duration of employment. The only significant increased mortality risk was from pleural mesothelioma. The incidence of all cancers combined was similar to the Australian rate. The cohort had a lower risk of incident lymphohaematopoietic cancer (SIR 0.50, 95% CI: 0.31-0.88) and a higher risk of melanoma (SIR 1.30, 95% CI: 1.00-1.69) although no dose-responses were seen. There was also an increased risk of mesothelioma (SIR 3.49, 95% CI: 1.82-6.71), which was associated with exposures outside the aluminium industry. This study is the first to examine cancer and mortality amongst workers in bauxite mines and alumina refineries and found little evidence for increased cancer incidence or mortality in these workers., ((c) 2008 Wiley-Liss, Inc.)

Objective: To describe the cancer experience of Quebec aluminum smelter workers., Methods: Cancer incidence was compared with that of the Quebec general population and examined in relation to benzo[a]pyrene (B[a]P) and smoking exposure indices., Results: There was a statistically significant increased incidence of stomach (two cohorts), pancreatic (one cohort), laryngeal (one cohort), lung (three cohorts), and bladder (four cohorts) cancers. Unlike lung and bladder cancers, pancreatic and stomach cancer risks do not relate meaningfully to cumulative B[a]P exposure. Laryngeal and buccal cavity cancer standardized incidence ratios (SIRs) seem to increase with increasing B[a]P exposure. SIRs from lung cancer have greatly diminished whereas bladder cancer SIRs remain elevated in all but one cohort., Conclusions: The cancer incidence results are consistent with those from mortality studies, but identify other cancers that deserve scrutiny in future follow-ups.

Objective: To monitor changes over time in mortality of workers first employed in Quebec aluminum smelters after January 1, 1950 (1951-one cohort)., Methods: Mortality of cohorts by decade of hire was compared with Quebec and same plant experience before 1950 (1951)., Results: Chronic obstructive pulmonary disease and respiratory cancer (one cohort each) mortality were statistically in excess. In the combined cohorts, standardized mortality ratios exceeded 110 for cancers of esophagus, rectum, and rectosigmoid junction, pancreas, larynx, lung, non-Hodgkin lymphoma, cerebrovascular disease, and asthma. These findings essentially mirrored the experience of pre-1950 workers. There was a significant downward trend in mortality from all causes, lung and bladder cancer., Conclusions: Statistically significant improvements were found in mortality from all causes combined, from cancer and from cancer of the lung and bladder. Slightly increased mortality continues from certain other causes.

Objectives: To determine whether the analysis of death certificate data would reveal the same relationship among race, occupational exposure, and lung cancer mortality observed by a large cohort study., Methods: An occupation-specific mortality odds ratio (MOR) for lung cancer (ICD-162) versus all other causes was calculated for 218,341 black men and white men who had been employed in the metal industries., Results: Black men were at increased risk for lung cancer mortality when compared with white men among the 4668 oven workers (MOR = 1.38, 95% CI = 1.10 to 1.73), but not among the 33,605 white-collar workers (MOR = 0.95, 95% CI = 0.74 to 1.23)., Conclusions: Our findings corroborate a previously demonstrated association among exposure to carcinogenic coke oven emissions, race, and lung cancer mortality, and support the use of death certificate data to help identify occupations with racial disparities in lung cancer mortality.

Objective: To describe the mortality of Quebec aluminum smelter workers employed before 1951., Methods: The mortality of 5,977 men hired at three plants on or before January 1, 1951 was compared with that of Quebec men. Relationships to benzo[a]pyrene, benzene-soluble material, and smoking were examined., Results: Statistically significant causes of death were lung cancer (three plants); bladder cancer; chronic obstructive lung disease (two plants each); cancers of the stomach, digestive system unspecified, rectum and rectosigmoid, pancreas, and larynx; Alzheimer's disease (one plant); and cerebrovascular disease (one plant). Not significant increases were also observed., Conclusions: Mortality from cancer of the lung and bladder and chronic obstructive pulmonary disease are related to exposure in Söderberg smelters. The cause of increased stomach cancer mortality is unclear. Excess mortality from some other diseases may be explained by factors other than coal tar pitch volatiles exposure.

Background: An earlier study of mortality among male former employees at a tin smelter in Humberside, UK, had identified excess mortality from lung cancer, which appeared to be associated with occupational exposure., Aims: The aim of the present study was to investigate the relationship between lung cancer mortality and quantitative measures of exposure., Methods: Using available records of occupational hygiene measurements, we established exposure matrices for arsenic, cadmium, lead, antimony and polonium-210 ((210)Po), covering the main process areas of the smelter. We established work histories from personnel record cards for the previously defined cohort of 1462 male employees. Three different methods of extrapolation were used to assess exposures prior to 1972, when no measurement results were available. Lung cancer mortality was examined in relation to cumulative inhalation exposure by Poisson regression analysis., Results: No significant associations could be found between lung cancer mortality and simple cumulative exposure to any of the substances studied. When cumulative exposures were weighted according to time since exposure and attained age, significant associations were found between lung cancer mortality and exposures to arsenic, lead and antimony., Conclusions: The excess of lung cancer mortality in the cohort can most plausibly be explained if arsenic is the principal occupational carcinogen (for which the excess relative risk diminishes with time since exposure and attained age) and if there is a contribution to excess mortality from an enhanced prevalence of smoking within the cohort. The implications of the dose-response for arsenic exposure for risk estimation merit further consideration.

Foundry workers have increased mortality and morbidity risks from numerous causes, including various cancers. A retrospective Chinese iron-steel cohort study was conducted to examine the mortality effects of exposure to foundry work. Standardized mortality ratios (SMRs) and standardized rate ratios (SRRs) were calculated to evaluate mortality risks among male workers with exposure to 15 hazardous factors, adjusting for confounders. During 14 years of follow-up, 13,363 of 121,846 male workers died. SMR analysis showed a healthy-worker effect in comparison with the general population. SRR analysis showed increased risks for all causes, all neoplasms, and others among the exposed workers compared with non-exposed blue-collar workers. Combined exposure to polycyclic aromatic hydrocarbons and two or more dusts increased the risks of lung cancer (SRR = 654; 95% CI: 113-3,780) and other malignancies. Foundry work has adverse health effects, including carcinogenic risks.

Objective: To evaluate the risk of lung cancer and nasal cancer among workers employed at the Clydach nickel refinery, South Wales since 1930 by combining data from the two most recently published papers on this cohort., Methods: Observed and expected numbers of cancer deaths were extracted for workers who had a minimum of five years service and were employed for the first time between 1902 and 1992. Standardised mortality ratios (SMR) were calculated for subgroups according to year of employment, time since first employment, and process work., Results: A persisting excess of respiratory cancer was found for workers employed in the period 1930-92, with a lung cancer SMR of 133 (95% CI 103 to 172) and a SMR for nasal cancer of 870 (95% CI 105 to 3141). The lung cancer excess was most clearly seen 20 years or more after first employment and seemed to be confined to process workers. There was no indication of a further reduction in risk since 1930., Conclusion: The extreme nickel related cancer hazard at the refinery before 1920 was greatly reduced during subsequent years. Some of the carcinogenic exposures seem to have remained after 1930, producing an elevated risk of nasal cancer and a 30% excess of lung cancer in the workforce. There was evidence of a persisting risk among process workers first employed since 1953.

Background: Between 1937 and 1991, Capper Pass and Sons Limited operated a tin smelter complex in North Humberside, UK, at which employees were potentially exposed to a number of substances, including lead, arsenic, cadmium and natural series radionuclides. Decommissioning and site clearance continued until 1995. Between 1967 and 1995 the company was a subsidiary of Rio Tinto plc., Aims: The aim was to identify any significant excess, or deficits, in mortality among former employees that might be attributable to factors associated with occupation., Methods: We defined a cohort of 1462 males who had been employed for at least 12 months between 1/11/1967 and 28/7/1995, followed-up through to 31/12/2001. The mortality of the cohort was compared against that expected for both national and regional populations., Results: Mortality from all causes and all cancers did not differ from that expected. Mortality from ischaemic heart disease showed a deficit and mortality from lung cancer showed a statistically significant excess. Mortality from smoking related diseases other than lung cancer showed a non-significant deficit., Conclusions: The pattern of lung cancer mortality is consistent with the hypothesis that the risk of lung cancer has been enhanced by occupational exposure to one or more carcinogens, the effect of which diminishes with time since exposure. The deficit in ischaemic heart disease may be attributed to a protective effect from manual labour. The results provide no evidence for attribution of other excess or deficits in mortality to factors associated with employment.

Background: Excess risks of respiratory cancer have been shown in some groups of nickel exposed workers. It is clear, however, that not all forms of nickel exposure are implicated in these excess risks., Aim: To determine whether occupational exposures received in a modern nickel carbonyl refinery lead to increased risks of cancer, in particular nasal cancer and lung cancer., Methods: The mortality experienced by a cohort of 812 workers employed at a nickel refinery was investigated. Study subjects were all male workforce employees first employed in the period 1953-92 who had at least five years' employment with the company. Observed numbers of cause specific deaths were compared with expectations based on national mortality rates; SMRs were also calculated by period from commencing employment, year of commencing employment, and type of work., Results: Overall, standardised mortality ratios (SMRs) were close to 100 for all causes (Obs 191, SMR 96, 95% CI 83 to 111), all neoplasms (Obs 63, SMR 104, 95% CI 80 to 133), non-malignant diseases of the respiratory system (Obs 18, SMR 97, 95% CI 57 to 153), and diseases of the circulatory system (Obs 85, SMR 94, 95% CI 75 to 116). There were no significantly increased SMRs for any site of cancer. There was a non-significant excess for lung cancer (Obs 28, Exp 20.17, SMR 139, 95% CI 92 to 201), and in subgroup analyses a significantly increased SMR of 231 (Obs 9) was found for those 142 workers with at least five years' employment in the feed handling and nickel extraction departments. In the total cohort there was a single death from nasal cancer (Exp 0.10)., Conclusions: The non-significant excess of lung cancer deaths may well be a chance finding, but in light of previous studies some role for nickel exposures cannot be excluded.

Our study, even if of small size, suggests an association between occupational exposure to inorganic lead and lung cancer risk.

To investigate the relationship between exposure to polycyclic aromatic hydrocarbons (PAHs) and mortality for specifc cancer sites, 1152 men, employed for at least 1 year at a prebake aluminium smelter, were followed-up from 1972 until 31 December 2001. Exposure to PAHs was estimated from a detailed reconstruction of the working history experienced in the plant by each cohort member and from several environmental and personal shift-sampling measurements available, by task and working department, since 1979. Furthermore, information on smoking habits, previous jobs before engagement in the smelter and main clinical findings observed during the follow-up were collected from the personal medical files. This study showed no increased mortality for lung cancer or bladder cancer associated to exposure to PAHs. Mortality for pancreatic cancer, based on 6 observed deaths, was significantly higher than expected in the whole cohort (SMR 2.4; 95%CI 1.1-5.2) and particularly among workers employed in the anodes factory of the plant (SMR 5.0, 95%CI 2.1-12.1), where a relatively consistent exposure to PAHs has been estimated. The nested case-control study planned for pancreatic cancer cases, confirmed that, also after controlling for cigarette smoking, PAH exposure experienced in the anodes factory was associated with a significant increased risk of pancreatic cancer. A pre-existent diabetes mellitus and a potential occupational exposure to pesticides experienced in previous agricultural jobs were found as concurrent significant covariates increasing the risk. In conclusion, the relatively high exposure to PAHs, experienced in the anodes factory and particularly in the green-mill department of this prebake aluminium reduction plant, cannot be ruled out as one of the main factors in the multifactorial aetiology of the pancreatic cancers observed in this study.

Objectives: To investigate associations between exposures in the silicon carbide (SiC) industry and mortality from non-malignant diseases., Methods: Mortality among 2562 men, working in one of three silicon carbide smelters was investigated, giving 52,618 person-years of follow up from 1962 to 1996. Dose-response relations were investigated by internal comparisons using Poisson regression and by stratified standardised mortality ratio (SMR) analyses., Results: Mortality from all causes was significantly raised compared with the Norwegian mortalities among men, SMR=1.12, (95% confidence interval (95% CI) 1.05 to 1.20). An excess mortality from asthma, emphysema, and chronic bronchitis combined was found, SMR=2.21 (95% CI 1.61 to 2.95), increasing from 1.05 in the unexposed category to 2.64 (95% CI 1.44 to 4.43) in the upper category of exposure to total dust. The Poisson regression analysis confirmed the results from the stratified SMR analyses, and suggested that smoking did not act as a confounder. No association was found for circulatory mortality., Conclusions: There was an increased mortality from asthma, emphysema, and chronic bronchitis combined among SiC workers exposed to dust.

Objective: To study the mortality experience of workers at a hydrometallurgical nickel refinery and fertiliser complex in Fort Saskatchewan, Alberta, Canada., Methods: A total of 1649 male employees of Sherritt International who worked for at least 12 continuous months during the years 1954 to 1978 at the Fort Saskatchewan, Alberta hydrometallurgical nickel refinery and fertiliser complex were followed up for an additional 17 years. Mortality was ascertained from the Canadian mortality data base maintained by Statistics Canada and covered the years 1954-95. Statistics were analysed with Monson's computer program., Results: Total mortality, when compared with the Canadian population, was significantly below expectation. Fewer deaths were found for circulatory disease, ischaemic heart disease, respiratory disease, neoplasms, digestive cancer, and accidents, poisonings, and violence. Among the 718 men in the group exposed to nickel, there were no deaths due to nasal cavity or paranasal sinus cancer. Fewer deaths were found for all causes, circulatory disease, ischaemic heart disease, neoplasms and digestive cancer. Lower death rates were observed than expected for respiratory malignancies and cancer of the bronchus and lung., Conclusion: No association was found in this study between exposure to nickel concentrate or metallic nickel in the hydrometallurgical refining process and the subsequent development of respiratory cancer.

Objectives: To investigate mortality from lung cancer in chrome platers, a group exposed to chromic acid., Methods: The mortality of a cohort of 1087 chrome platers (920 men, 167 women) from 54 plants situated in the West Riding of Yorkshire, United Kingdom, was investigated for the period 1972-97. All subjects were employed as chrome platers for >/=3 months and all were alive on 31 May 1972. Mortality data were also available for a cohort of 1163 comparison workers with no known occupational exposure to chrome compounds (989 men, 174 women). Information on duration of chrome work and smoking habits collected for a cross sectional survey carried out in 1969-72 were available for 916 (84.3%) of the chrome platers; smoking habits were available for 1004 (86.3%) comparison workers. Two analytical approaches were used, indirect standardisation and Poisson regression., Results: Based on serial mortality rates for the general population of England and Wales, significantly increased mortality from lung cancer was observed (obs) in male chrome platers (obs 60, expected (exp) 32.5, standardised mortality ratio (SMR) 185, p<0. 001) but not in male comparison workers (obs 47, exp 36.9, SMR 127). Positive trends were not shown for duration of employment exposed to chrome, although data on working after 1972 were not available., Conclusions: Confident interpretation is not possible but occupational exposures to hexavalent chromium may well have been involved in the increased mortality from lung cancer found in this cohort of chrome platers.

Several studies have linked inhalation of airborne arsenic with increased risk of respiratory cancer, but few have analyzed the shape of the exposure-response curve. In addition, since inhaled airborne arsenic affects systemic levels of inhaled arsenic, there is concern that inhaled arsenic may be associated with cancers of the skin, bladder, kidney, and liver, which have been linked to ingested arsenic. The authors followed 8,014 white male workers who were employed for 12 months or more prior to 1957 at a Montana copper smelter from January 1, 1938 through December 31, 1989. A total of 4,930 (62%) were deceased, including 446 from respiratory cancer. Significantly increased standardized mortality ratios (SMRs) were found for all causes (SMR = 1.14), all cancers (SMR = 1.13), respiratory cancer (SMR = 1.55), diseases of the nervous system and sense organs (SMR = 1.31), nonmalignant respiratory diseases (SMR = 1.56), emphysema (SMR = 1.73), ill-defined conditions (SMR = 2.26), and external causes (SMR = 1.35). Internal analyses revealed a significant, linear increase in the excess relative risk of respiratory cancer with increasing exposure to inhaled airborne arsenic. The estimate of the excess relative risk per mg/m3-year was 0.21/(mg/m3-year) (95% confidence interval: 0.10, 0.46). No other cause of death was related to inhaled arsenic exposure.

A survey of workplace exposures to 60-Hz magnetic fields was carried out at a large uranium enrichment facility to assign exposures for an updated mortality study. Stratified random selection was used to choose workers for measurement in all jobs and areas, to determine whether consistent distinctions could be made between job groups based on average magnetic field exposures. A total of 252 workdays was measured with a personal monitor, and individual average magnetic field exposures ranged from 0.20 to 82.6 mG. A priori job groups showed significant differences between geometric mean exposures, which ranged from 0.80 to 3.51 mG. Most of these groups showed widely ranging exposures, so they were subdivided based on location and job title to improve the precision of the exposure assignments for the mortality study. These final assignments were made up of 26 groups having arithmetic means ranging from 0.43 to 24.9 mG, with most groups defined by location in addition to job title. In general, electrical maintenance workers did not have elevated magnetic field exposures (> 3 mG), but the exposures of the electricians in switchyard (substation) jobs were elevated. Available employment records did not allow most electricians to be distinguished based on location, so they were assigned exposures based on their plantwide average (above 7 mG). An estimated 9% of the work time of this cohort was spent at daily average exposures above 3 mG, despite the very large electric power consumption at this plant.

Most analytic studies of human epidemiologic data have affirmed the linear association between excess lung cancer risk and airborne arsenic exposure. Recent Canadian analyses, however, based on the mortality follow-up of Tacoma smelter workers from 1940-1976, provided strong evidence of a nonlinear dose-response when lung cancer risk was expressed in terms of the standardized mortality ratio. Using recently updated data covering ten additional years of mortality experience among Tacoma workers (1940-1986), new analyses were undertaken to further explore nonlinearity in the lung cancer dose-response in this worker cohort. Lung cancer risk was expressed in terms of both the standardized mortality ratio (SMR) and the excess mortality rate (EMR). As in Canadian analyses, nonlinearity was assessed through a three parameter model containing both linear and negative exponential terms for dose. Dropping the negative-exponential dose-term lead to the standard suite of linear dose-response models, with and without intercept, used for comparative purposes. Analyses were undertaken by subcohort as there was strong evidence of confounding by year of initial hire, which largely explained the nonlinearity in the dose-response observed in Canadian analyses. Subcohort analyses based on initial employment, prior to 1940 or thereafter, showed that the nonlinearity in the dose-response was strongly influenced by date of initial hire. whether the cohort risk was measured by either the SMR or EMR, a nonlinear dose-response was evident only among workers hired prior to 1940. This, however, was strongly related to the artifactually low lung cancer mortality seen among workers hired between 1930 and 1939. Among workers hired after 1940, analyses showed that a linear dose-response provided a clearly superior fit. While analyses showed comparable goodness of fit when models were fitted to the SMR and EMR. Only those based on the EMR provided strong evidence of a dose-response. Overall, nonlinearity as observed in Canadian analyses was likely the result of several sources of bias not taken into account by Canadian investigators., (Copyright 1999 Academic Press.)

Background: We present the mortality experience for a cohort of women (n = 2,877) from a large epidemiologic study of production and fabrication high nickel alloys workers (n = 31,165). All the plants were located within the United States and cohort eligibility required some work experience within the period of the late 1940s through the mid 1960s., Methods: Vital status follow-up was through the end of 1988 and incorporated information from multiple sources. Cause-specific mortality was evaluated by comparing cohort mortality to the general United States female population and to local populations in geographic proximity to the plants. Relative risk estimates were determined for 62 cause of death categories using the Standardized Mortality Ratio (SMR) and were adjusted for age, race, gender, and calendar time by the indirect method using a modified life table technique., Results: Relative risks for all causes (0.98), all cancers (0.90), lung cancer (1.34), and breast cancer (0.96) were nonsignificant when mortality was compared to the US female population. No relationship between mortality and length of time employed in the industry or work area was identified., Discussion: Although there were some difficulties in tracing women due to name changes, comprehensive follow-up was obtained when using multiple sources of information. Our strategy resulted in resolving vital status for over 95% of the women, which is comparable to that of the male cohort. The type of jobs and work activities differed between genders. Females were employed predominantly in two work areas (allocated services, 87%, and grinding, 46%), whereas males were employed in several work areas (allocated services, 76%, grinding, 27%, hot working, 20%, and cold working, 17%). Considerable variation was noted among the study plants with respect to the percent of female production workers in the workforce. Generally, the patterns of relative risks derived for the total cohort and various subgroups are similar across the different comparison populations. Estimated elevated risks are usually lower when cohort mortality is compared to that of local populations. No increased risks were identified for any site-specific cancers or nonmalignant causes of death.

Background: In this study, we describe the methodology used for historical estimation of exposure to fluoride and to PAH in vertical stud Søderberg (VSS) potrooms at two Norwegian aluminum smelters. The assessment was performed in order to develop exposure data for epidemiological studies of cause-specific mortality and cancer incidence., Methods: The estimation was performed in several steps. In the first step, we estimated the area concentrations of fluoride and PAH in periods with no measurements. Relationships between measured area concentrations and process parameters were investigated by statistical modeling. Process parameters and the models were then used to estimate area concentrations in periods lacking area measurement data. In the second step, the relationships between the area measurements and job specific exposure (personal measurements) were investigated by use of a measurement model. In the last step, the obtained relationships were used to estimate job specific exposure in different periods., Results: The range for the annual exposure estimates in the VSS-potrooms was 0.05-1.7 mg/m3 for fluoride and 3-3,437 micrograms/m3 for PAH., Conclusions: Despite limitations of available measurements in the early production period, we have concluded that the exposure estimates from this study provide a reasonable tool for the estimation of dose-response relations in subsequent epidemiological analyses.

Objectives: To investigate the mortality of workers who had been exposed to asbestos, machining fluids and foundry work in a foundry and heavy engineering plant in the railway rolling stock manufacturing industry in New Zealand., Methods: Historical cohort study design., Results: For the total workforce of 3522 men employed between 1945 and 1991, follow up was 90% of person-years to 31 December 1991. Significantly increased standardised mortality ratios (SMRs) were found for all causes of death combined (SMR 1.07; 95% confidence interval (95% CI) 1.01 to 1.14), all malignancies (SMR 1.15; 95% CI 1.01 to 1.31), circulatory (SMR 1.16; 95% CI 1.07 to 1.27) and musculoskeletal diseases (SMR 3.06; 95% CI 1.39 to 5.84), all digestive cancers (SMR 1.29; 95% CI 1.04 to 1.59), all respiratory cancers (SMR 1.34; 95% CI 1.08 to 1.65), cancer of the oesophagus (SMR 1.97; 95% CI 1.01 to 3.45), and mesothelioma of the pleura (SMR 6.58; 95% CI 1.24 to 19.49). Three deaths from pleural mesothelioma were recorded, with latency times of 51, 53, and 57 years. There were no dose-response relations between exposure to asbestos, machining fluids or foundry work, or by duration of employment in the plant, and any cause of death., Conclusions: This study found small increases in risk for several causes of death among foundry and heavy engineering workers; however, these increases were small and the possible effects of smoking and other lifestyle factors could not be excluded. There was evidence of asbestos related disease in those involved in engineering work in the past.

Background: Some studies have reported an increased incidence of lung, kidney and colon cancer among steel workers, however this has not been an universal observation., Aim: To study the causes of death in a group of Chilean steel workers., Subjects and Methods: Between January 1985 and December 1994, 4718 steel workers were followed. Their crude and standardized death rates, adjusted for age and diagnosis, were calculated., Results: During the study period 153 deaths were registered (3.2%), 78 in active workers (2.7%), 68 in retired workers (5.9%) and 7 in hired workers (1%). Main causes of death were liver cirrhosis (19%), gastric cancer (9.2%) and myocardial infarction (8.5%). Cirrhosis was the only cause with a significantly higher standardized adjusted rate (= 171)., Conclusions: We did not find elevated standardized mortality rates for respiratory diseases, lung or kidney cancer in this group of steel workers. Thus, our results did not show a specific occupational risk among these workers.

Objectives: To investigate mortality from lung cancer in nickel/chromium platers., Methods: The mortality experience of a cohort of 1762 chrome workers (812 men, 950 women) from a large electroplating and light engineering plant in the Midlands, United Kingdom, was investigated for the period 1946-95. All subjects were first employed in chrome work at the plant during the period 1946-75, and had at least six months employment in jobs associated with exposure to chromic acid mist (hexavalent chromium). Detailed job histories were abstracted from original company personnel records and individual cumulative durations of employment in three types of chrome work were derived as time dependent variables (chrome bath work, other chrome work, any chrome work). Two analytical approaches were used--indirect standardisation and Poisson regression., Results: Based on mortalities for the general population of England and Wales, male workers with some period of chrome bath work had higher lung cancer mortalities (observed deaths 40, expected deaths 25.41, standardised mortality ratio (SMR) 157, 95% confidence interval (95% CI) 113 to 214, p < 0.01) than did other male chrome workers (observed 9, expected 13.70, SMR 66, 95% CI 30 to 125). Similar findings were shown for female workers (chrome bath workers: observed 15, expected 8.57, SMR 175, 95% CI 98 to 289, p = 0.06; other chrome workers: observed 1, expected 4.37, SMR 23, 95% CI 1 to 127). Poisson regression was used to investigate risks of lung cancer relative to four categories of cumulative duration of chrome bath work and four categories of cumulative duration of other chrome work (none, < 1 y, 1-4 y, > or = 5 y). After adjusting for sex, age, calendar period, year of starting chrome work, period from first chrome work, and employment status (still employed v left employment), there was a significant positive trend (p < 0.05) between duration of chrome bath work and risks of mortality for lung cancer. Relative to a risk of unity for those chrome workers without any period of chrome bath work, risks were 2.83 (95% CI 1.47 to 5.45), 1.61 (95% CI 0.75 to 3.44), and 4.25 (95% CI 1.83 to 9.87) for the second, third, and fourth exposure categories, respectively. Duration of other chrome work was not a useful predictor of risks of lung cancer. Similar findings for both variables were obtained when adjustment was made for sex and age only. Similar findings for both variables were obtained relative to risk of chrome nasal ulceration., Conclusions: The findings are consistent with the hypothesis that soluble hexavalent chromium compounds are potent human lung carcinogens.

Objective: To study cause specific mortality and cancer morbidity in workers exposed to the dust of grinding materials, grinding agents, and stainless steel, especially with regard to a possibly increased risk of respiratory, stomach, and colorectal cancer., Methods: Retrospective cohort study, using reference cohorts of blue collar workers and population rates for comparison. The exposed cohort comprises workers with at least 12 months employment time at two plants, producing stainless steel sinks and saucepans (n = 727). Also, reference cohorts of other industrial workers (n = 3965) and fishermen (n = 8092) were analysed. The observation period began 15 years after the start of employment. Standardised mortality or incidence ratios (SMRs, SIRs; county reference rates) were calculated for cause-specific mortality between 1952 and 1993, and for cancer morbidity between 1958 and 1992., Results: In the exposed cohort, overall mortality, cardiovascular mortality, and all malignant mortality and morbidity were slightly lower than expected. Also, the risk estimates for cancer in the upper and lower respiratory tracts and for stomach cancer were lower than expected. There was an increase in morbidity from colon cancer, which was explained by an excess of tumours in the sigmoid part only. Here, the risk estimates were higher in workers with long employment time (1-14 years: four observed cases, SIR 1.7, 95% confidence interval (95% CI) 0.4 to 4.5; > or = 15 years: three observed cases, SIR 4.3, 95% CI 0.9 to 13) and the increased risk was especially pronounced among those first employed before 1942. A slight nominal excess of rectal cancers (nine observed cases, SIR 1.4, 95% CI 0.6 to 2.6), and a significant excess of prostate cancer morbidity (36 observed cases, SIR 1.7, 95% CI 1.2 to 2.4) were found. These risk estimates did not, however, increase with employment time., Conclusions: The finding of an increased risk of cancer in the sigmoid part of the colon, which was not found in the reference cohorts, and with indication of a relation between duration of employment and response, is consistent with a causal relation. The limited size of the exposed cohort makes a detailed exposure-response analysis unstable, and the confidence limits are wide. Albeit slightly raised, the risk estimate for rectal cancer in the exposed cohort was not different from the estimate among the other industrial workers.
