Background: chest trauma may modify thoracic stiffness and chest wall elastance, which makes airway pressures unreliable to reflect lung status. This study aimed to describe respiratory mechanics of chest wall and its influence on respiratory system elastance in severe chest trauma patients under mechanical ventilation within first days following trauma. Methods: a monocentric prospective physiological study was carried out in the Montpellier Lapeyronie University Hospital from January 2019 to May 2021. Patients hospitalized with a severe chest trauma (AIS ≥3) and requiring a prolonged mechanical ventilation were included. An esophageal pressure monitoring was placed to distinguish chest wall and lung mechanical properties under mechanical ventilation and their evolution within 5 days. The chest wall /respiratory system elastance ratio (ECW/ERS) was specifically studied. Results: forty-two patients with mechanical ventilation requirement were enrolled. Seventy-nine percent were male and the median age was 52 years [32− 60 years]. Fifty-five percent had chest flails and 36% had pulmonary contusions involving 3 lobes or more. Mean respiratory system driving pressure (ΔPRS) on admission was 10±2 cmH2O with a respiratory system elastance (ERS) of 22 (SD 5) cmH2O/L. Chest wall driving pressure (ΔPCW) on admission was 4 cmH2O (IQR 4−5 cmH2O), chest wall elastance (ECW) 10 cmH2O/L (IQR 8−13 cmH2O/L) and ECW/ERS ratio 0.48 (IQR 0.34−0.60). In addition of the aftereffects associated with a critical illness and the aggressive intensive care management, diaphragm is affected by sepsis as well as other muscles such as psoas. Its mechanism and duration remain unclear but atrophic sequelae is a major factor of difficulties to ventilator withdrawal in ICU and long term physical rehabilitation. We tried to observe the importance of the diaphragm and psoas atrophy after ICU discharge comparing to anterior data.Methods: we retrospectively included 41 patients admitted in our ICU for a sepsis or a septic shock, and for whom CT-scans have been performed in the course of an oncological follow-up. We used a segmentation software to compare volumetric measurements of the diaphragmatic and the psoas before, and after the ICU stay. The primary endpoint was to the observable atrophy in this period. Results: between 2 CTs, 102±62 days apart, we found a significant loss of volume of 12,1% (that is 26±25 cm³, p59 years (p=0.01), whereas the atrophy of diaphragm was associated with height >166 cm (p=0.023) and absence of metastasis (p=0.031).Conclusion: the diaphragm and psoas are concurrently but independently affected by a loss of muscular mass, in case of severe infection admitted in ICU. We were able to be bring to light differential associated factors, giving reasons to believe that the pathophysiology behind the sepsis-linked atrophy of the diaphragm and psoas is not equivalent and comparable in all points.; En plus des effets néfastes de toute pathologie aiguë grave, et ceux secondaires à la prise en charge agressive de réanimation, le sepsis affecte tout spécifiquement le diaphragme ainsi que les autres muscles tels que le psoas.Mécanismes et évolution de cette agression demeurent obscurs, mais l'atrophie séquellaire est un facteur majeur de retard au sevrage de la ventilation mécanique puis à la rééducation et reprise d'une vie normale. Nous avons voulu observer la survenue de cette atrophie au décours d'un sepsis, en comparaison à des données antérieures au séjour en réanimation.Méthode : chez 41 patients admis réanimation (CHU Lapeyronie, 2015-2019) pour sepsis ou choc septique, nous avons calculé par méthode de segmentation informatique, le volume des psoas et du diaphragme sur des TDM antérieures et postérieures à l'hospitalisation, réalisées dans le cadre du suivi d'un cancer. Le critère principal était l'atrophie observable dans cet intervalle.Résultats : entre 2 TDM espacées de 102 ± 62 jours, nous avons confirmé une atrophie musculaire moyenne après séjour en réanimation pour sepsis, de 12.1 % (soit 26 ± 25 cm³, p < 0.01) pour le psoas et 10,7 % (soit 21 ± 25 cm³, p < 0.01) pour le diaphragme, sans corrélation statistique entre ces deux variations, mais la variation de diaphragme dépendant significativement du volume musculaire de base (p < 0.01). L'atrophie du psoas était associée a un antécédent artériopathique (p < 0.01) et l'âge > 59 ans (p < 0.01) ; alors que celle du diaphragme était associée à la taille > 166 cm (p = 0.023) et l'absence de métastases (p = 0.031).Conclusion : le diaphragme et le psoas semblent souffrir de manière concomitante mais indépendante d'une infection sévère en réanimation. Les facteurs qui s'y associent ne sont pas les mêmes, peut être du fait d'une réponse physiologique différente entre diaphragme et muscles périphériques.

Background: readiness for liberation from ventilatory support is evaluated with a spontaneous breathing trial (SBT) which assess the ability of a patient to sustain respiratory effort after extubation. Practices are heterogenous concerning the realization of the SBT, and there are few physiological studies on the topic. The objective of this study was to assess which SBT would best reproduce inspiratory effort to breathe after extubation.Material and method: this was a multicentric randomized cross-over physiological study, in a large population, in the era of modern intensive care unit (ICU) using the last generation of modern ventilators. Each included patient performed 3 SBTs of 15 min in random order: pressure support ventilation (PSV) level of 7 cmH2O with positive end expiratory pressure (PEEP) level of 0 cmH20 (PSV 7 PEEP 0), PSV 0 cmH2O with PEEP 0 cmH2O (PSV 0 PEEP 0), and T-piece trial (T-Piece). A rest period of baseline state ventilation was observed between the SBTs (10 min) and before extubation (30 min). Primary outcome was inspiratory muscle effort, reflected by the pressure time product per minute (PTPmin), calculated from esophageal pressure measurements at baseline state, before and after each SBTs, and 20 min after extubation.Results (median and interquartile range 25% -75%): planned interim analysis was performed after inclusion of 72 patients. Sixty-six were in final analysis, with five selected clinical phenotypes: 23 abdominal surgery, 10 brain injury, 7 chest trauma, 7 COPD and 19 miscellaneous (pneumonia, sepsis, heart disease). For all patients, the inspiratory effort after extubation calculated with PTPmin was 248 (189 - 329) cmH2O.s/min. The closest SBT was the T-Piece trial with 256 (184 - 344) cmH2O.s/min, versus 291 ± (219 - 359) cmH2O.s/min (p < 0.01) for PSV 0 PEEP 0 and 176 (116 - 264) cmH2O.s/min (p < 0.001) for PSV 7 PEEP 0. Nevertheless, these results differed according to clinical phenotypes. For brain injured patients and chest traumatized patients, PSV 0 PEEP 0 trial seemed to produce the effort closest to after extubation with respectively 208 (166 - 362) and 297 (265 - 361) cmH2O.s/min for the trial and 249 (195 - 354) and 281 (231 - 398) after extubation. PSV 7 PEEP 0 appeared to be the best trial for the COPD patients with 244 (218 - 279) cmH2O.s/min versus 267 (234 - 315) cmH2O.s/min after extubation.Conclusion: the main preliminary results of the present physiological randomized study suggest that the unassisted T-piece trial could be used for evaluating the after extubation effort to breathe in overall non selected patients. However, if we applied a more specific personalization of the SBT according to the clinical phenotype, it could be considered to use the unassisted PSV 0 PEEP 0 trial for brain injured patients and chest traumatized patients, and the assisted PSV 7 PEEP 0 trial for COPD patients.; Justificatif de l'étude : il est recommandé de réaliser une épreuve de ventilation spontanée (EVS) lors du sevrage de la ventilation mécanique invasive du patient de réanimation pour évaluer ses capacités à tenir un effort respiratoire sans assistance. Il existe aujourd'hui des pratiques hétérogènes concernant la réalisation des EVS, et peu d'études physiologiques sur le sujet. L'objectif de ce travail était de déterminer l'EVS se rapprochant le plus des conditions d'effort respiratoire après extubation chez 5 phénotypes cliniques de patient de réanimation.Matériel et Méthode : il s'agit d'une étude physiologique multicentrique randomisée en cross-over, avec un large effectif, utilisant des respirateurs modernes de dernière génération. Chaque patient inclus a réalisé 3 EVS de 15 min dont l'ordre était aléatoire : aide inspiratoire (AI) 7 cmH2O et pression expiratoire positive (PEP) 0 cmH20 (AI 7 PEP 0), AI 0 cmH2O et PEP 0 cmH2O (AI 0 PEP 0), épreuve avec pièce en T (T-Tube). Une période de repos a été observée entre les épreuves (10 min) et avant extubation (30 min). Le critère de jugement principal était l'effort inspiratoire, reflété par le pressure time product par minute (PTPmin) qui était calculé à partir de mesures de pression œsophagienne, avant de débuter les épreuves, avant et après chaque EVS, et 20 min après extubation. Résultats (médiane et interquartiles 25 % - 75 %) : une analyse intermédiaire a été réalisée après inclusion de 72 patients. Soixante-six patients sont analysables, répartis en cinq phénotypes cliniques : 23 en post opératoire de chirurgie abdominale, 10 neurolésés, 7 traumatisés thoraciques, 7 BPCO et 19 divers (pneumonie, sepsis, cardiopathie). Pour l'ensemble des patients, l'effort inspiratoire après extubation mesuré par le PTPmin était de 248 (189 – 329) cmH2O.s/min. L'EVS la plus proche était le T-Tube avec 256 (184 – 344) cmH2O.s/min, versus 291 ± (219 – 359) cmH2O.s/min (p < 0,01) pour AI 0 PEP 0 et 176 (116 – 264) cmH2O.s/min (p < 0,001) pour AI 7 PEP 0. Néanmoins ces résultats diffèrent selon le phénotype clinique. Pour le patient neurolésé et le traumatisé thoracique, l'épreuve AI 0 PEP 0 semblait entrainer l'effort le plus proche de celui après extubation avec respectivement 208 (166 – 362) et 297 (265 – 361) cmH2O.s/min pour l'épreuve et 249 (195 – 354) et 281 (231 – 398) après extubation. L'AI 7 PEP 0 semblait être la meilleure épreuve pour le patient BPCO avec 244 (218 – 279) cmH2O.s/min pour 267 (234 – 315) cmH2O.s/min après extubation.Conclusion et discussion : l'épreuve sans assistance de T-Tube pourrait être utilisée préférentiellement pour évaluer l'effort respiratoire après extubation de l'ensemble des patients non sélectionnés. Cependant, si nous appliquons une personnalisation plus spécifique de l'EVS en fonction du phénotype clinique, il pourrait être envisagé d'utiliser l'épreuve sans assistance PSV 0 PEEP 0 pour les patients victimes de lésions cérébrales et de traumatismes thoraciques, et l'épreuve avec assistance PSV 7 PEEP 0 pour les patients atteints de BPCO.

Background: the COVID-19 crisis has shown that we need to optimize sedation and analgesia management for patients treated with Neuro-Muscular Blocking Agents (NMBA) to ensure optimal patient’s comfort without wasting sedative and analgesic drugs in Intensive Care Units (ICU) in a context of drugs shortage. Also, optimizing sedation/analgesia management is mandatory to prevent drugs overuse, delayed awakening and prolonged utilization of ICU resources. There is no recommendation regarding the assessment of pain in ICU patients receiving NMBA. This study evaluated the validity of different physiological parameters to better assess pain in pharmacologically paralyzed ICU patients.Methods: four electrophysiological parameters (instant-Analgesia Nociception Index (ANI), Bispectralindex (BIS), Heart Rate (HR) and Mean Arterial Blood Pressure (MAP)) and one clinical physiological parameter (Behavioural Pain Scale (BPS)) were recorded before, during and after tracheal suctioning in all consecutive patients admitted in a medical-surgical ICU who required a sustained use of cisatracurium, as well as just after the complete paralysis recovery measured by the Train-of-Four ratio using an automatized accelometer. The validity of the five pain-related parameters was assessed comparing the values recorded during different situations (at rest before, during, and after thenociceptive procedure) (discriminant validation method), and the effect of paralysis was assessed comparing values obtained during and after paralysis (parameters’ reliability).Results: twenty patients were included for analysis. ANI, BIS and HR significantly changed during the nociceptive procedure in both paralyzed and non paralyzed patients, while BPS changed only in non paralyzed patients. ANI and HR were the only two parameters that were not significantly modified by paralysis, contrary to BIS and BPS (mixed effect model for repeated measurements). ANI showed the highest discriminant validity with values (min 0, max 100) decreasing from 71 [48-89] at rest to 41 [25-72] during the procedure in paralyzed patients and from 71 [53-85] at rest to 40 [31-52] in non paralyzed patients.Conclusions: ANI is the most accurate parameter to detect pain in both paralyzed and non paralyzed sedated ICU patients. Its decrease during tracheal suctioning is significant, clinically relevant, and not modified by the use of NMBA in ICU patients.; La crise du COVID-19 a montré que la gestion de la sédation/analgésie doit être optimisée chez les patients curarisés afin d'assurer un confort optimal des patients, tout en évitant de gaspiller les médicaments sédatifs et analgésiques en réanimation, dans un contexte de pénurie. En outre, l'optimisation de la sédation/analgésie est nécessaire pour prévenir la surconsommation de médicaments, le retard de réveil consécutif et l'utilisation prolongée des ressources de la réanimation. Il n'existe pas de recommandation concernant l'évaluation de la douleur chez les patients curarisés en réanimation. Cette étude a évalué la validité de différents paramètres physiologiques afin de mieux évaluer la douleur chez les patients curarisés en réanimation.Méthodes : quatre paramètres électrophysiologiques (l’Analgesia Nociception Index instantané (ANI), l'indice bispectral (BIS), la fréquence cardiaque (FC) et la pression artérielle moyenne (PAM)) et un paramètre physiologique clinique (Behavioural Pain Scale (BPS)) ont été enregistrés avant, pendant et après l'aspiration trachéale chez tous les patients admis dans une unité de réanimation médico-chirurgicale, nécessitant une perfusion continue de cisatracurium, ainsi que juste après la décurarisation complète, évaluée par le rapport du Train de Quatre à l'aide d'un accéléromètre automatisé. La validité des cinq paramètres liés à la douleur a été évaluée en comparant les valeurs enregistrées dans différentes situations (au repos avant, pendant, et après la procédure nociceptive) (méthode de validation discriminante), et l'effet de la curarisation a été évalué en comparant les valeurs obtenues avant et après décurarisation (reproductibilité).Résultats : vingt patients ont été inclus pour l'analyse. Les valeurs d’ANI, de BIS et de FC se sont modifiées de manière significative au cours de la procédure nociceptive, que les patients soient curarisés ou décurarisés, le BPS ne se modifiant que chez les patients décurarisés. L’ANI et la FC sont les deux seuls paramètres qui n'ont pas été modifiés de manière significative par la décurarisation, contrairement au BIS et au BPS (modèle statistique à effet mixte pour mesures répétées). L'ANI a montré la validité discriminante la plus élevée avec des valeurs (min 0, max 100) diminuant de 71 [48-89] au repos à 41 [25-72] pendant la procédure chez les patients curarisés, et de 71 [53-85] au repos à 40 [31-52] chez les patients décurarisés.Conclusion : l'ANI est le paramètre le plus approprié pour détecter la douleur chez les patients sous sédation en réanimation qu’ils soient curarisés ou non. Sa diminution lors de l'aspiration trachéale est significative, cliniquement pertinente, et non modifiée par l'utilisation de curares en réanimation.

Introduction: acute basilar artery occlusion (ABAO) is a devastating type of stroke, with a high mortality rate and functional dependence and a frequent need for intensive care. We studied the outcome of patients suffering from ABAO hospitalized in a neuro-intensive care unit (ICU) after an endovascular procedure, and we searched for prognostic factors.Method: we conducted, from 2011 to 2017, a retrospective, monocentric study in a French neuro-ICU. We determined the outcome at 90 days using the modified Rankin Scale (mRS) score and compared patients with a good outcome (mRS 0-2) to patients with a bad outcome (mRS 3-6).Results: seventy-one patients were included. The median age was 66 years old. The median NIHSS score was 14. The median PC-ASPECTS was 8. The median delay between the last neurological worsening and recanalization was 466 (366-603) minutes.
