We summarize current data and understanding about the prevalence of gun violence at schools and risk of rampage school shootings. Following that comes a discussion of gun availability among youth and strategies and practices in the United States and abroad that have been implemented to limit access to firearms for minors. Next, we review the current evidence for arming school teachers, a strategy that could also increase the number of firearms in schools. We then focus on three specific strategies that schools have implemented to reduce school shootings and improve school safety: (a) addressing the role of mental health in school shootings and violence perpetration; (b) implementing multidisciplinary threat assessment protocols; and (c) target hardening and utilizing school resource officers. Based on available evidence, a public health approach to addressing firearm violence in schools is the most effective way to prevent school shootings, even if these effects are indirect through improvements in student social competence and positive school climate. Finally, we offer some recommendations for future practice, policy and research.

Objective: To investigate the relations among certain mental health problems (MHPs; affective, anxiety, attention-deficit/hyperactivity disorder [ADHD], and substance use disorders), criminogenic risk, and outcomes in a sample of serious adolescent offenders. Method: Using data from a longitudinal study of serious adolescent offenders (N = 949; mean age = 16 years, SD = 1.10 years; 84% male; 78% minority), we evaluated the association of MHPs with three distinct outcomes (rearrest, self-reported antisocial activity, and gainful activity), tested whether having an MHP contributed any unique explanatory power regarding these outcomes over and above criminogenic risk markers, and examined whether MHPs moderated the relationship between risk markers and outcomes. Negative binomial and ordinal regressions were used. Data for the study were derived primarily from youth self-report over a 7-year period, with parent collaterals reporting on ADHD, and official records as the source for rearrest information. Results: Of the sample, 57.5% met the criteria for at least one of the assessed MPHs. The presence of a substance use disorder showed consistent associations with the outcomes. After controlling for risk markers and demographic characteristics, MHPs were not associated with most outcomes. The co-occurrence of a substance use disorder and an MHP moderated the relations between several risk markers and outcomes. Conclusions: Current juvenile justice policies that focus treatment efforts on both criminogenic and mental health factors (with particular emphasis on treating substance use disorders) appear to be well founded. It is unlikely that focusing solely on treating MHPs in serious offenders will have a distinct impact on later outcomes. (Contains 4 tables.)

This study examines employment and educational outcomes for justice-involved adolescents with and without mental health disorders in the Pathways to Desistance study. We examine the patterns of education and employment and the effects of several factors, including the presence of a mental health disorder, on these positive outcomes. Three findings emerge: (a) youth with mental health problems have a relatively high number of other criminogenic risk factors, (b) education and employment patterns do not differ by disorder status, and (c) the presence of a mental health disorder is not independently linked to poorer employment outcomes, but educational achievement is. These analyses provide information needed to target services for these adolescents in the juvenile justice system, who are at high risk for poor educational and employment outcomes. Results indicate a need for increased emphasis on higher levels of academic attainment as a way to promote positive employment outcomes in these youth. [ABSTRACT FROM AUTHOR]

Objective: Psychiatric symptoms play a crucial role in psychology and psychiatry. However, little is known about how dimensions of symptoms—other than symptom level—relate to psychiatric outcomes. Until recently, methods for measuring dynamic aspects of symptoms have not been available to clinicians or researchers. The authors sought to test whether systematic patterns of change in psychiatric symptoms can be recovered across weekly assessments of individuals at high risk for violence. A secondary objective was to explore whether dynamic features of symptoms (specifically, oscillation speed and dysregulation) are concurrently associated with violence, an important indicator of functional impairment for these individuals. Method: Participants (N=132) were drawn from a sample of patients evaluated at the emergency room of an urban psychiatric hospital, Patients actuarially classified as being at high risk for violence were eligible for participation in the study. Participants and collateral informants were interviewed weekly for 26 weeks following an acute psychiatric evaluation. Psychiatric symptoms were assessed using the Brief Symptom Inventory. Measures of symptom fluctuation and regulation were derived using dynamical systems models. Involvement in violence was assessed using self, informant, and official reports. Results: Individuals' symptom dynamics were recovered by a linear oscillator model that described how quickly symptoms oscillated and whether symptoms were amplifying or moving back toward equilibrium across time. Patterns of rapid symptom fluctuation and symptom amplification were concurrently associated with violence. Conclusions: Psychiatric researchers and clinicians have long been interested in adopting more dynamic approaches to understanding symptom change. This study is the first to demonstrate that systematic fluctuations in symptom patterns may be captured by dynamic models. Moreover, the concurrent association between symptom dynamics and violence suggests avenues for future research to test how features of symptom fluctuation could affect behavior. The Classification of Violence Risk (COVR®) is an interactive software program designed to estimate the risk that a person hospitalized for mental disorder will be violent to others. The software leads the evaluator through a chart review and a brief interview with the patient. At the end of this interview, the software generates a report that contains a statistically valid estimate of the patient's violence risk—ranging from a 1% to a 76% likelihood of violence—including the confidence interval for that estimate, and a list of the risk factors that the program took into account to produce the estimate. Given the availability of violence risk assessment tools, clinicians are now better able to identify high-risk patients. Once these patients have been identified, clinicians must monitor risk state and intervene when necessary to prevent harm. Clinical practice is dominated by the assumption that increases in psychiatric symptoms elevate risk of imminent violence. This intensive study of patients (N = 132) at high risk for community violence is the first to evaluate prospectively the temporal relation between symptoms and violence. Symptoms were assessed with the Brief Symptom Inventory and threat/control override (TCO) scales. Results indicate that a high-risk patient with increased anger in 1 week is significantly more likely to be involved in serious violence in the following week. This was not true of other symptom constellations (anxiety, depression, TCO) or general psychological distress. The authors found no evidence that increases in the latter symptoms during 1 week provide an independent foundation for expecting violence during the following week. [ABSTRACT FROM AUTHOR]

Prior research has consistently demonstrated an association between substance use and involvement in violence among individuals with mental illness. Yet little is known about the temporal quality of this relationship, largely because longitudinal data required to address this issue are not readily available. This study examined the relationship between substance use (alcohol, marijuana, and other drug use) and violence at the daily level within a sample of mentally ill individuals at high risk for frequent involvement in violence (N = 132). Results support the serial nature of substance use and violence, with an increased likelihood of violence on days following the use of alcohol or multiple drugs, but not the inverse relationship. Recent work suggests that predictors of violence are similar for individuals with and without mental illness. Although psychopathy is among the most potent of such predictors, the nature of its relation to violence is unclear. On the basis of a sample of 769 civil psychiatric patients, the authors explore the possibility that measures of psychopathy provide a glimpse of higher order personality traits that predispose individuals toward violence. Results indicate that general traits captured by a measure of the 5-factor model, particularly antagonism, were relatively strongly associated with violence and shared most of their violence-relevant variance with a leading measure of psychopathy. Because interpersonal and affective features of psychopathy are less important than basic traits of antagonism in postdicting violence, it may be appropriate to broaden focus in risk assessment to patients' basic personality traits. Abstract: Once convicted, the perpetrator of serious crime embarks upon a new journey: the challenge of adjusting to long-term imprisonment. Prisoners'' views of incarceration and the meaning of this experience may affect their later adjustment to life in the community. On the basis of brief narrative responses collected during an epidemiological survey of the psychological health of prisoners in France, this study examined the impact of incarceration on psychological state in a group of 59 inmates serving long sentences. Qualitative content analysis and computer-assisted linguistic analysis (using ALCESTE software) were performed on the textual data of open responses to three standard questions. Using a combination of these two approaches, seven categories of the subjective experience of prisoners in the sample were identified: the Outside World, Others, Punishment, Time, Affects and Impulses, Self-Concept, and Speech. Further qualitative analyses were then performed to compare the responses of Severely Mentally Ill (SMI) subjects and subjects with no psychiatric disorder. These analyses revealed contrasting attitudes towards incarceration. SMI subjects spoke in more hostile and persecutory terms about their experience in prison, attributing suffering to external circumstances, while subjects with no psychiatric disorder evoked similar themes, but with an introspective attitude. The themes evoked by mentally ill individuals in our sample suggest that their reactions to the prison environment arise in part from aspects of their psychiatric symptoms, and this may have relevance to future mental health policy and practices in criminal corrections.
