Abstract: The study is to evaluate the impact of COVID-19 in the Pediatric Oncology Units (POUs) of Pakistan. Data from 1 April 2019 to 30 June 2019 and 1 April 2020 to 30 June 2020 for the first and second cohort, respectively, in order to compare the registration, abandonment rate, and delay in treatment. Six hundred and thirty-four were registered cases, 379 and 255 in the first and second cohort, respectively, which was significantly different0.005. Seventy-seven were abandoned, 45 and 32 in the first and second cohort, respectively. Fifty-nine COVID-19 positive cases, 24, 4, 27, and 4 were admitted, referred, home isolated, and leave against medical advice (LAMA), respectively. Delayed treatment and reduction in new cases were observed.

Abstract: BACKGROUND & AIMS There is a lack of convenient, sensitive, non-invasive strategies for screening and surveillance for colorectal neoplasia. An assay combining the results of circulating gastrointestinal epithelial cells (CEC) and somatic mutations of cell-free DNA adjusting for age/sex using a unique algorithm is evaluated in patients requiring colonoscopy. METHODS A prospective single-site 458-subject study (asymptomatic: 43% screening/43% surveillance, enriched with 65 symptomatic subjects) undergoing colonoscopy was conducted. The test analyzed CEC and somatic mutations. The probability of advanced neoplasia (advanced adenoma (AA) and CRC) was determined by logistic regression methods adjusted for expected CRC incidence rate, prior history of AA, patient age and sex on a training subset. A linear predictor was developed to generate a score scaled from 0-100. The test performance was evaluated on an independent set of subjects using pre-specified algorithms and cutpoint. RESULTS Based on a pre-defined clinical threshold and predictive model derived from the training set (n=232), analysis of an independent asymptomatic validation set (n=194) yielded 89% (Lower exact one-sided 95% CI: 80%) specificity and 100% (95% CI: 37%)/78% (95% CI: 61%) sensitivity for detection of CRC/AA. In a secondary analysis, excluding surveillance subjects, the 97-subject screening cohort yielded 91% (95% CI: 79%) specificity and CRC/AA sensitivity at 100% (95% CI:37%)/83% (95% CI: 56%, 87% for advanced neoplasia 95% CI: 64%). Significant associations, (p CONCLUSIONS A multimodal blood test that included CEC and somatic mutations with adjustment for age and sex demonstrated high sensitivity for the diagnosis of advanced colorectal neoplasia. The resulting score captures prognostic information for CRC progression of index adenoma size and number and has the potential to enable stratification of patients for screening or post- polypectomy surveillance colonoscopy.

Abstract: Armed conflict disproportionately affects the morbidity, mortality, and wellbeing of women, newborns, children, and adolescents. Our study presents insights from a collection of ten country case studies aiming to assess the provision of sexual, reproductive, maternal, newborn, child, and adolescent health and nutrition interventions in ten conflict-affected settings in Afghanistan, Colombia, Democratic Republic of the Congo, Mali, Nigeria, Pakistan, Somalia, South Sudan, Syria, and Yemen. We found that despite large variations in contexts and decision making processes, antenatal care, basic emergency obstetric and newborn care, comprehensive emergency obstetric and newborn care, immunisation, treatment of common childhood illnesses, infant and young child feeding, and malnutrition treatment and screening were prioritised in these ten conflict settings. Many lifesaving women's and children's health (WCH) services, including the majority of reproductive, newborn, and adolescent health services, are not reported as being delivered in the ten conflict settings, and interventions to address stillbirths are absent. International donors remain the primary drivers of influencing the what, where, and how of implementing WCH interventions. Interpretation of WCH outcomes in conflict settings are particularly context-dependent given the myriad of complex factors that constitute conflict and their interactions. Moreover, the comprehensiveness and quality of data remain limited in conflict settings. The dynamic nature of modern conflict and the expanding role of non-state armed groups in large geographic areas pose new challenges to delivering WCH services. However, the humanitarian system is creative and pluralistic and has developed some novel solutions to bring lifesaving WCH services closer to populations using new modes of delivery. These solutions, when rigorously evaluated, can represent concrete response to current implementation challenges to modern armed conflicts.

Abstract: In advanced peripheral arterial disease (PAD), medial arterial calcification is known to inflate the ankle-brachial index. An alternative method of evaluating symptomatic patients is toe-brachial indexes (TBI), where a ratio less than .7 indicates PAD and less than .4 indicates a severe form. The objective of this retrospective analysis was to investigate the association between TBIs less than .7 and angiographically verified PAD. Patients were required to have either a leg angiogram 13 months prior to or 12 months after a 6-minute walk test. Of the 174 included patients, the mean overall TBI was .450. The mean TBI by location was highest at iliac and infra-geniculate with .544 and lowest at supra-geniculate with .372. Infra-geniculate lesions were also the most frequent ( n = 46). A TBI less than .4 was found in 47.7% of patients. TBIs greater than .7 were present in 36 patients; however, only 16 had significant angiographic stenosis. In conclusion, the majority of patients with angiographic PAD had a TBI less than .7, especially less than .4. Contrary to suspicion, infra-geniculate lesions were the most common and had the highest TBI.

Abstract: Objective: To determine the impact of endemicity of ascariasis in the manifestation of acute appendicitis. Study Design: Prospective observational study. Place and Duration of Study: General Surgery Department, Combined Military Hospital Skardu, from May 2012 to Apr 2015. Methodology: Patients presenting with symptoms of acute appendicitis were included in the study. Patients were diagnosed after taking detailed history, clinical examination and blood chemistry. All the patients underwent open appendectomy under general anesthesia. Per-operatively presence of worms was assessed in terminal ileum and after that in the lumen of the appendix initially by palpation and then by opening its lumen after it was removed. Histopathologic diagnosis could not be confirmed due to its non-availability as the surgeries were performed in a remote and resource constraint area of the country. Results: A total of 224 patients were included in the study. Out of these, 143 (63.8%) were male and 81 (36.1%) female patients. The mean age of patients was 22 ± 6 years. Per-operatively, 21 (9.3%) patients had worms in both ileum and appendicular lumen while 22 (9.8%) patients had worms only in the terminal ileum and there were no worms seen in either ileum or appendix in 181 (80.8%) patients. Conclusion: There is a relation of parasitic infestation especially Ascaris lumbricoides in the manifestation of acute appendicitis in endemic areas.

Abstract: Objective: To determine the effectiveness of therapeutic plasma exchange (TPE) in management of critically ill Covid-19 patients Materials and Methods: In this study non-randomized controlled trial, a total of 47 patients were included after thorough screening of the admitted patients in Covid ITC CMH, Multan from July-2020 to May-2021.Single administration of TPE was done to determine the results. The parameters assessed in this study included the efficacy of TPE in terms of improvement of critical end points such as Norepinephrine dose to maintain MAP of more than 65mmHg, 6-hour balance of fluid, MAP, CRP (C reactive protein), WBC count (white blood cell), platelets, INR (international normalized ratio), IL-6. Results: The variations before and after TPE in clinical and biochemical parameters shown in table. II. In clinical parameters and in gas exchange parameters no significant difference was found. Inflammatory biomarkers, before and after TPE, the parameters had not significant different, (p>0.005). The differences before and after TPE between acid base balance, cytokines and vasoactive substances were also statistically insignificant, (p>0.005). Conclusion: Even though there is evidence of slight improvement in clinical endpoints of Covid-19 patients with TPE, overall efficacy of TPE is still a question that needs an answer as no significant improvement could be seen in outcome values after TPE. Abstract: Objective: To determine the relationship between anemia, menstrual abnormalities and hematological parameters among students, who are studying at Sindh University and are the permanent residents of Hyderabad city. Methodology: This cross-sectional study was carried out at physiology department, university of Sindh Jamshoro. Study duration was one year from August 2018 to July 2019. All participants were the permanent resident of Hyderabad, aged from 18 to 25 years, and willing to contribute in the study. The questionnaire was designed to elicit comprehensive responses from participants regarding hematological parameters and menstrual abnormalities. Menstrual abnormalities such as poly-menorrhea, menorrhagia, oligo-menorrhea, dysmenorrhea, and amenorrhea were determined using the menstrual history. Participants with a hemoglobin level of 7 days) was 26.85% Anemia in the participants was 45.94%, out of this mild, moderate and severe anemia was 32.51%, 7.42% and 6.01% respectively. RBCs, HCT and WBCs were statistically significant in terms of menstrual abnormalities (P0.05). Conclusion: Prevalence of menstrual abnormalities and anemia were observed to be frequently high among female students of Sindh University’s permanent residents of Hyderabad. Abstract: Background: COVID 19 has become a major health issue and has globally affected masses and increased mortality in all parts of the world. Therefore, with limited information available healthcare workers faced a multitude of problems and challenges in managing the affected individuals globally and to no surprise with developing information database regarding this disease; many lost their lives. Aim: To assess the understanding of healthcare worker regarding COVID-19 pandemic and their competence in dealing with the current situation and to ascertain whether HCWs are actively participating in activities that hell them deal with the outbreak. Study design: Descriptive cross sectional survey Place and duration of study: Department of Medicine, Shaikh Zayed Hospital, Lahore from 26th September 2020 to 30th May 2020. Methodology: Three hundred and sixty eight individuals participated in survey throughout Pakistan. The questionnaire consisting of four parts designed according to available data on coronavirus and study participants were tested for their knowledge and preparedness through multiple questions. All healthcare workers from different designation, specialties and hospitals participated for this research. Results: Two hundred and eighty seven aged less than 35 and rest were above 35.68.5% were from public sector and rest worked in private hospitals.340 people participated from hospitals in Punjab and remaining were from other parts of Pakistan. Internet was major source of information. Study participants stated themselves as prepared in dealing with COVID 19 matters however, knowledge based questions were not answered correctly by many individuals. Conclusion: This research was meant to identify the gaps in information and take measures to provide better health practices. The correct answers percentage was higher for more experienced individuals and who are acutely dealing with Covid infection. Abstract: Blood pressure measurement is a marker of antenatal care quality. In well resourced settings, lower blood pressure cutoffs for hypertension are associated with adverse pregnancy outcomes. We aimed to study the associations between blood pressure thresholds and adverse outcomes and the diagnostic test properties of these blood pressure cutoffs in low-resource settings.We did a secondary analysis of data from 22 intervention clusters in the Community-Level Interventions for Pre-eclampsia (CLIP) cluster randomised trials (NCT01911494) in India (n=6), Mozambique (n=6), and Pakistan (n=10). We included pregnant women aged 15-49 years (12-49 years in Mozambique), identified in their community by trained community health workers, who had data on blood pressure measurements and outcomes. The trial was unmasked. Maximum blood pressure was categorised as: normal blood pressure (systolic blood pressure [sBP]120 mm Hg and diastolic blood pressure [dBP]80 mm Hg), elevated blood pressure (sBP 120-129 mm Hg and dBP80 mm Hg), stage 1 hypertension (sBP 130-139 mm Hg or dBP 80-89 mm Hg, or both), non-severe stage 2 hypertension (sBP 140-159 mm Hg or dBP 90-109 mm Hg, or both), or severe stage 2 hypertension (sBP ≥160 mm Hg or dBP ≥110 mm Hg, or both). We classified women according to the maximum blood pressure category reached across all visits for the primary analyses. The primary outcome was a maternal, fetal, or neonatal mortality or morbidity composite. We estimated dose-response relationships between blood pressure category and adverse outcomes, as well as diagnostic test properties.Between Nov 1, 2014, and Feb 28, 2017, 21 069 women (6067 in India, 4163 in Mozambique, and 10 839 in Pakistan) contributed 103 679 blood pressure measurements across the three CLIP trials. Only women with non-severe or severe stage 2 hypertension, as discrete diagnostic categories, experienced more adverse outcomes than women with normal blood pressure (risk ratios 1·29-5·88). Using blood pressure categories as diagnostic thresholds (women with blood pressure within the category or any higher category vs those with blood pressure in any lower category), dose-response relationships were observed between increasing thresholds and adverse outcomes, but likelihood ratios were informative only for severe stage 2 hypertension and maternal CNS events (likelihood ratio 6·36 [95% CI 3·65-11·07]) and perinatal death (5·07 [3·64-7·07]), particularly stillbirth (8·53 [5·63-12·92]).In low-resource settings, neither elevated blood pressure nor stage 1 hypertension were associated with maternal, fetal, or neonatal mortality or morbidity adverse composite outcomes. Only the threshold for severe stage 2 hypertension met diagnostic test performance standards. Abstract: Batu ginjal merupakan kasus terbanyak yang sering dijumpai pada batu saluran kemih yang menjadi satu dari tiga permasalahan terbanyak di bidang urologi. Prevalensi terbanyak terjadi pada laki-laki. Standar penatalaksanaan terbaik untuk batu ginjal berukuran >20mm adalah percutaneous nephrolithotomy (PCNL) suatu metode minimal invasif yang menggunakan akses perkutan untuk mengeluarkan batu. Salah satu komplikasi terbanyak adalah perdarahan yang dapat diakibatkan oleh beberapa faktor seperti indeks masa tubuh, stone burden, derajat hidronefrosis, metode dilatasi dan lama operasi. Penelitian ini bertujuan untuk melihat hubungan stone burden terhadap penurunan hemoglobin pasca operasi pada pasien batu ginjal yang diterapi dengan PCNL. Abstract: Metastatic castration-resistant prostate cancer (mCRPC) disproportionately affects the elderly. There is limited data assessing the efficacy and tolerability of abiraterone acetate (AA) versus enzalutamide in this population.To compare the clinical efficacy and tolerability of AA versus enzalutamide in patients ≥ 80 years with mCRPC.A retrospective propensity-weighted comparative cohort study of first-line AA versus enzalutamide among patients with mCRPC aged ≥80 years.Inverse probability treatment weights based on propensity scores were generated to assess the treatment effect of AA versus enzalutamide on time to PSA progression (TTPP), time to progression (TTP) (first of PSA/radiographic/clinical progression) and overall survival using a weighted Cox proportional hazards model. PSA response rate (PSA RR) was compared between groups using ΧOne hundred fifty-three patients received AA, and 125 received enzalutamide. Enzalutamide was associated with higher PSA RR (61.6% vs 43.8%, P 0.004), and TTP (hazard ratio [HR] 0.66, 95% confidence interval [CI] 0.50-0.88, P = 0.01) but not TTPP (HR 0.73, 95% CI 0.53-1.01, P = 0.06). There were significantly more dose reductions with enzalutamide (22.9% vs 44.8%, P 0.001) but there was no interaction between median proportion of full dose received and TTPP or TTP for either drug. Rates of treatment discontinuation (for reasons other than progression) were also significantly different between AA and enzalutamide (28.8% vs 40.8%, respectively, P = 0.04). The most common reason for dose reductions and discontinuation of enzalutamide was fatigue (30.4% and 5.6%, respectively).Despite more dose reductions and a higher treatment discontinuation rate, enzalutamide was associated with a higher PSA RR and longer time to progression, than AA. Given that clinical outcomes were not adversely impacted by decreased treatment exposure, dose modification may be a useful treatment strategy to balance toxicity and tolerance.

Abstract: Penetrating abdominopelvic trauma usually results from abdominal cavity violation from a firearm injury or a stab wound and is a leading cause of morbidity and mortality from traumatic injuries. Penetrating trauma can have subtle or complex imaging findings, posing a diagnostic challenge for radiologists. Contrast-enhanced CT is the modality of choice for evaluating penetrating injuries, with good sensitivity and specificity for solid-organ and hollow viscus injuries. Familiarity with the projectile kinetics of penetrating injuries is an important skill set for radiologists and aids in the diagnosis of both overt and subtle injuries. CT trajectography is a useful tool in CT interpretation that allows the identification of subtle injuries from the transfer of kinetic injury from the projectile to surrounding tissue. In CT trajectography, after the entry and exit wounds are delineated, the two points can be connected by placing cross-cursors and swiveling the cut planes obliquely in orthogonal planes to obtain a double-oblique orientation to visualize the wound track in profile. The path of the projectile and its ensuing damage is not always straight, and the imaging characteristics of free fluid of different attenuation in the abdomen (including hemoperitoneum) can support the diagnosis of visceral and vascular injuries. In addition, CT is increasingly used for evaluation of patients after damage control surgery and helps guide the management of injuries that were overlooked at surgery.

Abstract: The genitourinary region is one of the most common sites of extrapulmonary tuberculosis (TB) involvement. The imaging features of genitourinary TB are protean and can mimic other entities, including malignancy, and pose a diagnostic dilemma. Hematogenous seeding and lymphatic spread of mycobacteria from pulmonary, tonsillar, and nodal TB are implicated in the pathogenesis of genitourinary TB. In addition, contiguous extension from the urinary tract and sexual transmission are described as sources of genital TB. Genitourinary TB can be indolent and results in nonspecific signs and symptoms; thus, imaging has a vital role in the working diagnosis for these cases. Classic uroradiologic signs of genitourinary TB are primarily described from the era of intravenous urography and conventional radiography. Now, CT, CT urography, MRI, and US are used in the diagnosis and management. Familiarity with the imaging features of genitourinary TB may help guide the diagnosis and, in turn, lead to timely management. US has a vital role in the evaluation of scrotal and female genital TB. MRI offers superior soft-tissue contrast resolution and excellent depiction of anatomic detail. The various imaging manifestations of genitourinary TB are highlighted. ©RSNA, 2021.

Abstract: Objective: We aimed to find out the differences in degree of depression and anxiety experienced by working mother who leave their children at daycare as compared to those who leave them at home. Study Design: Cross Sectional Descriptive study. Setting: Lahore General Hospital Lahore. Period: October, 2019 to March, 2020. Material & Methods: Through random sampling, 100 working mothers participated by filling a structured questionnaire after informed consent. It consisted of 50 women leaving their children at home and 50 women preferring day care centers while being on work. SPSS was used for analyzing the data. Beck Anxiety Inventory (BAI) and CES-D was used to calculate anxiety and depression levels respectively. Results: Mild to moderate depression and possibility of major depressive disorder along with moderate and severe anxiety is seen more in women leaving children at day care. Low and middle age, leaving kids at hand of maids, grandparents or day care or less educated family members, concerns regarding upbringing of children contribute to stress of working mothers. Our work shows that married women with their spouses develop more major depressive disorders and severe anxiety as compared to divorcees and widows. Nuclear and high-income families have more trend of leaving their children at day care. Conclusion: This study suggests that the women who leave their children at day care show increased rate of moderate and severe anxiety, mild to moderate depression and the possibility of major depressive disorders as compared to those leaving at home.

Abstract: From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions., Wellcome Trust, Medical Research Council, peer-reviewed

Abstract: Objective: To determine the satisfaction rate of dermatology patients, after purposefully improving the consultation skills of residents. Study Design: Mix-method study. Place and Duration of Study: Department of Dermatology, Combined Military Hospital Lahore, from Jan to Sep 2020, Methodology: Empathy informed curriculum was delivered by the consultants and senior registrar prior to the conduction of study as well as an ongoing process to the post-graduate trainees in the department. Patients’ satisfaction with the quality of dermatological consultation was assessed by consultation and relational empathy (CARE) measure. One hundred and seventy six patients were recruited during the study period. Descriptive statistics such as frequencies, means, standard deviations and percentages were determined from the data. Advanced level analysis was done by Pearson’s correlation coefficients to analyse relationships between variables. Results: The mean age of study participant was 39.01 ± SD 19.81 year, with a minimum of 3 years and maximum of 91 years. Ninety two (52.3%) patients were male and 84 (47.7%) were female. The mean consultation and relational empathy score was 40.34 ± SD 8.56 with 16 minimum score and 50 maximum. One hundred and sixty six (94.3%) patients were overall satisfied with the consultation whereas 10 (5.7%) were not. Conclusion: We found high satisfaction rate amongst patients attending dermatology outpatient department. Empathy informed curricula should be emphasized upon to provide holistic patient care.

19. EFFECTS OF IRON DEFICIENCY ANEMIA IN SURGICAL SITE INFECTION

Abstract: Objective: To determine effect of iron deficiency anemia on the patients developing surgical site infection at surgical department of a tertiary care setup in Pakistan. Study Design: Comparative prospective study. Place and Duration of Study: Department of Surgery, Pak Emirates Military Hospital Rawalpindi, from Nov 2018 to Mar 2019. Methodology: A total of 152 indoor hospitalized patients were included in the study. Surgical site infection was assessed by a consultant surgeon according to definitions provided by the Centers for Disease Control and Prevention (CDC) National nosocomial infections surveillance system. Iron deficiency anemia was classed on the basis of hemoglobin and ferritin levels. Results: Eighty six (56.6%) patients were male while 66 (43.4%) patients included in the study were female. Commonest surgical procedure was laparotomy 29 (19.1%) followed by the hernioplasty 27 (17.7%). Out of 152 patients undergoing surgery & admitted in ward, 35 (23.1%) showed the presence of surgical site infection while 117 (76.9%) had no infection. Seventy eight (51.3%) patients had iron deficiency anemia while 74 (48.7%) patients were not anemic. History of transfusion during the surgery and presence of iron deficiency anemia was significantly associated with surgical site infection in the target population. Conclusion: Study exhibited high frequency of surgical site infection in the target population. Regular screening for anemia should be performed at surgery department and patients with history of transfusion during the surgery should be looked after especially. Iron deficiency anemia emerged as an independent factor associated with presence of surgical site infection among surgical patients.

Abstract: Iron deficiency anemia (IDA) is a common public health occurrence of pregnancy which is particularly prevalent in developing countries like Pakistan. During this critical period, the deficiency of iron is very common as the iron requirements are greatly enhanced. IDA during pregnancy is associated with intrauterine growth retardation, premature birth, low birthweight, increased labor time, higher risk of infection, elevated maternal and prenatal mortality, muscle dysfunction, and low physical capacity. The present study was aimed to check the prevalence, effect of diet on IDA and its association with the pregnancy outcome, that is, birthweight. Five different public sector hospitals of Rawalpindi and Islamabad were analyzed. A sample size of 500 pregnant females of third trimester was observed which were then followed after delivery from September 2020 to January 2021. A well‐designed questionnaire was developed where different demographic factors, dietary recalls, biomarkers, and other determinants were obtained. The data from the questionnaires were later coded for the purpose of analysis in the statistical package (SPSS) using chi‐square test. Our study indicated that IDA is a moderate public health problem among pregnant women in twin cities and more than half of study subjects have depleted iron stores. Approximately 63% of the subjects were having iron deficiency and 24.8% were facing severe iron deficiency anemia. Only 12% of the subjects were considered as normal. The high prevalence of anemia in our subjects was probably due to low iron intake and poor dietary habits. The results of the study may not be generalizable due to time constraints and other factors. Maternal anemia still remains a major public health concern that requires more attention especially in developing and underdeveloping countries., Maternal iron deficiency is associated with intrauterine growth retardation, premature birth, low birthweight, increased labor time, higher risk of infection, elevated maternal and prenatal mortality, muscle dysfunction, and low physical capacity. Our study indicated that IDA is a moderate public health problem among pregnant women in twin cities and more than half of study subjects have depleted iron stores. Maternal hemoglobin status was found to affect pregnancy outcome. Newborns born to women with low hemoglobin levels tended to have lower birthweight. Therefore, the etiological factors associated with maternal anemia during pregnancy in Pakistan should deserve more attention.

Abstract: Although it was thought that children were not susceptible to 2019-nCoV in the early days of the COVID-19 infection outbreak, there are currently reports of children and even one-day-old newborns being infected by the virus and hospitalized around the world. Recognizing the symptoms of the infection in children is of great value since a large number of children are asymptomatic or have mild symptoms, which may act as facilitators of the virus transmission. This review aimed to identify and summarize the existing evidence on clinical characteristics of COVID-19 in pediatric patients. A systematic search was conducted in the PubMed, Web of Science, Scopus, Google Scholar, and WHO database for eligible publications. The review proposal was registered with the PROSPERO. The quality assessment was done based on JBI Critical appraisal tools. The random-effects model was used to pool clinical features in the meta-analysis. From the identiﬁed 256 potentially relevant studies, 32 articles met the predetermined inclusion and exclusion criteria. Twenty-one studies fulfilled the criteria for this meta-analysis. Fever (58%) and cough (48%) were reported as the most common symptoms of infected children. Disease severity was mild in 51% and moderate in 39% of cases. A total of 63% of cases had respiratory and 25% of children had gastrointestinal symptoms, particularly diarrhea and nausea/vomiting. Approximately 24% of patients were asymptomatic. Results demonstrated that fever and cough were the most common symptoms of COVID-19-infected children and the majority of cases had mild-to-moderate disease severity.

22. Frequency of grading of complications using modified clavien classification system after transurethral resection of prostate

Abstract: Objective: To determine the frequency of post-operative complications of transurethral resection of prostate (TURP), in benign prostatic hyperplasia (BPH) patients, using the Modified Clavien Classification System (MCCS). Study Design: Descriptive study. Setting: Urology Department, Sindh Institute of Urology and Transplantation, Karachi. Period: 26th May, 2019 to 25th Nov, 2019. Material & Methods: A total number of 162 patients with benign prostatic hyperplasia planned for TURP were included in this study and Post-operative complications data was collected, and classified according to the Modified Clavien Classification System (MCCS). Patient’s demographics and other parameters like prostate volume, operative time, mean prostatic tissue resected and hospital stay was collected. Results: Mean age was 63.32±8.36 years. Mean prostate volume was 56.99±13.25 grams. Mean operative time was 26.55±9.46 mins. Mean prostate tissue resected was 16.75±12.09 grams. Mean hospital stay was 1.27±0.60 mins. Grade I complication was occurred in 06 (3.70%) patients, grade II in 03 (1.85%) patients, grade III in 00 patients, grade IV in 01 (0.62%) patients and grade V in no patient. While there were no complications in remaining 152 (93.83%) patients. Conclusion: Clavien–Dindo classification system can be easily applied by urologists to grade the post-operative transurethral resection of prostate (TURP) complications. We observed that TURP is a very safe procedure for surgical management of benign prostatic hyperplasia, and is having low morbidity and mortality.

Abstract: Study Design: An experimental, comparative, cross-sectional study Place and Duration of Study: Department of Physiology, Federal Post Graduate Medical Institute (FPGMI), Sheikh Zayed Medical Complex Lahore, Pakistan from August 2013 to 2014 Background: Chronic Obstructive Pulmonary Disease (COPD) is a preventable and treatable disease, but is a partially reversible chronic inflammatory condition characterized by airway obstruction. COPD remains under-diagnosed and under-treated because the only available diagnostic method at present is testing lung functions by spirometry which is not helpful to determine the severity and clinical outcomes of the disease. Circulating biomarkers are under consideration for various diseases worldwide. Plasma fibrinogen is emerging as one of the most promising biomarkers of COPD in smokers. Objective: The objective of this study is to investigate if plasma fibrinogen can serve as a diagnostic biomarker of COPD in smokers, and if its further higher levels are seen in the exacerbated state of the disease in comparison to the stable disease. Materials and Methods: 75 middle-aged to old-age smokers of either gender were selected. Lung functions of every participant were measured to determine Forced Expiratory Volume in the first second (FEV1), Forced Vital Capacity (FVC), and the ratio of FEV1/FVC by spirometry. On the basis of the results of the tests, subjects were divided into three groups; firstly, the control group of chronic smokers without COPD, secondly, smokers with COPD in a stable state, and thirdly, patients in an exacerbated state of COPD. Plasma fibrinogen was quantitatively estimated in every individual of all three groups by the Clauss method using the Hemostat Fibrinogen kit. Results: The average Plasma fibrinogen level was found to be 235.008 mg/dl in healthy smokers (control group), while an average of 440.12mg/dl was measured in patients with stable COPD. The difference in plasma fibrinogen levels was found to be significant, having a p-value of (0.000). In the third group with declined lung function predicting acute exacerbated COPD, fibrinogen was found to be > 453.2 mg/dl, which was significantly higher than in the stable disease group (p-value > 0.0017) Conclusion: Plasma fibrinogen level measurement is a reliable and accessible test in terms of a diagnostic marker of COPD, as compared to conventional lung function testing done in the past.

Abstract: Objective: To determine the effect of bilateral oophorectomy on the periodontal status of otherwise healthy women when compared with normal controls. Study Design: Mixed-method study. Place and Duration of Study: Riphah International University, Islamabad, from Apr to Sep 2016. Methodology: A total of 24 women with and without oophorectomy procedures were recruited. Twelve women who has undergone radical hysterectomies at least 6 months prior to the evaluation were recruited for this investigation. These women underwent a comprehensive oral examination with detailed documentation of their periodontal status following a thorough medical/dental history. An age- and sex-matched control population underwent a similar examination. Results: Statistically significant results were noted for probing, color, consistency, texture and contour of gingiva with the p-value of 0.007, 0.001, 0.002, 0.005 and 0.037 when the results were compared with the control population. Conclusion: Bilateral oophorectomy does not compromise the periodontal health of otherwise healthy women.

Abstract: Objective: To know the association between different anthropometric parameters for overweight and obese individuals to prevalence of prehypertension. Study Design: Correlational study. Setting: Vicinity of Kharian and at Outpatient Department, CMH Kharian Cantt. Period: February 2019 and January 2020. Material & Methods: Correlational study carried out in the vicinity of Kharian by employing different anthropometric measurements in accordance with guidelines to look at how blood pressure figure behaves in overweight, obese and morbidly obese population and further is there a difference in overall outcome if we use waist circumference rather than BMI and its significance in targeting preventive strategies. Results: Our results show a staircase pattern of increase in blood pressure, initially lying in different phases of prehypertension among normal weight (BMI 23±1.65; waist circumference (WC): 86.45±8.78; Systolic BP:127±11.99; Diastolic BP:80.44 ±9.45), overweight (BMI 27.35 ±1.23; WC:100±5.32; Systolic BP:128.72 ±11.29; Diastolic BP:80.05 ±6.66) and obese individuals (BMI 31.97 ±1.45; WC:112.12±9.22; Systolic BP:138.06 ±21.61; Diastolic BP:87.56 ±9.78), while this trend shifted to full blown hypertension among morbidly obese individuals (BMI 39.88 ±4.30; WC:120.38±12.84; Systolic BP:145 ±17.64; Diastolic BP:89.23 ±10.95) Conclusion: We conclude that prehypertension is already prevalent among normal weight and overweight individuals, and intensive follow up and lifestyle intervention strategy should be employed earlier at this level and waist circumference is a better predictor of cardiovascular disease than BMI and should be routinely done in local primary health care set up to prevent the onset of complications associated with this silent killer.

Abstract: Images of DaTscan (ioflupane [123I] SPECT) have been used as an adjunct to clinical diagnosis to facilitate the differential diagnosis of neurodegenerative (ND) Parkinsonian Syndrome (PS) vs. non-dopamine deficiency aetiologies of Parkinsonism. Despite several systematic reviews having summarised the evidence on diagnostic accuracy, the impact of imaging results on clinical utility has not been systematically assessed. Our objective was to examine the available evidence on the clinical utility of DaTscan imaging in changing diagnosis and subsequent management of patients with suspected PS. We performed a systematic review of published studies of clinical utility from 2000 to 2019 without language restrictions. A meta-analysis of change in diagnosis and management rates reported from each study was performed using a random-effects model and logit transformation. Sub-group analysis, meta-regression and sensitivity analysis was performed to explore heterogeneity. Twenty studies met the inclusion criteria. Thirteen of these contributed to the meta-analyses including 950 and 779 patients with a reported change in management and change in diagnosis, respectively. The use of DaTscan imaging resulted in a change in management in 54% (95% CI: 47–61%) of patients. Change in diagnosis occurred in 31% (95% CI: 22–42%) of patients. The two pooled analyses were characterised by high levels of heterogeneity. Our systematic review and meta-analysis show that imaging with DaTscan was associated with a change in management in approximately half the patients tested and the diagnosis was modified in one third. Regardless of time from symptom onset to scan results, these changes were consistent. Further research focusing on specific patient subgroups could provide additional evidence on the impact on clinical outcomes.

Abstract: Summary Evaluating whether an intervention works when trialled in groups of individuals can pose complex challenges for clinical research. Cluster randomised controlled trials involve the random allocation of groups or clusters of individuals to receive an intervention, and they are commonly used in global health research. In this paper, we describe the potential reasons for the increasing popularity of cluster trials in low-income and middle-income countries. We also draw on key areas of global health research for an assessment of common trial planning practices, and we address their methodological shortcomings and pitfalls. Lastly, we discuss alternative approaches for population-level intervention trials that could be useful for research undertaken in low-income and middle-income countries for situations in which the use of cluster randomisation might not be appropriate.

Abstract: Objective: To determine the impact of musculoskeletal disorders on quality of life of patients visiting Khyber Teaching Hospital, Peshawar. Method and Material: A cross sectional survey conducted in Khyber Teaching Hospital, Peshawar from September 2018 to March 2019 using a validated EQ-5D quality of life scale. The survey questionnaire was attended by 377 respondents having variety of musculoskeletal disorders (MSDs). EQ-5D can be effectively used for assessing the quality of life and health status using the VAS pain scale. Results: There were total 377 respondents. Mean age of the participants was 35.9 ± 12.5 years. Out of 377 participants, 204 (54.1%) were male, while 173 (45.9%) were female. Muscular weakness was the most commonly encountered problem (40.6%), followed by muscle stiffness (13.0%) and adhesive capsulitis (10.3%). The remaining respondents were having different kinds of MSDs. Patients with MSDs were having significantly moderately low score on EQ-5D. Conclusion: The finding of this study suggests that MSDs have negative impact on quality of life and health status of the sufferers. Individuals having any sort of MSDs should seek proper care in order to improve their quality of life, health status and work performance. Abstract: Background Transcatheter aortic valve replacement (TAVR) is now indicated in patients with symptomatic aortic stenosis and low, moderate, and high surgical risk. There are multiple types of valves available in TAVR. SAPIEN 3, and Evolut R are two of the most commonly used valves. Methods We conducted a systematic review and meta-analysis of all studies that compared SAPIEN 3 vs Evolut R in patients undergoing TAVR. The primary endpoint of this meta-analysis was 30-day mortality. Secondary outcomes included major of life-threatening bleeding, risk of stroke, need of permanent pacemaker implantation, and risk of moderate to severe paravalvular regurgitation (PVR). Results We included a total of 9 studies. One study was a randomized clinical trial, five were prospective observational studies and three were retrospective. 30-day mortality rate was similar between SAPIEN 3 and Evolut R (odds ratio (OR) 1.19; 95% confidence interval (CI) 0.72 to 1.93; p = 0.47). The risk of major or life-threatening bleeding (OR of 0.83, 95% CI 0.50 to 1.39; p = 0.48), and the risk of stroke (OR of 0.82, 95% CI 0.38 to 1.78; p = 0.62) were also similar between the two types of valves. Compared to SAPIEN 3, Evolut R was associated with statistically significant risk of permanent pacemaker implantation (OR of 1.40, 95% CI 1.15 to 1.70; p = 0.0007), and moderate to severe PVR (OR of 2.56, 95% CI 1.14 to 5.74; p = 0.02). Conclusions At 30 day follow up, both Evolut R and SAPIEN 3 shared similar risks of 30-day mortality, major or life-threatening bleeding, and stroke; however greater odds of pacemaker placement implantation and moderate to severe PVR were associated with Evolut R.

Abstract: Dravet Syndrome (DS) is a rare and severe infantile-onset epileptic encephalopathy. DS research focuses mainly on children. We did a systematic review, completed on January 18th, 2021, examining the number of clinical DS studies. We show that there are 208 studies on children exclusively, 28 studies on adults exclusively, and 116 studies involving adults and children combined. This 7:1 ratio of children to adult studies exclusively shows the dearth of research that addresses long-term natural history of DS into adulthood. Through this systematic review, we examine the most up-to-date information in DS adults as it pertains to seizures, electroencephalogram, imaging, treatment, motor abnormalities, cognitive and social behavior outcomes, cardiac abnormalities, sleep disturbances, diagnosis in adults, and mortality. Overall, the frequency of seizures increases in the first decade of life and then myoclonic, atypical absences and focal seizures with impaired awareness tend to decrease in frequency or even disappear in adulthood. Adults tend to have a notable reduction in status epilepticus, especially after 30 years of age. Parkinsonian features were seen in patients as young as 19 years old and are more severe in older patients, suggesting a progression of the parkinsonian symptoms. In adulthood, patients continue to present with behavior problems, associated with a lower health-related quality of life. The leading reported cause of death in DS adults is Sudden Unexpected Death in Epilepsy (SUDEP). Further studies in older adults are needed to understand the long-term outcomes of patients with DS.

Abstract: Aim: To evaluate the association of hepatitis with the hepatocellular carcinoma. Hepatitis is known as the inflammation of the liver. Methods: Hepatitis C and B may lead to HCC (hepatocellular carcinoma), such as Hepatitis B may cause 50% and hepatitis C is responsible for 70 to 80 % of cases of HCC. Obesity, alcohol and type 2 diabetes are also a predisposing factor for HCC. HCC is the malignant tumor, and have a low survival rate due to reoccurrence. Results: Eighty two patients were selected with hepatitis and HCC. This cross-sectional study was conducted at Allied Hospital Faisalabad. Ultrasound machine of Toshiba was used with a transducer of frequency 3-6MHz for the liver scanning. 82 patients with hepatitis B, C, and HCC were included in this study. Out of 82 patients, 55 were male and 27 were female. The mean age was 55yrs. Group A 45 (54%) patients were infected with hepatitis C and Group B 35(42%) patients were infected with hepatitis B. Hepatitis C is the most common cause of HCC as compared to hepatitis B. Conclusion: In this study, it is concluded that there is a positive association between hepatitis and Hepatocellular Carcinoma. All consecutive patients admitted in the labour ward with twin pregnancies were included. Before delivery, the fetal wellbeing of both the twins was evaluated also by ultrasonography. Data collection was done by interview schedule from the patients. Data were analyzed using descriptive statistics using SPSS version 17. Setting: Diabetes Clinic & Antenatal Clinic of Lahore General Hospital/ Post Graduate Medical Institute/ Ameer ud Din Medical College, Lahore. Period: July 2019 to December 2019. Material & Methods: 46 females who were 24-38 weeks pregnant and having gestational diabetes mellitus, type1 diabetes mellitus or having type 2 diabetes mellitus first diagnosed during pregnancy were enrolled. Data was collected by an interview based questionnaire, analyzed using SPSS version 23, and 95% confidence interval was used as test of significance. Results: The mean age was 27±9 years. 20% of the participants were Illiterate 80% were literate. GDM in the all three trimesters was 45.6%, 39% and15% respectively. 30.5% were hypertensive and 100% adopted lifestyle modifications, 89% were using insulin and 10.8% were taking metformin. 33% had history of instruments delivery and 36% had adverse pregnancy or perinatal outcomes. Regarding patients belief’s, 10% believed in diet control, 26% were not comfortable with lab and physician’s counselling, 38% were in state of denial, 26% were unaware of screening, 70% were frightened of unexpected diagnosis and consequences. 18% were concerned of target organs damage, 16% were concerned of mode of delivery, 25% regarding subsequent pregnancy, 53% were concerned about insulin treatment and its duration and 33% were keen to know about adverse pregnancy outcomes. Conclusions: Educational status is not known to influence gestational diabetes mellitus, Major concern of patient with GDM was her health and surroundings, treatment options, adverse effect on baby and subsequent pregnancies outcomes.

Abstract: Background: Intussusception is an important disease of childhood & has been recognized since ancient time. It commonly involves young infants of 3 ½ months to 10 months of age. Diagnosis is mostly clinical on the basis of classical triad or quarter of pain in abdomen, vomiting palpable abdominal mass and currant red jelly stools. It is confirmed by ultrasound, barium enema which also has a therapeutic potential. The parents in our region lacks awareness regarding the intussusception. They usually bring the child to hospitals after trying various homemade or herbal remedies. The delay in diagnosis always results in severity in symptoms and aggressive mode of treatment. Objective: The current study was aimed to identify the Frequency, associated symptoms and post-operative complications of intussusception in tertiary care hospital of Province Sindh Methodology: It was a cross sectional study conducted from July 2017 to June 2020 at department of Pediatric Surgery Ghulam Mohammad Mahar Medical College Teaching Hospital Sukkur. Data regarding symptoms and prognosis was noted on the file of patient and written informed consent was taken from parents. Data was analyzed by using SPSS version 20. Chi square was applied as test of significance at 95%confidance interval and p-value less than 0.05 was considered as significant. Results: Fourty patients of intussusceptions were reported the mean age was 5.88 ± 1.5 months while 8 (20%) patients were 1to10 years with mean age of 3.88± 3.18 years. Thirteen (32.5%) patients presented within 24 hours of onset of symptoms and 27 (67.5%) presented after 24 hours 19 (47.5%) patients had history of diarrhea while 6 (15%) patients had respiratory tract infection. The most common postoperative complication was gut gangrene. Conclusion: In three years of study we found only 40 children who came to our hospital with diarrhea and the triad i.e. abdominal pain, abdominal mass and bleeding per rectum. Ultrasound abdomen was found to be best diagnostic modality with impressive accuracy rate at our setup. Gut gangrene was postoperative complication and recurrence was uncommon.

Abstract: ObjectiveCompare paediatric COVID-19 disease characteristics, management and outcomes according to World Bank country income level and disease severity.DesignSystematic review and meta-analysis.SettingBetween 1 December 2019 and 8 January 2021, 3350 articles were identified. Two reviewers conducted study screening, data abstraction and quality assessment independently and in duplicate. Observational studies describing laboratory-confirmed paediatric (0–19 years old) COVID-19 were considered for inclusion.Main outcomes and measuresThe pooled proportions of clinical findings, treatment and outcomes were compared according to World Bank country income level and reported disease severity.Results129 studies were included from 31 countries comprising 10 251 children of which 57.4% were hospitalised. Mean age was 7.0 years (SD 3.6), and 27.1% had a comorbidity. Fever (63.3%) and cough (33.7%) were common. Of 3670 cases, 44.1% had radiographic abnormalities. The majority of cases recovered (88.9%); however, 96 hospitalised children died. Compared with high-income countries, in low-income and middle-income countries, a lower proportion of cases were admitted to intensive care units (ICUs) (9.9% vs 26.0%) yet pooled proportion of deaths among hospitalised children was higher (relative risk 2.14, 95% CI 1.43 to 3.20). Children with severe disease received antimicrobials, inotropes and anti-inflammatory agents more frequently than those with non-severe disease. Subgroup analyses showed that a higher proportion of children with multisystem inflammatory syndrome (MIS-C) were admitted to ICU (47.1% vs 22.9%) and a higher proportion of hospitalised children with MIS-C died (4.8% vs 3.6%) compared with the overall sample.ConclusionPaediatric COVID-19 has a favourable prognosis. Further severe disease characterisation in children is needed globally.

Abstract: Background: Poziotinib is a novel pan-HER inhibitor that irreversibly blocks the EGFR family of tyrosine-kinase receptors and inhibits the proliferation of tumor cells. This study evaluates the safety and clinical activity of poziotinib in patients with HER2-positive metastatic breast cancer (MBC) who received at least 2 therapies (trastuzumab and TDM-1) in dose-schedule ranging study. Methods: Patients were treated with oral poziotinib in 2 dose cohorts: 24mg daily 2 weeks/1 week off and 16mg daily continuously in a 21-day cycle. Dose reduction was allowed if toxicity observed. Patients continued treatment until disease progression, death, intolerable AE, or for a maximum of 24 months. The primary endpoint was the objective response rate (ORR), evaluated using RECIST 1.1. Secondary endpoints included disease control rate (DCR), duration of response (DOR), progression-free survival (PFS) and safety. Results: Sixty-seven patients (33 in 24mg; 34 in 16mg) were enrolled (57 evaluable) in 2 cohorts; all patients either completed or discontinued (36 PD, 5 deaths due to PD, 16 AEs) the study with 1 completed 25 months of treatment. The median (range) age was 57 (29-94) years. Patients were heavily pretreated and the median (range) lines of previous therapy were 7 (2-13) and 4 (2-16) in 2 cohorts respectively [unique drugs 3 (2-5) and 3 (1-9)]; 75% received pertuzumab in addition to trastuzumab and TDM-1 and 37% received at least one tyrosine kinase inhibitor (TKI). The mean relative dose intensity was 57% and 51% with 67% and 47% had dose reductions in 2 cohorts respectively. Common Grade ≥3 treatment-related AEs were similar to other 2nd generation TKIs and include diarrhea (30%), rash (28%) and stomatitis (7%). The ORRs were 27% and 26% with the corresponding median DORs of 5.6 and 13 months respectively. 3 patients in 16mg dose had a CR and another 2 patients had an unconfirmed CR. The DCRs were 50% and 70% in 2 cohorts along with median PFS of 4.1 and 5.8 months respectively. The ORRs were 25% each in 2 cohorts in 24 and 20 heavily pre-treated patients with ≥4 lines of therapy that included trastuzumab, TDM1 and pertuzumab. The ORRs were 23% and 0% in 2 cohorts with 13 and 10 patients received at least one tyrosine kinase inhibitor (TKI) as the sample sizes were small to make any meaningful evaluation. Conclusion: Poziotinib has demonstrated clinical activity in this dose-ranging study with tumor reduction shown in the majority of patients along with durable responses in this heavily pre-treated MBC patients. Abstract: Hypertrophic olivary degeneration is a kind of trans-synaptic degeneration, caused by the interruption of dentato rubro olivary pathway. Magnetic resonance imaging (MRI) has been the best modality to show the signals of olivary nucleus hypertrophy. It appears on T2-weighted magnetic resonance imaging as hyper-intensities. Here we present a unique case of a 27-year-old male with traumatic brain injury causing multiple intracranial haemorrhages and functional impairment, which was revealed on computerised tomography. Tracheotomy and thoracic drainage were performed immediately. Anti-infection therapy, brain protection, and comprehensive arousal therapy were part of the intervention along with a comprehensive rehabilitation programme including occupational therapy, balance training, coordination, bed mobility training, and strengthening exercises. A holistic diagnostic approach can reduce the chances of misdiagnosing post-traumatic Hypertrophic Olivary Degeneration cases. We concluded that a comprehensive physical rehabilitation programme and medical treatment works best to heal the lesion and its resulting traumatic symptoms in cases of Hypertrophic Olivary Degeneration cases.

46. Evaluation of endocrine and cardiac complications in survivors of childhood acute lymphoblastic leukemia: A single center study from south Punjab

Abstract: Objectives: The objective of our study was to evaluate the endocrine and cardiac complications in survivors of childhood ALL. Study Design: Descriptive Cross Sectional study. Setting: Department of Pediatric Hematology-oncology, Endocrinology and Cardiology at The Children Hospital & Institute of Child health Multan (CH& ICH). Period: January 2019 to June 2019. Material & Methods: Sixty patients were enrolled after taking informed consent from patient/parents. Endocrine complications include (growth failure, overweight/obesity, hyperlipidemia, thyroid disorders, precocious puberty) and cardiac complications include hypertension and low ejection fraction were evaluated. Results: Out of 60 ALL survivors 32 were males (53.3%) and 28 were females (46.7%). At diagnosis patient’s age range was 7-17 years with median age of 11.6 years. At study median age was 17 years with age range of 13 to 23 years. Among our ALL survivors 56.6% had one complication and 25% had multiple problems. Obesity (25%), overweight (20%) and hyperlipidemia (26.4%) were the three common complications followed by short stature (5%), thyroid disorders (3.3%) and hypertension (3.3%). Conclusion: We report high prevalence (56.6%) of these complications from south Punjab. Therefore extended follow up of ALL survivors is needed in order to identify an abnormality and timely treatment and improved quality of life.

Abstract: Lyme disease is a tick-borne illness that occurs in stages, multiple organs and tissue with highly variable clinical presentation. Most commonly, it presents with seventh cranial nerve palsy, often mimicking stroke and atypical rash (erythema migrans). Atypical presentations include abdominal pain, ileus/pseudo-obstruction and constipation thought to be due to autonomic dysfunction. Other less common presentations include Syndrome of Inappropriate Antidiuretic Hormones (SIADH). Lyme disease should be a differential when a patient presents from Lyme endemic areas with abdominal pain, constipation and SIADH in the setting of other causes of gastrointestinal and renal symptoms ruled out. Here we present a case of multisystem involvement in a single patient with Lyme Disease along with neuroborreliosis (neurological manifestation of Lyme disease).

Abstract: Background & Objectives: Vincristine has been used as chemotherapeutic agent for many decades. It implements its function by inhibiting the duplication of tumor cells by destroying the DNA. However, like all other drugs, its administration is not without any side effects. The most important of these are being the neurotoxic side effects. This study evaluated the degree of neurotoxicity induced by vincristine in children who underwent chemotherapy for acute lymphoblastic leukemia and Wilms tumor. Methods: A quasi experimental study was conducted at Children Hospital & the Institute of Child Health, Multan from January 2020 to October 2020 after taking informed written consent. In this study, 150 children of age group 1—12 years with pathological confirmation of acute lymphoblastic leukemia and Wilms tumor who had undergone a chemotherapy protocol including at least four consecutive weekly Vincristine injections were included, using probability consecutive sampling technique. Neurological examination was conducted on them on weekly basis. Results: There were 150 patients,90(60%) males and 60(40%) females with mean age of (5.5±2.2). Diminished patellar and Achilles tendon reflexes were seen in 48% and 52% of patients. Muscular weakness was seen in 60% of patients. Other side effects like hoarseness, jaw pain, constipation and petosis were observed in 10%, 8%,40% and 10% of patients respectively. Frequency of side effects was equally observed in both sexes and it was more among age group older than five years (p = 0.01). Abstract: Remdesivir is an antiviral drug currently being studied as a potential treatment of pneumonia caused by infection with SARS-CoV-2. The Adaptive Covid-19 Treatment Trial (ACTT-1) by NIH and the SIMPLE study by Gilead Sciences are two major trials that showed promising results of Remdesivir in the non-pregnant population. We are presenting the case of a pregnant patient who was diagnosed with COVID-19 pneumonia and successfully treated with Remdesivir.

Abstract: OBJECTIVE. Loss of the neurofibromatosis type 1 (NF1) tumor suppressor protein causes uninhibited activation of the RAS oncogene, which leads to tumorigenesis in patients with NF1. This case-based review discusses imaging manifestations of NF1 in the abdomen and pelvis, highlighting key genetic associations and management to elucidate features different from the general population. CONCLUSION. The spectrum of pathologic findings includes gastrointestinal tumors such as gastrointestinal stromal tumors, genitourinary lesions including urogenital neurofibromas, vascular entities such as renal artery stenosis, and less common associations like lymphoma.
