BackgroundPost-intensive care syndrome (PICS) seriously affects the quality of life of patients and their caregivers. This study systematically evaluates PICS related guidelines and consensuses to provide methodological and strategic guidance for the development and improvement of these guidelines and consensus.MethodsAn Internet search of PICS specific or related guidelines and consensus statements was conducted among academic databases and the websites. The AGREE II was used to evaluate their methodological quality. The themes and contents were evaluated based on the definition of PICS and the “patient-clinical problem-intervention-target” framework.ResultsWe included fourteen guidelines and 10 expert consensuses. The mean AGREE II score for scope and purpose was 86.89% ± 16.12%/ 79.47% ± 11.80%, 55.86% ± 29.17%/16.72% ± 16.39% for stakeholder involvement, 76.35% ± 15.82% /36.09% ±26.05% for rigor of development, 88.03% ± 15.55%/58.84% ± 28.94% for clarity of presentation, 61.79% ± 23.16/32.95% ± 20.91% for applicability, and 66.67% ± 37.96% /28.79% ± 36.54% for editorial independence in Guidelines/Consensus. Most only described a single or series of clinical related issues of PICS; only applicable for critically ill patients with clinical problems of ICUAW, pain, sleep disruption, agitation, and delirium. The interventions included risk assessment, monitoring/diagnosis, prevention, treatment, safety criteria, and home care of one or more symptoms of PICS.ConclusionsOur results suggested that development and improvement of guidelines/consensus should focus on refining the methodology, strengthening quality control, and improving presentation clarity, editorial independence, and applicability. The results may provide new idea for a future guidelines and consensus statements.

Background: To further determine the clinical efficacy and survival of intrathoracic infusion with TPs and chemical irritants and their therapeutic threshold and optimal control regimen to achieve a desired response in malignant pleural effusion (MPE). Methods: We collected all randomized controlled trials (RCTs) of TPs with chemical irritants from Chinese and English databases (from inception until September 2019), and performed a new meta-analysis following the PRISMA guidelines. We measured their bias risk, summarized data using meta-analysis, and evidence quality using the Grades of Recommendation Assessment, Development and Evaluation approach. Results: We collected 24 trials involving three TPs and platinum and 1,592 patients. Most trials had unclear bias risk. TPs with platinum significantly improved complete response [4.02 (3.12 to 5.18)] and quality of life [3.64 (2.34 to 5.66)], the 0.5-year overall survival (OS) rate [5.75 (3.02 to 10.92)], and 1-year OS rate [5.29, (1.71 to 16.36)], and reduced the treatment failure, myelosuppression, and gastrointestinal toxicity. For patients with moderate to large volume of pleural effusion, KPS score ≥50 to 60, or AST ≥3 months, the thymosin (200–300mg/time), thymopentin (2mg/time) or thymosin alpha 1 (3.2mg/time) with cisplatin (30–40mg/m 2 ), carboplatin (400mg/m 2 ), or oxaliplatin (100mg/m 2 ) are possible regimens for achieving a desired success, and low failure. Most results were robust and moderate quality. Conclusion: The moderate evidence suggests that the TPs with platinum is beneficial to the patient with MPE, and provides evidence for the therapeutic threshold and possible regimens that may achieve a desired success and reduce the failure.

Background: Opioid dependence, a major global public health concern, can be alleviated through methadone maintenance treatment (MMT). However, MMT possibly increases motor vehicle accident risk. The aim of this study was to determine motor vehicle accident risk in MMT users. Methods: We performed preliminary estimation of motor vehicle accident incidence rates in Taiwan. In this population-based retrospective cohort study with frequency-matched controls, adult opiate users receiving MMT and not receiving MMT (control) were identified at a 1:2 ratio by linking data from several nationwide administrative registry databases. All individuals were followed for motor vehicle accident events. Data were adjusted for potential covariates and assessed using Kaplan–Meier and Cox proportional hazard model analyses. Findings: From 2009 to 2016, the crude incidence rate of motor vehicle accidents was the lowest in the general adult population, followed by that in adult opiate users, and it was the highest in adults receiving MMT. Aderivative of Shiitake mushrooms, Lentinan is used to control malignant pleural effusion (MPE) through intrathoracic infusion.To determine the clinical response, survival and safety of Lentinan plus chemical irritants, and the optimal combinations with chemical irritants, indication, threshold and optimal regimen for achieving the desired responses.We performed a new systematic review and meta-analysis following the PRISMA guidelines.We collected all randomized controlled trials (RCTs) regarding Lentinan plus chemical irritants from Chinese and English electronic databases (from inception until March 2019). We evaluated their bias risk, synthesized data using meta-analysis, and summarized evidence quality following the Grades of Recommendation Assessment, Development and Evaluation approach.We included 65 RCTs involving 4,080 patients and nine chemical irritants. Most trials had unclear bias risk. Lentinan with cisplatin significantly improved complete response [Risk ratio (RR) = 1.68, 95% confidence intervals (CI) (1.51 to 1.87), p0.00001, Fig.3a] and quality of life [RR = 1.51 95% CI (1.41 to 1.62), p0.00001, Fig.4], and decreased the risk of treatment failure, myelosuppression, gastrointestinal reaction, and chest pain. For patients with moderate to large volume of the pleural effusion, primary treatment, KPS score ≥ 50-60, or anticipated survival time ≥ 3months, Lentinan (3-4 mg/time, once a week for three to four times) withcisplatin (30-40 mg/mThe results suggest that Lentinan with chemical irritants, especially cisplatin is beneficial to the patient with MPE, and provide evidence for the indication, threshold, and optimal regimen that may achieve success and decrease failure.

Background/Aim Cerebral white matter changes (WMC) are commonly observed in magnetic resonance imaging (MRI) scans of elderly people. Information about the prevalence of WMC is limited, and little is known about site-specific risk factors for the subcortical and periventricular regions in patients with ischaemic stroke. The study aims to analyse the prevalence and severity of WMC and investigate the risk factors of periventricular WMC (PVWMC) and deep WMC (DWMC) separately in patients with ischaemic stroke. Methods The data were collected between January and December 2013 from a medical centre in southern Taiwan. Every patient underwent a cerebral MRI scan, and WMC was separately rated as PVWMC and DWMC by using the modified Fazekas scale. Results In total, 527 patients who had experienced ischaemic stroke were included. The mean age of the patients was 67.0 ± 12.5 years (range: 31–94) and 62% of them were men. The mean age was significantly different among the four grades of severity in both the PVWMC (P < 0.001) and DWMC (P < 0.001) groups after adjustments for sex and vascular risk factors. Hypertension was independently correlated with severity of DWMC (P = 0.032) but not with PVWMC (P = 0.222). In multiple logistic regressions model, hypertension was a significant independent indicator of DWMC (odds ratio = 4.30; 95% confidence interval = 1.70–10.89). Conclusion Our results suggest a region-specific pathogenesis of cerebral white matter in Asian patients with ischaemic stroke that may differ from those in the general population.

To examine how playing exergames impacts the mood states of university students and staff, and whether such an impact depends on gender and players' previous exercise time. This study was designed as a randomized controlled trial. It enrolled 337 participants and randomly assigned them to an intervention group (n = 168) or a control group (n = 167). A 2-week exergame program was designed for the participants in the intervention group. They were required to play exergames for 30 consecutive minutes each week for 2 weeks and respond to the items measuring vigor, happiness, and perceived stress. All measures were administered before and after the study. Repeated measures analysis of variances were conducted. Playing exergames enhanced vigor and happiness for participants in the intervention group. This group exhibited more positive change in vigor and happiness than the control group. This effect of playing exergames was not moderated by gender, age, occupation (student or staff), or previous exercise time. Playing exergames may induce positive mood states among university students and staff.

We report the effect of in-pregnancy periodontal care intensity on the risk of preterm low birthweight (PLBW) births, and the role of household income in this association. Only a few studies have examined the role of socioeconomic status (SES) in the relationship between prenatal periodontal care and risk of PLBW delivery under universal health care. This study adopted a population-based, case-control study using medical claims data in Taiwan. Mothers of PLBW and non-PLBW babies with livebirth deliveries between 2001 and 2005 (cases, n=20430 and controls, n=81573, respectively) were identified using the National Health Insurance Research Database (NHIRD) and matched. Periodontal disease and disease-specific (gingivitis and periodontitis) dental service utilization was calculated during gestational period and analyzed for its association with risk for PLBW delivery. Recent studies showed that air pollution is a risk factor for hospitalization for chronic obstructive pulmonary disease (COPD). However, there is limited evidence to suggest which subpopulations are at higher risk from air pollution. This study was undertaken to examine the modifying effect of specific secondary diagnosis (including hypertension, diabetes, pneumonia, congestive heart failure) on the relationship between hospital admissions for COPD and ambient air pollutants concentrations. Hospital admissions for COPD and ambient air pollution data for Taipei were obtained for the period from 1999-2009. The relative risk of hospital admissions for COPD was estimated using a case-crossover approach. None of the secondary diagnosis we examined showed much evidence of effect modification.

It would appear that the correlation between air pollutant exposure and MI occurrence is not affected by predisposing factors present in other cardiovascular diseases.

Foreign workers employed in industries in Taiwan have been found at elevated risk of injuries. Less well known is whether the elevated risk persists in chemical exposure such as lead exposure at battery manufacturing. A cohort of 70 Thai workers and 55 native workers employed at a battery plant were followed up, after an education of job safety, from 2000 until 2002. This study compared the change of blood lead levels (BLLs) between these two groups of workers. With informed consent, BLLs were measured annually for participants and compared. The average baseline BLLs were approximately at similarly high levels between Thai workers and native workers with means +/- standard deviations of 36.9 +/- 16.4 and 36.2 +/- 12.4 microg/dl, respectively (p = 0.79). At the end of 2002, the average concentration was higher in Thai workers than in native workers. Using mixed models, Thai workers had an average of 5.95 microg/dl increase in BLLs over native workers during the 3-year study. Further measurements revealed that the average BLL for workers in the assembly department was 3.57 +/- 1.83 microg/dl in excess, compared with workers in the plate engineering department. Thai workers were more likely to drink alcohol but less likely to wear gloves at work and wash hands before meals. The BLL disparities between Thai workers and native workers can partly be explained by differences in risk-taking behaviors. Higher BLLs in Thai workers suggest the need of language appropriate health education to improve their personal hygiene. Workplace smoking ceasing program may be needed both in Thai and native workers.

Objective To evaluate the use of prospective screening for the HLA-B*58:01 allele to identify Taiwanese individuals at risk of severe cutaneous adverse reactions (SCARs) induced by allopurinol treatment. Design National prospective cohort study. Setting 15 medical centres in different regions of Taiwan, from July 2009 to August 2014. Participants 2926 people who had an indication for allopurinol treatment but had not taken allopurinol previously. Participants were excluded if they had undergone a bone marrow transplant, were not of Han Chinese descent, and had a history of allopurinol induced hypersensitivity. DNA purified from 2910 participants’ peripheral blood was used to assess the presence of HLA-B*58:01. Main outcome measures Incidence of allopurinol induced SCARs with and without screening. Results Participants who tested positive for HLA-B*58:01 (19.6%, n=571) were advised to avoid allopurinol, and were referred to an alternate drug treatment or advised to continue with their prestudy treatment. Participants who tested negative (80.4%, n=2339) were given allopurinol. Participants were interviewed once a week for two months to monitor symptoms. The historical incidence of allopurinol induced SCARs, estimated by the National Health Insurance research database of Taiwan, was used for comparison. Mild, transient rash without blisters developed in 97 (3%) participants during follow-up. None of the participants was admitted to hospital owing to adverse drug reactions. SCARs did not develop in any of the participants receiving allopurinol who screened negative for HLA-B*58:01. By contrast, seven cases of SCARs were expected, based on the estimated historical incidence of allopurinol induced SCARs nationwide (0.30% per year, 95% confidence interval 0.28% to 0.31%; P=0.0026; two side one sample binomial test). Conclusions Prospective screening of the HLA-B*58:01 allele, coupled with an alternative drug treatment for carriers, significantly decreased the incidence of allopurinol induced SCARs in Taiwanese medical centres.

Background. Although the link between hyperuricemia and metabolic syndrome had been recognized, the association of the dyslipidemia among individuals with hyperuricemia remains not comprehensively assessed.Methods. Using NHANES III study, we examined the relation between serum lipid profiles and different serum uric acid levels, including serum total cholesterol, LDL cholesterol, triglycerides, HDL cholesterol, apolipoprotein-B, lipoprotein (a), apolipoprotein AI, ratio of triglycerides to HDL cholesterol, and ratio of apolipoprotein-B to AI.Results. After adjusting for potential confounders, average differences (95% confidence interval) comparing the top to the bottom (reference) serum uric acid were 0.29 (0.19, 0.39) mmol/L for total cholesterol, 0.33 (0.26, 0.41) mmol/L for triglycerides, 0.14 (0.01, 0.27) mmol/L for LDL cholesterol, −0.08 (−0.11, −0.05) mmol/L for HDL, and 0.09 (0.05, 0.12) g/L for serum apolipoprotein-B. Notably, ratios of triglycerides to HDL cholesterol and apolipoprotein-B to AI were also linearly associated with uric acid levels (Pfor trend < 0.001).Conclusions. This study suggested that serum LDL cholesterol, triglycerides, total cholesterol, apolipoprotein-B levels, ratio of triglycerides to HDL cholesterol, and ratio of apolipoprotein-B to AI are strongly associated with serum uric acid levels, whereas serum HDL cholesterol levels are significantly inversely associated. In the clinical practice, the more comprehensive strategic management to deal with dyslipidemia and hyperuricemia deserves further investigation.

Background Shipbreaking workers are typically exposed to a wide range of hazardous chemicals. However, long-term follow-up studies of their mortality patterns are lacking. This study examined mortality among shipbreaking workers over a 24-year follow-up period. Methods A total of 4,962 shipbreaking workers were recruited from the database of the Kaohsiung Shipbreaking Workers Union. The data were then linked to the Taiwan National Death Registry from 1985 to 2008. The mortality ratios—standardized for age and calendar years—(SMRs) for various causes of deaths were calculated with reference to the general population of Taiwan. Results Among men workers, a statistically significant increased SMR was observed for all causes (SMR = 1.28), all cancers (SMR = 1.26; particularly noteworthy for lesions of oral and nasopharyngeal: SMR 2.03, liver: SMR 4.63, and lung: SMR 1.36), cirrhosis of the liver (SMR = 1.32), and accidents (SMR = 1.91). A statistically significant increase in mortality was observed for respiratory system cancer (SMR = 1.87) and lung cancer (SMR = 1.91) among workers with a longer duration of employment (≥7 years). The result also showed that among shipbreaking workers who were still alive, two people had mesothelioma and 10 people have asbestosis. Conclusions Those employed in shipbreaking industries experienced an increase in mortality from all causes. Among many studies on the health effects associated with lead burden in human, none has addressed for menstrual cycle. Studies have reported that blood lead levels (BLLs) may increase as estrogen levels decrease. We conducted a study to test whether the BLLs vary during the menstrual cycle. The subjects were 138 teen girls recruited at a junior nursing college. Blood specimens were collected in non-menses days in September 2003 (baseline BLLs) and during days of the menses in December 2003 (between 72nd and 96th hours of the period for follow-up measures). The paired test for differences of BLLs was used to compare between these two measures. There were no significant differences in baseline characteristics between students who had participated in both the baseline and follow-up studies and students who had participated only in baseline study. The average BLLs measured during the menses was 0.20 microg/dL higher than that measured at the baseline study (p0.01). This finding suggests that the average BLL was 7.6% increased at the menses among these teen girl students. In the future, the time to collect blood samples for measuring lead for women should be standardized.

Vascular remodelling is a critical vasculopathy found in atheromatous diseases and allograft failures. The local renin angiotensin system (RAS) has been implicated in vascular remodelling. However, the mechanisms by which the augmented local RAS is associated with the initial event of endothelial cell apoptosis in injured vasculature remain undefined. We induced the apoptosis of human umbilical vein endothelial cells (HUVECs) and vascular smooth muscle cells (VSMCs) through serum starvation (SS). After the cells were subjected to SS, we found that the mRNA expression of angiotensinogen (AGT) was increased by >3-fold in HUVECs and by approximately 2.5-fold in VSMCs. In addition, the expression of angiotensin-converting enzyme (ACE) mRNA was increased in VSMCs but decreased to 50% in HUVECs during the same apoptotic process. Increases in the expression of AGT protein and angiotensin II (Ang II) were found in a serum-free medium conditioned by HUVECs (SSC). The increased Ang II was suppressed using lisinopril (an ACE inhibitor) treatment. Moreover, the activation of ERK1/2 induced by the SSC in VSMCs was also suppressed by losartan. In conclusion, we first demonstrated that the augmented AGT released from apoptotic endothelial cells acts as a vital progenitor of Ang II to accelerate vascular remodelling, and we suggest that blocking local augmented Ang II might be an effective strategy for restraining intimal hyperplasia.

Although the link between impaired lung function and cardiovascular events and type 2 diabetes mellitus has been recognized, the association between impaired lung function and metabolic syndrome has not been comprehensively assessed in the United States (U.S.) population. The aim of our study was to explore the association between impaired lung function and metabolic syndrome in a nationally representative sample of men and women. This cross-sectional population-based study included 8602 participants aged 20-65 years in the Third National Health and Nutrition Examination Survey (NHANES III). We examined the relationship between the different features of metabolic syndrome and lung function, including forced vital capacity (FVC) and forced expiratory volume in 1 second (FEV1).
