Introdução: O cirurgião-dentista ocupa uma posição estratégica no reconhecimento das alterações que envolvem a cavidade bucal e o diagnóstico precoce constitui uma das formas mais eficazes de combater uma doença. Objetivo: Esta pesquisa relatou a prevalência das patologias bucais que acometeram os pacientes que procuraram o Serviço de Prevenção e Diagnóstico Precoce de Câncer de Boca na rede Municipal de Saúde de Passo Fundo (RS) entre janeiro de 2006 e dezembro de 2010. Materiais e Métodos: O estudo foi realizado por meio de análise dos prontuários de atendimento de todos 444 pacientes atendidos neste ambulatório. Foi feito um levantamento das lesões encontradas, associando os fatores de risco, gênero e idade visando determinar a prevalência dessas patologias. Resultados: O grupo das hiperplasias reacionais foi o mais prevalente com 31,3% dos casos. As lesões bucais acometem principalmente pacientes com mais de 40 anos. Conclusão: Pacientes do gênero masculino, com mais de 40 anos, que apresentam fatores de riscos associados, são o grupo mais propenso a desenvolver neoplasias malignas na cavidade bucal., Introduction: The dentist occupies a strategic position in the changes recognition involving the oral cavity and the early diagnosis is one of the most effective ways to combat a disease. Objective: This study reported the prevalence of oral diseases for Oral cancer Health Center users in Passo Fundo, RS, between January 2006 and December 2010. Materials and Methods: The study was done by medical records analysis of all patients. A lesion survey was done associating risk factors, gender and age to determine the prevalence of these pathologies. Results: The reactive hyperplasia group was the most prevalent with 31.3% of cases. The oral lesions mainly affect patients over 40 years. Conclusion: Male patients with more than 40 years, who have risk factors associated, are the most tendency group to develop oral cancer.

Objectives To assess the association between oral mucosa hyperpigmentation in patients with leukemia and imatinib mesylate use. Additionally, we compared our data to those obtained from a systematic review. Materials and methods A cross-sectional study was conducted with 74 patients undergoing treatment with imatinib mesylate. Sociodemographic characteristics, oral mucosa alterations, and medical history were evaluated. Oral hyperpigmentation was scored. The use of imatinib mesylate and hydroxyurea was evaluated. Association between oral hyperpigmentation and imatinib mesylate was assessed. A systematic review was also conducted to retrieve case reports or case series of patients with oral hyperpigmentation associated with imatinib mesylate. Results Among the 74 participants, 41 were male (55.4%) and 33 were female (44.6%). Participants’ mean age was 49.3 years. Sixty-six (89.2%) patients developed hyperpigmented lesions in the hard palate mucosa. In multivariate analysis, patients who had used imatinib mesylate for > 72 months had a hyperpigmentation score 1.62 times higher than those who had used this medication during a shorter period. Patients who had used hydroxyurea for > 30 days had a hyperpigmentation score 1.43 times higher than those who had used this medication during a shorter period. The systematic review retrieved 20 clinical cases of patients undergoing imatinib mesylate treatment and exhibiting oral hyperpigmentation. Conclusions The development of oral hyperpigmentation is associated with imatinib mesylate use. Hydroxyurea seems to increment such an association. Clinical relevance To assist providers in the differential diagnosis of hyperpigmented lesions associated with imatinib mesylate, as well as in the clinical management of such lesions. Objetivos Avaliar a associação entre hiperpigmentação da mucosa oral em pacientes com leucemia e mesilato de imatinibe usar. Além disso, comparamos nossos dados com os obtidos em uma revisão sistemática. Materiais e métodos Foi realizado um estudo transversal com 74 pacientes em tratamento com mesilato de imatinibe. Foram avaliadas características sociodemográficas, alterações da mucosa oral e histórico médico. A hiperpigmentação oral foi marcou. Foi avaliado o uso de mesilato de imatinibe e hidroxiureia. Associação entre hiperpigmentação oral e imatinibe mesilato foi avaliado. Uma revisão sistemática também foi realizada para recuperar relatos de casos ou séries de casos de pacientes com hiperpigmentação associada ao mesilato de imatinibe. Resultados Entre os 74 participantes, 41 eram do sexo masculino (55,4%) e 33 do sexo feminino (44,6%). A média de idade dos participantes foi de 49,3 anos. Sessenta e seis (89,2%) pacientes desenvolveram lesões hiperpigmentadas na mucosa do palato duro. Na análise multivariada, os pacientes que usaram mesilato de imatinibe por > 72 meses tiveram um escore de hiperpigmentação 1,62 vezes maior do que aqueles que usaram este medicação durante um período mais curto. Pacientes que usaram hidroxiureia por > 30 dias apresentaram escore de hiperpigmentação 1,43 vezes maior do que aqueles que usaram esse medicamento por um período menor. A revisão sistemática recuperou 20 casos clínicos de pacientes submetidos a tratamento com mesilato de imatinibe e exibindo hiperpigmentação oral. Conclusões O desenvolvimento de hiperpigmentação oral está associado ao uso de mesilato de imatinibe. A hidroxiureia parece incrementar tal associação. Relevância clínica Para auxiliar os profissionais no diagnóstico diferencial de lesões hiperpigmentadas associadas ao mesilato de imatinibe, bem como no manejo clínico dessas lesões.

A 77-year-old woman with a rare oral intraductal papilloma arising from the minor salivary gland located on the floor of the mouth and causing the mucus retention is reported. Microscopically, the lesion was characterized by unicystic cavity exhibiting the lumen partially filled by papillary projections of the ductal epithelium with varying degree of oncocytic metaplasia. Based on the histopathological analysis, the differential diagnosis of oral intraductal papillomas and other ductal neoplasms of salivary origin are discussed.

O cisto odontogênico calcificante (COC) foi primeiramente descrito em 1962, considerado como condição patológica benigna de ocorrência rara em maxila e mandíbula, caracterizado por revestimento cístico de células epiteliais odontogênicas contendo células fantasmas com propensão a regiões de calcificação. Neste artigo, descreveu-se a configuração clínica e histopatológica do COC por meio de relato de caso submetido a tratamento cirúrgico e acompanhamento pós-operatório. Uma paciente de 11 anos apresentou um aumento de volume em região maxilar a esquerda próxima ao sulco nasolabial sem sintomatologia dolorosa. Os exames radiográficos indicaram lesão radiolúcida, bem circunscrita e expansiva em maxila, que foi submetida a enucleação associada a ostectomia marginal das paredes corticais ósseas. O quadro histopatológico foi caracterizado por revestimento cístico com epitélio odontogênico ameloblástico, ninhos espalhados de células fantasmas e regiões eosinofílicas de material compatível com dentinóide. O relato de caso apresentou perfil clínico de COC, que pode ser confundido com outras lesões císticas ou tumorais, sendo essencial diagnóstico por meio de análise histopatológica. A abordagem cirúrgica proporcionou diagnóstico e tratamento. Após acompanhamento clínico e radiográfico, não houve recorrência do COC.

Granulocytic sarcoma (GS) is an extramedullary tumor associated with myelodysplas tic syndromes or myeloproliferative diseases. Intraoral manifestations are considered uncommon, with a reasonable number of cases, and are mostly related to leukemia. The association of oral GS and myelofibrosis is very rare and only three cases have been published. In this paper, we report the fourth case of oral lesion in a patient with a diagnosis of myelofibrosis. The aim of this study was to present a review of the literature, discussing the current and previous cases of oral GS associated with myelofibrosis or other hematological disorders and the importance of accurate diag nosis through clinical, microscopic, and immunohistochemical features. O sarcoma granulocítico (SG) é um tumor extramedular associado a síndromes mielodisplásicas ou doenças mieloproliferativas. As manifestações intraorais são consideradas incomuns, com número razoável de casos, e estão principalmente relacionadas à leucemia. A associação de SG oral e mielofibrose é muito rara e apenas três casos foi publicado. Neste trabalho, relatamos o quarto caso de lesão oral em paciente com diagnóstico de mielofibrose. O objetivo deste estudo foi apresentar uma reviso de na literatura, discutindo os casos atuais e anteriores de SG oral associada a mielofibrose ou outros distúrbios hematológicos e a importância do diagnóstico preciso por meio de características clínicas, microscópicas e imuno-histoquímicas.

Hybrid lesions containing a similar component to the giant cell granuloma (GCG) associated with the central odontogenic fibroma (COF) and fibro-osseous lesions have been reported in the literature since 1992; in contrast, peripheral cases are extremely rare. Thus, the purpose of this manuscript is to report a case of a hybrid lesion constituted by peripheral giant cell granuloma (PGCG) and peripheral ossifying fibroma (POsF), emphasizing the clinical, radiographic, histopathological, therapeutic and prognostic aspects of such lesion. In that way, our case presentation comprised a 31-year-old feodermic female patient that presented a large asymptomatic exophytic lesion in the left mandibular alveolar mucosa, radiographically showing an irregular radiopaque area within the soft tissue lesion. An excisional biopsy was performed under local anesthesia. Histopathological analysis displayed a fragment of oral mucosa lined by parakeratinized and atrophic stratified squamous epithelium with ulcerated areas; the lamina propria exhibited dense connective tissue containing numerous inflammatory multinucleated giant cells (MGCs) permeated by fusiform cells, blood vessels, hemorrhage and hemosiderosis; in addition, deposition of trabecular bone matrix intermixed by MGCs, proliferation of fibroblasts and collagen fibers was observed. When this association between PGCG and POsF occurs, the possibility of a PGCG exhibiting bone formation should be rule out. Therefore, the hybrid PGCG and POsF may represent a diagnostic challenge to pathologists, and more cases with histopathologic features similar to our case should be reported in order to elucidate the questionable histogenesis of this lesion.

The aim of this study is to evaluate the immunohistochemical expression of E-cadherin, N-cadherin and Bmi-1, and their association with clinical parameters and with the degree of histopathological differentiation in oral squamous cell carcinomas. 65 squamous cell carcinoma samples were used for constructing a tissue microarray block, and then immunohistochemistry was performed for different markers. Background The regeneration of integrity and tissue homeostasis after injury is a fundamental property and involves complex biological processes fully dynamic and interconnected. Although there are medications prescribed to accelerate the process of wound healing by reducing the exaggerated inflammatory response, comes the need to search for different compounds of Amazonian biodiversity that can contribute to the acceleration of the healing process. Among these products, the copaiba oil-resin is one of the most prominent feature in this scenario, as they have been reported its medicinal properties. Methods Aiming to evaluate the anti-inflammatory and healing effect of copaiba oil-resin (Copaifera reticulata Ducke) in transfixing injury of rats’ tongues first proceeded up the copaiba oil-resin oral toxicity test in 5 male mice to stipulate the therapeutic dose which was established at 200 mg/kg/day. Then it was induced transfixing injury in a total of 15 Wistar rats. The animals were randomly divided into three groups based on the treatment: control group, dexamethasone group and copaiba oil-resin group. After 7 days of treatment, histological slides stained with hematoxylin and eosin was prepared. Immunohistochemistry for CD68 (macrophage marker) was performed and analyzed by the cell counter Image J. Results The acute toxicity test showed that the oil-resin copal has low toxicity. Furthermore, copaiba oil-resin therapy modulates the inflammatory response by decreasing the chronic inflammatory infiltrate, edema and specifically the number of macrophages. Conclusions The results indicate the potential of the Amazon region and showed up relevant because therapy with this extract modulates the inflammatory process.

Intravascular papillary endothelial hyperplasia (IPEH) is an uncommon benign vascular lesion that comprises a reactive proliferation of endothelial cells arising from a consolidating thrombus. Oral presentations of IPEH are relatively rare, and when they do occur the most common affected site is the lower lip; in addition, IPEH can be mistaken for an angiosarcoma. This present report aims to describe an IPEH affecting the buccal mucosa, besides exposing 20 other cases of IPEH that were published over the past 40 years and that occurred within this same oral location. A literature review of those lesions along all other cases of oral IPEH was performed and took into consideration several aspects of the lesions, including their immunohistochemical profiles. The present case was diagnosed as IPEH due to characteristic histopathological findings and immunohistochemical reactions for CD34 and vimentin, which revealed strong positivity in the lining endothelial cells and confirmed the vascular origin of the lesion. Additionally, a positive reaction for laminin was observed in the basal membrane of the vessel walls. Low positivity for Ki-67 and an unexpected though interesting immunopositivity for GLUT-1 was observed. We emphasize that knowing the clinical characteristics, the specific histological arrangement and the immunophenotype of IPEH is extremely important for establishing the correct diagnosis and treatment among similar lesions.

Objective: To describe demographic and clinicopathologic profile of oral squamous cell carcinoma (OSCC) cases from incisional biopsies collected at the Nova Friburgo Health Institute of the Fluminense Federal University [Instituto de Saúde de Nova Friburgo da Universidade Federal Fluminense (ISNF/UFF)] and at the Raul Sertã Municipal Hospital [Hospital Municipal Raul Sertã (HMRS)]. Methods: Oral Pathology Laboratory records from the ISNF/UFF were reviewed and all cases of OSCC were primarily selected. Cases from ISNF/UFF and HMRS were selected and had their demographic data collected from the medical records. Clinical characteristics were evaluated using the images of the lesions taken on the biopsy day and the laboratory files. The histological slides were later reviewed. Results: Eighty cases of OSCC were identified. Most patients were male (56.7%), with a mean age of 60 years, smoker (62.71%) and/or alcoholic (44.55%). The most affected anatomic site was the tongue (39.49%), presenting mainly as an ulcer (39.49%). Microscopically, the well-differentiated lesions were more common (35.44%), and the mean counting of mitoses was 5.5/10 high-power field. Conclusion: The profile of OSCC patients in Nova Friburgo partly reflects the world literature, with emphasis on the following differences: low average number of mitoses found in the histopathological analysis and prevalence of well-differentiated lesions. Such differences may be a result of the characteristic variations of the local population, reinforcing the importance of conducting epidemiological studies that demonstrate OSCC peculiarities in different regions. RESUMEN Objetivo: Trazar el perfil sociodemográfico y clínico de los casos de biopsia incisional de carcinoma oral de células escamosas (COCE) recogidos en el Instituto de Saúde de Nova Friburgo de la Universidade Federal Fluminense (ISNF/UFF) y en el Hospital Municipal Raul Sertã (HMRS). Métodos: Se revisaron los archivos del Laboratorio de Patología Oral, y todos los casos de COCE han sido primariamente seleccionados. Los casos procedentes del ISNF/UFF y del HMRS han sido seleccionados y sus datos demográficos han sido recogidos a partir de los historiales médicos. Las características clínicas fueron evaluadas basándose en las fotografías de las lesiones sacadas en el día de la biopsia y en las informaciones descriptas en las boletas de solicitud del laboratorio. Los portaobjetos histopatológicos fueron posteriormente revisadas. Resultados: Ochenta casos de COCE fueron identificados. La mayoría de los pacientes era del sexo masculino (56,7%), con edad media de 60 años, fumadores (62,71%) y/o alcohólicos (44,55%). La localización anatómica más afectada fue la lengua (39,49%), presentándose, principalmente, como una úlcera (39,49%). Microscópicamente, las lesiones bien diferenciadas fueron las más comunes (35,44%), y la media de mitosis cuantificada fue de 5,5/10 campos de aumento mayor. Conclusión: El perfil de los pacientes con COCE em Nova Friburgo refleja, parcialmente, la literatura mundial, destacando las siguientes diferencias: bajo número medio de mitosis encontradas en el análisis histopatológico y prevalencia de lesiones bien diferenciadas. As características clínicas foram avaliadas com base nas fotografias das lesões tiradas no dia da biópsia e nas informações descritas nas fichas de requisição laboratorial. As lâminas histopatológicas foram revisadas posteriormente. Resultados: Oitenta casos de CCEB foram identificados. A maioria dos pacientes era do sexo masculino (56,7%), com idade média de 60 anos, tabagista (62,71%) e/ou etilista (44,55%). A localização anatômica mais acometida foi a língua (39,49%), apresentando-se, principalmente, como uma úlcera (39,49%). Microscopicamente, as lesões bem diferenciadas foram as mais comuns (35,44%), e a média de mitoses quantificada foi de 5,5/10 campos de grande aumento. Conclusão: O perfil dos pacientes com CCEB em Nova Friburgo reflete, em parte, a literatura mundial, com destaque para as seguintes diferenças: baixo número médio de mitoses encontradas na análise histopatológica e prevalência de lesões bem diferenciadas. Lichen planus is a chronic inflammatory disease involving the skin and mucosa, which often affects the oral cavity. The objective of this study is to report a case of oral lichen planus (OLP) with cutaneous manifestations and to discuss the clinical, histopathological aspects and the established treatment. A 61-years-old female white patient was referred for evaluation of white lesions in the oral mucosa. In the intraoral examination, multiple white lesions with striated appearance were observed in the jugal mucosa, tongue and border. The extraoral examination revealed scaly lesions on the arm, white spots on the legs, and nail dystrophy on feet. Based on biopsy of the oral lesions and the histopathological analysis, the diagnosis of OLP was confirmed. The patient underwent treatment with clobetasol propionate topical cream (0.5 mg), and was instructed to apply it to affected area, once or twice a day for four weeks. In the clinical follow-up after one month and 15 days, the improvement of the lesions could be analyzed. As OLP is a disease with an etiopathogenesis that is still poorly recognized, several factors may enable the development of this condition. Therefore, the dentist's clinical view is essential for the most effective treatment.

Background The combination of probiotics and prebiotics might be useful to treat oral halitosis. The aim of this study was to assess the effect of Lactobacillus salivarius G60 (LS) and inulin on oral halitosis and tongue coating. Methods In this double-masked, randomized, phase II clinical trial, 45 patients (aged 35 ± 15 years, 66% female) with oral halitosis and tongue coating were allocated to three treatment groups (n = 15) using gums of oral dissolution (one gum every 12 hours) for 10 days. Each gum contained LS (1 billion colony forming units [CFUs]) + inulin (1 g), LS (1 billion CFU) or placebo. Primary outcomes were organoleptic test, Halimeter, and tongue coating, whereas secondary outcomes were quality of life (QOL) and treatment safety. Generalized linear models were used, adjusting for age and sex. In vitro tests were performed to verify whether LS interacts with inulin and whether LS inhibits the growth of Porphyromonas gingivalis and Prevotella intermedia. Results Forty-four patients (97%) completed the study. Patients treated with LS + inulin showed greater reduction in halitosis measured by Halimeter compared with placebo (adjusted post-intervention average: 96.7 versus 142.5 ppb; P = 0.003), whereas LS and placebo did not differ (115.7 versus 142.5 ppb; P = 0.097). Organoleptic measurements and coating index showed a similar decrease for all groups. QOL improved in patients treated with LS + inulin compared with placebo (P = 0.029). Side effects were mild and transient in all groups. LS did not metabolize inulin but inhibited the growth of P. gingivalis and P. intermedia after 72 hours. Conclusions Treatment with L. salivarius G60 combined or not with inulin showed significant decrease in the outcomes organoleptic test, Halimeter, and coating index, improving oral halitosis. However, no significant difference was obtained between the groups.

Bone lesions of the jaws have their origin from odontogenic and non-odontogenic structures. They can be benign or malignant, asymptomatic, they can be located around the root of the tooth, around the crown and in the interradicular area or they may not be related to the teeth. OBJECTIVE: to determine the frequency of the different bone lesions and the concordance between the clinical and histopathological diagnosis, in the clinical internship of the Faculty of Dentistry of the University of Costa Rica (UCR). METHODOLOGY: retrospective study of bone lesions recovered from the biopsy archive of the Faculty of Dentistry of the UCR from 2008 to 2015. Information on sex, age, location of the lesion, clinical diagnosis and diagnosis were evaluated and described. The agreement between the clinical and histopathological diagnosis was verified by the Kappa test. RESULTS: The 77 cases of oral bone lesions preferentially affected men 53.8% (n=41), the average age was 34.7 years (s.d.±19.6) and with lesions predominantly located in the posterior jaw 36.4% (n=28) and anterior maxilla 35.1% (n=27). Odontogenic cysts (OC) 42.9% (n=33), non-specific or unclassified diagnosis 28.6% (n=22) and inflammatory lesions of pulp and periapical origin 14.2% (n=11). TOs accounted for 7.8% (n=6) of the lesions. The four most predominant lesions were the radicular cyst, nonspecific diagnosis, dentigerous cyst and periapical granuloma. Concordance with the first diagnostic hypothesis was presented in 24 (31.2%) cases, the value of Kappa was 0.274 (discrete concordance) and 20.8% without clinical diagnosis only a description of the lesion. CONCLUSIONS: The OC were the predominant; being individually the radicular cyst the most frequent lesion. The clinical and histopathological concordance was discrete. Las lesiones óseas de los maxilares tienen su origen a partir de estructuras odontogénicas y no odontogénicas. Pueden ser benignas o malignas, asintomáticas, pueden ubicarse alrededor de la raíz del diente, alrededor de la corona y en el área interradicular o pueden no tener relación con los dientes. OBJETIVO: determinar la frecuencia de las diferentes lesiones óseas y la concordancia existente entre el diagnóstico clínico e histopatológico, en el internado clínico de la Facultad de Odontología de la Universidad de Costa Rica (UCR). METODOLOGÍA: estudio retrospectivo de lesiones óseas recuperados del archivo de biopsias de la Facultad de Odontología de la UCR del 2008 a 2015. Se evaluaron y analizaron de forma descriptiva y cualitativa información sobre el sexo, edad, localización de la lesión, diagnóstico clínico y diagnóstico histopatológico. La concordancia entre el diagnóstico clínico e histopatológico fue verificada mediante el test Kappa. RESULTADOS: los 77 casos de lesiones óseas orales afectaron preferentemente a los hombres 53.8 % (n=41), el promedio de edad fue 34.7±19.6 años y con lesiones predominantemente localizadas en mandíbula posterior 36.4 % (n=28) y maxila anterior 35.1% (n=27). Los quistes odontogénicos (QO) 42.9% (n=33), diagnóstico no específico o sin clasificación 28.6% (n=22) y lesiones inflamatorias de origen pulpar y periapical 14.2% (n=11). Los TO representaron el 7.8% (n=6) de las lesiones. Las cuatro lesiones más predominantes fueron el quiste radicular, diagnóstico inespecífico, quiste dentígero y granuloma periapical. La concordancia con la primera hipótesis diagnóstica se presentó en 24 (31.2%) casos, el valor de Kappa fue de 0.274 (concordancia discreta) y un 20,8 % sin diagnóstico clínico solo una descripción de la lesión. CONCLUSIONES: Los QO fueron los predominantes; siendo en forma individual el quiste radicular la lesión más frecuente. La concordancia clínica e histopatológica fue discreta.

Plasmablastic Lymphoma (PBL) is a hematolymphoid malignant disease that has a predilection for the oral cavity and jaw. The aim of this paper is report a total resolution of oral manifestation of PBL without any oncological treatment; this process is extremely rare and we discuss the mechanism which can occur. We present a case of PBL in left maxilla and oral mucosa in a woman HIV-positive patient. After an incisional biopsy an unusual outcome of spontaneous regression of the disease occurred, we reported the diagnostic process, the management and the follow up of case. We revised the similar cases reported in the literature and we will discuss the hypotheses how the phenomenon can occur. Although the PBLs are aggressive lesions, with questionable prognosis, the spontaneous regression can occur and the patient should be monitored for the risk of metastases and possible recurrence of the disease.

Resumo Contexto A angiogênese tem sido associada à progressão de neoplasias malignas e, embora haja estudos acerca de marcadores angiogênicos no carcinoma epidermoide oral (CEO), existem resultados conflitantes na literatura. Objetivos Avaliar a expressão imunoistoquímica do CD105 e do fator de von Willebrand (FvW) em CEO e sua relação com parâmetros clínicos do tumor. Métodos A imunoexpressão dos referidos biomarcadores foi analisada em 30 casos de CEO e correlacionada a parâmetros clínicos do tumor (idade e sexo dos pacientes, localização anatômica e estadiamento clínico Tumor, Nodo e Metástase, TNM). Resultados A imunomarcação com o anticorpo anti-FvW foi mais efetiva que a do CD105 no CEO. No que concerne à localização anatômica, o assoalho bucal e a região retromolar apresentaram diferenças estatisticamente significativas quanto aos índices angiogênicos (p = 0,004), determinados pela técnica de contagem microvascular (MVC). Não houve relação estatisticamente significativa entre o estadiamento clínico TNM e os índices angiogênicos, com os dois biomarcadores. Conclusões Com base nos achados deste estudo, sugere-se um envolvimento da neoformação vascular na carcinogênese oral, embora não tenha sido evidenciada associação significativa com o estágio clínico da lesão.

O grau de queratinização da superfície influencia na cor, variando de branca a vermelha. Histopatologicamente é caracterizado pela presença de macrófagos com citoplasma espumoso (células de xantoma) acumulados e confinados ao tecido conjuntivo papilar. Criptas invaginadas alternam-se com extensões papilares. As cristas epiteliais alongadas estendem-se para a lâmina própria em profundidade uniforme. Sua etiologia e patogênese são desconhecidas, apesar de várias hipóteses terem sido sugeridas, como trauma local ou viral. O diagnóstico diferencial pode incluir o papiloma escamoso, o carcinoma escamoso papilar e o condiloma acuminado. O tratamento é a excisão conservadora e o prognóstico é excelente. The etiology and pathogenic mechanisms are unknown, although some hypothesis, such as local trauma or viral origin have been suggested. Introdução: O hemangioma é um tumor benigno que ocorre devido à proliferação anormal de vasos sanguíneos, acomete principalmente lábios, língua, mucosa jugal e palato. Geralmente, o diagnóstico é clínico, através de anamnese e exame físico detalhado. Entre as opções terapêuticas, a escleroterapia vem sendo utilizada com resultados satisfatórios clínicos e estéticos. O oleato de monoetanolamina (Ethamolin®) é um derivado do ácido oleico com propriedades coagulativas que realizam coagulação local da lesão, se tornando uma forma conservadora e segura de tratamento mesmo em pacientes idosos. Relato do caso: Paciente do sexo masculino, melanoderma de 80 anos, compareceu ao hospital da restauração com uma lesão na borda lateral da língua de caráter nodular, arroxeada o qual foi submetido ao tratamento com agentes esclerosantes. Discussão: A conduta terapêutica foi a escleroterapia química com Ethamolin com remissão total da lesão e resultado estético satisfatório, em aproximadamente 40 dias. Objetivo: Através deste relato observa-se que o tratamento foi efetivo, mesmo não utilizando métodos invasivos. A substância oleato de monoetanolamina (Ethamolin®) tem se mostrado bastante positiva em numerosas aplicações na área da saúde.

The aim of the present study was to compare negative impacts of oral conditions in Oral Heath Related Quality of Life (OHRQoL) assessed by the Oral Health Impact Profile-14 (OHIP-14) scores in pregnant women receiving or not comprehensive periodontal treatment. This randomized controlled clinical trial included pregnant women aged between 18 and 35 years old. Participants were randomized in a test group with 96 and a control group with 114 women. Patients in the test group received comprehensive periodontal treatment, supra and subgingival scaling and root-planning and periodontal maintenance appointments. The OHIP-14 was applied before and after treatment. The primary outcome was changes in OHIP-14 scores after follow-up period. The impact of having received or not comprehensive periodontal treatment on the change of the OHIP-14 scores was also investigated. Both groups showed significant reduction in OHIP-14 scores and effect size for the test group was 0.60 and 0.36 for the control group. Multinomial logistic regression analysis showed that participants of the control group had 5.9-fold odds (CI 95% 1.88-18.52) of worsening in OHIP-14 scores and their perception of oral conditions in relation to test group. Comprehensive periodontal treatment during pregnancy can reduce the negative impacts in OHRQoL.

Este estudo observou a prevalência e a localização de patologias ósseas, correlacionando-as com o sexo e a idade, por meio de exames radiográficos de 155 pacientes, que procuraram o Serviço de Radiologia Odontológica e Imaginologia e a clínica de Estomatologia e Patologia bucal da Odontoclínica Central da Marinha. Foram avaliadas imagens panorâmicas, periapicais, tomografias computadorizadas de feixe cônico (TCFC) e multi-slices (TC) destes pacientes. Realizaram- se testes estatísticos Qui-quadrado, razão de prevalência e ANOVA, com nível de significância de 5%. A amostra consistiu em 55,5% de mulheres e 45,5% de homens, com média de idade de 50 (±1,8) e 39,1 (±2,4) anos, respectivamente. O exame mais solicitado foi a radiografia panorâmica (97,4%), seguido das periapicais (44,5%), TCFC (26,5%) e TC (14,2%). As patologias mais detectadas foram as displasias ósseas (28,4%) posteriormente as escleroses ósseas (23,9%), tumores benignos (23,9%) e cistos (14,8%). A maioria das lesões foi observada na mandíbula (54,2%). Os molares foram os dentes mais associados às lesões (16,1%), seguido dos incisivos (11%) e pré-molares (7,1%). A razão de prevalência entre os gêneros revelou que os cistos foram aproximadamente 2,5 vezes mais comuns em homens e as displasias foram 2,7 vezes mais comuns em mulheres. A great number of lichenoid lesions have overlapping clinicopathological features, so the use of adjunct tests to establish definitive diagnosis is recommended for correct management and prognosis of the lesions. In this context, direct immunofluorescence (DIF) can be a useful tool. Thus, this study aimed to characterize the clinical, histopathological, and DIF pattern in patients with oral lichen planus (OLP) and patients with oral lichenoid lesions (OLLs). Patients with OLP and patients with OLL were characterized and compared with patients with mucous membrane pemphigoid, pemphigus vulgaris, and fibrous hyperplasia through a cross-sectional study. Patients with OLP (n = 30) and patients with OLL (n = 26) were mostly white women in the fifth decade of age, with reticular lesions mainly on the buccal mucosa. All patients with OLP and half of the patients with OLL showed liquefaction degeneration at the basal cell layer and a band-like lymphocytic infiltrate in the subepithelial tissue. Twenty-two patients with OLP (73.3%), 10 with OLL (38.4%), 25 with mucous membrane pemphigoid (96.1%), and all with pemphigus vulgaris (100%) had positive DIF. There was no positive DIF in patients with fibrous hyperplasia. The most frequent DIF pattern in patients with OLP and patients with OLL was linear fibrinogen at the basement membrane zone, and a logistic regression model for positive DIF found statistically significant difference in OLP versus OLL (odds ratio, 3.73; confidence interval, 1.23-11.38). Although clinical and histopathological features are sufficient for diagnosing most of the patients with OLP and OLL, DIF is a key tool in differentiating some lichenoid lesions and could improve the diagnosis of OLP and OLL, especially in lesions showing typical clinical and histological features of OLP.

Chronic Neutropenia (CNP) is a primary blood disorder characterized by very low neutrophil levels every 3 to 4 weeks. Manifesting as oral ulcers, fever, lymphadenopathy, indisposition, diarrhea, and severe infections, CNP is a systemic condition that may induce the emergence of oral ulcers whose clinical aspects are sometimes mistaken for a series of other diseases. In addition, the oral manifestation of CNP may be the first event to lead a patient to look for professional healthcare. This case report describes CNP in an adult male patient with oral lesions recurring for two years and who was diagnosed by his dentist.

Central ossifying fibroma is a lesion in which diagnosis has proved intriguing and unclear for presenting clinical, radiographic, and even histopathological similarities to other types of lesions such as the fibrous dysplasia of bone and cement-bone dysplasia. It is a benign neoplasm of uncertain etiology and slow development in which the mandible is affected more frequently than the maxilla. We described a case of central cemento-ossifying fibroma involving the right mandible of a thirty-five-female patient by presenting its clinical, radiographic, and histological characteristic sand discussing some differential diagnoses.

OBJECTIVE: This article presents a case of ossifying fibroma (OF), including its diagnosis, treatment, and 15-month clinical and radiographic follow-up as well as a review of the literature about this pathology.CASE REPORT: A routine panoramic radiograph of a 27-year-old, systemically healthy, white woman revealed a radiolucent lesion with well-defined sclerotic margins of approximately 3 × 2 cm in the mandible. The histopathological diagnosis revealed it was an OF. Lower Right Canine and Lower Right First Premolar teeth were endodontically treated for later surgical removal of the tumor. There was no history of trauma in the region.CONCLUSION: OF is a benign fibro-osseous tumor of the craniofacial bones commonly involving the jaws, especially the mandible. The diagnosis is based on a combination of clinical, radiographic and histologic criteria. Conservative treatment is an effective option that reduces morbidity and simplifies postoperative rehabilitation. We emphasize the importance of properly documenting individual cases and interpreting postoperative radiographic images along with the patients in order to avoid possible confusion between areas of bone repair and of potentially suspicious lesions in the future. OBJETIVO : Este artigo apresenta um caso de fibroma ossificante (OF), incluindo seu diagnóstico, tratamento e acompanhamento clínico e radiográfico de 15 meses, bem como uma revisão da literatura sobre esta patologia.RELATO DO CASO: Uma radiografia panorâmica de rotina de uma mulher branca de 27 anos, sistemicamente saudável, revelou uma lesão radiolúcida com margens escleróticas bem definidas de aproximadamente 3 × 2 cm na mandíbula. O diagnóstico histopatológico revelou que era um FO. O Canino Inferior Direito e o Pré-Molar Inferior Direito foram tratados endodonticamente para posterior remoção cirúrgica do tumor. Não havia história de trauma na região.CONCLUSÃO: O FO é um tumor fibro-ósseo benigno dos ossos craniofaciais que comumente envolve os ossos maxilares, especialmente a mandíbula. O diagnóstico é baseado em uma combinação de critérios clínicos, radiográficos e histológicos. O tratamento conservador é uma opção eficaz que reduz a morbidade e simplifica a reabilitação pós-operatória. Ressaltamos a importância de uma adequada documentação dos casos, bem como do esclarecimento sobre a evolução das imagens radiográficas pós-operatórias junto aos pacientes, de modo a evitar possíveis confusões entre as áreas de reparo ósseo e suspeitas de novas lesões no futuro.

ResumoObjetivoO objetivo desse estudo foi conhecer o perfil epidemiológico dos pacientes diagnosticados com quistos odontogénicos (QO) numa universidade particular de odontologia, num período de 8 anos.MétodosUm estudo retrospectivo foi realizado através da análise dos registros dos diagnósticos histopatológicos. Foram pesquisados os seguintes dados: género, idade, localização anatómica, tipo de quisto e história pregressa.ResultadosForam encontrados 158 casos, com predomínio de pacientes do género feminino (50,6%) com idade até 60 anos (88,6%) e história pregressa ao diagnóstico igual ou maior que 6 meses (17,7%). Com relação à localização anatómica, a maxila foi a região mais prevalente (47,5%). Foram encontrados 58 (36,7%) QO de desenvolvimento e 100 (63,3%) QO inflamatórios. Os QO mais prevalentes foram o quisto periodontal apical (57%) e o quisto dentígero (34,8%). Os resultados destacam uma maior prevalência de QO de etiologia inflamatória na maxila e em pacientes do género feminino com 60 anos ou menos.ConclusãoO perfil epidemiológico dos QO observado na presente investigação foi semelhante ao encontrado em outras populações, havendo discreto predomínio do género feminino e diferença em relação à localização anatómica.AbstractObjectiveThis study aimed to investigate the epidemiological profile of diagnosed odontogenic cysts (OCs) in patients of a private dentistry university over an 8‐year period.MethodsWe retrospectively analyzed the following characteristics from the histopathological records of patients: gender, age, anatomic location, type of cyst, and patient history.ResultsWe identified a total of 158 cases, showing a female predominance (50.6%), largely affecting patients younger than 60 years (88.6%), and with a lesion evolution time of ≥6 months (17.7%). The maxilla was the most prevalent anatomic location for OCs (47.5%). Further, of the 158 OC cases, 58 (36.7%) were developmental OCs and 100 (63.3%) were inflammatory OCs. The most prevalent types of OCs were apical periodontal cysts (57%) and dentigerous cysts (34.8%). Thus, our results have highlighted a higher prevalence of inflammatory OCs occurring in the maxilla, predominantly affecting female patients younger than 60 years.ConclusionThe epidemiological profile of OCs observed in this study was similar to that observed in different populations, with a discrete predominance of females and discrepancies regarding anatomic location.

To assess the immunocytochemical and immunohistochemical correlation of adhesion (E-cadherin) and cell differentiation (involucrin) molecules in oral leukoplakia and oral squamous cell carcinoma. Cytological samples and biopsies were obtained from male and female patients aged over 30 years with oral leukoplakia (n = 30) and oral squamous cell carcinoma (n = 22). Cell scrapings and the biopsy were performed at the site of the lesion and histological slides were prepared for the immunocytochemical analysis of exfoliated oral mucosal cells and for the immunohistochemical analysis of biopsy tissues using E-cadherin and involucrin. Spearman’s correlation and kappa coefficients were used to assess the correlation and level of agreement between the techniques. Immunostaining for E-cadherin and involucrin by both techniques was similar in the superficial layers of the histological sections compared with cell scrapings. However, there was no statistical correlation and agreement regarding the immunocytochemical and immunohistochemical expression of E-cadherin and involucrin in oral leukoplakia (R = 0.01, p = 0.958) (Kappa = 0.017, p = 0.92) or in oral squamous cell carcinoma (R = 0.26, p = 0.206) (Kappa = 0.36, p = 0.07). The immunoexpression of E-cadherin and involucrin in tissues is consistent with the expression patterns observed in exfoliated oral mucosal cells, despite the lack of a statistically significant correlation. There is an association of the histopathological characteristics of leukoplakia with the expression E-cadherin and of the microscopic aspects of oral squamous cell carcinoma with immunohistochemical expression of involucrin.

Cervicofacial actinomycosis (CA) is a rare infection caused by Actinomyces spp. and it is infrequently found in the maxilla. The factors associated with the host in the evolution of CA have not been clarified in the literature, and the relationship between actinomycosis and immunodepletion remains surrounded by controversy. CA diagnosis is not usually considered in maxilla injuries, and the potential aggressiveness of these lesions makes it more difficult to reach early diagnosis. This article reports a case of a 31 year old man with a history of drug abuse. The patient was HIV-positive and developed an extensive, destructive lesion in the maxilla. The diagnosis of CA was based on imaging examination and partial biopsy. The patient was treated with crystalline IV penicillin G and amoxicillin orally and preceded under follow-up. CA should be considered in the differential diagnosis of lesions in the maxilla, and health professionals have to pay particular attention when other systemic factors such as HIV infection and drugs abuse are present. CA should be included in the differential diagnosis of maxillary lesions, and healthcare professionals have to dedicate special attention to patients with systemic conditions such as HIV infection and drug abuse, which may intensify the development of more aggressive CA forms.

AIM: The etiology and epidemiology of maxillofacial injuries varies widely in different regions of the world due to socioeconomic status, cultural aspects in addition to road traffic and drug consumption. The aim of this study is to determine major causes and epidemiological characteristics of maxillofacial trauma in a 5-year period. MATERIALS AND METHODS: Reports of corporal trauma (n = 25,632) from 2007 to 2011 in the Department of Forensic Medicine were analyzed as to the presence of maxillofacial injuries. Data were submitted to Chi square test and to multivariate Poisson regression. RESULTS: 3262 reports referred maxillofacial trauma. The majority were men (55.8%), single (68.9%), most of them white (75.7%). The average age was 28.9 years (SD = 8.42), and victims with age between 16 and 30 years old were the most affected (48.0%). Women comprised 44% of total sample, 67.8% (971) were single, 76% (1.076) white and 46% (691) aged between 16 and 30 years old. Middle third injuries were associated after adjustment with females (PR 1.05; 95% CI 1.01–1.11), non-white subjects (PR 1.06; 95% CI 1.01–1.12) and physical aggression (PR 1.07; 95% CI 1.02–1.13). Injuries in the oral region was more prevalent in men (PR 1.24; 95% CI 1.09–1.41), in those aged between 16 and 30 (PR 1.97; 95% CI 1.48–2.61) and in subjects with injuries caused by traffic accident (PR 1.21; 95% CI 1.02–1.44). The presence of injuries in the lower third of face remained associated in the final model only with traffic accident (PR 1.75; 95% CI 1.43–2.15). CONCLUSION: Health care practitioners must recognize vulnerable population and most prevalent sites of lesion to identify cases of violence.

Objective To assess the prevalence of epidermoid carcinoma cases, georeference their locations, and seek to correlate them to areas with higher agricultural production and use of pesticides. Methods This was a cross-sectional, descriptive and retrospective study conducted by searching the histopathological reports with confirmed diagnosis of epidermoid carcinoma of the mouth issued from August 2005 to December 2011 by the public oral pathology laboratory of the State of Mato Grosso, Brazil. Data were plotted with the IBM SPSS Statistics software and spatial analysis used the ArcGIS 10.1 software. Results There were a higher number of diagnoses in 2007 (26.6%); 99 cases (75%) of patients were male; 96 cases (72.8%) were patients aged between 41 and 70 years; and the cities of Cuiabá, Várzea Grande and Rondonópolis respectively showed the highest number of occurrences. Conclusion No relationship was found between the occurrence of cases of epidermoid carcinoma and the municipalities with high agricultural production and pesticide use. RESUMO Objetivo Analisar a prevalência dos casos de carcinoma epidermóide e fazer seu georreferenciamento, buscando correlaciona-los às áreas de maior produção agrícola e utilização de agrotóxicos. Métodos Trata-se de um estudo transversal, descritivo e retrospectivo realizado por meio de pesquisa dos laudos histopatológicos com diagnóstico confirmado de carcinoma epidermóide de boca emitidos de agosto de 2005 a dezembro de 2011 pelo serviço de patologia bucal do laboratório público do Estado de Mato Grosso. Os dados foram tabulados com o software IBM SPSS Statistics e a análise espacial utilizou o software ArcGIS 10.1. Resultados Houve um maior número de diagnósticos no ano de 2007 (26,6%), 99 casos (75%) eram de pacientes do sexo masculino; 96 casos (72,8%) eram de pacientes na faixa etária compreendida entre 41 e 70 anos e os municípios de Cuiabá, Várzea Grande e Rondonópolis apresentam respectivamente o maior número de ocorrências. Conclusão Não foi encontrada relação entre a ocorrência dos casos de carcinoma epidermóide de boca e os municípios com grande produção agrícola e elevado uso de agrotóxicos.

Purpose: The present article describes the clinical, radiographic and histological features of an extremely rare case of lateral periodontal cyst as well as the treatment and postoperative follow-up. Case description: A 43-year-old male patient, mulatto, sought the Stomatology Clinic of the Caruaru Dental School (Brazil) with a complaint of increased volume on the right side of his face for approximately five years. The increase in volume was asymptomatic and facial asymmetry was the main reason he sought treatment. Based on the clinical and imaging exams as well as the lack of pulp vitality in tooth 13, the presumed clinical diagnosis was inflammatory radicular cyst and the surgical plan was total cystic enucleation. Excisional biopsy was performed and sent for pathologic microscopic analysis, which revealed multiple cavities with hyalinization of the connective tissue below the epithelium and the presence of clear non-epithelial cells. Conclusion: The case reported reveals that the extremely rare botryoid variant of lateral periodontal cyst exhibits multilocular presentation and excessive growth. Excisional biopsy was the treatment for this case and no sign of recurrence was observed over the nine-month follow-up period. Objetivo: O presente artigo descreve os aspectos clínicos, radiológicos e histopatológicos de um caso extremamente raro de cisto periodontal lateral, bem como o tratamento e acompanhamento pós-cirúrgico. Descrição do caso: Um paciente masculino de 43 anos de idade, mulato, procurou a Clínica de Estomatologia da Faculdade de Odontologia de Caruaru (Brasil) queixando-se de grande aumento de volume no lado direito de seu rosto de aproximadamente cinco anos. Tal aumento de volume era assintomático e a assimetria facial foi o principal motivo pelo qual procurou o tratamento. Com base nos exames clínicos e radiográficos, bem como a falta de vitalidade pulpar no elemento 13, a hipótese diagnóstica foi de cisto radicular inflamatório e o plano cirúrgico envolveu a competa enucleação cística. A biópsia excisional foi realizada e enviada para análise histopatológica, a qual revelou múltiplas cavidades com hialinização do tecido conjuntivo abaixo do epitélio e presença de células não-epiteliais. Conclusão: O caso relatado revelou que a extremamente rara variação botrióide do cisto periodontal lateral possui aparência multilocular e crescimento excessivo. A biópsia excisional foi o tratamento para esse caso e nenhum sinal de recorrência foi observado durante o acompanhamento de nove meses.

Sialocele is a subcutaneous cavity containing saliva, caused by trauma or infection in the parotid gland parenchyma, laceration of the parotid duct or ductal stenosis with subsequent dilatation. It is characterized by an asymptomatic soft and mobile swelling on the parotid region. Imaging studies are useful and help establishing the diagnosis, such as sialography, ultrasonography, computed tomography and magnetic resonance imaging. This paper describes a recurrent case of a parotid sialocele in a young female patient. She presented a 6 cm x 5 cm swelling on the left parotid region. The ultrasonographic scan of the area revealed a hypoechoic ovoid well defined image suggesting a cyst. A sialography of the left parotid showed a cavitary sialectasia in a panoramic and anteroposterior view. A conservative management was adopted by percutaneous needle aspiration of the swelling, which was useful to provide material for analysis and helped healing. Dentists should be aware of this pathology and the importance in adopting a conservative treatment whenever it is possible.

ResumoDescoberto geralmente de forma acidental, em avaliações de rotina, o cisto ósseo traumático, também chamado de cisto ósseo hemorrágico, cisto ósseo solitário ou cisto unicameral, não ocorre frequentemente. É centro de muitas discussões uma vez que apresenta etiologia desconhecida, podendo ser confundido com cisto dentígero, ceratocisto odontogénico, tumor odontogénico adenomatóide, ameloblastoma, granuloma central de células gigantes ou ainda, estar associado com outra patologia como púrpura trombocitopénica. O tratamento cirúrgico consiste na curetagem da cavidade e do seu conteúdo, podendo ser de característica gasosa, líquida ou semi-sólida. O presente artigo traz um levantamento bibliográfico, bem como o relato de um caso clínico-cirúrgico.AbstractDiscovered in a generally accidental, routine assessment of the traumatic bone cyst, also called hemorrhagic cyst bone, bone cyst lonely, or unicameral cyst, occurs with little frequency. It is center of much discussion since it presents unknown etiology. It can be confused with dentigerous cyst, odontogenic keratocyst, adenomatoid odontogenic tumors, ameloblastoma, granuloma central giant cell or, to be associated with another condition as trombocytopenic purple. The surgery is the curettage of the cavity and its contents, which may be characteristic of gas, liquid or semi-solid. This article brings a bibliographic survey and the report of a surgical clinic case.

Objective: To assess oral health status and its relationship with stomatological complications, oral mucositis (OM) and odontogenic infections (OI), after dental work in pediatric patients with cancer. Design: Prospective cohort study and analysis of oral health conditions with a 6-month follow-up. Setting: Dental Unit, Stomatology Unit and Oncology Service, Hospital de Clínicas de Porto Alegre (HCPA), Federal University of Rio Grande do Sul, Brazil. Participants: Sixty-five pediatric patients with cancer, 36 boys and 29 girls; aged 6 months to 18 years; 839 dental evaluations during oncologic treatment and comparisons between two patients groups - inappropriate oral health and appropriate oral health – considering the risk for stomatological complications manifesting as odontogenic infections and mucositis related to chemotherapy, gender, age group, head and neck radiation therapy and time to onset of complications Results: Thirty-three patients had inappropriate oral health. The group with appropriate oral health had lower dental infection rates (P = 0.003), and mucositis emerged later, compared with the inappropriate health group (P < 0.001). The data suggest that the group with inappropriate oral health had increased odontogenic infection rates, independently of the chemotherapeutic protocol adopted, when mucositis emerged earlier (when the patient presented hyperemic mucosa and burning sensation). Patients with deciduous teeth (6 months a 6 years) had lower incidence of mucositis (69.2%). Males had higher incidence of mucositis (80.6%). In the permanent dentition range (13-18 years) there was a higher incidence of dental infections (41.2%). Girls were more often associated with dental infections (41.4%). Conclusion: Studies on dental work in pediatric cancer patients are very limited. Our research contributed to clarify patient profile and needs. The results will be fundamental for the development of protocols and service strategies directed to this population. Educational measures alone do not reduce the risk of odontogenic infections.

The present study aimed to assess the rate of agreement between clinical and histopathological diagnoses and to report the frequency of completed forms for specimens that were subjected to histopathological examination and retrospectively examined. Data from 8,168 specimens submitted to histopathological examination were retrieved from the records. A total of 5,368 cases were included. Agreement was defined based on the definition of lesion nature according to its diagnostic category. Sensitivity, specificity, and positive and negative predictive values were calculated for each diagnostic category. The highest rate of agreement was observed for periapical lesions (92.6%), followed by potentially malignant disorders (90.1%) and non-neoplastic proliferative disorders (89.3%). Low rates of histopathological confirmation of the clinical impression were observed for mesenchymal tumors (25.0%) and cysts (44.2%). Sensitivity values were > 0.70 for all lesions, except for cysts (0.51). Specificity was relatively high, ranging from 0.97 to 1.00. The frequency of incomplete biopsy forms ranged from 16.8% (malignant tumors of oral mucosal epithelium) to 51.0% (nonspecific inflammatory reaction). The most frequently completed biopsy forms corresponded to epithelial malignant tumors (83.2%) and glandular inflammation (72.3%). In conclusion, there was an acceptable level of agreement. The low level of completeness of biopsy forms indicates little awareness about the relevance of gathering detailed information during clinical examination.

Salivary gland tumors (SGT) are rare neoplasms that generate interest due to their histopathological diversity and clinical behavior. The aims of the present study were to investigate clinicopathological aspects of SGTs diagnosed at a tertiary health center and compare the findings with epidemiological data from different geographic locations. Cases of tumor in the head and neck region at a single health center in the period between 1995 and 2010 were reviewed. Patient gender, age and ethnic group as well as anatomic location, histological type and clinical behavior of the tumor were recorded. Availability of complete information about these aspects was considered the inclusion criteria. Descriptive statistical analysis of the data was performed using the frequencies of categorical variables. Among the 2168 cases of tumors in the head and neck region, 243 (11.20%) cases were diagnosed in the salivary glands, 109 of which met the inclusion criteria: 85 (78%) benign tumors and 24 (22%) malignant tumors. Mean patient age was 46.47 years. The female gender accounted for 56 cases (51.4%) and the male gender accounted for 53 (48.3%). The major salivary glands were affected more (75.2%) than the minor glands. The most frequent benign and malignant SGTs were pleomorphic adenoma (81.2%) and adenoid cystic carcinoma (58.3%), respectively. In conclusion, pleomorphic adenoma and adenoid cystic carcinoma are the most frequent benign and malignant lesions, respectively. Comparing the present data with previous studies on SGTs, one may infer that some demographic characteristics and the predominance of malignant tumors vary in different geographic regions.

Dendritic cells (DCs) form a key link between innate and adaptive immune responses. The aim of this study is to analyze presence and distribution of immature (im) and mature (m) DCs in gingival tissue samples obtained from patients diagnosed with aggressive periodontitis (AgP), chronic periodontitis (CP), and clinically healthy periodontium (control group).Gingival tissue samples obtained from patients with: 1) AgP (aged35 years); 2) CP (aged ≥35 years); and 3) control group (aged18 years) (n = 10 per group) were collected. Two-way analysis of variance and posterior Fisher least significant difference test were used to observe differences between the means of cells positively marked for imDC (S100, CD1a, and CD207) and mDC (CD208) immunomarkers.imDCs were more numerous in AgP than CP and control groups, being statistically significant only for S100+ cells. Conversely, mDCs were visualized in higher numbers in CP than AgP and control groups (both P0.05). Considering frequency of immunostained cells, the number of S100+ cells was greater than CD207+ and CD1a+ cells, followed by a lesser number of CD208+ cells, in all groups.Considering that the ability of DCs to regulate immunity is dependent on DC maturation, results suggest that predominance of imDCs appears to be involved in AgP pathogenesis, probably due to lack of ability to induce immune cell activation. Further studies are necessary to elucidate the role of DC maturation in regulating immune responses in periodontal disease.

Oral verruciform xanthoma (OVX) is an uncommon lesion that appears on the oral mucosa. The aim of this paper was to discuss the probable etiopathogenesis of OVX in the hard palate, reinforcing the importance of including this lesion in the differential diagnosis of verrucous lesions. A 43-year-old male smoker presented with a painless lesion with a verrucous surface and erythematous spots on the hard palate. Excisional biopsy revealed oral mucosa consisting of hyperkeratosis, acanthosis, and elongated rete pegs. Subjacent connective tissue showed numerous foam cells with clear cytoplasm and pyknotic nucleus, negative on periodic acid-Schiff staining. Immunohistochemical analysis revealed foam cells positive for anti-CD68 antibody, while anti-KI-67 antibody was restricted to the basal layer of the oral epithelium. A final diagnosis of OVX was established. The patient showed no signs of recurrence after seven months of follow-up. Physical trauma and smoking habits can be directly related to the etiology of verruciform xanthoma because the lesion is chronic and inflammatory with slow growth, and sites if high trauma are more often affected by such a lesion. The hard palate is the second most commonly affected site, and local trauma caused by smoking can be a cause of this type of lesion.

The aim of this study was to assess the accuracy of clinical diagnosis for lip lesions based on sensitivity and specificity. The retrospective analysis focused on the detection of lesions caused by potentially malignant disorders (PMDs) and malignant lesions (n = 1195). All cases were classified as benign, PMD, and malignant lesions. Concordance between diagnoses based on clinical examination and those based on histopathological analysis was assessed, and accuracy for the identification of PMD and malignant lesions was calculated. Histopathological analysis revealed 44 lesion types; PMD and malignant lesions comprised 8.3% of all cases. Compared with histopathological analysis, clinical examination showed 97.4% accuracy for the identification of non-malignant and potentially malignant/malignant cases. Degrees of specific sensitivity ranged from 34% to 77% for different lesions, and were highest for autoimmune (77%) and reactive (72%) lesions. Positive and negative predictive values for the identification of PMD and malignant lesions were 81.9% and 98.9%, respectively. Clinical examination showed a high degree of accuracy for the detection of PMD and malignant lip lesions, indicating good reliability.

Podoplanin and ezrin connection through Rho-A phosphorylation have been suggested as part of the activation pathway, in the process of tumor invasion and cell movement in oral squamous cell carcinomas. The aim of this study was to evaluate the correlation among podoplanin, ezrin, and Rho-A immunoexpressions in 91 squamous cells carcinomas of the lower lip and their influence in patient’s prognosis. The immunoexpressions of podoplanin, ezrin, and Rho-A were evaluated through a semi-quantitative score method, based on the capture of 10 microscopic fields at the front of tumor invasion. The association and correlation of these proteins with the clinicopathological features were verified by Fischer’s exact test and Spearman’s test. The prognostic values were analyzed by Kaplan-Meier method and log-rank test. A statistically significant association between strong cytoplasmic podoplanin expression and alcohol (p = 0.024), loco-regional recurrences (p = 0.028), and lymph node metastasis (pN+) (p = 0.010) was found. The membranous (p = 0.000 and r = 0.384) and cytoplasmic (p = 0.000 and r = 0.344) podoplanin expression was statistically correlated with ezrin expression. Also, membranous podoplanin was significantly correlated with Rho-A expression (p = 0.006 and r = 0.282). The expressions of podoplanin, ezrin, and Rho-A were not significant prognostic factors for patients with squamous cell carcinomas of the lower lip. Therefore, our results confirm a correlation among podoplanin, ezrin, and Rho-A expressions in squamous cell carcinoma of the lip suggesting a cooperative participation of these proteins in cell movement and invasion. Furthermore, strong cytoplasmic podoplanin expression could be helpful to identify patients with squamous cell carcinoma of the lip and lower risk of loco-regional recurrences.

The botryoid odontogenic cyst is a rare asymptomatic lesion characterized by its typical multilocular aspect similar to a bunch of grapes commonly affecting the mandible bicuspids and canine region. The possibility of this lesion representing a clinical variation of the lateral periodontal cyst is source of discussion and doubt among many authors. Our article reports a rare case of the polemic odontogenic cyst and presents considerations related to its etiopathogenic, diagnostic and therapeutic aspects. O cisto odontogênico botrióide é uma lesão rara caracterizada pelo aspecto multilocular típico semelhante a um cacho de uvas. A possibilidade de essa lesão representar uma variante clínica do cisto periodontal lateral é fonte de discussão e indefinição entre diversos autores. O presente trabalho relata um caso desse polêmico cisto odontogênico e apresenta considerações relacionadas aos seus aspectos etiopatogênicos, diagnósticos e terapêuticos.

The inflammatory hyperplasias are lesions extremely common in the oral cavity. Nevertheless, in the literature there are different characteristics observed during the histopathologic diagnostic of this lesion. The aim of this study was to describe the microscopic characteristics observed in the inflammatory hyperplasia diagnosticated in the Oral Pathology Lab of the Federal University of Rio Grande do Sul (UFRGS), and recommend histologicals criteria for the diagnostic of this pathology. Twenty-five cases of inflammatory hyperplasia archived in the Oral Pathology Lab of the UFRGS were examinated between january and july of 2001. We concluded that the presence of cronic inflammatory infiltrated, epitelial hyperplasia and the uniform direction of the collagen fibers were the histological caracteristics finding in the majority of the lesions.

Introduction: Oral squamous cell carcinoma (OSCC), which represents more than 90% of head and neck malignant neoplasms, has a poor prognosis due to its high frequency of lymph node metastasis and local invasion. Previous studies have investigated parameters related to the biological behavior of OSCC and its correlation with disease outcome (DO). Objective: To evaluate clinical and morphological data in cases of tongue squamous cell carcinoma (TSCC), correlating these findings with prognosis. Material and methods: Fifty-seven specimens of TSCC were obtained from patients undergoing surgical excision at a referral hospital in Natal, Brazil. Clinical data, such as tumor-node-metastasis (TNM) stage and DO, were collected from medical records. Hematoxylin and eosin-stained sections were analyzed regarding histological grade of malignancy (HGM), based on the system proposed by Bryne (1998) Results: The majority of patients (38.6%) were diagnosed as TNM stage III, and 57.9% developed metastases. Remission of the tumor occurred in 77.2% of the cases. The parameter “metastasis” exhibited a significant association with DO (p = 0) and TNM stage (p = 0.001), thus constituting a good indicator of tumor progression. Correlation of HGM and TNM stage with DO was not evidenced. Nevertheless, statistical analysis showed a significant association between HGM and TNM stage (p = 0.006). Conclusion: TNM clinical staging and HGM, evaluated in association, may be useful to estimate the prognosis of TSCC. Introdução: O carcinoma de células escamosas oral (CCEO) representa mais de 90% das neoplasias malignas de cabeça e pescoço. Possui prognóstico desfavorável devido à alta frequência de metástase linfonodal e invasão local. Estudos anteriores investigaram parâmetros relacionados com o comportamento biológico do CCEO e sua correlação com o desfecho clínico. Objetivo: Avaliar dados clínicos e morfológicos em casos de carcinoma de células escamosas de língua (CCEL), correlacionando esses achados com o prognóstico. Material e métodos: Cinquenta e sete espécimes de CCEL foram obtidos de pacientes submetidos à excisão cirúrgica em um hospital de referência de Natal, Brasil. Dados clínicos, a exemplo do estágio tumor-linfonodo-metástase (TNM) e do desfecho da doença (DD), foram coletados dos prontuários médicos. Secções histológicas coradas em hematoxilina e eosina foram analisadas quanto à gradação histológica de malignidade (GHM), com base no sistema de Bryne (1998). Resultados: A maioria dos pacientes (38,6%) foi diagnosticada no estágio TNM III, e 57,9% desenvolveram metástases. A remissão do tumor ocorreu em 77,2% dos casos. O parâmetro metástase apresentou associação significativa com o DD (p = 0) e o estágio TNM (p = 0,001), constituindo, dessa forma, um bom indicador de progressão da neoplasia. Não se observou correlação de GHM e estágio TNM com DD, contudo a análise estatística revelou associação significativa entre GHM e estágio TNM (p = 0,006). Conclusão: O estadiamento clínico TNM e a GHM, avaliados em conjunto, podem ser úteis para determinar o prognóstico do CCEL.

Verruciform xanthoma represents a rare disease of unknown etiology that typically affects the oral mucosa. In most cases, it is asymptomatic and characterized by a well demarcated elevation with irregular or papillary surface. The gums are the most affected oral site, and the lesion is usually less than 2 cm in diameter. The aim of this study is to report a rare case of an extensive verruciform xanthoma in the hard palate and gums of a 55-year-old male. Clinically, the well delimitated, reddish, elastic lesion measuring approximately 2.8 X 1.5 cm had a slightly granular surface. Histopathological analysis found proliferative epithelial activity with evident papillomatosis, parakeratosis, and large epithelial ridges. Xanthomatous cells with clear, vacuolated cytoplasm were detected in the lamina propria. The patient has been disease free for 18 months. According to an extensive research in the PubMed, MEDLINE, and Lilacs databases, this paper describes one of the most extensive cases of verruciform xanthoma reported in the literature. O xantoma verruciforme é uma lesão rara de etiologia não determinada, que acomete, principalmente, a mucosa oral. Na maioria das vezes é assintomática, caracterizando-se por uma elevação bem-delimitada, de superfície rugosa ou papilar, e sua principal localização na boca é a gengiva. Com relação ao tamanho, a maioria das lesões não ultrapassa 2 cm no seu maior diâmetro. O objetivo do presente trabalho é relatar um caso raro de extenso xantoma verruciforme em paciente do gênero masculino, de 55 anos, apresentando lesão localizada no palato duro e na gengiva palatina. Clinicamente, a lesão possuía bordas bem-destacadas, de superfície levemente granular e coloração avermelhada, com consistência elástica, medindo aproximadamente 2,8 X 1,5cm. O exame histopatológico demonstrou a presença de proliferação do epitélio de revestimento, exibindo papilomatose, paraceratose e alongamento das cristas epiteliais. Na lâmina própria, células xantomatosas com citoplasma claro e vacuolado puderam ser observadas. O paciente encontra-se livre de recidivas após 18 meses de acompanhamento clínico. De acordo com extensa pesquisa em bases de dados (pubmed, medline, lilacs), o presente caso representa uma das maiores lesões do tipo xantoma verruciforme já relatadas na literatura.

The aim of this work was to identify risk genes related to the development and progression of squamous cell carcinoma head and neck (SCCHN) and do a meta-analysis of available estimates. Eligible gene/polymorphism studies were identified by electronic searches. Individual participant data of 8540 patients with HNC and 9844 controls from 19 genetic studies were analyzed, yielding adjusted (tobacco, gender, age and alcohol) odds ratios (OR) and 95% confidence intervals (CIs) comparing cases with controls. A meta-analysis was done on the studies that applied fixed and random models. People have an increase of polymorphism expression related to inflammation (NFKB1-294-ATTG, TNFα308-A2A2/A2A1, and TNFβ252- B2B2/B2B1) or carcinogenic metabolism (GSTM1 null, and CYP1A1 m1/m1), representative of malignancy development. Furthermore, the increased expression of genes associated with the stabilization and repair of the cellular (OGG1-Asp267Asn, Ser279Gly Ile253Phe, 1578A > T, 1582C > T Ala399Glu (1542C > A) 1582insG 1543_1544delCT), and genes associated with the regulation of proliferation, apoptosis or tumor survival (miRNA499-CT/CC, CRYABC802G-CG/GG) are considered as risk factors. In this scheme, only the polymorphisms of ADH7A92G-GG and DEC1606-T/C genes are protective against malignancy transformation. The TP53, GSTM1 and CYPA1genes have been evaluated in more than one study and analyzed for homogeneity in each genotype. The meta-analysis showed no significant association between different allelic variants of Arg72Pro rs1042522 and SCCHN risk. In a model of tumorigenesis, an increased risk of SCCHN is associated with DNA repair and DNA stabilization genes. This investigation was carried out in order to determine the prevalence of oral alterations among 200 infants aged 0 to 24 months (108 males and 92 females), who were seen at public pediatric dental clinics from Ponta Grossa - PR, Brazil. The infants were examined during routine appointments. The data were analyzed and the results revealed that 21.00% of the children had oral conditions, which were more frequent among children aged 0 to 3 months (26.98%). The most prevalent condition was the inclusion cyst (35.71%), followed by benign migratory glossitis (23.81%) and candidiasis (11.90%). The prevalence of inclusion cysts was higher among females (10.87%), and the benign migratory glossitis was more frequently seen in males (6.48%). No tumors were found. With regard to the management of the conditions, it was noticed that no treatment was required in 76.19% of the cases. These findings are in agreement with those reported in the literature, and it was concluded that most of the oral conditions in infants are benign and do not require any treatment. In spite of that, health professionals (dentists and pediatricians) must be aware of those alterations in order to tranquilize the children's parents and to detect the need for any intervention. Com o objetivo de contribuir com dados sobre a prevalência de alterações em cavidade bucal de bebês, foi realizado um estudo transversal envolvendo crianças entre 0 e 24 meses de idade, atendidas nas clínicas de bebês públicas do município de Ponta Grossa - PR. Foram examinados 200 bebês nessa faixa etária - 108 do sexo masculino e 92 do feminino - e os resultados obtidos foram: a) 21,00% das crianças apresentaram alteração de mucosa; b) a faixa etária na qual as alterações foram mais freqüentes foi a dos 0 a 3 meses (26,98%); c) considerando-se apenas as crianças afetadas, as alterações mais prevalentes foram os cistos de inclusão (35,71%), seguidos de língua geográfica (23,81%) e candidíase (11,90%). Os cistos de inclusão foram mais freqüentes no sexo feminino (10,87%) e a língua geográfica foi mais prevalente no sexo masculino (6,48%). Não foram encontrados tumores. O estudo revelou, ainda, que não foi necessário tratamento em 76,19% dos casos. Concluindo, os resultados indicam que a maioria das alterações bucais nessa faixa etária são benignas e não requerem nenhum tipo de tratamento específico. Mesmo assim, faz-se importante que os profissionais que atuam com crianças conheçam tais alterações, no sentido de tranqüilizar os pais e de estarem atentos para a necessidade de alguma intervenção.

Inverted Schneiderian papilloma is an uncommon benign tumor that presents tendency to recur and propensity to be associated with malignancy in approximately 10% of the cases. Some of these lesions are isolated in the maxillary sinus, and predominantly affect white males with mean age of 50 years. We report a case of squamous cell carcinoma arising from inverted Schneiderian papilloma in the maxillary sinus extending to the mouth. The patient was submitted to extraction of a maxillary molar tooth four months before the exacerbation of the symptoms of nasal airway obstruction and facial enlargement. Computed tomography scan revealed a sinonasal mass causing opacification of the right maxillary sinus with destruction of the lateral nasal wall and maxillary sinus floor. The patient was referred to an oncology center for treatment and died from tumor progression one year after the cancer was diagnosed. The intention of this report is to alert dentists to include the inverted Schneiderian papilloma, either associated with squamous cell carcinoma, or not, in the differential diagnosis of maxillary sinus tumors with aggressive behavior, which may extend to the oral cavity or involve roots of teeth.

Primary intraosseous carcinoma of the jaws (PIOSCC) might arise from odontogenic epithelium, more commonly from a previous odontogenic cyst. The aim of this case is to illustrate that the clinician should consider that an apparent benign dentigerous cyst can suffer malignant transformation and that all material removed from a patient must be evaluated histologically. A 44-year-old man presented in a routine periapical X-ray an impacted lower left third molar with radiolucency over its crown. Ten years later, the patient complained of pain in the same region and the tooth was extracted. After one month, the patient still complained of pain and suffered a fracture of the mandible. A biopsy was performed and carcinoma was diagnosed. The patient was treated surgically with adjuvant radio- and chemotherapy and after 8 years, he is well without signs of recurrences. This report describes a central mandibular carcinoma probably developed from a previous dentigerous cyst.

O termo hiperplasia está relacionado a proliferação do tecido conjuntivo que pode ser ocasionada por diversos fatores etiológicos. Dentre as varias hiperplasias ocasionadas por esses diferentes fatores, enfatizaremos aquelas causadas pelo uso de próteses dentárias mal adaptadas. A hiperplasia inflamatória caracteriza-se pela proliferação da mucosa bucal, devido a um aumento no ritmo de divisão celular com manutenção do seu padrão morfofuncional. A arteriosclerose é uma doença crônico-degenerativa que ocasiona um estado de inextensibilidade e enfraquecimento dos vasos pela presença de lipídios na túnica íntima. Ao considerarmos que o uso de próteses, assim como a arterioesclerose é comum em indivíduos acima dos 35-40 anos, buscou-se verificar a prevalência dessa patologia dos vasos sanguíneos em casos de hiperplasia inflamatória, visando a utilização dessas biópsias como um recurso adicional de diagnóstico da arteriosclerose. Para isso, foi realizado um levantamento dos casos de hiperplasia inflamatória da mucosa bucal, registrados no Laboratório de Patologia da FO-UFRGS nos últimos 5 anos. A partir da observação microscópica do material corado pela hematoxilina-eosina, verificou-se que dos 166 (100%) casos registrados, 75 (45,2%) apresentavam, além de capilares, artérias e/ou veias. Dessas 75 (100%) biópsias, 35 (46,6%) apresentavam sinais morfológicos de arteriosclerose evidenciados pela visualização de células vacuolizadas na parede interna dos vasos. Sendo assim, mediante a análise dos resultados obtidos sobre a prevalência de arteriosclerose em casos de hiperplasia inflamatória, verifica-se a possibilidade da utilização de biópsias de hiperplasia inflamatória provocadas por prótese mal adaptadas como um fator auxiliar no diagnóstico da arteriosclerose. The word hiperplasia is related to the cellular proliferation that can be caused by many ethiological factors. Considering these different types of hiperplasias, we will emphasize those that were caused by the use of non-adapted dental prosthesis. The inflammatory hiperplasia is characterized by the oral mucosa proliferation, due to the increase of the rythm of the cellular division with the mantainance of the morphofunctional pattern of these cells. The arteriosclerosis is a disease characterized by the inextensibility and weakness of vessels by the presence of lipid in the intima1 tunic. Considering that arteriosclerosis and the use of prosthesis are common findings in people over 35-40 years old, we tried to study the prevalence of this pathology in the blood vessels of hiperplasia biopsies. The correlation between arteriosclerosis and inflammatory hiperplasia was done using inflammatory hiperplasia biopsies as an additional source for arteriosclerosis diagnostic. After a checking inflammatory hiperplasia cases registered in the last 5 year at Pathology Laboratory of FO-UFRGS, it was done a microscopycal observation of the colored material by hematoxiline-eosine. As a result, we found that from 166 (100%) of the registered cases, 75 (45,2%) showed arteries and/or veins, beyond capillaries. From these 75 (100%) biopsies, 35 (46,6%) showed arteriosclerosis morphological signs. These signs were evidenced by the visualization of vacuolized cells in the inmost vessels wall. Our findings suggest that inflammatory hiperplasia biopsies are diagnostic tools to arteriosclerosis.

Rhabdomyomatous hamartoma is a rare disease that occurs predominantly in the skin. This paper describes a congenital lesion in a 17-year-old male, who came to our clinic presenting a circumscribed swelling involving the oral mucosa and vermillion border of the upper lip, purplish in color, and blanching under pressure. The patient reported that he had had lesion since his birth. A clinical diagnosis was of congenital haemangioma, and the patient was treated by photocoagulation using diode laser. When the lesion became smaller, by having its blood content reduced, the upper portion of the lesion was sliced off with CO2laser and the tissue was sent for microscopic analysis. Histopathological examination showed an oral mucosa fragment with proliferation of striated muscle bundles admixed with small blood vessels, collagen, and nerve fibres. A supplementary analysis with immunohistochemistry demonstrated positivity for desmin, HHF35, smooth muscle actin, S-100, and CD34. Based on these findings, the lesion was diagnosed as rhabdomyomatous hamartoma. The aesthetic result has been very satisfactory after a 14-month followup.
