To determine the effect of encounter methods on patient experience, we evaluated patient experience survey data comparing scores between telehealth and in-person visits and pre-COVID-19 and COVID-19 time periods. Pediatric subspecialty visits were either in-person or via telehealth and received the same 16-question patient experience survey. Top box (5/5) scores were compared between in-person and telehealth visits for pre-COVID-19 and COVID-19 periods as well as between periods for in-person and telehealth visits. In addition, for both time periods and encounter methods, correlation analysis was performed to evaluate best correlation between likelihood to recommend practice and the 15 other survey questions. Comparing the COVID-19 period data, there was a statistically significant difference in the top box likelihood to recommend practice score comparing in-person to telehealth numbers (81.01% vs 87.13%, p = 0.0003). Comparing pre-COVID-19 with COVID-19, this was not true for in-person scores (79.97% vs 81.01%, p = 0.4060) or telehealth scores (82.50% vs 87.13%, p = 0.2084). The question with the highest correlation coefficient to likelihood to recommend practice was how well staff worked together in both time periods and visit methods. We conclude that Likelihood to recommend experience scores were statistically significantly higher for telehealth as compared to in-person pediatric subspecialty ambulatory visits during the COVID-19 pandemic. There were no such differences in likelihood to recommend comparing pre- vs COVID-19 time periods for either in-person or telehealth visits so the change in scoring seems to be related to the mode of care delivery. Introduction: To evaluate the correlation between the presence of an independent EHR (compared to a shared EHR system within an adult hospital system) and an externally-derived third party ranking of children’s hospitals. Methods: Children’s hospitals that ranked in the top fifty of the 2019-2020 US News and World Report (USNWR) were included in the analysis. The mean and median ranking of children’s hospitals with independent versus a shared EHR was evaluated. The 2019-2020 USNWR rankings of the top twenty adult hospitals in the United States were then evaluated. For each children’s hospital with an associated adult hospital that was both ranked, it was noted as to whether the EHR for the children’s hospital was independent or shared and statistical differences in rankings compared. Results: Among the top 50 children’s hospitals included, the median USNWR ranking for hospitals was statistically different with an independent EHR than with a shared EHR (13 vs. 30.0) (p = 0.002). The 21 top ranked adult hospitals were associated with 17 children’s hospitals ranked in the top 50. The median ranking for those with an independent EHR was statistically different for those with independent EHR versus shared EHR (7 vs. 28) (p = 0.002). Conclusion: Children’s hospitals with an independent EHR are associated with higher scores on an independent external ranking of hospital quality compared to those which share an EHR with a partner adult hospital.

Hospital-acquired pressure injuries (HAPIs) cause patient harm and increase health care costs. We sought to evaluate the performance of the Braden QD Scale-associated changes in HAPI incidence.Using electronic health records data from a quaternary children's hospital, we evaluated the association between Braden QD scores and patient risk of HAPI. We analyzed how this relationship changed during a hospitalwide quality HAPI reduction initiative.Of 23,532 unique patients, 108 (0.46%, 95% confidence interval [CI] = 0.38%-0.55%) experienced a HAPI. Every 1-point increase in the Braden QD score was associated with a 41% increase in the patient's odds of developing a HAPI (odds ratio [OR] = 1.41, 95% CI = 1.36-1.46, p0.001). HAPI incidence declined significantly following implementation of a HAPI-reduction initiative (β = -0.09, 95% CI = -0.11 - -0.07, p0.001), as did Braden QD positive predictive value (β = -0.29, 95% CI = -0.44 - -0.14, p0.001) and specificity (β = -0.28, 95% CI = -0.43 - -0.14, p0.001), while sensitivity (β = 0.93, 95% CI = 0.30-1.75, p = 0.01) and the concordance statistic (β = 0.18, 95% CI = 0.15-0.21, p0.001) increased significantly.Decreases in HAPI incidence following a quality improvement initiative were associated with (1) significant deterioration in threshold-dependent performance measures such as specificity and precision and (2) significant improvements in threshold-independent performance measures such as the concordance statistic. The performance of the Braden QD Scale is more stable as a tool that continuously measures risk than as a prediction tool.

Because of issues with the previous staging system, the International Neuroblastoma Risk Group Staging System (INRG-SS) was created in 2009. The INRG-SS is based on preoperative imaging, rather than surgical, staging and emphasizes Imaging-Defined Risk Factors as the determining factors between L1 and L2 stages. Like with the introduction of any new tool, based on the authors' experience, there has been a time-lag related to adoption of the INRG-SS staging system by radiologists. This pictorial essay offers a practical approach to learning and utilizing the INRG system, emphasizing use of the descriptive terms which determine the presence or absence of imaging-defined risk factors.

Common cause analysis of learning opportunities identified in a peer collaborative improvement process can gauge the potential risk to patients and opportunities to improve. To study rates of learning opportunities based on radiologist assignment, patient type and exam priority at an academic children’s hospital with 24/7 in-house attending coverage. Actively submitted peer collaborative improvement learning opportunities from July 2, 2016, to July 31, 2018, were identified. Learning opportunity rates (number of learning opportunities divided by number of exams in each category) were calculated based on the following variables: radiologist assignment at the time of dictation (daytime weekday, daytime weekend and holiday, evening, and night) patient type (inpatient, outpatient or emergency center) and exam priority (stat, urgent or routine). A statistical analysis of rate differences was performed using a chi-square test. Pairwise comparisons were made with Bonferroni method adjusted P-values. Introduction: Despite being a participating Solutions for Patient Safety (SPS) children’s hospital and having attempted implementation of the SPS hospital-acquired pressure injuries (HAPIs) prevention bundle, our hospital remained at a HAPI rate that was 3 times the mean for SPS participating children’s hospitals. This performance led to the launch of an enterprise-wide HAPI reduction initiative in our organization. The purpose of this article is to describe the improvement initiative, the key drivers, and the resulting decrease in the SPS-reportable HAPI rate. Methods: We designed a hospital-wide HAPI reduction initiative with actions grouped into 3 key driver areas: standardization, data transparency, and accountability. We paused all individual hospital unit-based HAPI reduction initiatives. We calculated the rate of SPS-reportable HAPIs per 1,000 patient days during both the pre- and postimplementation phases and compared mean rates using a 2-sided t test assuming unequal variances. Results: The mean SPS-reportable HAPI rate for the preimplementation phase was 0.3489, and the postimplementation phase was 0.0609. The difference in rates was statistically significant (P < 0.00032). This result equates to an 82.5% reduction in HAPI rate. Conclusions: Having an institutional pause and retooled initiative to reduce HAPI with key drivers in the areas of standardization, data transparency, and accountability had a statistically significant reduction in our organization’s SPS-reportable HAPI rate.

Incident reporting can be used to inform imaging departments about adverse events and near misses. To study incident reports submitted during a 5-year period at a large pediatric imaging system to evaluate which imaging modalities and other factors were associated with a greater rate of filed incident reports. All incident reports filed between 2013 and 2017 were reviewed and categorized by modality, patient type (inpatient, outpatient or emergency center) and use of sedation/anesthesia. The number of incident reports was compared to the number of imaging studies performed during that time period to calculate an incident report rate for each factor. Statistical analysis of whether there were differences in these rates between factors was performed. During the study period, there were 2,009 incident reports filed and 1,071,809 imaging studies performed for an incident report rate of 0.19%. The differences in rates by modality were statistically significant (P=0.0001). There was a greater rate of incident reports in interventional radiology (1.54%) (P=0.0001) and in magnetic resonance imaging (MRI) (0.62%) (P=0.001) as compared to other imaging modalities. There was a higher incident report rate for inpatients (0.34%) as compared to outpatient (0.1%) or emergency center (0.14%) (P=0.0001). There was a higher rate of incident reports for patients under sedation (1.27%) as compared to non-sedated (0.12%) (P=0.0001). Using incident report rates as a proxy for potential patient harm, the areas of our pediatric radiology service that are associated with the greatest potential for issues are interventional radiology, sedated patients, and inpatients. The areas associated with the least risk are ultrasound (US) and radiography. Safety improvement efforts should be focused on the high-risk areas.

In 1998, the American Board of Radiology introduced the B. Leonard Holman Research Pathway (HRP) to initial certification for trainees in diagnostic radiology (DR) and radiation oncology (RO) motivated to pursue research-oriented careers in academic DR and RO.The HRP Committee anticipated that there would be a relatively even distribution between DR and RO participants, but with 18 years of experience that has not been the case. This article focuses on the HRP and DR.

Introduction: According to the National Healthcare Safety Network (NHSN) definitions for Catheter-associated urinary tract infections (CAUTI) rates, determination of the number of urinary catheter days must occur by calculating the number of catheters in place “for each day of the month, at the same time of day” but does not define at what time of day this occurs. The purpose of this review was to determine if a data collection time of 11 am would yield a greater collection of urinary catheter days than that done at midnight. Methods: During a 20-month period, the number of urinary catheter days was calculated using once-a-day electronic measurements to identify a urinary catheter presence. We used data collected at 11 am and collected at midnight (our historic default) in comparing the calculated urinary catheter days and resultant CAUTI rates. Results: There were 7,548 patients who had a urinary tract catheter. The number of urinary catheter days captured using the 11 am collection time was 15,425, and using the midnight collection time was 10,234, resulting in a 50.7% increase. The CAUTI rate per 1,000 urinary catheter days calculated using the 11 am collection method was 0.58, and using the midnight collection method was 0.88, a reduced CAUTI rate of 33.6%. Conclusion: The data collection time can significantly impact the calculation of urinary catheter days and on calculated CAUTI rates. Variations in how healthcare systems define their denominator per current National Healthcare Safety Network policy may result in significant differences in reported rates.

Until recently scurvy has been viewed in developed countries as a disease of the past. More recently there have been reports of case series of children with scurvy who have had a delayed diagnosis after an extensive diagnostic workup that included imaging. Most of these children have had underlying neurologic conditions such as autism. To review the medical records of children diagnosed with vitamin C (ascorbic acid) deficiency based on serum ascorbic acid levels at a large pediatric health care system, to determine imaging findings and utility of imaging in management, and to identify at-risk pediatric populations. We retrospectively identified cases of vitamin C deficiency in children tested for serum ascorbic acid levels during the last 5 years. We used the criteria of normal ascorbic acid >23 μmol/L and included children with ascorbic acid levels

This study uses electronic health record (EHR) data to evaluate differences in central line–associated bloodstream infection (CLABSI) rates by how central line days are counted: once a day at a fixed time for all patients; a sampling-based approximation; or a validated electronic count.

To use a natural language processing and machine learning algorithm to evaluate inter-radiologist report variation and compare variation between radiologists using highly structured versus more free text reporting.28,615 radiology reports were analyzed for 4 metrics: verbosity, observational terms only, unwarranted negative findings, and repeated language in different sections. Radiology reports for two imaging examinations were analyzed and compared - one which was more templated (ultrasound - appendicitis) and one which relied on more free text (chest radiograph - single view). For each metric, the mean and standard deviation for defined outlier results for all dictations (individual and group mean) was calculated. The mean number of outlier metrics per reader per study was calculated and compared between radiologists and between the two report types. Wilcoxon rank test and paired Wilcoxon signed rank test were applied. The radiologists were also ranked based on the number of outlier metrics identified per study.There was great variability in radiologist dictation styles - outlier metrics per report varied greatly between radiologists with the maximum 10 times higher than the minimum score. Metric values were greater (P0.0001) on the standardized reports using free text than the more structured reports.The algorithm successfully evaluated metrics showing variability in reporting profiles particularly when there is free text. This variability can be an obstacle to providing effective communication and reliability of care.

Sleep-disordered breathing has a spectrum of severity that spans from snoring and partial airway collapse with increased upper airway resistance, to complete upper airway obstruction with obstructive sleep apnea during sleeping. While snoring occurs in up to 20% of children, obstructive sleep apnea affects approximately 1-5% of children. The obstruction that occurs in obstructive sleep apnea is the result of the airway collapsing during sleep, which causes arousal and impairs restful sleep. Adenotonsillectomy is the first-line treatment of obstructive sleep apnea and is usually effective in otherwise healthy nonsyndromic children. However, there are subgroups in which this surgery is less effective. These subgroups include children with obesity, severe obstructive sleep apnea preoperatively, Down syndrome, craniofacial anomalies and polycystic ovarian disease. Continuous positive airway pressure (CPAP) is the first-line therapy for persistent obstructive sleep apnea despite previous adenotonsillectomy, but it is often poorly tolerated by children. When CPAP is not tolerated or preferred by the family, surgical options beyond adenotonsillectomy are discussed with the parent and child. Dynamic MRI of the airway provides a means to identify and localize the site or sites of obstruction for these children. In this review the authors address clinical indications for imaging, ideal team members to involve in an effective multidisciplinary program, basic anesthesia requirements, MRI protocol techniques and interpretation of the findings on MRI that help guide surgery.

Purpose The proliferation of pediatric urgent care centers has increased the need for diagnostic imaging support, but the impact of employing radiology technologists at these centers is not known. The purpose of this study was to evaluate radiographic impact and quality at urgent care centers with and without radiology technologists. Methods A retrospective case-control study was conducted comparing 235 radiographic examinations (study) performed without and 83 examinations (control) performed with a radiology technologist at the authors' pediatric urgent care centers. Studies were evaluated for quality using a five-point, Likert-type scale (1 = poor, 5 = best) regarding field of view, presentation, and orthogonal view orientation. Studies were also evaluated for the incidence of positive results, need for repeat imaging, and discrepancies between initial study and follow-up. Results Imaging quality comparisons between study and control groups were statistically different for field of view (3.98 versus 4.29, P = .014), presentation (4.39 versus 4.51, P = .045), and orthogonal view orientation (4.45 versus 4.69, P = .033). The incidence of repeat imaging was similar (4.7% versus 2.4%, P = 0.526), as well as the discrepancy rates (3.4 versus 2.4%, P = 1.00). The incidence of abnormal radiographic findings for the study and control groups was similar (40.9% versus 34.9%, P = .363). Conclusions Radiography is an important triage tool at pediatric urgent care centers. It is imperative to have optimal radiographic imaging for accurate diagnosis, and imaging quality is improved when radiology technologists are available. If not feasible or cost prohibitive, it is important that physicians be given training opportunities to bridge the quality gap when using radiographic equipment and exposing children to radiation.

Purpose When implementing or monitoring department-sanctioned standardized radiology reports, feedback about individual faculty performance has been shown to be a useful driver of faculty compliance. Most commonly, these data are derived from manual audit, which can be both time-consuming and subject to sampling error. The purpose of this study was to evaluate whether a software program using natural language processing and machine learning could accurately audit radiologist compliance with the use of standardized reports compared with performed manual audits. Methods Radiology reports from a 1-month period were loaded into such a software program, and faculty compliance with use of standardized reports was calculated. For that same period, manual audits were performed (25 reports audited for each of 42 faculty members). The mean compliance rates calculated by automated auditing were then compared with the confidence interval of the mean rate by manual audit. Results The mean compliance rate for use of standardized reports as determined by manual audit was 91.2% with a confidence interval between 89.3% and 92.8%. The mean compliance rate calculated by automated auditing was 92.0%, within that confidence interval. Conclusion This study shows that by use of natural language processing and machine learning algorithms, an automated analysis can accurately define whether reports are compliant with use of standardized report templates and language, compared with manual audits. This may avoid significant labor costs related to conducting the manual auditing process.

Unlike medical procedure (Current Procedural Terminology [CPT]) and diagnostic (International Classification of Diseases [ICD]) codes, imaging (IMG) codes are defined at the local, institutional level. IMG codes are used within an organization to define IMG procedures and how they are routed between information systems. Our purpose is to describe the implementation of a reorganization (referred to as the IMG code cleanup ) and governance structure deployed at a large children's hospital and the initial effects of that implementation. The number of IMG codes precleanup was 1388 and postcleanup was 826. This is a reduction by 40%. The mean number of addendums to radiology reports for billing reasons per month was 31.7 before and 18.6 after the IMG code cleanup . This represents a 41% decrease. We believe that the reorganization, standardization of the approach to naming and coding, reduction in the number of IMG codes, as well as governance structure put in place to maintain that organization has had both direct and indirect effects on the department's ability to both provide reliable IMG services and position the department to improve.

Integration was implemented of previously independently practicing pediatric radiology services across a pediatric health care system with sites 1,000 miles apart.The purpose of this manuscript is to describe the process utilized to integrate imaging services across our enterprise and to study the direct effects on report turnaround time and other parameters.Parameters were evaluated both the year before integration as well as the 2 years following integration and compared for improvement. Parameters studied included report turnaround time, academic productivity (as measured by peer review publications per year), and degree of sub-subspecialization within pediatric radiology (as measured by the percentage of neuroimaging CT and MRI studies read by pediatric neuroradiologists).Comparing pre- to post-integration measures, the median report turnaround time decreased from 2.3 h to 1.1 h (52.1% improvement), the percentage of neuroimaging studies read by neuroimaging faculty increased from 15.0% to 86.6% (477% improvement), and peer review publication by calendar year increased from 3 to 30 (1,000% improvement). Other benefits included increased hours of in-house coverage and execution on multiple quality improvement efforts.A pediatric health care system successfully integrated radiology services across multiple previously non-integrated locations into one functional group. This integration was associated with a positive effect on multiple parameters.

Purpose The National Quality Forum (NQF) is a nonprofit consensus organization that recently endorsed a measure focused on CT radiation doses. To comply, facilities must summarize the doses from consecutive scans within age and anatomic area strata and report the data in the medical record. Our purpose was to assess the time needed to assemble the data and to demonstrate how review of such data permits a facility to understand doses. Methods and Materials To assemble the data we used for analysis, we used the dose monitoring software eXposure to automatically export dose metrics from consecutive scans in 2010 and 2012. For a subset of 50 exams, we also collected dose metrics manually, copying data directly from the PACS into an excel spreadsheet. Results Manual data collection for 50 scans required 2 hours and 15 minutes. eXposure compiled the data in under an hour. All dose metrics demonstrated a 30% to 50% reduction between 2010 and 2012. There was also a significant decline and a reduction in the variability of the doses over time. Conclusion The NQF measure facilitates an institution's capacity to assess the doses they are using for CT as part of routine practice. The necessary data can be collected within a reasonable amount of time either with automatic software or manually. The collection and review of these data will allow facilities to compare their radiation dose distributions with national distributions and allow assessment of temporal trends in the doses they are using.

Button battery ingestion can lead to dangerous complications, including vasculoesophageal fistula formation. The presence of a vascular ring may complicate battery ingestion if the battery lodges at the level of the ring and its important vascular structures. We report a 4-year-old boy with trisomy 21 who was diagnosed with a vascular ring at the time of button battery ingestion and died 9 days after presentation due to massive upper gastrointestinal bleeding from esophageal erosion and vasculoesophageal fistula formation.

The hemoglobinopathies are a group of genetic disorders with a broad spectrum of clinical manifestations and radiologic findings. The imaging of pediatric hemoglobinopathies, which is influenced by concomitant hemosiderosis and the sequela of chelation therapy, has evolved over the years along with ever-improving technology. This article reviews and illustrates the most common radiographic and cross-sectional imaging findings of the 2 best known and clinically relevant hemoglobinopathies in pediatric patients, sickle cell disease and β-thalassemia.

Replaced right hepatic artery is a well-described normal anatomical variant, not previously associated with acute or chronic abdominal symptoms or long-term sequelae. We report a 15-year-old girl presenting with nearly a decade of symptoms secondary to external compression of the proximal duodenum by the ventral passage of a replaced right hepatic artery. Surgical bypass of the duodenal obstruction significantly improved her symptoms. Replaced right hepatic artery related duodenal compression should be considered in the differential diagnosis of proximal duodenal obstruction. The presence of the replaced right hepatic artery should be clearly communicated to allow optimal presurgical planning.

Quality-improvement practices and research are important in radiology for multiple reasons [1–9]. For all of us involved in medical imaging, regardless of the type of practice, part of our vision and mission is improving the health of all patients through imaging delivery. While certainly this involves direct patient care, improving the practice of medical imaging delivery as well as sharing clinical experiences can expand our vision and our mission beyond the immediate patients we are serving. In addition, patients as well as regulatory bodies are increasingly demanding that physicians practice quality improvement through mechanisms such as the maintenance of certification [10] and the potential maintenance of licensure. It can also be argued that participation in quality-improvement and research activities can increase work satisfaction and engagement of radiologists. Finally, active quality-improvement and research programs are essential for radiology to preserve its leadership role in medical imaging [1–6]. At our institution, we have put in place a Quality Improvement in Research and Imaging (QuIRI) program. The purpose of this manuscript is to describe the QuIRI process and how it has positively affected our research and qualityimprovement program. A productive research program can enhance the prestige and clout of a radiology department within its institution and of the subspecialty of radiology within medicine. Cultivation of quality-improvement and research activities is essential to optimizing radiology’s role in health care reform. Quality will be a differentiating factor in a commodity imaging market [9]. A number of barriers have been identified, making development of research and quality-improvement initiatives difficult in radiology [11–14]. These include the historical lack of research training as a component of radiology training, increased clinical responsibilities related to increasing imaging volumes and decreased reimbursement, the inability to cover total research expenses even in successful grant-funded programs, the NIH salary cap being lower than most radiologists’ salaries, alignment of radiologists’ incentives and culture, and vacant positions within academic radiology departments [11–14]. These problems have prompted a number of articles describing important steps in programmatic aspects necessary to create a successful quality-improvement and research program in a department of radiology [1–15]. In our department, we have put in place a number of ways to stress the importance of research among our clinical radiologists and to promote a culture of research [7] as well as establish processes to promote quality and safety [15–21]. Successful programs to improve patient safety require a component aimed at improving safety culture and environment, resulting in a reduced number of human errors that could lead to patient harm. Safety coaching provides peer accountability. It involves observing for safety behaviors and use of error prevention techniques and provides immediate feedback. For more than a decade, behavior-based safety coaching has been a successful strategy for reducing error within the context of occupational safety in industry. We describe the use of safety coaches in radiology. Safety coaches are an important component of our comprehensive patient safety program.

A comprehensive safety program can have a positive influence on safety performance and safety culture within a department of radiology. The program should include both vertical interventions aimed at specific areas of potential safety errors as well as horizontal interventions aimed at improving safety culture and decreasing the baseline rate of human error. In our opinion, the key cultural transformations that must occur to improve safety culture include recognition that safety is an issue, emphasis that everyone is accountable for patient safety, and creating a culture where people are expected and encouraged to speak up in the face of uncertainty. The article describes the horizontal interventions to improve patient safety used in our department.

Objective: To define the MRI findings of congenital high airway obstructionsequence (CHAOS) in a series of fetuses.Materials and Methods: Prenatal fetal MR images were reviewed in sevenfetuses with CHAOS at 21 to 27 weeks of gestation. The MRI findings werereviewed. The MRI parameters evaluated included the appearance of the lungsand diaphragm, presence or absence of hydrops, amount of amniotic fluid, airwayappearance, predicted level of airway obstruction, and any additional findings orsuspected genetic syndromes. Results: All the fetuses viewed (7 of 7) demonstrated the following MRI find-ings: dilated airway below the level of obstruction, increased lung signal, marked-ly increased lung volumes with flattened or inverted hemidiaphragms, massiveascites, centrally positioned and compressed heart, as well as placentomegaly.Other frequent findings were anasarca (6 of 7) and polyhydramnios (3 of 7). MRIidentified the level of obstruction as laryngeal in five cases and tracheal in twocases. In four of the patients, surgery or autopsy confirmed the MRI predictedlevel of obstruction. Associated abnormalities were found in 4 of 7 (genetic syn-dromes in 2). Postnatal radiography (n = 3) showed markedly hyperinflated lungswith inverted or flattened hemidiaphragms, strandy perihilar opacities, pneumoth-oraces and tracheotomy. Two fetuses were terminated and one fetus demised inutero. Four fetuses were delivered via ex utero intrapartum treatment procedure.Conclusion: MRI shows a consistent pattern of abnormalities in fetuses withCHAOS, accurately identifies the level of airway obstruction, and helps differenti-ate from other lung abnormalities such as bilateral congenital pulmonary airwaymalformation by demonstrating an abnormally dilated airway distal to the obstruc-tion. ongenital high airway obstruction sequence (CHAOS) is a rare, life-threatening condition characterized by complete or near complete intrin-sic obstruction of the fetal airway. CHAOS has been considered analmost invariably fatal condition (1) which often goes incorrectly diagnosed untilautopsy. However, advances in prenatal imaging, earlier diagnosis in utero, in uterosurgical treatment, and delivery via ex utero intrapartum treatment (EXIT) totracheostomy may contribute to improving the outcome of this condition (1-7). Withthe exception of case reports (2, 8-10), there is little in the literature describing theMRI findings of CHAOS. We present the MRI findings in a series of seven fetuses withprenatal diagnosis of CHAOS. To our knowledge, this is the largest described series ofpatients with CHAOS and the first article focusing on the description of MRI findings.Carolina V. A. Guimaraes, MD

As part of a patient safety program in the authors' department of radiology, operational rounds have been instituted. This process consists of radiology leaders' visiting imaging divisions at the site of imaging and discussing frontline employees' concerns about patient safety, the quality of care, and patient and family satisfaction. Operational rounds are executed at a time to optimize the number of attendees. Minutes that describe the issues identified, persons responsible for improvement, and updated improvement plan status are available to employees online. Via this process, multiple patient safety and other issues have been identified and remedied. The authors believe that the process has improved patient safety, the quality of care, and the efficiency of operations. Since the inception of the safety program, the mean number of days between serious safety events involving radiology has doubled. The authors review the background around such walk rounds, describe their particular program, and give multiple illustrative examples of issues identified and improvement plans put in place.

Although round pneumonia is a well-known clinical entity, there have been no large case reviews, with most knowledge based on case reports and small series from the 1960s and 1970s. To review the imaging findings of a large series of children with round pneumonia. A retrospective review of radiographic and CT findings in all children reported to have round pneumonia at a large children’s hospital from 2000 to 2006 was performed. Children with underlying medical conditions were excluded. Epidemiologic characteristics, radiographic and CT findings, and follow-up imaging were reviewed. The review identified 109 children (mean age 5 years, range 4 months to 19 years). Round pneumonias tended to be solitary 98% (107/109), have well-defined borders 70% (77/109), and be posteriorly located 83% (91/109), with the following lobar distribution: left lower lobe (36), right lower lobe (33), right upper lobe (28), left upper lobe (7), right middle lobe (4), and lingula (2). Round pneumonia tended to resolve on follow-up imaging (95%, 41/43) as compared to progression to lobar pneumonia (4.6%, 2/43). Three patients (2.6%, 3/112) originally suspected to have round pneumonia were later shown to have other diagnoses: cavitary necrosis in pneumonia (two) or pleural pseudocyst (one). Round pneumonia occurs in young children (mean age 5 years), tends to be a solitary lesion, and is most commonly posteriorly located. Misdiagnosis of other pathology as round pneumonia is uncommon.

New guidelines for medical credentialing and privileging were published by the Joint Commission in 2007. These guidelines require practitioner-specific data collection in the areas of patient care, medical and clinical knowledge, practice-based learning and improvement, interpersonal and communication skills, professionalism, and system-based practice. The authors describe their current process for privileging on the basis of practitioner performance with the aim that the description may serve as a template for other departments of radiology working on similar processes. The parameters chosen in each category were picked on the basis that they would contribute to quality improvement while creating minimal additional data collection beyond what was already occurring in the department.

Background— Abdominal fat is more related to health risk than is whole-body fat. Determining the factors related to children’s visceral fat could result in interventions to improve child health. Objective— Given the effects of physical activity on adults’ visceral fat, it was hypothesized that, after accounting for whole-body fat, physical activity would be inversely related to children’s visceral (VAT), but not to subcutaneous (SAT), abdominal adipose tissue. Design— In this cross-sectional observational study conducted in forty-two 8-y-old children (21 boys, 21 girls) at risk of obesity [>75th body mass index (BMI) percentile, with at least one overweight parent], familial factors (eg, maternal BMI), historic weight-related factors (eg, birth weight), and the children’s current physical activity (self-reported and measured with accelerometry) and diet were examined as potential correlates of the children’s whole-body composition (measured with BMI and dual-energy X-ray absorptiometry) and abdominal fat distribution (measured by magnetic resonance imaging). Harm from medical error is a difficult challenge in health care, including radiology. Modern approaches to patient safety have shifted from a focus on individual performance and reaction to errors to development of robust systems and processes that create safety in organizations. Organizations that operate safely in high-risk environments have been termed high-reliability organizations. Such organizations tend to see themselves as being constantly bombarded by errors. Thus, the goal is not to eliminate human error but to develop strategies to prevent, identify, and mitigate errors and their effects before they result in harm. High-level reliability strategies focus on systems and organizational culture; intermediate-level reliability strategies focus on establishment of effective processes; low-level reliability strategies focus on individual performance. Although several classification schemes for human error exist, modern safety researchers caution against overreliance on error investigations to improve safety. Blaming individuals involved in adverse events when they had no intent to cause harm has been shown to undermine organizational safety. Safety researchers have coined the term just culture for the successful balance of individual accountability with accommodation for human fallibility and system deficiencies. Safety is inextricably intertwined with an organization's quality efforts. A quality management system that focuses on standardization, making errors visible, building in quality, and constantly stopping to fix problems results in a safer environment and engages personnel in a way that contributes to a culture of safety.

A patent urachus, which is typically diagnosed as leakage from the umbilicus postnatally, can present as an allantoic cyst in the umbilical cord antenatally. We report a case of a patent urachus with an allantoic cyst diagnosed via fetal MR imaging at 24 weeks' gestation. Early detection allowed for appropriate counseling and prompt corrective surgery after birth.

The purpose of our study was to review the current practice of radiologists with respect to dictating the presence of obesity in imaging reports.Over 1 million radiology reports dictated at a large pediatric hospital from 1994 to 2002 were analyzed for several keywords relating to obesity. The number of cases in which the keywords appeared was recorded for each year, and a percentage was calculated. Reports done in 1999 and 2000 were further analyzed to determine where the keywords were positioned within the report.The number of reports containing a keyword ranged from 131 to 456 per year. During each year, documentation of obesity occurred in less than 0.4% of all reports. During that same time period, the national prevalence of pediatric obesity ranged from 6-16%. Detailed examination of the 1999 and 2000 reports showed that even in the reports that mentioned obesity, it was usually not listed in the diagnostic impression.Despite the increase in public awareness of obesity and increasing recognition of obesity-related disease, this study did not find a similar awareness among radiologists at a large pediatric radiology department. The reason for this discrepancy is speculative and likely multifactorial. Regardless, radiologists may be missing an opportunity to play a role in disease prevention and early recognition by documenting findings of obesity and thereby bringing them to the attention of referring physicians.

Background: Renal angiomyolipomata can reduce renal reserve and lead to renal insufficiency and failure. Angiomyolipomata often have abnormal vasculature, with aneurysms that can hemorrhage. Treatment of angiomyolipomata greater than 4 cm in diameter is suggested to decrease the risk for hemorrhage. Nephron-sparing procedures are critical in patients because of their limited renal reserve. Embolization has been used to treat these tumors, but there are limited studies examining efficacy. Our study examines the efficacy of selective embolization in decreasing tumor burden, preventing hemorrhage, and preserving renal function. Methods: We conducted a retrospective study of 16 patients with 20 angiomyolipomata on 18 kidneys who underwent 18 transcatheter transarterial embolization procedures. Aneurysm number and size were documented and tumor volumes were measured before and after embolization. Preprocedure and follow-up renal function also were measured. Changes in angiomyolipoma volume and kidney function were assessed for significance by using paired t -test. Results: Before embolization, 7 angiomyolipomata had more than 5 aneurysms, 9 had 1 to 5 aneurysms, and 4 had no aneurysms, but showed tortuous dysmorphic arteries. Mean aneurysm size was 5 mm. In patients available for follow-up, 15 of 16 tumors had decreased in volume (mean decrease, 56.1%; P = 0.001). At an average of 40 months' follow-up, there have been no subsequent hemorrhages. Patients' decline in renal function was not significantly different from that expected because of the natural course of the disease. Conclusion: Selective embolization decreases tumor size, prevents hemorrhage, and preserves kidney function in patients with tuberous sclerosis with renal angiomyolipomata.

Cine magnetic resonance (MR) imaging sleep studies have become a useful tool in the evaluation of obstructive sleep apnea in children with certain categories of pathologic conditions. In this article, the author describes a program for the use of cine MR sleep studies in the evaluation of children with obstructive sleep apnea. The following areas are discussed: clinical indications, patient preparation, anatomic considerations, MR technique, technical issues, image interpretation, commonly encountered diagnoses, volume segmentation processing of data, and controversial areas.

This article discusses the common clinical scenarios regarding otherwise healthy children who develop suspected pneumonia in which imaging becomes an issue. The following topics are covered concerning the roles of imagingin the management of pneumonia: evaluation for possible pneumonia, determination of a specific etiologic agent, exclusion of other pathology, evaluation of the child who has failure of pneumonia to clear, and evaluation of complications related to pneumonia.

Background: Pleuropulmonary blastoma (PPB) is a rare tumor of the chest seen in young children and recently recognized as distinct from the pulmonary blastoma typically encountered in adults. Objective: The purpose of this study is to review and describe the findings of PPB on radiography and CT in four patients. Methods: Radiographs and CT findings were reviewed in four patients with pathologically proven PPB. Results: All four cases demonstrated large masses in the right hemithorax with heterogeneous low attenuation, pleural effusion, contralateral mediastinal shift, and lack of chest wall invasion. Conclusion: When a large pleural-based mass is identified in a young child, PPB should be considered. Suggestive findings include absence of chest wall invasion, presence of pleural fluid, right-sided location, and heterogeneous low attenuation.

Objective/Hypothesis: Although usually successful, not all obstructive sleep apnea is cured by removal of the tonsils and adenoids (T&A). This is particularly true in children with Down syndrome and craniofacial anomalies. This is because of the multiple levels of obstruction in their airways, with obstruction present not only at the level of the tonsils and adenoids but also from base of tongue obstruction, soft palate collapse, and hypopharyngeal collapse. The cine magnetic resonance image (MRI) is useful in evaluating the upper airway in those patients who have not achieved a normal polysomnogram after T&A surgery. Study Design: Prospective case series. Methods: Fifteen children with Down syndrome who had previously undergone a T&A but continued to have abnormal postoperative polysomnograms underwent a cine MRI with fast gradient cine MRI images. Results: The cine MRI identified different areas and levels of obstruction that ultimately affected the children's treatment courses. Recurrent adenoid tissue, glossoptosis, soft palate collapse, hypopharyngeal collapse, and enlarged lingual tonsils were identified. The results as well as several illustrative cases are presented. Conclusion: Cine MRI evaluates upper airway obstruction in children who may have multiple sources of obstruction causing their obstructive sleep apnea. This is particularly helpful in children with Down syndrome and craniofacial anomalies. This technology is useful in all children with complex upper airway obstruction.

Dilute Hypaque Sodium is generally well accepted as an oral contrast agent for CT of pediatric patients who have experienced recent blunt abdominal trauma. However, Hypaque can cause complications. Using water as a substitute contrast agent eliminates these potential complications. The purpose of our study was to compare the performance of water with that of dilute Hypaque as an oral contrast agent. Our hypothesis was that we would find no significant difference in performance between the two agents in defining anatomic details of the hollow gastrointestinal tract.We performed a retrospective review of 74 CT scans obtained in infants and children who had received blunt abdominal trauma, scoring the quality of visualization of bowel structures, the presence of non-bowel-related findings, and the confidence level in making each assessment. The date range of the scans reviewed overlapped with the period in which the oral contrast material used for scanning such patients was switched from dilute Hypaque to water. Of the 74 CT scans that we reviewed, 53 were obtained with dilute Hypaque and 21 were obtained with water. The sex distribution between the two groups was compared using a chi-square test, whereas the mean age was compared using a two-sample two-sided Student's t test. A two-sample one-sided Student's t test of equivalence was used to analyze the data.Sex distribution for the two groups of patients was not significantly different (69.81% of the group who received dilute Hypaque were boys; 68.18% of the group who received water were boys). Furthermore, the difference in the mean age for the two groups was not statistically significantly (dilute Hypaque group, 8.86 years; water group, 10.18 years). No statistically significant difference in performance of the contrast agents was found with respect to the detection of intraabdominal abnormality. As an oral contrast material, water performed as well as dilute Hypaque in facilitating visualization of all intraabdominal anatomic structures.In defining anatomic details of the hollow gastrointestinal tract, water is as effective as dilute Hypaque as an oral contrast agent for CT in the setting of acute blunt abdominal trauma in pediatric patients.

There are few data regarding the frequency and type of diagnostic errors made by radiology residents and fellows (“trainees”). However, increasing interest in reducing medical errors highlights the need to analyze which areas of medical knowledge are most problematic for physicians-in-training, including radiology trainees. Once these areas are identified, they can be emphasized during training. To quantify the diagnostic errors made by radiology trainees interpreting radiographs from a pediatric emergency department. A total of 23,273 dictations of emergency radiographs performed over a 1-year period at a pediatric hospital were analyzed for corrections after staff interpretation and for type and incidence of missed abnormalities by radiology trainees. Errors were categorized by type of pathology and anatomic region. Of the 80 errors detected, 90% were false negatives and 69% were recurrent. Most errors (69%) involved the diagnosis of fractures and/or dislocations. Sixty-one percent of all recurrent errors involved buckle, Salter II, avulsion, and transverse fractures—yet these cases constituted only 3% of all cases seen during the study period. The most common errors made by radiology trainees can be identified. By targeting these errors, training programs can improve the quality and relevance of the education they provide.

Computed tomography (CT) is an extremely valuable diagnostic tool. Recent advances, particularly multidetector technology, have provided increased and more diverse applications. However, there is also the potential for inappropriate use and unnecessary radiation dose. Because some data indicate that low-dose radiation (such as that in CT) may have a significant risk of cancer, especially in young children, it is important to limit CT radiation by following the ALARA (as low as reasonably achievable) principle. There is a variety of strategies to limit radiation dose, including performing only necessary examinations, limiting the region of coverage, and adjusting individual CT settings based on indication, region imaged, and size of the child. The pediatric health care provider has a pivotal role in the performance of CT and may be the only individual who discusses these important CT radiation issues with the child and family. For this reason, this article will summarize the issues with CT patterns of use and radiation risk, and provide dose reduction strategies pertinent to pediatric health care providers.

Multidetector CT (MDCT) has quickly become established as an invaluable imaging option for children. Just as when single-detector helical CT replaced conventional CT, MDCT is now replacing single-detector CT (SDCT). Although much of this is caused by the overall benefits of CT [1], multidetector technology offers additional and unique advantages. Multidetector technology also presents additional and unique challenges for children. For example, the wide range of sizes, from small preterm infants to large adult-size adolescents, makes scanning more demanding. Because of these unique considerations in children, the following material briefly summarizes the importance of MDCT; reviews the general technical considerations for MDCT; discusses applications of this technology for chest and abdomen imaging; and provides general recommendations for body (chest and abdomen) imaging of the pediatric population. Emphasis is placed on those technical considerations unique to multidetector technology, as opposed to conventional CT or SDCT. Those applications in which multidetector technology either improves or provides new information compared with older CT technology are reviewed. Technical aspects are discussed to a greater extent than applications because image quality and diagnostic capability are dependent on appropriate technique. Importance of MDCT

To review the renal imaging findings and changes over time in a large series of young patients with tuberous sclerosis complex (TSC).One hundred thirty-nine renal imaging studies (113 ultrasonographic scans, 15 computed tomographic scans, and 11 magnetic resonance images) were identified in 59 patients with TSC (mean age, 11.4 years; age range, 3 days to 36 years). There were 31 male and 28 female patients. Angiomyolipomas and cysts were identified and characterized according to the following features: number, unilateral versus bilateral, largest diameter, and change over time. In patients whose initial examination results were normal, the age at onset of lesions was noted. The chi(2) test was used to determine whether cysts and angiomyolipomas occurred in different subpopulations.Angiomyolipomas were identified in 47 (80%) patients and were too numerous to count in 36 (76%), focal in 38 (81%), and bilateral in 42 (89%). The mean largest diameter was 21 mm. Cysts were identified in 28 (47%) patients, 18 (64%) of whom had fewer than five cysts. Cysts were bilateral in 17 (61%) patients. The mean largest diameter was 20 mm. In patients with initially normal examination results, the mean age at presentation was 9.0 years for those with cysts and 9.2 years for those with angiomyolipomas. In 80 follow-up examinations, size and/or number increased in 32 (40%) angiomyolipomas and 21 (26%) cysts. Cysts and angiomyolipomas did not occur in significantly different subpopulations (P =.13).Both angiomyolipomas and cysts occur commonly in pediatric patients with TSC and tend to increase in size and number with increasing age. Angiomyolipomas are more common than cysts and tend to be numerous.

To evaluate the frequency of "missing" sternal ossification center (asynchronous non-ossification) in young children.Lateral chest radiographs obtained in 229 children (mean age, 3.7 years) were retrospectively evaluated for sternal ossification. Four superior sternal segments were considered normal if they were ossified to a similar degree. A segment was considered asynchronous if decreased ossification, as compared with the remaining sternal segments, was demonstrated or if ossification was absent. Asynchronous ossification of inferior sternal segment 5 was recorded separately. Logistic regression analysis was applied to determine if there was a statistically significant relationship (P.05) between age or sex and pattern of sternal ossification (normal vs asynchronous).Of the 916 superior four sternal segments (four segments in each of 229 patients) evaluated, 32 (3.5%) showed asynchronously decreased or absent ossification. Locations of these 32 segments follow: segment 1, two (0.2%) instances; segment 2, 14 (1.5%) instances; segment 3, two (0.2%) instances; and segment 4, 14 (1.5%) instances. Inferior segment 5 was not ossified in 73 (31.9%) patients. There was a statistically significant relationship between decreased age and increased likelihood of occurrence of asynchronous ossification of one of the sternal ossification centers 1-4 (P.003) and of occurrence of asynchronous ossification at sternal segment 2 (P.018).Missing sternal ossification centers occur most commonly at segments 2 and 4. Such asynchronous non-ossifications become less common in older children.

OBJECTIVE. The purpose of this study was to review the imaging findings of children referred for cross-sectional imaging to evaluate persistent airway symptoms after surgical therapy for double aortic arch.CONCLUSION. Airway narrowing is clearly shown on cross-sectional imaging in patients with persistent airway symptoms after surgical therapy for double aortic arch. Two patterns of airway compression are typically seen: narrowing of the trachea at the level of the postsurgical arch and narrowing of the left main bronchus as a result of compression from a midline descending aorta. Both patterns may be seen in patients regardless of whether the left or the right arch has been ligated.
