Le cancer du rein represente 3% des cancers. Il est classe au troisieme rang des cancers urologiques apres celui de la prostate et de la vessie. Depuis la nephrectomie totale elargie de Robson en 1963, on assiste a un changement continu de la prise en charge de ce cancer. En 1990, on a mis en question le curage ganglionnaire systematique. En 1993, on a mis en question la surrenalectomie systematique. En 1995, on a commence a proposer la chirurgie conservatrice pour les tumeurs dont le diametre est inferieur a 4 cm. A partir de 2004, on l’a proposee pour les tumeurs < 7 cm. En fonction de la taille et de la localisation tumorale, on peut realiser une nephrectomie partielle polaire, une resection cuneiforme, une enucleation, une chirurgie ex situ ou zero ischemie. L’ischemie froide permet une meilleure conservation de la fonction renale, ainsi que le clampage parenchymateux par rapport au clampage pediculaire. La chirurgie conservatrice du cancer du rein est en nette progression certes, mais elle n’a pas encore la place qu’elle merite. Ceci peut etre lie a une meconnaissance des indications, ou crainte des morbidites. La limite arbitraire de 7 cm merite d’etre discutee. L’evolution naturelle se fait vers la chirurgie conservatrice quelque soit la taille de la tumeur et quelque soit le rein controlateral, a condition qu’elle soit techniquement realisable. RésuméLe carcinome chromophobe se voit essentiellement à la cinquième décennie et représente 5% des tumeurs rénales. Il existe deux sous types de cancer à cellules chromophobes: le type clair (70%) et le type éosinophile. La variante à cellules éosinophiles doit être distinguée de l'oncocytome. Ce dernier se caractérise par une cicatrice fibreuse centrale d'aspect stellaire. Le carcinome chromophobe et l'oncocytome peuvent même coexister dans le cadre du syndrome de Birt-Hogg-Dubé. Certaines tumeurs appelées hybrides partagent des caractéristiques architecturales et cytologiques de ces deux tumeurs. Le pronostic du carcinome chromophobe est favorable. Il est le plus souvent limité au rein et de bas grade nucléaire. Il semblerait donc licite dans les cas ou un examen extemporané mettrait en évidence une tumeur chromophobe, de limiter l'intervention à une néphrectomie partielle si techniquement réalisable.SummaryChromophobe carcinoma is seen mainly in the fifth decade and accounts for 5% of renal tumors. It originates at the level of type B intercalated cells of the collecting duct. There are two subtypes of chromophobe cell carcinoma: the clear type (70%) and eosinophilic type. The variant with eosinophilic cells must be differentiated from oncocytoma. The latter is characterized by a central fibrous stellate scar. Chromophobe carcinoma and oncocytoma may even coexist in the syndrome of Birt-Hogg-Dubé. Some tumors called hybrid share architectural and cytological features of these two tumors. The prognosis of chromophobe carcinoma is favorable. It is most often limited to the kidney and is of lower nuclear grade. RésuméIntroductionEnviron 90% des patients diabétiques présentent un diabète de type 2. L'hypoglycémie constitue le principal effet indésirable des antidiabétiques oraux. Elle est d'autant plus grave qu'elle s'accompagne d'une insuffisance rénale.Cas cliniqueNous rapportons le cas d'un homme âgé de 85ans suivi pour un diabète de type 2 et une hypertension artérielle. Il a été admis via les urgences dans un tableau de trouble de conscience associé à une fièvre. Le bilan réalisé a objectivé une hypoglycémie profonde, une insuffisance rénale obstructive et un taux de PSA élevé (antigène spécifique de la prostate). Les antidiabétiques oraux ont été arrêtés et un sondage vésical a été effectué avec comme résultats une correction des hypoglycémies et de la fonction rénale. Le patient a eu une résection trans-urétrale de la prostate (RTUP) et l'examen anatomopathologique était en faveur d'un adéno-myo-fibrome. Les troubles neurologiques en rapport avec l'hypoglycémie étaient sous forme d'agitations, délire, trouble des fonctions supérieures et troubles du sommeil. Cependant notre patient n'a pas présenté de déficit neurologique. L’évolution clinique était favorable.ConclusionL'insuffisance rénale obstructive peut constituer une complication de l'adénome de la prostate à un stade ultime. Elle est responsable d'accumulation dans le sang de médicaments y compris les antidiabétiques oraux. L'hypoglycémie qui en résulte est délétère pour la qualité de vie du patient et peut être responsable d'accidents neurologiques.AbstractIntroductionApproximately 90% of patients with diabetes have type 2 diabetes. Hypoglycemia is the main side effect of oral antidiabetics, and it can be more serious when it is accompanied by renal failure.Clinical caseWe report the case of a man aged 85 years followed for type 2 diabetes and hypertension. He was admitted via the emergency with a disturbed level of consciousness associated with fever. The medical evaluation revealed profound hypoglycemia, obstructive renal failure and an elevated PSA (prostate specific antigen). Oral hypoglycemic agents were discontinued and catheterization was performed resulting in correction of hypoglycemia and renal function. The patient underwent a transurethral resection of prostate (TURP) and pathologic examination was in favor of benign hyperplasia. Neurological disorders related to hypoglycemia were agitation, delirium, impaired higher functions and sleep disorders. However our patient did not present any neurological deficit. The clinical course was favorable.ConclusionThe obstructive renal failure can be a complication of benign prostatic hyperplasia in a final stage. It is responsible for accumulation in the blood of drugs including oral anti-diabetic agents. The resulting hypoglycemia is deleterious to the quality of life of the patient and may be responsible for neurological disorders.

Objective Percutaneous endopyeloplasty is a horizontal suturing of the endopyelotomy incision via a unique percutaneous tract. We present a feasibility study of our technique of percutaneous endopyeloplasty using a conventional suture with a laparoscopic needle holder via the nephroscope. Materials and Methods A longitudinal endopyelotomy incision is performed through a 24-Fr working sheath placed via an upper calyx. Then an initial suture is placed, approximating the endopyelotomy incision distal and proximal apex, using a conventional absorbable 13-mm needle suture with a lengthened 3.5-mm pediatric laparoscopic needle holder, via the nephroscope. If possible, an additional 2 sutures are eventually placed, 1 on either side of the initial midline suture. Results Percutaneous endopyeloplasty, using a conventional suture with a pediatric laparoscopic needle holder via a nephroscope, is technically possible, reproducible, and effective, and was performed in 10 consecutive patients. The tissue approximation provides a fast, full thickness ureteral healing. Endopyelotomy horizontal suturing leads to a wider caliber reconstruction of the ureteropelvic junction. Conclusion Percutaneous endopyeloplasty is a promising technique. Our procedure for endopyeloplasty is technically feasible and effective, with little need for highly specialized equipment. However, further technical experience and longer follow-up in a larger group of patients are necessary for more development of this technique.

We describe a new renal displacement technique using an 18-gauge needle to facilitate superior calix puncture and consequently to decrease intrathoracic morbidity. Initially, a lower or middle calix is punctured with an 18-gauge needle. Then, the proximal end of the needle is progressively pushed in the cephalic direction. Therefore, the kidney is pushed caudally by the lever maneuver. This technique has also been used to immobilize the kidney or to reorient complex and malrotated kidneys.

L’apparition de métastases péniennes dans l’évolution du carcinome urothélial est un événement rare (1 à 8%). Elles surviennent le plus souvent (65%) dans les 18 mois suivant le diagnostic de la lésion primaire et leur pronostic est sombre avec une survie dépassant exceptionnellement 20 mois. Le traitement des métastases caverneuses est multidisciplinaire. Les meilleurs résultats en termes de survie globale ont été obtenus avec l’association amputation de la verge associée à une chimiothérapie. L’hyperactivité vésicale (HAV) (overactive bladder [OAB]) se caractérise par une urgenturie, avec ou sans incontinence urinaire, associée à une pollakiurie diurne (> 8 épisodes) et à une nycturie (> 1 épisode). Elle touche 27 % des hommes et 43 % des femmes de plus de 40 ans. Elle a d’importantes répercussions sur les activités quotidiennes ainsi que sur le profil psychologique et sexuel. Elle résulte d’une contraction anormale du détrusor liée à une stimulation de certains récepteurs muscariniques. Il existe cinq sous-types de récepteurs muscariniques (M1, M2, M3, M4 et M5) situés dans la vessie, mais aussi dans d’autres organes : le cerveau, les yeux, les ganglions sympathiques, les glandes salivaires et le coeur. Même si les récepteurs M2 sont les plus nombreux, ce sont essentiellement les récepteurs M3 qui interviennent dans la contraction vésicale. Les anticholinergiques constituent la pierre angulaire du traitement de cette affection. Plusieurs molécules sont actuellement sur le marché, mais jusqu’à présent, aucun médicament n’exerce d’action sélective sur la vessie. De plus, ils ont tous des effets secondaires. Il est possible d’en atténuer l’intensité en amorçant un traitement progressif à raison de faibles doses croissantes.

RésuméLe score de Gleason des biopsies prostatiques est primordial dans le traitement du cancer de la prostate. Il évalue l'agressivité et l’évolutivité de la tumeur. L'objectif de ce travail était d’évaluer la corrélation entre le score de Gleason biopsique et celui des pièces de prostatectomie.Matériel et méthodesC'est une étude rétrospective concernant 30 patients traités pour cancer de la prostate par prostatectomie radicale. Les paramètres étudiés étaient: l’âge, le taux de PSA, le score de Gleason à la biopsie et à la pièce de prostatectomie. Nous avons également évalué la concordance des groupes de différenciation (bien, moyennement et peu différencié).RésultatsLa concordance du score de Gleason était de 58,82% avec un coefficient de concordance kappa de 0,329, ce qui correspond à une valeur faibe, avec une sous-stadification dans 35,29% et une surstadification dans 6%. Pour ce qui est des groupes de différenciation, cette concordance était de 50% dans le groupe bien différencié (Score de Gleason de 2 à 4) et augmente à 100% dans le groupe moyennement différencié (Score de Gleason de 5 à 7). Par ailleurs, aucun malade n’était classé dans le groupe peu différencié (Score de Gleason de 8 à 10) ni à la biopsie ni à la pièce de prostatectomie.ConclusionLe score de Gleason biopsique influence la décision thérapeutique dans le cancer de prostate localisé. Cependant, il ne reflète pas toujours le score de Gleason de la pièce de prostatectomie, en particulier dans les groupes bien différenciés.AbstractIntroductionThe Gleason score obtained on biopsies is an essential part of treatment decision making in prostate cancer. This score assesses aggressiveness and progression of the tumor.ObjectiveTo evaluate the correlation between Gleason score at needle biopsy and prostatectomy specimen.Subjects and MethodsWe reviewed the records of 30 patients treated for prostate cancer by radical prostatectomy and we evaluated the correlation between Gleason score on biopsy and prostatectomy specimen. Other parameters evaluated were age and PSA.ResultsThe concordance of Gleason score was 58.82%, upgraded in 35, 29% and downgraded in only 6%. The kappa coefficient was moderate=0,329. Regarding groups of differentiation, this correlation was 50% in the well differentiated group (Gleason score 2 to 4) and 100% in moderately differentiated group (Gleason score 5 to 7). No patient was classified as poorly differentiated (Gleason score 8 to 10).ConclusionGleason score of biopsies may influence treatment and prognosis of localized prostate cancer. It is important to keep in mind that it does not always reflect the true Gleason score, especially in well differentiated cases.

Objectives To report a technique of percutaneous endoscopic nephropexy, using a polyglactin suture passed through the kidney, in patients with nephroptosis. Patients and methods Four women presenting with symptomatic right nephroptosis underwent a percutaneous endoscopic nephropexy. An upper-pole calyx was accessed percutaneously and a 24-F working sheath was placed. Another needle access was made through a lower-pole calyx and a #2 polyglactin suture was passed into the renal pelvis. It was then pulled out through the upper-pole tract using the nephroscope. A retroperitoneoscopy was performed and the tip of the nephroscope was used to cause nephrolysis. After inserting the nephrostomy tube the polyglactin suture was passed into the subcutaneous tissue and then tied without too much tension, to avoid cutting the parenchyma. Results The operative duration was 33 min and the hospital stay after surgery was 3.5 days. The nephrostomy catheter was removed 5 days after surgery. There were no complications, especially no haemorrhagic, infectious, lithiasic or thoracic complications. The four patients were relieved of their initial symptoms, with a mean follow-up of 28 months. Ultrasonography and/or intravenous urography showed the kidney at a higher location with the patient standing. Conclusions This technique combines the nephrostomy tract used in percutaneous techniques with the suture and nephrolysis used in laparoscopic techniques. Moreover, this procedure seems to be safe, with satisfactory anatomical and clinical results and a lower morbidity. However, a larger series will be necessary to establish its long-term morbidity and success rate.

We present a technique of transurethral transvesical endoscopic management of an intraperitoneal retrovesical hydatid cyst. Cystoscopy is performed using a 20.8F nephroscope. The cyst is punctured using an 18-gauge, 36-cm needle through the nephroscope operating channel. Saline (20%) is used as scolicidal agent. After balloon tract dilation, the nephroscope is introduced into the cyst, and the hydatid material is evacuated.

We present a technique to replace a 24F with a 30F working sheath during percutaneous renal surgery, for larger stone fragments extraction. To gain time, the 30F sheath is directly glided over the 24F sheath under endoscopic control, without radiation exposure.

Objectives To present our experience with percutaneous nephrolithotomy (PCNL) in the split-leg modified lateral position. Material and Method The patient is placed with the thorax in the lateral position and the pelvis in an oblique position. Then the lower limbs are split and bent in the lowest position. Initial placement of a retrograde ureteral catheter, tract formation, stone fragmentation and retrieval, and optional extra procedures were accomplished with the patient in the same position. Results PCNL in the split-leg modified lateral position resulted in decreased operating room time, less manipulation of the anesthetized patient, and maintaining the sterility of the retrograde ureteral catheter. In addition, it allowed simultaneous antegrade and retrograde endoscopic approach to the upper urinary tract. Ureteral catheter placement, PCNL, and associated procedures were possible in all patients. Adjunct procedures were internal urethrotomy, transurethral resection of the prostate, rigid and flexible ureteroscopy, and endopyelotomy or endopyeloplasty. Conclusions Performing PCNL in the split-leg modified lateral position has several advantages for the patient and the urologist, with greater versatility of stone manipulation along the entire urinary tract.

The glove model is simple to set up, with a preparation time of about a few minutes. It is economical, by using widely available material. It can be performed in the operating room, since it is a non-biological model, so there is no risk of biologic contamination, nor any hygiene or sterilization issue. Percutaneous puncture and limited dilation were possible in all the fingers and with the same techniques used in our real percutaneous access [8, 9]. The needle mobilization and puncture of the glove’s fingertip are similar to the feeling of the calyx puncture. The fingers have to be tightly filled, in order to be firm to the needle puncture. Otherwise, if the finger is flaccid the needle will push the fingertip without puncturing it. Moreover, dilation and Amplatz sheath insertion are difficult or impossible; the finger is pushed by the dilator. It is a non-biological model so there is a limitation in terms of "tissue feeling" and for anatomic relations. In addition, ultrasound-imaging guidance cannot be used. Evaluations submitted by training session participants revealed a high degree of satisfaction with model effectiveness in the application of percutaneous calyx puncture. Also, for learning of calyx puncture we have a model without using radiation exposure that can be used in the classroom; it is using a retroprojector with the principle of Chinese-shadows. Model-based training that provides experience in percutaneous renal access may reduce the learning curve associated with the procedure and increase the safety profile for beginners as they gain experience on patients [2, 3]. Thus, many biological models with porcine kidney and virtual reality simulator have been developed, to facilitate the learning and training of percutaneous renal surgery [3, 4-7]. We present a cheaper and easily available model using a latex glove for percutaneous calyx puncture.
