Objective To better understand the risks of bisphosphonates in order to develop guidance for appropriate clinical usage, to compared femoral fracture healing at different time points and to explore the effects of Residronate on fracture healing. Methods Osteoporosis model was achieved by ovariectomy surgery, followed by surgical incision of left femoral shaft 4 weeks after ovariectomy surgery. Three days after fracture surgery, risedronateor saline was fed by intragastric administration. X ray examination was used to check the callus formation, Bone Mineral Density (BMD), Bone Mineral Content (BMC), biomechanical, imaging and micromorphological of bone tissue as well as the trabecular bone parameters were all examined. The femoral pathology tissue of each rat was used to analyze trabecular bone parameters, including trabecular bone volume/tissue volume (Tb. BV/TV), bone surface to tissue volume ratio (BS/TV), trabecular bone mineral density (Tb. BMD), trabecular bone number (Tb. N), trabecular bone thickness (Tb. Th) and small bone Trabecular bone space (Tb. Sp). Results Via X‐ray and pathologically, risedronate treatment promoted the callus forming at the fracture site during the following 6 weeks after osteoporotic fracture by X‐ray (P, Using a rat osteoporotic model and controls we compared femoral fracture healing at different time points to explore the effects of Residronate on fracture healing at different time points measured: (i) via x‐ray and pathologically; (ii) through evaluation of bone mineral density and bone mineral content; and (iii) through load testing. Our results demonstrate that Risedronate could promote fracture healing at the first 3 weeks but delayed healing at the following 3 weeks. During the first 3 weeks, risedronate could obviously increase the BMD, decrease the BMC and delay the decrease of BMC of the fracture site. During the healing process of osteoporotic fracture rats, risedronate can improve the structural strength and mechanical index of newborn callus. It strongly recommends that longer than 7 weeks usage of third generation bisphosphonate of risedronate does not contribute to osteoporotic fracture.

Femoral head osteonecrosis, also known as avascular necrosis, is a disease with a multifactorial etiology, characterized by a profound change of bone architecture, which leads to the diminishing of bone resistance and femoral head collapse. The main causes that lead to femoral head necrosis are represented by the decrease of local blood perfusion and increase of intraosseous pressure, because of an excessive development of adipose tissue in the areolas of the trabecular bone tissue in the femoral head. The histopathological and immunohistochemical (IHC) study performed by us showed that most of bone trabeculae were damaged by necrotic-involutive processes, their sizes being reduced, both regarding their length and their diameter; generally, the spans were thin, fragmented, distanced among them, which led to the occurrence of some large areolar cavities, full of conjunctive tissue, rich in adipocytes. Some of the residual bone spans even presented microfractures. In the structure of the trabecular bone tissue, numerous cavities showed lack of content, which indicates the death of osteocytes inside, while the endosteum appeared very thin, with few osteoprogenitor, flattened, difficult to highlight cells. The IHC study showed a low reaction of the bone reparatory processes and a reduced multiplication capacity of bone cells involved in the remodeling and remake of the diseased bone tissue. Nevertheless, there were identified numerous young conjunctive cells (fibroblasts, myofibroblasts), positive to proliferating cell nuclear antigen (PCNA), cells that have a high capacity of multiplication, participating in the formation of a fibrous conjunctive tissue (sclerous) instead of the damaged bone trabeculae. The formation of fibrous conjunctive tissue causes the reduction of mechanical resistance of the femoral head and its collapse. The IHC study of the microvascularization in the femoral head damaged by aseptic osteonecrosis showed the presence of a very low vascular system, both in the residual bone trabeculae and in the sclerous conjunctive tissue. Of the inflammatory cells present in the spongy bone tissue of the femoral head affected by osteonecrosis, the most numerous ones were the macrophages. Both macrophages and T- and B-lymphocytes had a heterogenous distribution.

We report the peculiar case of a parosteal osteosarcoma arising beneath the periosteum in a 12-year-old boy. He complained of difficulty in left knee flexion. Plain radiography showed a uniformly dense mineralized mass in the bone cortex and parosteal ossified nodules at the metaphysis and diaphysis of the left distal femur. Periosteal reaction was not evident. Uniquely, plain radiography had a smooth outline and revealed gradually thickening mass toward the center. Histologically, the tumor showed a proliferation of spindle-shaped cells with parallel-oriented dense bone trabeculae and hyaline cartilaginous tissue disclosing mild atypia. The periosteum was inverted along the polypoid mass, but there was no periosteum at the top. Immunohistochemically, the spindle cells, including those at the top of the polypoid mass, and cartilaginous cells were positive for MDM2 and CDK4. MDM2 gene amplification was detected in these cells by fluorescence in situ hybridization. Despite the peculiar feature of plain radiography, the lesion was diagnosed as parosteal osteosarcoma. This case report presents a case of parosteal osteosarcoma arising beneath the periosteum, although it is postulated to arise in the outer layer of the periosteum. The unique radiographic findings in this case suggest an association of parosteal osteosarcoma with vigorous bone growth before closure of the growth plate.

Introduction Ossifying fibroma (OF) is a rare type of tumor characterized by fibrous tissue proliferation with cementum- or bone-like hard tissue formation. Since its first report by Montgomery in 1927, several cases of OF have been reported; however, no cases of OF arising from the zygomatic arch have been reported. Herein, we report a case of OF arising from the zygomatic arch. Clinical case summary A 70-year-old female visited our department in February 2017 because of a gradually growing osseous protrusion in the right zygomatic region, which she was aware of since the previous 6 months. A 3.3 cm × 3.2-cm area of swelling was observed in the region. Computed tomography confirmed the presence of a granulated lesion on the surface of the right zygomatic arch. Accordingly, benign bone tumor was diagnosed, and tumor resection was subsequently performed. Histopathological analysis revealed irregularly arranged bone trabeculae, an increased number of fibroblasts, and collagen fibers between the bone trabeculae; accordingly, OF was diagnosed. No clinical or radiographic evidence of recurrence was observed during the 1.5-year follow-up period. Discussion A granulated lesion was present on the surface of the right zygomatic arch, and the boundary between the lesion and surrounding bone was clear. Resection of the lesion from the zygomatic arch was relatively easy. Thus, OF was diagnosed. If OF is suspected, a risk of recurrence persists; therefore, shaving the area including the bones surrounding the lesion may be necessary. Although detailed causes of OF and osteoma remain unknown, past trauma has been indicated as a common etiology. However, compared with the frequency of fractures in the zygomatic arch, the frequency of OF and osteoma is rare; thus, the etiology of OF and osteoma remains to be fully elucidated.

Objective. To study the osteoprotective activity of glucosamine sulfate, strontium chloride, and their combinations on a model of experimental osteoporosis in laboratory animals (OVX model) in comparison with the Strometta preparation and control (OVX without correction). Materials and methods. The material for the study was female Wistar rats weighing 250 ± 30 g. To simulate osteoporosis, bilateral ovariectomy was performed. Microcirculation was evaluated by Doppler flowmetry. Digital X-ray densitometry was used to determine bone density. Using histological morphometry, the transverse size of the trabeculae of the spongy bone tissue was studied. Results. In such parameters as bone density, microcirculation, and the average width of bone trabeculae, significant differences were established compared with the control (OVX without correction) for groups with correction of glucosamine sulfate, strontium chloride, and their combination, and thus their osteoprotective effect was confirmed. Conclusion. In the future, glucosamine sulfate, strontium chloride, and their combination can be used for drug prevention of osteoporosis in the postmenopausal period. However, for the introduction of strontium chloride and its combination with glucosamine sulfate into clinical practice, it is necessary to conduct complete preclinical and clinical trials.

Osseous metaplasia of the breast is considered to be one of the rare breast conditions. It is characterized by the presence of normal bone tissue within the breast. We report a case of benign osseous metaplasia in a 58-year-old woman presenting with mastalgia and mass in the right breast. A lumpectomy was performed. On the pathological examination, the mass was composed completely of benign bone trabeculae with no epithelial component which was confirmed by negativity of immunohistochemistry.

Extrapulmonary complications of different organ systems have been increasingly recognized in patients with severe or chronic Coronavirus Disease 2019 (COVID-19). However, limited information on the skeletal complications of COVID-19 is known, even though inflammatory diseases of the respiratory tract have been known to perturb bone metabolism and cause pathological bone loss. In this study, we characterized the effects of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection on bone metabolism in an established golden Syrian hamster model for COVID-19. SARS-CoV-2 causes significant multifocal loss of bone trabeculae in the long bones and lumbar vertebrae of all infected hamsters. The bone loss progressively worsens from the acute phase to the post-recovery phase. Mechanistically, the bone loss was associated with SARS-CoV-2-induced cytokine dysregulation which upregulates osteoclastic differentiation of monocyte-macrophage lineage. The pro-inflammatory cytokines further trigger a second wave of cytokine storm in the skeletal tissues to augment their pro-osteoclastogenesis effect. Our findings in this established hamster model suggest that pathological bone loss may be a neglected complication which warrants more extensive investigations during the long-term follow-up of COVID-19 patients. The benefits of potential prophylactic and therapeutic interventions against pathological bone loss should be further evaluated. Background: The etiology and natural history of Kienböck’s disease remain unclear. Traditionally it has been defined as avascular necrosis of the lunate. The authors tried to demonstrate different tissue distribution, the area ratio of necrotic tissue and vessel counts inside the whole Kienböck lunate to reveal a dynamic process of the lunate collapse. Methods: Five lunates from patients with stage III Kienböck’s disease and one cadaveric lunate not involved by Kienböck’s disease were sampled. They were sectioned, H&E stained, and evaluated. The thickness of trabecular bone and the area of necrotic tissue were measured with Image-Pro Plus. The number of vessels was counted manually. Results: In the normal lunate, the bone trabeculae showed a uniform distribution with fatty marrow filled the interspace between the trabeculae. In the lunates with Kienböck disease, the trabeculae fracture and necrosis located in the central part with massive fibrous granular tissue proliferation. There were also some chondroid metaplasia at the palmar and dorsal ends. The trabeculae of the lunates of the Kienböck’s disease [0.188 mm (0.153 mm, 0.236 mm)] was significantly thicker than the normal lunates [0.146 mm (0.124 mm, 0.164 mm)]. The necrosis was localized around the fracture sites instead of the whole lunate. The mean necrosis area only accounts for 16.3% ± 8.9% of the whole section. Such kind of focal necrosis is quite similar to those around the traumatic fracture ends of other bones. Even in stage III Kienböck lunates, the vessels are quite abundant (221 ± 42 in one sagittal section), while the vessels inside the normal lunate were 352 ± 28. Conclusions: There is neither massive nor obvious generalized avascular bone necrosis in our histopathology observations. The focal necrosis and vessel damage were more likely associated with the broken trabeculae inside the lunate. Based on our histopathology observations, we suggested that the progressive process of Kienböck’s disease could be described as lunate nonunion advanced collapse instead of avascular necrosis.

We carried out a differential diagnosis of a large frontoparietal lesion on a human skull from a Late Bronze Age archaeological site located on the Central Plain of China, dating to between 771 and 476 BC. The head of this individual was covered in cinnabar, a mercury-based pigment that later was used for medicinal purposes in China. The lesion was well-circumscribed and involved the outer and inner tables of the skull, slight diploë thickening, and coarsening of bone trabeculae with expansion of intertrabecular spaces. We show that the observed changes are most consistent with cavernous hemangioma of the skull, a benign vascular malformation that preferentially affects older adults. Hemangiomas are often neglected in the paleopathological literature because of their benign nature - they tend to be asymptomatic and do not affect quality of life to a significant degree. Nevertheless, they produce characteristic lesions that can be confused with several other conditions with unrelated etiologies, including congenital hemoglabinopathies, traumas, malignant or benign neoplasms, and Paget's disease. We outline the diagnostic criteria that distinguish cavernous hemangioma from other conditions affecting the skull.

Percutaneous vertebroplasty (PVP) is a minimally invasive intervention widely used for relief of pain caused by osteoporotic, traumatic, or neoplastic vertebral fractures. During PVP, polymethyl methacrylate (bone cement) is injected directly into the vertebral body via a vertebral pedicle to partially restore the vertebral body height, to stabilize bone trabeculae and to alleviate pain. Though the procedure is minimally invasive, complications are not rare; however, mostly, they are not clinically significant. Potentially serious complications of PVP include pulmonary embolism with polymethyl methacrylate with variety of manifestations ranging from asymptomatic postoperative x-ray findings to life-threatening extravasation (cement "leakage") which is quite rare and fatal. Epidemiology, pathogenesis, clinical and radiological signs of pulmonary embolism in PVP and approaches to treatment are discussed in this review of literature. The article also describes two clinical cases of pulmonary embolism in patients underwent PVP for hemangioma of the vertebral body with severe pain syndrome are also described.

Three outbreaks of osseous malformation in claves born of cows fed apple pomace throughout pregnancy are described. This study was carried out from historical surveys on properties where apple pomace was used in cattle feeding. The outbreaks occurred in the municipalities of Lages, Santa Catarina state, from 2007 to 2012, and Ipê, Rio Grande do Sul state, from 2011 to 2012. These calves presented bone deformity characterized mainly by arthrogryposis and chondrodysplasia of the anterior limbs. At necropsy, the observed changes were restricted to bending and thickening of the anterior limbs and skull. Microscopically, a disorganized, irregular, misaligned hypertrophic zone was observed in the humeral-radio-ulnar joints, in addition to absence of growth zone, thin bone trabeculae, and closed epiphyseal growth plate. RESUMO: Descrevem-se 3 surtos de malformações ósseas em bezerros, nascidos de vacas prenhes alimentadas com bagaço de maçã. Estudos foram realizados a partir de levantamento de históricos em propriedades onde bagaço de maçã foi utilizado na alimentação de bovinos. Os surtos ocorreram nos municípios de Lages em Santa Catarina, de 2007 a 2012, e Ipê em Rio Grande do Sul, de 2011 a 2012. Dentre 20 e 69,2% dos bezerros apresentavam deformidade de membros, caracterizada principalmente por membros anteriores curvos e curtos. Na necropsia as alterações observadas restringiam-se ao encurvamento e engrossamento dos membros e do crânio. Microscopicamente nas articulações úmero-radio-ulnar, foi observado zona hipertrófica desorganizada, irregular e sem alinhamento, além de ausência da zona de crescimento e trabéculas ósseas finas e placa epifisária fechada.

Cemento-osseous dysplasia is a benign lesion which affects the jaw bones. It is a frequent incidental finding but no aggressive therapy is necessary. However, it may be confused with more aggressive entities, such as ossifying and cementifying fibroma, which do require treatment. There are no clear-cut histopathological criteria to differentiate between these entities. We present a case of a 13-year old boy who underwent a biopsy of a clinically undiagnosed maxillary lesion. Histopathological analysis revealed moderately cellular fibrous tissue with bone trabeculae with no osteoblastic rimming and clusters of intensely basophilic material corresponding to cementum. The final diagnosis was cemento-osseus dysplasia. The main histopathological features important in the distinction of these entities are discussed.

Cleidocranial dysplasia (CCD) is an autosomal dominant disorder caused by heterozygous mutations in RUNX2. Affected individuals exhibit delayed maturation or hypoplasia in various bones, mainly including those formed by intramembranous ossification. Although several reports described deformation of the sphenoid bone in CCD patients, details of the associated changes have not been well documented. Most parts of the sphenoid bone are formed by endochondral ossification; however, the medial pterygoid process is formed by intramembranous ossification associated with secondary cartilage. We first investigated histological changes in the medial pterygoid process during different developmental stages in Runx2+/+ and Runx2+/- mice, finding that mesenchymal cell condensation of the anlage of this structure was delayed in Runx2+/- mice as compared with that in Runx2+/+ mice. Additionally, in Runx2+/+ mice, Osterix-positive osteoblastic cells appeared at the upper region of the anlage of the medial pterygoid process, and bone trabeculae appeared to associate with subsequent secondary cartilage formation. By contrast, few Osterix-positive osteoblastic cells appeared at the upper region of the anlage of the medial pterygoid process, and no bone trabeculae appeared thereafter in Runx2+/- mice. At more advanced embryonic stages, endochondral ossification occurred at the lower part of the medial pterygoid process in both Runx2+/+ and Runx2+/- mice. After birth, well-developed bone trabeculae occupied two-thirds of the cranial side of the medial pterygoid process, and cartilage appeared beneath these bones in Runx2+/+ mice, whereas thin trabecular bone appeared at the center of the cartilage of the medial pterygoid process in Runx2+/- mice. In adult mice, the body and medial pterygoid processes of the sphenoid bone comprised mature bones in both Runx2+/+ and Runx2+/- mice, although the axial length of the medial pterygoid processes was apparently lower in Runx2+/-mice as compared with that in Runx2+/+mice based on histological and micro-computed tomography (CT) examinations. Moreover, medical-CT examination revealed that in CCD patients, the medial pterygoid process of sphenoid bone was significantly shorter relative to that in healthy young adults. These results demonstrated that the medial pterygoid process of the sphenoid bone specifically exhibited hypoplasia in CCD.

Osteoporosis is a systemic bone disease that results in the loss of bone mass and impared bone structure. Animal models for osteoporosis are generated by ovariectomy. Adult female Sprague-Dawley rats were divided into control, bilateral ovariectomy and bilateral ovariectomy and subcutaneous nicotine administered (received nicotine sulphate, 2 mg/kg) groups, daily for 28 days. At the end of the period, rats were sacrified under anesthesia blood samples were taken and femoral tissues were dissected. Estrogen, calcium and alkaline phosphatase levels were measured. Tissue samples were prepared for histopathogical examination. Sections were stained with Hematoxylin and Eosin and examined under light microcope. Biochemical parameters were decreased depending on the overiectomy, also decrement was notable with nicotine intake. In the ovariectomy group; increased inflammatory cells with degenerative changes around the femoral compact bone and dilatation of osteon structures in bone trabeculae and apoptotic changes in osteocyte cells in bone lacuna were apparent. In the ovariectomy with nicotine administration group, excessive dilatation of the havers lamellae in the compact bone region, increased osteoclastic activity, picnosis and apoptotic nucleus of the osteoclastic cells located in the lacuna, and increased collagen fibers in the matrix were observed. We suggest that ovariectomy and nicotine administration together effect estrogen and calcium metabolism negatively, stimulate alterations in the structural properties of bone matrix, also affect osteocyte development and bone lamellar structure that may accelerate osteoporosis development.

Objective: Diabetes Mellitus is well known to be associated with several oral complications. Thus, this study investigated the effect of diabetes on the cementum-periodontal interface by light and scanning electron microscopy. Methodology: This investigation was carried out on twenty eight male albino rats weighing from 200 to 220 gm; rats were divided into two groups: group I (control): fourteen animals received intraperitoneal single dose of 1 ml Citrate buffer, group II (diabetic): fourteen animals that were rendered diabetic by intraperitoneal single dose of streptozotocin 40 mg/kg body weight dissolved in 1 ml Citrate buffer and sacrificed 3 weeks after detection of diabetes. Plasma glucose level >300 mg/dl confirmed diabetes after 3 days. Half of lower jaws specimens were processed for H&E examination by light microscopy of cementum-periodontal interface. From the other half of specimens; extracted mandibular first molars were examined by SEM for changes of cementum surfaces. Results: Comparing to control group, diabetic rats showed periodontal fibers disorganization and degeneration with loss of Sharpey's fibers attachments. Increased cementoid, resorptive areas of both cementum surface and alveolar bone were evident in addition to the alterations of bone trabeculae. Conclusions: Diabetes mellitus was associated with variable deleterious effects on periodontium. The histological and scanning electron microscopy changes were most obviously on PDL and least on cementum. Background We report a case of osteitis fibrosa cystica, a rare benign resorptive bone lesion caused by hyperparathyroidism, that presented on imaging as an aggressive bone tumor. Case presentation The patient is a 51-year-old male complaining of severe sustained pain of the right hip region. Imaging studies were suspicious for a malignant tumor of the right iliac bone. Biopsy under CT guidance was performed and showed remodeled bone trabeculae with numerous osteoclasts, excluding bone tumor and raising the possibility of osteitis fibrosa cystica. Complementary tests disclosed elevated blood level of parathyroid hormone and a partially cystic enlarged left inferior parathyroid gland consistent with adenoma. After parathyroidectomy, the clinical symptoms were relieved and the radiological findings were significantly improved, which confirmed the diagnosis. Conclusions Metabolic diseases-associated bone lesions should always be considered in the differential diagnosis of bone tumors, to avoid unnecessary surgeries and treatments.

Background The fibula is only indirectly involved in the composition of the human knee joint and has therefore been neglected in the research on knee osteoarthritis. Nonuniform settlement of the proximal tibia plateau is clinically defined as when the height of the medial tibial plateau is lower than that of the lateral side in medial compartment knee osteoarthritis (KOA). The non-uniform settlement of the proximal tibia plateau may be caused by fibular support on the lateral side. Orthopedic surgeons practice partial fibulectomy based on the clinical manifestation of nonuniform settlement, and this technique has been shown to reduce pain and improve function in patients with medial compartment KOA. However, this hypothesis of the mechanism of nonuniform settlement lacks an anatomical basis. Methods The P45 polyester plastination technique was used to prepare sections of the proximal tibiofibular joint to investigate the distribution of the bone trabeculae in the region of the lateral tibial plateau. Results There was uneven distribution of trabeculae in the lateral condyle of the tibia and the head and neck of the fibula. The fibula and the posterolateral cortex of the shaft of the tibia united to form an arch beam via the tibiofibular joint. Many thick, dense trabeculae were present in a longitudinal direction above the tibiofibular arch. Conclusions The fibula supports the lateral tibial plateau, and the trabeculae were concentrated above the tibiofibular arch.

Background : Craniomandibular osteopathy (CMO) is a degenerative, osteopetrotic, and self-limiting bone disease that is rare in the canine species. The most affected bones and joints are the branches of the mandible, tympanic bulla, and temporomandibular joint (TMJ) with generalized thickening of the bone trabeculae, increased lines of bone cementation, and bone proliferation. The aim of the present study was to evaluate 2 cases of CMO in mixed-breed dogs belonging to the same litter sent to the Laboratorio Regional de Diagnostico, Faculdade de Veterinaria, Universidade Federal de Pelotas (LRD / FV / UFPel). Cases : Three canines (mixed-breed, 2 female and 1 male) belonging to the same litter presented at eight months of age with increases in volume and pain in the face, difficulties in moving the jaw, anorexia, and sialorrhea. The 2 female dogs were euthanized and sent to LRD / FV / UFPel for necropsy. The animals underwent radiography of the left and right lateral and dorsoventral lateral projections in the imaging sector of the Hospital de Clinicas Veterinaria (HCV) at UFPel. During the necropsy, fragments of organs from the abdominal and thoracic cavities, as well as the central nervous system and head bones, were collected and fixed in 10% buffered formalin. The tissue samples were cleaved, processed routinely, embedded in paraffin, cut into sections of 3 µm thickness, and stained using hematoxylin and eosin (HE). The bone fragments were decalcified in a 50/50 solution of 8% hydrochloric acid and 8% formic acid and subsequently cleaved and stained with HE. The head and jaw of one of the animals was dissected and subjected to maceration in water at room temperature. Radiographic examination of the two remaining animals showed irregular and exuberant bone proliferation in the mandible and temporomandibular region. In the tympanic bullae, occipital bone, and zygomatic process, thickening of the structures was observed with increased opacity. On necropsy, the animals were cachectic with marked muscle atrophy and increased volume of the mandibular branches and head bones, in addition to marked edema in the adjacent subcutaneous tissue. The regional lymph nodes were enlarged, and in one of the animals, bilateral lesions in the femurs were also observed. In the head subjected to biological maceration, areas of bone proliferation and rarefaction were observed, with increased volume and destruction of the TMJ. On histopathological evaluation of the ventral portion of the mandibular bodies, a network of bony trabeculae was observed, composed of a chondroid matrix with different degrees of mineralization. The cortical portion of the bones exhibited loss of lamellar compactness, with greater spread of the bone trabeculae. The bone trabeculae were perpendicular to the long axis of the cortical of the mandible and were distributed in the medullary spaces, presenting some dense foci of inflammatory infiltrates composed predominantly of neutrophils, lymphocytes, and plasma cells. Discussion : The breeds most predisposed to developing CMO are the West Highland White Terrier and Scottish Terrier; however, the disease has also been described in other non-Terriers. Nevertheless, there are no descriptions in the literature of mixed-breed animals (SRD), with this report being the first of CMO in SRD dogs. The bones most frequently affected in CMO are those of the head. In Terrier dogs, the TMJ is also affected. In the cases of this study, severe CMO occurred, affecting both the head bones and TMJ, and in 1 of the animals, it also affected the femurs. Although CMO has not previously been reported in SRD dogs, the condition can seriously affect these animals, and differential diagnosis should always be made. Introduction: Distraction osteogenesis is defined as an operation that can lengthen bone gradually. Many complications may occur during this time, including refractures, local infections and loosening of the distractor device or pin. The most important disadvantage applies to the extended period of consolidation stage. Several methods have been tested for their ability to enhance new bone development, including the use of different adjuvant therapies, in order to minimize the time needed to complete the MDO phases Strontium is a medication used for treatment of osteoporosis. It's been shown that bone formation is promoted while bone resorption is reduced and in osteoporotic patients. Aim of this study: This study aimed to determine whether strontium citrate can be used in a rabbit model to enhance bone healing during MDO. Materials and Methods: Fourteen rabbits weighing 3.0-3.5kg were divided into 2 groups: group I; (control group), group II; (received strontium citrate). Surgical procedures to place the distractor system were performed in both groups. After 5 days of latency period, the distractor was activated. After 4 weeks all rabbits were euthanized and the distracted mandibles were surgically removed and prepared for light microscopic examination and histomorphometry. Results: Comparing to the control group, histological results revealed thicker bone trabeculae in the study group with narrower bone marrow. The histomorphometric analysis also showed significantly higher percentage of new bone per field in the study group. Conclusions: Strontium citrate supplementation could enhance bone consolidation during distraction osteogenesis.

Ossifying fibro myxoid tumor (OFMT) is a rare soft tissue neoplasm and may present diagnostic difficulty. Only a few cases of oral cavity are described. A 49 –year-old male presented with a mass in the vestibular region of the mandible middle line, complaining of pain for the last three months. The mass contains spindle and polygonal mesenchymal cells on the myxoid ground in histochemical examinations. The tumor had a partial fibrous capsule surrounded by a non-metaplastic bone trabeculae. Histopathological examination was typical for OFMT. Although most cases of OFMT follow a benign course, recurrence and metastasis have been reported. Therefore, this type of tumor should be followed carefully.

Primary sclerosing epithelioid fibrosarcoma (SEF) of bone is a rare and scarcely reported neoplasm. We document clinicopathological and molecular features of 9 additional cases. Five males and 4 females had a mean age of 39 years (14-71 years). Most tumors affected flat/irregular bones; only 3 cases involved a long bone. By radiology, it has characteristic radiographic features of a predominantly lytic expansile lesion with a sclerotic rim. Referring diagnoses were SEF (n = 2), low-grade osteosarcoma (n = 2), chondrosarcoma (n = 1), and chondromyxoid fibroma (n = 1). Histologically, five cases revealed classical morphology of SEF of soft tissue. Remaining cases were classified as hybrid SEF/low-grade fibromyxoid sarcoma, characterized by spindle or stellate cells, prominent stroma, and giant hyalinized areas. Various morphological deviations such as prominent vasculature (n = 3), osteoid-like material (n = 4), or parallel bone trabeculae (n = 2) were observed. Immunohistochemically, all cases showed diffuse and strong MUC4 expression. SATB2 was observed in 5/8 cases. Using FISH, EWSR1, and FUS rearrangements were detected in 4 cases and 1 case, respectively. EWSR1-CREB3L1 fusion was identified in 1 additional case by next-generation sequencing. Recurrence and metastasis were observed in 1 case and 2 cases, respectively. All but one patient were alive with disease for a mean interval of 31 months. SEF of bone is a relatively indolent sarcoma of adults, most commonly located in the flat/irregular bones. Due to overlapping histological features, it is often misdiagnosed as osteosarcoma or a chondroid tumor. Most SEF of bone exhibit EWSR1 rearrangements, but rare cases may harbor a FUS gene fusion.

The main goal in the treatment of large bone defects is to guarantee a rapid loading of the affected limb. In this paper, the authors proposed a new reconstructive technique that proved to be suitable to reach this purpose through the use of a custom-made biomimetic porous titanium scaffold. An in vivo study was undertaken where a complete critical defect was experimentally created in the diaphysis of the right tibia of twelve sheep and replaced with a five-centimeter porous scaffold of electron beam melting (EBM)-sintered titanium alloy (EBM group n = 6) or a porous hydroxyapatite scaffold (CONTROL group, n = 6). After surgery, the sheep were allowed to move freely in the barns. The outcome was monitored for up to 12 months by periodical X-ray and clinical examination. All animals in the CONTROL group were euthanized for humane reasons within the first month after surgery due to the onset of plate bending due to mechanical overload. Nine months after surgery, X-ray imaging showed the complete integration of the titanium implant in the tibia diaphysis and remodeling of the periosteal callus, with a well-defined cortical bone. At 12 months, sheep were euthanized, and the tibia were harvested and subjected to histological analysis. This showed bone tissue formations with bone trabeculae bridging titanium trabeculae, evidencing an optimal tissue-metal interaction. Our results show that EBM-sintered titanium devices, if used to repair critical bone defects in a large animal model, can guarantee immediate body weight-bearing, a rapid functional recovery, and a good osseointegration. The porous hydroxyapatite scaffolds proved to be not suitable in this model of large bone defect due to their known poor mechanical properties.

Therefore, this study tested the hypothesis that the use of a bioactive coating (by plasma electrolytic oxidation, PEO) on titanium microplates could improve the fracture healing of low bone mineral density (BMD) rats. Thirty female rats underwent bilateral ovariectomy surgery (OVX), and 35 rats underwent fake surgery (SHAM). Three months later, animals were subjected to femoral fracture simulation and were fixed with either non-coated (CONV) or coated (PEO) titanium miniplates. Eight weeks postoperatively, microplate/bone complexes were analyzed through computed microtomography, histometric, confocal microscopy, molecular, and biomechanical analysis. Bioactive elements (Ca and P) were incorporated on the PEO microplate and the surface was modified in a volcano-like structure. In the microCT analysis the OVX/PEO group had greater values for Tb.Th (bone trabecular thickness), Tb.Sp (separation of bone trabeculae) and Tb.N (number of trabeculae) parameters compared to the OVX/CONV group. According to histometric analysis, the OVX/PEO group showed significantly higher new bone formation than the OVX/CONV group (P < 0.05). For the fluorochrome area, the OVX groups (PEO and CONV) showed greater values for calcein precipitation (old bone) than alizarin red (new bone). Molecular results showed greater values for proteins related to the final phase of bone formation (P < 0.05) in the OVX/PEO group. The OVX/PEO group showed higher bone/miniplate system resilience compared to the others (P < 0.05). It was concluded that PEO coating optimizes bone healing on simulated femoral fractures in low bone mineral density rats. Objective The healing of osteoporotic fractures in the elderly patients is a difficult clinical problem. Currently, based on the internal fixation of fractures, the available drug treatments mainly focus on either inhibiting osteoclast function, such as bisphosphonate, calcitonin, oestrogen or promoting osteogenesis, such as parathyroid hormones. However, the availability of current antiosteoporotic drugs in promoting osteoporotic fracture healing is limited. The objective of the present study was to investigate the ability of the MiR-21/nanocapsule to enhance the early bone repair of osteoporotic fractures. Methods Based on the presence of matrix metalloproteinases that are overexpressed at the fracture site, we designed the matrix metalloproteinase–sensitive nanocapsules which were formed by in situ free radical polymerisation on the surface of MiR-21 with 2-(methacryloyloxy) ethyl phosphorylcholine and the bisacryloylated VPLGVRTK peptide. The MiR-21/nanocapsule [n (miR-21)] and O-carboxymethyl chitosan (CMCS) were mixed until they formed a gel-like material [CMCS/n (miR-21)] with good fluidity and injectability. Thirty elderly Sprague Dawley (SD) rats (female, 14-month-old, 380 ± 10 g) were subjected to bilateral removal of the ovaries (ovariectomised). All rats were subjected to bilateral bone defects (2 mm diameter) of the proximal tibia and randomly divided into three groups (groups A, B, and C): separately injected with CMCS/n (miR-21), CMCS/n (NC-miR), and saline. Micro-computed tomography (CT) imaging was performed to evaluate newly formed bone volume and connectivity. Nondecalcified histology and toluidine blue staining were performed to measure the effects of CMCS/n (miR-21) on bone repair. In vitro, the effect of n (miR-21) on osteogenic differentiation to bone marrow mesenchymal stem cells (BMSCs) which derived from the ovariectomised rat model was observed. Results The morphology of n (miR-21) was a regular spherical nanocapsule with a uniform small size (25–35 nm). The results confirmed that n (miR-21) could be efficiently phagocytosed by BMSCs and released in the cytoplasm to promote osteogenesis. The expression level of alkaline phosphatase and Runt-related transcription factor 2 mRNA in the n (miR-21) group was higher than that in the n (NC-miR) group. Animal experiments proved that CMCS/n (miR-21) produced better bone repair compared with the CMCS/n (NC-miR) group in the early stages of fracture healing at 4 weeks. In the late stage of fracture healing (8 weeks), micro-CT quantitative analysis showed that the new bone trabeculae in the CMCS/n (miR-21) group has decreased compared with the CMCS/n (NC-miR) group. In the CMCS/n (miR-21) group, the new cancellous bone had been absorbed, and the process of bone healing was almost completed. In contrast, the new bone in the CMCS/n (NC-miR) and the control groups was still in the healing process. Conclusion The cytological tests confirmed that n (miR-21) can promote osteogenic differentiation of BMSCs derived from the osteoporosis rat model. Furthermore, the results of animal tests demonstrated that local injection of CMCS/n (miR-21) promoted the early healing of osteoporotic bone defects. Consequently CMCS/n (miR-21) promoted the bone repair process to enter the moulding phase earlier. The translational potential of this article CMCS/n (miR-21) can be widely applied to elderly patients with osteoporotic fractures. This method can help patients with osteoporotic fractures recover earlier and avoid serious complications. It provides a potential approach for the clinical treatment of osteoporotic fractures in the elderly.

Background: Nanotechnology is a rapidly growing scope in recent research, it has a widely acceptable range of potential implications in humans, particularly for dentistry. Among various metallic nanoparticles, silver nanoparticles (AgNPs) are considered the most animated and charming nanoparticles that are involved in multiple biomedical applications. Aim: The goal of the current study was to determine the possible biological reparative influences of silver nanoparticles on age changes of the alveolar bone, cementum and periodontal ligaments of rats Material and methods: Forty five male albino rats (fifteen adults aging six months and thirty old aging eighteen months) were divided equally into three groups. Group I (adult age) that get infused with saline intravenously to serve as control group, group II (old age) which also get infused with saline intravenously and group III (old age received an oral dose of silver nanoparticles 100 mg/kg) once daily for 4 weeks. Results: Old age group revealed sever histological alterations as thinning of bone trabeculae and widening of bone marrow cavities. Marked hypercementosis with multiple signs of degeneration in cementum, periodontal ligaments (PDL) and alveolar bone were evident, while old group received silver nanoparticles revealed a considerable improvement of histological features of the tissues under investigation which became nearly similar to that of control group. DEXA examination revealed statistical significant difference in the alveolar bone mineral density between different groups. Conclusion: Oral administration of silver nanoparticles considerably ameliorated most of histological features of alveolar bone, cementum and periodontal ligaments of old rats. Bone mineral density of old rats was improved by silver nanoparticles.

This study focuses on mechanisms which regulate the process of fracture healing. We studied the form and position of the fibrin-blood clots (FBC) in the zone near the fragments among patients with limb fractures: macroscopically (during open surgical operations to reposition bone fragments) and using sonography. We conducted a histological and immuno-histochemical analysis of biopsy material obtained from the zone around the fracture during surgical procedures on 16 patients in 1–18 days after the fracture. We determined that the density of FBC and their form depends on the volume of damage to the periosteum-muscular fascia and the extent of the shift of fragments. In most cases, with a closed fracture, fibrin-blood clots had a spindle-shaped form. Fibrin along the periphery and in the zone between the fragments has a dense structure, and becomes cellular in central zones. The cells surrounded by fibrin partitions contain blood cells and serum. In many places, fibrin partitions had a one-direction orientation position, and the cells were oval-elongated, which indicated hydrostatic pressure in them. Proliferation of mesenchymal cells began in the vital tissues around the FBC, then during reproduction they pentrated to fibrin. Lengthwise axis of the cells was parallel to fibrin partitions. The bone trabeculae which form on the third week after fracture repeated the orientation of the fibrin partitions. It was determined that the vascular endothelial growth factor (VGEF) concentrates in fibrin and remains in it over the first week after the fracture, later it was found in endotheliocytes, fibroblasts and osteoblasts. The process of filling of the cells with around-fracture FBC lasted 12–18 days and during this period, their osteogenic differentiation occurred. Such tempi and orientation of the process is caused by fibrin with a concentration of growth factors in it. Using the results of the study, it could be assumed that the main conditions for osteogenic differentiation of cells are high concentration of VGEF in the fibrin, which initiates neoangiogenesis and internal tension of fibrin partitions. The formation of structured FBC around the ends of the fragments, which contain VGEF should be considered an initial stage of the process of forming of bone regeneration.

The healing capacity of bone marrow mesenchymal stem cells (BMMSCs) has been evaluated in various studies. This study aimed to evaluate the effect of BMMSCs on the healing of temporomandibular joints (TMJs) with induced rheumatoid arthritis. Fifty healthy male Sprague Dawley rats were divided into three groups: group I (n = 10), negative control; group II (n = 20), positive control (induction of arthritis by adjuvant followed by intravenous injection of 0.1 ml of PBS); and group III (n = 20), intervention (as for group II but injected intravenously with 1 × 106 cells ml-1 of BMMSCs suspended in PBS). Half of the rats in each group were euthanized 3 wk after the start of the experiment and the other half was euthanized after 5 wk. Group I revealed normal TMJ features. Group II showed thickening of disc, thinning of cartilage, disordered bone trabeculae, and decreased in mean % area staining positive of collagen fibers at 3 wk, while at 5 wk these effects were more aggravated. Group III showed nearly normal thickness of disc and condylar cartilage, nearly normal arrangement of bone trabeculae and regenerated collagen fibers at 3 wk, while after 5 wk the TMJ features were almost normal. Two-way anova revealed statistically significant differences between groups. Thus, treatment of induced rheumatoid arthritis with BMMSCs shows promising results that need to be further investigated in humans.

Objectives: The aim of the present study was to evaluate periodontal regeneration/ alveolar bone fill with the use of autologous bone marrow concentrate alone and with a combination of demineralized freeze dried allograft in the treatment of intrabony defects (IBDs). The evaluation was based on histological, immunohistochemical and radiographic analysis.Materials and Methods: Forty adult male albino rats with experimentally induced periodontitis were included in the present study and divided as follows, group I involved 10 rats and served as positive controls, group II involved 10 rats were treated with demineralized freeze dried allograft (DFDBA) only, group III involved 10 rats were treated with autologous bone marrow concentrate (BMSCs) only and group IV involved 10 rats were treated with combined demineralized freeze dried allograft and autologous bone marrow concentrate. Five rats of each group were sacrificed two weeks after the beginning of the experiment and the other 5 rats were sacrificed six weeks after the beginning of the experiment. Harvested jaw specimens were fixed in formalin, then embedded in paraffin wax, and serially sectioned at 4µm for histological, immunohistochemical assessment using of TGF-β1. Furthermore radiographic assessment was done at the end of study period.Results: The histological examination of control positive rats revealed degeneration of the collagen fibers associated with alveolar bone resorption. In group II DFDB allograft treated rats showed signs of PDL regeneration associated with increase in the thickness of bone trabeculae. BMSCs treated rats (group III) showed formation of new bundles of collagen fibers of PDL with increase in alveolar bone thickness. In group IV rats treated with combined DFDBA and BMSCs showed the best regenerative features in the PDL fibers and cells with more increase in the thickness of alveolar bone. Immunohistochemical results of TGF-β1 of group I showed negatively to weakly positive TGF-β1 reaction. While, PDL of group II presented moderately positive TGF-β1staining. PDL of group III revealed weakly to moderately positive TGF-β1 stain. However, group IV presented strongly positiveTGF-β1 staining. Radiographic analysis: showed a statistical significant difference between the three treated groups compared to the positive control group at 2 and 6 weeks respectively indicating that the bone density increased gradually in the three treated groups with the best radiographic evidence of increased bone density in group IV (treated with combined DFDBA and BMSCs).Conclusion: The present investigation revealed that all the treatment modalities showed favorable regeneration of the intrabony defect. The combination of DFDBA with BMSCs may positively influence the treatment of a periodontal attachment gain with enhancing resolution of osseous defects compared with other treatment modalities. Regenerative combination (DFDBA with BMSCs) emerging as a promising option in the treatment of periodontal osseous defects.

Background Hibernoma is a rare benign tumor of adults that is composed of multivacuolated adipocytes resembling brown fat cells. Hibernoma typically occurs in soft tissue, and intraosseous examples are very rare. Intraosseous hibernomas can radiologically mimic metastatic carcinoma and other tumorous conditions. Methods To collect the intraosseous hibernomas, we searched the pathologic database and reviewed the hematoxylin and eosin (H&E)-stained slides of bone biopsy samples performed to differentiate radiologically abnormal bone lesions from 2006 to 2016. A total of six intraosseous hibernoma cases were collected, and clinical and radiological information was verified from electronic medical records. H&E slide review and immunohistochemical staining for CD68, pan-cytokeratin, and S-100 protein were performed. Results Magnetic resonance imaging of intraosseous hibernomas showed low signal intensity with slightly hyperintense foci on T1 and intermediate to high signal intensity on T2 weighted images. Intraosseous hibernomas appeared as heterogeneous sclerotic lesions with trabecular thickening on computed tomography scans and revealed mild hypermetabolism on positron emission tomography scans. Histopathologically, the bone marrow space was replaced by sheets of multivacuolated, foamy adipocytes resembling brown fat cells, without destruction of bone trabeculae. In immunohistochemical analysis, the tumor cells were negative for CD68 and pan-cytokeratin and positive for S-100 protein. Conclusions Intraosseous hibernoma is very rare. This tumor can be overlooked due to its rarity and resemblance to bone marrow fat. Pathologists need to be aware of this entity to avoid misdiagnosis of this rare lesion.

PURPOSE: The hematopoietically active (or red) bone marrow is the target tissue assigned in skeletal dosimetry models for assessment of stochastic effects (leukemia induction) as well as tissue reactions (marrow toxicity). Active marrow, however, is in reality a surrogate tissue region for specific cell populations, namely the hematopoietic stem and progenitor cells. Present models of active marrow dosimetry implicitly assume that these cells are uniformly localized throughout the marrow spaces of trabecular spongiosa. Data from Watchman et al. and Bourke et al., however, clearly indicate that there is a substantial spatial concentration gradient of these cells with the highest concentrations localized near the bone trabeculae surfaces. The purpose of the present study was thus to explore the dosimetric implications of these spatial gradients on active marrow dosimetry. METHODS: Images of several bone sites from a 45-year female were retagged to group active marrow voxels into 50 μm increments of marrow depth, after which electron and alpha-particle depth-dependent specific absorbed fractions were computed for four source tissues – active marrow, inactive marrow, bone trabeculae volumes, and bone trabeculae surfaces. Corresponding depth-dependent S values (dose to a target tissue per decay in a source tissue) were computed and further weighted by the relative target cell concentration. These depth-weighted radionuclide S values were systematically compared to the more traditional volume-averaged radionuclide S values of the MIRD schema for both individual bones of the skeleton and their skeletal-averaged quantities. RESULTS: For both beta-emitters and alpha-emitters localized in the active and inactive marrow, depth-weighted S values were shown to differ from volume-averaged S values by only a few percent, as dose gradients across the marrow tissues are non-existent. For bone volume and bone surface sources of alpha-emitters and lower-energy beta-emitters, when marrow dose gradients are expected, explicit consideration of target cell spatial concentration gradients are shown to significantly impact marrow dosimetry. CONCLUSIONS: For medical isotopes currently utilized for treatment of skeletal metastases, namely (153)Sm and (223)Ra, accounting for hematopoietic stem and progenitor cell concentration gradients resulted in maximum percent differences to reference skeletal-averaged S values of ~21% and 55%, respectively.

Objective: The aim of the present study was to investigate the efficiency of zinc sulphate supplementation in treatment of the alveolar bone osteoporosis in rats induced by calcium deficient diet.Design: Thirty adult male Wistar rats with an average weight of 200-250 grams were randomized into 3 groups. Group Ι (control group): received standard rat chow, Group ΙΙ (osteoporotic group): received calcium deficient diet and Group ΙΙΙ (zinc treated group): received calcium deficient diet plus oral supplementation of zinc sulphate. After 3 months, the rats were sacrificed. The mandibles were examined histologically, histomorphometrically, ultrastructurally and by energy dispersive X ray microanalysis (EDX). Results: Histologically the impaired bone quality induced by calcium deficiency were significantly improved by zinc supplementation. The alveolar bone restored its structural organization in form of thick bone trabeculae, decrease number of osteoclasts and regularly distributed osteocytes. PURPOSE: The hematopoietically active tissues of skeletal bone marrow are a prime target for computational dosimetry given potential risks of leukemia and, at higher dose levels, acute marrow toxicity. The complex three-dimensional geometry of trabecular spongiosa, however, complicates schema for dose assessment in such a way that only a few reference skeletal models have been developed to date, and which are based upon microimaging of a limited number of cadaveric bone spongiosa cores. The question then arises as to what degree of accuracy is achievable from reference skeletal dose models when applied to individual patients or specific exposed populations? METHODS: Patient variability in marrow dosimetry were quantified for three skeletal sites - the ribs, lumbar vertebrae, and cranium – for the beta-emitters (45)Ca, (153)Sm, and (90)Y, and the alpha-particle emitters (223)Ra, (219)Rn, and (215)Po, the latter two being the immediate progeny of the former. For each radionuclide and bone site, three patient parameters were altered from their values in the reference model: (1) bone size as a surrogate for patient stature, (2) marrow cellularity as a surrogate for age- or disease-related changes in marrow adiposity, and (3) the trabecular bone volume fraction as a surrogate for bone mineral density. Marrow dose variability is expressed as percent differences in the radionuclide S value given by the reference model and the patient-parameterized model. The impact of radionuclide biokinetics on marrow dosimetry was not considered. RESULTS: Variations in overall bone size play a very minor role in active marrow dose variability. Marrow cellularity is a significant factor in dose variability for active marrow self-irradiation, but it plays no role for radionuclides localized to the trabecular bone matrix. Variations in trabecular bone volume fractions impact the active marrow dose variability for short-range particle emitters (45)Ca, (223)Ra, (219)Rn, and (215)Po in the vertebrae and ribs, skeletal sites with small spongiosa proportions of trabecular bone. In the cranium, with its relative high proportion of trabecular bone, significant differences in marrow dosimetry from the reference model were noted for all radionuclides. CONCLUSIONS: Skeletal models of active marrow dosimetry should be more fully parameterized to permit closer matching to patient bone density and marrow cellularity, particularly when considering short-range particle emitters localized to either the bone trabeculae or active marrow, respectively.

Hemangiomas of the rib are extremely rare benign neoplasm. Here we present a case in a 47-year-old female, detected by chest X-ray and underwent a surgical resection. Histologically, the tumor was composed of a homogeneous conglomerate, irregular, thin walled and dilated blood vessels containing red blood cells, supported by fibrous stroma and intermingled to regular bone trabeculae. The postoperative courses were uneventful, and there was no recurrence during 64 months follow-up.

Bone tissue is a biological system in which the dynamic processes of, among others, bone formation or internal reconstruction will determine the spatial structure of the tissue and its mechanical properties. The appearance of a factor disturbing the balance between biological processes, e.g. a disease, will cause changes in the spatial structure of bones, thus affecting its mechanical properties. One of the bone diseases most common in an increasingly ageing population is osteoarthritis, also referred to as degenerative joint disease. It is estimated that in 2050 about 1300 million people will show symptoms of OA. The appearance of a pathological stimulus disturbs the balance of the processes of degradation and synthesis of articular cartilage, chondrocytes and the extracellular matrix, and the subchondral bone layer. As osteoarthritis progresses, study of the epiphysis reveals increasingly widespread changes of the articular surface and the internal structure of bone tissue. In this paper, the authors point out the differences in the mechanical properties of cancellous bone tissue forming the proximal epiphysis of the femoral bone during the progressive stages of OA. In order to determine microproperties of bone trabeculae, specimens from different stages of the disease (N=9) were subjected to microindentation testing, which made it possible to determine the material properties of bone tissue, such as microhardness HV and Young׳s modulus E. In addition, mechanical tests were supplemented with Raman spectroscopy, which determine the degree of bone mineralization, and measurements of structural properties based on analysis using microCT. The conducted tests were used to establish both quantitative and quantitative description of changes in the structural and mechanical properties connected with reorganization of trabeculae making up the bone in the various stages of osteoarthritis. The proposed description will supplement existing knowledge in the literature about identification of the processes occurring during the development of this disease.

No. of bitstreams: 0 Previous issue date: 2019-07-01 Differential diagnosis among fibrous dysplasias, cemento-ossifying fibromas and cemento-osseous dysplasias is difficult, since there is considerable overlap of histologic features, but also extremely important, since they differ greatly in etiology, clinical behaviour, prognosis and terapeuthic approach. There is no data about the use of immunohistochemistry, a viable and accessible technique, for this purpose. The objective of this study was to investigate, comparatively, the immunohistochemical expression of major non-collagenous proteins (osteonectin [ON], osteopontin [OP], bone sialoprotein [BSP] and osteocalcin [OC]) of mineralized tissue extracellular matrix in 22 cases of fibrous dysplasias, 16 of cemento-ossifying fibromas and 16 of cemento-osseous dysplasias. ON maintained the same expression profile in all cases; the staining for OP was negative in fusiform cells producing cementoid globules and weak, as well as heterogeneous, in high mineralized matrixes; there was negativity for BSP in cementoid globules and in the fusiform cells that produce them, differently from the strong positive expression found in the majority of bone trabeculae and their peripheral cuboidal osteoblasts; and finally, the immuno-reactivity for OC was weak, except in cuboidal osteoblasts and osteocytes. We can conclude that the nature of mineralized structure and the cellular phenotype are much more responsible for variability in immunohistochemical profile than the type of lesion (fibrous dysplasias, cemento-ossifying fibromas and cemento-osseous dysplasias) which makes difficult, at least for a while, the use of these proteins with diagnosis purpose. Osteoporosis (OP) is a skeletal disorder responsible for the weakening of the bone structure and, consequently, for an increased fracture risk in the elderly population. In the past, bone mineral density (BMD) variation was considered the best OP indicator, but recently the focus has shifted toward the variation of microstructural bone parameters. This work is based on the characterisation of 8-mm cylindrical biopsies harvested from proximal humeral heads belonging to healthy and osteoporotic patients, in order to assess the OP-related variations of bone properties at different scale lengths. In particular, bone biopsies underwent micro-computed tomography analysis to study the most relevant features of bone architecture and extrapolate the tissue mineral density (TMD) value of bone trabeculae. Compression tests and nanoindentations were performed to investigate the macro- and micromechanical properties of bone biopsies, respectively. In addition, XRD analyses were performed to obtain the mean hydroxyapatite (HA) crystallite size, while Raman spectroscopy investigated the collagen secondary structure. Thermogravimetric analysis was performed to evaluate the ratio between organic and inorganic phases. From the obtained results, OP samples showed a more anisotropic and less interconnected structure responsible for reduced compression strength. From this, it can be supposed that OP caused an alteration of bone structure that led to inferior macroscopic mechanical properties. Furthermore, OP samples possessed higher TMD and bigger HA crystals that are correlated to an increase of the hardness value obtained by means of nanoindentation. This less controlled HA crystal growth is probably due to an alteration of the organic matrix structure, as revealed by the increase of the random coil contribution in the Raman spectra of the OP bone. This higher crystal content led to an increase in trabecular density and hardness. In conclusion, the obtained data showed that OP affects bone properties at different scale lengths causing an alteration of its morphological, structural and mechanical features.

A device and techniques for vacuum-bolus irrigation of root canals are developed on the basis of the analysis of existing methods of medical treatment of root canals in endodontic treatment. A comparative assessment of the impact of different variables of vacuum on the degree of cleaning of the root canal model was carried out in an invitro experiment and the optimal values of vacuum and exposure time were pointed out. In the clinical part of the study 52 patients aged from 25 to 44 years with a diagnosis of apical periodontitis were treated and dynamically monitored during one year (the author`s vacuum-bolus technique was applied to 32 patients of the main study group in the course of endodontic irrigation of root canals; while 20 patients of the control group experienced a standard irrigation method with the use of endodontic syringe in the course of endodontic treatment). Criteria of the modified periapical index (PAI) were used to assess the efficacy of treatment. It was found that after 12 months, 90,5 % of patients of the main study group had no signs of demineralization of bone tissue, the orientation of bone trabeculae of cancellous bone and cortical plate were recovering. Thus, the vacuum-boluc irrigation of root canals can help us improve the efficiency of treatment of destructive forms of apical periodontitis.

Purpose To assess the distributions of stress, strain, and fractured areas using a finite element model (FEM), and examine the osteoclastic activity histopathologically in osteonecrosis of the femoral head. Methods Three femoral heads were obtained during hip arthroplasty for femoral head osteonecrosis. One sample with a normal area, two samples with a non-sclerotic boundary without collapse (Type 1), two samples with a non-collapsed sclerotic boundary (Type 2), and two samples with a collapsed sclerotic boundary (Type 3) were collected from each femoral head for the FEM and histopathological analyses. FEM was performed using CT data, and the distributions of von Mises equivalent stress, octahedral shear stress, octahedral shear strain, and simulated fractured area were evaluated. Furthermore, the osteoclast count at the boundary was compared for each type. Results In normal and Type 1 samples, the distributions of von Mises equivalent stress, octahedral shear stress, octahedral shear strain, and the fractured area were equally concentrated along the whole analytical range; however, in the Type 2 and 3 samples, they were concentrated along the thickened bone trabeculae at the boundary, which corresponded to the fractured area. Histopathologically, a significantly increased osteoclast number was observed only at the collapsed sclerotic boundary. Conclusion These results demonstrated that both shear stress and shear strain tend to be concentrated on thickened bone trabeculae at the boundary. Fracture analyses revealed that the boundary of sclerotic changes, which results from the repair process, may be the starting point of the fracture. Additionally, the osteoclastic activity increases after collapse.

Odontogenic myxoma (OM) is a benign neoplasm often affecting mandibular molars. Although small tumors may be asymptomatic, bigger lesions may result in bone swelling, and aggressive cases show fast growth. Radiographies show unilocular lesions, multilocular lesions, and bone trabeculae arranged at right angles, with irregular margins and dental displacement/resorption. Sometimes soap bubbles appearance is present. A 50-year-old male patient was referred for examination of an asymptomatic radiographic finding in the right mandible. Cone beam computed tomography showed an irregular hypodense area, with poorly defined margins, in the right mandibular body extending to the anterior region, with small cortical expansion. After incisional biopsy, the histopathologic analysis showed proliferation of spindled- and stellate-shaped cells in an abundant loose and myxoid stroma. The diagnosis of OM was given. Subsequent hemimandibulectomy was performed for total tumor excision. The patient remains in follow-up.

Craniofacial fibrous dysplasia (CFD) is a benign bone development anomaly characterized by involvement of the skull and facial bones. This disease can affect 1 bone or multiple bones resulting in deformity, fractures, and pain. An 18-year-old male with facial asymmetry presented with an extensive and not well-demarcated lesion involving the left craniofacial side. Tomographic examination showed an expansive bone lesion with loss of corticomedullary differentiation affecting the left maxilla, maxillary sinus, ethmoid, sphenoid infundibulum, and nasal and frontal bones. After biopsy, the microscopic examination revealed irregular trabeculae of the lamellar bone immersed in a moderately cellularized and vascularized dense connective tissue. Curvilinear bone trabeculae arranged in a pattern of Chinese letter form was noted. The diagnosis of CFD was made. The treatment has consisted of multiple surgeries to restore the facial contour and asymmetry. The patient remains in follow-up.

Using of biomaterial and growth factoris one of the most commonly used as new tissue engineering strategies.Aim of the study: To evaluate the effect of application of a combination bone morphogenic protein 2 (BMP-2)with brain-derived neurotrophic factor (BDNF) as biomaterial to enhance bone healing in osteoporotic rat .Materials and Method: Twenty female Wistar rat(10 normal rat and other 10 induced osteoporotic rat).Bone defects (3mm)was created in femur of each rat,one in the left side that left to heal spontaneouslywithout any application,while the right side with application of a combination of 0.5 ?l of BMP2 and 1?lof BDNF .Evaluation of histological changes includes bone cell counting and scoring of inflammation wereestimated at different periods .Results: Histological examination of Bone healing with application of BMP2 & BDNF for normal andosteoporotic rat shows formation of woven bone and bone trabeculae that filled most of bone hole withminimal inflammatory response and with a significant differences value in comparism to inapplicable bone .Conclusion: The study concludes that application of the combination of BMP-2 & BDNF enhanced bonerepair in osteoporotic rat.

Osteoid metaplasia with functioning marrow is a rare pathological process in arterial calcifications, associated to the asymptomatic development of atherosclerotic plaques. Objective: To describe morphological aspects of bone metaplasia in arterial calcifications and its pathogenesis in two cases. Macroscopic examination of iliac arteries was performed and fragments of approximately 2 cm of the right and left iliac arteries were collected for microscopic analysis of atherosclerosis. These fragments were processed histologically and stained with hematoxylin and eosin (HE). A pronounced atherosclerosis, characterized by diffuse atheroma plaques and foci of calcifications, was evidenced. In both cases, in intima layer of atherosclerotic plaque in right iliac artery, a dilatation, and the presence of bone metaplasia with bone trabeculae, calcifications, and functioning bone marrow with red blood cells and megakaryocytes were observed. Due to the rarity of this lesion, its description allows better characterization of morphological data and pathogenesis.

Adenoid cystic carcinoma (ACC) represents less than 10% of all salivary gland tumors, rarely occurring centrally in the jaws. Herein we describe the case of a 36-year-old female patient presenting a painless swelling in the right maxilla, resulting in marked facial asymmetry. Intra-orally it was observed a swelling covered by an erythematous and irregular-surfaced mucosa, affecting the gingiva and crossing the midline of the hard palate. Imaging studies showed a mixed radiolucent-radiopaque lesion with ill-defined borders, involving the right side of the maxilla and the maxillary sinus. Incisional biopsy revealed basophilic cribriform tumoral islands and solid sheets of neoplastic cells invading bone trabeculae. Most of the tumoral cells presented myoepithelial characteristics, while few true luminal/epithelial cells were observed. To illustrate the epithelial-myoepithelial pattern, immunohistochemical reactions were performed, as well as double immunohistochemical staining. The diagnosis was intraosseous ACC, which features were discussed as well as the potential differential diagnosis.

Background Existing preclinical murine models often fail to predict effects of anti-cancer drugs. In order to minimize interspecies-differences between murine hosts and human bone tumors of in vivo xenograft platforms, we tissue-engineered a novel orthotopic humanized bone model. Methods Orthotopic humanized tissue engineered bone constructs (ohTEBC) were fabricated by 3D printing of medical-grade polycaprolactone scaffolds, which were seeded with human osteoblasts and embedded within polyethylene glycol-based hydrogels containing human umbilical vein endothelial cells (HUVECs). Constructs were then implanted at the femur of NODscid and NSG mice. NSG mice were then bone marrow transplanted with human CD34 + cells. Human osteosarcoma (OS) growth was induced within the ohTEBCs by direct injection of Luc-SAOS-2 cells. Tissues were harvested for bone matrix and marrow morphology analysis as well as tumor biology investigations. Tumor marker expression was analyzed in the humanized OS and correlated with the expression in 68 OS patients utilizing tissue micro arrays (TMA). Results After harvesting the femurs micro computed tomography and immunohistochemical staining showed an organ, which had all features of human bone. Around the original mouse femur new bone trabeculae have formed surrounded by a bone cortex. Staining for human specific (hs) collagen type-I (hs Col-I) showed human extracellular bone matrix production. The presence of nuclei staining positive for human nuclear mitotic apparatus protein 1 (hs NuMa) proved the osteocytes residing within the bone matrix were of human origin. Flow cytometry verified the presence of human hematopoietic cells. After injection of Luc-SAOS-2 cells a primary tumor and lung metastasis developed. After euthanization histological analysis showed pathognomic features of osteoblastic OS. Furthermore, the tumor utilized the previously implanted HUVECS for angiogenesis. Tumor marker expression was similar to human patients. Moreover, the recently discovered musculoskeletal gene C12orf29 was expressed in the most common subtypes of OS patient samples. Conclusion OhTEBCs represent a suitable orthotopic microenvironment for humanized OS growth and offers a new translational direction, as the femur is the most common location of OS. The newly developed and validated preclinical model allows controlled and predictive marker studies of primary bone tumors and other bone malignancies.

The experiment was carried out on 120 female white Wistar rats, to study the endothelio- and osteoprotective action of the combination of rosuvastatin with L-norvaline in the model of experimental osteoporosis. It was found that, after ovariectomy in rats, endothelial dysfunction of the vessels of the microcirculatory bed of bone tissue develops, leading to the appearance of osteoporosis, but the combination of the studied drugs prevents the decrease in the level of microcirculation in the bone tissue, thereby preventing the thinning of bone trabeculae and preventing the occurrence of microfractures in them.

Objectives To describe a stepwise process to obtain fused images from micro-computed tomography (μCT) and histological images of bone specimens. Material and methods Four surgically resected human femoral heads from four patients who had total hip replacement were imaged at a spatial resolution of 12-microns by using μCT. Histological sections of four focal bone lesions including bone cyst in osteoarthritis ( n = 2) and subchondral bone plate fracture in osteonecrosis ( n = 2) were prepared and digitized. μCT images were reformatted and adjusted to match the histological images using a landmark-based visual co-registration. Fused μCT and histological images were displayed in a cine-loop video mode with a gradual transition from one image to the other. Results μCT images of the four focal bone lesions could be successfully fused with the corresponding histological images with a near perfect match of the bone trabeculae. Conclusion We present a stepwise process to obtain fused images from histological and reformatted μCT images of human femoral heads.

Extraskeletal osteosarcoma is a rare, highly malignant, osteoid formation mesenchymal neoplasm in the absence of bone involvement, associated with exceptionally poor prognosis. It frequently arises in the soft tissues of the extremities or in the retroperitoneum, but rarely in visceral organ. We describe a primary osteosarcoma of the liver in a 70-year-old man who presented with an episode of fever, accompanied by abdominal discomfort, after an accident abdominal strike. Ultrasonography and computed tomography revealed a large heterogeneous mass with areas of dense calcification involving most of the right lobe of liver. Radiography did not show evidence of primary tumor or primary bone lesion at any other site. Histologically, the tumor showed an essentially similar appearance as osteosarcoma originating in the skeleton, comprised of polygonal or spindle shaped cells, along with abundant eosinophilic lace-like osteoids, or irregularly arranged bone trabeculae. Immunohistochemistry showed that the tumor cells were positive for vimentin, CD10, and focally for SMA and CD56, but negative for other lineage-specific markers. Thus, the findings favored a primary hepatic osteosarcoma. This patient received palliative chemotherapy to ease the signs of his sickness due to the large size of the tumor and he died 4 moths later.

When central lesions are associated with an unerupted permanent tooth, they are also known as the follicular type. Histological variants of AOT may complicate diagnosis with possible adverse effects on treatment and prognosis. The aim of this study is to report a case of a follicular AOT with extensive calcifying epithelial odontogenic tumor (CEOT)-like histopathological areas in the anterior region of the mandible that was treated by enucleation. The teeth displaced by the tumor were repositioned with orthodontic treatment, and after 7 years of follow-up, the bone trabeculae were normal with no evidence of recurrence. The clinical, radiographic and histopathologic aspects of these tumors are discussed and the debate surrounding whether these cases are true combined lesions or histologic variants of the primary tumor is raised. Intraosseous hibernoma is a rare benign bone tumor, with only 4 cases reported in literature. Aims: Our aim was to study the anatomo-clinical features of hibernoma. We report a case of intraosseous hibernoma with review of the litterature In this report, we describe a 42-years-old man with no past medical history and right chest pain. Imaging studies showed an osteolytic lesion with fat attenuation lesion in the right fourth rib. The lesion was completely resected and microscopic study showed mildly thickened bone trabeculae and multivacuolated brown fat cells replacing the normal white fat and hematopoietic elements. The diagnostic of intraosseous hibernoma was made. Although it is rare, hibernoma should be included in the differential diagnosis of lipomatous tumors.

The aims of this study were to evaluate in vivo the biological responses to implants composed of biodegradable anodized WE43 (containing magnesium yttrium, rare earth elements and zirconium; Elektron SynerMag®) magnesium alloy, monolithic WE43 magnesium alloy and poly-l-lactic acid (PLLA), which are commonly used materials in clinic settings, and to evaluate the effectiveness of the materials as bone screws. The effectiveness of the magnesium alloy implants in osteosynthesis was evaluated using a bone fracture model involving the tibia of beagle dogs. For the monolithic WE43 implants, radiological, and histological evaluation revealed that bone trabeculae around the implanted monolithic WE43 decreased because of an inflammatory response. However, there was no damage due to hydrogen gas or inflammatory response in the bone tissue around the anodized WE43 implants. After 4 weeks, all the PLLA implants (n = 3) had broken but the WE43 implants had not (n = 6). These results suggest that the WE43 implants had sufficient strength to fix bone fractures at load-bearing sites in orthopedic and oral maxillofacial surgery. Therefore, these biodegradable magnesium alloys are good candidates for replacing biodegradable polymers. © 2015 Wiley Periodicals, Inc.
