Abstract Background Seroma after breast cancer surgery is a frequent entity; therefore, different products have been described in literature with the aim to reduce it. The most studied ones have been the sealants products, being tested with aspirative drains. Symptomatic seroma represents the 19% after axillary lymphadenectomy without drains. The aim of this study is to analyze the effect of a sealant in the seroma control after axillary lymphadenectomy without drains and identify the risk factors related to symptomatic seroma. Methods This is a prospective, multicenter, international, and randomized clinical trial. Patients undergoing conservative surgery and axillary lymphadenectomy for breast cancer will be randomized to control group (lymphadenectomy without sealant) or interventional group (lymphadenectomy with sealant Glubran 2®). In any of the study groups, drains are placed. Patients who received neoadjuvant treatment are included. Measurements of the study outcomes will take place at baseline; at 7, 14, and 30 days post-surgery; and at 6–12 months. The primary outcome is symptomatic seroma. Secondary outcomes are seroma volume, morbidity, quality of life, and lymphedema. Discussion Several studies compare the use of sealant products in axillary lymphadenectomy but generally with drains. We would like to demonstrate that patients who underwent axillary lymphadenectomy could benefit from an axillary sealant without drains and reduce axillary discomfort while maintaining a good quality of life. Assessing the relationship between axillary volume, symptoms, and related risk factors can be of great help in the control of seroma in patients who received breast cancer surgery. Objective: Frailty is a syndrome that predisposes older individuals to adverse health outcomes, such as disability, dependence, falls, hospitalization, post-operative complications, and poor health in general. This study aimed to identify factors associated with frailty in older people with chronic diseases in Colombia. Methods: A cross-sectional study was conducted with a nonprobabilistic sample of 230 older people (aged ⩾ 60 years) from four Colombian cities. Frailty was based on Fried’s phenotype (frail defined as having ⩾3 criteria). Comorbidities were classified based on Charlson’s Comorbidity Index and key questionnaires for activities of daily living (ADLs) were evaluated. Results: Most were female (57.8%) with a mean ± SD age of 71.9 ± 8 years. Among them, 27.4% were frail and 58.7% were prefrail. Female gender, age ⩾ 75 years, low educational and socioeconomic level, dependence on ADLs, and cognitive impairment were associated with higher odds of prefrailty/frailty. Conclusions: Prefrailty and frailty are common among older people with chronic diseases in Colombia. This syndrome is associated with social and health-related conditions, which should be addressed when providing care for these patients.

Introduction: Mechanical ventilation is a common practice in intensive care units and anesthesiology with both therapeutic and potentially harmful implications for the respiratory system and distant organs, that is why it is of utmost importance to continually monitor ventilation parameters. Objective: To describe the equation of motion of the respiratory system and its clinical applications in the patient under invasive mechanical ventilation. Main: The equation of motion of the respiratory system integrates the dynamic forces generated by the ventilator with the intrinsic properties of the lung and chest wall. It expresses the pressure in the respiratory system in relation to volume, elastance, resistance, air flow and pressures generated by the ventilator and the patient. Elevated pressures in the respiratory system during mechanical ventilation are associated with greater mortality, that is why the identification of the components responsible for elevation of pressures through the equation of motion of the respiratory system allows to modify ventilator programmed parameters to maintain a protective ventilation. Conclusion: Decision-making based on the equation of motion of the respiratory system allows to modify ventilatory parameters according to the characteristics and diseases of the patient under mechanical ventilation.

IntroductionThere is no consensus on whether invasive ventilation should use low tidal volumes (VT) to prevent lung complications in patients at risk of acute respiratory distress syndrome (ARDS). The purpose of this study is to determine if a low VT strategy is more effective than an intermediate VT strategy in preventing pulmonary complications.MethodsA randomized clinical trial was conducted in invasively ventilated patients with a lung injury prediction score (LIPS) of >4 performed in the intensive care units of 10 hospitals in Spain and one in the United States of America (USA) from 3 November 2014 to 30 August 2016. Patients were randomized to invasive ventilation using low VT (≤ 6 mL/kg predicted body weight, PBW) (N = 50) or intermediate VT (> 8 mL/kg PBW) (N = 48). The primary endpoint was the development of ARDS during the first 7 days after the initiation of invasive ventilation. Secondary endpoints included the development of pneumonia and severe atelectases; the length of intensive care unit (ICU) and hospital stay; and ICU, hospital, 28– and 90–day mortality.ResultsIn total, 98 patients [67.3% male], with a median age of 65.5 years [interquartile range 55–73], were enrolled until the study was prematurely stopped because of slow recruitment and loss of equipoise caused by recent study reports. On day 7, five (11.9%) patients in the low VT group and four (9.1%) patients in the intermediate VT group had developed ARDS (risk ratio, 1.16 [95% CI, 0.62–2.17]; p = 0.735). The incidence of pneumonia and severe atelectasis was also not different between the two groups. The use of a low VT strategy did neither affect the length of ICU and hospital stay nor mortality rates.ConclusionsIn patients at risk for ARDS, a low VT strategy did not result in a lower incidence of ARDS than an intermediate VT strategy.Clinical Trial Registration: ClinicalTrials.gov, identifier NCT02070666.

Abstract Background iPSC (induced pluripotent stem cells) banks of iPSC lines with homozygous HLA (human leukocyte antigen) haplotypes (haplobanks) are proposed as an affordable and off-the-shelf approach to allogeneic transplantation of iPSC derived cell therapies. Cord blood banks offer an extensive source of HLA-typed cells suitable for reprogramming to iPSC. Several initiatives worldwide have been undertaken to create national and international iPSC haplobanks that match a significant part of a population. Methods To create an iPSC haplobank that serves the Spanish population (IPS-PANIA), we have searched the Spanish Bone Marrow Donor Registry (REDMO) to identify the most frequently estimated haplotypes. From the top ten donors identified, we estimated the population coverage using the criteria of zero mismatches in HLA-A, HLA-B, and HLA-DRB1 with different stringencies: high resolution, low resolution, and beneficial mismatch. Results We have calculated that ten cord blood units from homozygous donors stored at the Spanish cord blood banks can provide HLA-A, HLA-B, and HLA-DRB1 matching for 28.23% of the population. Conclusion We confirm the feasibility of using banked cord blood units to create an iPSC haplobank that will cover a significant percentage of the Spanish and international population for future advanced therapy replacement strategies.

Introduction: The new coronavirus specimen (COVID-19), has generated scenarios of sanitary emergencies in many countries from all continents. Objective: To characterize the variability of the PCR evolutionary results in patients with COVID-19. Methods: An observational, descriptive, prospective and cross-sectional study was carried out in 68 patients with COVID-19 (children and adults from both sexes) admitted in Dr. Joaquín Castillo Duany Military Hospital in Santiago de Cuba, from March 12th to April 30th, 2020. To collect the virological, the nasal and pharyngeal use of isotope was carried out, which analysis of PCR-RT was processed in the territorial reference center. Management, social, demographic and epidemiological variables were included, among others. Results: Female sex prevailed in the series (52.9 %). Two PCR-RT fast tests were made to all the patients, through their clinical course, 40.0 % of the children and 10.0 % of the aged patients requiered a third virological test. In the second and third third of the clinical and epidemiological observation period, the report of the result averaged 1.5 days (SD ± 0.6) and 3.2 days (SD±1.6), respectively. After 15 days of treatment there were active positive cases in all populational groups; likewise 60.0 % of those with viral charge between 16-20 days were asymptomatic, as the 66.7 % of those with more than 20 days under medication. Conclusions: Independently from the age and duration of the medical treatment, some patients remained with detectable viral charge. It is not adviced to discharge patients without a negative PCR-RT, above all for asympthomatic cases, in whom the evolutionary clinical course has been less favorable.

Adeno-associated virus (AAV) vectors are promising candidates for gene therapy. However, a number of recent preclinical large animal studies failed to translate into the clinic. This illustrates the formidable challenge of choosing the animal models that promise the best chance of a successful translation into the clinic. Several of the most common AAV serotypes use sialic acid (SIA) as their primary receptor. However, in contrast to most mammals, humans lack the enzyme CMAH, which hydroxylates cytidine monophosphate-N-acetylneuraminic acid (CMP-Neu5Ac) into cytidine monophosphate-N-glycolylneuraminic acid (CMP-Neu5Gc). As a result, human glycans only contain Neu5Ac and not Neu5Gc. Here, we investigate the tropism of AAV1, 5, 6 and 9 in wild-type C57BL/6J (WT) and CMAH knock-out (CMAH−/−) mice. All N-linked SIA-binding serotypes (AAV1, 5 and 6) showed significantly lower transduction of the heart in CMAH−/− when compared to WT mice (5–5.8-fold) and, strikingly, skeletal muscle transduction by AAV5 was almost 30-fold higher in CMAH−/− compared to WT mice. Importantly, the AAV tropism or distribution of expression among different organs was also affected. For AAV1, AAV5 and AAV6, expression in the heart compared to the liver was 4.6–8-fold higher in WT than in CMAH−/− mice, and for AAV5 the expression in the heart compared to the skeletal muscle was 57.3-fold higher in WT than in CMAH−/− mice. These data thus strongly suggest that the relative abundance of Neu5Ac and Neu5Gc plays a role in AAV tropism, and that results obtained in commonly used animal models might not translate into the clinic.

Objetivo: elaborar estrategias de fortalecimiento para la articulación de los servicios municipales que componen la red de atención psicosocial. Método: pesquisa participante con aproximación cualitativa, realizada en marzo y abril de 2019 en un municipio en el sur de Brasil. La recolección de datos implico el análisis documental, cuestionario y grupo focal. Muestra compuesta por trece profesionales de Atención Básica, Núcleo Ampliado de Salud Familiar, Policlínica Municipal, Centro de Atención Psicosocial y Hospital General. El análisis de los datos según Minayo y elaboración de estrategias mediante la triangulación de los datos. Resultados: potencialidades y fragilidades encontradas en la vida profesional cotidiana, organización y articulación de la red, y estrategias para fortalecerla. Entre las estrategias elaboradas se destacan la necesidad de apoyo matricial en los servicios de salud, la capacitación y el empoderamiento de los profesionales. Conclusión: Hay escasez de literatura sobre estrategias para la articulación de la red y se recomiendan nuevos estudios. La actual investigación indica la importancia e inevitabilidad de la articulación, lo que contribuye a mejorar la práctica.

Catalase is a powerful antioxidant metalloenzyme located in peroxisomes which also plays a central role in signaling processes under physiological and adverse situations. Whereas animals contain a single catalase gene, in plants this enzyme is encoded by a multigene family providing multiple isoenzymes whose number varies depending on the species, and their expression is regulated according to their tissue/organ distribution and the environmental conditions. This enzyme can be modulated by reactive oxygen and nitrogen species (ROS/RNS) as well as by hydrogen sulfide (H2S). Catalase is the major protein undergoing Tyr-nitration [post-translational modification (PTM) promoted by RNS] during fruit ripening, but the enzyme from diverse sources is also susceptible to undergo other activity-modifying PTMs. Data on S-nitrosation and persulfidation of catalase from different plant origins are given and compared here with results from obese children where S-nitrosation of catalase occurs. The cysteine residues prone to be S-nitrosated in catalase from plants and from bovine liver have been identified. These evidences assign to peroxisomes a crucial statement in the signaling crossroads among relevant molecules (NO and H2S), since catalase is allocated in these organelles. This review depicts a scenario where the regulation of catalase through PTMs, especially S-nitrosation and persulfidation, is highlighted.

According to published meta-analyses, no significant efficacy differences have been demonstrated between erlotinib and afatinib. However, the incidence of EGFR–TKI-related adverse events (AEs) was lower with erlotinib. This study compares the cost of management of the AEs associated with these two drugs from the perspective of the Spanish National Health System (NHS).Methods: The frequency of AEs was established from a recently published meta-analysis. In Spain, the daily cost of both drugs can be considered similar; as a result, only the costs of management of the AEs were considered. Costs and resource utilization in the management of the AEs were estimated by a panel of Spanish oncologists and from studies previously carried out in Spain. A probabilistic analysis was performed based on a Monte Carlo simulation.Results: The model generated 1,000 simulations. The total cost per patient treated with erlotinib and afatinib was €657.44 and €1,272.15, respectively. With erlotinib, the cost per patient and per AE of grades ≤2 and ≥3 was €550.86 and €106.58, respectively, whereas the cost with afatinib was €980.63 and €291.52, respectively. The reduction in the number of AEs with erlotinib could avoid a mean cost for the NHS of €614.71 (95% CI: €342.57–881.29) per patient.Conclusion: In advanced EGFR mutation-positive NSCLC patients, first-line treatment with erlotinib could reduce health care costs for the NHS, due to a decrease in the AE rate compared with afatinib. In long-term treatments, the avoidance of complications and the lowering of costs associated with the management of AEs are relevant factors that contribute to the sustainability of the health system. Rotator cuff injuries are reason for consultation frequent in elderly patients. Most of the time there are no background traumatic acute generating progressive limitations in activities of daily living (ADLS). The objective of this study is to show results in tendonitis of the rotator cuff, in patients treated with extracorporeal shock wave therapy (ESWT). It is a prospective descriptive observational study which presents clinical and functional outcomes in patients with described lesion, treated with ESWT with poor response to conventional treatments and clinical pictures of longstanding through implementing visual analog scale (VAS) of pain and evaluation of range of motion. The greater presence of lesion is present in women 63.6%. Mostly affected shoulder was right in a 63.6%. Found significant changes in VAS pre and post treatment with averages of 7.9 and 0.5 respectively and different statistical p < 0.001. We were conclude that the ESWT is an effective method in the treatment of the tendonitis of the rotator cuff with relief from pain and return to functional levels.

Los objetivos del presente estudio han sido identificar y analizar publicaciones que han indagado acerca de la funcionalidad del sistema respiratorio en pacientes con Ataxia Espinocerebelosa Tipo 2. Se presentan los datos obtenidos a partir de la búsqueda avanzada en Google Académico y en bases de datos tales como PubMed, Scopus, en los idiomas español e inglés, se tuvieron en cuenta los tipos artículos y resúmenes de eventos científicos de varios países, se acotó en la indagación realizada a las palabras claves o en resumen del artículo que hicieran referencia a: Ataxia Espinocerebelosa Tipo 2, Sistema Respiratorio y Capacidad Vital Pulmonar, en un período o desde 1998 hasta 2015, cuyos resultados permitieron revelar que el sistema respiratorio es el menos sistematizado en la búsqueda realizada, aunque existen evidencias de sus alteraciones en los pocos trabajos revisados respecto al tema; se demostró, además, desde el punto de vista teórico la existencia de un impacto positivo de este en la rehabilitación física. Como conclusión, hay indicios que permiten profundizar en este sistema en pacientes con SCA2, los que permiten la identificación de nuevas líneas de investigación en esta temática. Objetivos. Analizar la implementación y la utilidad del portafolio como herramienta evaluativa de las prácticas clínicas en enfermería comunitaria en la Escuela de Enfermería Puerta de Hierro de la Universidad Autónoma de Madrid (UAM) y valorar la influencia de variables sociodemográficas y académicas en el proceso de evaluación continua del portafolio. Sujetos y métodos. Estudio descriptivo en el marco de un proyecto de innovación docente financiado por la UAM. La población objeto de estudio fueron estudiantes de segundo y tercer curso de enfermería de la escuela Puerta de Hierro (UAM), que realizaron sus prácticas clínicas de enfermería comunitaria en el medio de la atención primaria de salud (n = 38). Variables principales: calificaciones intermedias y finales en los trabajos que componían el portafolio (diario reflexivo, caso clínico, análisis de salud de la comunidad), puntualidad en las entregas, dificultad, utilidad y mejora del aprendizaje percibida, horas dedicadas a la semana, autoevaluación de las prácticas clínicas, calificación de la tutora, evaluación final del portafolio. Para el análisis bivariante se usaron pruebas no paramétricas. Se calcularon intervalos de confianza para un 95% de seguridad. Resultados. Se analizaron 38 estudiantes con una edad media 21,5 ± 4,3 años, 81,6% mujeres. Para el 56,7%, la dificultad fue alta. La utilidad se señaló como alta en el 72,9% de los casos. La media en la autoevaluación del estudiante fue de 7,68; la evaluación de la tutora, de 8,31, y la calificación definitiva del portafolio final por parte de las profesoras, de 7,07. Conclusiones. La implementación del portafolio ha puesto de manifiesto la dificultad sentida por los estudiantes en un proceso que requiere escritura reflexiva, trabajo diario y autoevaluación. Ahora bien, estas dificultades se traducen en una alta percepción de utilidad y de mejora para el aprendizaje.Aim. Subjects and methods. cross-sectional study in the context of a teaching innovation project funded by the UAM. The target population: students of 2nd and 3rd courses of Puerta de Hierro Nursing School (UAM) that made their Community Nursing Clinical Practice Learning in the setting of the Primary Health Care (n = 38). Main outcomes: intermedium and final scores in the works that includes the portfolio ("reflective diary", "clinical case" and "analysis of community health"), timeliness of delivery, difficulty, usefulness and perceived improvement of learning, hours spent/week, student self-assessment, qualification of the tutor, final evaluation of the portfolio. Bivariate analysis was done using nonparametric tests. Confidence intervals were calculated for 95% confidence. Results. 38 students were studied. The mean age was of 21.5 ± 4.3 years-old, 81.6% women. Difficulty was high in 56.7%. The utility was noted as high in 72.9%. The student self-assessment average score was 7.68, the evaluation of the tutor: 8.31 and portfolio final score by the teachers: 7,07. Conclusions. The implementation of portfolio has shown the difficulty felt by students in a process requiring reflective writing, daily work and self-evaluation. However, these difficulties translate into a high perception of usefulness and improve learning.

ResumenSe presenta un estudio descriptivo de corte transversal, en el cual treinta y seis pacientes fueron intervenidos quirúrgicamente para reconstrucción artroscópica de ligamento cruzado anterior con auto injerto de tendón patelar en el periodo comprendido de enero del 2004 y enero del 2006. De los treinta y seis pacientes que hicieron parte del presente estudio, en 25 pacientes se practicó la fijación con tornillos de interferencia femoral y tibial, y en los 11 restantes en el componente femoral clavijas reabsorvibles y en el componente tibial tornillo de interferencia. Las variables consideradas fueron rango de movilidad, reintegro a la vida deportiva, presencia de lesiones asociadas, complicaciones y nivel de satisfacción. En todos los pacientes fue aplicada la escala de Lisholm. Los resultados obtenidos se expresan en términos de relación porcentual y son comparables con los reportados en otros estudios. (Duazary 2008; 82-89)AbstractWe are present a descriptive transversal study with 36 patients, who were operative treatment with arthroscopic reconstruction of anterior cruciate ligament with patellar-ligament graft, between January 2004 – January 2006.Of 36 patients included in this studied were operated 25 patients with interference screw fixation in femoral and tibial, in order to, 11 patients were treated with absorbable femoral pins and interference screw in tibia. The variables considerated were morbidity, sport practice again, associated lesions, complications and satisfaction level. In all patients the Lisholm scale was aplicated. The results were expressed in porcental relation and were similarly with other studies.Key Words: Stability; Cruciate Anterior Ligament; Reconstruction; Autogenous patellar-ligament graft.

ResumenEl trastorno por déficit de atención con o sin Hiperactividad (TDAH) se define como un trastorno crónico sintomáticamente evolutivo, esta alteración del neurodesarrollo en el niño suele caracterizarse por la presencia de problemas para concentrarse y dedicarse a una tarea asignada o al cumplimiento de reglas impartidas; permaneciendo en constante movimiento. Por tanto, estos niños son propensos a presentar accidentes con un alto riesgo de lesionarse, fracturarse o sufrir algún tipo de trauma físico de grados variables, a diferencia de los niños que miden sus actos y no presentan déficit de atención. Este estudio se llevó a cabo, en el marco de la investigación descriptivo – correlacional, con el objetivo de describir la relación existente entre el trastorno por déficit de atención con y/o sin hiperactividad (TDHA) y las presencia de lesiones y/o traumas ortopédicos en Niños entre 6 y 12 Años en la Clínica El Prado de la Ciudad de Santa Marta. Pacientes y Métodos: se conformó un grupo de 31 niños con su respectiva autorización de padres, tutores o representantes legales, 21 de ellos eran hombres (67,7%) y 10 mujeres (32,3%) los cuales asistían por lesiones ortopédicas al servicio de urgencias de la Clínica El Prado y fueran diagnosticados con déficit de atención con y sin hiperactividad, pudiendo así obtenerse una muestra del 15,5% en relación a la población total. Para el diagnostico del TDHA se utilizaron los Criterios diagnósticos del DSM IV, DSM IV Checklist o lista de síntomas, y la escala Multidimensional del Comportamiento BASC para Padres y Docentes. Resultados: De acuerdo con los datos estadísticos se puede observar que aunque un gran número de niños que conforman la muestra presentan características propias del diagnóstico de déficit de atención y sus subtipo (60%), no existe evidencia estadísticamente significativa que demuestre la relación entre ésta y la presencia de lesiones y traumatismo ortopédico, por el contrario la prueba de asociación de variables demuestra que estas son completamente independientes. (Duazary 2009-II 124-133)AbstractThe disruption of attention deficit disorder with or without hyperactivity (TDAH) is defined as an evolutionary symptomatic chronic disruption. The alteration of this neuro development in a child is usually characterized by the presence of problems to concentrate and dedicate in an assignment duty or failure to comply with given rules; remaining in constant movement. Therefore, these children are prone to present accidents with high risk of injuring themselves, fracturing or undergoing some type of physical trauma of variable degrees, unlike the children who measure their acts and they do not present/display attention deficit. This studio, We are carried out within the framework descriptive-correlative investigation, with the objective to describes the existing relation between the upheaval by and/or deficit of attention with without hyperactivity (TDHA) and the presence of injuries and/or orthopedics traumas in children between 6 and 12 years in the Clinic the Prado of Santa Marta City. Patients and methods: Conform a group of 31 children with respective authorization of their parents, legal tutors or representatives, 21 of them was men (67.7%) and 10 women (32.3%) who assisted by orthopedics injuries to the service of urgency of the Clinic the Prado and was diagnosed with deficit of attention with and without hyperactivity, thus being able to obtain a sample of 15.5% in relation to the total population. For diagnostic of the TDHA the criteria were used diagnoses of DSM IV, DSM IV Checklist or list of symptoms, and the Multidimensional scale of Behavior BASC for parents and docents. Results: According to the statistical data it is possible to be observer that although a great number of children who conform the sample present/display own characteristics of diagnostic of attention deficit and their subtypes (60%), does not exist statistically significant evidence that it shows the relation between this and the presence of lesions and orthopedic traumatism; on the contrary the test of association of variables demonstrates that these are completely independent. The rest of participants of the sample (40%) do not fulfill the classification criteria.Key words: Deficit of attention; Upheavals; Hyperactivity; Trauma; Fracture; Neurodevelopment.

Las lesiones de la articulación esterno clavicular son poco frecuentes y generalmente se producen por traumas contundentes en esta región anatómica. Se reporta caso clínico de un paciente masculino victima de accidente de transito quien presenta luxo fractura esterno clavicular. Se presenta técnica quirúrgica y método de fijación mediante osteosíntesis con placa y tornillos de pequenos fragmentos con excelente resultado post operatorio y rehabilitación funcional completa. Se realizó un estudio de tipo descriptivo en una serie de casos en el cual se presentan los resultados de 12 fracturas de clavícula tratadas en forma quirúrgica mediante clavos intramedulares flexibles. Todos los pacientes fueron tratados por los autores en la Clínica El Prado del D.T.C.H. de la Ciudad de Santa Marta mediante reducción cerrada bajo control radioscópico. Presentando resultados excelentes y buenos en todos los casos. Así mismo, se evidenció consolidación clínica a las cuatro semanas con reintegro a la vida laboral alrededor de las seis semanas. Todos los pacientes mostraron alto grado de satisfacción con el resultado obtenido. Se concluye que el tratamiento empleado es una alternativa terapéutica a tener en cuenta en aquellas fracturas desplazadas, sean estas del tercio externo, medio o interno que permite consolidación sin las complicaciones que con alguna frecuencia se presentan con el tratamiento conservador. En la actualidad las fracturas abiertas de tibia representan uno de los motivos de consulta más frecuente en nuestras instituciones, los accidentes de tránsito constituyen una de las causas principales en la incidencia de esta patología. Lo anterior encuentra soporte en diversos factores tales como la falta de precaución en la conducción de vehículos y motocicletas o la no utilización de las medidas de protección en los mismos. Las heridas por arma de fuego que generan fracturas en uno a más huesos son consideradas fracturas abiertas. En esta revisión bibliográfica examinaremos aspectos importantes sobre las fracturas abiertas tocando los diversos tópicos alrededor de las mismas, con la finalidad de aportar criterios claros para el manejo de este tipo de lesiones. Abstract Actually the tibiae’s open fractures represent one of the most frequent motive of consulting in our institutions, the traffic accidents constitute one of the principal reasons in the incidence of this pathology. This find support in several facts such as lack of precaution in vehicles and motorcycle driving or don’t use protection measure in the same. The firearm wounds that produce fractures in one or more bone are considerate open fractures.In this bibliographic compilation we will exam important aspects about the open fractures touching different topics around the same, with the purpose of contribute in the clear criterion’s for the management of this kind of injuries.

Se realizó un estudio de tipo descriptivo transversal que tiene como objetivo conocer la frecuencia de trauma en miembros en el principal centro hospitalario del orden estatal de la ciudad de Santa Marta (Magdalena) DTCH en el periodo entre enero de 2003 y junio de 2004. Las variables incluyen región anatómica afectada, edad, sexo, lugar de procedencia, causa externa que origina el trauma y estancia hospitalaria. El manejo fue realizado por médicos especialistas del servicio de ortopedia y traumatología de este centro hospitalario de tercer nivel de atención. La totalidad de consultas en el periodo establecido por traumatismo de miembros fue de 2.052. De estas 1.420 (69.2%), correspondieron a traumas en el miembro superior y 632 (30.8%) correspondieron al miembro inferior. La totalidad de la consulta por trauma fue de 3.838 pacientes. Los resultados obtenidos permiten concluir que los datos encontrados corroboran los hallazgos de estudios internacionales y nacionales.

Se presenta un caso de paciente masculino de 33 años de edad de oficio mensajero con fractura abierta del tercio distal de la tibia izquierda quien recibe tratamiento combinado inicialmente quirurgico y cinco meses despues, tratamiento ortopedico con seguimiento a cinco años y resultado satisfactorio desde el punto de vista clínico y radiologico.

Los traumatismos graves de miembros representan un porcentaje importante dentro de las patologías que consultan los servicios de urgencias de diversos centros hospitalarios. En la actualidad las estadísticas mundiales, nacionales y locales guardan unas proporciones similares en cuanto a frecuencia de presentación, sin embargo en países desarrollados la infraestructura en atención prehospitalaria con personal altamente calificado, permite obtener mejores resultados con respecto a atención y pronóstico de paciente politraumatizados, logrando en los últimos años una disminución significativa en cuanto a frecuencia de mortalidad. En todo traumatismo grave de miembros se requiere un manejo cuidadoso y ordenado a fin de disminuir complicaciones que en muchas ocasiones resultan verdaderamente catastróficas. Al considerar el tratamiento de traumatismos graves de miembros la amputación representa en algunas ocasiones una alternativa, sobre todo en aquellos pacientes con patologías asociadas que contribuyen a una evolución tórpida cuando se realiza reconstrucción.

ResumenSe presentan los resultados de un estudio descriptivo retrospectivo sobre la incidencia de procesos infecciosos en cirugía ortopédica y traumatológica en la Clínica El Prado de la Ciudad de Santa Marta durante el año 2007. Fueron evaluadas 3.884 historias clínicas de las cuales se incluyeron para el presente estudio 2.659 correspondientes a igual número de pacientes. Se excluyeron por diversos factores 1.225 historias clínicas correspondientes a igual número de pacientes. 65 pacientes presentaron Infección en el sitio operatorio con una incidencia del 2.4%. El sexo más implicado fue el masculino representado por 1793 (67.4%) pacientes y presentando una tasa de infecciones del 2.5%. La cirugía de tipo traumatológica se realizó en el 82% de los casos, mientras que la tipo ortopédica solo en 18% con una tasa de infección de 2.4% y 2.3% respectivamente. La infección temprana, se presentó en 63 pacientes del total de infectados que equivalen al 96.9%. El mayor porcentaje de infección se presenta en las heridas quirúrgicas sucias con un 11.1%. ResumenSe presenta un estudio descriptivo de corte transversal, en el cual 26 pacientes fueron intervenidos quirúrgicamente para reconstrucción artroscópica de ligamento cruzado anterior con auto injerto semitendinoso cuádruple en el periodo comprendido entre junio del 2008 y junio del 2009.De los 26.pacientes que hicieron parte del presente estudio 22 fueron de sexo masculino y 4 de sexo femenino, la edad promedio fue de 32 años. En todos los casos se realizo fijación en fémur con tornillo transfixiante en titanio y tornillo de interferencia reabsorbible en tibia.El rango de movilidad fue completo en el 92.3%, el reintegro a la actividad deportiva realizada previa a la lesión fue del 84.6% y a actividad de menor exigencia en 11.54%. Las lesiones asociadas en cartílago y meniscos estuvieron presentes en el 65.38% de los pacientes.Al aplicar la escala de Lisholm encontramos un bajo porcentaje de dolor residual y limitaciones en la realización de actividades físicas, sin embargo, en 26.92 % de los pacientes presentaron algún grado de limitación al realizar cunclillas.El nivel de satisfacción expresado por los pacientes con el procedimiento quirúrgico empleado, fue entre excelentes y buenos en el 92.3 % de los casos regular en el 3.8% y malo en el 3.8%. (DUAZARY 2011, 34 - 41) Nivel de evidencia IVAbstractThis is a descriptive study of 26 patients were seen for an arthroscopic surgical reconstruction of the anterior cruciate ligament using a quadruple-stranded semitendinosus auto graft. This study was conducted between June 2008 and June 2009. Of the 26 patients that took part of the study, 22 were male and 4 were female. The average age was 32 years. In cases, the femur was transfixated with a titanium screw and the tibia with a resorbable interference screw. The range of motion was full in 92.3% of patients. 84.6% of patients were able to resume their sport activities and 11.54% were able to return to less demanding activities. There was cartilage damage and meniscal tears present in 65,38% of the patients.Using the Lisholm knee scale, the percentage of residual pain and limitations in some physical activities was 26,92% of the patients and consisted of some limitations with kneeling. The level of satisfaction with this surgical procedure was excellent and good in the 92.3%, regular in the 3,8% and poor in 3.8%. ResumenSe realizó un estudio descriptivo, retrospectivo, tipo serie de casos analizando las características de las fracturas de radio manejadas quirúrgicamente en pacientes mayores de 15 años entre enero del 2004 y diciembre del 2005 en la clínica El Prado de la ciudad de Santa Marta. Se revisaron 498 historias clínicas de las cuales 219 tenían criterios de inclusión. Se excluyeron del estudio pacientes con tratamiento conservador, trauma craneoencefálico y menores de quince años. Los resultados fueron expresados en relación porcentual. El rango de edad más afectado fue el 15 – 35 años. El sexo prevalente fue el masculino y la ocupación en mayor proporción fue ama de casa. Las lesiones en la extremidad superior derecha y el segmento distal los más frecuentemente afectados. El tratamiento quirúrgico fue la reducción más osteosíntesis siendo los clavos de Kirschner el material quirúrgico utilizado con mayor frecuencia. We excluded patients with orthopedic treatment, encephalic trauma and 15 years old minor patients. The results were expressed in porcentual relation. The more affected age ranges were 15 - 35 years old. The prevalent sex were man and the frequently extremity affected were right arm and the distal third the segment affected more frequently. Se realizó un estudio descriptivo tipo serie de casos en el cual se analizan diversos aspectos inherentes al tratamiento quirúrgico de 92 fracturas de tibia diafisiarias cerradas y abiertas tipo I y II de Gustillo, tratadas con Clavos Intramedulares Bloqueados No Fresados (CIMBNF) de las 125 atendidas servicio de ortopedia y traumatología de la Clínica El Prado de la Ciudad de Santa Marta D.T.C.H., entre diciembre del 2005 y diciembre del 2007. Se excluyeron de este estudio 33 casos en los cuales por diversas causas se realizó otro tipo de tratamiento fuera ortopédico o quirúrgico mediante colocación de fijadores externos. Utilizamos la clasificación del AO en la cual a este tipo de lesiones corresponde el número 42. Encontramos una alta incidencia en accidentes de transito como factor productor de este tipo de lesión y dentro de ellos la motocicleta como vehículo mas frecuente. Fueron evaluados 86 de los 92 pacientes que corresponde al 93.5% y dentro de ellos se presentaron complicaciones mayores en el 4.6%, cabe mencionar Infecciones post operatorias y deformidades a nivel proximal y distal por diversas causas, las cuales fueron corregidas con procedimientos ulteriores. Title: Hip fractures Treatment Alternatives.ResumenLas fracturas de cadera son motivo de consulta frecuente en nuestras clínicas y hospitales en pacientes mayores de 60 años. Este tipo de lesiones se producen generalmente ante traumas por caída de baja altura, la mayoría de las veces por caídas en su domicilio. Se han descrito una gran cantidad de factores predisponentes presente en ese grupo etario y dentro de ellos la osteoporosis juega un papel protagónico. Diversas clasificaciones han sido descritas dependiendo de la localización del trazo de fractura, sin embargo, el limite anatómico de la capsula articular del fémur proximal es determinante y permite clasificarlas como intra capsulares y extra capsulares. En la práctica ortopédica actual el tratamiento médico de estas lesiones no tiene indicación, salvo en situaciones muy excepcionales en las cuales la cirugía implica riesgo inminente de muerte en razón a patologías asociadas y es el tratamiento quirúrgico el que ofrece los mejores resultados. La resolución quirúrgica de las fracturas de cadera implica la utilización de una variedad de implantes cuya elección está sujeta a una serie de factores que dependen no solo del tipo de fractura sino situaciones inherentes a cada paciente en particular. El objetivo de esta revisión es buscar consenso en cuanto al tipo de implante a utilizar en las fracturas de cadera dependiendo de diversos factores y dentro de ellos la ubicación del trazo de fractura. A large number of predisposing factors present in this age group have been described and within them the osteoporosis plays a leading role.Several classifications have been described depending on the location of the fracture stroke, however the anatomical limit of proximal femoral articular capsule is crucial and allows to classify them as intracapsular and extra capsular fractures.In current orthopaedic practice medical treatment of these injuries not have indication, except in very exceptional situations in which surgery implies imminent danger of death because of associated diseases and is surgical treatment that offers the best results.Surgical hip fractures resolution involves the use of a variety of implants whose election will be subject to a number of factors that depend not only on the type of fracture, but inherent to each patient situations in particular. En décadas anteriores la cirugía ortopédica y traumatológica constituía la excepción a la regla en el tratamiento de estas lesiones en razón a la preferencia de los tratamientos conservadores tales como los yesos y tracciones. Resulta innegable que muchos de estos tratamientos mostraban resultados satisfactorios en cuanto a la consolidación de la fractura1, sin embargo, los problemas de consolidación en posición no adecuada resultaban altamente frecuentes con el tratamiento ortopédico.

Objetivos. Analizar la implementación y la utilidad del portafolio como herramienta evaluativa de las prácticas clínicas en enfermería comunitaria en la Escuela de Enfermería Puerta de Hierro de la Universidad Autónoma de Madrid (UAM) y valorar la influencia de variables sociodemográficas y académicas en el proceso de evaluación continua del portafolio. Sujetos y métodos. Estudio descriptivo en el marco de un proyecto de innovación docente financiado por la UAM. La población objeto de estudio fueron estudiantes de segundo y tercer curso de enfermería de la escuela Puerta de Hierro (UAM), que realizaron sus prácticas clínicas de enfermería comunitaria en el medio de la atención primaria de salud (n = 38). Variables principales: calificaciones intermedias y finales en los trabajos que componían el portafolio (diario reflexivo, caso clínico, análisis de salud de la comunidad), puntualidad en las entregas, dificultad, utilidad y mejora del aprendizaje percibida, horas dedicadas a la semana, autoevaluación de las prácticas clínicas, calificación de la tutora, evaluación final del portafolio. Para el análisis bivariante se usaron pruebas no paramétricas. Se calcularon intervalos de confianza para un 95% de seguridad. Resultados. Se analizaron 38 estudiantes con una edad media 21,5 ± 4,3 años, 81,6% mujeres. Para el 56,7%, la dificultad fue alta. La utilidad se señaló como alta en el 72,9% de los casos. La media en la autoevaluación del estudiante fue de 7,68; la evaluación de la tutora, de 8,31, y la calificación definitiva del portafolio final por parte de las profesoras, de 7,07. Conclusiones. La implementación del portafolio ha puesto de manifiesto la dificultad sentida por los estudiantes en un proceso que requiere escritura reflexiva, trabajo diario y autoevaluación. Ahora bien, estas dificultades se traducen en una alta percepción de utilidad y de mejora para el aprendizaje.
