Background Among cardiac causes for postpartum maternal collapse, severe mitral stenosis is not listed as a potential cause in current literature. We report a rare case of severe mitral stenosis that presented with severe hypoxia and maternal decompensation in early postpartum period for the first time. Case summary A 30-year-old para 2, abortus 1, Ethiopian woman developed severe hypoxia and rapid deterioration on her sixth postoperative day after cesarean delivery for fetal bradycardia with a good fetal outcome. She was put on a mechanical ventilator when she developed respiratory failure. Initially, a diagnosis of pulmonary embolus was considered. After admission to the intensive care unit, severe mitral stenosis was diagnosed with the help of echocardiography. She was managed successfully for congestive heart failure and discharged from the intensive care unit with improvement. Conclusion The possibility of mitral stenosis, as a differential diagnosis, should be considered whenever a case of postpartum maternal collapse is encountered. We recommend a routine immediate echocardiography in any patient that experiences postpartum collapse, especially in the presence of a known trigger for heart failure such as long hours of labor, bleeding, anemia, hypotension, and infection.

Cardiac disease in pregnancy remains an important etiology of maternal and fetal morbidity and mortality.1 Mitral stenosis is the most commonly acquired valve lesion encountered in pregnant women and is almost invariably caused by Rheumatic heart disease (RHD).1 Pregnancy and peripartum period represent a physiologic burden that may worsen symptoms in even moderate degrees of cardiac disease.1 Consequently, many women are first diagnosed with the cardiac disease during pregnancy.1 In this case report, we describe the peripartum management of a 38 year old woman with congestive heart failure functional class II, severe mitral stenosis (MS), moderate mitral regurgitation (MR), and moderate tricuspid regurgitation because of RHD. She successfully underwent cesarean section with low-dose spinal anaesthesia using 7 mg hyperbaric bupivacaine intrathecally. This report highlights that low-dose spinal anaesthesia remains a good option in anaesthesia management for cesarean section in gravida with RHD, especially with severe MS.

Mitral stenosis is the most common manifestation of rheumatic heart disease. Hemodynamic changes during pregnancy increase the burden of the heart in pregnant women with cardiac abnormalities. A 33 years old woman, G3P1A1 is 21 weeks pregnant came to our hospital with a shortness of breath during daily activity, which was worsened since 3 months of pregnancy. The patient has been diagnosed with severe mitral stenosis. The results of echocardiography examination was left atrial dilatation, normal global function and left ventricular with ejection fraction 51%, severe mitral stenosis Wilkin Score 8 with Mitral Valve area 0.53 cm2 and Mitral Valve Gradient 33 mmHg. Subsequently, Balloon Mitral Valvuloplasty was successfully performed. During and after the procedure there was no complication. The fetus was considered in stable condition.

Even though, the incidence of Mitral Stenosis (MS) has reduced grossly, it still remains a health problem in developing countries and is the most common rheumatic valvular lesion encountered in pregnant patients. The already compromised cardiac status of a pregnant female deteriorates further by the presence of MS. So, pregnancy is a common situation during which untreated and frequently poorly tolerated MS are first diagnosed. Percutaneous Balloon Mitral Valvuloplasty (BMV) may be life saving in such a setting and a multidisciplinary approach in the management reduces the mortality and morbidity during the procedure. Anaesthetic management of such a procedure has hardly ever been reported. We report a case of a 23-year-old woman presenting at 28 weeks gestation with severe MS and severe pulmonary hypertension (52 mmHg) who underwent successful percutaneous BMV under monitored anaesthesia Care. The anaesthetic considerations in such situations are being discussed.

Mitral stenosis (MS) is the most common valvular heart disease associated with pregnancy. The increase in cardiac output during pregnancy results in deterioration of the patient's condition with progress to higher New York Heart Association class. Both general and regional anesthesias have been described for cesarean section in these patients. Anesthetic management of these patients depends upon the severity of the disease. General anesthesia has traditionally been preferred for cesarean section in patients with severe MS. Regional anesthesia has become popular as a safe choice in the recent past for cesarean section in all parturients including those with heart disease. We report three parturients with severe MS who underwent cesarean delivery under graded epidural block successfully. The parturients and the neonates were fine after the procedure.

Le rétrécissement mitral (RM) rhumatismal demeure une valvulopathie fréquente dans les pays en voie de développement. Cependant, les pays industrialisés ont vu l'émergence ces dernières années de nouvelles étiologies de RM; notamment l'origine médicamenteuse et/ou toxique responsable de valvulopathies restrictives aussi bien sténosantes que régurgitantes. Pour cette raison, l'évaluation échocardiographique du RM et surtout, la définition de critères objectifs pour conclure au caractère serré du RM reste toujours d'actualité. Les objectifs du travail sont: évaluer l'existence ou non d'une corrélation directe entre le gradient moyen transmitral (GMT) et la sévérité du RM chez les patients porteurs d'un RM serré ou très serré (critère primaire) et analyser les différents paramètres qui conditionnent le gradient moyen transmitral (GMT) (Critère secondaire). Il s'agit d'une étude transversale monocentrique incluant tous les patients admis au service de Cardiologie du CHU Ibn Rochd de Casablanca pour un RM serré ou très serré, sur une période d'une année (Janvier 2014 à Décembre 2014). Nous avons analysés séparément deux groupes de patients : ceux avec un gradient moyen transmitral10mmHg (groupe2). 50 patients porteurs d'un RM serré ou très serré ont été inclus. L'âge moyen de nos patients est de 41,7 ans avec prédominance féminine (sex ratio: 0,25). 64% de nos patients avaient un RM serré et 36% avaient un RM très serré. 52% (26 patients) avaient un GMT 10mmHg, ce qui suggère l'absence de corrélation directe entre la sévérité du RM et le GMT (coefficient de Pearson R: -0,137). Pour la dyspnée, 80% des patients du groupe 1 étaient dyspnéiques stade II de la NYHA et 70% des patients du groupe 2 étaient dyspnéiques stade III (41%) ou IV (29%) de la NYHA, ce qui signifie l'existence d'une corrélation significative entre le GMT et la sévérité de la dyspnée (R: 0,586 et p: 0,001). L'étude analytique de la fréquence cardiaque et la présence d'une décompensation cardiaque par rapport au gradient moyen transmitral a montré une corrélation significative. En effet, parmi les patients du groupe 1, 96% avaient une FC entre 60 et 100 bpm et aucun patient n'était en décompensation cardiaque. Dans le groupe 2,54% (13 patients) avaient une FC >100 bpm et 7 parmi eux (53%) étaient en décompensation cardiaque gauche. L'étude de la pression artérielle pulmonaire systolique dans les deux groupes de l'étude a révélé l'existence dans notre série d'une corrélation statistiquement significative (R : 0,518 et P : 0,001) entre la PAPS et le GMT. La régularité du rythme et la fonction ventriculaire droite n'étaient pas corrélées au GMT (R : 0,038 et R :-0,002 respectivement). Le gradient moyen transmitral est un bon indicateur de la tolérance du rétrécissement mitral, mais il reflète imparfaitement sa sévérité vue qu'il dépend de plusieurs paramètres hémodynamiques. D'authentiques rétrécissements mitraux très serrés peuvent exister avec des gradients moyens transmitraux
