The use of these traditional peacetime methodological approaches to accounting and analysis of non-combat injuries has led to an underestimation of its high level in the planning of inpatient care for servicemen of the Armed Forces of Ukraine in the area of the Joint Forces operation (anti-terrorist operation). The purpose of the study is a general assessment of the place of combat and non-combat injuries (including poisoning) in the structure of cases of hospitalization of servicemen of the Armed Forces of Ukraine in the area of the Joint Forces operation and anti-terrorist operation. Statistical mathematical processing was performed using computer programs Microsoft Excel and STATISTICA (version 6, StatSoft, Inc.). Based on annual medical reports on 3/med form, the ICD-10 structure of Ukrainian Armed Forces servicemen treated in MTF located in area of Joint Forces Operation for 2015-2020 has been presented. Ranks of disease classes have been calculated. Based on medical reports according Temporary instruction on codification of battle injures, non-battle trauma and diseases in Ukrainian Armed Forces, the structure of casualties admissions to MTF have been pointed out. The leading places in structure of treated servicemen belongs to patients with diagnoses on XIX class of ICD-10 (1st place for 2015-2019, 2nd place in 2020) that was stipulated by relative high levels of non-battle trauma in troops on the East of Ukraine. The comparison of structure of servicemen treated in MTF located in and out area of Joint Forces Operation as well as with structure of adult population (over 18 years old) treated in civil hospitals has been conducted. Prevalence of traumatic pathology levels among patients treated in MTF located in area of Joint Forces Operation in comparison with others treatment facilities data has been revealed. For 2015-2020 the shares of trauma and poison cases in structure of treated servicemen in MTF located in area of Joint Forces Operation significantly decreased that was connected with gradual lowing of battle actions intensity and non-battle trauma level. Based on result of research the necessity to improve curative and diagnostic base of military treatment facilities in area of Joint task operation as well as medical report and non-battle trauma prevention systems has been concluded.

The huge global burden of oncological diseases is growing and measures to counter this complex challenge are high on national health agendas. The Russian National Long-Term Oncology Strategy 2030 defines priorities, goals and directions in the fight against cancer. Italso contains action plans for more effective prevention, earlier and more specific diagnosis and more effective treatment options. Against this backdrop, experts now suggest to complement standard oncology treatment strategies by adding Patient Blood Management (PBM). For many clinical disciplines where a low blood count and considerable blood loss are commonly encountered, this bundle of care is the new standard. Based on clinical and scientific evidence, it aims to optimise medical and surgical patient outcomes by clinically managing and preserving a patients blood. The principles of this comprehensive concept can and must be transferred to oncology, thus offering value in improving cancer care and the efficacy of medical institutions. Accumulating evidence demonstrates that anaemia and iron deficiency, but also thrombocytopenia, blood loss and coagulopathy are independent risk factors for adverse patient outcomes including morbidity, mortality, reduced quality of life and prolonged average length of hospital stay in both surgical and medical patients. For the timely and effective detection and correction of these risk factors, an international network of multi-disciplinary clinicians and researchers has developed PBM. The rapidly growing body of evidence for PBM not only shows improved patient outcomes, but also reduced resource utilisation including the use of allogeneic blood components. The reduction of allogeneic blood transfusion further improves patient safety and outcomes, since transfusion is another independent risk factor for adverse outcomes. Supported by WHO endorsements and following the recommendations of an increasing number of state or national health authorities, PBM is about to become a new standard of care. However, even though the aforementioned risk factors are highly prevalent in oncology settings due to chemo-/radiotherapy and the pathology of the disease, the integration of PBM in standard oncology treatment pathways is lagging behind. Thus, and in support of the Russian National Long-Term Oncology Strategy 2030 to improve quality of oncological care, with the support of the National Association of Specialists in PBM (NASPBM), the PBM Oncology Working Group of the Russian Federation was created, consisting of national and international experts in oncology and PBM. On July 9, 2020, the Working Group met to discuss the rationale for PBM in oncology and to assess the need to implement PBM in Russian oncology care. As a result, the Group recommended to include PBM as an integral part of standard oncology treatment pathways, delineated the action required from facilitating stakeholders in the Russian Federation, determined a roadmap for implementation and developed a national resolution as a call to action on the matter. Presented herein, this resolution acknowledges the global and local impetus to reduce cancer mortality, and the rationale for PBM interventions to improve patient outcomes and alleviate the social and economic burden of cancer on the healthcare system.

Вступ. Відомо, що вплив різних забруднювачів навколишнього середовища, таких, як важкі метали і фосфорорганічні сполуки, викликає різні зміни в організмі людини, які характеризуються вивільненням цитокінів, утворенням активних форм Оксигену, що пояснює виникнення різних імунологічних та запальних процесів. Іони важких металів можуть індукувати або пригнічувати синтез запальних та прозапальних цитокінів. На сьогодні комбінована дія важких металів і фосфорорганічних пестицидів на імунну сис­тему залишається недостатньо вивченою. Мета дослідження – вивчити вплив Плюмбуму ацетату, Купруму сульфату і гліфосату в формі раундапу на стан імунної системи та вміст інтерлейкінів у щурів різного віку. Методи дослідження. Досліди проводили на лабораторних нелінійних білих щурах-самцях трьох вікових груп: статевонезрілих, статевозрілих і старих, яким внутрішньошлунково протягом 30-ти днів вводили водні розчини Плюмбуму ацетату, Купруму сульфату і гліфосату (у формі гербіциду раундапу). Стан імунної системи оцінювали за рівнем імуноглобулінів класів A, G, M, циркулюючих імунних комплексів та цитокінів у сироватці крові тварин. Результати й обговорення. Встановлено, що при введенні щурам водних розчинів Плюмбуму ацетату, Купруму сульфату і гліфосату (у формі гербіциду раундапу) в комбінації й окремо порушилось співвідношення імуноглобулінів та інтерлейкінів, підвищилась концентрація циркулюючих імунних комплексів у сироватці крові уражених тварин. При введенні досліджуваних ксенобіотиків окремо та в комбінації у тварин усіх вікових груп пригнічувалась гуморальна ланка імунітету, про що свідчили підвищення концентрації циркулюючих імунних комплексів та зниження вмісту імуноглобулінів, особливо в молодих і старих щурів. Результати досліджень показали, що при дії токсикантів у сироватці крові тварин усіх вікових груп збільшувалась концентрація інтерлейкіну-1β, інтерлейкіну-6, фактора некрозу пухлини-α та зменшувався вміст інтерлейкіну-4, інтерлейкіну-10. Інтоксикація Купруму сульфатом, Плюмбуму ацетатом і фосфорорганічним пестицидом супроводжувалася порушенням балансу цитокінів з переважанням прозапальних, розвитком запальних процесів, що може викликати функціональні та структурні ушкодження клітинних мембран і накопичення токсичних метаболітів. Висновок. Вплив Плюмбуму ацетату, Купруму сульфату і раундапу на запальні процеси призводить до пригнічення імунної системи тварин.

The article presents the case of the development of acute thrombosis of two coronary arteries in a 35-year-old patient without atherosclerotic lesion of the heart vessels according to optical coherence tomography. Acute myocardial infarction in a patient developed on the background of previous diffuse myocarditis, foci of which of different time were identified during magnetic resonance imaging. Smoking and chronic intoxication due to contact with paint and varnish materials and regular intake of alcohol were the only risk factors for myocarditis and coronary thrombosis that were able to be detected in this patient.

The aim – to determine the prevalence and major risk factors of intramyocardial hemorrhage (IMH) in timely revascularized patients with ST elevation myocardial infarction (STEMI), and to evaluate its importance for the development of postinfarction left ventricular (LV) dilatation. Materials and methods. We examined 24 patients with acute first anterior STEMI, who were admitted in the first six (on average 2.8±1.4) hours from symptoms development. The presence of IMH was assessed by cardiovascular magnetic resonance examination 3-4 days after primary percutaneous coronary intervention (pPCI). Echocardiography was performed during the first 24 hours and day 90 after acute myocardial infarction. LV dilatation was defined as at least 20 % increase of end-diastolic volume at 90 days. Endothelium-dependent flow-mediated brachial artery dilatation (FMD) was measured using high-resolution ultrasound at admission. Results and discussion. More than a third (37.5 %) of patients with anterior STEMI who underwent pPCI had signs of IMH. Hemorrhagic transformation of acute myocardial infarction was more often manifested in patients who were prescribed enoxaparin at the prehospital stage (RR = 3.75; 95 % CI 1.47–9.56) and less often in patients with multivessel (≥3) coronary artery disease (RR = 0.21; 95 % CI 0.03–1.00). There is a tendency to a more frequent detection of IMH in patients with endothelial dysfunction. Impaired reactive hyperemia (FMD ≤ 4.9 %) was associated with IMH development (RR 3.5; 95 % CI 0.9–13.5). The patients with IMH had a greater extent of myocardial damage according to CK-MB AUC and LGE at MRI and a more frequent development of postinfarction LV dilatation (RR 5.0; 95 % CI 1.3–19.7). The addition of intravenous quercetin started before pPCI to the standard basic treatment of acute myocardial infarction was associated with a significant decrease in the probability of hemorrhagic transformation (RR 0.21; 95 % CI 0.03–1.00). Conclusions. Pre-hospital administration of enoxaparin and endothelial dysfunction were the main predictors of IMH after pPCI in STEMI patients, whereas it was detected much less frequently in patients with multivessel (≥3) coronary artery disease. The presence of IMG has been associated with a greater extent of necrotized myocardium and more frequent development of postinfarction dilatation and dysfunction of the LV.

We have assessed the implication of analysis of amydolitic activity of hepsin, which is a trans-membrane serine protease of II type, in the detection of prostate cancer diagnosis. According to literature review over-expression of hepsin is characteristic for prostate cancer whereas prostatic cells do not express this protease in normal states and benign tumors of prostate. Our results have demonstrated the value of screening for hepsin activity in the diagnosis of prostatic pathologies which possibly is an adequate substitution for widely used test of prostate specific antigen determination.

Bladder cancer relapses are associated with diffuse pattern of neoplastic changes in the mucous layer of the bladder; treatment efficacy depends on the degree of structural alterations in the bladder wall.Materials and methods: for evaluation of microcirculation characteristics and structural alterations of mucous layer of the bladder we have used laser analyzer of capillary blood circulation («ЛАКК-01» produced by НПП «ЛАЗМА», Russia) and compact mobile optic tomograph (ИПФ РАН, Russia).Results: conducted researches have shown alterations affecting both epithelial and submucous layers with their microcirculatory bed in perifocal to bladder tumor tissues.Conclusions: complimentary application of laser Doppler flowmeter and compact mobile optic tomograph allows defining the type and extent of pathologic process in the bladder wall in superficial tumors and choosing optimal method of correction.

Background. Differentiated thyroid cancer (DTC) is the most common endocrine tumour. Its prevalence varies from 1.0% to 2.2% of all malignant neoplasms. The standard strategy of special treatment of DTC consists of surgery, radionuclide therapy and hormone therapy being sequentially applied. Theoretically, each component of the treatment process can cause adverse somatic consequences in future, the study of which can help to prevent and correct them. Purpose – to evaluate possible long-term effects of the treatment of differentiated thyroid cancer in the form of the urinary system (US) pathologies on the basis of follow-up data of long-term observation using sophisticated information technologies. Materials and methods. The study was based on follow-up data of 157 individuals who were undergoing combination treatment of DTC at the Institute clinic from 1993 to 2015, received it in full and underwent regular screening examinations after treatment. The database created for the study contained, as much as possible, digitized arrays of follow-up data of paper case-records on the disease and its consequences in patients with a follow-up period exceeding 1 year after special treatment. The number of logical records of long-term consequences in the generated database was 463 units – one record for one type of long-term consequences of each of 157 patients. Statistical processing of data was carried out in two ways: comparing the incidence of US pathologies before treatment and at a long time after special treatment and identifying factors of statistically significant influence on the occurrence of US pathologies among the treatment peculiarities. WizWhy packages (Data Mining category) and the general purpose software package STATISTICA were used to make hypotheses and test them. Results and discussion. The analysis of the available references has shown that there is only a limited number of papers dealing with the US status of DTC patients. However, a comprehensive analysis of the long-term consequences of treatment of DTC patients revealed a statistically significant increase of US disease cases within the period of 3.75 – 4.8 years after special treatment. It was shown that the total number of US pathologies was 2.04 times higher in comparison with US incidence before the oncet of special treatment. These differences did not depend on age: the median for age of patients before treatment was 51 years, after treatment – 50 years. Additionally, it has been found that patients with or without episodes of postoperative hypothyroidism decompensation with a dose of L-thyroxine not exceeding 2.5 μg/kg need special attention due to the occurrence of urolithiasis. The obtained dependences are quite predictable, since, firstly, it is known that thyroid hormones affect kidney development and physiology, and secondly, the major percent of 131I in radioiodine therapy is excreted with the urine and deposited in the bladder, that can result in a radio-induced carcinogenic effect. Conclusions. Special treatment of DTC increases US pathologies more than twice. The term of post-treatment pathologies is 2.5 (3.75 – 4.8) years. Patients who have episodes of postoperative hypothyroidism during hormone therapy with a dose of L-thyroxine not exceeding 2.5 μg / kg represent the high-risk group., Актуальність. Диференційований рак щитоподібної залози (ДРЩЗ) є найбільш поширеною пухлиною ендокринної системи. Його розповсюдженість коливається у межах 1,0 – 2,2% від усіх злоякісних новоутворень. Стандартна тактика спеціального лікування ДРЩЗ складається з послідовного застосування хірургічного втручання, радіонуклідної терапії і гормонотерапії (ГТ). Теоретично кожна ланка лікувального процесу може бути ініціатором появи в майбутньому небажаних соматичних наслідків лікування, вивчення яких може сприяти їх запобіганню та корекції. Мета роботи – оцінити можливі віддалені негативні ефекти лікування ДРЩЗ у вигляді патологій сечовидільної системи (СВС) на підставі катамнестичних даних тривалого спостереження з використанням сучасних інформаційних технологій. Матеріали та методи. Підставою для проведення дослідження були катамнестичні дані 157 фізичних осіб, які проходили комбіноване лікування ДРЩЗ у клініці інституту з 1993 по 2015 роки у повному обсязі та регулярні скринінгові обстеження після його завершення. База даних, сформована для проведення дослідження, містила у максимально доступному обсязі цифровані масиви катамнестичних даних паперових історій хвороби про перебіг та наслідки захворювання пацієнтів із періодом спостереження, що перевищував 1 рік після спеціального лікування. Кількість логічних записів про віддалені наслідки у сформованій базі даних становила 463 одиниці – один запис на один вид віддаленого наслідку кожного з 157 пацієнтів. Статистичну обробку даних проводили за двома напрямками: порівняння частоти зустрічальності патологій СВС до початку лікування та на віддалених термінах після спеціального лікування й виявлення факторів статистично значущого впливу на появу патологій СВС серед характеристик лікування. Для висунення гіпотез та їх перевірки використовували пакети WizWhy (категорія Data Mining) та пакет програм загального призначення STATISTICA. Результати та їх обговорення. Аналіз доступних джерел літератури продемонстрував, що існує лише обмежена кількість робіт, присвячених стану СВС у хворих на ДРЩЗ. Проте комплексний аналіз віддалених наслідків лікування таких пацієнтів виявив суттєве статистично значуще підвищення рівня захворювань органів СВС у період 3,75–4,8 роки після спеціального лікування. Показано, що загальна кількість патологій СВС збільшилася в 2,04 рази по відношенню до захворюваності СВС перед початком спеціального лікування. Вікової залежності ці відмінності не мали: медіана віку пацієнтів до лікування – 51 рік, після нього – 50 років. Додатково було з’ясовано, що особливої уваги з огляду на виникнення сечокам’яної хвороби потребують пацієнти, в яких на фоні гормонотерапії спостерігаються епізоди декомпенсації післяопераційного гіпотиреозу на фоні прийому дози L-тироксину, що не перевищувала 2,5 мкг/кг Отримані залежності є доволі прогнозованими, оскільки, по-перше, відомим є факт, що гормони щитоподібної залози впливають на нирковий розвиток та фізіологію, і, по-друге, значний відсоток 131I протягом радіойодотерапії виводиться із сечею і депонується в сечовому міхурі, що може сприяти появі радіоіндукованих ефектів. Висновки. Спеціальне лікування ДРЩЗ призводить до збільшення патологій СВС більш ніж удвічі. Термін появи післялікувальних патологій становить 2,5 (3,75–4,8) роки. Групою підвищеного ризику визнані пацієнти, в яких під час гормонотерапії мають місце епізоди декомпенсації післяопераційного гіпотиреозу на фоні прийому дози L-тироксину, що не перевищує 2,5 мкг/кг.

Introduction. Lead is a protoplasmic poison and has detrimental effects on the nervous, cardiovascular system and blood as all heavy metals. It violates enzymatic reactions, depress the immunobiological resistance of the body. Lead affects humoral immune response, that is impairment of antibody production, inhibits the production of M, E and A immunoglobulins classes and increases respiratory diseases frequency in people affected by Plumbum compounds. The aim of the study– to reseach the effect of cystile-histidyl-tyrosyl-histidil-isoleucine peptide on the immune system state and content of protein fractions in blood serum in all ages rats with chronic toxicosis, heavy metals and glyphosate. Research Methods.Experiments were carried out on laboratory nonlinear white male rats of three age groups: puberty, mature and old animals, which were injected intragastrically for 30 days with Plumbum acetate, Cuprum sulfate, glyphosate (in the form of a herbicide roundup). For the purpose of correction for 21 days 6 hours after the administration of the toxicants for 10 days, the cystile-hystidyl-tyrosyl-histidil-isoleucine peptide was injected. The state of the immune system was assessed by the level of A, M, G immunoglobulins and circulating immune complexes in the blood serum of rats. Results and Discussion.It was established that in intact rats when combined action of Plumbum acetate, Cuprum sulfate, glyphosate (in the form of a herbicide roundup), the concentration of circulating immune complexes (CIC) and immunoglobulins in the blood serum of animals increased with age. The introduction of the peptide leads to partial restoration of the ratio of protein fractions in blood serum of rats and the content of immunoglobulins. Conclusion. The use of the peptide as a correction factor partially normalizes the state of the immune system and the ratio of protein fractions in blood serum of rats with toxicosiscaused by Plumbum acetate, Cuprum sulfate and the roundup.

Although perceived stress has been shown to play an important role in insomnia, little is known about the mediating and moderating mechanisms underlying this relation. In particular, the outbreak of COVID-19 has undoubtedly had a significant impact on the physical and mental health of college students. The present study focused on Chinese college students during the COVID-19 pandemic and examined the mediating role of sense of security between perceived stress and insomnia and the moderating role of family cohesion. Chinese college students (N = 1,187) completed the measures of perceived stress, sense of security, insomnia, and family cohesion. The results indicated that perceived stress was significantly and positively associated with insomnia, and sense of security partially mediated this relation. Moderated mediation analysis further indicated that family cohesion moderated the relation between perceived stress and sense of security. This relation became weaker for college students with higher levels of family cohesion. Results highlight the significance of identifying the mechanisms that moderate the mediate paths between perceived stress and college students’ insomnia.

Aim of investigation. To estimate the effect of carriage of endothelial dysfunction and hemostasis gene allelic variants on development of cryoglobulinemic vasculitis (CryoVas) and its clinical symptoms in patients with chronic hepatitis C (CHC). Material and methods. Original study included overall 72 patients with CHC and cryoglobulinemia: 21 patients had CryoVas, 51 - CHC without vasculitis. Results. The T allele of NOS3 894 G/T gene was more frequent, T allele of ITGA2 807 C/T gene (OR =0.419) was less common and C allele of ITGB3 1565 T/C gene was more frequent (OR=2.33) in CryoVas patients. Multivariate assay revealed the number of mutant alleles («total score») of NOS3 894 G/T, ITGA2 807 C/T and ITGB3 1565 T/C genes as independent risk factor. The T allele of NOS3 894 G/T gene was found in patients with renal disease significantly less frequently (15% vs 45.5%; р=0.035) as well as in patients with systemic hypertension (30% and 72.7%; р=0.086). Conclusions. Carriage of GT genotype of NOS3 894 G/T gene and CC genotype of ITGB3 1565 T/C gene is associated with high risk of CryoVas development. The TT genotype of ITGA2 807 C/T gene possess «protective» properties. The «total score» is independent predictor of CryoVas development.

Aim of investigation. To study the dynamics of viral hepatitis detection rate after implementation of donors preliminary testing for alanine aminotransferase (ALT) activity. Material and methods. Viral hepatitis markers detection rate by immuno-chemiluminescence assay at testing for ALT activity in venous (n=47080) and capillary (n=22530) blood of donors was studied. Results. Males had higher ALT activity in comparison to that in females, both in venous (for 54.1%), and in capillary (for 27.2%) blood. ALT capillary blood activity was higher on the average, than in venous, for 10.1% in men and 33.3% in women. Conclusions. ALT activity elevation is associated by twofold increase in the rate of lacteous serum and more frequent detection of hepatitis serological markers. In the samples received after capillary blood culling the anti-hepatitis C virus antibodies positivity rate was decreased. No relation of ALT activity to results of parenteral virus genomes screen was revealed.

In conditions of the vegetation experiment with barley under the artificial illumination, we assessed the ability of lanthanum, which is one of the most widely spread lanthanons, to accumulate in leaves and stems of plants. Also we assessed its ability to affect the elemental composition, biomass and chlorophyll and carotinoides content of plants. The separate studies allowed us to investigate the impact of lanthanum on root length and coleoptiles of barley seedlings. It is shown that La3+ is able to depress significantly the growth of rootlet if its concentration will exceed 10 mg/l and simultaneously stimulate the growth of coleoptile at the concentration exceeding 50 mg/l. The vegetation experiment showed the significant accumulation of lanthanum in the leaves and stems beginning from the exceeding the concentration of 20 and 50 mg/kg of lanthanum within the soil. It is marked that the accumulation of lanthanum in leaves is higher than in stems. The significant decrease of the chlorophyll and carotinoides in the variant with the implementation of 100 mg/kg of lanthanum comparing to the control and the accumulation of ground biomass at the concentration of the implemented lanthanumof 100-200 mg/kg. Theaccumulationofbiomass, whichwasrevealed in the vegetation experiment, correlates with the stimulating impact of lanthanum on the elongation of coleoptile. Taking into account the simultaneous decrease of the root length, the elongation of coleoptile may be explained by the hormonal process. The results obtained may be useful for re-assessment of the security of the implementation of lanthanons in agriculture and the impact of lanthanum on the biochemical processes in plants.

Objective: to define clinical, epidemiological and entomological characteristics of malaria in the Krasnodar Territory in 2001-2014. Materials and methods. The clinical and epidemiological aspects of malaria in 25 patients hospitalized in the regional clinical hospital for the period 2005-2014 were studied. A comparative analysis of the incidence of malaria was done, entomological indicators of malaria season in the region for pathogens vivax in the last 3 years were considered. Results and discussion. In the Krasnodar Territory malaria is imported mainly from Africa (68%), more than half of cases (54.2%) - local residents, Russian citizens. Foreign citizens (Africans, migrants from Tajikistan) amounted to 45.8%. Basically middle-aged (21-59 years) men (88%) are subjected, traveling abroad for professional purposes, the tourists among the cases amounted to 15.4%. The residence time in an endemic area ranged from 16 days to 24 months, with regular chemoprophylaxis absence in 80% of cases. Cases of imported malaria in were recorded mainly from May to September, which corresponds to the epidemiological season with a high risk of disease transmission. In the etiological structure of imported malaria Pl.falciparum (64%) prevailed, cases of Pl.vivax were 32%, in one case (4%) Pl.ovale was found. Reference clinical and epidemiological features of imported malaria were acute onset of high fever, chills, headache, general weakness with the presence of the majority of patients with hepato-splenic syndrome, thrombocytopenia, in half the cases at a tropical malaria dyspeptic symptoms with increasing theirfrequencies in parallel of the disease severity were observed. For timely diagnosis, in addition to knowledge of the main clinical symptoms and disease complications, of paramount importance was the identification and registration of epidemiological history (stay in endemic area), knowledge of the disease phases, the duration of the incubation period for different kinds of invasions and timely screening for malaria patients with fever of unknown origin. Conclusion. Epidemiological and entomological studies confirm the presence in Krasnodar Territory of high malariogenic potential with the duration of the epidemic season, the possible transfer of the three-day malaria from May to October. Retention of the threat of imported malaria requires necessary level of knowledge among doctors in the diagnosis and prevention of malaria and ensuring effective health care facilities with antimalarial drugs.

Об’єкт дослідження – лікарський препарат 0,05 % розчин хлоргексидину для місцевого та зовнішнього застосування фармакопейної якості. Дослідження виконали на приладі Капель-104Т, що обладнаний ультрафіолетовим фотометричним детектором, який працює при довжині хвилі 254 нм, а також кварцевим капіляром завдовжки0,5 м до детектора, з внутрішнім діаметром 75 мкм, джерелом високої напруги позитивної полярності з регульованою напругою від 1 кВ до 25 кВ і персональним комп’ютером із програмним забезпеченням Мультихром.Електрофоретичне визначення кількісного вмісту хлоргексидину в 0,05 % розчині для місцевого та зовнішнього застосування виконали при позитивній напрузі на капілярі 16 кВ і рекомендованому термостатуванні капіляра від 20 °С до 30 °С. Введення проб здійснювали пневматично в режимі 30 мБар протягом 5 секунд. Час аналізу – 5 хвилин. Як провідний електроліт застосовували водний розчин, що складається з 0,6 % розчину імідазолу, 0,6 % розчину натрію тетраборату та 0,5 % розчину кислоти винної. Визначення хлоргексидину в розчині, що вивчали, виконали в УФ-області спектра при довжині хвилі 254 нм, оскільки при цій довжині хвилі спостерігали один із максимумів поглинання цього лікарського препарату.Результати. Валідаційний аналіз показав, що методика характеризується специфічністю, лінійністю в діапазоні концентрації від 100 мкг/мл до 700 мкг/мл (y = 0,3438х - 12.4789; r = 0,9913), правильністю для вмісту хлоргексидину 80–120 % (R = 98,82–100,44 %), прецизійністю (Sr= 0,44–0,61 %). Відносна похибка результатів окремого визначення хлоргексидину в 0,05 % розчину з використанням капілярного електрофорезу становила 1,28 %.Висновки. Виконали кількісне визначення 0,05 % розчину хлоргексидину методом капілярного електрофорезу. Запропонували електрофоретичні умови, котрі можуть бути основою для розроблення методики кількісного визначення хлоргексидину біглюконату у складі цього лікарського препарату., The aim of the work was to estimate the quantitative content of 0.05 % chlorhexidine solution by capillary electrophoresis.Materials and methods. The object of the study was the drug 0.05 % solution of chlorhexidine for local and external use of pharmacopoeia quality. The study was carried out on a Kapel-104T device, equipped with an ultraviolet photometric detector and personal computer with the Multichrom software.Electrophoretic determination of the quantitative content of chlorhexidine was carried out in the UV region of the spectrum at a wavelength of 254 nm with a positive voltage on a capillary of 16 kV and a recommended capillary thermostating from 20 °C to 30 °C. The sample was introduced in pneumatical mode in the 30 mbar for 5 seconds. The analysis time was 5 minutes. As the leading electrolyte an aqueous solution consisting of 0.6 % solution of imidazole, 0.6 % solution of sodium tetraborate and 0.5 % solution of tartaric acid were used. The determination of chlorhexidine in the test solution was carried out in the UV spectral region at a wavelength of 254 nm, since at this wavelength one of the absorption maxima of the indicated drug was observed.Results. The validation analysis showed that the technique is characterized by specificity, linearity in the concentration range from 100 μg/ml to 700 μg/ml (y = 0.3438х - 12.4789; r = 0.9913), accuracy for chlorhexidine levels of 80–120 % (R = 98.82–100.44 %), precision (Sr = 0.44–0.61 %). The relative error of the result of separate determination of chlorhexidine in 0.05 % solution using capillary electrophoresis was 1.28 %.Conclusions. Quantitative determination of 0.05 % chlorhexidine solution by capillary electrophoresis was carried out and the parameters of the validation of the electrophoretic technique were established. The proposed electrophoretic conditions can serve as the basis for the development of methods for the quantitative determination of chlorhexidine bigluconate as part of this drug., Цель работы – оценка количественного содержания 0,05 % раствора хлоргексидина методом капиллярного электрофореза.Материалы и методы. Проведено количественное определение 0,05 % раствора хлоргексидина методом капиллярного электрофореза и установлены параметры валидации электрофоретической методики. Предложенные электрофоретические условия могут быть основой для разработки методики количественного определения хлоргексидина биглюконата в составе данного лекарственного препарата.

Abstract Background: IPN and its related septic complications are the major causes of death in patients with ANP. Therefore, prevention of pancreatic infection is of great clinical value in the treatment of ANP. This study aimed to evaluate the role of Thymosin Alpha 1 in treating ANP. Methods: Eligible patients were randomised to the treatment group(Thymosin alpha 1 group) or the control group. Occurrence of IPN will serve as the primary outcome measure of the TRACE trial. Occurrence of new-onset organ failure and persistent organ failure; CRP, HLA-DR, WBC day0, day 7 and day14; other related pancreatic complications and prognosis of the patients were also calculated. Discussion: The TRACE trial is designed to test the effect of a new therapy focusing on the immune system in preventing secondary infection following ANP, which is a potentially lethal complication causing substantial morbidity and mortality. We also aimed to prospectively investigate the effect of immunomodulatory treatment with thymosin alpha 1 in patients with different severity of diseases with predefined subgroup analysis. The results of the TRACE trial would potentially provide a novel therapeutic option in the early management of ANP and identify the patient population who may benefit most from immunomodulation. The trial was registered on the June 16, 2015 in the CT.gov register (NCT02473406)

The article presents data on the peculiarities of the prescription of cereal complementary feeding for young children. It is emphasized the importance of manufactured cereals in young child feeding. The article analyzes data on the nutritional value and chemical composition of the most popular buckwheat and oatmeal porridge in the Russian Federation, as well as the benefits of cereal as the first complementary feeding for an infant. The characteristics of the different manufactured cereals for infant feeding prescribed to healthy children are given: dry instant, ready-to-use liquid, and fruit and grain cereals. The article also presents the rationale for the prescription of cereals at certain pathological conditions — food allergy, cow's milk protein intolerance, celiac disease, digestive problems, as well as information about the nutritional value of milk and dairy-free cereals.

The main goal of the current research includes studying the biological signal field (BSF) characteristics of the wolf (Canis lupus lupus Linnaeus, 1758) at different values of anthropogenic load on territories with conservation (Białowieża National park (Poland)) and hunting status of the species (Chornobyl Exclusion Zone (Ukraine)). The research in Białowieża Primeval Forest was conducted in two stages: study of the BSF characteristics of the wolf and finding correlation between data acquired from Ukraine (the first stage), and over-time study of intensity of the biological signal field (the second stage). In result of the first stage, there was no significant dependence on the characteristics of the territory and the differences between the behavior of wolves in the Białowieża Primeval Forest (conservation status of the species) and the Exclusion Zone (game status). During the second stage it was determined that provided variance of the intensity between territory groups was insufficient, the degree of significance to animals of area categories varied with the stages of the pack’s life. It was found that the main factors which govern the character of wolf activity are not the level of the anthropic load and hunting pressure, but periods of the life cycle and spatial structure of groups.

Background: Vitamin D insufficiency is a widespread phenomenon, having an adverse effect on human health. This problem is also actual in Russia, which most part of the territory is located above the 42nd latitude and is at risk for vitamin D insufficiency. Objective: Our aim was to evaluate the vitamin D sufficiency of the child population of a junior age group and the adequacy of a drug treatment/ prevention of vitamin D insufficiency in various regions of the Russian Federation. Methods: Children of the first 3 years of life, who received in-patient treatment in medical institutions of Russia, were examined. When they were involved in the study, they took a blood test to determine a plasma 25(OH)D concentration. The study period: November, 2013 – October, 2014. Results: 1,230 children aged from 1 month to 3 years were examined. Vitamin D insufficiency (25(OH)D concentration in the blood plasma 21–29 ng/ml) is found in 300 (24.4%), deficiency (25(OH)D < 20 ng/ml) — in 513 (41.7%) children. The prevalence of vitamin D insufficiency and deficiency did not depend on a geographical location and level of insolation of regions, where the surveyed lived. Conclusion: In the studied selection more than 2/3 of children of a junior age group have a vitamin D insufficiency or deficiency. In this regard, it is expedient to revise the current guidelines taking into account modern approaches to the prevention and treatment of a vitamin D insufficiency, to make the appropriate changes and to implement them in a clinical practice.

Objective: to detect previously undiagnosed arterial hypertension in patients with chronic obstructive pulmonary disease (COPD) as a risk factor for cardiovascular mortality.Materials and methods. 43 patients with stage I–II of COPD and the absence of clinical signs of cardiovascular diseases were examined. Spirometry, body plethysmography and diffusing lung capacity (DLCO) were included in the respiratory system assessment. The cardiovascular system was assessed with echocardiography and ambulatory blood pressure monitoring (ABPM).Results. Despite the absence of obvious signs of cardiovascular lesions (an increase of office blood pressure, intracardiac hemodynamic changes), the following cardiovascular risk factors were identified: age (58.2 ± 2.0 years), male gender, smoking, hypercholesterolemia and dyslipidemia (total cholesterol 5.9 ± 0.9 mmol / l, low density lipoproteins 3.8 ± 0.5 mmol / l, triglycerides 1.8 ± 0.2 mmol / l). Correlation analysis has revealed the relation between several respiratory parameters and the severity of dyspnea and quality of life in patients with COPD, as well as its relation with lipid levels.Conclusion. The patients with COPD have a large number of risk factors for CVD. According to ABPM data, arterial hypertension was verified in 18 (41.9 %) of 43 patients with COPD at normal level of office blood pressure; moreover, 51.2 % of patients demonstrated low reduction of blood pressure during the night-time that nowadays, is considered to be a predictor of cardiovascular disease and sudden death.

Most of methods for lung diseases diagnosis are invasive or not suitable for children and patients with severe disease. One of the promising methods of clinical diagnosis and disease activity monitoring of bronchopulmonary system is analyzing of human breath. Directly exhaled breath or exhaled breath condensate are using for human breaths analyzing. Analysis of human breath can apply for diagnostic, long monitoring and evaluation of efficacy of the treatment bronchopulmonary diseases. Differential diagnostic between chronic obstructive lung disease (COPD) and bronchial asthma is complicated because they have differences in pathogenesis. Analysis of human breath allows to explore features of COPD and bronchial asthma and to improve differential diagnostic of these diseases. Human breaths analyzing can apply for diagnostic dangerous diseases, such as tuberculosis, lung cancer. The analysis of breath air by spectroscopy methods is new noninvasive way for diagnosis of bronchopulmonary diseases.

Endophytes and plants can establish specific long-term symbiosis through the accumulation of secondary metabolites. Interactions between microbial inhabitants represent a novel area of study for natural products research. In this study, a strain of endophyte 3R-2 that can enhance the biomass and contents of ginsenoside Rc, ginsenoside Rg2 and ginsenoside Rg3 ofPanax ginsenghairy roots was screened out via HPLC, which was identified asSchizophyllum communethrough the morphological and molecular identification. On the base, we found the infection of the endophyte were obviously observed widely in theP. ginsengand the strain formed a stable relationship withP. ginsenghairy roots in parenchyma cells around through tissues embedding slicing, HE ammonium silver staining and immunofluorescence staining. On the other hand, elicitors of fungus 3R-2 can also significantly promote hairy root growth and contents of several ginsenosides, even several times higher than 3R-2 mycelium did. Moreover,S. commune3R-2 mycelium and its elicitor could enhance the transcriptional activity of key genes during the ginsenosides biosynthetic pathway dramatically. Thus, endophyteS. commune3R-2 and its elicitor change the chemical substance content by regulating the expression of genes involved in the secondary metabolite biosynthetic pathway.

Introduction. In recent years, scientific articles of domestic and foreign scientists obtained numerical data on the important role of oxidative modification of proteins in the development of many diseases. It is known that entering the organism of toxic compounds, including toxins of Amanita phalloides , sharply oppress antioxidant system increases the contents of peroxidation products lipids and proteins in liver, kidneys. The aim of the study – to examine the dynamics of oxidative modification of proteins in rats of different ages affected by poison Amanita phalloides toxins in different periods of the experiment. Methods of the research. In all experimental groups poisoning was modeled by single intraperitoneal administration Amanita phalloides extract obtained by the method of H. Wieland (85 mg / kg body weight). Euthanasia was performed under sodium thiopental anesthesia at 6, 24 and 72 hours after poisoning. Oxidative modification of proteins in blood serum was investigated by Meschyshen method. Results and Discussion. Protein oxidative modification indices in all age groups rats on the model of toxic injury by Amanita phalloides toxins were investigated. It was found out that the progression of the pathological process in the liver and the depletion of the compensatory capacity in animals with acute poisoning by Amanita phalloides leads to the activation of free radical oxidation of proteins and cause the increasing of protein oxidative modification indices. The most pronounced changes occur in 6- and 18- month rats in 24 hours after intoxication. Conclusions. It was defined, that the progression of the pathological process in the liver and the depletion of the compensatory capacity as a result of poisoning by toxins of Amanita phalloides cause the increasing of protein oxidative modification indices. It can be considered as one of the earliest markers of damage to organs and tissues including hepatocytes and reliable early predictor of the development and pathology related oxidative stress.

Objective: to evaluate morphofunctional changes in the cardiovascular system of pregnant women with arterial hypertension (AH) to detectchronic AH - hypertensive disease.Subjects and methods. 126 pregnant women with AH (at 28–34 weeks gestation; mean age 26.1 ± 1.7 years) were examined. All the pregnantwomen underwent assessment of risk factors for AH, double measurement of office blood pressure, 24-hour blood pressure monitoring, electrocardiography, carotid ultrasonography, a microalbumin urine test, and eyeground examination. The pattern of AH was specified 12 weeks after childbirth.Results. Chronic AH – hypertensive disease – was diagnosed in 51 % of the examined pregnant women with AH. There was a considerable spread of risk factors for AH in this patient group. 26 % of them were found to have lesions of target organs (heart and/or arterial vessels); the rate of microalbuminuria registration was 41 %.Conclusion. Among our examined group of pregnant patients with AH, the prevalence of hypertensive disease is 51 %. In the identified patientgroup, cardiac and arterial vascular changes that could be considered as target organ lesions were found in almost a third of cases. Pregnant women with AH need to be meticulously examined and followed up by a therapist and a cardiologist in the postpartum period.

Objective: to compare the therapeutic possibility of the decompressiveviedecompressive with stabilization surgeries using the standard posterior and anterior retroperitoneal approaches in patients with combination of inherent and obtaining lateral stenosis and to demonstrate the adequacy of using. At the main group we removed the herniated disc with stabilization using anterior and posterior approaches – 82 patients. The control group was treated by standard microdiscectomy – 40 patients. More excellent and good results were in the main group where decompression was combined with stabilization, and at the same group were less negative results. The decompressive-stabilizing surgery with anterior interbody fusion is a pathogenetic and technically adequate treatment for combined lateral stenosis.

Although the Val34Leu polymorphism in blood coagulation factor XIII-A (FXIII-A) has been implicated in the pathogenesis of intracerebral hemorrhage (ICH), the results of research conducted thus far have been inconclusive. In this meta-analysis, we have assessed the association between the FXIII-A Val34Leu polymorphism and ICH risk. Published reports pertaining to this association were retrieved from the PubMed database, and the data from these studies were pooled and statistically analyzed with Stata 12.0. Summary odds ratios (OR) and 95% confidence intervals (95%CI) were calculated according to a fixed-effect or a random-effect model (as appropriate). The initial search identified 520 articles, only seven of which (retrospective studies) met the inclusion criteria and were included in this meta-analysis. These studies comprised 727 ICH patients and 1968 controls. The results of a combined analysis showed no significant association between the FXIII-A Val34Leu polymorphism and ICH risk in the overall population (Leu/Leu vs Val/Val: OR = 1.41, 95%CI = 0.82-2.43; Val/Leu vs Val/Val: OR = 1.08, 95%CI = 0.89-1.30; dominant model: OR = 1.14, 95%CI = 0.95-1.36; recessive model: OR = 0.72, 95%CI = 0.43-1.22). The results of this meta-analysis suggest that the FXIII-A Val34Leu polymorphism is not associated with ICH risk in a Caucasian population. Further large and well-designed studies must be conducted to confirm this preliminary conclusion.

The aim of this work was to examine the effect of heavy metal ions and phosphororganic pestecies which contain glyphosate on activity antioxidative enzymes in the serum and liver of rats. The effect of peptide tsysteil-histidiltyrosilhistidil-isoleucine on the state of antioxidant protection (superoxide dismutase, catalase) and lipid peroxidation was reached. The peptide exhibits antioxidant activity, the correction of the peptide increases antioxidant enzymes activity and concentration of glutathione.

The reduced glutathione content and the glutathione metabolism enzymes activity in organs and blood plasma of rats under conditions of lead acetate, copper sulphate and glyphosate in Roundup form were studied. The glutathioneperoxidase, the glutathionereductase activity and the concentration of glutathione decreases during action of heavy metals and glyphosate. It was found out that peptide exhibits antioxidant activity, the correction of the peptide and glutathione content.

Original method of nonneoplastic tracheoesophageal fistula disjunction using compression construction with shape memory has been developed. The experimental study was performed on mongrels with preliminarily made tracheoesophageal fistula. The technique consists of fistula covering with pedicle fat flap and following clipping by the construction. In all cases tracheoesophageal fistula was eliminated simply and reliably. Macro- and microscopic investigation of intervention area revealed, that continuity of typical epithelium of trachea and esophagus was regenerated by the 14 day after operation.

Original surgical treatment for tracheobronchomalacia using external stabilization of movable segment with titanium nikelide implant has been developed. The experimental study was performed on 10 mongrels. Tracheomalacia was created by submucosal resection of tracheal cartilages. Efficacy of the method was tested using clinical, radiological, endoscopic and morphological examination. It is ascertained, that the technique permits to eliminate redundant mobility of tracheal segment simply and reliably.

Our research is of theoretical and practical importance and aims at the development of ophthalmological dosage form comprising mineral component (bischofite). It is mineral complex consisting of 96% magnesium chloride with macro- and microelements. Having carried out complex physicochemical and biopharmaceutical researches in vitro, we developed composition and technology of 10% bischofite solution in the form of eye drops. After is one of the perspective tools in technological researches. After using of the analytic hierarchy process by T. Saaty the optimal eye drops composition was selected according to all chosen criteria for this pharmaceutical form.

The research objective consisted in current studying puberty the period at boys with type 1 diabetes. Boys have entered into research 11—18 years. The estimation of physical, sexual development, a lipid and hormonal spectrum was spent; ultrasonic research inguinal and scrotal areas were estimated. Prolongation of early stages of puberty was marked, in 9,4 % of cases the delay of sexual development is revealed.

Method of postresection pericardial defect closure using fabric implant on basis of nanostructural nikelide titanium fiber has been developed and tested on ten mongrels. Morphology of reclaim generated after pericardial reconstruction using fabric implant was studied. A new generated tissue has been shown to extend through implant and form single tissue reclaim, which keeps anatomico-physiological restoration of pericardial defect.

Carried out beam monitoring and studied histologic preparations. Results have shown that implanted into the organism devices of titanium nickelid are deformed in accordance with regularity of organism tissue elastic behavior thus providing with harmonious functioning of the whole system «implanted tissue — organism».

The correlation of the 308G/A polymorphism of α TNF (tumor necrosis factor) gene and 251А/Т polymorphism of 8 IL8 (interleukin 8) gene with clinical peculiarities of the course of chronic obstructive lung disease (COLD) in Siberian Tatars and Russians has been studied. Associations of these genetic markers with quantitative indices pathogenetically significant in the COLD formation (biochemical and immunological indices of blood serum and sputum) have been analyzed.The analysis of associations of polymorphous versions of TNF and IL8 genes with biochemical and immunological indices of blood serum and sputum has revealed the presence of interethnic peculiarities in the regulation of the level of proinflammatory and protective mechanisms participating in the formation of chronic inflammation at COLD.

The condition of a bone fabric at 102 sick diabetes mellitus type 1 (48 men and 54 women, middle age (38,2 ± 12,2) years) is studied, also are specified influence of various factors on development osteopenic syndrome. Research methods included clinical and history of development of disease, beam, biochemical, statistical methods of processing of results. As a result of research it has been revealed that indicators of bone durability at sick of a diabetes corresponded osteopenia which was accompanied by infringement micro-arhcitecture bones in the form of decrease in relative volume trabecular bones and quantities trabecula. Osteosonographie it was accompanied by changes of markers of a bone metabolism (osteocalcin and C-trailer tilopeptid) testifying to activation of processes osteolysis at sick of a diabetes.
