We are presenting the case of a 53-year-old woman with a history of Sjögren syndrome and a secondary antiphospholipid syndrome admitted at the Nephrology department for the evaluation of renal failure. The patient was initially diagnosed with tubulointerstitial nephritis and subsequently a membranoproliferative type I glomerulonephritis, secondary to cryoglobulins during the course of the disease. Repeated renal biopsies were required to confirm the diagnosis.

Se realizó un estudio observacional, descriptivo, prospectivo con todos los pacientes que habían recibido un trasplante renal en el Instituto de Nefrología desde el 2002 hasta el 2004. Se les realizó gastroduodenoscopia con prueba de ureasa para diagnóstico de Helicobacter pylori y alteraciones asociadas en el primer mes del trasplante y se repitió el estudio a los 6 meses y al año. Se encontró una prevalencia de la infección de 34,1 por cada 100 pacientes. Se halló con mayor frecuencia entre los 40 y 49 años de edad. Se encontraron como alteraciones digestivas asociadas a la infección de forma significativa: la gastritis eritematosa y la úlcera duodenal. Se comprobó que todos los pacientes tratados lograron curar la infección y solo uno con alteración digestiva asociada no resolvió con el tratamiento.A prospective, descriptive and observational study was conducted on all the renal transplant recipients at the Institute of Nephrology from 2002 to 2004. Gastroduodenoscopy in addition to an urease test were performed to diagnose Helicobacter pylori and associated alterations in the first month of transplantation, and then they were repeated six and 12 months afterwards. The infection prevalence rate was 34,1 per 100 patients. It was more frequent in the 40-49 years age group. The most significant infection-associated digestive disorders were erythematous gastritis and duodenal ulcer. It was demonstrated that all the treated patients managed to get over the infection and only one patient with associated digestive alteration did not suceed.

Se hizo la presentación clínica de un paciente de 59 años de edad atendido en el Instituto de Nefrología de Cuba con antecedentes de insuficiencia renal crónica terminal que después de haber estado 73 d en método dialítico recibió un trasplante renal, el 2 de mayo de 1970, de donante cadáver, sin realizarse prueba cruzada ni tipaje HLA como se realiza en la actualidad, solo con estudios de grupo sanguíneo; se usó terapia inmunosupresora imurán y prednisona. Actualmente se encuentra con buena función del injerto por lo que es el trasplantado renal con estas características de mayor tiempo de evolución.The case of a 59-year-old patient that recevied attention at the Nephrology Institute of Cuba with history of end-stage chronic renal failure and that after 73 days on dialysis received a kidney transplant from a dead donor on May 2, 1970, is presented. Neither cross test nor HLA typing were perfomed, as it is usual at present. Only blood group studies were made, and immunosuppression therapy with imuran and prednisone was used. At present, the kidney allograft function is good, which makes him the renal transplanted with the longest time of evolution under these conditions .

Se presentó la evolución clínica de un paciente de 59 años de edad en hemodiálisis desde el año 1994, que llega a la insuficiencia renal crónica (IRC) terminal por la vía aparente de la hipertensión arterial, que luego de 11 años en el proceder dialítico desarrolla un aumento de tamaño de los riñones con grandes quistes, cuyo aspecto en la ecografía y en la tomografía es indistinguible de una poliquistosis renal dominante del adulto.The clinical evolution of a 59-year-old patient on hemodialysis since 1994 that apparently reaches the end-stage chronic kidney failure (CKF) by arterial hypertension is presented. After 11 years under the dialytic procedure, it is observed an increase of the size of the kidneys with large cysts, whose aspect in the echography and in the tomography is undistinguishable from an adult dominant polycystic kidney disease.

Se realizaron 208 biopsias de riñón en 197 trasplantes renales efectuados entre el 2000 y el 2004, 31,8 % correspondieron a donantes vivos y el resto procedieron de donantes cadáveres. Se observó rechazo hiperagudo o agudo acelerado en 6 casos (3,1 %), sospechoso de rechazo en 15 casos (7,9 %), el rechazo agudo tipo 1 resultó ser el más frecuente, fue encontrado en 20 casos (10,5 %), se diagnosticaron 16 casos de rechazo agudo tipos 2 y 3 (8,3 %), todos los pacientes del tipo 3 perdieron la función del injerto, la nefropatía crónica esclerosante se vio en 42 casos (22,1 %). También se analizaron cambios no considerados por rechazo donde la necrosis tubular aguda fue la más frecuente y se encontró en 71 pacientes para un 37,5 %.208 kidney biopsies were performed in 197 renal transplants between 2000 and 2004. 31.8 % corresponded to living donors and the rest were from dead donors. Hyperacute or acute accelerated rejection was observed in 6 cases (3.1 %), and suspicion of rejection in 15 cases (7.9 %). The type 1 acute rejection was the most frequent. It was found in 20 cases (10.5 %). 16 cases of type 2 and 3 acute rejection (8.3 %) were diagnosed. All the type 3 patients lost the graft function. Sclerosing chronic nephropathy was detected in 42 cases (22.1 %). Changes that were not considered as rejection were also analyzed. The acute tubular necrosis was the most common in them, since it was found in 71 patients, accounting for 37.5 %.

Se realizó la presentación clínica de un paciente de 32 años con antecedentes de salud hasta los 29 años de edad en que comienza a presentar una insuficiencia renal crónica terminal, se le aplicó tratamiento dialítico inmediatamente y 5 meses después se le diagnosticó una hepatitis C. Se decidió realizar trasplante renal donante vivo el cual no se puede efectuar dada la marcada citolisis existente, fue tratado inicialmente con interferón 2b durante un año sin negativizar el PCR por lo cual recibió posteriormente tratamiento combinado de interferón 2b y ribavirina (primer caso reportado en nuestro país en un paciente en hemodiálisis), se logró negativizar PCR, desapareció la citolisis y, finalmente, se logró el trasplante renal exitoso (donante vivo emparentado) 2 meses después de culminar el tratamiento. Se presentó la evolución bioquímica, hematológica y virológica con ambas terapéuticas.The case of a 32-year-old male patient with past health until he was 29 years old, when he began to present an end-stage chronic renal failure, was reported. Dialytic treatment was immediately applied and 5 months later hepatitis C was diagnosed. It was decided to perform a kidney transplant from a living donor, but it was not possible due to the existence of a marked histolysis. He was initially treated with interferon 2b for a year without making the PCR negative. Therefore, he received combined treatment of interferon 2b and ribavirine (first case reported in our country of a patient undergoing hemodialysis). The PCR yielded negative, the cytolysis disappeared and the kidney transplant was successfully performed (from a living donor that was his relative) 2 months after concluding the treatment. Se realizó un estudio en 23 pacientes que habían recibido trasplantes renales en el Instituto Nacional de Nefrología, Cuba. Se realizó endoscopia con prueba de ureasa para diagnóstico de alteraciones gastroduodenales e infección por Helicobacter pylori (HP) en los primeros 3 meses posteriores al trasplante. Se encontró que las alteraciones más frecuentes fueron la hernia hiatal, gastritis crónica y úlcera duodenal. Del total, 8 pacientes tenían infección por Helicobacter pylori para un 34,7 % asociada con más frecuencia a gastritis, duodenitis aguda y a úlcera duodenal. Fueron tratados 4 pacientes por tener infección por HP asociada a úlcera duodenal y los 4 fueron curados sin recidiva de la infección ni la úlcera a los 6 meses de terminado el tratamiento.A study was conducted among 23 patients that had received kidney transplants at the National Institute of Nephrology, Cuba. Endoscopy with urease test for diagnosis of gastroduodenal alterations and Helicobacter pylori (HP) infection was performed during the first three months after transplantation. It was found that the most frequent alterations were hiatus hernia, chronic gastritis and duodenal ulcer. Of the total, 8 patients had Helicobacter pylori infection, accounting for 34.7 % associated more frequently with gastritis, acute duodenitis and duodenal ulcer. 4 patients were treated for having HP infection associated with duodenal ulcer and they were all cured. They did not have any relapses either of the infection, or of the ulcer, six months after concluding the treatment.

Se realizó la presentación clínica de un paciente de 40 años de edad a quien se le practicó un trasplante renal de donante cadáver en el Instituto Nacional de Nefrología en Cuba y a los 10 años de evolución se le diagnosticó un abceso de pulmón, primero reportado en Cuba en pacientes con trasplantes renales; respondió a las medidas terapéuticas indicadas que incluyeron antibióticoterapia, reducción de la inmunosupresión y drenaje postural. Se presentaron las lesiones radiológicas evolutivas de esta lesión.The case of a 40-year-old patient who underwent a kidney transplantation from a dead donor at the National Institute of Nephrology, in Cuba, and 10 years later was diagnosed a lung abscess, was reported for the first time in Cuba among patients with kidney transplants. The patient responded to the indicated therapeutic measures that included antibiotic therapy, reduction of immunosupression and postural drainage. The evolutive radiological lesions were presented.

Se realizó la presentación clínica de un paciente de 56 años de edad a quien se le practicó un trasplante renal de donante vivo en el Instituto Nacional de Nefrología, en Cuba y a los 4 ½ meses de evolución se le diagnosticó un sarcoma de Kaposi el cual tuvo una evolución desfavorable, no respondió a las medidas terapéuticas prescritas que incluyeron reducción de la inmunosupresión y radioterapia, se diseminó el proceso tumoral por todo el cuerpo e hizo metástasis a estómago. Se presentaron las lesiones propias de este proceso tumoral.The case of a 56-year-old patient who underwent a kidney transplant from a living donor at the National Nephrology Institute, Cuba, and at the 4 ½ months was diagnosed a Kaposi's sarcoma, was clinically presented. His evolution was not favorable, since he did not answer to the therapeutical measures prescribed that included the reduction of immunosuppression and radiotherapy. The tumoral process spreaded over the whole body and it metastasized to the stomach. The characteristic lesions of this tumoral process were presented.
