Plain films of the calcaneus of 768 patients with confirmed rheumatoid arthritis were examined retrospectively with reference to inflammatory rheumatic changes. 42 patients (5.5%) showed an erosion of the posterior upper calcaneal margin related to an Achilles bursitis. In three patients there were additional plantar erosions. The Achilles bursitis was bilateral in 50% of cases, particularly in patients in stages 2 and 3 according to Steinbrocker. In the majority of bilateral cases (62%) the size or shape of the lesions was asymmetrical. Our observations indicate that involvement of the os calcis is not uncommon in rheumatoid arthritis; routine examination of this bone would appear to be indicated even in patients without symptoms. Since the defect is unilateral in half the patients, unilateral occurrence of an erosive lesion cannot be regarded as a criterion for a bacterial-inflammatory bursitis. Contrary to the symmetrical involvement of joints in the hands in rheumatoid arthritis, defects in the calcaneus are often unilateral or asymmetrical.

The 72 kD heat shock protein (hsp 72) has been postulated to play a role in the development of autoimmune disease and has been shown to be over-expressed in the Graves' disease thyroid gland. The expression and modulation of the 72 kD heat shock protein were therefore studied in cultured human thyroid epithelial cells (TEC). TEC from normal thyroid tissue as well as from non-toxic goitres, thyroid adenomas and Graves' disease thyroids were analysed by Western blotting. Potential modulatory effects of heat shock treatment, TSH, IFNγ, different serum supplements, sodium iodide and sodium selenite were investigated. Hsp 72 was detectable in cells from all tissue types under basal culture conditions and, at increased concentrations, after heat shock treatment. Quantitative evaluation of Western blotting results by densitometer scanning revealed that hsp 72 concentrations were identified in TEC from normal and diseased thyroids. IFNγ increased the expression of hsp 72 under basal culture conditions as well as after heat shock treatment. TSH had no effect. Sodium iodide did not affect hsp 72 under basal culture conditions, but augmented the susceptibility of TEC to the effect of heat shock treatment. In contrast, sodium selenite had no effect on the expression of hsp 72. These results demonstrate that local as well as environmental factors may facilitate the induction of hsp 72 in TEC. This may be an important factor for the initiation and progression of thyroid autoimmunity. The stimulatory effect of iodide on hsp induction may also provide an explanation for the frequent occurrence of thyroid autoimmune diseases after iodine exposure.

Glucuronidation of harmol in microsomal preparations and sulphate conjugation in hepatic 105,000 g supernatant fractions were studied comparatively in human fetal and adult liver subcellular preparations. The formation of harmol sulphate in the fetal liver was slightly lower than in the adult liver and considerably lower than in rat liver. No glucuronidation of harmol was found in fetal liver, while the activity in adult liver was 30-80 nmol glucuronide formed/2 mg/20 min. In an ontogenic perspective, our in vitro findings are consistent with drug metabolic studies in the human neonate in whom negligible glucuronidation but well-developed sulphate conjugation of paracetamol has been demonstrated.

The thyroglobulin (TG) protein is essential to thyroid hormone synthesis, plays a vital role in the regulation of metabolism, development and growth and serves as intraglandular iodine storage. Its architecture is conserved among vertebrates. Synthesis of triiodothyronine (T3) and thyroxine (T4) hormones depends on the conformation, iodination and post-translational modification of TG. Although structural information is available on recombinant and deglycosylated endogenous human thyroglobulin (hTG) from patients with goiters, the structure of native, fully glycosylated hTG remained unknown. Here, we present the cryo-electron microscopy structure of native and fully glycosylated hTG from healthy thyroid glands to 3.2 Å resolution. The structure provides detailed information on hormonogenic and glycosylation sites. We employ liquid chromatography–mass spectrometry (LC-MS) to validate these findings as well as other post-translational modifications and proteolytic cleavage sites. Our results offer insights into thyroid hormonogenesis of native hTG and provide a fundamental understanding of clinically relevant mutations., The iodinated thyroglobulin functions as iodine storage and carrier protein and a precursor for thyroid hormone (TH) biogenesis. Here, the authors report the structure of native, fully glycosylated human thyroglobulin, revealing the location of the hTg hormonogenic and glycosylation sites.

Die Lebertransplantation allein hat sich wegen der raschen Progression systemischer Mikrometastasen zur Therapie von Lebermetastasen nicht bewahrt. Zur Behandlung von Chemotherapie-resistenten und nicht resezierbaren Lebermetastasen haben wir ein neues Konzept entwickelt: Nach orthotoper Lebertransplantation werden die Patienten zur Vernichtung von Mikrometastasen in anderen Organen Ganzkorper-letal-bestrahlt und erhalten ultrahohe Dosen Cyclophosphamid gefolgt von autologer Knochenmarkstransplantation. Zwei Patienten wurden bisher so behandelt: eine Frau mit Brustkrebs ist nach 20 Monaten ohne Anhalt fur ein Rezidiv am Leben, ein zweiter Patient (Coloncarcinom) ist jedoch nach 7 Monaten seinen Lungenmetastasen erlegen.

Obwohl die orthotope Form der Herztransplantation nur bei Patienten mit normalem oder nur geringgradig erhohtem Lungengefaswiderstand (bis maximal 8 Wood-Einheiten) durchgefuhrt werden kann, fand die von Barnard entwickelte Methode der heterotopen Transplantation auserhalb Kapstadts kaum Anwendung. Ein Zustand nach Klappenersatz und eine Angina pectoris sind Kontraindikationen gegen diesen Eingriff, der im Vergleich mit der orthotopen Form sicherlich mit einem erhohten Thromboembolierisiko verbunden und technisch etwas anspruchsvoller ist. Wahrend zwei sonst inoperable Patienten aus dem eigenen Krankengut nach heterotoper Herztransplantation voll rehabilitiert sind, haben wir einen Patienten an einer Aspergillus-Sepsis verloren.

Die Feinnadelbiopsie (FNB) von transplantierten Nieren stellt eine wesentliche Bereicherung der Abstosungsdiagnostik dar. Durch die rasterelektronenmikroskopischen Untersuchungen wurde besonders an den Tubuluszellen (TZ) zusatzliche Information gegenuber der lichtmikroskopischen Untersuchung gewonnen: Wahrend normale TZ eine leicht gewellte Oberflache aufweisen, findet man bei rein ischamischer Schadigung eine rauhe Zelloberflache mit grobem Muster. Bei gleichzeitig bestehender Abstosungsreaktion zeigen ischamisch geschadigte TZ ebenfalls eine rauhe Oberflache, jedoch mit feiner Struktur. Somit scheint durch die rasterelektronenoptische Untersuchung von TNB-Material eine Abstosungsdiagnose an den geschadigten TZ moglich.
