1. Antibiotic Resistance of Kidney Stones and Urine Isolated Bacteria from the Patients Under Going Nephrolithotomy

Abstract

Abstract Background and objectives: Infection stones are the directly due to persistent or recurrent infection with Urease producing bacteria. they may be exacerbated by urinary obstruction or stasis. Also, some drug components like ciprofloxacin can be the cause of stone formation. We aimed at studing antibiotic resistance of bacteria isolated form kidney stones and Urinary tract infection(UtI). Material and Methods: This descriptive study, during 2008-2010,we obtained kidney stones and urine of 45 patients under gone nephrolithotomy in shahid Beheshti hospital of Babol, Iran. The stones were transferred to microbiology Lab in a strile Condition, after grinding, cultured on Blood Agar and Eosin-Methylene Blue(EMB) media. In Addilion, urin Samples were cultured on the for mentioned media. The Positive cultures were qualitatively evaluated and then, antibiogram was done by using Kirby Bauer method. Results: The resulte show that 10 kidney stone specimens and 8 urine samples were infected by bacteria, mostly by E.coli. In one stone sample and two urine samples, we can isolate more than one bacteria. The bacteria isolated in the stone and urine samples of six patients was the same. We could find two cases of staphylococcus inurine and one in stone culture. The bacteria isolated from stone are resistance to oflaxacin(80%) , which is higher than that to the other antibiotics. Conclusion: Based on the results, there is no relationship between the kind of kidney stone and type of bacteria. Further study needs to be done to prove this relation. 2. The etiologies of hydronephrosis in neonates

Abstract

BACKGROUND AND OBJECTIVE: Hydronephrosis is defined as abnormal dilation of pyelocaliceal system. There is a controversy about the etiologies and follow up in antenatal and neonatal hydronephrosis. This study was done to determine the etiologies of hydronephrosis in neonates who were born with this disorder. METHODS: This study was carried out on all neonates who were referred to nephrology department and clinic of Amirkola children hospital, Babol Medical University from 1998 to 2004. In whom it was diagnosed hydronephrosis before or after birth VCUG (voiding cystourethrography), IVP (Intravenous pyelography ) and/or DTPA (diethylene triamine-pentaacetic acid) were used as needed. Then all data were evaluated according to their etiologies.FINDINGS: Of 42 neonates, 38(90%) were male and 4(10%) were female. Twenty-four (57%) and 17 (40%) patients had UPJO (ureteropelvic junction obstruction (and VUR (vesicouretral reflux), respectively. Twenty-six (62%) had been diagnosed before birth. The most common causes of admission were prenatal diagnosis (57%) and UTI (24%). Among 54 hydronephrotic kidneys unites in 42 neonates, 30 (55.5%), 23 (42.5%) and 1 had VUR, UPJO and UVJO respectively.CONCLUSION: The most common cause of hydronephrosis in neonates was UPJO. It is needed to give more attention to male neonates who showed higher prevalence of hydronephrosis and also early prenatal sonographic diagnosis.

Abstract

A simple, precise and sensitive reversed-phase HPLC method using Photo-diode array detector for simultaneous determination of dextromethorphan (DXM) and chlorpheniramine (CLP) in various matrices had been developed and validated. The separation was achieved within 8 minutes on a C-18 column (15 cm x 4.6 mm x 5 μm) using methanol/ pH 3.0 potassium dihydrogen phosphate buffer (60:40, v/v) mobile phase in an isocratic elution mode with flow rate 0.8 mL/min and UV-detection at 230 nm. External standard was employed for quantification. The current method demonstrated good linearity ranged 1-200 µg/mL for CLP (retention time 3.83 ± 0.10 min) with r2 of 0.9994 and 2-1000 µg/mL for DXM (retention time 5.02 ± 0.10 min) with R2 of 0.9993. The limits of detection and quantification were 0.25 µg/mL and 1.0 µg/mL for CLP and 0.50 µg/mL and 1.5 µg/mL for DXM. No interference from matrices was observed and samples remained stable in the HPLC injector for 12 h. The developed method was accurate that employed a simple liquid/liquid extraction procedure with recovery ranged 90.0%-101.9% for both analytes. Intra and inter-day precision were less than 2.5%. The method was proved robust and reproducible that is applicable to pharmaceutical (active raw materials, syrups) and biological (blood) matrices.

5. Testicular Metastasis from Prostate Carcinoma: A Case Report

Abstract

BACKGROUND AND OBJECTIVE: The testis is a rare site for prostate cancer metastases and is usually found in autopsies or incidentally after therapeutic orchiectomies. The prognosis for such patients is very poor. Bilateral orchiectomies were done for patients. The aim of this report was to introduce one case of testicular metastases from prostate carcinoma.CASE: The patient was a 75 year old male that has prostatism signs. Primary examination showed the increase of prostate-specific antigen (PSA). Biopsy and nuclear scan were requested for patient. Pathology reported multifocal infiltrating prostate adenocarcinoma, also estimated Gleason score of 4+5=9 and vertebral column involvement. The patient was candidate for bilateral orchiectomies and ten sessions of radiation for vertebral column was requested. After pathology feature and immunohistochemical (IHC) study for testis, deposits of carcinoma were posed. Now, symptoms related to bone pain are decreased, and PSA=68 ng/ml change to PSA=6 ng/ml. CONCLUSION: Considering the reported case, evaluation in patients with prostate carcinoma by clinical examination, pathology assessment of testis tissue and biomarkers seems to be necessary.

6. Diagnostic value of ultrasonography and laparoscopy in undescended testes

Abstract

BACKGROUND AND OBJECTIVE: Cryptorchidism or absence of one or both of testes in the scrotum (undescended testes) is the most common urologic disorder in children. Two major complications in this condition are infertility and malignancy. Most diagnostic imaging procedure is ultrasound, but in cases that testes are not seen in ultrasound, laparoscopy is very useful. The aim of this study was to assess diagnostic value of ultrasonography and laparoscopy in undescended testes.METHODS: This diagnostic study was performed on all patients with undescended testes in teaching hospitals of Babol University of medical sciences, Babol, Iran. All patients were referred to sonography, if testes were seen in sonography, these patients were candidate for open surgery but if testes were not seen in two different sonographies, the patients were candidate for laparoscopy. All information is gathered in questionnaire and then the results of sonography, open surgery and laparoscopy were compared together.FINDINGS: One hundred and eight patients with 131 undescended testes, aged between 1 and 42 years with mean age of 13.9±8.99 years were studied. 78.7% of patients had unilateral undescended testes and 21.3% had bilateral undescended testes. The unilateral undescended testes were left sided in 47% and right sided in 53%. Sensitivity and specificity of sonography for intraabdominal testes were 16% and 97%, respectively. Whereas, sensitivity and specificity of sonography for inguinal canal testes were 85% and 76%, respectively. Also, sensitivity and specificity of laparoscopy for intraabdominal and inguinal canal testes were 100%.CONCLUSION: The results showed that diagnostic laparoscopy has a high sensitivity and specificity. When intraabdominal testes are not diagnosed by sonography, laparoscopy can be helpful.
