O objetivo deste estudo é relatar um caso clínico de remoção de um pino pré-fabricado rosqueável metálico com auxílio de magnificação por microscópio e uso de ultrassom. Paciente de 47 anos, gênero feminino, compareceu à clínica de odontologia queixando-se da insatisfação com seu sorriso. Durante a anamnese observou-se que as restaurações nos dentes 44, 45, 46 e 47 apresentavam-se insatisfatórias. Nos exames radiográficos foi possível observar lesão periapical nos dentes 44 e 47, presença de tratamentos endodônticos insatisfatórios no 44, 45, 46 e 47, e a presença de retentores intrarradiculares metálicos em todos os dentes citados. A partir dos exames, planejou-se a remoção do pino pré-fabricado do dente 45. O processo de remoção iniciou-se com isolamento absoluto e remoção do material restaurador, em seguida, com auxílio de um microscópio óptico para magnificação do campo, foi realizada a técnica de sistema integrado e simultâneo de ultrassom, em que utiliza-se duas pontas de ultrassom em vibrações opostas sobre o retentor. Após sequências de aplicação da técnica houve fratura da película de cimento e o pino foi removido. Assim, a associação da magnificação proporcionada pelo microscópio e a vibração do ultrassom foram resolutivos, pois ofereceram melhor acuidade visual e segurança no procedimento.

Statement of problem Dual-polymerizing self-adhesive resin cements present mechanical properties that may adversely affect the clinical performance of luted fiber-reinforced posts. Purpose The purpose of this in vitro study was to evaluate the properties of dual-polymerizing self-adhesive resin cements after the addition of an onium salt. Material and methods The experimental groups were set according to the molar concentration of diphenyliodonium hexafluorphosphate (DPIHFP) (0.5, 1, and 2 mol%). The resin cements were submitted to a push-out bond strength test and assessed for flexural strength (ISO 4049/2009), degree of conversion (Fourier-transformed infrared spectroscopy), depth of polymerization, swelling coefficient, and degradation in solvent. The data were statistically analyzed by ANOVA, Tukey test, Kruskal-Wallis test, Dunn multiple comparison, and paired Student t test (α=.05). Results All the RelyX U100 groups with onium salt showed the highest degree of conversion after 24 hours and 7 days ( P P =.054). The addition of 0.5 mol% DPIHFP increased the push-out bond strength and microhardness of RelyX U100 and promoted less degradation after immersion in solvent. The BisCem control group did not present a statistical difference from the experimental groups in terms of bond strength; the control group and the 0.5 mol% group showed no degradation in solvent. For swelling coefficient and flexural strength, no difference was found between the BisCem groups ( P =.067 and P =.173), and the RelyX U100 2 mol% group presented the lower value ( P P =.048). Depth of polymerization was not statistically different in the experimental groups for either resin cement ( P =.999). Conclusions The addition of 0.5 mol% DPIHFP improved the physical properties of dual-polymerizing self-adhesive resin cements.

4. Intérêt des interventions psychologiques en dialyse : étude exploratoire

Patients suffering from end-stage renal disease (ESRD) have a very reduced quality of life accompanied by a severe emotional distress (high worries-anxiety-depression). However, in Belgium, no regular psychological intervention is proposed to dialyzed patients. Our objective is to show that psychological intervention can significantly decrease the emotional distress of patients with ESRD. Eleven sessions of structured interventions are proposed to ESRD patients. Eligibility criteria are to be major, to not present confusion or/and dementia, to have been on dialysis treatment for at least 3months, to have obtained 14 or more on HAD-scale. Interventions carry on the management of anxious and depressive symptoms and of the disease itself. This constitutes three independent modules. Questionnaires are filled in by the patients at various stages to evaluate the anxiety and the depression (HADS), the worries (Penn State) and the quality of life (KDQoL-SF). Results for the 47 ESRD patients show a significant reduction of the scores of anxiety, depression and worries and a significant growth of quality of life. In parallel, a decrease in the serum calcium-phosphorus product analyzed before dialysis has been noted.

Aim: To evaluate the long-term post push-out bond strength to dentin, water sorption, solubility and swelling of conventional and self-adhesive dual-cure resin cements. Methods: Forty-eight bovine roots were prepared for fiber post cementation with RelyX ARC and RelyX U100. According to resin cement and storage time (24 h and 6 months), 4 groups were assessed using the push-out test. Water sorption and solubility were performed according to ISO 4049:2009. The swelling coefficient was obtained using cement disks of each material immersed in distilled water until the swelling equilibrium was reached. The mass of dry and swelled polymer and solvent density were used to calculate the coefficient. Statistical data analysis was performed using Student's t-test for water sorption, solubility and swelling coefficient and the Kruskal-Wallis and Dunn multiple comparison tests for push-out analysis with a significance level of 0.05. Results: The immediate bond strength was not significantly different between RelyX ARC (3.09 MPa) and RelyX U100 (3.78 MPa) (p>0.05). RelyX U100 showed higher (p0.05) between groups.
