Abstract The crude leaf extract of Phragmanthera macrosolen L. has been utilized for the first time as an effective reducing, capping and stabilizing agent to synthesize silver oxide nanoparticles (Ag2O NPs) through a green approach. The prepared Ag2O NPs were analyzed by scanning electron-microscopy (SEM), High Resolution Transmission electron microscope (HR-TEM), X-ray diffractions (XRD), Fourier transforms infrared (FTIR) spectroscopy, energy dispersive spectroscopy (EDS), and Ultra-violet visible spectrometry (UV-Vis). The biosynthesized Ag2O NPs applied on gram-positive (S. aureus) and gram-negative (E. coli) bacterial types. FTIR spectral peaks indicate that the phytochemicals in the extract are responsible for the formation of Ag2O nanoparticles. The XRD result shown, poly-crystalline nanoparticles and the average crystalline size calculated was 45.8 nm. UV-Vis analysis shows absorbance existed at 590.5 nm, and the energy band gap calculated through the Tauc relation was 2.1 eV. The SEM images gave a globular and some rod like morphology with diameter of particle obtained between 20.11 and 46.50 nm with some hollow cubic microstructure of Ag2O NPs which makes it suitable for antimicrobial application. The EDS confirms the elemental composition and existence of Ag and O2. The HR-TEM images, specific area electron diffraction (SAED), and XRD patterns confirmed the morphology of Ag2O NPs mean 45.84 nm and polycrystalline nature of the nanoparticles. The minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) were 15 and 30 µl respectively for the samples tested. In this study, it was observed that Ag2O NPs highly sensitive to E. coli than s. aureus. The present study revealed that the possible use of green synthesized Ag2O NPs as potential antibacterial agent.

Abstract In present study the green synthesis of silver oxide nanoparticles has been effectively achieved using novel plant extract Phragmanthera Macrosolen. This method provides sustainable alternative for nanoparticle synthesis, demonstrating the potential of Phragmanthera Macrosolen as a reducing and stabilizing agent in the production of Ag2O NPs. The synthesized nanoparticles were characterized for their structural, morphological, and optical properties, confirming their successful formation and potential applications in various fields. The effects of different pH values and annealing temperature of the samples on the properties of Ag2O NPs formations, as well as photo-catalytic activities towards Toluidine Blue dye degradations, were studied. Powder XRD reveals that the crystallite natures of Ag2O NPs a long with crystalline size ranges from 25.85 to 35.90 nm. FIB-SEM and HR-TEM images displayed that the Ag2O NPs as spherical shapes. UV-vis spectroscopy displayed that Ag2O NPs belong to a direct-band gap of 2.1–2.6 eV. FTIR- study shown that the green synthesized Ag2O NPs may be steadied via the interfaces of –OH as well as C = O groups in the carbohydrate, flavonoid, tannin, as well as phenolic acid existing in P. macrosolen L. leaf. The chemical states, electron-hole recombinations and purity of Ag and O in the synthesized Ag2O NPs were confirmed through X-ray Photoelectron Spectroscopy (XPS) and PL analysis respectively. Fascinatingly, the synthesized Ag2O NPs at pH 12 displayed high photo-catalytic degradations for TB dyes. The photo-catalytic degradations of the TB dyes were monitored spectro-photo-metrically in wave-length ranges of 200–900 nm, as well as high efficiency (98.50%) with half-life of 9.5798 min and kinetic rate constant of 0.07234 min−1, was obtained after 45 min of reactions. From this study, it can be concluded that Ag2O NPs synthesized from Phragmanthera Macrosolen aqueous extract are promising in the remediation of environmental pollution and water treatment. In this light, the study reports that Phragmanthera Macrosolen green synthesis of Ag2O NPs can effectively address environmental pollution in cost-effective, eco-friendly, and sustainable ways.

Abstract The current study describes current conduction mechanisms in BiFeO3 thin films prepared by using a modified chemical solution-based technique. In particular, in these films, X-ray photoelectron spectroscopy revealed that the defect causing Fe2+ ions reduced by about 18% compared to the solution-based synthesis methods reported before, indicating better film quality. The leakage current density was measured to be 1.7 × 10–6 A/cm2 at an applied voltage of 1.5 V, which is one order of magnitude less than the previously reported work in a similar system. Oxygen annealing was found to be effective in further reducing the current conduction, making these films a suitable choice for device applications. The current–voltage curves exhibited three different behaviours of current conduction mechanisms in these films. In particular, when the applied electric field was increased, a transition from Ohmic conduction to trap-filled space charge limited conduction was observed. The presented investigations demonstrate the importance of deposition methods in determining the suitability of thin films for device applications.

Abstract Herbal medicines are important for ensuring sustainable development goals (SDGs) in healthcare, particularly in developing countries with high rates of antimicrobial resistance (AMR) and little access to medical facilities. Thymus vulgaris is a widely used herbal medicinal plant known for its secondary metabolites and antimicrobial properties. The present study involved a comprehensive examination of the isolation, characterization, and antibacterial activity of Thymus vulgaris obtained from Ethiopia. The aerial part of the plant Thymus vulgaris was successively extracted with hexane, chloroform, and methanol based on differences in polarity. Phytochemical screening tests conducted against hexane, chloroform and MeOH crude extracts indicated the presence of some secondary metabolites. Based on the thin-layer chromatography tests, the chloroform extract was subjected to column chromatography, yielding Tv-2 compounds, namely 5-isopropyl-2-methylphenol. The structures of the compounds were elucidated via spectroscopic methods (UV–Vis, FT-IR and NMR). We investigated the antibacterial properties of hexane crude extract, chloroform crude extract, MeOH crude extract, and isolated fractions derived from T. vulgaris against various bacterial strains. This study contributes to a better understanding of the bioactive components present in Thymus vulgaris crude extracts and their potential role in tackling microbial infections.

Abstract In recent times, the number of malware on Android mobile phones has been growing, and a new kind of malware is Android ransomware. This research aims to address the emerging concerns about Android ransomware in the mobile sector. Previous studies highlight that the number of new Android ransomware is increasing annually, which poses a huge threat to the privacy of mobile phone users for sensitive data. Various existing techniques are active to detect ransomware and secure the data in the mobile cloud. However, these approaches lack accuracy and detection performance with insecure storage. To resolve this and enhance the security level, the proposed model is presented. This manuscript provides both recognition algorithms based on the deep learning model and secured storage of detected data in the cloud with a secret key to safeguard sensitive user information using the hybrid cryptographic model. Initially, the input APK files and data are preprocessed to extract features. The collection of optimal features is carried out using the Squirrel search optimization process. After that, the Deep Learning-based model, adaptive deep saliency The AlexNet classifier is presented to detect and classify data as malicious or normal. The detected data, which is not malicious, is stored on a cloud server. For secured storage of data in the cloud, a hybrid cryptographic model such as hybrid homomorphic Elliptic Curve Cryptography and Blowfish is employed, which includes key computation and key generation processes. The cryptographic scheme includes encryption and decryption of data, after which the application response is found to attain a decrypted result upon user request. The performance is carried out for both the Deep Learning-based model and the hybrid cryptography-based security model, and the results obtained are 99.89% accuracy in detecting malware compared with traditional models. The effectiveness of the proposed system over other models such as GNN is 94.76%, CNN is 95.76%, and Random Forest is 96%.

Abstract Deep learning-assisted digital pathology has demonstrated the potential to profoundly impact clinical practice, even surpassing human pathologists in performance. However, as deep neural network (DNN) architectures grow in size and complexity, their explainability decreases, posing challenges in interpreting pathology features for broader clinical insights into physiological diseases. To better assess the interpretability of digital microscopic images and guide future microscopic system design, we developed a novel method to study the predictive feature length-scale that underpins a DNN’s predictive power. We applied this method to analyze a DNN’s capability in predicting brain metastasis from early-stage non-small-cell lung cancer biopsy slides. This study quantifies DNN’s attention for brain metastasis prediction, targeting features at both the cellular scale and tissue scale in H&E-stained histological whole slide images. At the cellular scale, the predictive power of DNNs progressively increases with higher resolution and significantly decreases when the resolvable feature length exceeds 5 microns. Additionally, DNN uses more macro-scale features associated with tissue architecture and is optimized when assessing visual fields greater than 41 microns. Our study computes the length-scale requirements for optimal DNN learning on digital whole-slide microscopic images, holding the promise to guide future optical microscope designs in pathology applications and facilitating downstream deep learning analysis.

Abstract In light of the unprecedented growth in internet usage, safeguarding data from unauthorized access has emerged as a paramount concern. Cryptography and steganography stand as pivotal methods for ensuring data security during transmission. This study introduces an innovative adaptive video steganography approach featuring three tiers of security for extracting concealed information, thereby facilitating secure communication. The embedding process operates within the spatial domain of cover video frames, enabling a remarkable hiding ratio of up to 28.125% (equivalent to 2.25 bits per pixel in payload) without compromising the quality of video frames. Users are afforded the flexibility to select between partial or full embedding capacity of CVF through the proposed adaptive control block (ACB). The chaotic key generator (CKG), which combines a logistic map and sine map, is employed to generate highly sensitive initial seeds for permutation order (PO), frame selection (FS), and random position for hiding (RPH), thereby ensuring three levels of security. Prior to transmission, both CVF and hidden data (SD) are encrypted using PO. Encrypted CVFs are then randomly selected using FS for embedding, with RPH employed during the embedding process. Subsequently, for transmitting the stego-video frame, embedded CVFs are decrypted using the same PO. Experimental results demonstrate the efficacy of the proposed approach, achieving an adaptive hiding ratio ranging from 7.1 to 28.125% (equivalent to 0.56 to 2.25 bits per pixel in payload) and maintaining a peak signal-to-noise ratio (PSNR) within the range of 50.25 to 62.05 dB.

Background: Sleep quality in cancer patients often does not receive sufficient attention from their treating physicians and nurses. This report aims to highlight this under-researched issue among colorectal cancer patients scheduled to receive adjuvant chemotherapy. Materials and Methods: This is a secondary analysis of data from 129 participants from a randomized controlled trial conducted to determine the nurse-led intervention’s effect on cognitive and non-cognitive functions and serum TNF-alpha levels in patients receiving chemotherapy for colorectal cancer at Tata Memorial Hospital. The Pittsburgh Sleep Quality Index was used to collect data on sleep quality. The study also explored potential reasons for the reported poor sleep quality in the selected cohort. Results: The prevalence of poor sleep quality was approximately 60%. Sleep was primarily affected due to poor sleep latency and sleep efficiency. Also, women were more likely to experience issues related to sleep. Patients did not consider it a significant issue to report to their treating physicians and nurses. Factors contributing to poor sleep quality included financial concerns, child-rearing responsibilities, and anxiety about disease outcomes. Conclusion: Cancer patients do experience poor sleep quality, and it is crucial to assess and address this issue promptly to prevent adverse responses to cancer therapy, which can lead to increased morbidity and mortality.

Systemic lupus erythematosus (SLE) is an autoimmune disorder with protean manifestations. most commonly affecting women Cardiovascular manifestations are common but rare at initial presentation. Our patient presented with impending cardiac tamponade and was subsequently diagnosed with SLE. A 31-year-old female presented with fever, rash, and progressive exertional dyspnea. On examination, she had an elevated JVP and muffled heart sounds and features of pulmonary hypertension her baseline blood investigations showed acute kidney injury, albuminuria, and grossly elevated BNP. Chest radiography showed an enlarged cardiac silhouette and echocardiography showed large pericardial effusion with impending cardiac tamponade for which the patient underwent emergency pericardiocentesis. ANA profile confirmed our diagnosis of SLE with positive anti-dsDNA, anti-nucleosomes, anti-histones, anti-SSA, and anti-Jo 1. Being positive she was treated with pulse steroids and hydroxychloroquine. Renal biopsy was also done which showed class IV lupus nephritis. She improved clinically and was discharged with oral steroids, mycophenolate mofetil, and hydroxychloroquine. Cardiac tamponade is a life-threatening condition and SLE is an important differential to be considered during evaluation.

BackgroundUpwards of ninety percent of individuals living with depression in India do not have access to evidence-based treatments, especially in rural areas. Integrating these treatments into primary care is essential for bridging this care gap. This trial aims to evaluate whether a remote coaching implementation support strategy, referred to as Enhanced Implementation Support, is superior to routine support, referred to as Routine Implementation Support, in supporting the delivery of collaborative depression care in rural primary care centers.MethodsEmploying a cluster-randomized hybrid type-III implementation trial design, 14 primary care facilities in Sehore district, Madhya Pradesh, will implement a collaborative depression care package based on the WHO's mhGAP program. Facilities will be randomized to either Enhanced Implementation Support or the Routine Implementation Support control condition. Enhanced Implementation Support consists of remote coaching and technical assistance, supplemented with in-person visits, and guided by the Plan-Do-Study-Act implementation cycles. The primary implementation outcome is the proportion of outpatients screened for depression by facility staff, with secondary outcomes including the proportions of outpatients who screen positive for depression, are referred to the medical officer, and initiate treatment. Secondary patient outcomes include proportion of patients who achieve reduction in depression symptom severity at 3-month follow up. Acceptability, feasibility, and fidelity of the depression care package will be assessed through routine observations collected during field visits, facility audits, and qualitative exit interviews with facility staff. Costs of delivering the Enhanced Implementation Support strategy will also be estimated.DiscussionThis trial can inform efforts to integrate depression care in rural primary care facilities in a low-resource setting, and illuminate whether external coaching support is superior relative to existing implementation support for achieving these goals. Transcatheter sinus venosus defect (SVD) closure with covered stents is emerging as an alternative to surgery. An adequate anchor zone in the superior vena cava is mandatory for the stability of the covered stent to prevent caudal embolization. There is a potential risk of innominate vein occlusion by the fabric of the covered stent in patients with a very short superior caval vein. Three among a total of 105 patients who underwent SVD closure at our institution developed innominate vein occlusion. Predisposing anatomical factors, identification and management of occluded innominate vein, and follow-up outcomes are discussed.

Abstract The online channel has affected many facets of an individual's identity, commercial, social policy, and culture, among others. It implies that discovering the topics on which these brief writings are focused, as well as examining the qualities of these short texts is critical. Another key issue that has been identified is the evaluation of newly discovered topics in terms of topic quality, which includes topic separation and coherence. A topic modeling method has been shown to be an outstanding aid in the linguistic interpretation of quite tiny texts. Based on the underlying strategy, topic models are divided into two categories: probabilistic methods and non-probabilistic methods. In this research, short texts are analyzed using topic models, including latent Dirichlet allocation (LDA) for probabilistic topic modeling and non-negative matrix factorization (NMF) for non-probabilistic topic modeling. A novel approach for topic evaluation is used, such as clustering methods and silhouette analysis on both models, to investigate performance in terms of quality. The experiment results indicate that the proposed evaluation method outperforms on both LDA and NMF.

Abstract Nanotechnology is rapidly becoming more and more important in today's technological world as the need for industry increases with human well-being. In this study, we synthesized SnO2 nanoparticles (NPs) using an environmentally friendly method or green method from Croton macrostachyus leaf extract, leading to the transformation of UV absorbance to visible absorbance by reducing the band gap energy. The products underwent UV, FTIR, XRD, SEM, EDX, XPS, BET, and DLS for characterization. Characterization via UV–Vis spectroscopy confirmed the shift in absorbance towards the visible spectrum, indicating the potential for enhanced photocatalytic activity under visible light irradiation. The energy band gap for as-synthesized nanoparticles was 3.03 eV, 2.71 eV, 2.61 eV, and 2.41 eV for the 1:1, 1:2, 1:3, and 1:4 sample ratios, respectively. The average crystal size of 32.18 nm and very fine flakes with tiny agglomerate structures of nanoparticles was obtained. The photocatalytic activity of the green-synthesized SnO2 nanoparticles was explored under visible light irradiation for the degradation of rhodamine B (RhB) and methylene blue (MB), which were widespread fabric pollutants. It was finally confirmed that the prepared NPs were actively used for photocatalytic degradation. Our results suggest the promising application of these green-synthesized SnO2 NPs as efficient photocatalysts for environmental remediation with low energy consumption compared to other light-driven processes. The radical scavenging experiment proved that hydroxyl radicals (_OH) are the predominant species in the reaction kinetics of both pollutant dyes under visible light degradation.

Objectives: While several food assistance programs in the United States tackle food insecurity, a relatively new program, “Food is Medicine,” (FIM) initiated in some cities not only addresses food insecurity but also targets chronic diseases by customizing the food delivered to its recipients. This review describes federal programs providing food assistance and evaluates the various sub-programs categorized under the FIM initiative. Methods: A literature search was conducted from July 7, 2023 to November 9, 2023 using the search term, “Food is Medicine”, to identify articles indexed within three major electronic databases, PubMed, Medline, and Cumulative Index to Nursing and Allied Health Literature (CINAHL). Eligibility criteria for inclusion were: focus on any aspect of the FIM initiative within the United States, and publication as a peer-reviewed journal article in the English language. A total of 180 articles were retrieved; publications outside the eligibility criteria and duplicates were excluded for a final list of 72 publications. Supporting publications related to food insecurity, governmental and organizational websites related to FIM and other programs discussed in this review were also included. Results: The FIM program includes medically tailored meals, medically tailored groceries, and produce prescriptions. Data suggest that it has lowered food insecurity, promoted better management of health, improved health outcomes, and has, therefore, lowered healthcare costs. Conclusions: Overall, this umbrella program is having a positive impact on communities that have been offered and participate in this program. Limitations and challenges that need to be overcome to ensure its success are discussed.

BackgroundFalls in older patients can lead to serious health complications and increased health care costs. Fall risk–increasing drugs (FRIDs) are a group of drugs that may induce falls or increase the tendency to fall (ie, fall risk). Deprescribing is the process of withdrawal from an inappropriate medication, supervised by a health care professional, with the goal of managing polypharmacy and improving outcomes. ObjectiveThis study aims to assess the effectiveness of a deprescribing intervention based on the Assess, Review, Minimize, Optimize, and Reassess (ARMOR) tool in reducing the risk of falls in older patients and evaluate the cost-effectiveness of deprescribing FRIDs. MethodsThis is an open-label, parallel-group randomized controlled academic trial. Individuals aged 60-80 years who are currently taking 5 or more prescribed drugs, including at least 1 FRID, will be recruited. Demographic data, medical conditions, medication lists, orthostatic hypotension, and fall history details will be collected. Fall concern will be assessed using the Fall Efficacy Scale, and fall risk will be assessed by the Timed Up and Go test and Tinetti Performance-Oriented Mobility Assessment tool. In this study, all treating physicians will be randomized using a stratified randomization method based on seniority. Randomized physicians will do deprescribing with the ARMOR tool for patients on FRIDs. Participants will maintain diaries, and monthly phone follow-ups will be undertaken to monitor falls and adverse events. Physical assessments will be performed to evaluate fall risk every 3 months for a year. The rationality of prescription drugs will be evaluated using the World Health Organization’s core indicators. ResultsThe study received a grant from the Indian Council of Medical Research–Safe and Rational Use of Medicine in October 2023. The study is scheduled to commence in April 2024 and conclude by 2026. Efficacy will be measured by fall frequency and changes in fall risk scores. Cost-effectiveness analysis will also include the incremental cost-effectiveness ratio calculation. Adverse events related to deprescription will be recorded. ConclusionsThis trial will provide essential insights into the efficacy of the ARMOR tool in reducing falls among the geriatric population who are taking FRIDs. Additionally, it will provide valuable information on the cost-effectiveness of deprescribing practices, offering significant implications for improving the well-being of older patients and optimizing health care resource allocation. The findings from this study will be pertinent for health care professionals, policy makers, and researchers focused on geriatric care and fall prevention strategies. Viral hepatitis is a common cause of acute liver injury that can lead to chronic liver disease, cirrhosis, and liver cancer. Viral hepatitis shares similarities with kamalai in Siddha medicine and is managed in accordance with Siddha treatment. Present case is of a 14-year-old male patient who presented with symptoms of fever, fatigue, malaise, and jaundice. Laboratory investigations, abdominal ultrasound features, and clinical findings indicated positive viral hepatitis. The patient was started on Siddha management to suppress Hepatitis B Virus (HBV) replication and prevent progression to chronic hepatitis B infection. The patient’s symptoms gradually improved over the next few weeks, and follow-up blood tests showed a significant reduction in the values of elevated liver enzymes, indicating a good response to Siddha medicine treatment. The unique aspect of this case study was that it demonstrates a successful management of viral hepatitis B infection with the Siddha system of medicine.

Abstract Smart cities and the Internet of Things have enabled the integration of communicating devices for efficient decision-making. Notably, traffic congestion is one major problem faced by daily commuters in urban cities. In developed countries, specialized sensors are deployed to gather traffic information to predict traffic patterns. Any traffic updates are shared with the commuters via the Internet. Such solutions become impracticable when physical infrastructure and Internet connectivity are either non-existent or very limited. In case of developing countries, no roadside units are available and Internet connectivity is still an issue in remote areas. Internet traffic analysis is a thriving field of study due to the myriad ways in which it may be put to practical use. In the intelligent Internet-of-Vehicles (IOVs), traffic congestion can be predicted and identified using cutting-edge technologies. Using tree-based decision-tree, random-forest, extra-tree, and XGBoost machine learning (ML) strategies, this research proposes an intelligent-transport-system for the IOVs-based vehicular network traffic in a smart city set-up. The suggested system uses ensemble learning and averages the selection of crucial features to give high detection accuracy at minimal computational costs, as demonstrated by the simulation results. For IOV-based vehicular network traffic, the tree-based ML approaches with feature-selection (FS) outperformed those without FS. When contrasted to the lowest KNN accuracy of 96.6% and the highest SVM accuracy of 98.01%, the Stacking approach demonstrates superior accuracy as 99.05%.

Abstract Being a developing country, Pakistan needs sustainable and cost-effective strengthening/ retrofitting solution to be adopted/ practiced in the construction industry. The research reported in this paper was aimed to study the effectiveness of PMM, an indigenous product, for repairing reinforced concrete beams, resulting more effective and cost benefiting repair and strengthening for restoration of pre-cracked RC structures. This article presents the research results of experimental investigation conducted for repairing of cracks in shear deficient reinforced concrete (RC) beams with locally available novel material polymer modified mortar (PMM). A total of 6 beams; divided in three groups i.e. short beams, medium beams and deep beams with varying depths and same mix design were tested to four point loading under monotonic loading conditions until failure loads. Afterwards, these beams were repaired with PMM and cured with water for 72 h for retesting until failure. Load at first crack and at failure, crack pattern and deflections were recorded for all specimens during testing. Results from the experimental investigation indicate that load carrying capacity of the repaired beams was significantly restored in comparison to the control specimens. However, repaired specimens of medium group showed more improvement in load carrying capacity as compared with those of repaired specimens of short and deep group. The specimens of medium group restored up to 90% of their original load carrying capacity. The ductility is improved significantly for all shear critical repaired RC beams up to opening of cracks. Sudden brittle failure was observed after opening of repaired cracks. The contribution of PMM to load carrying capacity was found more significant for medium beams as compared to short and deep beams. The results of this study indicated that application of polymer modified mortar is effective technique for repairing of cracks in shear deficient RC beams.

Abstract In this research, the performance pertaining to tire crumb obtained from scrap tire processing plants is discussed. These tire crumbs are blended with soil at a 30% ratio. When subjected to seismic load, the performance of the 30% tire crumb combination is superior to the 0% tire crumb combination. The investigation is classified into two phases. Phase 1 of the study involves conducting an experimental investigation by applying cyclic loads to a model footing that was resting on the soil with and without tire crumbs. This study reveals that a 30% tire crumb combination achieves optimum energy absorption and minimal footing stiffness, which is a crucial component needed for base isolation. Additionally, using the PLAXIS 2D software package, finite element analysis was carried out during the second phase of the study. For this study, a three-story residential building close to the border between India and Nepal is used. Three different disastrous seismic excitations are applied to the building. From this analytical analysis, it is reported that a 60–70% reduction in acceleration is attained for 30% tire crumb combination with soil. Therefore, from the two phases, it is evaluated that the inclusion of tire crumbs with soil is an excellent seismic base isolation material.

Abstract Multisystem Inflammatory Syndrome in Children (MIS-C) is a rare manifestation of Severe Acute Respiratory Syndrome-CoronaVirus-2 (SARS-CoV-2) infection that can result in increased morbidity and mortality. Mounting evidence describes sex disparities in the clinical outcomes of coronavirus disease 2019 (COVID-19). However, there is a lack of information on sex-specific differences in immune responses in MIS-C. This study is an observational and cross-sectional study and we wanted to examine immune parameters such as cytokines, chemokines, acute phase proteins (APPs), growth factors, microbial translocation markers (MTMs), complement components and matrix metalloproteinases (MMPs) in MIS-C children, based on sex. Male children were associated with heightened levels of pro-inflammatory cytokines—IFNγ, IL-2, TNFα, IL-1α, IL-1β, IL-6, IL-12, G-CSF and GM-CSF, chemokines-CCL2, CCL11, CXCL1, CXCL8 and CXCL10, acute phase proteins-α-2M, CRP, growth factors VEGF and TGFα, microbial translocation markers- iFABP, LBP, EndoCAb, complement components—C1q, MBL and C3 and matrix metalloproteinases MMP-8 and MMP-9 compared to female children with MIS-C. These results indicate that the heightened immune response in males is a characteristic feature of MIS-C. These findings might explain the differential disease pathogenesis in males compared to females with MIS-C and facilitate a deeper understanding of this disease.

Abstract Studies of genetic factors associated with severe COVID-19 in young adults have been limited in non-Caucasian populations. Here, we clinically characterize a case series of patients with COVID-19, who were otherwise healthy, young adults (N = 55; mean age 34.1 ± SD 5.0 years) from 16 Asian, Middle Eastern, and North African countries. Using whole exome sequencing, we identify rare, likely deleterious variants affecting 16 immune-related genes in 17 out of 55 patients (31%), including 7 patients (41% of all carriers or 12.7% of all patients) who harbored multiple such variants mainly in interferon and toll-like receptor genes. Protein network analysis as well as transcriptomic analysis of nasopharyngeal swabs from an independent COVID-19 cohort (N = 50; 42% Asians and 22% Arabs) revealed that most of the altered genes, as identified by whole exome sequencing, and the associated molecular pathways were significantly altered in COVID-19 patients. Genetic variants tended to be associated with mortality, intensive care admission, and ventilation support. Our clinical cases series, genomic and transcriptomic findings suggest a possible role for interferon pathway genes in severe COVID-19 and highlight the importance of extending genetic studies to diverse populations to better understand the human genetics of disease.

Abstract In the Anamallais region of south India, crop shoots from the UPASI-3, UPASI-9, UPASI-17, Assam seedlings, and TRI-2043 cultivars were examined for seasonal variations in total phenolics, antioxidant activity, and minerals during four harvest seasons: summer (January to March), premonsoon (April and May), monsoon (June to September), and winter (October to December) of two consecutive years. The total phenolics of all cultivars were lower in monsoon period and grew over rest of the seasons and it was greater during summer. Crop shoot antioxidant activity as measured by the DPPH radical scavenging experiment exhibited a similar pattern to total phenolics. Summer was the season with the highest antioxidant activity across all cultivars, followed by premonsoon, winter, and monsoon. On the other hand, the employed cultivars differed noticeably in terms of seasonal change of minerals. These results appear to indicate that the harvest period is hypercritical in deciding the antioxidant potency of tea crop shoots.

Abstract This research works discuss about the effective utilization of waste Ramie Root, that has been in reinforced polyester composites, powdered fillers that have not been treated are used. Four different composites plate were formed with compression moulding technique process consisting of 20, 30 and 40% of Powdered Ramie Root with 80, 70 and 60% unsaturated polyester resin, respectively. The maximum mechanical properties were observed for the composite with 30:70 weight volume percentages of milled ramie root synthetic reinforced polyester. The findings show that the glass transition temperature, storage modulus, and loss factors all rise when the composition of composites changes. Additionally, the powder cohesion force (bonding strength) has a greater impact on dynamic mechanical properties. Thermo-gravimetric the inclusion of Ramie Root powder caused the thermal deterioration peak of the composite to move from 370 °C to 418 °C, according to analysis (TGA) conducted under flowing oxygen. According to the measurement of water absorption, the ideal weight ratio of Fiber: Unsaturated Polyester Resin is 30:70, which modifies the fibres’ surfaces and ensures optimal adhesion between the fibre and matrix in composite materials. Scanning electron microscopic investigation is done to ascertain the fracture behaviour of the composite. As a result of their stability, high tensile strength, and bending stiffness, the produced composites can be used in light-load applications by material technologists.

Abstract Post-operative urinary retention is a medical condition where patients cannot urinate despite having a full bladder. Ultrasound imaging of the bladder is used to estimate urine volume for early diagnosis and management of urine retention. Moreover, the use of bladder ultrasound can reduce the need for an indwelling urinary catheter and the risk of catheter-associated urinary tract infection. Wearable ultrasound devices combined with machine-learning based bladder volume estimation algorithms reduce the burdens of nurses in hospital settings and improve outpatient care. However, existing algorithms are memory and computation intensive, thereby demanding the use of expensive GPUs. In this paper, we develop and validate a low-compute memory-efficient deep learning model for accurate bladder region segmentation and urine volume calculation. B-mode ultrasound bladder images of 360 patients were divided into training and validation sets; another 74 patients were used as the test dataset. Our 1-bit quantized models with 4-bits and 6-bits skip connections achieved an accuracy within $$3.8\%$$ 3.8 % and $$2.6\%$$ 2.6 % , respectively, of a full precision state-of-the-art neural network (NN) without any floating-point operations and with an $$11.5\times$$ 11.5 × and $$9.0\times$$ 9.0 × reduction in memory requirements to fit under 150 kB. The means and standard deviations of the volume estimation errors, relative to estimates from ground-truth clinician annotations, were $$5.0\pm 33$$ 5.0 ± 33 ml and $$6.8\pm 29$$ 6.8 ± 29 ml, respectively. This lightweight NN can be easily integrated on the wearable ultrasound device for automated and continuous monitoring of urine volume. Our approach can potentially be extended to other clinical applications, such as monitoring blood pressure and fetal heart rate.

Abstract Foundation plays a vital role in weight transfer from the superstructure to substructure. However, foundation characteristics such as pile group, piled raft, and footing remain unfolded due to their highly non-linear behaviour in different soil types. Bibliography analysis using VOSvierwer algorithm supported the significance of the research. Hence, this study investigates the load-bearing capacity of different types of foundations, including footings, pile groups, and piled rafts, by analyzing experimental data using finite element tools such as PLAXIS 2D and GEO5. The analysis involves examining the impact of various factors such as the influence of surcharge and the effect of different soil types on the load-bearing capabilities of the different types of foundation. For footing, parametric investigations using PLAXIS 2D are conducted to explore deformational changes. Pile groups are analyzed using GEO5 to assess their factor of safety (FOS.) and settling under various criteria, such as pile length and soil type. The study also provides insight into selecting the right type of foundation for civil engineering practice. Findings showed that different soil types have varying deformational behaviours under high loads with sandy soil having less horizontal deformation than clayey soil. Also, it was observed that increasing the pile thickness by 50% resulted in a reduction of 13.88% in settlement and an improvement of 16.66% in the FOS. In conclusion, this study highlights the importance of professionalism, exceptional talent, and outstanding decision-making when assessing the load-bearing capabilities of various foundation types for building structures.

Abstract Geopolymer is an environment friendly construction material that could be synthesized using either the natural source or the industrial byproducts such as flyash and GGBS. The characteristics of the Geopolymer rely on the proportion of the flyash and GGBS and the concentration of the activator solution used. In this research work, the effect of partial replacement of flyash with GGBS in proportions such as 0, 10, 20, 30 and 40% is investigated. Also Molarity of NaOH are tested from 8 to 14 M and both the parameters are optimized. In this optimized Geopolymer concrete, the utilization of iron slag as a partial substitute for river sand in various proportions such as 10, 15, 20, 25, 30 35, 40 and 45% are investigated. The optimized Geopolymer concrete with iron slag is investigated for its performance as a paver block with incorporation of banana fiber in proportions such as 0, 0.5, 1 and 1.5 and is compared with conventional cement concrete paver block. The results show that there is a significant enhancement in the properties of Geopolymer concrete with the different levels of optimization and the utilization of natural banana fiber. The developed sustainable paver block was found to with stand medium traffic conditions as per IS 15658:2006. Further this study employed random forest (RF) algorithm for the prediction of compressive strength of geopolymer concrete specimens for the variable parameters such as molarity of alkaline solution, Flyash/GGBS ratio and partial replacement of river sand with iron slag. The performance evaluation parameters represented high accuracy of developed RF model. This research work unleashes a heft potential of Geopolymer concrete to develop economical eco-friendly sustainable paver blocks to the society through mitigation of environmental strain on the ecosystem.

Tripartite ATP-independent periplasmic (TRAP) transporters are secondary-active transporters that receive their substrates via a soluble-binding protein to move bioorganic acids across bacterial or archaeal cell membranes. Recent cryo-electron microscopy (cryo-EM) structures of TRAP transporters provide a broad framework to understand how they work, but the mechanistic details of transport are not yet defined. Here we report the cryo-EM structure of the Haemophilus influenzae N-acetylneuraminate TRAP transporter (HiSiaQM) at 2.99 Å resolution (extending to 2.2 Å at the core), revealing new features. The improved resolution (the previous HiSiaQM structure is 4.7 Å resolution) permits accurate assignment of two Na+ sites and the architecture of the substrate-binding site, consistent with mutagenic and functional data. Moreover, rather than a monomer, the HiSiaQM structure is a homodimer. We observe lipids at the dimer interface, as well as a lipid trapped within the fusion that links the SiaQ and SiaM subunits. We show that the affinity (KD) for the complex between the soluble HiSiaP protein and HiSiaQM is in the micromolar range and that a related SiaP can bind HiSiaQM. This work provides key data that enhances our understanding of the ‘elevator-with-an-operator’ mechanism of TRAP transporters.

Abstract Renewable sources like biofuels have gained significant attention to meet the rising demands of energy supply. Biofuels find useful in several domains of energy generation such as electricity, power, or transportation. Due to the environmental benefits of biofuel, it has gained significant attention in the automotive fuel market. Since the handiness of biofuels become essential, effective models are required to handle and predict the biofuel production in realtime. Deep learning techniques have become a significant technique to model and optimize bioprocesses. In this view, this study designs a new optimal Elman Recurrent Neural Network (OERNN) based prediction model for biofuel prediction, called OERNN-BPP. The OERNN-BPP technique pre-processes the raw data by the use of empirical mode decomposition and fine to coarse reconstruction model. In addition, ERNN model is applied to predict the productivity of biofuel. In order to improve the predictive performance of the ERNN model, a hyperparameter optimization process takes place using political optimizer (PO). The PO is used to optimally select the hyper parameters of the ERNN such as learning rate, batch size, momentum, and weight decay. On the benchmark dataset, a sizable number of simulations are run, and the outcomes are examined from several angles. The simulation results demonstrated the suggested model's advantage over more current methods for estimating the output of biofuels.

Abstract Decreased estrogens during menopause are associated with increased risk of anxiety, depression, type 2 diabetes and obesity. Similarly, depleting estrogens in rodents by ovariectomy, combined with a high-fat diet (HFD), increases anxiety and adiposity. How estrogens and diet interact to affect anxiety and metabolism is poorly understood. Mounting evidence indicates that gut microbiota influence anxiety and metabolism. Here, we investigated the effects of estradiol (E) and HFD on anxiety, metabolism, and their correlation with changes in gut microbiota in female mice. Adult C57BL/6J mice were ovariectomized, implanted with E or vehicle-containing capsules and fed a standard diet or HFD. Anxiety-like behavior was assessed and neuronal activation was measured by c-fos immunoreactivity throughout the brain using iDISCO. HFD increased anxiety-like behavior, while E reduced this HFD-dependent anxiogenic effect. Interestingly, E decreased neuronal activation in brain regions involved in anxiety and metabolism. E treatment also altered gut microbes, a subset of which were associated with anxiety-like behavior. These findings provide insight into gut microbiota-based therapies for anxiety and metabolic disorders associated with declining estrogens in menopausal women.

Introduction Timely diagnosis of interstitial lung disease (ILD) is limited by obstacles in the current patient pathway. Misdiagnosis and delays are common and may lead to a significant burden of diagnostic procedures and worse outcomes. This Delphi survey aimed to identify consensus on the key steps that facilitate the patient journey to an accurate ILD diagnosis and appropriate management in the US.Methods A modified Delphi analysis was conducted, comprising three online surveys based on a comprehensive literature search. The surveys spanned five domains (guidelines, community screening, diagnosis, management and specialist referral) and were completed by a panel of US physicians, including primary care physicians and pulmonologists practising in community or academic settings. A priori definitions of consensus agreement were median scores of 2–3 (agree strongly/agree), with an IQR of 0–1 for questions on a 7-point Likert scale from −3 to 3, or ≥80% agreement for binary questions.Results Forty-nine panellists completed the surveys and 62 statements reached consensus agreement. There was consensus agreement on what should be included in the primary care evaluation of patients with suspected ILD and the next steps following workup. Regarding diagnosis in community pulmonology care, consensus agreement was reached on the requisition and reporting of high-resolution CT scans and the appropriate circumstances for holding multidisciplinary discussions. Additionally, there was consensus agreement on which symptoms and comorbidities should be monitored, the frequency of consultations and the assessment of disease progression. Regarding specialist referral, consensus agreement was reached on which patients should receive priority access to ILD centres and the contents of the referral package.Conclusions These findings clarify the most common issues that should merit further evaluation for ILD and help define the steps for timely, accurate diagnosis and appropriate collaborative specialty management of patients with ILD.

Abstract Perturbations in the autonomic nervous system occur in individuals experiencing increasing levels of motion sickness. Here, we investigated the effects of transauricular electrical stimulation (tES) on autonomic function during visually induced motion sickness, through the analysis of spectral and time-frequency heart rate variability. To determine the efficacy of tES, we compared sham and tES conditions in a randomized, within-subjects, cross-over design in 14 healthy participants. We found that tES reduced motion sickness symptoms by significantly increasing normalized high-frequency (HF) power and decreasing both normalized low-frequency (LF) power and the power ratio of LF and HF components (LF/HF ratio). Furthermore, behavioral data recorded using the motion sickness assessment questionnaire (MSAQ) showed significant differences in decreased symptoms during tES compared to sham condition for the total MSAQ scores and, central and sopite categories of the MSAQ. Our preliminary findings suggest that by administering tES, parasympathetic modulation is increased, and autonomic imbalance induced by motion sickness is restored. This study provides first evidence that tES may have potential as a non-pharmacological neuromodulation tool to keep motion sickness at bay. Thus, these findings may have implications towards protecting people from becoming motion sick and possible accelerated recovery from the malady.

Abstract The forward model design was employed in the Diffuse Optical Tomography (DOT) system to determine the optimal photonic flux in soft tissues like the brain and breast. Absorption coefficient (mua), reduced scattering coefficient (mus), and photonic flux (phi) were the parameters subjected to optimization. The Box–Behnken Design (BBD) method of the Response Surface Methodology (RSM) was applied to enhance the Diffuse Optical Tomography experimental system. The DC modulation voltages applied to different laser diodes of 850 nm and 780 nm wavelengths and spacing between the source and detector are the two factors operating on three optimization parameters that predicted the result through two-dimensional tissue image contours. The analysis of the Variance (ANOVA) model developed was substantial (R2 = > 0.954). The experimental results indicate that spacing and wavelength were more influential factors for rebuilding image contour. The position of the tumor in soft tissues is inspired by parameters like absorption coefficient and scattering coefficient, which depend on DC voltages applied to the Laser diode. This regression method predicted the values throughout the studied parameter space and was suitable for enhancement learning of diffuse optical tomography systems. The range of residual error percentage evaluated between experimental and predicted values for mua, mus, and phi was 0.301%, 0.287%, and 0.1%, respectively.

Background: Cerebrovascular accidents (CVAs) represent a substantial public health concern due to their high prevalence and associated mortality. Reports indicate that 85% of CVA survivors experience hemiplegia, with 69% suffering from limitations in their upper limb functionality. Despite the existing standard therapeutic approaches, achieving active recovery in the upper extremities remains a challenge. Mirror therapy is an uncomplicated, cost-effective, and patient-centered approach to stroke management. Materials and Methods: This study is a prospective observational study with a quasi-experimental design, with two groups, namely interventional and control groups. Patients who had been provisionally diagnosed and hospitalized with unilateral ischemic stroke and had upper and lower weakness or both on one side and were willing to participate in the study were included. Patients who are bedridden, ventilated, unconscious, and psychologically unstable are excluded from the study. A total of 60 patients were included in the study, 30 each in the case and control groups. Results: In total, 60 patients were included in the study, 30 each in the case and control groups. Most of the study participants belong to the age group of over 55 years (66.66% in the experiment group and 53.33% in the control group). Overall, 80% (n = 24) of the experiment group participants and 66.6% (n = 20) of the control group participants were males. Conclusion: The observations of this study depict a significant improvement in the sensorimotor function after receiving mirror therapy, which validates the findings of the previous studies and hypotheses.

Background: Hemiplegia, which means paralysis of one side of the body, is one of the most common and debilitating effects of a stroke. Stroke is a leading cause of permanent disability in adults. It is estimated that there will be 23 million new cases of strokes and 8 million stroke deaths in 2030. Mirror therapy (MT), in addition to a traditional rehabilitation program, was found to be useful in terms of motor recovery and upper limb function. Materials and Methods: The current study aimed at assessing the effect of audiovisual education on MT exercises among the hospitalized stroke patients. A quasi-experimental design was adopted in this study, where the audiovisual education was given to the experimental group, whereas routine treatment was given to the control group. Results: The result shows that in the experimental group, the majority (40%; n = 6) of the patients belonged to the age group of 61–70 years, whereas in the control group, majority (33.3%; n = 5) of the patients belonged to the age group of 51–60 years and 61–70 years. Majority (60%) of the patients in this study were male. Conclusion: MT exercises were found to be an effective, low-cost, and nonpharmacological method of lowering pain in stroke patients in the hospital.

Abstract Mathematically study mass transfer phenomena involving chemical reactions in the flow of Sisko Ferro nanofluids through the porous surface. Three ferronano particles, manganese-zinc ferrite (Mn1/2Zn1/2Fe2O4), cobalt ferrite (CoFe2O4), and nickel–zinc ferrite (Ni–Zn Fe2O4) are considered with water (H2O) and ethylene glycol (C2H6O2) as base liquids. Appropriate resemblance transitions are used to convert the governing system of a nonlinear PDE to a linear ODE. The Runge–Kutta method, as extended by the shooting technique, is used to accomplish the reduction governing equations. The effects of various associated parameters on fluid concentration and mass transfer rate are investigated: magnetic criterion (M), Siskofluid material factor (A), Solid volume fraction (ϕ) for nanofluids, permeability parameter (Rp), Chemical reaction criterion (γ), Brownian motion factor (Nb), and Thermophoretic parameters (Nt). The current findings indicate that the diffusion proportion of Sisko Ferronanofluid Ni–Zn Fe2O4–H2O and CoFe2O4–H2O is higher than that of Ni–Zn Fe2O4–C2H6O2 and CoFe2O4–C2H6O2 respectively but it is opposite in the case of Mn–Zn ferrite. The comparison study was carried out to validate the precision of the findings.

Abstract Dimethyl ether (DME) and its blend of refrigerants (R429A, R435A, and R510A) are considered in this study's second law analysis as potential replacements for R134a. The performance of various refrigerants in a vapour compression refrigeration system is examined using the Design package CYCLE D. The software REFPROP 9.0 is used to extract all of the thermal and physical parameters of DME and its blend of refrigerants. The Second law performance parameters such as Efficiency Defects, Entropy generation and ExergyEfficiency are discussed. The refrigerants R429A and R510A are more energy efficient than R134a across a condensing temperature range of 30 to 55 °C at − 10 °C evaporation temperature. R134a was exceeded by R429A and R510A in terms of exergetic efficiency by 2.08 and 0.43%, respectively. In comparison to other losses in different components, the compressor's exergy loss is larger at 37–40% of the total exergy loss. By employing RE170 and its blends, the Vapour Compression Refrigeration System often performs better under the second law than R134a. The result shows that the efficiency defects in the compressor are the largest, followed by the condenser and evaporator. Thus, the design improvement of a compressor is of at most importance to improve the system performance by lowering the overall irreversibility.

Abstract The household heating and cooling system often use the capillary device. The use of the helical capillary eliminates the need for lightweight refrigeration devices in the system. Capillary pressure is noticeably affected by the capillary geometric parameters, such as length, mean diameter, and pitch. This paper is concerned with the effects of the capillary length on the performance of the system. Three separate length capillary tubes were used in the experiment. The data on R152a were studied under various conditions to assess the impact of varying the length. Maximum COP is obtained at an evaporator temperature of − 12 °C and capillary length of 3.65 m. The result is drawn that the system performance enhances when the capillary length is improved to 3.65 m when compared to 3.35 m and 3.96 m. As a result, as the capillary length increases up to a specific amount, the system's performance improves. The findings from the experiment were compared with those from the computational fluid dynamics (CFD) analysis.

Abstract The translucent and nano-crystalline PbS films were equipped with the CBD techniques on metal substrates by the temperature of 90 °C through aqueous solutions of Lead Nitrate and Thiourea. The XRD phases verify the crystalline property of synthesized thin films that the shape falls in the cubic structures with favourite orientations. It revealed that the prepared material is cubic crystal oriented as (111), (110), (100) and (101) crystal planes. The crystalline size varied between 0.4 and 0.7 nm. The band gap was assessed using UV–vis captivation spectra and Tau relations. The average energy band gap was found to be 2.43 eV which is greater than bulk materials of PbS; because of quantum confinements of Lead Sulfide Nano Crystalline thin films, and PL also confirms this result. The variation in band gap with Leaf extracts and particle sizes displayed blue shifts characteristic of electrons quantum confinements. SEM micrograph shows extremely uniform and adherent PbS films are found at higher PH values. It was evidently observed that the viscosity of the synthesized thin films reduced from 563 to 111 nm with a rise in pH value. The sample prepared at pH 4 shows good performance, and thin films deposited from Avocado (Glycosmis cochinchinensis) leaf extracts are a promising method to empower pollution remediation and future energy.

Abstract Extracted Mango and Orange juices were investigated by using spectroscopic techniques such as UV/Visible and Fluorescence. Three portions of samples (fresh juice) were stored at 22 °C for eight days, stored in a water bath and heated at 40 °C, 60 °C, and 80 °C for ten minutes. The highest wavelengths (455 nm) were observed from the UV/Vis results for fresh Mango juices, while 270 nm and 460 nm were observed for stored Mango juices. Furthermore, wavelengths of 320 nm were observed in heat-treated mango juice (40 °C). No absorption peaks were observed at 60 °C and 80 °C due to temperature effects. Absorption peaks of fresh fruit were observed at 330 nm and 390 nm, while 260 nm and 320 nm reflect stored orange juices absorptions peaks. From heat-treated stored (40 °C and 60 °C) samples, 320 nm and 260 nm absorption peaks were observed, respectively. Wavelength observed (454 nm, 540 nm & 700 nm) peaks represent the fresh mango juice spectra, while 460 nm and 700 nm are for stored Mango juices. The peaks observed in the region of 400–500 nm and at 700 nm represent heat-treated mango juices at 40 °C. Heat stored Mango juices (60 °C & 80 °C) have peaks at 700 nm. Peaks observed at 700 nm, 500 nm, and 455 nm reflect fresh orange juice, while 460–500 nm and 700 nm represent the emission spectra of the samples. The stored orange juice peaks at 460–500 nm and at 700 nm, but heated-stored orange juice peaks only at 700 nm. The pH values for orange and mango juices were 3.52–3.73 and 4.02–4.72, respectively.

Abstract The widespread use of plastic goods creates huge disposal issues and environmental concerns. Increasing emphasis has been paid to the notion of a circular economy, which might have a significant impact on the demand for plastic raw materials. Post-consumer plastics recycling is a major focus of the nation’s circular economy. This study focuses on energy recovery from waste plastics as an alternative fuel source to meet the circular economy demand. Waste plastic fuel produced through pyrolysis has been claimed to be utilized as a substituted fuel. This work focuses to determine the performance and emission standards of Waste Plastic Fuel (WPF) generated from the pyrolysis of High-Density Polyethylene (HDPE) in a single-cylinder Direct Injection Diesel Engine (DIDE). Three different ratios of WPF were combined with 10% ethanol and 10% ethoxy ethyl acetate as an oxygenated additive to create quaternary fuel blends. The ethanol has a low viscosity, a high oxygen content, a high hydrogen-to-carbon ratio as favourable properties, the quaternary fuel results the improved brake thermal efficiency, fuel consumption and reduced emissions. The blend WEE20 exhibits 4.7% higher brake thermal efficiency, and 7.8% reduced fuel consumption compared to the diesel. The quaternary fuel blends demonstrated decreased carbon monoxide of 3.7 to 13.4% and reduced hydrocarbons of 2 to 16% under different load conditions.

The National TB Elimination Programme (NTEP) of India is implementing tuberculosis preventive treatment (TPT) for all household contacts (HHCs) of pulmonary tuberculosis patients (index patients) aged 5 years with TB infection (TBI). We conducted an explanatory mixed-methods study among index patients registered in the Kolar district, Karnataka during April-December 2022, to assess the TPT cascade and explore the early implementation challenges for TPT provision. Of the 301 index patients, contact tracing home visits were made in 247 (82.1%) instances; a major challenge was index patients’ resistance to home visits fearing stigma, especially among those receiving care from the private sector. Of the 838 HHCs, 765 (91.3%) were screened for TB; the challenges included a lack of clarity on HHC definition and the non-availability of HHCs during house visits. Only 400 (57.8%) of the 692 eligible HHCs underwent an IGRA test for TBI; the challenges included a shortage of IGRA testing logistics and the perceived low risk among HHCs. As HHCs were unaware of their IGRA results, a number of HHCs actually eligible for TPT could not be determined. Among the 83 HHCs advised of the TPT, 81 (98%) initiated treatment, of whom 63 (77%) completed treatment. Though TPT initiation and completion rates are appreciable, the NTEP needs to urgently address the challenges in contact identification and IGRA testing.

Fortification of staple food items with iron is a potential strategy to address anemia for a larger population. We reviewed studies to assess the impact of iron-fortified rice (IFR) on hemoglobin levels of individuals more than six months of age. We included studies assessing the effectiveness of IFR (with or without other micronutrients) conducted in any part of the world available in PubMed, Embase, Web of Science, Cochrane Library, Google Scholar, clinicaltrials.gov, International database of prospectively registered systematic reviews in health and social care (PROSPERO), unicef.org, and who.int databases and published from January 1, 1990 to April 1, 2019 (PROSPERO registration number: RD42020139895). We extracted the changes in hemoglobin, serum ferritin, and serum transferrin levels among the participants. Finally, data from 15 trials and their 21 subsets were analyzed. Mean difference in hemoglobin was 0.53 g/dl (95% CI: 0.26, 0.80, P < 0.001, I2 = 84%) in the IFR group compared to the control group. In the subgroup analysis after removing studies with small sample size and high risk for bias, a mean difference of 0.44 g/dl (95% CI: 0.20, 0.69, P < 0.001, I2 = 82%) was observed. No significant effect on serum ferritin or transferrin levels was observed. From this review it can be concluded that fortification of rice with iron can be adopted as an intervention to improve hemoglobin levels, especially in countries where rice is the major part food of staple diet. Research is needed on an optimal iron compound for fortification and the acceptance of IFR.

Mirror therapy (MT) is a sort of therapy that makes use of vision for managing pain in persons with amputated limbs or weakness in stroke patients. Mirror treatment works by deceiving the brain by making it appear as though the missing limb or weaker limb is moving while the patient looks at the actual, functional limb in a mirror. By doing this, the brain conceals the fact that the amputated limb itself does not provide any signals of movement. Although very encouraging, MT has notable drawbacks that researchers are desiring and working hard to resolve so that it could be applied in treating more composite movements or pain of several types. Stroke has been the third biggest cause worldwide for the years of life lost (YLL); the age-standardized YLL grew by 12.9% (10.6–15.2) between 1990 and 2007 and by 12.1% (9.9–14.1) between 2007 and 2017. MT is a kind of rehabilitation method where the movement of an unaffected limb creates the illusion that the affected limb is also moving, for which a mirror is positioned amidst the legs or arms.

Introduction: Stress activates hypothalamo-pituitary-adrenal axis leading to the release of glucocorticoid that mediates the stress response. This adaptive response is self-limited but if persistent for prolonged periods can lead to disease states. Nature has endowed the body with efficient buffer systems to attenuate the stress effects and Dehydroepiandrosterone (DHEA), a steroid hormone with neuromodulatory functions is implicated as an efficient candidate to buffer stress. Aim: To assess the effect of prophylactic administration of DHEA in the attenuation of acute stress in male Wistar rats. Materials and Methods: This interventional study was carried out at centre for Toxicology and Developmental Research, Sri Ramachandra Institute of Higher Education and Research, Chennai, between June 2021 and August 2021, in compliance with the animal welfare guidelines of CPCSEA, and in accordance to the protocol approved by Institutional animal ethics committee. The 18 male Wistar rats approved for the study were segregated into 3 groups with 6 animals in control (no stress) group, 6 in stress group and 6 in intervention group that received DHEA prophylactically 30 min before stress procedure. Animals in stress and intervention groups were subjected to one hour immobilisation stress. Blood samples were collected from all animals after the stress period and serum corticosterone, the stress marker, was estimated.
