Acquired hemophilia is a bleeding disorder caused by the development of autoantibodies against plasma coagulation factors, most frequently against the factor VIII, or type A. We report a case of a 69-year-old patient with rheumatoid arthritis for 6 years with ecchymoses, hematomas and macroscopic hematuria, diagnosed with acquired hemophilia A secondary to rheumatoid arthritis, treated with methylprednisolone, desmopressin, activated prothrombin complex concentrate and cyclophosphamide, with remission. Visceral leishmaniasis (VL) is an endemic parasitic disease frequently found in Northeast Brazil and may cause acute kidney injury (AKI) and glomerulonephritis. After appropriate treatment, renal function recovery may occur. We describe the rare case of a patient with VL, who developed severe AKI requiring dialysis and was subsequently diagnosed with Chagas disease coinfection. After specific treatment for VL, there was partial recovery of the renal function, followed by the onset of Chagas disease cardiomyopathy.

Introduction: Emphysematous pyelonephritis (EP) is an acute renal infection, potentially fatal, with high mortality, which is more frequent in women at fifth decade of life. Herein, we describe a case of EP successfully treated with surgery. case report: A 50-year-old female was admitted with complaints of lumbar pain, irradiating to ipsilateral lower limb, associated with nausea and fever (38.9ºc), with chills. Abdominal computed tomography (ct) scan showed amorphous echogenic image, with gaseous component, inside the right kidney, and pyelogram showing lithiasis in the right renal pelvis and filling defect. Urine culture was positive for Escherichia coli . Antibiotic therapy was initiated with piperacillin- tazobactam and opioids. On 15th day of hospital stay, she underwent a right nephrectomy. the histopathological analysis evidenced numerous sclerosed glomeruli, tubules with atrophy, inflammatory infiltrate in the interstitium. she was discharged with complete resolution of the infection, asymptomatic and with normal renal function. conclusion: Emphysematous pyelonephritis is a rare, atypical, and severe form of renal parenchyma infection. Introduction This study aimed to describe the clinical spectrum of dengue in children and adolescents from a hyperendemic region who were admitted for hospitalization. Methods A retrospective study was conducted on patients diagnosed with dengue infection upon admission to a reference center in Fortaleza, Brazil. Results Of the 84 patients included, 42 underwent confirmatory testing. The main symptoms were fever, abdominal pain and vomiting. The median level of serum aspartate aminotransferase was 143.5±128mg/dL. Conclusions A peculiar clinical profile was evident among children and adolescents with dengue infection in a reference center in northeast Brazil, including gastrointestinal symptoms and liver involvement.
