Patients with lupus anticoagulants are at high risk of systemic arterial and venous thrombosis and arterial stroke. We present an unusual case of a young man presenting inguinal pain. Doppler ultrasound revealed spermatic vein thrombosis on the left side. Hematologic workup revealed positive lupus anticoagulant. The patient was treated with therapeutic heparin.

La tumeur de Buschke-Löwenstein appartient au groupe des carcinomes verruqueux. Plus fréquente chez les hommes et les immunodéprimés, elle se développe au niveau génital ou péri-anal, réalisant une volumineuse lésion bourgeonnante parfois ulcérée ou fistulisée aux organes voisins. L´histologie est proche de celle d´un condylome acuminé, avec cependant une prolifération plus marquée et une pénétration profonde dans les tissus adjacents mais sans rupture de la membrane basale. Une infection par Human papillomavirus (HPV) 6 et 11 est volontiers associée à ces tumeurs. La recherche d´autres infections sexuellement transmissibles associées est nécessaire. Un examen clinique rigoureux et une imagerie complémentaire permettent d´établir un bilan lésionnel précis. Le traitement est avant tout chirurgical, et doit être large du fait du haut potentiel récidivant de la tumeur. Des traitements néoadjuvants à type de chimiothérapie ou immunothérapie mériteraient d´être mieux évalués pour limiter le caractère délabrant de la chirurgie.

4. Urodynamic profile of pelvic prolapse before surgical treatment

This is a descriptive prospective study spread over 2 years, involving 30 patients with symptomatic urogenital prolapse, collected at the urodynamic consultation in the urology department at the CHU Mohammed VI in Oujda. The average age is 57 years, the majority of patients had several risk factors for pelvic floor disorders, all patients had a urinary complaint associated with urge prolapse (66.7%), urinary Incotinence iue (63.3%), mixed urinary incontinence (40%), dysuria (40%), and pollakiuria (70%). Twenty six percent point seven had cystocele stage II and 73.3% had cystocele stage III, 26.7% had a prolapse of more than one floor according to the classification of Baden and Walker. Their mean body mass index (BMI) was 27.5 kg/m2. For the urodynamic assessment (BUD): at the flowmeters: dysuria was found in 11 patients (36.7%), with a significant post-voiding residue in 5 patients (16.7%), the normal flow was found in 63.3% of cases. At cystomanometry: bladder hypersensitivity was found in 8 patients (26.7%), reduced bladder capacity in 4 patients (13.3%) and detrusor overactivity in 9 patients (30%). Profilometry had demonstrated sphincter insufficiency in 17 patients (56.7%), whereas it was normal in 13 patients (43.3%). Urodynamic explorations are part of the diagnostic and prognostic elements in the evaluation of bladder and sphincter disorders associated with urogenital prolapse. They make it possible to objectify the impact of prolapse on the bladder sphincter equilibrium.

Nutcracker syndrome (NCS) results most commonly from the compression of the left renal vein by the aorta and vertebral column. Although NCS had been described in many studies, her prevalence is still not known, usual clinical signs are gross hematuria and flank pain. Diagnosis requires a high index of suspicion and treatment consists of a wide range of vascular surgical options with a more recent focus using an endovascular approach. We present a case of a 23-year-old male with intermittent hematuria and flank pain.

The long-term drainage of the urinary tract of patients with unresectable ureteric stenosis, remains a major challenge for urologists. For decades, researchers experimented with different alternatives to the prosthetic ureter with minimal success. However, these patients are offered a minimally invasive procedure in the form of percutaneous nephrostomy or reconstructive surgery. Percutaneous nephrostomy, although minimally invasive, is far from ideal for long-term use. We report the case of a 43-years-old woman who was followed for obstructive renal failure secondary to locally advanced cervical cancer, for which she has benefited a right JJ stent and left nephrostomy. Renal-bladder bypass was indicated as an alternative to nephrostomy. We report on the established implantation procedure and the experiences collected in our service with the Detour® system in the various application areas.

Urinary incontinence is a disastrous complication in men following definitive treatement of prostate cancer or less commonly prostate surgery for benign conditions. In cases where conserative management fails, surgical intervention is indicated. We report our experience in Morocco with artificial sphincter type ZEPHYR ZSI 375 PF in patient with urinary incontinence after radical prostactemy (RP), with discussion of the factors that affect the response rate.

8. Penile gangrene: an unusual complication of malignant priapism in a patient with renal cell carcinoma

A 68-year-old man presented with priapism and penile gangrene. The patient had no history of penis trauma or medications for erectile dysfunction. Corpus cavernosa aspiration cytology were positive for malignant cells. Total penectomy was performed. Enhanced chest and abdominal computed tomography showed a left renal tumor with pulmonary and hepatic metastases. Ultrasound-guided renal biopsy showed clear cell renal cell carcinoma.

Highlights • Penile necrosis by calciphylaxis is a rare disease. • The diagnosis of CUA is often difficult. • The Management of this rare condition is still a matter of debate., Introduction Penile calciphylaxis is a rare disease whose diagnosis is not easy. It is associated with chronic renal failure. It is a result of media calcification and blood vessels’ fibrosis such as penile arteries that eventually lead to gangrene formation in extremities and penis. Calcific uremic arteriolopathy is commonly associated with secondary hyperparathyroidism and high level of calcium and phosphate. Presentation of case In this paper, we report a case of penile necrosis induced by calciphylaxis associated with chronic renal failure and diabetic macroangiopathy. We performed a partial and total amputation in two stages because of the wound infection. The patient died one week after a total penectomy. Discussion The skin biopsy is the gold standard for the diagnosis despite being a risky process. In most cases, appropriate medical management is advocated. The treatment of this pathology is still controversial. Conclusion The management of this rare situation is controversial, and its diagnosis is still difficult due to the scarcity of reported cases.

Small cell carcinoma of the upper urinary tract is very rare. Their aggressiveness and their poor prognosis make it grave. We report the management of a patient who consulted in the emergency room for low back pain and to whom the radiological and endoscopic assessments found a tumor of the upper urinary tract with lung, liver, bone and adrenal damage. Anatomopathological study of biopsies obtained by flexible ureteroscopy and percutaneous liver biopsy confirmed the diagnosis and the metastatic nature of secondary lesions.

The Common sites for breast cancer metastasis include bones, lungs, brain and liver. Kidney metastases from the breast are rare. We report a case of metastatic kidney cancer. A 33-year-old woman who underwent a left mastectomy at the age of 30 for breast cancer followed by pulmonary and cerebral metastasis under chemotherapy. The CT scan revealed two right kidney lesions. He imitated a primary kidney tumor. Percutaneous kidney tumor biopsy confirmed metastases from breast cancer. Kidney surgery was avoided and treated with systemic chemotherapy.

Renal cell carcinoma (RCC) is an extremely rare and fatal tumor for women of childbearing age. Consequently, the cases published in the literature are limited and medical experience with therapeutic management of newly diagnosed RCC in pregnant women is poor. We report our clinical experience with a renal tumor diagnosed in first-trimester pregnancy complicated in postoperative by an inferior vena cava thrombosis. Percutanous nephrolithotomy is the gold standard method for large kidney stones currently. It offers high success and low complication rates. Aim There are only a few reports of fractured guidewires inside the pelvicalyceal system, removed with endourology and open surgery techniques, and this is a tour knowledge the first one presenting a broken guidwire in the gerota fat. A 60-year-old man is hospitalized in our department of urology, Mohammed IV university medical center for the management of a PCNL failure in a private clinic.A CT scan showed the presence of a metal device in the gerota fat, we performed an open nephrolithomy, the evolution was favorable. Some equipment used during nlpc can lead to severe issues (broken guidwire and laser fibers) and and open surgery is the only way to resolve the situation.

The adrenal lipoma is an extremely rare, benign, and non-functional tumor. We present the first case of adrenal lipoma on the African continent and the youngest patient reported to date. computed tomography (CT) scan guided diagnosis and laparoscopic adrenalectomy was performed given symptomatic and large mass. Histological examination confirmed the diagnosis. At 12 months after the surgery, the patient had no evidence of recurrence.

Collecting duct carcinoma, also known as Bellini duct carcinoma (BDC) is a rare type of renal tumor, arising from the distal collecting ducts. The prognosis of this disease is extremely poor due to its rapid progression with widespread metastasis. The present study reported a case of CDC involving the left renal region of a 68-year-old female patient. CT scan showed a huge mass occupying the lower portion of the left kidney. The patient underwent enlarged nephrectomy. Anatomopathological examination showed collecting duct carcinoma of the kidney. Patient's evolution was exceptionally favorable: no recurrence, no locoregional metastasis and no distant metastasis.

Testicular ENKTCL is a rare disease. Asia is the most affected. Primary testicular NK/T-cell lymphoma is rare. Metastases are early and the prognosis is poor. Metastases mainly involve the lymph nodes, skin, contralateral testis, bone marrow, spleen and central nervous system. Nasal metastasis giving rise to bifocal presentation is extremely rare. We report the management of a patient initially seen for a nasal swelling with a hidden history of scrotal swelling, in whom nasal biopsies as well as the analysis of the orchidectomy part made it possible to retain the diagnosis of ENKTCL of the testis with a nasal metastasis.

Acute pain is the only manifestation of kidney hematoma and should be explored. Conservative treatment is generally the treatment of choice. We reported the case of a 60-year-old man who underwent extracorporeal lithotripsy, 4 h after the procedure, he was presented to the emergency with severe abdominal pain, and a CT scan showed a hematoma of the left kidney. Conservative treatment was performed, and the patient was discharged at home in 48h.

The discovery of a kidney stone in a living kidney donor is rare. The managing technique of this situation and the timing of the treatment of the stone is not well codified. Should it be treated before the removal of the kidney or in ex-vivo after nephrectomy and in cold ischemia ? We report a case of a 60-year-old mom who donates kidney willingly to her son. The decision of the transplant team was for the removal of the left kidney (upper calyx stone of 9 mm) and the treatment of the stone in ex-vivo by flexible ureteroscopy.

Usual location of metastasis are in renal cell carcinoma (RCC) the lung, liver, bone, adrenal and brain. Here we report on an unusual case of a 76-year-old male with a solitary sternal metastasis as an initial presentation of clear-cell RCC. The management of metastatic kidney cancer has been disrupted over the last decade by the emergence of new treatments and the progress of surgery and ablative techniques. the Carmena study suggests that sunitinib alone is not inferior to the NCR followed by sunitinib, the patient has therefore been transferred to oncology for medical treatment (targeted therapy).

Objective To assess the long-term follow-up after ileocaecal continent cutaneous reservoir (ICCR) and to review the late complications. Patients and methods In all, 756 patients underwent an ICCR in our department, with long-term follow-up data available in 50 patients. The inclusion criterion was ICCR regardless of the indication and the exclusion criteria were orthotopic neobladder or other continent urinary diversions not performed with the ileocaecum. Patients were followed to record primary outcomes and late complications. Complications were stratified according to the Clavien–Dindo classification. Results The mean patient age was 44 years and pelvic malignancies were the first indication for urinary diversion. The mean (range) follow-up was 19 (9–36) years. A stoma stenosis was the most frequent outlet-related complication requiring re-intervention, followed by ischaemic outlet degeneration, and stoma incontinence. Six renal units (RUs) developed obstruction at the anastomotic site and were managed by open surgery. Three RUs had to be removed due to deterioration. A dederivation was necessary in three patients (6%). Conclusion The ICCR is a safe and established technique when an orthotopic pouch is impossible. The long-term follow-up shows acceptable complication rates and satisfactory continence conditions. However, large population studies are necessary to confirm this observation.

Objectifs L’endometriose est definie par la presence de tissu endometrial en position ectopique. C’est une pathologie frequente chez la femme en âge de procreer. L’endometriose de l’appareil urinaire est beaucoup plus rare et se classe dans les endometrioses pelviennes profondes. L’atteinte de la vessie est la localisation la plus frequente des localisations urinaires. Methodes Nous rapportant un cas d’endometriose vesicale isolee traite dans notre formation en 2018. Les informations analysees ont interesse les donnees cliniques, endoscopie et d’imagerie ainsi que la technique operatoire d’une cystectomie partielle laparoscopique. Resultats Patiente de 19 ans, celibataire, avec antecedent de laparotomie pour peritonite appendiculaire, admise dans notre formation pour des douleurs pelviennes associees a une hematurie macroscopique catameniale evoluant depuis 3 ans. L’examen clinique etait sans particularites. L’ECBU etait sterile. L’echographie pelvienne a objective un epaississement du dome vesical, l’uterus et les annexes avaient un aspect normal. La cystoscopie montrait une plaque erythemateuse au niveau de la paroi supero-anterieure de la vessie, des biopsies realisees revenues en faveur d’endometriose vesicale. Une IRM pelvienne a objective un processus vesical en isosignal etendu de la face anterieure jusqu’au cul-de-sac vesico-uterin. Une cystectomie partielle, par voie cœlioscopique etait pratiquee. La sonde vesicale a ete retiree le huitieme jour postoperatoire. L’examen histologique etait en faveur d’une endometriose vesicale avec marges de resection saines. Conclusion Devant une douleur pelvienne ou un trouble urinaire cyclique, a recrudescence catameniale, il faut penser, malgre sa rarete, a la possibilite d’une endometriose urinaire. Actuellement, les signes radiologiques d’IRM sont tres evocateurs. La cystectomie partielle est l’intervention de reference pour l’endometriose vesicale profonde. Elle doit idealement etre realisee par voie cœlioscopique.
