2. Psychometric Properties of the Persian Version of the Assessment of Time Management Skills

Background and Objectives: Time management is an essential skill that enables individuals to participate in daily activities effectively. The assessment of time management skills (ATMS) is a widely used instrument to evaluate time management skills, but its psychometric properties have not been examined in Persian-speaking populations. This study aimed to assess the cross-cultural adaptation and psychometric properties of the Persian version of the ATMS (ATMS-P). Methods: A convenience sample of 144 healthy adults aged 18-55 years completed the ATMS-P, along with several other questionnaires. Face and content validity were determined using the impact score of each item, content validity ratio (CVR) and content validity index (CVI) methods. The construct validity was evaluated using exploratory factor analysis (EFA) and convergent validity was assessed by correlating the ATMS-P with other related measures. The reliability was assessed using internal consistency and test re-test methods. Results: Face validity impact scores ranged from 2.2 to 4 and the CVR was in an acceptable range of 0.57 to 1, with a CVI of 0.87 to 1. The EFA identified three factors for the Persian version of the ATMS: Time management, organization and planning and emotion regulation. The convergent validity of the ATMS-P was supported by significant correlations with other related measures. The internal consistency of the ATMS-P was excellent (Cronbach’s α coefficient=0.926) and it demonstrated good test re-test reliability (ICC=0.903). Conclusion: The ATMS-P has demonstrated good psychometric properties, making it a reliable and valid instrument for evaluating time management skills in Persian-speaking individuals.

3. Token Economy for People With Severe Mental Illness: A Narrative Review of the Literature

Background: People with chronic mental illness have long been rejected from society and lost their self-identity and self-efficacy due to individual obstacles, such as continuous negative symptoms, lack of motivation, or social obstacles, like stigma, which reduce their self-confidence and prevent them from engaging in healthy activity patterns. One of the behavioral therapy methods used to increase the motivation of people with severe mental illness is token economy. Objectives: The purpose of this narrative review was to review and investigate the impact of the token economy on people with severe mental illness. Methods: A search for studies published from 2002 to 2022 in PubMed and Google Scholar databases was conducted using the keywords “token economy”, “token reinforcement”, “contingency management”, “severe mental disorder”, “serious mental illness”, “chronic psychiatric disorder”, and “participation”. Results: In the initial search, 92 articles were obtained, and after reviewing and rejecting duplicates, 44 articles related to severe mental illness and token economy remained. Finally, 15 articles met the inclusion criteria and were selected for review. token economy can be an effective method in improving the performance of daily life activities, reducing the symptoms of mental illnesses and avoiding drugs, reducing alcohol consumption, and quitting smoking Conclusion: token economy can be an effective method in improving the performance of daily life activities, reducing the symptoms of mental illnesses, drug abstinence, reducing alcohol use, and quitting smoking, but in reducing violence, the results of the studies were contradictory. It is recommended to carry out wider studies on the impact of the token economy method on the occupational performance and participation of chronic mental patients.

4. Predictors of Interest in Performing Activities Among Iranian Adolescents with Cerebral Palsy

Objectives: Generally, adolescents who are interested in performing activities experience positive emotions and report high levels of well-being. However, adolescents with Cerebral Palsy (CP) encounter various issues affecting their interests. The present research was planned to investigate the predictor factors that affect the interest in performing activities in Iranian adolescents with CP. Methods: In total, 312 adolescents with CP (10-19-year-olds, Mean±SD age: 14.08±2.34 y) participated in this cross-sectional study. Their gross motor function was assessed by the Gross Motor Function Classification System (GMFCS), and their manual ability was evaluated by the Manual Ability Classification System (MACS) in random order by an occupational therapist. All study participants also completed the Adolescent Interest Inventory-Cerebral Palsy (AII-CP). The predictive model was analyzed using a multiple linear regression model by the enter method. Results: The multiple linear regression analysis data presented that the highest independence in motor performance, personal values, social environment support, and age appeared to be the strongest predictors of interest in performing activities in the explored adolescents with CP (R2: 0.475). Discussion: Interest as a significant factor for participation and motivation in conducting daily living activities is associated with the highest independence in motor performance, personal values, social environment support, and the age of adolescents with CP.

5. The Comparative Content Review of the Persian Participation Assessment Measures: A Systematic Review

Objectives: Facilitating participation is regarded as the primary goal and outcome of rehabilitation. According to the importance of comprehensive assessment of participation in occupational areas (social participation, work, education, instrumental activities of daily living, activities of daily living, rest and sleep, leisure, and play), all available studies on the development or translation of measures, which assess participation in Persian, were systematically reviewed. Methods: A search for Persian versions of measures, which assess participation in at least one occupational area, was carried out. Eight bibliographic databases, including 4 Iranian (IranDoc, Medlib, Magiran, and SID) and 4 international (Cochran, PubMed, Scopus, and ScienceDirect) databases from 1990-2018 were searched by 2 independent researchers based on the predetermined criteria. Any disagreements during the selection processes were resolved in consultation with the third researcher. Results: Through 3 refining steps, by 2 independent researchers, 68 articles exactly related to this study were selected and studied. A total of 50 measures were extracted. The content of the measures and their psychometric properties were reported; 44 were translated into Persian and 6 developed based on the Iranian population. Discussion: This critical review will help Persian-speaking therapists to select an appropriate measure for assessing participation in different occupational areas. Considering the importance of participation in the outcomes of rehabilitation and since it is mostly cultural and familial-based, developing new scales based on Persian-speaking people culture seems necessary.

6. The Effectiveness of Body Percussion Rhythmic Exercises on Motor Skills in Children with Mild Intellectual Disability Between 8-12 Years Old

Background and Objective: Intellectual disability is a common neurological disorder that involves concomitant impairments in mental and adaptive functioning and begins during the developmental period. Percussion is a rhythmic movement activity and the art of body- slapping to produce a variety of sounds for educational, therapeutic, ethnological and social purposes. The purpose of this study was to investigate the effectiveness of percussion rhythmic exercises on motor skills of mild mentally disabled children. Methods: This is a randomized clinical trial in which 60 children with mild intellectual disability aged 8 to 12 years were selected by convenience sampling and divided into two groups: experimental (n = 31) and control (n = 29). The experimental group received body percussion exercises as a group for 12 weeks, 2 sessions per week and 30 minutes per session and the control group received only the usual school schedule. Both groups were evaluated before and after by Bruininks-Oseretsky Test of Motor Proficiency. Paired T-test was used for intra-group comparisons and independent T-test was used to compare the two groups. Results: Results indicated that body percussion rhythmic exercises had a significant effect on some motor items including: fine motor skills, bi-lateral coordination, upper-limb coordination, visual-motor control, speed and upper- limbs speed and dexterity. Conclusion: Therefore, it can be stated that group body percussion rhythmic exercises is a good way to improve motor skills in children with mild intellectual disability.

7. Preparation and Validation of Persian Version of Behavior Rating Inventory of Executive Function (BRIEF-SR) in 11-18 Year Old Normal Adolescence

Introduction: Executive function includes a set of cognitive processes that are essential for controlling behaviors. Executive function helps manage and control thoughts and actions and refers to complex cognitive processes that require coordination with other subprojects. Measuring executive functions is associated with challenges, however, the most authoritative way to evaluate executive function is to use the report of executive functionof teenagers in the natural environment of life. The purpose of this study was to provide and validate a Persian Version of Behavior Rating Inventory of Executive Function (BRIEF-SR) of 11-18 years old normal adolescence. Methods: In this psychometric study, at first English version of the BRIEF-SR questionnaire was translated in Persian and the reliability of each scale was evaluated. The content validity of this questionnaire was determined through a survey of 10 to 15 experienced occupational therapists. The second step was assessing the test-retest reliability of the Persian version of the BRIEF-SR questionnaire. Results: Content validity results showed that almost all items of the BRIEF-SR questionnaire in the Content Validity Index scored above 0.79, and in the content validity ratio, all items except 5 items scored above 49.9. The internal consistency of the Persian version of the BRIEF-SR questionnaire was also obtained by Cronbachchr('39')s alpha of 0.9, which indicates a good reliability. Moreover, the Pearson correlation coefficient was above 0.8, which indicates a high correlation. Conclusion: The Persian version of the BRIEF-SR questionnaire showed that this questionnaire is a valid and reliable tool for assessing executive function in children and adolescents aged 11-18 years old.

Background Health promotion of children and adolescents through participation in purposeful and meaningful activities is one of the focuses of occupational therapy. In this study, in line with the Iranian culture, a tool for assessment of Iranian adolescents’ participation in daily occupations/activities was developed and validated. Method An exploratory sequential mixed method was used to develop and validate the tool. Content and structural validity and reliability of the initial version were estimated. Results During the validation process, the items were reduced to 54. Data were collected from 481 adolescents to investigate factor analysis. According to principal component analysis, the Kaiser–Meyer–Olkin index was 0.88, and the analysis yielded seven factors explaining 47.2% of the total variance. The tool showed excellent internal consistency and test–retest reliability. Conclusion This newly developed Participation Assessment Tool for Iranian adolescents is a valid and reliable measure for assessing the participation of this group in daily activities.

Introduction Enhancing participation is the focus of occupational therapy. Comprehensive and accurate assessment ensures that clinicians can tailor an intervention to the client’s needs. This systematic review was completed to identify Persian adolescents’ participation measures and critically appraise them. It would be helpful in the selection of the most appropriate instrument to use in adolescent-related research and clinical practice. Method Ten bibliographic databases, four Iranian and six international, without year limits were searched up to June 2019. A systematic search was directed according to COSMIN guidelines for systematic reviews of patient-reported outcome measures and PRISMA guidelines (Systematic review registration: CRD42017073581). Results Seven measures were extracted from the articles. Reviewing the content and psychometric properties of the measures as well as the methodological quality of the studies indicated that the Modified Activity Questionnaire is the only measure with consistent and moderately reliable results. It measures adolescent participation in leisure activities but not all domains of participation. Conclusion There may be a growing need for adapting existing Persian measures or developing new ones based on specific age features related to puberty-stage alongside cultural, social and academic demands, which have a significant effect on adolescents’ participation in meaningful occupations. High methodological quality in designing such studies also has great importance.

10. Identifying Return to Work Perception in People With Serious Mental Illness: A Qualitative Study

The current qualitative study sought to identify return to work (RTW) perception among people with serious mental illness (SMI) referred to a psychiatric hospital affiliated with Hamadan University of Medical Sciences. In total, 24 individuals were recruited, including working people with SMI, families, employers, and mental health team members who participated in in-depth, semi-structured interviews. Forty interviews were conducted. Interviews were analyzed based on the conventional content analysis approach. Accordingly, two themes were found, including The Need to Continue Working and Flexible and Supportive Strategies . The first theme included two categories: personal obligations and socioeconomic necessities. The second theme included three categories: coping with mental illness, requesting support from others, and coping with work conditions. Background: Many factors affect return to work (RTW) in employees and workers (hereinafter referred to as working people) with serious mental illness (SMI). Identifying these factors can prevent long-term sick leave absence and work disability. Aim: The current study was conducted to identify facilitators and barriers RTW in working people with SMI admitted to a psychiatric hospital and clinic affiliated with the Hamadan University of Medical Sciences, Iran.Methods: A qualitative study was conducted in 2020 on 27 participants with SMI, families, supervisors, managers, and mental health team members. The participants were selected through purposive sampling. Data were collected using in-depth and semi-structured interviews. A total of 40 interviews were conducted face-to-face and fallow-up by the telephone. The interviews were based on a conventional content analysis approach.Results: Two main themes emerged, including social support and RTW's challenges. Social support consisted of four main categories: emotional support, information support, instrumental support, and supervisor's flexibility. RTW's challenges consisted of two main categories: personal obstacles and general issues.Conclusion: We found that two facilitator and barrier factors including social support and RTW's challenges affected the RTW process in working people with SMI. Identifying facilitator and barrier factors gives us comprehensive information to use in RTW programs and developing policies RTW in working people with SMI.

Introduction: This case report aimed to report the effectiveness of creative occupations on a child with anxiety problems in a mental health center in Iran. Case Presentation: This case report has been performed in 12 sessions with a three-month follow-up. The Spence Children's Anxiety Scale (SCAS) and a structured clinical interview were used to collect information. The child's score on SCAS was 66 at the first session, which was then reduced to 41 at the sixth session, and 27 at the twelfth session. The child's father also confirmed the positive effects of the intervention on the child's behavior. Conclusions: Creative occupations showed a positive effect on reducing the severity of anxiety symptoms. These effects remained to a large extent during the intervention.

Background. Communication, behavioural disturbances and low motivation influence the functional potential and the effectiveness of interventions in adolescents with cerebral palsy (CP). While the model of human occupation (MOHO) is a conceptual model in occupational therapy, no research on the volition of adolescents with CP in daily activities has been undertaken. Aim. To explore the elements contributing to the volition of adolescents with CP based on their own experience Methods. A qualitative approach using directed content analysis with volition of MOHO as a framework was applied. Semi-structured in-depth interviews were carried out with five adolescents with CP and five parents of the same adolescents. Results. In this study primary codes were categorized into eight categories: family and community-related values, individual values, individual perceptions of ability level, sense of control over conditions, enjoying performing activities, enjoying interpersonal relationships, physical context features and social context features. Conclusions. Personal causation in interaction with environmental features is strongly influenced by adolescents’ motivation. Significance. Occupational therapists could improve the motivation of adolescents with CP in interaction with the individual’s volition with focussing on self-efficacy for promoting personal causation. Adapting to the physical environment and changing the attitudes of others to these adolescents is necessary.

One effective way to meet the demands of clients with mental health issues is through collaboration between the formal educational system and clinical setting. This report shares example of such a collaboration and its benefits for various aspects of services delivery. In this report we discuss a relationship between a psychiatric rehabilitation centre and one department of occupational therapy education in Iran, in the form of three activities: collaboration, education and research activities. This cooperation has improved the services. Moreover, it has encouraged occupational therapy students to have a better understanding of their involvement in the field of psychosocial rehabilitation.

Background Depression is one of the common non-motor symptoms in patients with Parkinson disease (PD), which can have a negative effect on the quality of life of the patients. Therefore, it is necessary to have suitable, accurate, cheap, and simple diagnostic tools to identify depression in patients with PD and apply a proper treatment. The current study aimed at comparing the single-item visual analog scale (VAS) with the multi-item hospital anxiety and depression scale (HADS) and patient health questionnaire-9 (PHQ-9) in respect to their sensitivity and specificity to diagnose depression in patients with idiopathic PD. Methods The current cross sectional, comparative study employed the simple non-probability method and selected 79 patients with idiopathic PD, with the mean (SD) age of 59.10 (10.84) years and the average (SD) of disease progression level of 3.24 (1.13), based on Hoehn and Yahr scale. The single-item scale VAS and multi-item tests HADS and PHQ-9 were used as an index tests, and Beck depression inventory (BDI) was employed as the reference test to evaluate depression in 3 levels of lack of (or mild) depression, moderate depression, and severe depression using 90% sensitivity and 90% specificity. To determine the sensitivity, specificity, and the ability of each index test in order to diagnose depression, agreement percent of index tests with reference test and receiver operating characteristic (ROC) analysis were used. Results The results of ROC analysis showed that VAS (AUC = 0.84) and HADS (AUC = 0.87) and PHQ-9 (AUC = 0.85) possessed similar abilities to discriminate depression in patients with PD. The sensitivity of VAS, HADS, and PHQ-9 tests in moderate or greater threshold were 92%, 96%, and 89%, respectively, and in severe threshold were 64%, 79%, and 49%, respectively. Specificity of these tests in moderate or greater threshold were 42%, 50%, and 62%, respectively, and in severe threshold were 88%, 77%, and 88%, respectively. The highest agreement level with BDI in mild/moderate and severe levels of depression were found in VAS and PHQ-9, respectively. Conclusions The single-item scale VAS, similar to multi-item tests HADS and PHQ-9, had a low to high sensitivity and specificity to diagnose depression in moderate or greater and severe thresholds in patients with PD.
