Background: Thyroid dysfunction influences both menstrual flow and fertility, likely through changes in sex hormone levels, gonadotrophin release and possibly ovarian function. Objectives of this work were to study thyroid related complaints and thyroid function tests in patients with menstrual irregularities like menorrhagia, oligomenorrhoea, amenorrhoea, hypomenorrhoea and ploymenorrhoea, to study menstrual patterns in women with diagnosed thyroid disease-hypothyroidism/hyperthyroidism and to study changes in menstrual patterns, if any in these patients during the course of treatment of thyroid disease, who receive correct treatmentMethods: This study was conducted in the Department of Obstetrics and Gynecology in Government L.D. Hospital, Government Medical College, Srinagar during the period from 2006 to 2007. There were two groups under which the study was conducted. Group A: Seventy-five patients of DUB from Department of Gynecology in reproductive age group (15-45 years) presenting with menstrual irregularities like menorrhagia, oligomenorrhoea, amenorrhoea, hypomenorrhoea and ploymenorrhoea were studied for thyroid profile. Group B comprised of 25 patients including already diagnosed 17 hypothyroid and 8 hyperthyroid patients. The study protocol included thorough history taking, general physical examination, meticulous per speculum and pelvic examination and routine investigations like Hb, BT, CT, TLC, DLC, Platelet count and ABO-Rh in all patients, Serum T3, T4, TSH estimation.Results: Most of the patients were in 35-45 years age group. Prevalence of infertility was more in hypothyroid group. 22.66% patients with DUB were detected as hypothyroid where as 13.33% patients were detected as hyperthyroid.Conclusions: Thyroid function tests, many of which are sensitive radioimmunoassay, radiometric assays and even new chemiluminescence method, which can detect minute changes in hormone levels must be done in women presenting with menstrual disorders.

3. Abdominal tuberculosis: a clinicopathologic study in Kashmir valley

Background: Tuberculosis is a major health hazard in India. Because of its diverse manifestations, difficult diagnosis, widespread complications, prolonged morbidity and increased mortality the study of this disease becomes even more important. The objectives of the study were to study age and sex distribution of abdominal tuberculosis, to study association of abdominal tuberculosis with pulmonary tuberculosis, to study various clinical presentations of abdominal tuberculosis, to study various modalities for diagnosis of abdominal tuberculosis and to study the surgical management in relevant patients of abdominal tuberculosis.Methods: This study was carried out in 50 patients of abdominal tuberculosis of different age group and sex admitted in Department of Surgery, SMHS Hospital, Govt. Medical College, Srinagar during the period from 2002 to 2004. The patients of abdominal tuberculosis fell into two broad categories - those presenting with acute/sub-acute intestinal obstruction and those without obstruction. All patients were worked up with the elicitation of proper history, clinical examination, laboratory investigation, etc. The diagnosis was confirmed by IgM ELISA for tuberculosis, peritoneocentesis, FNAC, laparotomy and biopsy of the specimen obtained at the time of surgery.Results: Maximum incidence of abdominal tuberculosis was noted in third and fourth decades followed by second decade of life. Males constituted 60% and females constituted 40% of total patients. 23 patients presented with acute and 27 patients with chronic onset of signs and symptoms respectively. Abdominal pain, anorexia, weight loss, weakness, vomiting and constipation were the main presenting symptoms. No haemoglobin and raised ESR were the commonest finding on routine laboratory investigations in majority of the patients. IgM ELISA for tuberculosis was positive in 41 patients (95.34%). Surgical intervention was required in 50% of the patients.Conclusions: Good clinicopathological workup in patients of abdominal tuberculosis results in earlier diagnosis and timely management of this curable disease.

4. A clinical study of normothermic open heart surgery

Background: Normothermic open heart surgery is a new approach which is envisaged for better myocardial protection during cardiac surgery based on the concept that chemically arrested heart is perfused continuously with blood and maintained at 37ºC.Methods: This study was conducted in 50 patients at Super Specialty Hospital, Jammu in the Department of CTVS for a period of one year from 2015 to 2016. The patients of all age groups and both sexes were taken up for surgery which included patients of congenital and acquired heart diseases. All patients were undergone thorough history taking, investigations like cardiac enzymes estimation, LFT, RFT, echocardiography, ECG. Cardioplegia used during surgery consisted of warm oxygenated blood, KCL, sodium bicarbonate, hydrocortisone, Mannitol.Results: Majority of patients were in age group of 11-20 years which comprised 30% of all patients. the male and female patients were equal and ratio was 1:1. Per-operative diagnosis was same as pre-operative echocardiographic diagnosis except that one (2%) patient had ASD with partial anomalous pulmonary veins opening into right atrium. One patient (2%) had single ventricle with pulmonary stenosis. Maximum number of patients underwent mitral valve replacement followed by ASD repair. Mean aortic cross-clamp time was 62 patients and mean total CPB was 71.5 minutes. Mean value of volume of blood cardioplegia used was 1100 ml and for priming solution it was 1500 ml. Cardiac enzyme level showed peak rise above normal after 24 hours after surgery in majority of patients.Conclusions: Normothermic open heart surgery can be used in any diverse surgical procedure safely and effectively.
