Although survival is superior after the arterial switch procedure, data on pregnancy outcomes are scarce and transposition of the great arteries after arterial switch is not yet included in the modified World Health Organization classification of maternal cardiovascular risk. Methods and Results The ROPAC (Registry of Pregnancy and Cardiac disease) is an international prospective registry of pregnant women with cardiac disease, part of the European Society of Cardiology EURObservational Research Programme. Pregnancy outcomes in all women after an arterial switch procedure for transposition of the great arteries are described. The primary end point was a major adverse cardiovascular event, defined as combined end point of maternal death, supraventricular or ventricular arrhythmias requiring treatment, heart failure, aortic dissection, endocarditis, ischemic coronary events, and thromboembolic events. Altogether, 41 pregnant women (mean age, 26.7±3.9 years) were included, and there was no maternal mortality. A major adverse cardiovascular event occurred in 2 women (4.9%): heart failure in one (2.4%) and ventricular tachycardia in another (2.4%). One woman experienced fetal loss, whereas no neonatal mortality was observed. Conclusions Women after an arterial switch procedure for transposition of the great arteries tolerate pregnancy well, with a favorable maternal and fetal outcome. During counseling, most women should be reassured that the risk of pregnancy is low. Classification as modified World Health Organization risk class II seems appropriate. Background Maternal congenital heart disease (CHD) is the most common cardiac condition to complicate pregnancy and women now become pregnant even with complex CHD. The Registry Of Pregnancy And Cardiac disease (ROPAC) examines the relationship between maternal heart disease and pregnancy outcome. Methods The ESC EORP ROPAC is a worldwide prospective registry of pregnancies in women with structural heart disease (n=5739, recruiting between 2007-2018), including CHD. Maternal and fetal outcomes were examined in all women with CHD. Multivariable regression was used to identify associations with a composite endpoint of maternal mortality and/or heart failure. Results In CHD pregnancies (n=3295, mean age 29 years), maternal mortality was 0.3% and heart failure occurred in 6.6%. Preterm births (16%) and Caesarean section (46.3%) were higher than global averages, but otherwise cardiac, obstetric and fetal outcomes were good. The composite endpoint was highest in complex CHD: Eisenmenger syndrome (58.1%), congenitally corrected transposition of the great arteries (12.8%), Fontan circulation (11.2%), double outlet right ventricle (11.1%). Pre-pregnancy signs of heart failure (OR 10.6, 95% CI 7.1-16), multiple gestation (4.6, 2-10.8), pulmonary hypertension (2.5, 1.5-4), estimated LVEF Conclusion Overall pregnancy outcomes for women with CHD are good but women with complex CHD are at increased risk of complications. Pre-pregnancy assessment can identify women at increased risk of an adverse outcome and should be used to counsel women appropriately.

Introduction The screening of carotid and/or ileo-femoral subclinical atherosclerosis using vascular ultrasound could be a cost-effective tool in the reclassification of cardiovascular risk in primary cardiovascular prevention. But until today we don't have evidence indicating which atherosclerotic load indicator variable (such as plaque area, number of atherosclerotic plaques or affected territories) has the most predictive power for future cardiovascular events. Objectives a. Evaluate the prevalence of carotid and ileo-femoral subclinical atherosclerosis (SubAth). b. Evaluate the predictive power of each atherosclerotic load indicator variable for cardiovascular events (CVE). c. to compare those methods with Framingham Score for prediction of CVE. Material and methods We retrospectively evaluated patients from our registry called CARFARE (CARDIOMETABOLIC RISK FACTORS REGISTRY) conducted in the context of a cardiovascular primary prevention program (n=6108). In this analysis we included only those patients who completed a follow up ≥24 months from the first vascular evaluation. Exclusion criteria: 65 years old, BMI>40 kg/m2, previous cardiovascular and/or cerebrovascular event, chronic stable angina. We used Framingham score for the cardiovascular risk assessment. We evaluated the following atherosclerotic load variables using high resolution vascular ultrasonography (Mannheim Consensus): carotid plaques area (AR-C), ileo-femoral plaques area (AR-IF), and total (carotid+ileo-femoral, AR-TOT), number of affected territories (N-TER). Primary composite endpoints (P-CVE): myocardial infarction, stroke, hospitalization due to angina or acute heart failure. For prediction power analysis we used ROC curves and logistic regressions (unadjusted and adjusted by age, sex, BMI, major cardiovascular risk factors and treatment). Results We included 1431 patients (55.0±9.78 y.o., 38% women, follow-up period of 789±23 days). The total P-CVE rate was 3.77% during this period (54 events). The overall prevalence of atherosclerosis in this population was 60.7%. The AUC of the ROC curves for P-CVE prediction was 0.648, for Framingham 0.706, for AR-C 0.726, for AR-IF 0.746, for AR-TOT and 0.79 for N-TER (graph). The AUC of AR-TOT and N-TER were significantly higher than the AUC of the Framingham score (p=0.017 and p=0.0004, respectively) for prediction of P-CVE. The dichotomization criteria according to ROC were: AR-C>5.80 mm2, AR-IF>23.0 mm2, AR-TOT>43.9 mm2, and N-TER>1. In the logistic regressions adjusted for prediction of P-CVE the OR were: Framingham 1.05 (95% CI: 1.02–1.07), AR-C 4.00 (95% CI: 1.89–8.47), AR-IF 4.01 (95% CI: 2.02–8.32), AR-TOT 4.35 (95% CI: 2.13- 8.91) and N-TER 6.95 (95% CI: 3.05–15.8). Comparison of ROC curves for CVE Conclusions The carotid/ileo-femoral SubAth scan was a more potent predictor of cardiovascular events than the Framingham score, particularly those variables that indicate extension of multiterritorial affection like AR-TOT or N-TER.

Climate change is one of the greatest public health threats of the twenty-first century. Recent surveys of veterinary students and practicing veterinarians have highlighted their concerns about the impacts of climate change on animal health and a strong desire to be knowledgeable about the practice and promotion of environmental sustainability within clinical practice. Most American Veterinary Medical Association (AVMA)–accredited veterinary schools have a veterinary teaching hospital (VTH) where veterinary students receive their core clinical education. Given this, VTHs may provide opportunities for students to learn how veterinary clinics can decrease their environmental footprint and actions they could incorporate into their future clinical work. To assess the feasibility of and support for introducing environmentally sustainable practices into VTHs, we distributed an anonymous online survey to all AVMA-accredited veterinary schools with an associated VTH. Responses were received from 843 individuals representing 23 VTHs in 7 countries. While the overwhelming majority of responding personnel believe this is an important topic, there is little evidence that sustainable behaviors are being practiced or showcased within VTHs. Respondents were most interested in working to increase recycling and reduce general waste and energy consumption within their hospitals. In addition to a lack of educational resources, funding was a commonly identified barrier to incorporating more environmentally sustainable practices. These results add to the growing evidence that enhanced incorporation of sustainability into veterinary medical education at all stages is needed and that VTHs provide a unique opportunity to lead by example.

The role of apoptosis in the persistence of hepatitis C virus (HCV) infection is controversial. Moreover, conflicting data on the modulation of this process by HCV proteins have been provided. We evaluated the susceptibility of peripheral lymphocytes from patients with chronic hepatitis C to apoptosis both spontaneous and after incubation with a chimeric Cucumber mosaic virus (CMV) carrying 180 copies of the synthetic R9 mimotope obtained from more than 200 hypervariable region-1 sequences of HCV. Resting T lymphocytes were found to be sensitized to apoptosis as a result of chronic HCV infection. The plant virus-derived vector R9-CMV displayed a strong pro-apoptotic effect associated with activation of both caspase-8 and -9, indicating the involvement of both extrinsic and intrinsic apoptotic pathways. A parallel R9-CMV-mediated activation of endoplasmic reticulum-stress was suggested by the significant induction of BiP/GRP78, GADD153 and caspase-12. These data contribute to define the complex HCV/host interaction, and open new prospects for developing a plant-derived antigen-presenting system to strengthen host defences against persistent pathogens.

Emtricitabine (FTC) is a synthetic nucleoside analogue of cytosine, which is intracellularly phosforylated to form the active form emtricitabine 5'triphosphate (E5TP). E5TP inhibits both HIV and Hepatitis B reverse transcriptase. Clinical trials have shown FTC to be active as part of highly active anti-retroviral therapy in naive HIV-positive patients. FTC may be dosed once daily and in vitro is less associated with the M184V mutation, which is classically associated with failure of treatment with lamivudine. In clinical practice, toxicity with FTC is unusual. The most common treatment-related adverse events are diarrhoea, headache, nausea, dizziness, abdominal pain, aesthenia and rash. Skin discolouration, which is typically reported as hyperpigmentation and usually affecting either the palms of the hands or the soles of the feet, is reported on under 2% of individuals and is almost exclusive to patients of African origin.

The clinical course of thrombotic thrombocytopenic purpura has dramatically improved after the introduction of plasma-based therapy, including plasma exchange and plasma infusion. However, a considerable number of patients still experience relapse after initial successful treatment. In this study, vincristine (VCR) was given as salvage treatment in 12 episodes of recurrent thrombotic thrombocytopenic purpura in seven patients, concomitantly with short-term plasma infusion. Complete remission (CR) was defined by normal platelet, hemoglobin, and serum lactic dehydrogenase (LDH) values as well as by absence of clinical signs. Of 12 patients, 12 achieved CR following therapy with VCR. The median duration of CR was 15 months (range: 2-16). Toxicity was mild consisting of paresthesias in three cases, leukopenia in one case, and autonomic neuropathy leading to paralytic ileus in one case. We conclude that VCR is remarkably effective for recurrent thrombotic thrombocytopenic purpura with acceptable toxicity.

The purpose of this study was to describe the pattern of the ocular late phase reaction in patients with ocular hay fever. The authors monitored the clinical and cytological conjunctival response after topical allergen challenge. A conjunctival provocation test was performed in six ryegrass-sensitive patients and six healthy volunteers with 320,000 Biological Units (Pharmacia, Uppsala) of ryegrass antigen, diluted in albumin, in one eye; albumin diluent alone was used in the second (control) eye. Clinical symptoms were evaluated before challenge after 20 min, then hourly for 12 hours and 24 hours after challenge. Cell counts in the tear fluid were performed at the same time interval The results obtained show a significant clinical reaction during the entire period, differing in peaks in individual cases. This reaction was associated with a persistent inflammation, with neutrophils in the early phase and eosinophils and lymphocytes in the late phase period The data thus show that ocular challenge with high allergen doses induces clinical and cellular evidence of an ocular late phase reaction and suggest that the conjunctival provocation test may be used for a better understanding of the pathogenesis of mild but persistent ocular inflammation in ocular hay fever.

Cognitive impairment is a clinically complex symptom commonly experienced by cancer survivors. Although research in this area has grown, many questions remain regarding underlying mechanisms, trajectory, and specific interventions nurses can offer patients to prevent, treat, and manage cognitive impairment effectively. As part of the Oncology Nursing Society (ONS) Putting Evidence Into Practice (PEP) initiative, a comprehensive examination of the current literature was conducted to identify effective interventions for cognitive impairment in cancer survivors. The studies were categorized into nonpharmacologic interventions, including complementary and alternative therapies and cognitive training, and pharmacologic interventions, including psychostimulants and erythropoietin-stimulating agents. Using the ONS PEP Weight of Evidence Classification Schema, the levels of evidence for these interventions were consistent with the categories of effectiveness not established or not recommended for practice. Additional research is needed to identify effective preventive and treatment strategies for cognitive impairment in cancer survivors.

Treatment of patients continues to shift toward the outpatient setting, with the volume and acuity of patients increasing (Peberdy, Boze, & Ornato, 2002). Patients with cancer who are treated in ambulatory care have the potential to develop life-threatening complications because of their disease and treatments. Many biologic and chemotherapeutic agents given in the outpatient setting can cause anaphylaxis, requiring immediate interventions to prevent further complications and death (Polovich, Whitford, & Olsen, 2009). Consequently, oncology nurses working in outpatient treatment centers are faced with increasing numbers of patients with higher acuity levels, often receiving medications that cause life-threatening complications. The purpose of this article is to demonstrate how one outpatient cancer center responded to such demands and developed a system to rapidly detect changes in patient status and improve responses to emergencies.

Budd-Chiari syndrome is a rare disease due to occlusion of the hepatic veins often presenting with acute liver failure. Common causes of Budd-Chiari syndrome are chronic myeloproliferative disorders, while acute leukemia has been associated with hepatic vein thrombosis in only two cases in the literature to date. We report a case of Budd-Chiari syndrome complicating a non-promyelocytic acute myelogenous leukemia leading to fulminant hepatic failure.

YNK01 (Starasid) is a prodrug that is adsorbed in the gut and is transformed in the liver in arabinosyl cytosine (AC). Low-dose AC (LDAC) is useful for the treatment of Philadelphia positive (Ph+) chronic myeloid leukemia (CML), especially in combination with alpha-interferon (alphaIFN). The use of YNK01 can avoid the daily s.c. injection of conventional AC. To assess the safety and the efficacy of alphaIFN and YNK01, we enrolled 86 consecutive previously untreated chronic phase Ph+ CML patients in a phase II study of alphaIFN (Intron-A) 5 MIU/m(2) daily and YNK01 600 mg daily 14 days a month. The 6-month complete hematologic response and the 12-month major cytogenetic response rates were 78 and 28%, respectively. In a prior study of alphaIFN and conventional LDAC, they were 62 and 22%, respectively. However, the compliance to the treatment was poor, with 25% of cases discontinuing the treatment within the first year. This was not because of the severity of the side effects but because of the frequency, duration and repetition of the side effects, for an overall frequency of 13.17 adverse events, mostly grade 1 and 2, per patient per year. Therefore, the study of this effective combination is being pursued, testing lower doses of alphaIFN and YNK01.

The authors report a series of conjunctival provocation tests in a single allergic patient indicating that tears collected during the late phase of a conjunctival reaction induced by allergen challenge, can act as inducers of a new late-phase reaction when applied again to the eye of the same patient. This short report is aimed at stimulating more extensive investigations to confirm the authors' observation and to prove the hypothesis here suggested that soluble mediators present in tears during the late-phase conjunctival reaction can act as inducers of a de novo recruitment of inflammatory cells.

The purpose of the study is to evaluate in a double-blind, randomized study the efficacy of lodoxamide tromethamine 0.1% versus placebo.Signs and symptoms, tear tryptase, and tear fluid cytology were evaluated in 20 asymptomatic subjects with allergic conjunctivitis. The study included three allergen challenges in skin test-positive patients. At the first visit, a threshold dose of allergen was established. At the second visit, a bilateral ocular challenge was performed without pretreatment. At the third visit, either lodoxamide or placebo eye drops were used for 1 week before ocular challenge.Lodoxamide significantly reduced tryptase levels (P0.01), neutrophils (P0.04), and eosinophils (P0.01) in the tear fluid and significantly inhibited ocular itching (P0.02) when compared with that of placebo.Lodoxamide is effective in reducing tryptase levels and the recruitment of inflammatory cells in the tear fluid after allergen challenge.

We measured tryptase, a neutral protease stored in the secretory granules of mast cells, by solid-phase radioimmunoassay in tears of 12 subjects with vernal keratoconjunctivitis (VKC) during remission phases, nine subjects with seasonal or perennial allergic conjunctivitis, and eight healthy controls. Mean values of tear tryptase levels were significantly (P < 0.02) increased in VKC patients (14.5 +/- 13 micrograms/l) when compared to those measured in patients with seasonal or perennial allergic conjunctivitis (0.6 +/- 0.1 microgram/l) and in controls (3.3 +/- 3.2 micrograms/l). In subjects with allergic conjunctivitis, the levels of tryptase, almost undetectable before allergen conjunctival challenge, showed a significant increase in the challenged eye 20 min-but not 6 h-after provocation in 5/9 cases. Our results indicate that VKC a severe ocular disease characterized by an increased number and abnormal distribution of mast cells in the conjunctiva, also shows elevated levels of tryptase in tears even during remission phases. Evidence of mast-cell activation, as revealed by a significant increase of tryptase levels in tears, in documented during the early-phase reaction, but not during the late-phase reaction, of allergic conjunctivitis patients challenged topically by specific allergen.

We compared the conjunctival responsiveness to histamine diphosphate in patients with vernal conjunctivitis and in healthy control subjects. Fourteen asymptomatic patients with vernal conjunctivitis and 10 healthy volunteers were challenged in one eye with 10 microliters of increasing doses (0.01, 0.05, 0.5, and 1 mg/ml in phosphate-buffered saline) of histamine diphosphate. The contralateral eye was challenged with the diluent only. Photographs of each eye were taken for evaluation of conjunctival redness by two masked investigators. Both patients with vernal conjunctivitis and control subjects reacted to histamine with a dose-dependent conjunctival redness 2 to 5 minutes after ocular challenge. Conjunctival redness was more intense in patients with vernal conjunctivitis than in control subjects after ocular challenge with 0.01 and 0.05 mg/ml of histamine diphosphate solution (p less than 0.05). Moreover, the threshold concentration of histamine diphosphate, extrapolated from each individual dose-response curve, was significantly (p less than 0.02) lower in patients with vernal conjunctivitis than in control subjects. Our findings suggest that patients with vernal conjunctivitis demonstrate conjunctival hyperresponsiveness to a nonspecific challenging agent. Nonspecific conjunctival hyperreactivity, a novel concept in allergic eye disease, may be relevant for a better understanding of the pathogenesis and clinical variability of vernal conjunctivitis.

Three cases of secondary gastric lymphoma are presented in which diagnosis was suggested by ultrasound (US) and confirmed by endoscopy and microscopical examination. Three different US patterns are illustrated and compared with endoscopy. US findings paralleled endoscopy during follow-up under antiblastic treatment: both improvement and lack of change in the gastric lesions were reliably predicted by US.

Introduction Repair of vascular lesions can be particularly difficult in complex post-traumatic injuries of the extremities due to subversion of the anatomic planes and unexpected anatomical anomalies. Report We wish to report a case of complex post-traumatic injury of the upper extremities, in the presence of early brachial artery branching. A thorough examination of the wound revealed two proximal vascular stumps at inferior third of the arm level. Arterial continuity was reconstructed using two segments of reversed GSV to restore the damaged arteries' length. Discussion In complex extremity traumas awareness of anatomical vascular anomalies is fundamental in planning an appropriate and durable vascular reconstruction.

To evaluate the efficacy of Mipragoside, a ganglioside derivative, in vernal keratoconjunctivitis we performed a controlled randomised clinical trial involving 24 patients (mean age 10 +/- 3.4 years, range 5-20 years). Patients received either Mipragoside 0.5% aqueous ophthalmic gel or placebo four times a day for 2 weeks after a week of treatment with placebo. Ocular signs and symptoms were evaluated and considered for statistical analysis. Results show that Mipragoside significantly reduces all symptoms, being most effective on itching (p = 0.01; p = 0.0001) and hyperaemia (p = 0.01; p = 0.0006) after 1 and 2 weeks respectively when compared with placebo. Physician judgement of drug efficacy at the end of treatment was significantly in favour of Mipragoside (p = 0.0001) compared with placebo. We conclude that Mipragoside topical treatment improves symptoms of patients with vernal keratoconjunctivitis and we postulate a possible anti-inflammatory activity of this compound.
