2. Somatostatin-positive Interneurons Contribute to Seizures inSCN8AEpileptic Encephalopathy

SCN8Aepileptic encephalopathy is a devastating epilepsy syndrome caused by mutantSCN8Awhich encodes the voltage-gated sodium channel NaV1.6. To date, it is unclear if and how inhibitory interneurons, which express NaV1.6, influence disease pathology. We found that selective expression of the R1872W mutation in somatostatin (SST) interneurons was sufficient to convey susceptibility to audiogenic seizures. SST interneurons from mutant mice were hyperexcitable but hypersensitive to action potential failure via depolarization block under normal and seizure-like conditions. Remarkably, GqDREADD-mediated activation of wild-type SST interneurons resulted in prolonged electrographic seizures and was accompanied by SST hyperexcitability and depolarization block. Aberrantly large persistent sodium currents, a hallmark ofSCN8Amutations, were observed and were found to contribute directly to aberrant SST physiology in computational and pharmacological experiments. These novel findings demonstrate a critical and previously unidentified contribution of SST interneurons to seizure generation not only inSCN8Aencephalopathy, but epilepsy in general.

Resume But Evaluer retrospectivement la capacite des differentes methodes d’evaluation de la taille des calculs urinaires, par tomodensitometrie, a predire le succes de l’ureteroscopie souple (URSS). Materiel Soixante-sept patients admis pour premier traitement endoscopique d’au moins un calcul renal mis en evidence par tomodensitometrie abdominale ont ete revus. A ete etudiee la relation entre l’absence de fragment residuel apres URSS et la charge lithiasique evaluee selon : le diametre maximal (DM), la surface calculee (SC), le volume calcule (VC1 et VC2), le diametre cumule (DC), les volumes tridimensionnels total (V3Dt) et maximal (V3Dm) ; au moyen de courbes ROC et par regression logistique uni- et multivariee. Resultats Toutes les mesures radiologiques etudiees predisent efficacement l’absence de fragment residuel significatif apres URSS. Aucune difference significative n’a ete observee entre les aires sous la courbe (AUC) correspondant aux differentes mesures radiologiques. Conclusion L’evaluation de la taille des calculs urinaires est essentielle dans l’estimation de la probabilite de succes d’une URSS. Notre etude rapporte une capacite comparable des differentes mesures tomodensitometriques disponibles, qu’elles soient planaires ou volumetriques. Si les mesures volumetriques semblent plus precises et plus reproductibles, la mesure du diametre maximal reste la plus simple d’utilisation et la plus facilement accessible dans la pratique clinique quotidienne. Les tumeurs endocrines fonctionnelles du pancréas sont des lésions rares, parmi lesquelles les insulinomes sont les plus fréquents. La majorité des patients atteints d'insulinome ont entre 30 et 60 ans et 59% d'entre eux sont des femmes. La plupart des insulinomes sont sporadiques, alors que 10% sont développés chez des patients atteints de NEM. Le diagnostic repose sur la clinique et sur le bilan biologique. Le bilan d'imagerie préopératoire (Echo-endoscopie, TDM, IRM) est essentiel localisant la tumeur dans plus de 80% des cas. Le traitement de référence est l'exérèse chirurgicale de la tumeur. Nous rapportons ici un cas d'insulinome de haut grade développé sur la face antérieure de la queue du pancréas chez une patiente de 50 ans.

5. Un accès psychotique aigu révélateur d’un neurolupus avec images atypiques à l’IRM encéphalique

Resume La frequence des manifestations neuropsychiatriques lors du lupus erythemateux dissemine varie selon les series entre 14 et 75 %. L’imagerie par resonance magnetique reste l’examen de choix et les lesions encephaliques sont dominees par des hypersignaux punctiformes de la substance blanche. Nous rapportons le cas d’une patiente de 17 ans sans antecedent particulier qui a presente brutalement un acces psychotique aigu avec fievre. L’examen neurologique a objective un syndrome pyramidal. L’IRM encephalique a revele des hypersignaux diffus de la substance blanche et grise, bilateraux et symetriques. Le diagnostic de neurolupus a ete retenu devant l’association des troubles neuropsychiatriques, une anemie, une lymphopenie, une pleuresie et des anticorps antinucleaires positifs. Le traitement etait a base de bolus de methylprednisolone puis relais par une corticotherapie orale. La patiente est decedee un mois et demi apres le debut de sa maladie. Le diagnostic de neurolupus doit etre evoque systematiquement devant un tableau neuropsychiatrique atypique surtout devant des anomalies minimes du bilan biologique ou de l’imagerie cerebrale.

The present study aimed at verifying the impact of a Moroccan strategy against scorpion stings and, specifically, at identifying the epidemiological features of the patients envenomed or just stung by scorpions. The investigation included 4089 patients from a province of Morocco which were evaluated over three years (2001, 2002 and 2003). Most stings occurred during the hot period and mainly at night (between 6:00 p.m. and 12:00 p.m.). The average incidence was 2.8‰, the average age of the patients was 26.7±18.2 years, and the envenomation rate was 6.7%. Mortality rate was 0.05‰, and average lethality rate was 0.7%. Analysis of variance showed that young age, symptoms at admission, and long time elapsed between sting and admission were correlated with poor outcome. Comparison among data of the three years revealed an increasing number of reported cases and decreasing morbidity and mortality.

Resume Objectif Tester l'efficacite d'une faible dose d'etomidate, administree 60 secondes avant l'induction de l'anesthesie, sur la prevention des myoclonies liees a l'etomidate. Type d'etude Etude prospective randomisee en double insu. Methodes Inclusion de 46 patients ASA I et II prevus pour une chirurgie digestive sous anesthesie generale randomises en deux groupes : groupe « priming dose » a recu une faible dose d'etomidate (0,03 mg/kg) avant l'induction de l'anesthesie et un groupe « temoin » a recu du serum physiologique. Soixante secondes apres administration de la priming dose ou du placebo, l'induction de l'anesthesie a ete realisee par etomidate 0,3 mg/kg. Puis 60 secondes plus tard, celle-ci a ete completee par fentanyl (3 μg/kg) et vecuronium (0,1 mg/kg). La survenue des myoclonies a ete evaluee par une echelle de 0 a 3 (0 : pas de myoclonies ; 1 : myoclonies legeres ; 2 : moderees et 3 : severes). Analyse statistique : test de Fisher exact pour les variables qualitatives et test T de Student pour les variables quantitatives. Resultats Les caracteristiques demographiques des deux groupes etaient comparables. De meme, les doses d'etomidate utilisees a l'induction pour chaque groupe etaient comparables (0,29 ± 0,032 mg/kg pour le groupe priming dose vs 0,30 ± 0,029 mg/kg pour le groupe temoin) ( p = 0,27). En revanche, l'incidence des myoclonies etait significativement plus basse dans le groupe priming (6/23 patients) comparativement au groupe temoin (20/23 patients) ( p Conclusion L'administration d'une priming dose d'etomidate, 60 secondes avant l'induction, est efficace pour reduire les myoclonies induites par l'etomidate.

We present a report of a patient with an atypic palmar rotation dislocation of the scaphoid associated with the multiple fractures of carpus (fracture of the trapezium and the trapzoideum, fracture of the hamatum, fracture of radial styloid, and subluxation of the pisiform. Injury was treated surgically as a matter of urgency with a 32 months follow-up. This dislocation takes palce during an industrial accident of the right hand factory employee. It involves an atypical pathological lesion (palmar and rotatory dislocation of scaphoid), and also atypical mechanism (dorso-palmar loading associeted with prono-supination torsion). Therapeutic attitude and the evolution will be discussed in comparison with literature. Management of anterior dislocations of the carpus is not well codified and the majority of the authors recommend an anterior approach, a direct (posterior) approach on the scapho-lunar was used to verify the reduction, suture the scapho-lunar ligament, and temporary Kirschner- wire fixation was used to ensure maintenance of reduction.

We report the case of a 24-year old man with a past history of vesical extrophy and ureterosigmoidostomy in childhood, admitted with acute left flank pain and acute renal failure. The complaints started the day following the endoscopic resection of a sigmoid polyp. The stricture of the ureterosigmoid junction was diagnosed by intravenous urography. Conservative treatment with endo-ureteral dilatation was successfully performed. At 2 years of follow-up, the patient is still asymptomatic, without any residual hydronephrosis.
