Free-living amoebae (FLA) are prevalent in diverse environments, representing various genera and species with different pathogenicity. FLA-induced infections, such as the highly fatal amoebic encephalitis, with a mortality rate of 99%, primarily affect immunocompromised individuals while others such as Acanthamoeba keratitis (AK) and cutaneous amebiasis may affect immunocompetent individuals. Despite the prevalence of FLA, there is a lack of standardized guidelines for their detection near human habitats. To date, no studies on the isolation and identification of FLA in environmental soil samples in Warsaw have been published. The aim of this study was to determine the presence of amoebae in soil samples collected from Warsaw parks and squares frequented by humans. The isolated protozoa were genotyped. Additionally, their pathogenic potential was determined through thermophilicity tests. A total of 23 soil samples were seeded on non-nutrient agar plates (NNA) at 26 °C and monitored daily for FLA presence. From the total of 23 samples, 18 were positive for FLA growth in NNA and PCR (78.2%). Acanthamoeba spp. was the most frequently isolated genus, with a total of 13 positive samples (13/18; 72.2%), and the T4 genotype being the most common. Moreover, Platyamoeba placida (3/18; 16.7%), Stenamoeba berchidia (1/18; 5.6%) and Allovahlkampfia sp. (1/18; 5.6%), also potentially pathogenic amoebae, were isolated. To our knowledge, this is the first report of FLA presence and characterization in the Warsaw area.

Abstract Allergies to grass pollen affects about 20% of the population worldwide. In the last few decades, the South American grass Cortaderia selloana (CS, Pampas grass) has expanded worldwide in a variety of countries including the USA, Australia and Western Europe. In many of these locations, CS has strikingly spread and has now been classified an invasive species. Many pernicious consequences of CS have been reported for local biodiversity, landscape and structures. However, the effect on human health has not been studied. To investigate this issue, we have chosen a European region on the northern cost of Spain where CS spread is overwhelming, Cantabria. We obtained CS pollen extract and analysed the allergenic reaction of 98 patients that were allergic to pollen of local grasses. We determined the skin reaction and the presence of specific IgE antibodies (sIgE) to CS or to a typical autochthonous grass, Phleum pratense. We also compared the seasonal symptoms with reported grass pollen counts in the area. The results strongly suggest that CS can cause respiratory allergies at a similar extent to the local grasses. Given that CS pollinises later than the local grasses, this would extend the period of grass allergies in the region for about three months every year, as stated by most of the patients. This is the first study reported on the effects of the striking expansion of CS on human health. Considering the strong impact that respiratory allergies have on the population, our results suggest that CS can currently constitute a relevant environmental health issue.

Objetivo: Diversos estudios realizados sobre el linfoma de Hodgkin (LH) han determinado la expresión de la proteína de muerte programada (PD-L1) en las células de Reed-Sternberg-Hodgkin (HRS), con resultados variables. Esta proteína ha adquirido relevancia por su papel en la respuesta inmunitaria en las neoplasias malignas. El objetivo de este trabajo fue determinar la expresión inmunohistoquímica de PD-L1 en las HRS y su relación con la edad, sexo, estadio clínico y supervivencia global en una cohorte de pacientes peruanos. Materiales y métodos: Se evaluaron 25 biopsias de pacientes con diagnóstico de LH, lo que permitió determinar la expresión inmunohistoquímica de PD-L1 en las HRS en asociación con la sobrevida global (SG) y los datos clínicos de los pacientes. Resultados: Todos los casos mostraron expresión de PD-L1 en más del 1 %. No hubo diferencia estadística significativa en la SG cuando se comparó dos grupos de acuerdo con la expresión de PD-L1 con punto de corte en 50 %, ni tampoco con el estadio clínico (EC), la edad y el sexo. Conclusiones: Se ha encontrado una alta expresión de PD-L1 en los tumores con LH previos al tratamiento. No se ha encontrado asociación entre la expresión de PD-L1, la SG, la edad, el sexo o el EC. Son necesarios otros estudios con mayor cantidad de pacientes para reevaluar el impacto pronóstico de la expresión de esta proteína en LH.

Abstract We report a patient with mucopolysaccharidosis type VI, on long‐term enzyme replacement home therapy. Results support the efficacy and safety benefits, with additional advantage of home therapy to minimize the risk of community‐transmitted infections.

Introduction: Kidney biopsy is one of the essential tests for the characterization of chronic kidney disease (CKD). Objective: To determine the most frequent primary and secondary glomerulonephritis, complications of renal biopsy and the number of patients on hemodialysis when undergoing renal biopsy. Methods: Observational, descriptive, case series, retrospective and cross-sectional study, carried out at the E. Rebagliati Matins-Peru national hospital. The medical records of patients older than 15 years of age who underwent kidney biopsy between 2008-2016 were reviewed. The data were processed in Stata 16.1 Results: Of 2278 medical records, they corresponded to 1234 women and 1044 men. The most frequent primary and secondary glomerulonephritis were focal and segmental glomerulosclerosis (FSGS) 683 (29.98%) and systemic lupus erythematosus (SLE). ) 434 (19.04%) respectively. Of 1644 biopsied patients, 36 (2.19%) and 241 (14.66%) had major and minor complications respectively. Major complications were more frequent when the procedure was performed by radiology with ultrasound guidance compared to that performed by the nephrologist without ultrasound guidance (p = 0.03694). In 1,324 patients, 329 underwent kidney biopsy while on hemodialysis; of them 43 (13.07%) managed to get out of this therapy and 144 (51.99%) had major or minor complications. Conclusions: GEFyS and SLE were the most frequent primary and secondary glomerulonephritis, respectively. In 1644 patients the major and minor complications were 36 (2.19%) and 241 (14.66%) respectively. In 1,324 patients, 329 (24.8%) were biopsied while on hemodialysis and 144 (51.99%) of them had major or minor complications.

Introducción: La insuficiencia renal es una de las complicaciones extrapulmonares más frecuente en pacientes hospitalizados con COVID-19 condicionando peores desenlaces que podría afectar al sistema de salud en Perú. Sin embargo, estudios comparan pacientes con insuficiencia renal aguda (IRA) o crónica (ERC) con pacientes sanos. Objetivo: Determinar características clínicas de pacientes con COVID-19 e insuficiencia renal hospitalizados y evaluar el efecto del tipo de insuficiencia renal y el recibir hemodiálisis en los desenlaces clínicos negativos. Métodos:Cohorte descriptiva incluyó pacientes con algún tipo de insuficiencia renal y COVID-19 hospitalizados durante marzo y julio del 2020, que tuvieron una interconsulta con nefrología. La insuficiencia renal se clasificó como aguda, crónica, y crónica en estadio V con hemodiálisis crónica. Se recolectó información sobre mortalidad, uso de inotrópicos, ventilación mecánica y recibir hemodiálisis aguda. Resultados: Se analizó a 279 pacientes, 22.6% tenían IRA, 33.3% tenían ERC, y 44.1% tenían ERC V. Se describe una mortalidad general de 32.9%, y 27% usaron inotrópicos y recibieron ventilación mecánica. Entre los pacientes con IRA y ERC el 12.9% recibió hemodiálisis por primera vez. Los adultos con ERC y ERC V en HD estudiados tienen menor prevalencia de diabetes mellitus (23.7.3% y 43.9%, respectivamente) e hipertensión arterial (31.2% y 59.4%, respectivamente) en comparación con los adultos que desarrollan IRA (81.0% y 73%, respectivamente) (valor p

In response to the COVID-19 pandemic, COVID-19 vaccines have been developed, and the World Health Oraganization (WHO) has granted emergency use listing to multiple vaccines. Studies of vaccine immunogenicity data from implementing COVID-19 vaccines by national immunization programs in single studies spanning multiple countries and continents are limited but critically needed to answer public health questions on vaccines, such as comparing immune responses to different vaccines and among different populations.

Reporte de un caso de mielopatía asociada (HAM) a virus humano linfotrofico de células T tipo 1 (HTLV1) que ocurre en paciente mujer de 60 años con enfermedad renal crónica en hemodiálisis, además, con antecedente de diabetes mellitus 2. Paciente residente de Lima, Perú, zona endémica de HTLV-11,2,3. El cuadro clínico se caracterizó por síntomas y signos neurológicos de inicio abrupto y evolución rápida: paraparesia con imposibilidad de caminar y vejiga neurogenica con sonda vesical permanente. En la región, se reporta prevalencia de HTLV-1 en pacientes en hemodiálisis similar a la de hepatitis B y C4,5,6. No existen cifras de esta infección en el Perú. Además, no se realiza despistaje en donantes y/o receptores de trasplante renal.

Primary amoebic encephalitis (PAM) caused by the opportunistic pathogen Naegleria fowleri is characterized as a rapid and lethal infection of the brain which ends in the death of the patient in more than 90% of the reported cases. This amoeba thrives in warm water bodies and causes infection after individuals perform risky activities such as splashing or diving, mostly in non-treated water bodies such as lakes and ponds. Moreover, the infection progresses very fast and no fully effective molecules have currently been found to treat PAM. In this study, naphthyridines fused with chromenes or chromenones previously synthetized by the group were tested in vitro against the trophozoite stage of two strains of N. fowleri. In addition, the most active molecule was evaluated in order to check the induction of programmed cell death (PCD) in the treated amoebae. Compound 3 showed good anti-Naegleria activity (61.45 ± 5.27 and 76.61 ± 10.84 µM, respectively) against the two different strains (ATCC® 30808 and ATCC® 30215) and a good selectivity compared to the cytotoxicity values (>300 µM). In addition, it was able to induce PCD, causing DNA condensation, damage at the cellular membrane, reduction in mitochondrial membrane potential and ATP levels, and ROS generation. Hence, naphthyridines fused with chromenes or chromenones could be potential therapeutic agents against PAM in the near future.

The need for speeding up the diagnosis and treatment of sleep-disordered breathing has increased in recent years. In order to achieve that, a split-night protocol has been implemented, in which the patient with suspected sleep-disordered breathing undergoes a diagnostic and therapeutic study in one night. Elshaug et al. (1), by using the split-night protocol, were able to reduce the time to initiate continuous positive airway pressure (CPAP) therapy by 15% in patients with severe obstructive sleep apnea. However, reviewing the limitations of said protocol is necessary to make it more streamline.

Free living, cosmopolitan amoebae from Acanthamoeba genus present a serious risk to human health. As facultative human parasites, these amoebae may cause Acanthamoeba keratitis (AK). Acanthamoeba keratitis is a severe, vision-threatening corneal infection with non-specific symptoms. The number of reported AK cases worldwide has been increasing every year. Moreover, 90% of Acanthamoeba keratitis cases are related to contact lens use. Wearing and storage contact lenses not in accordance with the physicians and manufacturers recommendations are the primary key risk factors of this disease. Amoebae can easily adhere to the contact lens surface and transmit to the corneal epithelium. Preventing amoebae adhesion to the contact lens surface could significantly decrease the number of AK infections. Until now, the effective therapy against AK is still under development. Currently proposed therapies are mainly limited to the chlorhexidine digluconate combined with propamidine isethionate or hexamidine applications, which are insufficient and very toxic to the eye. Due to lack of effective treatment, looking for new potential preventive agents is crucial to decrease the number of Acanthamoeba keratitis infections, especially among contact lens users. Nanoparticles have been already included in several novel therapies against bacteria, viruses, fungi, and protist. However, their anti-amoebic potential has not been fully tested yet. The aim of this study was to assess silver nanoparticles (AgNPs) and platinum nanoparticles (PtNPs) anti-amoebic activity and influence on the amoebae adhesion to the surface of four different groups of contact lenses—classified according to the Food and Drugs Administration (FDA) guidelines. The obtained results show that both tested nanoparticles were effective against Acanthamoeba trophozoites and decreased the amoebae adhesion to the contact lens surface. AgNPs showed better anti-amoebic activity to cytotoxicity dependence and reduced amoebae adhesion in a wider spectrum of the tested contact lenses. Our studies also confirmed that ionization next to hydration of the contact lens material is a crucial parameter influencing the Acanthamoeba adhesion to the contact lens surface. In conclusion, silver nanoparticles might be considered as a novel preventive agent against Acanthamoeba keratitis infection.

Objetivo: Evaluar la supervivencia a largo plazo de pacientes con enfermedad renal crónica estadio 5 (ERC-5) que ingresaron a un programa de hemodiálisis en Lima, Perú y se determinó las variables de pronóstico que afectaron la supervivencia. Materiales y métodos: Se evaluó una cohorte prospectiva de 604 pacientes que ingresaron a un programa de hemodiálisis (1982-2009) y fueron seguidos hasta diciembre del 2015. La supervivencia fue estudiada mediante las curvas de Kaplan Meier; se estableció comparaciones, utilizando la prueba Log-rank según: edad, etiología, trasplante renal y período de ingreso a hemodiálisis. Mediante el análisis de regresión de Cox, se determinó variables pronósticas de supervivencia. Resultados: La edad promedio fue de 51,95 ± 16,17 años. El 43,1% era ≥ 60 años; el 43,4% fueron mujeres. Las etiologías más frecuentes fueron las glomerulopatías primarias (21%), diabetes mellitus (20%), hipertensión arterial (12%) y las nefropatías intersticiales y/o uropatías obstructivas (11%). El porcentaje de supervivencia (IC95%) en los años 1, 5, 10, 15 y 20 fue 80% (intervalo de confianza al 95% [IC 95%]: 77% - 83%), 47% (IC 95%: 43% - 51%), 25% (IC 95%: 21% a 29%); 17% (IC 95%: 13% - 21%) y 11% (IC 955: 8% - 14%), respectivamente. La regresión de Cox identificó al adulto mayor y al diabético como variables desfavorables en la supervivencia y al trasplantado renal como variable favorable. Conclusión: La supervivencia a largo plazo es comparable a otros estudios internacionales. La menor supervivencia de nuestros pacientes se relacionó con la condición de ser adulto mayor, diabético y no tener posibilidad de trasplante renal. Los cambios tecnológicos durante el período de seguimiento no modificaron significativamente las curvas de supervivencia.

Introducción: En el proceso de feminización, los transexuales masculinos usan hormonas sexuales femeninas, cuya seguridad en el sexo genéticamente masculino no ha sido definida. Objetivos: Evaluar el grado de conocimiento y prácticas de la administración de hormonas femeninas, la frecuencia de uso, reacciones adversas, fuentes de información y acceso a estos productos por parte de transexuales femeninos. Diseño: Investigación cualitativa y cuantitativa prospectiva comunitaria. Lugar: Lima. Participantes: Transexuales masculinos de 18 años o más. Intervenciones: Cuatro transexuales capacitados identificaron mediante la modalidad ‘bola de nieve’ a transexuales femeninos. Luego, se les realizó una entrevista semi-estructurada validada, el examen clínico respectivo y la toma de una muestra de 10 mL de sangre venosa. Medidas de resultados: El nivel de conocimiento, redes y calidad de información recibida, lugares de adquisición y efectos de las hormonas femeninas. Resultados: Solo el 11,8% de los participantes conseguía las hormonas con receta médica y poseía conocimiento sobre su efecto. Los principales efectos secundarios presentados fueron sensibilidad mamaria y aumento del peso corporal (88,2%), y disminución de la libido y erección (58,8%). La hormona más usada fue una combinación de dihidroxiprogesterona y estradiol vía intramuscular (47,1%). Conclusiones: Es necesario implementar servicios multidisciplinarios que cubran las necesidades de salud, incluyendo la de feminización que tiene esta población, y así evitar o disminuir el riesgo inherente a este proceso.

Leishmaniasis and American trypanosomiasis are parasitic diseases that cause significant clinical, social and economic impact on the population of tropical and subtropical countries. Their current treatment is limited and presents multiple drawbacks, including high toxicity, high cost, lengthy treatment plans, as well as the emergence of resistant species. Therefore, there is a need to find new lead compounds with high potency against parasites and low toxicity in patients. In the present work, the bioguided fractionation of an endemic plant from the Canary Islands, Withania aristata, led to the identification of withanolide-type metabolites (1−3) with leishmanicidal and trypanocidal activities. Compounds 1 and 3 showed a significant dose-dependent inhibition effect on the proliferation of L. amazonensis promastigotes and T. cruzi epimastigotes, higher than the reference drugs, miltefosine and benznidazole, respectively. Moreover, compounds 1−3 were more potent (IC50 0.055−0.663 µM) than the reference drug against the intracellular amastigote stage of L. amazonensis, with a high selectivity index on murine macrophage cells (SI 58.66−216.73). Studies on the mechanism of death showed that the compounds induced programmed cell death or that which was apoptosis-like. The present findings underline the potential of withanolides as novel therapeutic antikinetoplastid agents.

Naegleria fowleri causes a deadly disease called primary amoebic meningoencephalitis (PAM). Even though PAM is still considered a rare disease, the number of reported cases worldwide has been increasing each year. Among the factors to be considered for this, awareness about this disease, and also global warming, as these amoebae thrive in warm water bodies, seem to be the key factors. Until present, no fully effective drugs have been developed to treat PAM, and the current options are amphotericin B and miltefosine, which present side effects such as liver and kidney toxicity. Statins are able to inhibit the 3-hydroxy-3-methylglutaryl-coenzyme A (HMG-CoA) reductase, which is a key enzyme for the synthesis of ergosterol of the cell membrane of these amoebae. Therefore, the in vitro activity of a group of statins was tested in this study against two types of strains of Naegleria fowleri. The obtained results showed that fluvastatin was the most effective statin tested in this study and was able to eliminate these amoebae at concentrations of 0.179 ± 0.078 to 1.682 ± 0.775 µM depending on the tested strain of N. fowleri. Therefore, fluvastatin could be a potential novel therapeutic agent against this emerging pathogen.

The current chemotherapy of Acanthamoeba keratitis relies on few drugs with low potential and limited efficacy, for all this there is an urgent need to identify new classes of anti-Acanthamoeba agents. In this regard, natural products play an important role in overcoming the current need and medicinal chemistry of natural products represents an attractive approach for the discovery and development of new agents. Ursolic acid, a natural pentacyclic triterpenoid compound, possesses a broad spectrum of activities including anti-Acanthamoeba. Herein, we report on the development by chemical transformation of an ursolic acid-based series of seven compounds (2−8), one of them reported for the first time. The structure-activity relationship (SAR) analysis of their anti-Acanthamoeba activity revealed that acylation/ether formation or oxidation enhances their biological profile, suggesting that the hydrophobic moiety contributes to activity, presumably by increasing the affinity and/or cell membrane permeability. These ursolic acid derivatives highlight the potential of this source as a good base for the development of novel therapeutic agents against Acanthamoeba infections.

Acanthamoeba is a free-living amoebae genus which is present worldwide in natural and artificial environments. These amoebae are clinically important as causative agents of diseases in humans and other animals such as a fatal encephalitis or a sight threatening Acanthamoeba keratitis (AK). Lately; studies have focused on the search of novel therapeutic options for AK but also to prevent infections. Furthermore; the evaluation of commercialized products seems to be an option for this case since not clinical assays would be required. Thus; we aimed to test the amoebicidal activity of different mixtures of two commercial ophthalmic solutions: Systane® Ultra; which has already shown anti-Acanthamoeba properties; and Naviblef® Daily Care. In addition, we tested their cytotoxic effect against murine macrophages. At the individual level; Naviblef® Daily Care showed to be the most active product against Acanthamoeba spp. Nevertheless; the combinations of Systane® Ultra and Naviblef® Daily Care; showed an improvement in the activity against trophozoites and cysts of Acanthamoeba castellanii Neff. Moreover; the concentration necessary to generate cytotoxic effect against murine macrophages (J774.1) was much higher than the required for the amoebicidal and cysticidal effect achieved in the most effective mixtures.

Estudio que evaluó el conocimiento de 182 estudiantes de los dos últimos años de medicina y 70 residentes de un hospital nacional de Perú, sobre los indicadores de riesgo utilizados en la presentación de resultados en ensayos clínicos. Se realizó un estudio transversal en el que se aplicó un cuestionario que evaluó la capacidad de reconocer y calcular los indicadores de riesgo epidemiológico más utilizados en la literatura médica. El 19,4% no reconoció ninguno de los indicadores y el 81,4% no logró calcularlos. La reducción de riesgo relativo fue el indicador más reconocido (55,2%), seguida del número necesario a tratar (51,6%); reducción de riesgo absoluto (26,6%), y hazard ratio (9,5%). En conclusión, los alumnos de los dos últimos años de medicina y los residentes, no reconocen ni calculan adecuadamente los indicadores de riesgo utilizados en ensayos clínicos.A cross-sectional study evaluated 182 students in the last two years of medical school and 70 residents of a national hospital in Peru on the risk indicators used for reporting results in clinical trials. A questionnaire was used to assess the ability to recognize and calculate risk indicators most widely used in the epidemiological literature. From the participants, 19.4% did not recognize any of the indicators and 81.4% was not able to calculate them. The relative risk reduction was the most recognized indicator (55.2%), followed by the number needed to treat (51.6%), the absolute risk reduction (26.6%), and the hazard ratio (9.5%). In conclusion, medical students in the last two years of school and medical residents do not recognize or are able to calculate properly the risk indicators used in clinical trials.

La ingestión de cuerpos extraños esofágicos es una urgencia frecuente. Es habitual en pacientes psiquiátricos, presos y edades extremas de la vida. La endoscopía digestiva alta realizada de forma precoz es el tratamiento de elección en los pacientes sintomáticos. La imposibilidad de extracción del cuerpo extraño o la presencia de una complicación implican la necesidad de un tratamiento quirúrgico. Presentamos el caso de un varón de 34 años que ingiere de forma voluntaria un cuerpoextraño poco habitual. La impactación del cuerpo extraño tras las maniobras endoscópicas y la sospecha deperforación obligaron a realizar un tratamiento quirúrgico para su extracción.

Los recursos humanos son el eje del accionar del sector salud; sin embargo, no necesariamente son el aspecto mejor atendido, por ello el Ministerio de Salud del Perú (MINSA) conjuntamente con los gobiernos regionales generó el Plan Sectorial Concertado y Descentralizado para el Desarrollo de Capacidades en Salud 2010-2014 (PLANSALUD) con el propósito de fortalecer las capacidades de los Recursos Humanos en Salud (RHUS) y contribuir para que la atención de salud se desarrolle con eficiencia, calidad, pertinencia, equidad e interculturalidad en el marco de la descentralización, el Aseguramiento Universal de la Salud (AUS) y las políticas de la salud. Con ese objeto se han propuesto tres componentes (asistencia técnica, capacitación y articulación educación - salud) que agrupan a un conjunto importante de intervenciones, las cuales son planteadas y definidas de acuerdo al contexto nacional, regional y local, contribuyendo de ese modo a la mejora de las capacidades de gobierno, de gestión por competencias y la prestación de servicios de salud. El presente artículo muestra una primera aproximación de PLANSALUD, incluyendo aspectos relacionados a su planificación, gestión, financiamiento, estructura y funcionamiento, así como las medidas de monitoreo y evaluación.Human resources are the backbone of health sector actions; however, they are not necessarily the area with the greatest attention, therefore, the Ministry of Health of Peru (MINSA) together with regional governments, led the Decentralized and Agreed Sector Plan for the Capacity Development in Health 2010-2014 (PLANSALUD) with the aim of strengthening the capacities of Human Resources for Health (HRH) and contribute to health care efficient development, quality, relevance, equity and multiculturalism, in the context of descentralization, the Universal Health Insurance (AUS) and health policies. To achieve this goal, they have proposed three components (technical assistance, joint training and education - health articulation) that bring together an important set of interventions, which are planned and defined according to the national, regional and local levels, thus contributing to improve the government capacity, capability management and delivery of health services. This paper presents a first approach of PLANSALUD, including aspects related to planning, management, financing, structure and functioning, as well as monitoring and evaluation measures.

Objetivos. Comparar los niveles de cansancio, somnolencia y sus repercusiones entre conductores formales e informales de ómnibus interprovinciales. Evaluar las condiciones laborales en ambos grupos de estudio. Materiales y métodos. Se realizó un estudio transversal comparativo con muestreo no probabilístico. Se incluyó 100 empresas de transporte terrestre, de las cuales 17 fueron formales según registros oficiales del Ministerio de Transportes y Comunicaciones (MTC), asimismo, los conductores se catalogaron como formales o informales. La encuesta incluía un cuestionario y la versión peruana validada de la escala de somnolencia de Epworth. Resultados. Participaron 71 conductores formales y 274 informales, todos fueron varones. De 134 conductores que pertenecían a las empresas formales de acuerdo al MTC, sólo 43 (32%) pertenecen al grupo formal en base a los criterios propuestos. El 48% (34) de los conductores formales y el 43% (118) de los informales duermen menos de siete horas al día. Admitieron haberse accidentado o "casi accidentado" el 48% (34) de los formales y 135 (49%) informales; el horario más frecuente fue entre la 01.00 y 04.00 horas. La madrugada es el período en que ambos grupos sienten más cansancio. El 44% (30) de los conductores formales y el 54% (144) de informales realizan cinco o más turnos nocturnos por semana. Del total de los entrevistados, el 16% (56) presentaron somnolencia. La asociación con accidentes de tránsito fue similar. Conclusiones. Los niveles de cansancio y somnolencia fueron similares entre conductores formales e informales. Aquellas empresas catalogadas como formales, presentan alto porcentaje de informalidad entre sus conductores.Objectives. To compare the levels of fatigue, sleepiness and their consequences between formal and informal drivers of interprovincial buses. To evaluate labor conditions between both study groups. Materials and methods. A comparative cross-sectional study was performed with non-probabilistic sampling. 100 companies of land transport were included, out of which 17 were formal according to the official registries of the Ministry of Transport and Communications (MTC), the drivers were also classified as formal or informal. The survey included one questionnaire and a Peruvian validated version of the Epworth sleepiness scale. Results. 71 formal drivers and 274 informal drivers participated, all were males. Out of the 134 drivers that worked for the formal companies according to the MTC, only 43 (32%) belong to the formal group based on the proposed criteria. 48% (34) of the formal drivers and 43% (118) of the informal sleep less than 7 hours a day. 48% (34) of the formal and 49% (135) of the informal admitted having had an accident or "almost" having had it, the most frequent time of the day was between 01.00 and 04.00 in the morning. The dawn is the period in which both groups feel most tired. 44% (30) of the formal drivers and 54% (144) of the informal ones perform 5 or more night shifts per week. Out of the total of interviewed, 16% (56) had sleepiness. The association with road traffic accidents was similar. Conclusions. The fatigue and sleepiness levels were similar between formal and informal drivers. Companies classified as formal, have a high percentage of informality amongst their drivers.

Objetivo: Describir las características clínicas y epidemiológicas de la epilepsia-ausencias en niños en el Hospital Nacional Cayetano Heredia (HNCH). Material y Métodos: Estudio descriptivo-retrospectivo. Se identificaron los pacientes con diagnóstico de ausencias del Servicio de Neuropediatría atendidos desde enero de 1998 hasta diciembre del 2002. Criterios de inclusión: manifestaciones clínicas de ausencias y Electroencefalograma característico. Exclusión: pacientes con lesiones cerebrales. Resultados: Se incluyeron 33 pacientes (6.4% del total). La edad promedio al inicio de enfermedad fue 6.6 años, sexo femenino: 54.5% y tiempo de enfermedad antes del diagnóstico: 12.5 meses. Presentaron antecedente familiar de convulsiones 30.3% y antecedente de eventos perinatales adversos 18.2%. El 39.4% de pacientes presentó ausencias simples y 60.6% ausencias complejas. El 36.4% tuvo inicialmente un patrón electroencefalográfico bilateral asimétrico que posteriormente se tornó simétrico. Todos recibieron ácido valproico pero en 6% se tuvo que cambiar de medicación debido a efectos adversos y 12% tuvieron que recibir terapia combinada por falta de control. Edad, sexo, tipo de crisis, EEG, antecedentes familiares o perinatales, no influyeron en las características clínicas, electroencefalográficas o respuesta terapéutica. Conclusión: Las ausencias representan 6.4% de todas las epilepsias. La edad promedio es de 6.6 años, los antecedentes familiares están presentes en 30.3% de los pacientes y los eventos adversos perinatales en 18.2%. Las ausencias complejas con automatismos son las más frecuentes y el ácido valproico es un tratamiento efectivo. El tiempo de enfermedad antes del diagnóstico fue 12.5 meses por lo cual se recomienda mayor difusión a nivel de padres, educadores y personal de salud para diagnóstico y control oportunos. Objetivo: Revisar las variaciones del medio interno y morbimortalidad en DP aguda no convencional en pacientes con insuficiencia renal que requieren soporte dialítico. Material y Métodos: Se trata de un estudio de serie de casos prospectivo y analítico realizado en el Hospital Nacional Cayetano Heredia (HNCH) sobre diálisis peritoneal (DP) aguda no convencional. Se incluyeron pacientes con insuficiencia renal con criterios de diálisis de urgencia o emergencia enrolados en el HNCH, a quienes se les sometió a DP aguda no convencional, entendida como el uso de un número de recambios de bolsas de DP menor a los 24 recambios diarios de la DP aguda convencional. Resultados: Se analizaron 30 pacientes, obteniéndose una tasa de reducción (TR) de urea 34.44 ± 24.85%, Kt/V de la DP 0.20 ± 0.24, variación (antes y después) de potasio -1.24 ± 0.9 mEq/L, variación de HCO3 +8.30 ± 4.57 mEq/L. La TR de urea y Kt/V del procedimiento fueron semejantes a la DP convencional. Además hubo normalización de K, HCO3 y pH séricos. Se usaron 21.26 ± 6.85 bolsas vs 60-120 del procedimiento convencional. Se presentó sangrado por el catéter de DP en 7 pacientes (23.3%) y no hubo peritonitis asociada a DP. No se presentó mortalidad asociada al procedimiento. Conclusiones: las variaciones en el medio interno en la DP no convencional son similares a la DP convencional. La morbilidad estuvo dada por complicaciones de colocación del catéter de DP. No hubo peritonitis ni mortalidad asociada. (Rev Med Hered 2003; 14:12-17).

El sueño hace parte de nuestras vidas y es una necesidad fisiológica que cumple con funciones regeneradoras, reparatorias y de consolidación de algunas funciones cognitivas intelectivas como la memoria, al decir de recientes teorías explicatorias. Los seres humanos pasamos alrededor de 25 años durmiendo, sin embargo, sólo hasta hace algunas décadas se empieza a dilucidar las alteraciones ligadas al hecho de dormir, siendo los trastornos respiratorios del sueño, desde el ronquido simple, pasando por el síndrome de resistencia aumentada de la vía aérea, la apnea obstructiva posicional y la dependiente del sueño R, así como el síndrome de apnea e hipopnea obstructiva del sueño (SAHOS), la apnea central, la apnea mixta y la recientemente denominada apnea compleja, constituyen un espectro muy amplio de esta patología. El SAHOS cada vez más conocido por la comunidad médica, diagnosticado y tratado adecuadamente, constituye un factor de riesgo para otras patologías muy prevalentes en la población general, como son las enfermedades cerebrovasculares y cardiovasculares.

An outbreak of Q fever occurred in February-April 2014 among workers at a waste-sorting plant in Bilbao (Spain). The outbreak affected 58.5% of investigated employees, 47.2% as confirmed cases (PCR and/or serology) and 11.3% as probable cases (symptoms without laboratory confirmation). Only employees who had no-access to the waste processing areas of the plant were not affected and incidence of infection was significantly higher among workers not using respiratory protection masks. Detection by qPCR of Coxiella burnetii in dust collected from surfaces of the plant facilities confirmed exposure of workers inside the plant. Animal remains sporadically detected among the residues received for waste-sorting were the most probable source of infection. After cleaning and disinfection, all environmental samples tested negative. Personal protection measures were reinforced and made compulsory for the staff and actions were taken to raise farmers' awareness of the biological risk of discharging animal carcasses as urban waste.
