Multicenter collaborative networks are essential for advancing research and improving clinical care for a variety of conditions. Research networks are particularly important for central nervous system infections, which remain difficult to study due to their sporadic occurrence and requirement for collection and testing of cerebrospinal fluid. Establishment of long-term research networks in resource-limited areas also facilitates diagnostic capacity building, surveillance for emerging pathogens, and provision of appropriate treatment where needed. We review our experience developing a research network for encephalitis among twelve hospitals in five Peruvian cities since 2009. We provide practical suggestions to aid other groups interested in advancing research on central nervous system infections in resource-limited areas., Highlights • This report provides a comprehensive review of implementing a research network at twelve hospitals in five Peruvian cities. • The network helped build testing capacity, provide education about clinical procedures, and gather data about CNS infections. • Lessons learned offer guidance to others wishing to build similar networks for CNS infections or other unrelated disorders.

La miastenia gravis (MG) es un trastorno autoinmune caracterizado por debilidad y fatigabilidad de los músculos esqueléticos debida a la disfunción de la unión neuromuscular. Se presenta el caso de una adolescente de 11 años de edad, con diagnóstico de miastenia gravis de tipo bulbar. La paciente presentó dos crisis de MG previamente diagnosticadas de manera equivocada como crisis asmáticas. Recibió tratamiento con inmunoglobulina humana, corticoides, piridostigmina y timectomía. Se discute la evolución clínica y riesgos atribuibles a situaciones sociales que pueden retrasar el diagnóstico y su manejo efectivo.

El objetivo del estudio fue describir las características de consulta neurológica y referencia de los niños con parálisis cerebral (PC). Estudio descriptivo retrospectivo. Se evaluaron las historias clínicas de los niños con PC asistentes a consulta externa de la unidad de Neuropediatría del Hospital Nacional Cayetano Heredia entre los años 2011 y 2012. Se incluyeron 81 niños, el 53,1% fueron varones; la PC espástica fue la más frecuente (72,8%), la encefalopatía hipóxico isquémica (28,1%) y las malformaciones cerebrales (28,1%) fueron las principales causas en neonatos a término. La edad media al momento del diagnóstico fue a los 4,1 ± 3,2 años, la epilepsia fue el motivo más frecuente de consulta neuropediátrica. El 58% fue hospitalizado al menos una vez. El tiempo de espera para ser atendido por terapia física tuvo una mediana de 2 meses (rango intercuartílico = 0,8 - 9). Concluimos que el diagnóstico de PC fue tardío, el tiempo de espera para acceder a la atención especializada fue prolongado. Se requieren protocolos y mayor eficiencia para atender en forma adecuada a los niños con PC. The aim of this study was to describe the neurological consultation and baseline characteristics of children with cerebral palsy (CP). The clinical records of children with CP attending an external consultation of the Neuropediatric department of Hospital Nacional Cayetano Heredia between 2011 and 2012 were assessed in this retrospective descriptive study. A total of 81 children were included: 53.1% were boys. Spastic CP was the most frequent (72.8%), hypoxic-isquemic encephalopathy (28.1%) and cerebral malformations (28.1%) were the main causes in term newborn. The average age at the time of diagnosis was 4.1 ± 3.2 years; epilepsy was the most frequent reason for neuropediatric consultation, and 58% were admitted to the hospital at least once. The waiting period to be seen by a physical therapist was around 2 months (interquartile range = 0.8-9). We reached the conclusion that CP was delayed, and the waiting period to access specialized care was prolonged. Protocols and increased efficiency are required to provide adequate medical care to children with CP.

Con el objetivo de describir las características del traumatismo encéfalocraneano (TEC) en niños atendidos en el Hospital Nacional Cayetano Heredia entre los años 2004 y 2011 se realizó un estudio de serie de casos. Se incluyeron 316 menores de 14 años, la mediana de edad fue 4 ± 3 años. El 63,9% fueron hombres. Las principales causas del TEC fueron las caídas (80,1%), y los accidentes de tránsito (10,4%). El 42,4% (134/316) ocurrieron durante horas de la tarde. Hubo mayor frecuencia de accidentes en el hogar (59,2%). El hematoma epidural (26,9%) fue la lesión intracraneal más frecuente. Se requirió manejo en cuidados intensivos en 15,8% e intervenciones quirúrgicas en 14,2% de los casos. Se concluye que las caídas en el hogar fueron la causa principal de TEC y el hematoma epidural la lesión intracraneal más común donde son los menores de 5 años los más afectados. In order to describe the characteristics of traumatic brain injury (TEC) in children seen at the Cayetano Heredia National Hospital from 2004 to 2011, a case study was conducted. 316, 14 year-old minors were included, the median age was 4 ± 3 years old. 63.9% were male. The main causes of TEC were falls (80.1%) and car accidents (10.4%). 42.4% (134/316) occurred in the afternoon. There was a higher incidence of falls at home (59.2%). The epidural hematoma (26.9%) was the most common intracraneal injury. Intensive care management and neurosurgical interventions were required in 15.8 and 14.2% of cases respectively. It is concluded that domestic falls were the main cause of accidents for TEC and epidural hematoma the most common intracranial injury where children under 5 were the most affected.

Se reporta el caso de una niña de 6 años de edad que se presentó con tres meses de enfermedad caracterizada porvómitos recurrentes, cefalea frontal progresiva y vértigo intenso al adoptar el decúbito. El examen clínico demostróhemiparesia derecha, paresia del VI y del VII nervio craneal derecho y paresia del VII nervio craneal izquierdo. LaResonancia Magnética evidenció un tumor ocupando el ángulo pontocerebeloso izquierdo originándose en el IVventrículo.

Objective Because human T-cell lymphotropic virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) may occur in some children infected with HTLV-1, we sought to determine the prevalence of neurologic abnormalities and any associations of neurologic abnormalities with infective dermatitis in these children. Study design We enrolled 58 children infected with HTLV-1 and 42 uninfected children (ages 3 to 17) of mothers infected with HTLV-1 in a family study in Lima, Peru. We obtained medical and developmental histories, surveyed current neurologic symptoms, and conducted a standardized neurologic examination without prior knowledge of HTLV-1 status. Results HTLV-1 infection was associated with reported symptoms of lower extremity weakness/fatigue (odds ratio [OR], 6.1; confidence interval [CI], 0.7 to 281), lumbar pain (OR, 1.7; 95% CI, 0.4 to 8), and paresthesia/dysesthesia (OR, 2.6; CI, 0.6 to 15.8). HTLV-1 infection was associated with lower-extremity hyperreflexia (OR, 3.1; CI, 0.8 to 14.2), ankle clonus (OR, 5.0; CI, 1.0 to 48.3), and extensor plantar reflex (OR undefined; P = .2). Among children infected with HTLV-1, a history of infective dermatitis was associated with weakness (OR, 2.7; CI, 0.3 to 33), lumbar pain (OR, 1.3; CI, 0.2 to 8), paresthesia/dysesthesia (OR, 2.9; CI, 0.5 to 20), and urinary disturbances (OR, 5.7; CI, 0.5 to 290). Conclusions Abnormal neurologic findings were common in Peruvian children infected with HTLV-1, and several findings were co-prevalent with infective dermatitis. Pediatricians should monitor children infected with HTLV-1 for neurologic abnormalities.

Se reporta el caso de un niño de 10 años que se presentó con dos semanas de enfermedad caracterizada por cefalea, vómitos, marcha tambaleante y somnolencia. El examen clínico reveló bradilalia, ataxia, hiperreflexia, nistagmo horizontal y vertical, parálisis del VI nervio craneal izquierdo y signos de frontalización. En la tomografía yresonancia cerebrales se encontró hidrocefalia obstructiva por un tumor de la fosa posterior. La anatomía patológica reveló Astrocitoma Pilocítico grado I (WHO).

La toxoplasmosis congénita es una enfermedad prevalente entre las infecciones intrauterinas. Describimos tres casos clínicos – radiológicos con diferentes formas de compromiso neurológico. Caso 1. feto con hidrocefalia detectada a las 36 semanas, cuya madre presentó anticuerpos positivos (Ig M: 2,26 UI/ml), nació por cesárea y al examen presentó macrocefalia, uveítis, hepatoesplenomegalia, hipotonía, Moro incompleto y succión débil, en suero IgM 0,95 UI/ml, en LCR se observaron taquizoítos viables, y proteínas en 1204 mg/dl. Caso 2: neonato a término, letárgico desde el nacimiento, con coriorretinitis, hemiparesia izquierda, hipotonía, hiporreflexia y midriasis derecha (síndrome de Weber). LCR con pleocitosis ( leucocitos 52 mm3), glucosa en 63mg/dl y proteínas de 300 mg/dl. IgM en sangre de 1,9 UI/ml, y múltiples lesiones parenquimales. Caso 3: lactante de 3 meses, desnutrido, con fontanela grande, hipotonía axial, hipertonía apendicular e hiperreflexia. IgM 2,1 UI/ml. Hidrocefalia por estenosis del Acueducto de Silvio y multiples lesiones parenquimales. En los tres casos el esquema de tratamiento fue sulfadiazina, pirimetamina, ácido folínico y prednisona. Se colocó drenaje ventricular a los niños con hidrocefalia. En la evolución se observó sordera, perdida de la agudeza visual y retardo del desarrollo. Se concluye que la Toxoplasmosis congénita puede tener diferentes formas de presentación neurológica dependiendo del periodo de infestación, inoculo y de la respuesta inmune del huésped.
