Introduction: Hyperbaric Oxygen Therapy (HBOT) consists of administering oxygen at pressures higher than atmospheric pressure. Though HBOT remains one of the safest therapies used today, it is not completely devoid of side effects. Pulmonary oxygen toxicity can result as a result of continuous exposure of elevated levels of oxygen for increased duration and at increased pressures. The main pathological manifestations occur as acute exudative phase followed by subacute proliferative phase. Effects of HBOT on pulmonary functions has not been studied in detail. This study was conducted to ascertain the effects of HBOT on pulmonary functions and determine if these changes are reversible in nature. Material and Method: A prospective observational study was conducted at a tertiary care hospital over a period of 02 years. A total of 72 patients who were undergoing HBOT for any indication normal chest radiograph and normal spirometry/DLCO were included in the study. Spirometry and DLCO was done initially at the time of inclusion and repeated after half of the total planned HBOT sessions and finally at the end of all sessions of HBOT. Spirometry and DLCO was also repeated 6 months post completion of HBOT. The HBOT protocol consisted of breathing 100% oxygen at 2.4 atmospheric absolute (ATA) for 90 minutes which was referred as one session. Total sessions varied from 20-30 based on indication and response of their primary disease to HBO. Results: The mean age score in study population was 47.12 ± 12.88. Among the study population, 12(24%) participants were male and 38(76%) participants were female. The mean BMI was 24.49 ± 5.45. The mean DLCO was 6.67 ± 1.45 ml/min/mm Hg pre intervention, it was 6.38 ± 1.43 ml/min/mm Hg at mid cycle and 6.21 ± 1.41 ml/min/mm Hg post intervention and at 6 months follow up it was 6.43 ± 1.46 ml/min/mm Hg. The difference in the DLCO at mid cycle, post intervention and 6 months follow up period with baseline value were statistically significant (P value 0.005). Conclusion: HBOT although a safe intervention, it is not totally devoid of complications. Although our study did reveal statistically significant changes in pulmonary function in most of the instances, it did not show any clinically significant changes in pulmonary function with use of HBOT protocol at 2.4 ATA. Studies with larger sample size will be required to ensure evidence based safety of HBOT use.

Abstract Background Dysbiosis of the gut microbiota is pivotal in Crohn’s disease (CD) and modulated by host physiological conditions. Hyperbaric oxygen therapy (HBOT) is a promising treatment for CD that can regulate gut microbiota. The relationship between HBOT and the gut microbiota in CD remains unknown. Methods CD patients were divided into an HBOT group (n = 10) and a control group (n = 10) in this open-label prospective interventional study. The fecal samples before and after HBOT were used for 16 S rRNA gene sequencing and fecal microbiota transplantation (FMT). A colitis mouse model was constructed using dextran sulfate sodium, and intestinal and systematic inflammation was evaluated. The safety and long-term effect of HBOT were observed. Results HBOT significantly reduced the level of C-reactive protein (CRP) (80.79 ± 42.05 mg/L vs. 33.32 ± 18.31 mg/L, P = 0.004) and the Crohn’s Disease Activity Index (CDAI) (274.87 ± 65.54 vs. 221.54 ± 41.89, P = 0.044). HBOT elevated the declined microbial diversity and ameliorated the altered composition of gut microbiota in patients with CD. The relative abundance of Escherichia decreased, and that of Bifidobacterium and Clostridium XIVa increased after HBOT. Mice receiving FMT from donors after HBOT had significantly less intestinal inflammation and serum CRP than the group before HBOT. HBOT was safe and well-tolerated by patients with CD. Combined with ustekinumab, more patients treated with HBOT achieved clinical response (30%vs.70%, P = 0.089) and remission (20%vs.50%, P = 0.160) at week 4. Conclusions HBOT modulates the dysbiosis of gut microbiota in CD and ameliorates intestinal and systematic inflammation. HBOT is a safe option for CD and exhibits a promising auxiliary effect to ustekinumab. Trial registration Chinese Clinical Trial Registry, ChiCTR2200061193. Introduction: Decompression sickness (DCS) is a clinical syndrome, i.e. commonly seen in divers. The global prevalence of DCS among professional dive instructors is approximately 3.4%. This study aimed to assess the level of knowledge concerning DCS among divers in Saudi Arabia. Materials and Methods: This is a cross-sectional study conducted among divers in Saudi Arabia. The research team visited several diving resorts to distribute the questionnaires. Furthermore, diving instructors from different cities across the nation were requested to distribute the questionnaire to their colleagues and diving communities. Results: A total of 262 divers participated in the study, of which 252 (96.18%) had a scuba-certified license. The majority of divers (94.27%) reported that they were aware of DCS. Furthermore, 27.48% of the participants reported that they had experienced clinical symptoms of DCS. Only 14 (19.44%) received first aid care, and 17 (6.489%) needed a deco dive/decompression dive. Females demonstrated a higher risk of developing DCS-related symptoms than males (odds ratio = 2.57; 95% confidence interval = 5.44–1.21; P = 0.0135). Conclusion: Majority of the participants were familiar with the nature of the disease and were found to have practiced safe diving. Practically, one-quarter of the participants experienced DCS-related symptoms, and only very few of them received first aid and/or a deco dive. Despite the nature of the study and its small sample size, this study adds to the growing nature of published literature in giving a better understanding of the disease by divers.

Hyperbaric oxygen therapy (HBOT) is an adjunctive treatment for chronic pain. The current study aimed to perform a systematic evaluation and meta-analysis of randomised controlled trials (RCTs) to assess the clinical efficacy and safety of HBOT for pain reduction in fibromyalgia (FM) patients compared to conventional therapy. A systematic search of medical subject headings and keywords associated with HBOT for FM patients was conducted using electronic databases: MEDLINE/ PubMed, Web of Science, Cochrane Library Central Register of Controlled Trials, and Scopus. Google Scholar was also combed to detect likely non-indexed available studies. We also searched manually through reference lists of identified original articles or reviews for relevant articles. The http://www.clinicaltrial.gov was examined for all research protocols being conducted/ planned. The Cochrane Collaboration tool and Higgins and Thompson's I2 method determined the risk of bias and heterogeneity. The primary outcome was the effect of HBOT on pain; secondary outcomes included adverse effects of HBOT, assessment of the quality of life, functional impairment, psychological symptoms, and sleep quality. The overall quality of evidence was assessed with the GRADE approach. Cochrane RevMan software (version 5.4) was used for data analysis. Results were synthesised through a general summary of the characteristics and findings of each study. We also analysed different HBOT protocols and their outcomes across the studies. Five RCTs enrolling 91 and 89 patients in FM and control groups were included in the present study. HBOT results in decreased tender points (-6.23 (95 % CI: -7.76, -4.71) P < 0.0001), increased pain threshold ( 0.57 (95% CI: 0.3, 0.83) P < 0.00001) and a decreased Visual Analog scale (VAS) score for pain (-5.31(95 % CI: -8.17, -2.45)). Quality of life, sleep, functional impairment, etc., also improved after HBOT. HBOT is effective in pain and other associated outcomes in FM patients. In FM patients, high-quality evidence in favour of HBOT to improve pain, fatigue, quality of life, sleep, psychological outcomes, etc., is required.

Earthquakes are catastrophic natural disasters. Staying under rubble for a long time may cause compartment syndrome, especially in the extremities, and crush injuries due to severe trauma. In addition, frostbite injuries may occur due to extreme climate in the winter. In hyperbaric oxygen therapy (HBOT), patients breathe 100% oxygen in a closed chamber at pressures greater than 1 atmosphere absolute. HBOT provides elevated partial oxygen pressure, increased oxygen diffusion distance, decreased edema of the areas with circulatory disorder, augmented bactericidal effect, additive-synergistic effect with antibiotics, and enhanced wound healing. HBOT is a successful adjunctive treatment option for crush injuries, acute skeletal compartment syndromes, and frostbite injuries.

Background: Radiation-induced hemorrhagic cystitis (RHC) is a chronic inflammatory disease in patients undergoing radiation therapy that causes a cluster of symptoms which may have a latent period of months to years. The current non-invasive treatments include drug treatment and hyperbaric oxygen therapy (HBOT), which has been widely applied for RHC so far but with limited evidence. Thus, we conducted a systematic review and meta-analysis to clarify the effects and safety of HBOT for RHC. Methods: A systematic review and meta-analysis were utilized, searching in the databases of Embase, Pubmed, and Web of Science. The primary endpoint of the present study was complete remission of hematuria. The meta-analysis was conducted using a random effects model, and a pooled odds ratio with 95% CI was calculated. Results: A total of 317 studies were searched and fourteen articles with 556 patients were collected. The results showed that a total of 500 patients (89.9%) had symptom improvement, and the pooled results demonstrated that 55% of patients with HBOT had complete remission of hematuria (95% CI 51–59%). Conclusions: A significant improvement of symptoms when treated with HBOT was shown in this meta-analysis for patients with RHC.

Background: Research advancing effective treatments for breast cancer is crucial for eradicating the disease, reducing recurrence, and improving survival rates. Nipple-sparing mastectomy (NSM), a common method for treating breast cancer, often leads to complications requiring re-operation. Despite advancements, the use of hyperbaric oxygen therapy (HBOT) for treating these complications remains underexplored. Therefore, we analyze the efficacy of HBOT in the post-operative care of patients undergoing NSM. Methods: A systematic search was conducted using PubMed, Scopus, and the Cochrane Library. Studies were assessed for eligibility using the PICO (Population, Intervention, Comparison, Outcome) framework and classified based on American Society of Plastic Surgeons (ASPS) levels of evidence. Seven studies, totaling a pool of 63 female patients, met the inclusion criteria. Among these studies, four were categorized as Level III (57.1%), one as Level IV (14.3%), and two as Level V (28.6%). These studies focused on HBOT’s role in wound healing, the successful salvage of breast reconstruction, and the optimal timing for HBOT. Results: This review revealed that HBOT indeed has potential for improving tissue oxygenation, vascularization, and, consequently, wound healing. It is noted that HBOT is efficacious for mitigating post-NMS complications, including infections, re-operation, flap loss, seroma, and hematoma. Conclusions: Overall, HBOT could be beneficial in standard post-surgical care protocols for patients undergoing NSM due to its role in mitigating common adverse effects that occur after mastectomy. Despite promising outcomes, the recent literature lacks rigorous clinical trials and well-defined control groups, underscoring the need for further research to establish standardized HBOT protocols.

Introduction Hyperbaric oxygen (HBO 2 ) therapy involves the inhalation of pure oxygen in a pressure chamber under increased ambient pressure. Recent research indicates that circulating small extracellular vesicles (sEVs) play important roles in human physiology and pathology. Therefore, the objective of this pilot study was to monitor the impact of HBO 2 therapy on the levels of circulating sEVs in the serum of patients with necrotizing soft-tissue infections (NSTI), aseptic bone necrosis (ABN) or idiopathic sudden sensory neural hearing loss (ISSNHL). Material and methods Serum-derived sEVs were isolated and quantified in 80 patients before and after HBO 2 therapy applied for NSTI, ISSNHL and ABN patients as well as in normal controls who received neither HBO 2 therapy nor steroids. Results We observed a significant increase of circulating sEVs in patients with ISSNHL after HBO2 therapy (p < 0.05), as well as significantly elevated levels of sEVs after HBO 2 therapy compared to patients with NSTI (p < 0.05) and ABN (p < 0.01). Conclusions The increase in the levels of sEVs in ISSNHL may be evidence for both the intended reduction of inflammation as a result of steroid therapy and the inhibitory effect of oxidative stress induced by HBO 2 therapy. Thus, sEVs released during HBO 2 therapy might play an important biological role in mediating the response to therapy and might be a promising approach to gain further insights into the therapeutic efficacy of HBO 2 therapy.

Abstract Background Hyperbaric oxygen (HBO) plays positive roles in the therapy of traumatic brain injury (TBI); however, the mechanism underlying its effects on TBI is largely unknown. The study aims to elucidate the molecular mechanism implicated with the interaction between platelet-derived growth factor-BB (PDGF-BB) and extracellular signal-regulated kinase 1/2 (ERK1/2) signaling pathway, which may play critical roles during HBO treatment both in the astrocyte scratching model in vitro and rat TBI model in vivo. Methods Changes in neurological function and wound healing were evaluated using the neurological severity scores (NSS) scale, immunohistochemistry, western blotting, and qRT-PCR, respectively. Results The results showed that PDGF-BBi (PDGB interfered with small RNA) dramatically improves neuronal viability in vitro when transfected into the scratched astrocytes derived from the cerebral cortex of neonatal rats. Moreover, in vivo experiments revealed that HBO therapy substantially elevated the NSS scores and simultaneously reduced the mortality in TBI rats, as indicated by the NSS scales. Notably, HBO therapy was found to possess the ability to inhibit glial cell proliferation, promote the regeneration of neurons and synapses, and ultimately facilitate the wound healing, as revealed by immunohistochemistry and glial scar formation found in TBI rats. Importantly, HBO markedly decreased the expression levels of PDGF-BB and ERK1/2. It can clearly be seen that downregulated PDGF-BB and ERK1/2 levels were corresponding with the status of significant amelioration of the therapeutic effect of HBO. Conversely, the upregulation of PDGF-BB and ERK1/2 levels was in line with the opposite effect. Conclusion It has been concluded that HBO therapy may play its active role in TBI treatment dependent on astrogliosis inhibition, which may be achieved by downregulating the ERK1/2 signaling pathway mediated by PDGF-BB.

Hyperbaric Oxygen Therapy (HBOT) can be used as an emergency therapy for acute pathology or as a supplementary treatment for chronic illness. Abstract Background: Suicide is the second most common cause of death and is a major public health problem in many countries globally. Chemical poisoning, such as hydrochloric acid (HCl), accounted for 18.8% of the total suicide cases in Indonesia. Through ingestion, this acid chemical contributes as the most common cause of swallowing injuries that can adversely affect the gastrointestinal mucosal through various pathological processes, primarily through an excessive inflammatory process. On the other hand, hyperbaric oxygen therapy (HBOT) has been widely used as a non-pharmacological therapy in many diseases, although its mechanism for reducing inflammation in HCl poisoning has remained unclear. Objective: This study aimed to provide a better understanding on hyperbaric oxygen's biomolecular mechanism as a potential adjuvant therapy in HCl poisoning. Discussion: HCl poisoning causes an excessive inflammatory process, leading to tissue hypoxia indicated by increased expression of Hypoxia Inducible Factor 1 (HIF-1). Hyperbaric oxygen therapy decreases the expression of HIF-1 through activation of the HIF-1 hydroxylation pathway via prolyl hydroxylase (PDH) in the proline pathway and HIF inhibiting factor (FIH) in the asparagine pathway. Reactivating both pathways will decrease HIF-1 activity, eventually reducing the ongoing inflammatory process. In addition, HBOT also plays a role in wound healing by stimulating angiogenesis growth factors. Conclusion: Hyperbaric oxygen therapy has the potential to be used as adjuvant therapy in HCl poisoning due to its beneficial effects on reducing inflammatory mediators and wound healing.

Objective(s): Spiders of the Loxosceles genus, known as violin spiders, produce venom with dermonecrotic and systemic effects, as it is a species widely distributed in the world, its study represents a high medical relevance. Systemic loxoscelism, which occurs in 1 in 5 cases and is the most frequent in children, can be fatal, so the study of effective therapy is of great relevance. In the present study, we compared different therapeutic options to mitigate the systemic effects of Loxosceles boneti venom in a model in which prepubertal rats were used.Materials and Methods: A model of systemic intoxication by L. boneti venom was provoked in male Wistar rats. Study groups were formed: healthy control, with venom and untreated control, treatment with N-acetylcysteine, and/or hyperbaric oxygenation therapy. Subsequently, pathological analysis of the kidney and lung was performed. The oxidant-antioxidant response was evaluated, and molecular analysis of the COX-1 and COX-2 enzymes was performed.Results: Regenerative changes were observed at the cellular level in both treatments, being more noticeable in the HBO group. The anti-oxidant response was outstanding in the same group. Conclusion: Both treatments offer considerable benefits, however; further studies are needed to provide adequate therapeutics.

Nowadays, the evolution of cardiac surgery is impossible without a continuous improvement of all the treatment stages. One of the promising ways to achieve this goal is the active use of hyperbaric oxygen therapy in the preoperative preparation and postoperative rehabilitation. In this review, we present a short history of the hyperbaric oxygen therapy development in cardiac surgery, the pathophysiological and pathobiochemical mechanisms of its therapeutic effect and the scenarios for its use in the preoperative preparation and postoperative rehabilitation of cardiac surgery patients. The introduction of hyperbaric oxygen therapy into cardiac surgery can improve the results of the surgical treatment, as well as reduce the times of the preoperative preparation and postoperative rehabilitation of cardiac surgery patients, that will significantly increase the quality and efficiency of cardiac surgery.

For the purposes of this guideline, a diving accident is defined as an event that is either potentially life-threatening or hazardous to health as a result of a reduction in ambient pressure while diving or in other hyperbaric atmospheres with and without diving equipment. This national consensus-based guideline (development grade S2k) presents the current state of knowledge and recommendations on the diagnosis and treatment of diving accident victims. The treatment of a breath-hold diver as well as children and adolescents does not differ in principle.In this regard only unusual tiredness and itching without visible skin changes are mild symptoms.The key action statements: on-site 100% oxygen first aid treatment, immobilization/no unnecessary movement, fluid administration and telephone consultation with a diving medicine specialist are recommeed.Hyperbaric oxygen therapy (HBOT) remains unchanged as the established treatment in severe cases, as there are no therapeutic alternatives. The basic treatment scheme recommended for diving accidents is hyperbaric oxygenation at 280 kPa.

Background/Aims Existing therapeutic options for complicated Crohn’s disease (CD) like biologics and surgery are limited by inadequate long-term efficacy, cost, and adverse effects. Tissue hypoxia is important in CD pathogenesis and may be ameliorated with hyperbaric oxygen therapy (HBOT). We assessed the efficacy and tolerability of HBOT in small bowel stricturing CD. Methods This pilot study included patients of small bowel stricturing CD (from April 2019 to January 2020) who underwent HBOT. These patients were refractory to conventional medical treatment or had multiple strictures not amenable to resection. Each session of HBOT was given for 60 minutes with a pressure of 1.5–2.5 atm. Clinical, biochemical responses and Short Inflammatory Bowel Disease (SIBD) questionnaire were evaluated at 2 and 6 months, and radiological response was evaluated at 6 months. Results Fourteen patients (mean age, 42.9±15.7 years; male, 50%) were subjected to 168 HBOT sessions. Thirteen patients (92.7%) had strictures and 1 patient had enterocutaneous fistula in addition. Median number of HBOT sessions was 11 (range, 3–20) which were administered over a median of 4 weeks. Most patients tolerated it well except 1 who had hemotympanum. At 2 and 6 months of follow-up, 64.2% of patients had a clinical response, 50% and 64.2% of patients had clinical remission respectively. Steroid-free clinical remission was seen in 8 (57%) of patients with radiological improvement in 50%. There was a significant improvement in SIBD scores at 2-month follow-up (59.4 vs. 44.5, P=0.03). Conclusions HBOT can be a safe and effective therapeutic option in patients with stricturing small bowel CD refractory to conventional medical treatment.

Abstract Background The coronavirus disease 2019 pandemic has resulted in a growing population of individuals who experience a wide range of persistent symptoms referred to as “long COVID.” Symptoms include neurocognitive impairment and fatigue. Two potential mechanisms could be responsible for these long-term unremitting symptoms: hypercoagulability, which increases the risk of blood vessel occlusion, and an uncontrolled continuous inflammatory response. Currently, no known treatment is available for long COVID. One of the options to reverse hypoxia, reduce neuroinflammation, and induce neuroplasticity is hyperbaric oxygen therapy. In this article, we present the first case report of a previously healthy athletic individual who suffered from long COVID syndrome treated successfully with hyperbaric oxygen therapy. Case presentation A previously healthy 55-year-old Caucasian man presented 3 months after severe coronavirus disease 2019 infection with long COVID syndrome. His symptoms included a decline in memory, multitasking abilities, energy, breathing, and physical fitness. After evaluation that included brain perfusion magnetic resonance imaging, diffusion tensor imaging, computerized cognitive tests, and cardiopulmonary test, he was treated with hyperbaric oxygen therapy. Each session included exposure to 90 minutes of 100% oxygen at 2 atmosphere absolute pressure with 5-minute air breaks every 20 minutes for 60 sessions, 5 days per week. Evaluation after completing the treatment showed significant improvements in brain perfusion and microstructure by magnetic resonance imaging and significant improvement in memory with the most dominant effect being on nonverbal memory, executive functions, attention, information procession speed, cognitive flexibility, and multitasking. The improved cognitive functions correlated with the increased cerebral blood flow in brain regions as measured by perfusion magnetic resonance imaging. With regard to physical capacity, there was a 34% increase in the maximum rate of oxygen consumed during exercise and a 44% improvement in forced vital capacity. The improved physical measurements correlated with the regain of his pre-COVID physical capacity. Conclusions We report the first case of successfully treated long COVID symptoms with hyperbaric oxygen therapy with improvements in cognition and cardiopulmonary function. The beneficial effects of hyperbaric oxygen shed additional light on the pathophysiology of long COVID. As this is a single case report, further prospective randomized control studies are needed.

Acute ischemia of the glands is a severe complication after circumcision. We outline the challenging case of a seventeen-year-old boy with glandular ischemia (GI) that appeared shortly after circumcision. Methods: We present a case report and literature review related to glans ischemia, and the complications of circumcision are reviewed. We note that there are very few cases described in the literature. Our patient was successfully treated with hyperbaric oxygen therapy (HBOT) after four days of no positive effect after all medical and surgical treatments written in the literature: Subcutaneous enoxaparin, local application of a glyceryl trinitrate, continuous epidural perfusion, intravenous pentoxifylline, alprostadil, intraoperative drainage, and aspiration with saline solution and epinephrine. Clinical improvement was noted at the first session of HBOT. A number of days after the operation, the penis looked normal and was healing. Complete healing of the penile glans was successfully realized one month after surgery. Conclusion: Based on the review and the case presented, we conclude that HBOT is the treatment of choice for acute ischemia of the penile glans, especially when other treatments do not work.

Background: The aim is to study the sequence of events contributing the survival of wrist electrical burns and reconstructive options for salvage of soft tissue of electrical burns of wrist. Methods: Data of patients with high tension electrical burns involving the wrist admitted in our hospital from January 2018 to December 2020. There were 17 patients with electrical burns with wrist injury. Patients from age 18 to 65 years were included in the study. Patients were subjected to bio immunoassay, infrared thermography, indocyanine fluorescent scan, hyperbaric oxygen therapy and reconstructive procedures. Results: Among 17 patients, 16 were male. Their age ranged from 20 to 61 years. The injury resulted from 11 kv voltage line most often and the patients were admitted within a day of injury. All patients were subjected to multiple staged procedures of debridement and reconstruction. 7 patients were subjected to amputation of the limb, 5 patients subjected to reconstruction in terms of abdominal flap and 4 with groin flap and one perforator plus flap was used to cover the wrist defect. Conclusions: Wrist electrical burns due to its crucial location, requires knowledge on disease process, if ignored will directly lead to significant morbidity. Utilisation of newer modalities will help in timely intervention and salvage of the limb.

A team of 10 personnel from a mountaineering team has undertaken an expedition to Mt Menthosa at an altitude of 21138 ft. in October 2018. After scaling the peak and in the following days during descent, the three members of the team had developed cold injuries. They were given first aid during descent. Later, they were diagnosed as cases of frostbite. All three personnel were evacuated to a Military Hospital, Delhi, India and were treated with oral vasodilators, antiplatelet drugs, analgesics, and hyperbaric oxygen therapy (HBO2T) was administered at a Recompression chamber at Delhi for 1 week. These personnel were thereafter referred to INHS Asvini for further management and continuation of HBO2T. After 30 sessions of HBO2T administration at this hospital, there was a significant improvement in the wounds of all three personnel with minimal surgical intervention and no major amputation. The treatment of frostbite includes treating hypothermia before thawing of cold injury, nonsteroidal anti-inflammatory drugs, antibiotics, tissue plasminogen activators and iloprost, a potent vasodilator which is used in severe cases. Planning of surgery is vital with the goal of obtaining the best functional outcome. Premature amputation increases morbidity and is likely to lead to poor subsequent function. This article aims to discuss the advantages of HBO2T administration as an adjuvant therapy in the management of frostbite cases along with wound care management.

Abstract Kienböck disease, also known as lunatomalacia, is a rare condition which can lead to progressive wrist pain and abnormal carpal motion. We present the case of a 30‐year‐old patient with Down syndrome who came to our observation for treatment of stage III C Kienböck disease. In September 2019, the patient reported wrist pain with limitation in movements and initially underwent conservative treatment without benefit. In October 2020, pain symptoms and difficult movements with reduced strength worsened and surgical treatment was proposed, but the patient and his family declined. Thereby the patient underwent conservative treatment with hyperbaric oxygen therapy (HBOT) 60 sessions, 100% oxygen at 2.5 absolute atmospheres (ATA), oxygen total time 60 min, once daily, five times per week. After 6 months, a positive clinical and radiological evolution were observed, with an improvement in the patterns of pain, motion, and strength and an almost complete involution of the process of aseptic necrosis of the semilunar. To the best of our knowledge, this is the first report of stage III C Kienböck's disease in Down's syndrome patient treated with HBOT.

Abstract Background Oxygenation serves as a cornerstone in the treatment of COVID-19, and several methods have been extensively studied so far. Herein, we aimed to systematically review the studies discussing hyperbaric oxygen therapy (HBOT) to examine its reported efficacy and adverse events in patients with COVID-19. Methods We systematically searched and retrieved the relevant articles using keywords on the online databases, including PubMed, Scopus, Embase, Web of Science, and Cochrane databases up to April 11th, 2021. The retrieved records underwent a two-step title/abstract and full-text screening process, and the eligible papers were identified. National Institutes of health (NIH) quality assessment tool was used for this study. This study was registered in the International Prospective Register of Systematic Reviews (PROSPERO) with ID CRD42021269821. Results Eight articles from three countries were included. All the included studies had good and fair quality scores, with no poor studies included in this systematic review (Good: n = 5, Fair: n = 3). Studies were divided into clinical trials and case reports/series. Most of the studies used HBOT less than 1.5–2 absolute atmospheres (ATA) for 90 min sessions and thereafter sessions were decreased to 60 min. Trials demonstrated most of the patients recovered after receiving HBOT, and blood oxygen saturation increased after several sessions of HBOT. Conclusion Overall, HBOT seems to be a safe and effective oxygenation method in patients with COVID-19. However, there is limited knowledge and evidence regarding the effects and mechanism of HBOT in COVID-19 treatment, and further evaluations require extensive well-designed studies.

Abstract Background and Aims Hyperbaric oxygen therapy (HBOT), utilizes 100% oxygen at pressures greater than sea‐level atmospheric pressure, for the treatment of conditions in which the tissues starve for oxygen. The Undersea and Hyperbaric Medical Society (UHMS) has granted HBOT approval for the treatment of various conditions. On the other hand, applying informatics registry systems can improve care delivery, ameliorate outcomes, and reduce the costs and medical errors for the patients receiving HBOT treatment. Therefore, we aimed to design, develop, and evaluate a registry system for patients undergoing HBOT. Methods In the first phase, the conceptual and logical models were designed after conducting symposiums with experts and having other experts review the models. In the second phase, the system was developed on the web using ASP.NET and C# programming languages frameworks. The last phase involved Nielsen's heuristic evaluation method for the system's usability. Five experts evaluated the system, including three health information management specialists and two medical informatics specialists. Results The hyperbaric patient information registry system (HPIRS) interacts with three types of users—a specialist physician, a nurse, and a system administrator. A scenario for each predefined activity was designed, and all the information was stored in the SQL servers. The five experts independently found 152 issues, of which 84 were duplicates. The 68 distinct issues of the system were then resolved. Conclusions The design and development of such registry systems can make data available and stored carefully to improve clinical care and medical research and decrease costs and errors. These registries can provide the healthcare systems with E‐health applications, improved data management, more secure data transfer, and support for statistical reporting. The implemented heuristic evaluation method can also provide a low‐cost and readily available system to fix the issues of the designed systems.

Background: A few prospective trials and case series have suggested that hyperbaric oxygen therapy (HBOT) may be efficacious for the treatment of severe COVID-19, but safety is a concern for critically ill patients. We present an interim analysis of the safety of HBOT via a randomized controlled trial (COVID-19-HBO). Methods: A randomized controlled, open-label, clinical trial was conducted in compliance with good clinical practice to explore the safety and efficacy of HBOT for severe COVID-19 in critically ill patients with moderate acute respiratory distress syndrome (ARDS). Between 3 June 2020, and 17 May 2021, 31 patients with severe COVID-19 and moderate-to-severe ARDS, a ratio of arterial oxygen partial pressure to fractional inspired oxygen (PaO2/FiO2) < 26.7 kPa (200 mmHg), and at least two defined risk factors for intensive care unit (ICU) admission and/or mortality were enrolled in the trial and randomized 1:1 to best practice, or HBOT in addition to best practice. The subjects allocated to HBOT received a maximum of five treatments at 2.4 atmospheres absolute (ATA) for 80 min over seven days. The subjects were followed up for 30 days. The safety endpoints were analyzed. Results: Adverse events (AEs) were common. Hypoxia was the most common adverse event reported. There was no statistically significant difference between the groups. Numerically, serious adverse events (SAEs) and barotrauma were more frequent in the control group, and the differences between groups were in favor of the HBOT in PaO2/FiO2 (PFI) and the national early warning score (NEWS); statistically, however, the differences were not significant at day 7, and no difference was observed for the total oxygen burden and cumulative pulmonary oxygen toxicity dose (CPTD). Conclusion: HBOT appears to be safe as an intervention for critically ill patients with moderate-to-severe ARDS induced by COVID-19. Periodontitis is a chronic inflammatory disease with multifactorial aetiology. The relationship between periodontal disease and systemic diseases such as diabetes, obesity, metabolic syndrome, atherosclerotic, cardiovascular disease, and cognitive disorders has been the subject of many studies. The purpose of this study was to evaluate the effectiveness of hyperbaric oxygen therapy on periodontal health in patients suffering from periodontitis and type 2 diabetes. The study was conducted with 14 patients. A total of 369 periodontal pockets in the study group and 431 in the control group were examined. For further analysis, the pockets were classified as moderately deep (4–5 mm) and deep (≥6 mm). All patients received standard non-surgical treatment: scaling and root planing (SRP). Additionally, a series of 30 hyperbaric chamber sessions was carried out in the study group. The following parameters were compared between groups: PD (probing depth), CAL (clinical attachment level), and BOP (bleeding on probing). The results of the study showed significantly better results in terms of PD reduction and CAL gain in the study group in comparison to the control group. Both groups showed a reduction in BOP (bleeding on probing) after treatment. The use of hyperbaric oxygen therapy seems to have considerable benefits in patients with type 2 diabetes.

INTRODUCTION: The aim of this study was to investigate the clinical and radiological results of exposure to high level of oxygen under high level of atmospheric pressure [Hyperbaric Oxygen (HBO) Therapy] in patients with Ficat Stage 1 and 2a avascular necrosis of the femoral head (AVNFH). METHODS: A retrospective examination was performed on the patients, who were administered HBO therapy and followed up due to Ficat Stage 1 and 2a AVNFH between February 2016 and November 2019.The Harris Hip Score forms, conventional radiography of the hip, and MRI were analyzed. The Ficat classification was used for grading of the aascular necrosis. RESULTS: A total of 25 patients with a mean age of 45.4 +- 11.4 years were evaluated. It was observed that the patients were administered an average of 30.4 +- 3.55 (range: 27-47) sessions of HBO therapy within an average of 40.6 +- 5.32 (range: 36-65) days. After the HBO therapy, it was found that the Ficat stage of 11 patients progressed, while the stages of two patients regressed, and seven patients remained stable (p: 0.002). While the mean total Harris Hip Score was 43.82 +- 18.55 before the procedure, it was found to be 78.47 +- 17.05 after the procedure. Objective: Radiation-induced hemorrhagic cystitis (RHC) is an accepted hyperbaric oxygen therapy (HBOT) indication. We aimed to analyze the knowledge and the opinions of urology physicians on HBOT in RHS patients with a survey. Material and Methods: The questionnaires were conducted face to face or online on urology physicians. Results: Seventy-seven urology physicians participated in our study. Physicians have been working for 11 ± 10.5 years in the field of Urology. The 84.4% of our physicians had an average of 0-10 RHC patient administration in a year. However, the majority of the participants stated that they have never referred RHC patients to HBOT. Similarly, 48.1% of the physicians stated that they have insufficient knowledge of HBOT for their specialty. On the other hand, the majority were not sure about the HBOT as an effective treatment option in RHC patients (54.5%), about the cost-effectiveness of HBOT for RHC (66.2%), and the ability of HBOT on shortening the recovery period of RHC patients (49.9%). Cystoid macular edema (CME) is the most common cause of reduced vision in branch retinal vein occlusion (BRVO). The treatment protocols for macular edema in BRVO have changed considerably in the previous decade because of the increased popularity of the wide use of optical coherence tomography (OCT) and intravitreal drugs. Intravitreal injection of anti-vascular endothelial growth factors (anti-VEGF) and steroids result in the reduction of CME. However, the effect is transient, and multiple injections may be required. On repeated use, few patients become non-responders to these drugs. Nonetheless, anti-VEGF drugs may have potentially life-threatening complications because of systemic absorption. Intravitreal steroids may lead to severe ocular side effects such as cataract and glaucoma. Chances of endophthalmitis, retinal tear, and inflammation can occur with any of the intravitreal injections. We present a case of BRVO with recalcitrant CME following focal laser, multiple intravitreal anti-VEGF, and steroid injections. However, he had cardiovascular risk factors, so repeated anti-VEGF injections were not given. He was planned for hyperbaric oxygen therapy and showed favorable response. His visual acuity improved and macular edema reduced to more than half from baseline values on OCT.

INTRODUCTION: This study; It was performed to determine the anxiety levels of patients who received hyperbaric oxygen therapy for the first time. METHODS: Research; It was performed in the Hyperbaric Oxygen Therapy unit of a city hospital between May 2018 and February 2020. The sample of the descriptive study consisted of 76 patients who underwent hyperbaric oxygen therapy for the first time. Expression of anatomical landmarks of the ear results in the success of ear reconstruction in microtia. Ear reconstruction in India is routinely done with a two-dimensional (2D) template by tracing the contralateral ear with an X-ray sheet. There is a need of a proper template to understand the finer details of the ear reconstruction. In this report, a three-dimensional (3D) template in association with a 2D template is utilised to augment the reconstruction ability of the plastic surgeon. After computed tomography scan, an accurate 3D scan of the normal ear is obtained. Using the 3D ear model file, a negative mould of the ear 3D model is developed and fed into a 3D printer. The model is sterilised and used for reconstruction. Hyperbaric oxygen therapy is used as an adjunct for the procedure. Simultaneous utilisation of 3D and 2D templates for ear reconstruction appears to increase the ability of plastic surgeons to recreate the ear.

Abstract The diagnosis of decompression sickness (DCS) is mostly based on clinical suspicion, and there is currently no available modality to fully confirm the diagnosis. However, the use of echocardiography in suspected DCS cases has become more common. In this case, transthoracic echocardiography (TTE) was used to detect microbubbles in the right cardiac chambers and monitor the patient after hyperbaric oxygen therapy (HBOT), suggesting the possible applicability of TTE in diagnosing and monitoring DCS patients. This report describes a 54‐year‐old Fisherman who was referred to the emergency department with dyspnea and mild confusion after a rapid ascent of a saturation dive at 50 m sea depth. After the initial evaluation, he was assessed using TTE to exclude the presence of structural heart disease, where it surprisingly showed spontaneous echo contrast inside the right cardiac chambers similar to agitated saline echo testing. The patient was then admitted for HBOT and follow‐up; rapid improvement was noticed after the first HBOT session and the TTE findings were fully resolved. TTE could be considered in the initial workup when DCS is suspected, and it might have a role in monitoring DCS patients if echocardiographic findings of bubble formation were documented in the pre‐hyperbaric therapy settings.

Definitively diagnosed disease-directed therapy leads to the rapid resolution of clinical illness and decreases the chances of developing chronic ailments. Appropriate diagnosis is crucial in initiating such therapies which can be made straightforward with the assistance of modern diagnostic aids. Osteomyelitis is an inflammatory disease of the bone and its marrow contents. This disease can have multifarious clinical and histopathologic presentations. Osteomyelitis of the jaws has proved to be a challenging condition to effectively diagnose, treat, and cure. However, there are various diagnostic aids which help in the identification of the condition. The diagnosis is usually based on a thorough evaluation of the patient history and clinical presentation, imaging techniques, culturing, and histologic analysis. Cone-beam computed tomography (CBCT) has proved to emerge as a promising diagnostic aid in relation to osteomyelitis. CBCT offers different views that give a clear insight into the extent of involvement of the disease. Treatment of this condition includes two lines of management – conservative approach and surgical approach. The conservative approach includes the use of antibiotics, hyperbaric oxygen therapy which improves the availability of antibiotics at the localized site, especially with the sclerosing variant. Surgical interventions are usually confined to the extraction of extremely mobile teeth, debridement of fragments of bone, and incision and drainage of fluctuant areas. Additional surgical procedures are considered if the infection persists, and these include sequestrectomy, saucerization, decortication, or resection followed by reconstruction. A case of diffuse sclerosing osteomyelitis arising from nonvital teeth managed using the surgical approach is presented here.

Introduction: Sudden sensorineural hearing loss (SSNHL) is a medical emergency and if treated early, may improve chances of recovery of lost hearing. Delay in diagnosis and/or initiation of treatment may cause a permanent hearing loss. A relative lack of standardized management protocol has resulted in wide variation among various institutes for investigations and treatment of SSNHL. Aims and Objectives: To provide an overview of causes, methods of evaluation, and management of SSNHL pertaining to the Indian Naval scenario, based on cases evaluated and managed at a zonal level hospital of Indian Navy, over a period of 1 year. Materials and Methods: A prospective cohort study was conducted at the department of otorhinolaryngology at a zonal hospital of Indian Navy from January 2018 to December 2018 and subsequent follow-up until June 2019, giving a minimum follow-up of 6 months for each case. Ten cases were included in the study, evaluated, and managed and the outcomes were analyzed. Among these, six were idiopathic SSNHL (ISSNHL) cases and four were noise-induced (after small-arm firing) SSNHL cases. Results and Conclusion: Hearing outcomes were better and statistically significant for ISSNHL cases compared to noise-induced SSNHL cases. There was a strong negative correlation between the delay in initiation of treatment with improvement in hearing after treatment. The pattern of audiogram is a good predictor for recovery/no recovery, wherein flat and ascending types showed complete recovery, whereas descending type and profound hearing loss type showed no/partial recovery.

Abstract Flexible fiber‐optic bronchoscopy is used to determine diagnoses in pulmonary diseases. It is considered as a safe procedure, although some complications might occur, one of which is cerebral air embolism. In this case, we present the air embolism after the bronchoscopy procedure, ending in fatality. We strongly recommend that bronchoscopists should keep this complication in mind and be aware of early symptoms pertaining to the patient's state of consciousness during bronchoscopy examination. Early treatment is essential in this situation.

The growing use of dermal fillers, specifically the use of hyaluronic acid, may be explained by their effectiveness and versatility and their favorable safety profiles. Nevertheless, early and late complications with varying levels of severity may occur. The incidence of complications is low; however, more severe events such as skin ischemia and necrosis, and even visual loss may occur. The main objective of this case report on intravascular filler injection was to provide information on early signs of ischemic skin necrosis to physicians administering cosmetic fillers. This knowledge will facilitate timely management of symptoms, and enable treatment with hyperbaric oxygen therapy (HBOT) to prevent permanent scarring. A case note study of a single 41-year-old female who underwent a combination of treatments including the “high-dose pulsed hyaluronidase” technique, intravenous dexamethasone, HBOT, open dressing with normal saline, and bactigras dressing. Early recognition and immediate application of the correct combination of treatments for intravascular filler injury prevented further skin necrosis, permanent scarring, and the requirement for surgical reconstruction. Recently, as the application of dermal fillers for soft-tissue augmentation has increased, the number of cases reporting adverse reactions has also increased. The most severe and alarming early complication is vascular occlusion. Thus, it is crucial that physicians are familiar with the anatomy of the treated region, type of product used, correct application technique, and possible adverse events to allow immediate recognition and treatment.

Abstract We present an unusual case of Lemierre´s syndrome complicated by multiple brain abscesses, a literature review and suggested management. A young man with multiple brain abscesses deteriorated despite two weeks of directed antibiotics. A multidisciplinary approach was successful. Hyperbaric oxygen treatment (HBOT) should be considered in refractory or severe cases.

Carbon monoxide (CO) can occur in numerous situations and ambient conditions, such as fire smoke, indoor fireplaces, silos containing large quantities of wood pellets, engine exhaust fumes, and when using hookahs.Symptoms of CO poisoning are nonspecific and can range from dizziness, headache, and angina pectoris to unconsciousness and death.This guideline presents the current state of knowledge and national recommendations on the diagnosis and treatment of patients with CO poisoning.The diagnosis of CO poisoning is based on clinical symptoms and proven or probable exposure to CO. Negative carboxyhemoglobin (COHb) levels should not rule out CO poisoning if the history and symptoms are consistent with this phenomenon. Reduced oxygen-carrying capacity, impairment of the cellular respiratory chain, and immunomodulatory processes may result in myocardial and central nervous tissue damage even after a reduction in COHb.If CO poisoning is suspected, 100% oxygen breathing should be immediately initiated in the prehospital setting.Clinical symptoms do not correlate with COHb elimination from the blood; therefore, COHb monitoring alone is unsuitable for treatment management. Especially in the absence of improvement despite treatment, a reevaluation for other possible differential diagnoses ought to be performed.Evidence regarding the benefit of hyperbaric oxygen therapy (HBOT) is scant and the subject of controversy due to the heterogeneity of studies.If required, HBOT should be initiated within 6 h.All patients with CO poisoning should be informed about the risk of delayed neurological sequelae (DNS).

Abstract In a young man affected by skin soft tissue infections complicated with myositis, the use of hyperbaric oxygen treatment as an adjuvant therapy to surgical debridement and antibiotic therapy could improve management and prognosis.

Fournier’s gangrene (FG) is a Necrotizing Soft Tissue Infection (NSTI) of the perineal region characterized by high morbidity and mortality even if appropriately treated. The main treatment strategies are surgical debridement, broad-spectrum antibiotics, hyperbaric oxygen therapy, NPWT (Negative Pressure Wound Therapy), and plastic surgery reconstruction. We present the case of a 50-year-old woman with an NSTI of the abdomen, pelvis, and perineal region associated with a rectal fistula referred to our department. After surgical debridement and a diverting blow-out colostomy, an NPWT system composed of two sponges connected by a bridge through a rectal fistula was performed. Our target was to obtain healing in a lateral-to-medial direction instead of depth-to-surface to prevent the enlargement of the rectal fistula, promoting granulation tissue growth towards the rectum. This eso-endo-NPWT technique allowed for the primary suture of the perineal wounds bilaterally, simultaneously treating the rectal fistula and the perineum lesions. A systematic review of the literature underlines the spreading of NPWT and its effects.

Abstract Acute carbon monoxide (CO) intoxication during pregnancy causes fetal death and teratogenic effects. Hyperbaric oxygen (HBO2) therapy has the potential to improve them. HBO2 therapy should be considered to treat CO intoxication during pregnancy.

Human existence is known for its phenomenon of expansion and extension in the various natural habitat over a period of time. This has always accompanied with more frequent encounter with the natural inhabitants of that territory. Marine environment is not an exception to this. With increasing encroachment in the marine life, the incidences of the injuries due to marine animals have increased by manifold. Herein we report a case of stingray injury to a diver, its presentation and management. Stingray injuries are one of the rare forms of marine injuries as the fish attacks on provocation or accidentally only. The stingray injury causes both local and systemic effects on humans however in our case the local effects were late. Moreover the local damage was much more as compared to the external appearance of the stinger injury. This is an important aspect of the clinical part of this report as the external appearance of the injury is very deceptive in comparison with the actual damage caused within. He was managed with IV antibiotics, supportive care, timely local wound debridement and hyperbaric oxygen therapy. The response to HBOT was superb and resulted in speedy recovery of the patient. It can be concluded that though the stingray injuries are rare but the disabilities caused by extensive venomous damage to soft tissue and time taken for complete recovery. Systemic effects though not seen in our case may cause sudden death as reported in some case reports.

Hyperbaric oxygen therapy (HBOT) has been suggested as a valuable addition to conventional treatment for a variety of indications, including delayed radiation injury, necrotizing soft tissue infections, and chronic wounds. By increasing the blood oxygen content, HBOT creates a favorable gradient for the diffusion of oxygen into the tissues. In hypoxic tissues, the enhanced oxygen supply has multiple effects that may benefit wound healing.

The aim of this research was to study the parameters of fetal ultrasound imaging and Doppler ultrasound study in pregnant women with extragenital diseases (EGDs) during the treatment regimes with and without hyperbaric oxygen therapy (HBOT). Materials and Methods: A total of 235 pregnant women were examined prospectively at 5 to 40 weeks of gestation. The main group included 191 women with EGDs (anemia, arterial hypertension, chronic pyelonephritis); the control group included 44 women with physiological pregnancy without EGDs. Evaluation of treatment efficacy was based on data from clinical and laboratory findings before treatment and after its completion. The following hardware methods of research were performed: ultrasonography, fetometry, dopplerometric study of fetoplacental complex. Results: Based on data obtained from this study, the following findings were made: - In the early stages of gestation, there were no disturbances in fetoplacental blood circulation. - Starting the 19th week of pregnancy, there is a significant increase in the uterine artery resistive index in pregnant women with arterial hypertension. - In women with a high perinatal risk on the background of the studied EGDs, the third trimester of pregnancy, despite the ongoing conventional treatment, is characterized by persistent impairment in fetoplacental blood circulation. - The inclusion of HBOT in complex therapy in the early stages of pregnancy in women with a high perinatal risk allows leveling out the inevitable disturbances in fetoplacental blood circulation on the background of the studied EGDs.

Abstract Background Acute ischemic stroke is a leading cause of mortality and long-term disability, and profiles of endothelial progenitor cells (EPCs) reflect the degree of endothelial impairment. This study tested the hypothesis that hyperbaric oxygen therapy (HBOT) both improves the clinical short-term outcomes and increases the number of circulating EPCs and antioxidant capacity. Methods The numbers of circulating EPCs [CD133+/CD34+ (%), KDR+/CD34+ (%)], biomarkers for oxidative stress (thiols and thiobarbituric acid-reactive substances), and clinical scores (National Institutes of Health Stroke Scale [NIHSS], Barthel index [BI], and modified Rankin Scale [MRS]) were prospectively evaluated in 25 patients with acute non-cardioembolic stroke under HBOT at two time points (pre- and post-HBOT). The biomarkers and clinical scores were compared with those of 25 age- and sex-matched disease controls. Results The numbers of KDR+/CD34+ (%) in the HBOT group following HBOT increased significantly, whereas the numbers of CD133+/CD34+ (%) also showed a tendency to increase without statistical significance. The mean high-sensitivity C-reactive protein levels showed significant decrease post-HBOT follow-up in the HBOT group. The changes in KDR+/CD34+EPC (%) numbers were positively correlated with changes in clinical outcomes scores (BI, NIHSS, and MRS) in the HBOT group. Conclusions Based on the results of our study, HBOT can both improve short-term clinical outcomes and increase the number of circulating EPCs in patients with acute non-cardioembolic stroke.

Infectious purpura fulminans (PF) is a rare presentation of disseminated intravascular coagulopathy (DIC) due to diffuse intravascular thrombosis and haemorrhagic infarction of the skin. PF can present in infancy/childhood or adulthood and usually presents as ecchymotic skin lesions, fever and hypotension. It is most commonly a consequence of sepsis related to Neisseria meningitidis, Streptococcus pneumoniae or Haemophilus influenzae. Despite aggressive management of sepsis with intravenous fluids, antibiotics, and conventional and nonconventional therapies, the condition still carries a mortality rate of 43%[1]. Streptococcus pneumoniae mostly presents with community-acquired pneumonia. We present a case of PF secondary to DIC related to Pneumococcal sepsis in an otherwise healthy and immunocompetent patient.

Introduction: Carbon monoxide (CO) poisoning may cause severe cellular hypoxia. Materials and methods: A 28-year-old male presented reduced levels of consciousness and dyspnoea after CO exposure. Clinical examination revealed tachypnoea, bilateral rales, dilated jugular veins and confusion. Troponin I, lactate and carboxyhaemoglobin levels were increased. Thoracic X-ray depicted pulmonary oedema and an echocardiogram, severe heart failure (HF; EF55%). He was discharged 1 week later with full recovery. Discussion: Acute HF is a rare serious complication of CO poisoning, even in healthy young individuals.

Abstract Introduction Idiopathic sudden sensorineural hearing loss (ISSNHL) is hearing loss of at least 30 dB in at least 3 contiguous frequencies within at least 72 hours. There are many different theories to explain it, and many different modalities are used for its management, such as: systemic steroids (SSs), intratympanic steroid injection (ITSI), hyperbaric oxygen therapy (HOT), antiviral drugs, and vasodilators or vasoactive substances. Objectives This study aims to evaluate the efficacy of the combination of the most common treatment modalities of ISSNHL and to compare the results if HOT was not one of the treatment modalities administered. Methods The study was conducted with 22 ISSNHL patients with ages ranging from 34 to 58 years. The patients were divided into 2 groups; group A included 11 patients managed by SSs, ITSI, antiviral therapy, and HOT simultaneously, and group B included 11 patients exposed to the aforementioned modalities, with the exception of HOT. Results After one month, all of the patients in group A showed total improvement in hearing in all frequencies, with pure tone average (PTA) of 18.1 ± 2.2, while in group B, 5/11 (45.5%) patients showed total improvement, and 6 /11 (54.5%) patients showed partial improvement, with a total mean PTA of 28.1 ± 8.7. Conclusion The early administration of HOT in combination with other clinically approved modalities (SSs, ITSI, antiviral therapy) provides better results than the administration of the same modalities, with the exception of HOT, in the treatment of ISSNHL.

Introduction: Oxygen is emerging as an important factor in the local regulation of bone remodeling. Some preclinical data suggest that hyperoxia may have deleterious effects on bone cells. However, its clinical relevance is unclear. Hence, we studied the effect of hyperbaric oxygen therapy (HBOT) on serum biomarkers reflecting the status of the Wnt and receptor activator of NF-κB ligand (RANKL) pathways, two core pathways for bone homeostasis. Materials and methods: This was a prospective study of 20 patients undergoing HBOT (mean age 58 yrs., range 35–82 yrs.) because of complications of radiotherapy or chronic anal fissure. Patients were subjected to HBOT (100% oxygen; 2.4 atmospheres absolute for 90 min). The average number of HBOT sessions was 20 ± 5 (range 8–31). Serum hypoxia-inducible factor 1-α (HIF1-α), osteoprotegerin (OPG), RANKL, and the Wnt inhibitors sclerostin and dickkopf-1 (DKK1) were measured at baseline and after HBOT by using specific immunoassays. Results: HIF-1α in eight patients with measurable serum levels increased from 0.084 (0.098) ng/mL at baseline to 0.146 (0.130) ng/mL after HBOT (p = 0.028). However, HBOT did not induce any significant changes in the serum levels of OPG, RANKL, sclerostin or DKK1. This was independent of the patients’ diagnosis, either neoplasia or benign. Conclusion: Despite the potential concerns about hyperoxia, we found no evidence that HBOT has any detrimental effect on bone homeostasis.

Background: Serum concentrations of adhesion molecules and oxidative stress is thought to participate in the pathobiology of secondary brain injury after acute traumatic brain injury (TBI). We aimed to study the hypothesis that hyperbaric oxygen therapy (HBOT) both improves the adhesion molecules levels and antioxidant capacity. Methods: Thirty blood samples from ten patients after acute TBI were obtained after injury and before and after HBOT. Four patients received early HBOT started two weeks after injury, four patients received late HBOT started ten weeks after injury and two patients did not receive HBOT and served as control in this study. The HBOT patients received total 30 times HBOT in six weeks period. Results: Those serum biomarkers in patients with TBI had not significantly difference in glutathione (GSH), thiobarbituric acid reactive substances (TBARS), soluble intercellular cell adhesion-molecule-1 (sICAM-1) and soluble vascular cell adhesion molecule-1 (sVCAM-1) concentrations on admission between early HBOT, late HBOT, and control group (p = 0.916, p = 0.98, p = 0.306, and p = 0.548, respectively). Serum GSH levels were higher at 10 weeks after injury in the early HBOT group than in the late HBOT group and control group (mean, 1.40 μmol/L, 1.16 μmol/L, and 1.05 μmol/L, respectively). Then the serum GSH level was increased at 18 weeks after injury in the late HBOT group (mean, 1.49 μmol/L). However, there was only statistically significant difference at Weeks 18 (p = 0.916, p = 0.463, and p = 0.006, at Week 2, Week 10, and Week 18, respectively). Serum TBARS levels were decreased at 10 weeks after injury in the early HBOT group than in the late HBOT group and control group (mean, 11.21 μmol/L, 17.23 μmol/L, and 17.14 μmol/L, respectively). Then the serum TBARS level was decreased at 18 weeks after injury in the late HBOT group (mean, 12.06 μmol/L). There was statistically significant difference after HBOT (p = 0.98, p = 0.007, and p = 0.018, at Week 2, Week 10, and Week 18, respectively). There was no statistically significant difference between the three groups on sICAM-1 and sVCAM-1 levels from Week 2 to Week 18. Conclusions: HBOT can improve serum oxidative stress in patients after TBI. These molecules may be added as evaluation markers in clinical practice. Perhaps in the future it may also become part of the treatment of patients after acute traumatic brain injury. Further large-scale study may be warrant.

Objective: Oxidative stress (OS) is involved in the pathogenesis of atherosclerosis. Hyperbaric oxygen therapy (HBOT), in which 100% oxygen is inhaled under hyperbaric pressure, may create OS. Therefore, the aim of this research was to measure the serum oxidized low-density lipoprotein (oxLDL) level in patients undergoing HBOT. Methods: Twenty-nine patients who underwent HBOT to treat various diseases were enrolled in this study. The serum oxLDL level was measured at the beginning of the first and after the 10th therapy session. Results: There was no significant difference between the oxLDL level of patients before and after HBOT (4.96+-0.1 vs. 4.94+-0.1 U/mL; p=0.36). Conclusion: HBOT seems to be safe in terms of oxLDL production up to 10 sessions. However, further large-scale studies investigating longer duration of HBOT treatment are required to understand the role of OS.

Abstract Background Although gastrointestinal involvement in patients with granulomatosis with polyangiitis is uncommon, it is associated with mild to severe life-threatening complications. We present a case of pneumatosis cystoides intestinalis in a patient with granulomatosis with polyangiitis that was treated successfully with hyperbaric oxygen. Case presentation A 70-year-old Japanese man with a 3-year history of granulomatosis with polyangiitis consulted our hospital with a complaint of severe back pain. Computed tomography showed a large amount of gas located in his bowel wall and mesentery. He underwent urgent exploratory laparotomy, which led to a diagnosis of pneumatosis cystoides intestinalis without intestinal perforation or necrosis. He consequently underwent 13 sessions of hyperbaric oxygen therapy and was discharged from our hospital without complications. Conclusions Several previous reports have supported the efficacy of hyperbaric oxygen for treating pneumatosis cystoides intestinalis. The present case, however, is the first in which pneumatosis cystoides intestinalis in a patient with granulomatosis with polyangiitis was successfully treated with hyperbaric oxygen. We therefore suggest that hyperbaric oxygen therapy could be a candidate treatment for pneumatosis cystoides intestinalis in patients with granulomatosis with polyangiitis.
