Influenza A virus (IAV) was a neglected swine pathogen in South America before the 2009 H1N1 pandemic (A(H1N1)pdm2009). The A(H1N1)pdm2009 strain has widely spread among the Chilean swine population and co-circulates with endemic H1N2 and H3N2 viruses. The presence of IAV as a swine pathogen in Chilean swine before the 2009 pandemic is unknown. To understand the IAV in swine prior to 2009, a retrospective study of samples from pigs affected with respiratory diseases was conducted. Ninety formalin-fixed and paraffin-embedded (FFPE) lung tissues belonging to 21 intensive pig production companies located in five different administrative regions of Chile, collected between 2005 and 2008, were evaluated. The tissues were tested by immunohistochemistry (IHC), identifying that 9 out of 21 farms (42.8%) and 31 out of 90 (34.4%) samples were IAV positive. Only three out of the 31 IHC positive samples were positive upon RNA extraction and rtRT-PCR analysis. Partial nucleotide sequences were obtained from one sample and characterized as an H3N2 subtype closely related to a human seasonal H3N2 IAVs that circulated globally in the mid-90s. These results indicate that IAV was circulating in swine before 2009 and highlight the value of conducting retrospective studies through genomic strategies to analyze historical samples. The correlation of viral growth capability (n = 156) with the viral load in nasopharyngeal swabs (n = 76) was assessed. Epidemic influenza A/H1N1, A/H3N2, and B viruses showed a wide range of growth capability (104–1011 copies/mL) in Madin-Darby canine kidney cells. The growth was correlated with the nasopharyngeal viral load (r = 0.53). Six selected strains showed growth-dependent cell death (r = 0.96) in a growth kinetics assay. Epidemic influenza viruses exhibit a wide range of growth capability. Growth capability should be considered one of the key factors in disease prognosis.

This study estimates the overall effect of two influenza vaccination programs consecutively administered in a cluster-randomized trial in western Senegal over the course of two influenza seasons from 2009 to 2011. We apply cutting-edge methodology combining social contact data with infection data to reduce bias in estimation arising from contamination between clusters. Our time-varying estimates reveal a reduction in seasonal influenza from the intervention and a non-significant increase in H1N1 pandemic influenza. We estimate an additive change in overall cumulative incidence (which was 6.13% in the control arm) of -0.68 percentage points during Year 1 of the study (95% CI: −2.53, 1.18). When H1N1 pandemic infections were excluded from analysis, the estimated change was −1.45 percentage points and was significant (95% CI, −2.81, −0.08). Because cross-cluster contamination was low (0–3% of contacts for most villages), an estimator assuming no contamination was only slightly attenuated (−0.65 percentage points). These findings are encouraging for studies carefully designed to minimize spillover. Further work is needed to estimate contamination – and its effect on estimation – in a variety of settings.

The influenza A/H1N1 pandemic in 2009 taught us that the monitoring of vaccine benefits and risks in Europe had potential for improvement if different public and private stakeholders would collaborate better (public health institutes (PHIs), regulatory authorities, research institutes, vaccine manufacturers). The Innovative Medicines Initiative (IMI) subsequently issued a competitive call to establish a public-private partnership to build and test a novel system for monitoring vaccine benefits and risks in Europe. The ADVANCE project (Accelerated Development of Vaccine benefit-risk Collaboration in Europe) was created as a result. The objective of this paper is to describe the perspectives of key stakeholder groups of the ADVANCE consortium for vaccine benefit-risk monitoring and their views on how to build a European system addressing the needs and challenges of such monitoring. These perspectives and needs were assessed at the start of the ADVANCE project by the European Medicines Agency together with representatives of the main stakeholders in the field of vaccines within and outside the ADVANCE consortium (i.e. research institutes, public health institutes, medicines regulatory authorities, vaccine manufacturers, patient associations). Although all stakeholder representatives stated they conduct vaccine benefit-risk monitoring according to their own remit, needs and obligations, they are faced with similar challenges and needs for improved collaboration. A robust, rapid system yielding high-quality information on the benefits and risks of vaccines would therefore support their decision making. ADVANCE has developed such a system and has tested its performance in a series of proof of concept (POC) studies. The system, how it was used and the results from the POC studies are described in the papers in this supplementary issue.

Background To increase the influenza vaccination rate in Chiburijima Island, we diversified vaccination places from a single clinic to the seven town halls from all the districts. Methods We retrospectively analyzed influenza vaccination coverage, vaccinees' district of residence, and location of vaccination place, from prevaccination screening questionnaires between October 2007 and March 2018. Except for home‐visiting vaccination services, before 2011, influenza vaccination was provided only at a single clinic; after 2012, it was provided at the seven town halls in addition to the clinic. We calculated the vaccination rates among residents of Chiburijima Island, stratified by age groups and districts of residence. Results Estimated influenza vaccination rates for all ages increased from 38% in 2007 to 58% in 2017. There was a 14% increase in coverage in 2010 (the year following the 2009 H1N1 pandemic) and a slight increasing trend after 2012. The vaccination coverage of residents, particularly of people ≥65 years old and living >1 km from the former vaccination place, increased after diversification of the vaccination place. Conclusions Influenza vaccination rate in Chiburijima Island increased by 20% between 2007 and 2017, although the rate among the elderly Japanese people decreased by 4% based on the national data of routine immunization. Shortening the distance to vaccination places might improve the coverage as elderly people are often restricted due to transportation means. Sustained and multifaceted strategies are necessary for better immunization coverage.

During the H1N1 outbreak of 2009, local public health units engaged in consultations with various levels of government to develop clinical practice guidelines. These guidelines provide specific clinical considerations around prevention, management, and treatment associated with the particular pathogen involved and are used by frontline healthcare professionals across many healthcare settings. In this article, we report on the lessons learned by Medical Officers of Health from across Canada on the guideline development and deployment processes and provide suggestions to improve guidelines development and deployment during future pandemic situations.

International audience; The French-style organization in the field of rare diseases allows a close contact between reference centres and regional pharmacovigilance centres thanks to their implementation within the French university hospital. This collaboration leads to highlight more and more drug-induced rare diseases. Through several historical examples (eosinophilia-myalgia syndrome due to L-tryptophan, type 1 narcolepsy with H1N1 pandemic influenza vaccine, capillary leak syndrome, acquired von Willebrand syndrome), it remains clear that pharmacovigilance is the cornerstone of the alert system. Clinicians from the rare disease reference centres can easily report adverse drug reactions (ADRs) to pharmacologists from their regional pharmacovigilance centre. Through experience, collaboration between countries, large database, and sometimes pharmacoepidemiological studies, an alert can then be raised. This collaboration underlines also similarities between the two disciplines, through the frequency of ADRs and diseases, the difficulty of the diagnosis in front of scarce data, and through the unusual worsening symptoms. Patients and associations of patients play also a proactive role as research partners at different steps, to quantify and qualify symptoms and ADRs, and also to develop orphan drugs. These several collaborations are a precious tool to improve patients' outcomes. These close contacts between the different actors are important to make earlier diagnosis of rare diseases and severe ADRs. Rare disease does not have to mean overlooked diseases.

Aim. In this study we developed and characterized a mouse model of secondary S. aureus and S. pneumoniae pneumonia following influenza virus infection with H1N1 pandemic and laboratory strains and their reassortment. Materials and methods. BALB/с mice were infected intranasally with A/California/04/2009/(H1N1 pndm), A/Puerto Rico/8/34 or their reassortment NIBRG-121xp followed by different strains of S. аureus и S. pneumoniae. The pathogenicity of infection was assessed by mouse survival and weight change, viral titre and bacterial count in the lungs. Results. It was shown that the infection of mice with three strains of the H1N1 influenza virus with a comparable level of pathogenicity leads to a different severity of secondary bacterial infection. The mouse adapted A/California/04/2009 pandemic strain possessed the greatest ability to alter antibacterial immunity. Conclusion. An experimental model of post-influenza bacterial pneumonia utilizing three strains of the H1N1 influenza virus and various strains of S. aureus or S. pneumoniae was established. The ability of viruses to provoke bacterial superinfection of different severity is characterized.

Background: Health care professionals are a high risk group for H1N1 and it is important for them to have correct knowledge about the disease in order to be able to prevent themselves from the disease.Objectives: To assess the knowledge, attitude and practices regarding H1N1 pandemic influenza among the health care providers working at a tertiary care hospital.Methodology: Data was collected from 279 health care providers who were selected by convenience sampling.Results: Overall the participants had some/sufficient knowledge about the disease. 44.8% of the study participants had received training on hand hygiene while only 24% had received training on the use of personal protective equipment. Only 54.8 % of the study participants were vaccinated against H1N1.Conclusion: It was seen that there were some gaps in the knowledge about the disease. Educational and training programmes for health care providers should be conducted regularly.

Lower respiratory infections remain one of the top global causes of death and the emergence of new diseases continues to be a concern. In the first two decades of the 21st century, we have born witness to the emergence of newly recognized coronaviruses that have rapidly spread around the globe, including severe acute respiratory syndrome virus (SARS) and Middle Eastern respiratory syndrome virus (MERS). We have also experienced the emergence of a novel H1N1 pandemic influenza strain in 2009 that caused substantial morbidity and mortality around the world and has transitioned into a seasonal strain. Although we perhaps most frequently think of viruses when discussing emerging respiratory infections, bacteria have not been left out of the mix, as we have witnessed an increase in the number of infections from Legionella spp. since the organisms' initial discovery in 1976. Here, we explore the basic epidemiology, clinical presentation, histopathology, and clinical laboratory diagnosis of these four pathogens and emphasize themes in humans' evolving relationship with our natural environment that have contributed to the infectious burden. Histology alone is rarely diagnostic for these infections, but has been crucial to bettering our understanding of these diseases. Together, we rely on the diagnostic acumen of pathologists to identify the clinicopathologic features that raise the suspicion of these diseases and lead to the early control of the spread in our populations.

Background The high volume of US-Mexico land border crossings can facilitate international dissemination of influenza viruses. Methods We surveyed adult pedestrians crossing into the United States at two international land ports of entry to assess vaccination coverage during the 2009H1N1 influenza pandemic and 2011–2012 influenza season. Results Of 559 participants in 2010, 23.4% reported receipt of the 2009H1N1 vaccine. Of 1423 participants in 2012, 33.7% received the 2011–2012 influenza vaccine. Both years, those crossing the border ≥8 times per month had lower vaccination coverage than those crossing less frequently. US-border residents had lower H1N1 coverage than those in other locations. Vaccination coverage was higher for persons age ≥65 years and, in 2010 only, those with less than high school education. Although most participants believed it is important to get vaccinated, only half believed the influenza vaccine was safe and effective. The main reasons for not receiving the influenza vaccine were beliefs of low risk of disease, time constraints, and concerns about vaccine safety (in 2010) or efficacy (in 2012). Conclusions International land border crossers are a large and unique category of travelers that require targeted binational strategies for influenza vaccination and education.

One hundred years have passed since the 1918 influenza pandemic caused substantial illness globally, with an estimated 50 million deaths. A number of factors, including World War I, contributed to the spread of the pandemic virus, which often caused high symptomatic attack rates and severe illness. Major achievements over the last 100 years have been made in influenza prevention, diagnosis, and treatment; however, the potential for a severe pandemic to emerge remains unchanged. We provide a review of the historical context and clinical aspects of illness due to the influenza A(H1N1) virus as it emerged and spread in 1918, with a focus on the experience in the United States. Understanding the significant social disruption and burden of illness from the 1918 pandemic can help us imagine the possible impacts of a high severity pandemic if it were to emerge now.

Background: A population‐based pregnancy cohort was established in Norway to study potential effects of exposure to the 2009 influenza pandemic or pandemic vaccination during pregnancy. Objectives: We studied maternal A(H1N1)pdm09‐specific hemagglutination inhibition (HI)‐titer levels and waning in women with influenza‐like illness (ILI) in pregnancy compared to vaccinated women. Moreover, we studied the association between HI‐titers and self‐reported severity and duration of ILI. Methods: HI‐titers against the pandemic virus were measured in maternal blood samples obtained at birth, 3‐9 months after exposure, and linked with information about pregnancy, influenza and vaccination from national registries and a cohort questionnaire. Results: Among 1821 pregnant women included, 43.7% were unvaccinated and 19.3% of these had ILI. HI‐titers were low (geometric mean titer (GMT) 11.3) in the unvaccinated women with ILI. Higher HI‐titers (GMT 37.8) were measured in the vaccinated women. Estimated HI‐titer waning was similar for vaccinated women and women with ILI. Most ILI episodes were moderate and lasted 3‐5 days. Women with ILI reporting specific influenza symptoms such as fever or cough had higher HI‐titers than women without these symptoms. Women who reported being “very ill” or illness duration of >5 days had higher HI‐titers than women reporting less severe illness or illness of shorter duration, respectively. Conclusions: Antibody waning was similar in vaccinated women and women with ILI. More severe ILI or longer duration of illness was associated with higher HI‐titers. Most unvaccinated pregnant women with ILI had low HI‐titers, probably due to moderate illness and HI‐titer waning between exposure and sampling. publishedVersion

The 2009 H1N1 pandemic and seasonal influenza outbreaks have, at times, overwhelmed capacity in children's hospitals.1 The COVID-19 pandemic is different. The 2019/2020 influenza season in the United States began earlier than any season since the 2009 H1N1 pandemic, with an increase in influenza-like illnesses observed as early as August. Also noteworthy was the numerical domination of influenza B cases early in this influenza season, in contrast to their typically later peak in the past. Here, we dissect the 2019/2020 influenza season not only with regard to its unusually early activity, but also with regard to the relative dynamics of type A and type B cases. We propose that the recent expansion of a novel influenza B/Victoria clade may be associated with this shift in the composition and kinetics of the influenza season in the United States. We use epidemiological transmission models to explore whether changes in the effective reproduction number or short-term cross-immunity between these viruses can explain the dynamics of influenza A and B seasonality. We find support for an increase in the effective reproduction number of influenza B, rather than support for cross-type immunity-driven dynamics. Our findings have clear implications for optimal vaccination strategies.

As courts throughout the country decide coronavirus-related liability issues, judges can look to claims that arose out of three recent health crises: the H1N1 pandemic, the Ebola crisis, and the Zika crisis. This Article makes a novel contribution to this emerging legal literature on the coronavirus pandemic by analyzing the cases that rose out of these crises and how these past decisions may apply to fact patterns that arise out of the current coronavirus pandemic.

The efficacy of anti-influenza drugs (neuraminidase inhibitor—NAI) has been examined before and after the 2009 H1N1 pandemic. Recent studies indicated that NAI administration reduced risk of lower respiratory tract complications, and admittance to hospital, moreover, shortened the duration of fever and viral RNA shedding. NAI administration was also associated with reduced ICU length of stay and mortality.

Background: On February 26, 2020, the first case of coronavirus disease 2019 (COVID-19) was detected in Israel. The Ministry of Health (MoH) instructed people to take isolation measures and restrict their movement. Similarly, there was a gradual decrease in the number of visits to our emergency department (ED). Objectives: To describe the decline in the referrals to the ED and in-hospital beds occupancy during the COVID-19 pandemic and to compare it to the H1N1 2009 pandemic. Methods: Employing a cross-sectional epidemiologic study, the pattern of visits to the ED during the COVID-19 was compared with the pattern of visits during the 2009 H1N1 pandemic, as well as a year without a pandemic. The data was adjusted to consider changes in population size. The Welch t test for unpaired, unequal samples was used to analyze the data. Results: Within 2 months of the COVID-19 outbreak, the average number of visits to the ED dropped by 30.2% and the hospital occupancy by 29.2% (a minimum of 57%), compared to the same period, the year before. In comparison to the same period during the H1N1 outbreak, we witnessed a significant decline in the number of visits to the ED during the COVID-19 outbreak. Conclusion: The behavior of people during the COVID-19 pandemic was different from their behavior during the H1N1 pandemic. People seemed to avoid visiting the ED. The boundary between precaution and panic in the generation of the media could be very thin. Decision-makers must take this into account.

The coronavirus disease (COVID-19) caused by the severe acute respiratory syndrome coronavirus (SARS-CoV-2) rapidly became a public health emergency of international concern requiring urgent attention. It emerged late in 2019 in China as a respiratory pathogen, with the first epicentres observed in Iran, Spain and northern Italy. At the time of writing there were about 4 525 497 cases and 307 395 deaths worldwide, of which 1 409 452 cases and 85 860 deaths were in the USA, which is the most affected country in absolute terms.[1] The COVID-19 pandemic has been marked by extreme heterogeneity in both geography and population across the world. Consequently, leveraging local data aligned to the unique South African (SA) context is critical. Moreover, comprehensive mathematical models should integrate the full complexity of transmission dynamics observed with COVID-19 in the context of rapidly changing individual, social and structural determinants in SA

The COVID-19 pandemic has been characterized by an unprecedented amount of published scientific articles. The aim of this study is to assess the type of articles published during the first 3 months of the COVID-19 pandemic and to compare them with articles published during 2009 H1N1 swine influenza pandemic. Two operators independently extracted and assessed all articles on COVID-19 and on H1N1 swine influenza that had an abstract and were indexed in PubMed during the first 3 months of these pandemics. Of the 2482 articles retrieved on COVID-19, 1165 were included. Over half of them were secondary articles (590, 50.6%). Common primary articles were: human medical research (340, 59.1%), in silico studies (182, 31.7%) and in vitro studies (26, 4.5%). Of the human medical research, the vast majority were observational studies and cases series, followed by single case reports and one randomized controlled trial. Secondary articles were mainly reviews, viewpoints and editorials (373, 63.2%). Limitations were reported in 42 out of 1165 abstracts (3.6%), with 10 abstracts reporting actual methodological limitations. In a similar timeframe, there were 223 articles published on the H1N1 pandemic in 2009. During the COVID-19 pandemic there was a higher prevalence of reviews and guidance articles and a lower prevalence of in vitro and animal research studies compared with the H1N1 pandemic. In conclusions, compared to the H1N1 pandemic, the majority of early publications on COVID-19 does not provide new information, possibly diluting the original data published on this disease and consequently slowing down the development of a valid knowledge base on this disease. Also, only a negligible number of published articles reports limitations in the abstracts, hindering a rapid interpretation of their shortcomings. Researchers, peer reviewers, and editors should take action to flatten the curve of secondary articles. We aimed to compare HLA-DQB1-associations in narcolepsy type 1 (NT1) patients with disease onset before and after the 2009 H1N1 pandemic in a large Dutch cohort. 525 NT1 patients and 1272 HLA-DQB1*06:02-positive healthy controls were included. Because of the discussion that has arisen on the existence of sporadic and post-H1N1 NT1, HLA-DQB1-associations in pre- and post-H1N1 NT1 patients were compared. The associations between HLA-DQB1 alleles and NT1 were not significantly different between pre- and post-H1N1 NT1 patients. Both HLA-DQB1-associations with pre- and -post H1N1 NT1 reported in recent smaller studies were replicated. Our findings combine the results of studies in pre- and post-H1N1 NT1 and argue against considering post-H1N1 NT1 as a different entity.

This paper discusses the ways in which 2009 novel swine‐origin influenza A (H1N1) was announced and resonated with current pandemic anxieties. In particular, the US Centers for Disease Control and Prevention (CDC) are used as a lens through which recent pandemic anxieties can be analysed and understood. This entails a closer look at the securitisation of public health and the challenges and struggles this may have caused within public health agencies. In that light, CDC' formal entanglement with global health security and its announcement of the H1N1 pandemic are interpreted, followed by an ethnographically informed focus on various people who were engaged in the H1N1 emergency response and their practices and practical struggles in the face of pandemic anxiety.

Existing literature suggests that hospital occupancy matters for quality of care, as measured by various patient outcomes. However, estimating the causal effect of increased hospital busyness on in-hospital mortality remains an elusive task due to statistical power challenges and the difficulty in separating shocks to occupancy from changes in patient composition. Using data from a large public hospital system in Mexico, we estimate the impact of congestion on in-hospital mortality by exploiting the shock in hospitalizations induced by the 2009 H1N1 pandemic, instrumenting hospital admissions due to acute respiratory infections (ARIs) with measures of ARI cases at nearby healthcare facilities as a proxy for the size of the local outbreak. Our instrumental-variables estimates show that a 1% increase in ARI admissions in 2009 led to a 0.25% increase in non-ARI in-hospital mortality. We show that these effects are nonlinear in the size of the local outbreak, consistent with the existence of tipping points. We further show that effects are concentrated at hospitals with limited infrastructure, suggesting that supply-side policies that improve patient assignment across hospitals and strategically increase hospital capacity could mitigate some of the negative impacts. We discuss managerial implications, suggesting that up to 25%–30% of our estimated deaths at small and non-intensive-care-unit hospitals could have been averted by reallocating patients to reduce congestion. Introduction: Influenza A (H1N1) is the most recent of the pandemic diseases that has affected the world’s population. The aim of this study was to assess knowledge, attitudes and behaviours regarding H1N1 pandemic among Malaysian dental students. Methodology: The data was collected using a self-administered questionnaire and results were analysed using SPSS version 15. Results: The most common symptoms identified were fever (50.8%), breathing difficulty (41.0%), and cough (39.3%). It was considered to be a moderately dangerous infection by approximately one third of the students (31.1%). Majority (88.9%) of the students agreed that washing hands with soap is the most effective preventive measure. It was followed by travel restriction that was reported by 77% of them as a preventive measure for the pandemic. Conclusion: This study concluded that a large number of students did not have complete awareness about disease transmission, preventive measures, vaccinations, and treatments available.

Influenza pandemics considerably burden affected health systems due to surges in inpatient admissions and associated costs. Previous studies underestimate or overestimate 2009/2010 influenza A/H1N1 pandemic hospital admissions and costs. We robustly estimate overall and age‐specific weekly H1N1 admissions and costs between June 2009 and March 2011 across 170 English hospitals. We calculate H1N1 admissions and costs as the difference between our administrative data of all influenza‐like‐illness patients (seasonal and pandemic alike) and a counterfactual of expected weekly seasonal influenza admissions and costs established using time‐series models on prepandemic (2004–2008) data. We find two waves of H1N1 admissions: one pandemic wave (June 2009–March 2010) with 10,348 admissions costing £20.5 million and one postpandemic wave (November 2010–March 2011) with 11,775 admissions costing £24.8 million. Patients aged 0–4 years old have the highest H1N1 admission rate, and 25‐ to 44‐ and 65+‐year‐olds have the highest costs. Our estimates are up to 4.3 times higher than previous reports, suggesting that the pandemic's burden on hospitals was formerly underassessed. Our findings can help hospitals manage unexpected surges in admissions and resource use due to pandemics.

During the 2009 H1N1 pandemic, individuals with obesity were disproportionately affected by H1N1 with increased levels of mortality and morbidity. This led to questions regarding the potential impact of lifestyle on the effectiveness of immunization. Currently, the research is limited on influenza vaccination and the associated changes in immune response with body composition and physical activity. The purpose of this pilot study was to investigate the potential role of adiposity and physical activity in the immune response elicited by the 2014/2015 seasonal trivalent influenza vaccine. A prospective cohort study examining the 2014/2015 seasonal trivalent influenza vaccine was conducted by collecting baseline and 4-week postvaccination fasting blood samples from 45 male Albertans between the ages of 18 and 35 years. Percent body fat (%BF) was assessed through dual X-ray absorptiometry imagining and physical activity through self-reported survey scores. While no differences in median %BF were associated with seroconversion rates in participants, the median physical activity score was higher among those that did not seroconvert to the vaccine. Significant differences were found for the A/Texas strain (p < 0.01) and a similar trend of lower magnitude observed for the remaining 2 influenza strains. These results suggest that higher physical activity levels may influence immune response to vaccination and that assessing factors beyond those commonly used can be of value when identifying vaccine response in the population.

Not all individuals with Influenza like illness (ILI) seek healthcare. Knowing the proportion that do is important to evaluate the actual burden and fatality rate of ILI-relevant diseases, such as seasonal influenza and human infection with avian influenza. A number of studies have investigated the healthcare seeking rate, but the results varied from 0.16 to 0.85. We conducted this analysis for better understanding the healthcare seeking rate for ILI, and providing fundamental data for researchers in relevant fields.In this meta-analysis, a total of 799 articles, published as of 13 December 2016, were retrieved from Pubmed, Embase, Web of Science and Cochrane, and 11 of them were included after screening. The pooled estimates and factors which influence healthcare seeking rates were analysed.The overall pooled healthcare seeking rate was 0.52 (95% CI: 0.46-0.59). The rate was significantly higher during the H1N1 pandemic in 2009 (0.61, 95% CI: 0.51-0.74), in children (0.56, 95% CI: 0.55-0.57) and in patients with documented fever (0.62, 95% CI: 0.53-0.72) than during non-pandemic periods (0.39, 95% CI: 0.33-0.45), in adults (0.45, 95% CI: 0.42-0.48) and in patients without documented fever (0.44, 95% CI: 0.38-0.50). Meta-regression indicated that these three factors could jointly explain 70.1% of the total heterogeneity among published studies.The healthcare seeking rate of ILI patients is needed for estimation of the burden of ILI in the general population based on data from routine ILI sentinel surveillance systems.

The Canadian Pandemic Influenza Preparedness: Planning Guidance for the Health Sector (CPIP) is a guidance document that outlines key health sector preparedness activities designed to ensure Canada is ready to respond to the next influenza pandemic. This article outlines Canada’s pandemic influenza surveillance strategy as described in the CPIP Surveillance Annex. The strategy builds on the surveillance activities used for seasonal influenza and incorporates lessons learned from the 2009 H1N1 pandemic, including improved information sharing, improved electronic links among federal, provincial and territorial (FPT) partners and improved surveillance for Indigenous communities. Key elements of the surveillance strategy include early detection and investigation of a novel influenza virus through the reporting of cases or clusters of severe acute respiratory infections and laboratory detections of novel influenza viruses. Community-based surveillance will provide information on clinical severity, age groups affected and risk factors associated with severe disease. Severe outcome surveillance will capture data on hospitalizations and deaths. Laboratory surveillance will include weekly reports of respiratory virus detections. The response activities are adaptable to the demands of different levels of pandemic activity and impact, supported by a set of triggers for the activation and deactivation. Surveillance will be linked with other response components, such as communications, research, assessment and evaluation. This is an evergreen document that will be updated regularly.

Influenza pandemic occurs when a new strain from other animal species overcomes the inter-species barriers and supports rapid human-to-human transmission. A critical prerequisite to this process is that hemagglutinin (HA) acquires a few key mutations to switch from avian receptors to human receptors. Previous studies suggest that H1 and H2/H3 HAs use different sets of mutations for the switch. This report shows that HA from the 1918 H1N1 pandemic virus (1918H1 HA) adopts the set of mutations used by H2/H3 HAs in receptor-preference switch when its 130-loop is made similar to those of H2/H3 HAs. Thus, the 130-loop appears to be the key determinant for the different mutations employed by pandemic H1 or H2/H3 HA. The correlation of the mutational routes and the 130-loop as unraveled in this study opens the door for efficient investigation of mutations required by other HA subtypes for inter-human airborne transmission.

Following the 2009/10 influenza A/H1N1 pandemic, it was recognised that pregnant women were disproportionately represented in cases of hospitalisation and complications of the infection. This, coupled with evidence showing a strong link between influenza infection in pregnancy and increased risk of miscarriage, low birth weight, premature birth, and maternal morbidity and mortality, led to the introduction of pregnant women as an ‘at risk’ group who, in 2010, were recommended to receive annual seasonal influenza immunisation. Uptake of the immunisation remains relatively low, with only 44.9% of women in England receiving the vaccination in the 2016/17 season. Midwives should discuss the potential risks of influenza with pregnant women, promoting vaccination and administering it where commissioned and able to do so, or signposting them to their GP or local pharmacy to receive it, thereby protecting the health of the women and babies.

Highlights • We examine the impact of the H1N1 pandemic in Mexico on health outcomes. • The pandemic led to a decline in diarrheal diseases among children 5 and under. • The unexpected nature and magnitude of the pandemic led to changes in behavior. • Implications for the production of health outcomes and policy are discussed., Worldwide, the leading causes of death could be avoided with health behaviors that are low-cost but also difficult to adopt. We show that exogenous health shocks could facilitate the adoption of these behaviors and provide long-lasting effects on health outcomes. Specifically, we exploit the spatial and temporal variation of the 2009 H1N1 influenza pandemic in Mexico and show that areas with a higher incidence of H1N1 experienced larger reductions in diarrhea-related cases among young children. These reductions continue even three years after the shock ended. Health improvements and evidence of information seeking via Google searches were consistent with changes in hand washing behaviors. Several robustness checks validate our findings and mechanism.

The role of extracorporeal support in form of extracorporeal membrane oxygenation (ECMO) has been well established in cases of acute cardiopulmonary failure. The initial enthusiasm dampened, and there was no new development in this field before the recent H1N1 pandemic. Encouraging results in H1N1 patients saw a revised interest in use of ECMO. Gradually, the scope of ECMO widened and is now being used for various indications both in pediatric and adult patients. One of the newly added roles of ECMO is to provide interval support for organ retrieval in cases of donation after cardiac death (DCD). In this article, we will try to elaborate the concept and outcomes of the use of ECMO in donation after cardiac death.

Medical countermeasures, including new drugs and vaccines, are necessary to protect the public's health from novel diseases and terrorist threats. Experience with the 2001 anthrax attack and the 2009 H1N1 pandemic suggest that there is limited willingness to accept such drugs and that minority groups may respond differently from others. We conducted 148 intercept interviews in the metropolitan Washington, DC, area, examining 2 hypothetical scenarios: a new respiratory virus and public exposure to high levels of radiation. Findings provide insights into key factors that affect whether diverse members of the public comply with recommended protective actions like taking emergency authorized vaccines. These insights can help improve how public health practitioners communicate during uncertain times.

With influenza vaccination rates in the United States recently exceeding 45% of the population, it is important to understand the impact that vaccination is having on influenza transmission. In this study, we used a Bayesian modeling approach, combined with a simple dynamical model of influenza transmission, to estimate this impact. The combined framework synthesized evidence from a range of data sources relating to influenza transmission and vaccination in the United States. We found that, for seasonal epidemics, the number of infections averted ranged from 9.6 million in the 2006-2007 season (95% credible interval (CI): 8.7, 10.9) to 37.2 million (95% CI: 34.1, 39.6) in the 2012-2013 season. Expressed in relative terms, the proportion averted ranged from 20.8% (95% CI: 16.8, 24.3) of potential infections in the 2011-2012 season to 47.5% (95% CI: 43.7, 50.8) in the 2008-2009 season. The percentage averted was only 1.04% (95% CI: 0.15, 3.2) for the 2009 H1N1 pandemic, owing to the late timing of the vaccination program in relation to the pandemic in the Northern hemisphere. In the future, further vaccination coverage, as well as improved influenza vaccines (especially those offering better protection in the elderly), could have an even stronger effect on annual influenza epidemics.

The 2009 influenza A H1N1 pandemic was responsible for considerable global morbidity and mortality. In 2009, several research studies in the UK were rapidly funded and activated for clinical and public health actions. However, some studies were too late for their results to have an early and substantial effect on clinical care, because of the time required to call for research proposals, assess, fund, and set up the projects. In recognition of these inherent delays, a portfolio of projects was funded by the National Institute for Health Research in 2012. These studies have now been set up (ie, with relevant permissions and arrangements made for data collection) and pilot tested where relevant. All studies are now on standby awaiting activation in the event of a pandemic being declared. In this Personal View, we describe the projects that were set up, the challenges of putting these projects into a maintenance-only state, and ongoing activities to maintain readiness for activation, and discuss how to plan research for a range of major incidents.

Background The use of extracorporeal membrane oxygenation (ECMO) in France has increased since the H1N1 pandemic in 2009. By contrast, neonatal and pediatric ECMO support in France was known to be limited to a few centers offering congenital cardiac surgery. The purpose of this survey conducted in 2017 was to identify the neonatal and pediatric ECMO centers in France as well as networks existing between ECMO and non-ECMO centers. Results Seventy-two neonatal or pediatric intensive care unit medical directors answered the survey (84% of the centers surveyed). Twenty were identified as ECMO centers, defined as a unit able to start ECMO with its own resources. ECMO centers ranged from 470,000 to 1,180,000 inhabitants (neonates or children under 18). Thirteen of them (65%) reported that they were affiliated with a congenital cardiac surgery department. A total of 187 patients were supported with ECMO in these centers in 2016. Only six of these centers estimated an activity greater than 15 cases per year over the last 5 years. Nearly 30% of ECMO runs were indicated before or after congenital heart surgery. Four of the ECMO centers offered off-site facilities (mobile team). Non-ECMO centers are likely to be neonatal intensive care units. Nine of them (18.7%) declared knowing an ECMO center that provided mobile care with predefined organization, 11 (22.9%) reported knowing an ECMO center providing a mobile activity without predefined organization, nine (18.%), and 18 (37.5%) ICUs declared they knew of the existence of an ECMO program but did not report any possibility of mobile care or any procedure for transfer. Conclusions Of the centers reporting the highest case volumes, four offered mobile ECMO abilities. Well-organized networks for the most severe neonates and children were not identified in France.

The seasonality of individual influenza subtypes/lineages and the association of influenza epidemics with meteorological factors in the tropics/subtropics have not been well understood. The impact of the 2009 H1N1 pandemic on the prevalence of seasonal influenza virus remains to be explored. Using wavelet analysis, the periodicities of A/H3N2, seasonal A/H1N1, A/H1N1pdm09, Victoria and Yamagata were identified, respectively, in Panzhihua during 2006–2015. As a subtropical city in southwestern China, Panzhihua is the first industrial city in the upper reaches of the Yangtze River. The relationship between influenza epidemics and local climatic variables was examined based on regression models. The temporal distribution of influenza subtypes/lineages during the pre-pandemic (2006–2009), pandemic (2009) and post-pandemic (2010–2015) years was described and compared. A total of 6892 respiratory specimens were collected and 737 influenza viruses were isolated. A/H3N2 showed an annual cycle with a peak in summer–autumn, while A/H1N1pdm09, Victoria and Yamagata exhibited an annual cycle with a peak in winter–spring. Regression analyses demonstrated that relative humidity was positively associated with A/H3N2 activity while negatively associated with Victoria activity. Higher prevalence of A/H1N1pdm09 and Yamagata was driven by lower absolute humidity. The role of weather conditions in regulating influenza epidemics could be complicated since the diverse viral transmission modes and mechanism. Differences in seasonality and different associations with meteorological factors by influenza subtypes/lineages should be considered in epidemiological studies in the tropics/subtropics. The development of subtype- and lineage-specific prevention and control measures is of significant importance.

Predicting the magnitude of the annual seasonal peak in influenza-like illness (ILI)-related emergency department (ED) visit volumes can inform the decision to open influenza care clinics (ICCs), which can mitigate pressure at the ED. Using ILI-related ED visit data from the Alberta Real Time Syndromic Surveillance Net for Edmonton, Alberta, Canada, we developed (training data, 1 August 2004–31 July 2008) and tested (testing data, 1 August 2008–19 February 2014) spatio-temporal statistical prediction models of daily ILI-related ED visits to estimate high visit volumes 3 days in advance. Our Main Model, based on a generalised linear mixed model with random intercept, incorporated prediction residuals over 14 days and captured increases in observed volume ahead of peaks. During seasonal influenza periods, our Main Model predicted volumes within ±30% of observed volumes for 67%–82% of high-volume days and within 0.3%–21% of observed seasonal peak volumes. Model predictions were not as successful during the 2009 H1N1 pandemic. Our model can provide early warning of increases in ILI-related ED visit volumes during seasonal influenza periods of differing intensities. These predictions may be used to support public health decisions, such as if and when to open ICCs, during seasonal influenza epidemics.

OBJECTIVES To determine the overall use of extracorporeal membranous oxygenation for influenza-associated illness and describe risk factors associated with mortality in these patients. DESIGN Retrospective multicenter cohort analysis. SETTING The international Extracorporeal Life Support Organization database was queried for patients with influenza-associated illness on extracorporeal membranous oxygenation from 1992 to 2014. PATIENTS In total, 1,654 patients with influenza-associated illness on extracorporeal membranous oxygenation. INTERVENTIONS None. MEASUREMENTS AND MAIN RESULTS Demographic and clinical data collected included age, type of support, duration of support, type of microbial codetection, complications, and survival status at discharge. The primary outcome of interest was survival to hospital discharge. From 1992 to 2014, 1,688 (3%) of the 61,336 extracorporeal membranous oxygenation runs were due to influenza-associated illness reflecting 1,654 unique patients: 30 (2%) were neonates, 521 (31%) were pediatric patients, and 1,103 (67%) were adults. Extracorporeal membranous oxygenation use for influenza-associated illness increased from 1992 to 2014, with a marked increase in use after the 2009 H1N1 pandemic. Survival to hospital discharge of patients with influenza-associated illness on extracorporeal membranous oxygenation was 63% and was not affected by bacterial codetection. However, when patients with Staphylococcus aureus codetection were compared with those with another bacterial codetection, their survival to hospital discharge was significantly lower (52% vs 67%; p < 0.01). In a logistic regression model, the effect of S. aureus on in-hospital mortality varied by age group, with younger patients with S. aureus having increased in-hospital mortality. CONCLUSIONS Extracorporeal membranous oxygenation use for individuals with influenza increased over time, particularly after the 2009 H1N1 pandemic, most notably among older adults. Survival to hospital discharge for patients with influenza on extracorporeal membranous oxygenation was slightly higher than survival to hospital discharge for respiratory illness due to any cause. Bacterial codetection was common among patients with influenza on extracorporeal membranous oxygenation and was associated with increased days on extracorporeal membranous oxygenation but not increased mortality. Only S. aureus codetection in children was associated with increased in-hospital mortality.

We wanted to determine the diagnostic performance of a rapid influenza diagnostic test (RIDT) used bedside in a pediatric emergency department (PED). This was a prospective study over four consecutive winters (2009-2013), comparing the results of a RIDT (QuickVue®) with RT-PCR in children admitted to a PED. Among the 764 children included, we did not observe any significant differences in the diagnostic performance of RIDT except during the H1N1 pandemic. The overall sensitivity of the test was 0.82; the specificity 0.98; the positive and negative likelihood ratios 37.8 and 0.19. The positive and negative post-test probabilities of infection were 98% and 17%. The diagnostic performance was increased for influenza B cases (P = 0.03). RIDTs are suitable for use every winter with few differences in its diagnostic value, except during specific pandemic periods. This test could limit unnecessary complementary exams and guide the prescription of antivirals during influenza epidemic periods in PEDs.

In previous research on severe acute respiratory syndrome (SARS) and West Nile virus, empathic responding has been associated with higher perceived threat during a pandemic as well as the implementation of recommended health precautions. The goal of the current study was to investigate the role of empathic responding outside of a Western context by examining the endorsement of specific health precautions during the 2009–2010 H1N1 pandemic in India. Responses to questions about Centers for Disease Control recommended health precautions, perceived threat and empathic responding were collected from 100 individuals living in the city of Dharwad in India’s Karnataka state. Analyses revealed that individuals who responded to the threat of H1N1 with greater empathy were more likely to endorse recommended health behaviours (i.e. vaccination, handwashing and disinfectant use).
