An elevated tricuspid regurgitant jet velocity (TRV) is associated with hemolysis and early mortality in sickle cell disease, yet risk factors, clinical parameters, and mortality associated with this biomarker in thalassemia are poorly defined. This report summarizes the prevalence of an elevated TRV in 325 patients screened by Doppler echocardiography in the Thalassemia Clinical Research Network. A documented TRV was reported in 148 of 325 (46%) of patients. Average age was 25.9 years (range, 5-56 years) and 97% were transfusion-dependent. Mean TRV was 2.3 ± 0.4 m/s (range, 0.2-3.5 m/s). An abnormal TRV ≥ 2.5 m/s was identified in 49 of 148 (33%) of patients with a documented TRV, 5% (8/148), with a TRV ≥ 3.0 m/s, suggesting significant PH risk. Older age was strongly associated with a high TRV; however, 16% of children had a TRV ≥ 2.5 m/s. A history of splenectomy, hepatitis C, smoking, or high white blood cell count was associated with TRV elevation. In summary, an elevated TRV is noted in one-third of transfusion-dependent thalassemia patients with a documented value and develops in both children and adults. Age, splenectomy, hepatitis C, and smoking are significant univariate risk factors, with splenectomy surfacing as the dominant risk factor over time. Mortality was low in this cohort. To gain an insight into current transfusion and chelation practice in patients with sickle cell disease (SCD), a survey of international experts has been conducted. The findings demonstrate that general utilization of transfusion therapy is low, the primary barrier to treatment being concerns over resultant iron overload and the subsequent need for iron chelation therapy. Where patients were transfused, many physicians indicated that a high proportion of patients had hemosiderosis. As evidence suggests more patients with SCD could benefit from regular transfusion therapy, it is apparent that greater awareness of the need to monitor and treat iron overload in transfused patients is required.

Pulmonary hypertension (PH) commonly develops in thalassaemia syndromes, but is poorly characterized. The goal of this study was to provide a comprehensive description of the cardiopulmonary and biological profile of patients with thalassaemia at risk for PH. A case-control study of thalassaemia patients at high versus low PH-risk was performed. A single cross-sectional measurement for variables reflecting cardiopulmonary status and biological pathophysiology were obtained, including Doppler-echocardiography, 6-minute-walk-test, Borg Dyspnea Score, New York Heart Association functional class, cardiac magnetic resonance imaging (MRI), chest-computerized tomography, pulmonary function testing and laboratory analyses targeting mechanism of coagulation, inflammation, haemolysis, adhesion and the arginine-nitric oxide pathway. Twenty-seven thalassaemia patients were evaluated, 14 with an elevated tricuspid-regurgitant-jet-velocity (TRV) ≥2.5m/s. Patients with increased TRV had a higher frequency of splenectomy, and significantly larger right atrial size, left atrial volume and left septal-wall thickness on echocardiography and/or MRI, with elevated biomarkers of abnormal coagulation, lactate dehydrogenase levels and arginase concentration, and lower arginine-bioavailability compared to low-risk patients. Arginase concentration correlated significantly to several echocardiography/MRI parameters of cardiovascular function in addition to global-arginine-bioavailability and biomarkers of haemolytic rate, including lactate dehydrogenase, haemoglobin and bilirubin. Thalassaemia patients with a TRV ≥2.5m/s have additional echocardiography and cardiac-MRI parameters suggestive of right and left-sided cardiac dysfunction. In addition, low arginine bioavailability may contribute to cardiopulmonary dysfunction in β-thalassaemia.

Background The acute chest syndrome is the leading cause of death among patients with sickle cell disease. Since its cause is largely unknown, therapy is supportive. Pilot studies with improved diagnostic techniques suggest that infection and fat embolism are underdiagnosed in patients with the syndrome. Methods In a 30-center study, we analyzed 671 episodes of the acute chest syndrome in 538 patients with sickle cell disease to determine the cause, outcome, and response to therapy. We evaluated a treatment protocol that included matched transfusions, bronchodilators, and bronchoscopy. Samples of blood and respiratory tract secretions were sent to central laboratories for antibody testing, culture, DNA testing, and histopathological analyses. Results Nearly half the patients were initially admitted for another reason, mainly pain. When the acute chest syndrome was diagnosed, patients had hypoxia, decreasing hemoglobin values, and progressive multilobar pneumonia. The mean length of hospitalization was 10.5 days. Thirteen percent of patients required mechanical ventilation, and 3 percent died. Patients who were 20 or more years of age had a more severe course than those who were younger. Neurologic events occurred in 11 percent of patients, among whom 46 percent had respiratory failure. Treatment with phenotypically matched transfusions improved oxygenation, with a 1 percent rate of alloimmunization. One fifth of the patients who were treated with bronchodilators had clinical improvement. Eighty-one percent of patients who required mechanical ventilation recovered. A specific cause of the acute chest syndrome was identified in 38 percent of all episodes and 70 percent of episodes with complete data. Among the specific causes were pulmonary fat embolism and 27 different infectious pathogens. Eighteen patients died, and the most common causes of death were pulmonary emboli and infectious bronchopneumonia. Infection was a contributing factor in 56 percent of the deaths. Conclusions Among patients with sickle cell disease, the acute chest syndrome is commonly precipitated by fat embolism and infection, especially community-acquired pneumonia. Among older patients and those with neurologic symptoms, the syndrome often progresses to respiratory failure. Treatment with transfusions and bronchodilators improves oxygenation, and with aggressive treatment, most patients who have respiratory failure recover.

Ineffective erythropoiesis, the hallmark of β-thalassemia, is a result of α/non-α globin chain imbalance.1 One strategy to redress globin-chain imbalance is to induce γ-globin gene (HBG) expression. Repression of HBG in adult erythroid cells involves DNA methylation and other epigenetic changes. Therefore, the cytosine analog decitabine, which can deplete DNA methyltransferase 1 (DNMT1), can potentially activate HBG. In 5 patients with β-thalassemia intermedia, a dose and schedule of decitabine intended to deplete DNMT1 without causing significant cytotoxicity (0.2 mg/kg subcutaneous 2 times per week for 12 weeks) increased total hemoglobin from 7.88 ± 0.88 g/dL to 9.04 ± 0.77 g/dL (P = .004) and absolute fetal hemoglobin from 3.64 ± 1.13 g/dL to 4.29 ± 1.13 g/dL (P = .003). Significant favorable changes also occurred in indices of hemolysis and red blood cell densitometry. Consistent with a noncytotoxic, differentiation altering mechanism of action, the major side effect was an asymptomatic increase in platelet counts without erythrocyte micronucleus or VDJ recombination assay evidence of genotoxicity. Acute chest syndrome (ACS) is the leading cause of death in sickle cell disease. Severe ACS often develops in the course of a vaso-occlusive crisis (VOC), but currently there are no predictors for its development. Secretory phospholipase A2(sPLA2), a potent inflammatory mediator, is elevated in ACS, and previous work suggests that sPLA2 predicts impending ACS. We prospectively evaluated sPLA2concentration during 21 admissions for VOC; 6 of these patients went on to develop ACS. Elevation of sPLA2 was detected all 6 patients 24 to 48 hours before ACS was clinically diagnosed. Adding the requirement for fever raised the specificity of sPLA2 to 87% while retaining 100% sensitivity. These data indicate that sPLA2 can be useful in alerting the clinician to patients with impending ACS. In addition, sPLA2 may be useful for instituting early therapies to prevent or reduce the clinical morbidity of ACS.

THE sickle cell trait is generally considered to be benign, because the presence of hemoglobin A in a concentration of more than 50 percent in the red cells of persons heterozygotic for hemoglobin A and hemoglobin S (hemoglobin A/S) prevents the polymerization of the remaining hemoglobin S under physiologic conditions.1 Occasional reports suggest, however, that after extreme physical stress or hypoxia, the sickle cell trait can be associated with serious morbidity and even death.2–4 In most cases, the diagnosis of this trait is straightforward: hematologic measures such as hemoglobin, hematocrit, red-cell indexes, and the reticulocyte count are normal, and electrophoretic . . .

Chronic red blood cell (RBC) transfusion has been used for decades to prevent the classical bone changes and life-threatening anemia in patients with homozygous β-thalassemia (Thal).1 More recently, transfusion therapy has also been used to minimize disease-specific morbidity in patients with sickle cell disease (SCD).2,3 Long-term transfusion therapy without adequate chelation, however, results in iron overload–associated organ failure.4–7 Consensus documents have been developed for patients with Thal8–10 and adopted for use in SCD,11,12 to provide clinicians guidance in the monitoring and treatment of chronically transfused patients. Quantitative iron assessment by percutaneous liver biopsy, the most viable clinical option at the time of this study, was recommended to be performed every 2 years, in addition to quarterly assessment of serum ferritin. Details for annual monitoring of cardiac, endocrine, and hepatic function were also provided. In 2002, the National Institutes of Health (NIH) released a consensus document for the care of patients with SCD receiving transfusion therapy.13 The document summarized that “there is no evidence that SCD patients are spared the fatal consequences of iron overload. Therefore a comprehensive program to monitor and treat iron overload is necessary.” To date, there have been no studies directly comparing the adequacy of care provided in these two similarly transfused patient populations. Disparity in the delivery of health care has gained attention over the past decade.14–16 In its report of racial and ethnic disparities in health care, the Institute of Medicine (IOM) concluded that there were significant disparities in the quality and quantity of health care received by minority groups in the United States.17 In 1994, the Sickle Cell Treatment Act was passed designed to improve funding and resources for specialized SCD centers. Despite significant advances in the treatment of SCD, a 2006 report suggests that major funding gaps remain when support is compared between patients with SCD and cystic fibrosis, a chronic disease affecting far fewer Americans who are primarily Caucasian.18 This report highlights federal and philanthropic research support; however, the quality of clinical care provided is undoubtedly impacted. Although solutions remain elusive, it is clear that health care disparity is a complex issue that cannot be explained solely on the basis of research support, a patient’s race, socioeconomic status, access to health care, behavior, or physician knowledge or pre-paredness. The objective of this article is to evaluate the adequacy in the assessment of iron overload and management of iron-related organ toxicity in two similarly transfused but distinct patient groups.
