Purpose: The authors investigated the association between risk of tendinopathies and genetic markers in professional team sports. Methods: The authors studied 363 (mean [SD]; 25 [6] y, 89% male) elite players (soccer, futsal, basketball, handball, and roller hockey) from a top-level European team (FC Barcelona, Spain). Of 363, 55% (cases) had experienced 1+ episodes of tendinopathy during 2008–2018 and 45% (controls) remained injury free. The authors first examined the association between single-nucleotide polymorphisms (SNPs) and tendinopathy risk in a hypothesis-free case-control genome-wide association study (495,837 SNPs) with additional target analysis of 58 SNPs that are potential candidates to influence tendinopathy risk based on the literature. Thereafter, the authors augmented the SNP set by performing synthetic variant imputation (1,419,369 SNPs) and then used machine learning-based multivariate modeling (support vector machine and random forest) to build a reliable predictive model. Results: Suggestive association (P −5) was found for rs11154027 (gap junction alpha 1), rs4362400 (vesicle amine transport 1-like), and rs10263021 (contactin-associated protein-like 2). Carriage of 1+ variant alleles for rs11154027 (odds ratio = 2.11; 95% confidence interval, 1.07–4.19, P = 1.01 × 10−6) or rs4362400 (odds ratio = 1.98; 95% confidence interval, 1.05–3.73, P = 9.6 × 10−6) was associated with a higher risk of tendinopathy, whereas an opposite effect was found for rs10263021 (odds ratio = 0.42; 95% confidence interval, 0.20–0.91], P = 4.5 × 10−6). In the modeling approach, one of the most robust SNPs was rs10477683 in the fibrillin 2 gene encoding fibrillin 2, a component of connective tissue microfibrils involved in elastic fiber assembly. Conclusions: The authors have identified previously undescribed genetic predictors of tendinopathy in elite team sports athletes, notably rs11154027, rs4362400, and rs10263021.

Objective To assess multifidus muscle thickness, cross-sectional area (CSA) and disability in patients with chronic non-insertional Achilles tendinopathy (AT) who developed an eccentric exercise (EE) vibration program compared to an EE program with cryotherapy. Design Single-blinded randomized clinical trial. Setting Outpatient clinic. Participants A total sample of 61 patients diagnosed with chronic non-insertional AT was recruited and randomly divided into two groups. A group (n = 30) developed the EE program plus vibration and B group (n = 31) received the EE program plus cryotherapy for 12 weeks. Multifidus thickness and CSA were measured at rest and during maximal isometric contraction by ultrasound imaging. The Victorian Institute for Sport Assessment (VISA-A) was used to asses functionality. Results Multifidus CSA was statistically significant increased (P < 0.05) for the EE vibration program group with respect to EE plus cryotherapy during maximal isometric contraction and at rest at 12-weeks after intervention in individuals with chronic non-insertional AT. Despite both interventions showed differences for the multifidus thickness and AT disability variables over time, there were not between-groups differences. Conclusions Authors encourage the use of vibration with respect to cryotherapy added to EE programs in order to enhance multifidus CSA in addition to lower limb functionality in individuals who suffer from chronic non-insertional AT. Given that athletes cannot dedicate their time exclusively to their sports career, some universities have already developed specific academic programs that support the duality of university education and sports training. These programs allow students to attend official competitions throughout the academic year. The present article describes the university programs followed by 27 Spanish Athletic sNational Team athletes that competed in the Olympic and Paralympic games in Tokyo 2020, celebrated in the summer of 2021. They’re 27 sportsman and women that, in addition to being leaders in their disciplines both nationally and internationally, have chosen to follow a Health Sciences program at university, such as Medicine, Physical Therapy, Nursery, Psychology, Chiropody or Physical Activity and Sport Science.

Tendinopathy is a very common disease in the general population as well as in athletes. The aim of the present study was to examine the tendon thickness and cross-sectional area (CSA) in subjects with chronic mid-portion Achilles tendinopathy (AT) who engaged in either an eccentric exercise (EE) programme with vibration training or an EE programme combined with cryotherapy. A sample of 61 patients with chronic mid-portion AT were recruited and divided into two groups: EE programme vibration training (n = 30) and EE programme combined with cryotherapy (n = 31). Three ultrasound assessments were performed: pre-intervention and at 4, and at 12 weeks. The comparison of thickness and CSA measures at baseline, 4, and 12 weeks showed a significant (P < 0.05) increase at 0, 2, 4, and 6 cm in maximal isometric contraction and at rest in subjects with chronic mid-portion AT. The EE vibration training resulted in a statistically significant CSA increase compared with the cryotherapy group in patients with chronic mid-portion AT Sin financiación 2.825 JCR (2019) Q2, 21/68 Dermatology, 53/210 Surgery 0.877 SJR (2019) Q1, 28/146 Dermatology, 90/451 Surgery No data IDR 2019 UEM

Triceps surae (TS) and tibialis anterior (TA) activation patterns have not yet been studied under different types of sport shoes. We hypothesized that sports shoes may reduce the activity patterns of these muscles in relation to barefoot condition. Thus, our main aim was to evaluate the activity patterns of TS and TA muscles in healthy people during all gait phases using five types of sport shoes with respect to barefoot condition. A total sample of thirty healthy participants, mean age 36.20 ± 8.50, was recruited in a podiatry laboratory following an observational research design. During walking and running, electromyography signals were recorded from TS and TA muscles using surface electrodes in the following experimental situations: 1.) barefoot, 2.) minimalist, 3.) pronated control, 4.) air chamber, 5.) ethyl-vinyl-acetate and 6.) boost. The TS and TA showed significant reductions (P < 0.05) in the peak amplitude of different sport shoes types with respect to the barefoot condition in different phases of the gait cycle during walking and running. Nevertheless, the boost sport shoe produced statistically significant increases in the peak amplitude of the gastrocnemius medialis muscle in comparison with the barefoot condition in the midstance phase of the gait cycle during running (P = 0.047). In addition, the pronation control and air chamber sport shoes produced statistically significant increases in the peak amplitude of the TA muscle with respect to the barefoot condition in the contact phase of the gait cycle (P = 0.021; P = 0.013), respectively, during running. Despite TS and TA muscles activity patterns seem to be reduced using different sport shoes types with respect to the barefoot condition in different phases of the gait cycle during walking and running, some sport shoes may increase this muscular activity in specific phases of the gait cycle during running. Flavanols-rich cocoa has positive effects on lipid metabolism and might enhance the performance of athletes through an improvement in their body composition. To test this hypothesis a placebo-controlled intervention study in training endurance athletes who received 5 g of cocoa daily (425 mg of flavanols) for 10 weeks was performed. Dietary intake, body composition, exercise performance, and plasma levels of follistatin, myostatin, and leptin were measured. Cocoa intake significantly reduced body fat percentage (p = 0.020), specifically in the trunk (p = 0.022), visceral area (p = 0.034) and lower limbs (p = 0.004). The reduction in body fat mass was accompanied by an increase in plasma follistatin and a decrease in leptin, while myostatin levels remained unchanged. The intake of cocoa reduced the percentage of body fat of athletes, without any impact on athletes' performance. The change in fat body composition did not improve athletes' performance. The main goal of the study was to evaluate the first-order descriptor and echostructure of lumbar multifidus at the L4 vertebral level in athletes with and without chronic lumbopelvic pain (CLPP). A case-control study was performed in 15 semiprofessional athletes with CLPP and 15 without (healthy athletes). Lumbar multifidus echointensity and echovariation were measured for muscle quality assessment. Echostructure was used to evaluate lumbar multifidus cross-sectional area (CSA) at resting and during muscle contraction, respective differences during both phases (CSADif.), activation patterns, and thoracolumbar fasciae morphology and thickness. Significant differences with a large effect size were observed in quantitative data from CLPP and healthy athletes for left lumbar multifidus CSADif. and thoracolumbar fasciae morphology. Categorical data showed statistically significant differences with a small-to-moderate effect size for lumbar multifidus activation pattern and thoracolumbar fasciae morphology. Athletes with CLPP showed a reduced CSA difference between lumbar multifidus contraction and at resting and higher disorganization of thoracolumbar fasciae morphology compared to healthy athletes. These findings suggest the importance of dynamic exploration of the lumbar region and connective tissue in sports performance and injury prevention. Objective To determine the influence of depression symptoms and levels in athletes with gastrocnemius myofascial pain with respect to healthy athletes. In addition, to determine a prediction model for kinesiophobia symptoms based on descriptive data and gastrocnemius myofascial pain presence. Design Secondary case-control. Setting Outpatient clinic. Participants A sample of 50 athletes was recruited and divided into athletes with chronic gastrocnemius myofascial pain (n = 25) and healthy athletes (n = 25). Main outcome measurements Depression symptoms scores and levels were self-reported by athletes using the Beck Depression Inventory – II (BDI-II). Results Statistically significant differences for depression symptoms scores (P = 0.011) with a moderate effect size (d = 0.77) and depression levels (P = 0.036) were found between both groups showing greater depression symptoms and levels in athletes with gastrocnemius myofascial pain (13.00 ± 13.50 points; range from 0 to 28 points) versus healthy athletes (4.00 ± 7.00 points; range from 0 to 19 points). Higher depression symptoms scores of BDI-II were only predicted by the presence of gastrocnemius myofascial pain in athletes (R2 = 0.134; β = +5.360; F[1,48] = 7.428; P = 0.009). Conclusions Greater depression symptoms and levels were exhibited for athletes with gastrocnemius myofascial pain compared to healthy athletes. In addition, depression score of athletes was only predicted by the presence of gastrocnemius myofascial pain. The study of anxiety, specifically its relations with sociodemographic variables, has been fruitfull in sport psychology research. This study aimed to investigate athletes' sport anxiety regarding differences in gender and sport played. An application of structural equation modeling was made, with 601 Portuguese athletes. From them 172 (28.6%) were female and 429 (71.4%) were male. They competed in a variety of individual (e.g., athletics, climbing, orienteering, surfing, swimming, tennis; 42.6%) and team sports (e.g., basketball, handball, rugby, soccer, volleyball; 57.4%). Participants' age ranged from 12 to 47 years (M = 17.44 years; SD = 4.99). After testing the measurement invariance of the first and second-order models, across gender and type of sport (individual vs. team), latent mean comparisons were investigated and Cohen's d (1988) statistic was computed to obtain the corresponding effect sizes (Kline, 2016). Significant differences were detected between male and female athletes and between individual and team sports. Female and individual sports athletes presented higher levels of general sports anxiety. The results of this research provided evidence that anxiety is appraised differently by athletes based on their gender and type of sport.

Ankle sprains have been defined as the most common injury in sports. The aim of the present study was to investigate the ankle taping for the reduction of ankle dorsiflexion range of motion (ROM) and inter-limb in elite soccer and basketball players U18 in a single training session. Methods: A cross-sectional pilot study was performed on 38 male healthy elite athletes divided into two groups: a soccer group and a basketball group. Ankle dorsiflexion ROM and inter-limb asymmetries in a weight-bearing lunge position were assessed in three points: with no-tape, before the practice and immediately after the practice. Results: For the soccer group, significant differences (p <, 0.05) were observed for the right ankle, but no differences for the asymmetry variable. The basketball group reported significant differences (p <, 0.05) for the right ankle and symmetry. Conclusions: Ankle taping decreased the ankle dorsiflexion ROM in youth elite soccer and basketball players U18. These results could be useful as a prophylactic approach for ankle sprain injury prevention. However, the ankle ROM restriction between individuals without taping and individuals immediately assessed when the tape was removed after the training was very low.

Importance Vigorous exercise (particularly endurance sports) might increase the risk of atrial fibrillation (AF), but there is variability in the reported frequency of this condition among elite athletes. The underlying pathophysiologic source remains unclear. Objective To determine AF incidence in a large cohort of elite athletes and its association with potential risk factors. Design, Setting, and Participants Retrospective observational cohort of all Spanish athletes (N = 6813) referred to a single center was used to determine AF incidence from January 1, 1997, to December 31, 2017, and cross-sectional analysis was conducted to compare athletes with and without reported AF. The cohort covered most sports disciplines, and the studied athletes were on national teams and competed in major international events. Cardiologists responsible for echocardiographic assessment were not blinded to the condition (AF or no AF) of the athletes. Exposures All participants underwent at least 1 cardiologic evaluation, including assessment at time of AF diagnosis in those with this condition. Main Outcomes and Measures Diagnosis of AF based on resting and/or exercise electrocardiogram, and/or 24-hour Holter monitoring and echocardiography-assessed atrial dimensions. Results A total of 6813 Spanish elite athletes (2385 [35.0%] women) were referred for cardiac evaluation during the study period. Mean (SD) age was 22 (7) years, and mean (SD) time of competition was 8 (5) years. Only 21 athletes (1 woman), participating in different types of sports, had AF (ie, paroxysmal [n = 18], persistent [n = 1], or long-standing persistent [n = 2]) during the 20-year study. In multivariate analysis, increasing values of age (odds ratio [OR], 1.07; 95% CI, 1.00-1.14), years of competition (OR, 1.14; 95% CI, 1.07-1.22), and left atrial anteroposterior diameter (OR, 1.21; 95% CI, 1.10-1.32) were associated with higher AF risk. Conclusions and Relevance The incidence of AF is low among young Spanish elite athletes, even when considering only endurance athletes. Yet, potential contributors (particularly atrial remodeling) need to be monitored. Elite athlete status is a partially heritable trait, as are many of the underpinning physiological, anthropometrical, and psychological traits that contribute to elite performance. In recent years, our understanding of the specific genetic variants that contribute to these traits has grown, such that there is considerable interest in attempting to utilise genetic information as a tool to predict future elite athlete status. In this review, we explore the extent of the genetic influence on the making of a sporting champion and we describe issues which, at present, hamper the utility of genetic testing in identifying future elite performers. We build on this by exploring what further knowledge is required to enhance this process, including a reflection on the potential learnings from the use of genetics as a disease prediction tool. Finally, we discuss ways in which genetic information may hold utility within elite sport in the future, including guiding nutritional and training recommendations, and assisting in the prevention of injury. Whilst genetic testing has the potential to assist in the identification of future talented performers, genetic tests should be combined with other tools to obtain an accurate identification of those athletes predisposed to succeed in sport. The use of total genotype scores, composed of a high number of performance-enhancing polymorphisms, will likely be one of the best strategies in the utilisation of genetic information to identify talent in sport. Objectives To estimate the water balance in a healthy population of sportspeople from Southern Spain and determine the sources of their fluid intake, evaluating the contribution of different types of drink and comparing the results by sex and province of residence. Methods Three hundred eighty-six sportspeople (231 males, 152 females) were enrolled in the study. A questionnaire was administered to calculate nutrient intake through diet and physical activity, and anthropometric measures were taken according to ISAK standards. SPSS-15 was used for data analyses. Results Fruit juice, tap water, bottled water, processed fruit juice, carbonated drinks, and isotonic drinks comprised 96% of the total water intake. Simple sugar consumption represented 4.44% of daily calorie intake. Significant differences were found between sexes and between professionals and amateurs. The amount of drinks consumed varied as a function of the quality of the drinking water, which significantly differed among the eight Andalusian provinces. Conclusion This study population did not fully meet fluid intake recommendations, compliance with hydration recommendations varied as a function of the sex and the amateur or professional status of these sportspeople. The pattern of drinks consumption also differed according to their place of residence.

Many studies have researched the psychophysiological response and energy balance of athletes in numerous ultraendurance probes, but none has investigated an ultraendurance mountain running event. The current study aims to analyze changes in blood lactate concentration, rating of perceived exertion, heart rate, heart rate variability, and energy balance after the performance of an ultraendurance mountain running event. The parameters in the 6 participants who finished the event were analyzed (age, 30.8 ± 3.1 years; height, 176.2 ± 8.6 cm; body mass, 69.2 ± 3.7 kg). The race covered 54 km, with 6441 m of altitude change, 3556 m downhill and 2885 m uphill. The athletes completed together the race in 14 h and 6 min. After the ultraendurance event, the athletes presented a negative energy balance of 4732 kcal, a blood lactate concentration of 2.8 ± 0.3 mmol/L, a heart rate mean/heart rate maximum ratio of 0.64, a heart rate mean of 111.4 ± 5.9 beats/min, a decrease in vagal modulation, and an increase in sympathetic modulation, and recorded 19.5 ± 1.5 points on the 6–20 rating of perceived exertion scale. The event was a stressful stimulus for the athletes despite the low intensity measured by blood lactate concentration and heart rate. The results obtained may be used by coaches as a reference parameter of heart rate, heart rate variability, rating of perceived exertion, and lactate concentration to develop specific training programs. In addition, the energy balance data obtained in this research may improve nutritional intake strategies. 1.910 JCR (2015) Q2, 26/82 Sports sciences; 54/78 Nutrition & dietetics, 53/83 Physiology UEM

Participants were 179 subjects practicing five different sports modalities, aged between 14 and 58 years (M = 21.04; SD = 4.21). The inventory assesses anxiety as state and trait, but in this study only the trait anxiety scale was used which is composed of 20 items arranged in a four-point Likert scale. Results of the confirmatory factor analysis indicated the items were not properly adjusted to the instrument's original model. This exploratory factor analysis indicated the existence of two factors that explained 33% of the variance. These factors were called present anxiety and absent anxiety, with reliability coefficients of 0.82 and 0.72 respectively. Sporting competitions are becoming more demanding in terms of intensity of effort, and this means controlling all aspects that affect athletic performance. Food, hydration and supplementation, before, during and after training or competition directly affect health, body composition, performance and recovery of the athlete. The assessment of nutritional status is required for proper advising of the athlete, through blood tests to control the process of adaptation to training. The aim of this paper is to provide practical tools for dietitians-nutritionists to control the health and nutritional status of athletes, as well as monitoring their adaptation to workloads and competition periods. Performing analytical tests to control of protein metabolism, lipid profile, ions, blood tests and iron metabolism, in addition to review some hormonal parameters, may be of interest in order to observe the potential existence of overtraining states. The correct understanding and interpretation of laboratory tests (under sports doctor’s supervision) will be most important and useful for dietitians-nutritionists, performing dietary and nutritional advice to athletes, because it will determine the status of the athlete and propose different individual feeding strategies depending on the training phase and response.

This study aimed to diagnose the nutritional status of the Weight Lifting Permanent Olympic Team Athletes of the Brazilian Olympic Committee (COB). The sample was composed of 24 athletes, aged 16-23 yr, 12 males (19.7 ± 2.4 yr) and 12 females (19.2 ± 1.8 yr). The following procedure was applied to diagnose the nutritional status: analysis of the adequability of energy and macronutrient intake - carbohydrates (CHO), lipids (LIP) and proteins (PRO) -, through the methods 24-hr diet records and the Food Consumption Frequency Questionnaire, besides anthropometrical profile characterization. The results obtained from the dietary evaluation showed that the energy distribution among the macronutrients was adequate, 54 ± 6.8% (CHO); 28,5 ± 5.9% (LIP); and 14.5 ± 3.4% (PRO) for the male team, and 56.3 ± 4.7% (CHO); 28.6 ± 4.6% (LIP); and 13.7 ± 2.4% (PRO) for the female team. However, regarding total energy intake, 83% of the athletes presented energy intake below the recommended values, considering the high level of physical activity, resulting in daily caloric deficiency. Body fat percentage of the male athletes (3.6 ± 0.7%) indicated that all of them were below the reference standard, while 58% of the female athletes had excess fat (17.9 ± 5.8%). It was concluded that although the athletes showed an adequate caloric distribution of macronutrients, it was still not sufficient to meet the energy requirements of their modality, thus these athletes should seek nutritional orientation.
