Abstract

Background The long-term outcomes of COVID-19 hospitalisation in individuals with pre-existing airway diseases are unknown. Methods Adult participants hospitalised for confirmed or clinically suspected COVID-19 and discharged between 5 March 2020 and 31 March 2021 were recruited to the Post-hospitalisation COVID-19 (PHOSP-COVID) study. Participants attended research visits at 5 months and 1 year post discharge. Clinical characteristics, perceived recovery, burden of symptoms and health-related quality of life (HRQoL) of individuals with pre-existing airway disease (i.e., asthma, COPD or bronchiectasis) were compared to the non-airways group. Results A total of 615 out of 2697 (22.8%) participants had a history of pre-existing airway diseases (72.0% diagnosed with asthma, 22.9% COPD and 5.1% bronchiectasis). Abstract

Background The impact of COVID-19 on physical and mental health and employment after hospitalisation with acute disease is not well understood. The aim of this study was to determine the effects of COVID-19-related hospitalisation on health and employment, to identify factors associated with recovery, and to describe recovery phenotypes. Methods The Post-hospitalisation COVID-19 study (PHOSP-COVID) is a multicentre, long-term follow-up study of adults (aged ≥18 years) discharged from hospital in the UK with a clinical diagnosis of COVID-19, involving an assessment between 2 and 7 months after discharge, including detailed recording of symptoms, and physiological and biochemical testing. Multivariable logistic regression was done for the primary outcome of patient-perceived recovery, with age, sex, ethnicity, body-mass index, comorbidities, and severity of acute illness as covariates. A post-hoc cluster analysis of outcomes for breathlessness, fatigue, mental health, cognitive impairment, and physical performance was done using the clustering large applications k-medoids approach. The study is registered on the ISRCTN Registry (ISRCTN10980107). Findings We report findings for 1077 patients discharged from hospital between March 5 and Nov 30, 2020, who underwent assessment at a median of 5·9 months (IQR 4·9–6·5) after discharge. Participants had a mean age of 58 years (SD 13); 384 (36%) were female, 710 (69%) were of white ethnicity, 288 (27%) had received mechanical ventilation, and 540 (50%) had at least two comorbidities. At follow-up, only 239 (29%) of 830 participants felt fully recovered, 158 (20%) of 806 had a new disability (assessed by the Washington Group Short Set on Functioning), and 124 (19%) of 641 experienced a health-related change in occupation. Factors associated with not recovering were female sex, middle age (40–59 years), two or more comorbidities, and more severe acute illness. The magnitude of the persistent health burden was substantial but only weakly associated with the severity of acute illness. Four clusters were identified with different severities of mental and physical health impairment (n=767): very severe (131 patients, 17%), severe (159, 21%), moderate along with cognitive impairment (127, 17%), and mild (350, 46%). Of the outcomes used in the cluster analysis, all were closely related except for cognitive impairment. Three (3%) of 113 patients in the very severe cluster, nine (7%) of 129 in the severe cluster, 36 (36%) of 99 in the moderate cluster, and 114 (43%) of 267 in the mild cluster reported feeling fully recovered. Persistently elevated serum C-reactive protein was positively associated with cluster severity. Interpretation We identified factors related to not recovering after hospital admission with COVID-19 at 6 months after discharge (eg, female sex, middle age, two or more comorbidities, and more acute severe illness), and four different recovery phenotypes. The severity of physical and mental health impairments were closely related, whereas cognitive health impairments were independent. In clinical care, a proactive approach is needed across the acute severity spectrum, with interdisciplinary working, wide access to COVID-19 holistic clinical services, and the potential to stratify care. Abstract

INTRODUCTION Tuberculosis (TB) remains the most common cause of death from infectious disease world-wide. In the UK, the incidence of TB has risen by 25% over the last 10 years; extrapulmonary diagnosis remains challenging and can be delayed. This study evaluates the epidemiology of musculoskeletal tuberculosis in a large multi-ethnic UK city. PATIENTS AND METHODS A review of prospectively recorded data of incidence, anatomical site, ethnic distribution, treatment and drug resistance of musculoskeletal tuberculosis over a 6-year period was performed. RESULTS From January 1999 to December 2004, there were 729 TB notifications; 61 cases (8.4%) had musculoskeletal involvement. Of the patients, 74% were immigrants from the Indian subcontinent; nearly 50% had spinal involvement; 24 patients underwent surgical intervention; 29 were subjected to either diagnostic or therapeutic radiological intervention; and resolution of symptoms was achieved in 59 out of 61 cases. CONCLUSIONS This study highlights the high proportion of musculoskeletal TB in immigrant patients in an area with a relatively large at-risk population, but will also serve to alert physicians, in areas with smaller at-risk populations, of the possibility of musculoskeletal TB.

Abstract

Introduction The GOLD guidelines recommend that the COPD Assessment Test (CAT) 1 can be used in guiding and optimising therapy, however there is little evidence on its use in monitoring treatment. Aim We have conducted a 6-month prospective observational study describing the impact of COPD on daily life, following the initiation of maintenance indacaterol, a once-daily long-acting beta-agonist. Method Subjects from 39 UK GP practices (April 2012 to May 2013) with a diagnosis of COPD and were newly-prescribed indacaterol for routine COPD management, either as maintenance mono-therapy or add-on therapy to long-acting muscarinic antagonists (LAMA) were recruited. Here we present interim results of completed CAT and a descriptive Daily Life Impact Questionnaire (DLIQ), developed for specifically for the study, at treatment initiation and 6–8 weeks. Further assessments will be made at 6 months, when treatment changes in addition to indacaterol will be recorded. Results One hundred and three subjects (61 males, 42 females), with a mean age of 67 years (range 44–86) and a median baseline FEV1% predicted of 61% (n = 100; IQR 51–70%) were recruited. Median time from diagnosis to indacaterol initiation was 6 months (IQR 0–37). Of the 86 (83.5%) subjects evaluated for change in CAT score, 65 (76%) had a reduction (i.e. improved health status), 18 (21%) increased and 3 (3%) remained the same (Figure 1); with a mean overall change of -4.1 (SD ± 5.6; p Conclusion The CAT appears responsive to treatment for COPD with indacaterol when assessed in routine practice and the average size of improvement was large. Alongside this mean improvement, nearly half of patients reported the ability to do more activities that previously they had found difficult. Abstract

Introduction and Objectives The Optimum Patient Care (OPC) service provides a comprehensive asthma assessment, analysing both GP-recorded and patient-reported outcomes to generate patient-specific management recommendations (based on British guidelines) for considerations by practise staff. This study evaluates the effect of the OPC asthma service evaluation on real-life asthma control outcomes in a UK primary care adult asthma population compared with a control population. Methods Routine and patient-reported questionnaire data were collected for 2952 patients with clinician-diagnosed asthma and 1 year outcome data managed in practises across Surrey and Leicester County and Rutland Primary Care Trusts. The effect of the OPC service was evaluated by assessing change in markers of asthma control between initiation and re-assessment: exacerbation frequency (Read code defined acute exacerbations and number of courses of acute oral steroids in previous 12 months) and risk status (high risk: ≥2 annual exacerbations). The number of OPC management suggestions implemented by each practise was also considered. Change in exacerbations over the 12 month period was compared with a control group of patients from UK practises who did not receive OPC management recommendations. Exacerbation rate ratios (RR) for treatment group were produced (relative to control), adjusted for baseline confounders (95%CI). Results There were 2952 patients in the study from 22 practises. At time of re-assessment 10% of OPC management suggestions had been implemented and 22% of patients managed at British Thoracic Society (BTS) steps 4/5 had improved their risk status. 17.1% of patients in the control group (n=22,952) suffered from ≥1 exacerbation during the baseline year, increasing to 17.3% of patients after 12 months. For patients receiving the OPC management review, 24.8% had ≥1 exacerbation during baseline, decreasing to 15.3% following review. Conclusions OPC’s combined patient-reported and practise data assessment allowed thorough patient assessment and the generation of tailored management recommendations. Although only 10% of recommendations were implemented at 12 months, the OPC service appeared to have a beneficial effect on risk status and exacerbation rates.

Abstract

Introduction EUS-FNA allows access to the posterior mediastinum and tissue acquisition under real-time ultrasound guidance through the oesophageal wall. There is ample evidence for effectiveness of EUS-FNA in staging lung cancer but data on its utility in the diagnostic work up of sarcoidosis is limited. The aim of this study was to report our experience of mediastinal EUS-FNA as a whole and its diagnostic yield in sarcoidosis in particular. Methods The study included all patients who underwent mediastinal EUS-FNA in our institution from January 2008 to December 2011. Data on patient demographics, mediastinal lesion characteristics and EUS-FNA details were collected from endoscopy reports. Cytology reports and microbiology culture results were analysed. Final clinical diagnoses made during the follow-up were obtained from medical records. We calculated sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of mediastinal EUS-FNA for individual diagnoses. Results 195 patients (n=195, males 65%, mean age 58.6) underwent mediastinal EUS-FNA during the study period. Mean size of the lesions was 15.82 mm (range 3.9–43) in short axis and 28.23 mm (range 8–60) in long axis. Sub-carinal lymph nodes (LN) were the commonest (145/195, 70.3%) target lesions. Mean number of needle pass was 3.18 (range 1–6) and 22G (53.5%) was the commonest needle used. There were no procedure related complications or deaths. Of the 195 patients, FNAs were positive for malignancy in 61 (61/195, 31.2%), sarcoidosis in 40 (20.5%) and tuberculosis (TB) in 15 (7.6%) patients. Of the 64 (31.7%) cases where FNA was reported normal, 42 (65.6%) were accurate and 22 (34.3%) were inaccurate (final diagnosis: 8 cancer, 9 sarcoidosis and 5 TB). In 4 (2%) patients, FNA showed other diagnoses (3 anthracotic LNs, 1 sinus histiocytosis). Abstract PWE-221 table 1 Overall and condition specific results of mediastinal EUS-FNA. Conclusion Our large series on mediastinal EUS-FNA shows that it is an important and useful tool for the assessment of mediastinal lymphadenopathy of unknown aetiology and has overall high sensitivity (≈80%) and high specificity (≈94%). For sarcoidosis in particular, sensitivity (≈83%) and specificity (≈99%) of EUS-FNA is comparable to those for cancer. Competing interests None declared.
