Cutaneous leishmaniasis (CL) is one of the most disregarded tropical neglected disease with the occurrence of self-limiting ulcers and triggering mucosal damage and stigmatizing scars, leading to huge public health problems and social negative impacts. Pentavalent antimonials are the first-line drug for CL treatment for over 70 years and present several drawbacks in terms of safety and efficacy. Thus, there is an urgent need to search for non-invasive, non-toxic and potent drug candidates for CL. In this sense, we have implemented a shape-based virtual screening approach and identified a set of 32 hit compounds.In vitrophenotypic screenings were conducted using these hit compounds to check their potential leishmanicidal effect towardsLeishmania amazonensis(L. amazonensis). Two (Cp1 and Cp2) out of the 32 compounds revealed promising antiparasitic activities, exhibiting considerable potency against intracellular amastigotes present in peritoneal macrophages (IC50values of 9.35 and 7.25μm, respectively). Also, a sterile cidality profile was reached at 20μmafter 48 h of incubation, besides a reasonable selectivity (≈8), quite similarly to pentamidine, a diamidine still in use clinically for leishmaniasis. Cp1 with an oxazolo[4,5-b]pyridine scaffold and Cp2 with benzimidazole scaffold could be developed by lead optimization studies to enhance their leishmanicidal potency.

Resumo Trata-se de uma revisão integrativa da literatura acerca dos conhecimentos produzidos pela comunidade científica sobre automutilação em adolescentes no período de janeiro de 2012 a junho de 2017. A busca foi realizada nas bases de dados SCIELO e Portal CAPES, em português, inglês e espanhol com amostra final de 71 artigos. Este estudo mostrou grande número de publicações a nível internacional, com destaque para artigos epidemiológicos e fragilidade na publicação de estudos de intervenção que investiguem a eficácia de terapias e programas de prevenção. Abstract This study involves an integrative review of the literature regarding the knowledge produced by the scientific community about self-mutilation among adolescents in the period between January 2012 and June 2017. A search was conducted in the SCIELO and CAPES Portal databases in Portuguese, English and Spanish with a final sample of 71 articles. This study revealed a large number of publications at international level, with an emphasis on epidemiological articles and deficiency in the publication of intervention studies that investigate the efficacy of therapies and prevention programs.

Background The protozoan Trypanosoma cruzi is auxotrophic for purines and causes Chagas’ disease (CD), a neglected illness affecting >6 million people. Combining the 3-deoxyribofuranose part of cordycepin with the modified purine ring of a nucleoside ‘hit’ led to the discovery of 4-amino-5-(4-chlorophenyl)-N7-(3′-deoxy-β-d-ribofuranosyl)-pyrrolo[2,3-d]pyrimidine (Cpd1), revealing promising anti-T. cruzi activity. Objectives To further evaluate Cpd1 in vitro and in vivo to fully assess its therapeutic potential against CD, covering cell culture sterilization through washout assays, drug combination with benznidazole and long-term administration in T. cruzi-infected mice. Results Although less susceptible to Cpd1 than amastigotes, trypomastigotes present an impaired capacity to successfully establish intracellular infection of cardiac cultures. Combination of benznidazole with Cpd1 indicated no interaction (additive effect) (FIC index = 0.72) while administration to mice at one-tenth of the optimal dose (2.5 mg/kg and 10 mg/kg for Cpd1 and benznidazole, respectively) suppressed parasitaemia but failed to avoid mortality. Long-term treatment (60 days) gave a rapid drop of the parasitaemia (>98% decline) and 100% mice survival but only 16% cure. In vitro washout experiments demonstrated that although parasite release into the supernatant of infected cardiac cultures was reduced by >94%, parasite recrudescence did occur after treatment. Conclusions Parasite recrudescence did occur after treatment corroborating the hypothesis of therapeutic failure due to subpopulations of dormant forms and/or genetic factors in persister parasites involved in natural drug resistance.

Objetivo: Realizar uma sintese epidemiologica de incidencia da dengue no periodo de 2007 a 2017 nos municipios que integram a Regional de Saude Sao Patricio II. Metodos: Foi realizado um estudo de analise descritiva, com natureza quantitativa de series temporais da ocorrencia e distribuicao dos casos de dengue, utilizando as seguintes variaveis: municipio de residencia do paciente, faixa etaria, genero, casos notificados e confirmados, e periodos de anos e meses. A pesquisa foi conduzida com auxilio das plataformas do Sistema Unico de Saude e Sistema de Informacoes de Notificacoes de Agravos. Para caracterizacao das variaveis utilizou-se a estatistica descritiva, o teste estatistico nao-parametrico Kruskal-Wallis com nivel de significância α= 1%, e calculos de incidencia dos casos de dengue para os municipios e series temporais. Resultados: Foram notificados 19.650 e confirmados 18.748 casos de dengue, os anos de 2013 e 2015 se destacaram em numeros de casos. No que se refere as maiores incidencias anual, o periodo de 2013, 2015 e 2016 apresentaram unanimidades entre os municipios. Ha diferenca significativa na incidencia dos casos de dengue entre os grupos I (janeiro a junho) e II (julho a dezembro) em quase todos os anos analisados. Nao houve diferenca significativa na incidencia de casos de dengue entre genero. As faixas etarias com maior numero de casos de dengue foram de 20 a 39 anos e 40 a 59 anos. Conclusao: A sazonalidade influencia na incidencia da dengue. A compreensao epidemiologica da patologia permitira refinamentos nas acoes e elaboracao de programas de controle do combate ao vetor.

Concomitant transcranial direct current stimulation (tDCS) is suggested to enhance the functional effects of other physical rehabilitation methods in individuals with motor impairment stemming from a chronic cerebrovascular disease. Thus, the primary aim of the proposed study is to analyze the electrical activity of the tibialis anterior (TA) muscle of the paretic limb in stroke survivors following an intervention involving the combination of tDCS over the motor cortex and peripheral electrical stimulation (PES) administered over the paretic TA. The secondary objective is to analyze the effect on dynamic balance. Methods: Thirty-six adult stroke survivors will be randomized into three groups: 1) Active tDCS + active PES; 2) Sham tDCS + active PES and 3) Active tDCS + sham PES. TDCS active will be positioned bilateral over the primary motor cortex of the damaged hemisphere (C1 or C2) and the cathode will be positioned over the primary motor cortex of the undamaged hemisphere (C1 or C2) with a current of 2 mA for 20 minutes. For sham tDCS, will follow the same standarts, however, the equipment will be switched on for only 20 seconds. PES will be administered to the paretic TA at 50 Hz for 30 minutes. Evaluations: the median frequency and root mean square (RMS) of the paretic TA will be analyzed using electromyography (EMG) and dynamic balance will be evaluated using the Mini-Balance Evaluation System (Mini-BESTest) at baseline (pre-intervention), after 10 treatment sessions at a frequency of five times a week for two weeks (post-intervention) and 30 days after the end of the interventions (follow up). Discussion: PES has proven to facilitate the conduction of sensory-motor afferences to the cerebral cortex in stroke survivors. Combining PES with tDCS, which has a direct effect on increasing cortical excitability, could favor motor acquisition and neuronal plasticity in this population.

Compared to rodents, sheep offer several attractive features as an experimental model for testing different medical and surgical interventions related to pathological gait caused by neurological diseases and injuries. To use sheep for development of novel treatment strategies in the field of neuroscience, it is key to establish the relevant kinematic features of locomotion in this species. To use sheep for development of novel treatment strategies in the field of neuroscience, it is crucial to understand fundamental baseline characteristics of locomotion in this species. Despite their relevance for medical research, little is known about the locomotion in the ovine model, and next to nothing about the three-dimensional (3D) kinematics of the hindlimb. This study is the first to perform and compare two-dimensional (2D) and 3D hindlimb kinematics of the sagittal motion during treadmill walking in the ovine model. Our results show that the most significant differences took place throughout the swing phase of the gait cycle were for the distal joints, ankle and metatarsophalangeal joint, whereas the hip and knee joints were much less affected. The results provide evidence of the inadequacy of a 2D approach to the computation of joint kinematics in clinically normal sheep during treadmill walking when the interest is centered on the hoof's joints. The findings from the present investigation are likely to be useful for an accurate, quantitative and objective assessment of functionally altered gait and its underlying neuronal mechanisms and biomechanical consequences.

Chagas disease is a tropical infectious disease resulting in progressive organ-damage and currently lacks efficient treatment and vaccine options. The causative pathogen, Trypanosoma cruzi, requires uptake and processing of preformed purines from the host because it cannot synthesize these de novo, instigating the evaluation of modified purine nucleosides as potential trypanocides. By modifying the pyrimidine part of a previously identified 7-aryl-7-deazapurine nucleoside, we found that substitution of a 6-methyl for a 6-amino group allows retaining T. cruzi amastigote growth inhibitory activity but confers improved selectivity towards mammalian cells. By keeping the 6-methyl group unaltered, and introducing different 7-aryl groups, we identified several analogues with sub-micromolar antitrypanosomal activity. The 7-(4-chlorophenyl) analogue 14, which was stable in microsomes, was evaluated in an acute mouse model. Oral administration of 25 mg/kg b.i.d. suppressed peak parasitemia and protected mice from infection-related mortality, gave similar reductions as the reference drug of blood parasite loads determined by qPCR, but as benznidazole failed to induce sterile cure in the short time period of drug exposure (5 days).

Objective: To analyze the results of the use of narratives in group therapy for the qualification process of professionals in the Psychosocial Care Centers on the use of group dynamics. Method: This is an action research involving 21 workers from 11 Psychosocial Care Centers in the Brazilian Central-West region. For data collection, narratives written by the participants about their work practices with therapeutic groups and theoretical discussion based on in-person workshops were used. Data were subjected to narrative analysis with the support of qualitative analysis software. Results: The category “Concerns and Questioning” showed the workers’ lack of knowledge regarding the concept of group and group dynamics, the group management challenges, and the discomfort with the frustrations experienced by the group. The category “Learning for Group Coordination” showed knowledge acquisition for group planning and functioning, and understanding of the role of the group coordinator for mental health care. Conclusion: Narrating the scenes of group care allowed participants to exercise critical reflexive thinking, recognize the theory of group dynamics, and develop skills to coordinate groups. RESUMEN Objetivo: Analizar los resultados sobre el uso de narrativas de atención grupal en el proceso de cualificación de los profesionales de los Centros de Atención Psicosocial para el aprendizaje del uso de la tecnología grupal. Método: Investigación-acción realizada con 21 trabajadores de 11 Centros de Atención Psicosocial ubicados en la región centro occidental de Brasil. Los datos se recogieron mediante narrativas escritas por los participantes acerca de sus prácticas de trabajo con grupos terapéuticos y talleres presenciales de discusión teórica basados en ellas. Estos datos se sometieron a un análisis narrativo con el apoyo de un software de análisis cualitativo. Resultados: La categoría “Angustias y Cuestionamientos” evidenció el desconocimiento de los trabajadores con respecto a la conceptualización de grupo y de dinámica de grupo; los desafíos de la gestión de grupo y la incomodidad con las frustraciones vividas en el grupo. La categoría “Aprendizajes para la Coordinación de Grupos” reveló la adquisición de conocimientos para la planificación y el funcionamiento del grupo, y la comprensión del papel de coordinador de grupos para la atención a la salud mental. Conclusión: La narración de las escenas de atención grupal les posibilitó a los participantes ejercitar el pensamiento crítico reflexivo, reconocer la teoría de la tecnología grupal y desarrollar habilidades para coordinar grupos. Método: Pesquisa-ação envolvendo 21 trabalhadores de 11 Centros de Atenção Psicossocial do Centro-Oeste brasileiro. Para coleta de dados foram utilizadas narrativas escritas pelos participantes sobre suas práticas de trabalho com grupos terapêuticos e oficinas presenciais para discussão teórica a partir das mesmas. Os dados foram submetidos à análise de narrativas com o suporte do software de análise qualitativa. Resultados: A categoria “Angústias e Questionamentos” evidenciou o desconhecimento dos trabalhadores quanto à conceituação de grupo e dinâmica de grupo, os desafios do manejo grupal e o desconforto com as frustrações vividas no grupo. A categoria “Aprendizados para Coordenação de Grupo” revelou aquisição de conhecimento para o planejamento e funcionamento grupal, e compreensão do papel de coordenador de grupos para o cuidado em saúde mental. Conclusão: Narrar as cenas de cuidado grupal possibilitou aos participantes exercitar o pensamento crítico reflexivo, reconhecer a teoria da tecnologia grupal e desenvolver habilidades para coordenar grupos.

Analysis of locomotion is often used as a measure for impairment and recovery following experimental peripheral nerve injury. Compared to rodents, sheep offer several advantages for studying peripheral nerve regeneration. In the present study, we compared for the first time, two-dimensional (2D) and three-dimensional (3D) hindlimb kinematics during obstacle avoidance in the ovine model. This study obtained kinematic data to serve as a template for an objective assessment of the ankle joint motion in future studies of common peroneal nerve (CP) injury and repair in the ovine model. The strategy used by the sheep to bring the hindlimb over a moderately high obstacle, set to 10% of its hindlimb length, was pronounced knee, ankle and metatarsophalangeal flexion when approaching and clearing the obstacle. Despite the overall time course kinematic patterns about the hip, knee, ankle, and metatarsophalangeal were identical, we found significant differences between values of the 2D and 3D joint angular motion. Chagas disease and visceral leishmaniasis are two neglected tropical diseases responsible for numerous deaths around the world. For both, current treatments are largely inadequate, resulting in a continued need for new drug discovery. As both kinetoplastid parasites are incapable of de novo purine synthesis, they depend on purine salvage pathways that allow them to acquire and process purines from the host to meet their demands. Purine nucleoside analogues therefore constitute a logical source of potential antiparasitic agents. Earlier optimization efforts of the natural product tubercidin (7-deazaadenosine) involving modifications to the nucleobase 7-position and the ribofuranose 3'-position led to analogues with potent anti-Trypanosoma brucei and anti-Trypanosoma cruzi activities. In this work, we report the design and synthesis of pyrazolo[3,4-d]pyrimidine nucleosides with 3'- and 7-modifications and assess their potential as anti-Trypanosoma cruzi and antileishmanial agents. One compound was selected for in vivo evaluation in an acute Chagas disease mouse model.

Phenotypic assay against Leishmania amazonensis in vitro and in vivo led to identification of an adamantyl-based phenyl sulfonyl acetamide (compound 1) as a promising antileishmanial agent. Compound 1 inhibited the growth of intracellular forms of L. amazonensis (50% inhibitory concentration [IC 50 ] = 4 μM) and exhibited low toxicity to host cells, with a selectivity index (SI) of >125. However, in a cutaneous leishmaniasis (CL) mouse model, compound 1 did not reduce lesions and parasite load when administered as monotherapy or when given simultaneously with a suboptimal dose of miltefosine.

Obesity leads to an imbalance in the autonomic nervous system, especially in increased sympathetic modulation and decreased vagal tone, and some anthropometric, metabolic, and lifestyle variables may increase the risk of developing cardiovascular disease. Objective. To analyze the association between cardiovascular autonomic modulation and biochemical and anthropometric markers, food intake, and physical activity level in severely obese individuals. Methodology. The present study is a cutout of a randomized clinical trial “Effect of nutritional intervention and olive oil in severe obesity” (DieTBra Trial), where the baseline data were analyzed. Anthropometric data, biochemical exams, heart rate variability (HRV), accelerometry, and 24 h recall (R24H) of obese patients (body mass index BMI ≥35 kg/m2) were collected. Results. 64 obese patients were analyzed, with a mean age of 39.10 ± 7.74 years (27 to 58 years). By HRV analysis, in the frequency domain, the obese had a higher predominance of sympathetic autonomic modulation (low frequency (LF) 56.44 ± 20.31 nu) and lower parasympathetic modulation (high frequency (HF) 42.52 ± 19.18 nu). A negative association was observed between the variables Homeostasis Evaluation Model (HOMA-IR) and HF (p=0.049). In the physical activity analysis, there was a negative association between moderate to vigorous physical activity and the sympathetic component (p=0.043), and for sedentary time (ST), there was a negative association with HF (p=0.049) and LF/HF (p=0.036) and a positive association with LF (p=0.014). For multiple linear regression, waist circumference (WC) and HOMA-IR values were negatively associated with HF (β = −0.685, p=0.010; β = −14.989, p=0.010; respectively). HOMA-IR (β = 0.141, p=0.003) and the percentage of lipids ingested (β = −0.030, p=0.043) were negatively associated with LF/HF. Conclusion. Among the cardiovascular risk variables studied, insulin resistance and central adiposity showed the greatest influence on cardiac autonomic modulation of obese, increasing the risk for cardiovascular disease.

Spinal cord trauma induced by ballistic projectiles is considered uncommon in domestic animals, particularly in cats. Outdoor, and indoor-outdoor cats are at greater risk of receiving gunshot wounds. Despite their small size, moderate speed and poor aerodynamic design, projectiles from compressed air guns (pellet guns) can cause severe injury. Treatment and prognosis of animals presented with gun-related injuries can vary considerably, depending on the affected spinal segment location of the lesions, and extent of tissue damage. Due to the unusual occurrence, of this type of trauma in feline patients, the goal of this report is to describe the neurologic, radiographic, and surgical findings in five cats with spinal injury secondary to air gun projectiles.

Currently, the best resources for assessment of bone tissue using imaging techniques are expensive and available in few medical facilities, thus serum or urinary bone turnover biomarkers could be useful as early indicators of prognosis. However, there is a wide range of variability in bone turnover markers due to several factors, such as different ages and metabolic stages, thus it is important to have as much data published on the subject as possible. Introduction: The objective of this study was to evaluate the short-term stability of the serum samples used as internal quality control (IQC) of human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (Aids) and syphilis immunodiagnostic assays. These samples were produced at the Center of Immunology-Instituto Adolfo Lutz (IAL), and they were distributed to laboratories participating in the IAL Quality Control Program. Method: The sera analyzed by chemiluminescence assay were stored at varied temperature conditions: from 2ºC to 8ºC (refrigerator), from 15ºC to 25ºC (room temperature), at 37ºC (incubator) and at -20ºC (reference temperature) for 12 and 24 hours. Results: Comparative analysis of IQC results for anti-HIV and anti-T. pallidum (anti-treponemal) showed stability in the reference temperature and at the various simulated temperatures for transporting the samples at the established lengths of time. The data from the simple linear regression analysis of negative serum samples (incubator/24 hours) and in one batch of HIV IQC (room temperature/24 hours) were statistically significant at the level of 5% (p-value < 0.05). Conclusion: The sera presented necessary requirements as reference material to be transported to laboratories at refrigeration temperature (2ºC to 8ºC), at the maximum shipping time of 12 hours. RESUMO Introdução: O objetivo deste estudo foi avaliar a estabilidade de curta duração de amostras de soro utilizadas como controle de qualidade interno (CQI) de testes imunodiagnósticos de vírus da imunodeficiência humana (HIV)/síndrome da imunodeficiência adquirida (Aids) e sífilis, produzidas no Centro de Imunologia do Instituto Adolfo Lutz (IAL) e distribuídas aos laboratórios participantes do Programa de Controle de Qualidade do IAL. Método: Os soros analisados por meio de ensaio de quimioluminescência foram armazenados em diferentes condições de temperaturas: de 2ºC a 8ºC (geladeira), de 15ºC a 25ºC (ambiente), 37ºC (estufa) e de -20ºC (referência) durante 12 e 24 horas. Resultados: A análise comparativa dos resultados do CQI HIV e T. pallidum (antitreponêmico) demonstrou que os materiais permaneceram estáveis, tanto na temperatura de referência quanto nas diferentes temperaturas simuladas para o transporte, no período de tempo estabelecido. No entanto, os resultados da análise de regressão linear simples das amostras de soro negativas (estufa/24 horas) e de um lote de CQI HIV (ambiente/24 horas) foram estatisticamente significativos ao nível de 5% (valor de p < 0,05). Conclusão: Os soros apresentaram requisitos necessários de material de referência para serem transportados aos laboratórios em temperatura de refrigeração (2ºC a 8ºC) no tempo máximo de 12 horas.

Bone turnover markers (BTMs) are product of bone cell activity and are generally divided in bone formation and bone resorption markers. The purpose of this review was to structure the available information on the use of BTMs in studies on small ruminants, especially for monitoring their variations related to diet, exercise, gestation and metabolic lactation state, circadian and seasonal variations, and also during skeletal growth. Pre-clinical and translational studies using BTMs with sheep and goats as animal models in orthopaedic research studies to help in the evaluation of the fracture healing process and osteoporosis research are also described in this review. The available information from the reviewed studies was systematically organized in order to highlight the most promising BTMs in small ruminant research, as well as provide a wide view of the use of sheep and goat as animal models in orthopaedic research, type of markers and commercial assay kits with cross-reactivity in sheep and goat, method of sample and storage of serum and urine for bone turnover markers determination and the usefulness and limitations of bone turnover markers in the different studies, therefore an effective tool for researchers that seek answers to different questions while using BTMs in small ruminants., José Arthur de A. Lower limb motor dysfunction and lack of balance is one of the most common and disabling sequelae affecting stroke. Objectives: Analyze the effects of the combination of Transcranial Direct Stimulation (tDCS) with peripheral stimulation (PES) on the activity of the tibialis anterior (TA) paretic and balance of hemiparetic post stroke.

Background Chagas’ disease, caused by the protozoan parasite Trypanosoma cruzi, needs urgent alternative therapeutic options as the treatments currently available display severe limitations, mainly related to efficacy and toxicity. Objectives As phosphodiesterases (PDEs) have been claimed as novel targets against T. cruzi, our aim was to evaluate the biological aspects of 12 new phthalazinone PDE inhibitors against different T. cruzi strains and parasite forms relevant for human infection. Methods In vitro trypanocidal activity of the inhibitors was assessed alone and in combination with benznidazole. Their effects on parasite ultrastructural and cAMP levels were determined. PDE mRNA levels from the different T. cruzi forms were measured by quantitative reverse transcription PCR. Results Five TcrPDEs were found to be expressed in all parasite stages. Four compounds displayed strong effects against intracellular amastigotes. Against bloodstream trypomastigotes (BTs), three were at least as potent as benznidazole. In vitro combination therapy with one of the most active inhibitors on both parasite forms (NPD-040) plus benznidazole demonstrated a quite synergistic profile (xΣ FICI = 0.58) against intracellular amastigotes but no interaction (xΣ FICI = 1.27) when BTs were assayed. BTs treated with NPD-040 presented disrupted Golgi apparatus, a swollen flagellar pocket and signs of autophagy. cAMP measurements of untreated parasites showed that amastigotes have higher ability to efflux this second messenger than BTs. NPD-001 and NPD-040 increase the intracellular cAMP content in both BTs and amastigotes, which is also released into the extracellular milieu. Conclusions The findings demonstrate the potential of PDE inhibitors as anti-T. cruzi drug candidates.

Objectives This study aimed to test the extensor carpi radialis and cranial tibial reflexes in cats before and after anesthetic block of the brachial and lumbosacral plexus, respectively, to determine whether they depend on a myotatic reflex arc. Methods Fifty-five cats with a normal neurologic examination that were referred for elective gonadectomy were divided into group 1 (29 cats) for testing the extensor carpi radialis reflex, and group 2 (26 cats) for testing the cranial tibial reflex. In group 1, the extensor carpi radialis reflex was tested after anesthetic induction and 15 mins after brachial plexus block with lidocaine. In group 2, the cranial tibial, withdrawal and patellar reflexes were elicited in 52 hindlimbs and retested 15 mins after epidural anesthesia. Results In group 1, before the anesthetic block, 55.17% of the cats had a decreased and 44.83% had a normal extensor carpi radialis reflex. After the block, 68.96% showed a decreased and 27.59% a normal reflex. No cat had an increased or absent reflex before anesthetic block. In group 2, prior to the anesthetic block, 15.38% of the cats had a decreased cranial tibial reflex and 84.62% had a normal response, whereas after the block it was decreased in 26.92% and normal in 73.08% of the cats. None of the cats had an increased or absent reflex. Regarding the presence of both reflexes before and after anesthetic block, there was no significant difference at 1% ( P = 0.013). Conclusions and relevance The extensor carpi radialis and cranial tibial reflexes in cats are not strictly myotatic reflexes, as they are independent of the reflex arc, and may be idiomuscular responses. Therefore, they are not reliable for neurologic examination in this species.

The recovery of walking function following spinal cord injury (SCI) is of major importance to patients and clinicians. In experimental SCI studies, a rat model is widely used to assess walking function, following thoracic spinal cord lesion. In an effort to provide a resource which investigators can refer to when seeking the most appropriate functional assay, the authors have compiled and categorized the behavioral assessments used to measure the deficits and recovery of the gait in thoracic SCI rats. These categories include kinematic and kinetic measurements. Within this categorization, we discuss the advantages and disadvantages of each type of measurement. The present review includes the type of outcome data that they produce, the technical difficulty and the time required to potentially train the animals to perform them, and the need for expensive or highly specialized equipment. The use of multiple kinematic and kinetic parameters is recommended to identify subtle deficits and processes involved in the compensatory mechanisms of walking function after experimental thoracic SCI in rats.

Objetivou-se avaliar as implicações e os resultados obtidos frente aos tratamentos conservativo e cirúrgico em 37 cães sem distinção de sexo, raça ou idade, que apresentaram fraturas e luxações vertebrais (FLV) toracolombares. Cada paciente passou por anamnese, seguida por exames clínico, neurológico e radiográficos para se obter o diagnóstico neuroanatômico e a severidade da lesão. Os cães foram submetidos ao tratamento conservativo ou cirúrgico de acordo com as características de cada caso. Dos 37 animais deste estudo, 37,7% tinham percepção de dor profunda (PDP) intacta. Vinte animais foram submetidos ao tratamento conservativo, 14 ao tratamento cirúrgico e três animais faleceram antes do tratamento. Dos animais que mantiveram a PDP intacta, a taxa de recuperação foi de 100%, com tempo de recuperação total variando de sete a 75 dias, ao passo que, dos animais que perderam a PDP, nenhum recuperou a deambulação voluntária. Dos 14 animais operados, 42,8% apresentaram alterações vertebrais não visibilizadas ao exame radiográfico convencional, sendo necessária mudança na técnica cirúrgica planejada em 14,2% desses casos. Nos animais que mantiveram a PDP, não houve diferença significante nas taxas nem no tempo de recuperação da deambulação voluntária, independentemente do tipo de tratamento. Entretanto, houve diferença altamente significante entre os graus que mantiveram a PDP intacta com os animais que perderam a percepção de dor profunda em relação à taxa de recuperação. A taxa de eutanásia foi menor que em outros trabalhos descritos na literatura. Dos animais sem PDP submetidos ao tratamento conservativo ou cirúrgico, 31,25% readquiriram a capacidade de caminhar sem recuperar a PDP, sendo essa deambulação involuntária atribuída ao caminhar espinal. Our aim of this paper was to study the implications and results associated with conservative and surgical treatment of dogs with thoracolumbar Vertebral fractures and luxations (VFL) so that the information obtained can be used to obtain guidelines that lead to greater recovery rates. For such, 37 dogs presenting VFL were examined, without distinction of sex, breed, or age. Each patient had detailed history obtained, followed by clinical, neurologic and radiographic exams for obtaining neuroanatomic location and severity of the nervous and vertebral lesions. The dogs were submitted to conservative or surgical treatment according to each case. Deep pain perception (DPP) was intact in 37.7% of the 37 dogs in this study. Twenty dogs received conservative treatment, 14 underwent surgery and three dogs died before treatment. Of the dogs with intact DPP, 100% recovered, with a recovery time varying between seven and 75 days, while none of the dogs that lost DPP recovered voluntary motion. Of the 14 dogs submitted to surgery, 42.8% had vertebral lesions that weren't detected using conventional radiography, which demonstrates the low sensitivity of this exam, and led to a need for a change in the planned surgical technique in 14.2% of these cases. In dogs with intact DPP there was no significant difference in time or rate of recovery of voluntary motion, regardless of the treatment method. However, there was a highly significant difference between the dogs which retained and lost DPP regarding recovery rates. The percentage of euthanasia was less than in other reports, probably due to the greater awareness and protectiveness of owners nowadays. Of the dogs without DPP treated either surgically or conservatively, 31.25% recovered ambulation without recovery of DPP, characterizing this ambulation as involuntary, attributed to spinal walk.

Five bis-arylimidamides were assayed as anti- Trypanosoma cruzi agents by in vitro , in silico , and in vivo approaches. None were considered to be pan-assay interference compounds. They had a favorable pharmacokinetic landscape and were active against trypomastigotes and intracellular forms, and in combination with benznidazole, they gave no interaction. The most selective agent (28SMB032) tested in vivo led to a 40% reduction in parasitemia (0.1 mg/kg of body weight/5 days intraperitoneally) but without mortality protection. In silico target fishing suggested DNA as the main target, but ultrastructural data did not match.

Functional recovery following general nerve reconstruction is often associated with poor results. Comparing to rat and mice experimental studies, there are much fewer investigations on nerve regeneration and repair in the sheep, and there are no studies on this subject using gait analysis in the sheep model as an assessment tool. Additionally, this is the first study evaluating obstacle negotiation and the compensatory strategies that take place at each joint in response to the obstacle during locomotion in the sheep model. This study aims to get kinematic data to serve as a template for an objective assessment of the ankle joint motion in future studies of common peroneal nerve (CP) injury and repair in the ovine model. Our results show that a moderately high obstacle set to 10% of the sheep's hindlimb length was associated to several spatial and temporal strategies in order to increase hoof height during obstacle negotiating. Sheep efficiently cleared an obstacle by increasing knee, ankle and metatarsophalangeal flexion during swing, whereas the hip joint is not affected. This study establishes the bounds of normal motion in the neurologically intact hindlimb when approached and cleared an obstacle and provides baseline data for further studies of peripheral nerve research in the ovine model.

The objective of the present study was to evaluate if extradural contact during hemilaminectomy would cause neurological deterioration in the early and/or late postoperative period in dogs with intervertebral disc extrusion. Nineteen dogs with thoracolumbar intervertebral disc extrusion underwent hemilaminectomy for spinal cord decompression and removal of extruded disc material. Meningeal contacts during surgery were quantified. Paraplegia (with nociception) and paraparesis were observed in 11/19 and 8/19 of dogs, respectively, before surgery. At the end of our study, only two (2/19) had paraplegia and one (1/19), paraparesis. There were more extradural contacts when extruded intervertebral disc material was at a ventrolateral position. Extradural contacts during surgery had no influence on neurological progression nor on time to recovery of motor function. Immediately (24 and 48 hours) after surgery, 13/19 dogs had the same neurological stage before surgery. At 7 and 90 days, 13/19 and 17/19 dogs, respectively, showed neurological improvement, compared with their preoperative stage. There was no influence of the number of extradural contacts on neurological recovery. These findings indicate that a careful inspection of the vertebral canal for removal of as much extruded disc material as possible does not cause neurologic deterioration. RESUMO O presente trabalho teve como objetivo avaliar se contatos extradurais durante hemilaminectomia em cães com extrusão de disco intervertebral causariam piora neurológica no pós-operatório imadiato e/ou tardio. Dezenove cães com extrusão toracolombar de disco intervertebral foram submetidos à hemilaminectomia para descompressão medular e remoção do material extruso. Durante o procedimento cirúrgico, os contatos meningomedulares foram quantificados. Antes da cirurgia, 11/19 cães apresentavam paraplegia (com nocicepção) e 8/19 cães, paraparesia. Ao fim do estudo, apenas dois cães (2/19) mostravam paraplegia com dor profunda e um (1/19), paraparesia. Observou-se maior quantidade de contatos extradurais quando o material discal extruso encontrava-se em posição ventrolateral. Os contatos extradurais não mostraram influência estatística na evolução neurológica dos animais, bem como no tempo de recuperação das funções motora. Vinte e quatro e 48 horas após a cirurgia, 13/19 cães apresentavam o mesmo grau neurológico de antes da cirurgia. Após sete e 90 dias de pós-operatório, 13/19 e 17/19 demonstraram melhora neurológica em comparação com o pré-operatório, respectivamente. A quantidade de contatos extradurais não influenciou na recuperação neurológica dos cães. Esses achados indicam que uma inspeção minuciosa do canal vertebral pode ser recomendada, a fim de remover o máximo de material discal extruso, evitando-se piora neurológica por compressão medular.

Metronidazole (Mtz) is a commercial broad-spectrum nitroimidazolic derivative with relevant antimicrobial activity and relative safety profile. Therefore, it is fair to consider Mtz a candidate for drug repurposing for other neglected conditions such as Chagas disease (CD), a parasitic pathology caused by Trypanosoma cruzi. CD is treated only with benznidazole (Bz) and nifurtimox, both introduced in clinics decades ago despite important limitations, including low efficacy on the later disease stage (chronic form) and severe side effects. New cheap and fast alternative treatments for CD are needed, thus the repurposing of Mtz was assessed in vitro and in vivo in mono- and combined therapy. In vitro assays demonstrated EC50>200µM for Mtz, while for Bz the values ranged from 2.51µM (intracellular forms) to 11.5µM (bloodstream trypomastigotes). When both drugs were combined in fixed-ratio proportions, Mtz promoted Bz potency (lower EC50 values). In vivo toxicity assays for Mtz in mice showed no adverse effects neither histopathological alterations up to 2000mg/kg. Regarding experimental T. cruzi infection, Bz 100mg/kg suppressed parasitemia while Mtz (up to 1000mg/kg) in monotherapy did not, but prolonged animal survival at 250 and 500 regimen doses. The combination of both drugs (Bz 10+Mtz 250) prevented mortality (70%) besides protected against electric cardiac alterations triggered by the parasite infection. Although not able to reduce parasite load, the combination therapy prevented animal mortality; this was possibly due to a protection of the electric cardiac physiology that is normally altered in experimental infection of T. cruzi. It also suggested that the interaction with Mtz could have improved the pharmacokinetics of Bz. Our study emphasizes the importance of drug repurposing and combined therapy for CD to contribute to alternative therapies for this neglected and silent pathology.

Introduction: The present study reports the data from the first homogeneity assessment of samples composing the serum panels produced at the Immunology Center of Instituto Adolfo Lutz, São Paulo. These samples have been distributed to the public laboratories and those partaking in the Brazilian Unified Health System, and to the participants in the Internal Quality Control Program for human immunodeficiency virus (HIV) antibody (Ab) testing. Objective: To assess the homogeneity of serum samples in panels from different lots for HIV/acquired immunodeficiency syndrome (AIDS) immunodiagnosis by using the statistical method to ensure quality of the reference material. Method: Sera homogeneity was evaluated by means of enzyme-linked immunoassay/enzyme immunoassay (ELISA/EIA) for detection of HIV Ab, and the one-way analysis of variance was employed for analyzing the data. No statistically significant differences were found among the several serum vials. Conclusion: The sera dispensed in the vials were homogeneous in the respective lots. Introdução: No presente estudo estão descritos os resultados das primeiras análises feitas sobre a avaliação da homogeneidade das amostras componentes de painéis de soros produzidos no Centro de Imunologia do Instituto Adolfo Lutz e distribuídos aos laboratórios públicos e conveniados ao Sistema Único de Saúde e participantes do Programa de Controle de Qualidade Interno para imunodiagnóstico de vírus da imunodeficiência humana/síndrome da imunodeficiência adquirida (HIV/AIDS). Objetivo: Avaliar a homogeneidade das amostras de soro componentes de painéis de diferentes lotes para imunodiagnóstico de HIV/Aids por meio de método estatístico para garantir a qualidade do material de referência. Material e método: A homogeneidade das amostras de soro foi avaliada por meio de enzyme-linked immunoassay/enzyme immunoassay (ELISA/EIA) para detecção de anticorpos anti-HIV, e os resultados foram submetidos à análise de variância fator único. Não foram encontradas diferenças significativas entre os resultados obtidos para os diversos frascos de soro. Conclusão: As amostras distribuídas nos frascos foram homogêneas entre si nos respectivos lotes.

Introduction: The use of reference materials in order to assure and perform the quality control of analytical measurements is a requirement in clinical laboratories. Objectives: Stability of serum samples, kept frozen at -20°C for long-term storage and at varied temperatures during short periods, was evaluated by investigating the persistency of anti-human immunodeficiency virus (HIV) antibodies reactivity on enzyme-linked immunosorbent assay/enzyme immunoassay (ELISA/EIA), Western blot and indirect immunofluorescence assays. Method: The analyzed sera were part of serum panels (comprised of anti-HIV positive and negative samples), produced at the Immunology Center of Instituto Adolfo Lutz, which have been the reference specimens for producing the internal quality assurance sera of HIV/acquired immunodeficiency syndrome (AIDS) immunodiagnostic assays. Sera stability was assessed in samples stored at -20°C for 56 weeks, and at various temperature conditions: from 2°C to 8°C (refrigerator), from 15°C to 25°C (room temperature), at 37°C (incubator) and at -80°C (freezer) for 24 and 48 hours. The statistical analyses on HIV-negative serum samples (long-term storage) were significant (p < 0.05), and neither adverse effects on these samples as the occurrence of false-positive results nor false-negative results in HIV antibody positive sera were found in both studies. Conclusion: It was possible to conclude that the reference material remained stable for 48 hours at different temperatures (short-term) and it remained stable at -20°C for 56 weeks (long-term). Introdução: A utilização de materiais de referência para assegurar e desempenhar o papel de controle de qualidade das medições analíticas é requisito em laboratórios clínicos. Objetivo: O presente estudo avaliou a estabilidade das amostras de soro, armazenadas a -20°C por um longo período de tempo e durante curto prazo em diferentes temperaturas, quanto à invariabilidade da reatividade de anticorpos contra o vírus da imunodeficiência humana (HIV), por meio de enzyme-linked immunosorbent assay/enzyme immunoassay (ELISA/EIA), Western blot e imunofluorescência indireta. Método: As amostras analisadas foram provenientes de painéis de soros (constituídos de amostras anti-HIV positivo e negativo), produzidos no Centro de Imunologia do Instituto Adolfo Lutz (IAL), os quais têm sido material de referência para o preparo de amostras do controle de qualidade interno de testes imunodiagnósticos de HIV/síndrome da imunodeficiência adquirida (AIDS). A avaliação da estabilidade dos soros foi efetuada em amostras armazenadas a -20°C por 56 semanas e nas diferentes condições de temperaturas: de 2°C a 8°C (geladeira), de 15°C a 25°C (ambiente), 37°C (estufa) e -80°C (freezer), durante 24 e 48 horas. Os resultados das análises de tendência das amostras de soro HIV negativo (armazenamento de longo prazo) foram significantes (p < 0,05) e nenhum efeito adverso foi observado nessas amostras, como a ocorrência de resultados falso-positivos, tampouco foram detectados resultados falso-negativos em amostras de soro positivas para detecção de anticorpos anti-HIV em ambas as avaliações. Conclusão: Foi possível concluir que o material de referência manteve-se estável por 48 horas nas diferentes temperaturas (curto prazo) e permaneceu estável a -20°C por 56 semanas (longo prazo).

Background Quality of life must be one of the main purposes for the treatment of drug users, requiring a better understanding of the association between the quality of life and the severity of dependency. This study aimed to investigate the correlation between severity of substance use in various areas of human functioning and quality of life of illicit drug users in a psychosocial care center for alcohol and drugs. Methods This cross-sectional study included 60 participants – illicit drug users – treated at a psychosocial care center for alcohol and drugs. Participants were evaluated with the short version of World Health Organization Quality of Life (WHOQOL-Bref) instrument to measure the quality of life, the 6th version of Addiction Severity Index (ASI-6) to assess the severity of dependence in several areas and the Mini International Neuropsychiatric Interview (MINI) to identify the presence of psychiatric disorders. Pearson and Spearman correlation tests and linear regression were applied to verify the association between the severity of dependence and the quality of life, and Student’s t-test to compare the mean quality of life between individuals with and without psychiatric comorbidities. Results Negative correlation was found between the severity of dependence on the drugs dimensions: alcohol, psychiatric, medical, legal, family/social support and family/social problems of ASI-6, and the quality of life domains measured by the WHOQOL-Bref. The evidence was strongest in the psychiatric and medical dimensions. There was a significant difference in the quality of life mean among participants presenting or not presenting psychiatric comorbidities, for the psychological domain in anxiety disorders, and for the physical and psychological domains in mood disorders. Conclusion The quality of life decreased as the severity of dependence increased, with different results in the various areas of the participant’s life. This result emphasizes the need for training the professional team which works in the substance use disorders area for more comprehensive diagnostic evaluations and more appropriate therapeutic interventions for each area. The associations were more evident in the medical and psychiatric fields, indicating the need for greater attention to be paid in relation to medical and psychiatric comorbidities. Of all the detrimental effects of spinal cord injury (SCI), one of the most devastating is the disruption of the ability to perform functional movement. Very little is known on the recovery of hindlimb joint kinematics after clinically-relevant contusive thoracic lesion in experimental animal models. A new functional assessment instrument, the dynamic feet distance (DFD) was used to describe the distance between the two feet throughout the gait cycle in normal and affected rodents. The purpose of this investigation was the evaluation and characterization of the DFD during treadmill locomotion in normal and T9 contusion injured rats, using three-dimensional (3D) instrumented gait analysis. Despite that normal and injured rats showed a similar pattern in the fifth metatarsal head joints distance excursion, we found a significantly wider distance between the feet during the entire gait cycle following spinal injury. Peripheral nerve injury and regeneration is a challenging scientific field with relevant clinical implications. Most peripheral nerve regeneration studies have been mainly carried out on rodents. However, it is important to note that the validity of the rodent as a model to study nerve injury and regeneration and translate these results into clinical practice has been questioned by several researchers. To overcome this problem, some investigators have used companion animals and large animal species as models for experimental peripheral nerve regeneration studies. Live sheep are often used in biomedical research because of availability, simplicity of care and housing, cost and body weight similar to humans and acceptance by society as a research animal. Despite these advantages, studies on nerve regeneration and repair in sheep have only been undertaken a few decades ago and compared to rat and mice experimental studies, there are much fewer investigations. The authors have compiled and sorted the available literature on experimental ovine nerve studies in order to guide the peripheral nerve investigator in choosing clinically relevant and interpretable models for studies on neural regeneration that are much needed in order to make progress towards new surgical and medical treatment of peripheral nerves.

Patients and methods: In this retrospective analysis, mRCC patients in two institutions in Latin America received sunitinib at a starting dose of either 50 mg/day for 4 weeks followed by 2 weeks off treatment (Schedule 4/2) in repeated 6-week cycles or sunitinib 37.5 mg on a continuous daily dosing schedule. Clinical characteristics, tolerability, and PFS data were collected. Results: Twenty-nine patients with long-term clinical benefit from sunitinib were identified between September 2005 and August 2009. Median PFS was 23 months (range: 15–54 months). Two of the 29 patients with prolonged PFS achieved a complete response and additional eleven had a partial response. Most patients were aged

Fleet enemas are hypertonic solutions with an osmotic action and a high concentration of phosphate. When retained in the human body they have a great toxic potential, causing severe hydro-electrolyte disorders in children, especially in newborns. We report the case of a previously healthy 8-day-old newborn who needed neonatal intensive care treatment after the inadvertent administration of an osmotically active hypertonic phosphate enema. Taking into account that phosphate removal by peritoneal dialysis (PD) strongly depends on total dialysate turnover, we chose continuous flow PD (CFPD) as the treatment option, with a successful outcome. Clinical experience with this dialytic modality is limited to a few case reports in pediatric and adult patients. To the best of our knowledge, we report here the first description of CFPD in the setting of acute phosphate nephropathy in the neonatal period. The modality of PD described here has potential as an alternative management option as it is a highly efficient, methodologically simple, and low-cost method without any need for sophisticated equipment. Physicians and parents should be aware of the adverse effects of a hypertonic phosphate enema and should never use these medications in infants and newborns.

They require similar medical and surgical care as small animals, and orthopedic lesions are more and more common. Fracture diagnosis is based on history, orthopedic exam, and diagnostic imaging. The preferred treatment for femur fractures is surgery, which has a favorable outcome in the majority of cases. However, surgical fracture repair in ferrets can be challenging due to the small size of the bones. The objective of this paper is to report the surgical repair of a femoral fracture in a ferret using an external skeletal fixation system type IA connected to an intramedullary pin (tie-in configuration). Case: An adult ferret was referred to the veterinary hospital after being stepped on. The ferret was diagnosed with an oblique diaphyseal fracture of the left femur and surgery was recommended. Open fracture reduction was performed with a 1 mm intramedullary pin (Kirschner wire) placed in a retrograde direction into the proximal bone segment. After fracture reduction, the intramedullary pin was inserted into the distal bone segment. The external skeletal fixator type IA was created by the insertion of a 1.5 mm Schanz pin into the distal diaphysis and another, also 1.5 mm, inserted into the proximal metaphysis, both percutaneously. The three pins were connected externally to an aluminum bar with the aid of three staples to form a tie-in system. Rest and restriction of activity were recommended during the first month after surgery. The ferret recovered well and, at 120 days, orthopedic and radiographic exams showed complete fracture healing. The implant was removed and no functional changes related to ambulation were observed. Discussion: The orthopedic knowledge applied to ferrets commonly originates in procedures performed on cats and dogs. Overall, the main objective when dealing with fractures is to stabilize the broken bone to allow early ambulation. Due to the location and type of fracture, an external immobilization was contraindicated in this case. A plate was considered, but the small diameter of the femur and the consequent risk of iatrogenic fractures made this option not feasible. Therefore, the external skeletal fixator in a tie-in configuration was selected. This method permitted control of the forces acting on the fracture site and had the advantage of low cost and ease of application and removal. Nevertheless, the vast musculature that covers the femur could hinder the use of the external pins and lead to discomfort. The method has been reported with some changes in ferrets, though with inconclusive results due to lack of follow-up. In the present report, a less rigid method, type I, was used since types II and III have a biplanar configuration and were thus inadequate due to the possible contact between the fixator and the abdominal or inguinal region. The precautions taken during placement of the implant were the same as in cats and dogs since their anatomy follows the same pattern. The implant was rigid, light, and effective, allowing early weight-bearing on the affected limb without discomfort to the patient. As such, the external fixator type IA connected to the intramedullary pin in a tie-in configuration was an effective method for the treatment of the oblique diaphyseal femoral fracture in this ferret. Although it is not the recommended method for this type of fracture in cats and dogs, there were no complications in the ferret, with complete fracture healing and return to function of the affected limb. Since there has yet to be a consensus on the recommended procedures in these animals, reports of this nature are important due to the growing number of ferret patients in veterinary practice. Primary erythrocytosis, or polycythemia vera, is a myeloproliferative disease caused by the exaggerated increase of erythroid precursor cells in the bone marrow. We report the case of an 11-year-old male mixed-breed dog that had tachypnea and spastic tetraplegia. There was a significant increase in hematocrit. After phlebotomy and fluid therapy, the dog's condition improved. A diagnosis of primary erythrocytosis was supported by serum levels of erythropoietin. The dog responded well to treatment with administration of hydroxyurea (15 mg/kg), phlebotomies, and fluid therapy. However, after 18 months, he had an acute recurrence of clinical signs and was euthanized. We observed that long-term maintenance with hydroxyurea at a dosage of 15 mg/kg every 48 hours was adequate for managing polycythemia vera, with a survival time of 18 months in the present case. However, longer dose intervals are likely not appropriate. We believe that this may be helpful to other veterinarians facing the same problems in the treatment of polycythemia vera.

This is a cross-sectional descriptive study aimed at identifying the epidemiological profile of the patients treated at a Psychosocial Care Center for Alcohol and other type III drugs.Data were collected in 2014 from the records of 519 users, using a previously prepared epidemiological form. Most of patients are men attheproductive age; with low schooling and low family income.Most make use of alcohol, have undergone previous treatments in psychiatric hospitals and attended the unit for treatment by influence of third parties, even when it is contemplated. The users of the service are people with serious social and health commitments and although they are increasing use of crack, which is the current concern of public policies, alcohol is still the drug of predilection and represents the gateway to other drugs

Tea presents a diverse phenolic composition which is responsible for its alleged biological activities, including the in vivo and in vitro antioxidant capacity. It is very usual to find researches applying statistical methods, such as analysis of variances (ANOVA) and linear Pearson correlation coefficients to analyze the strength of correlation between phenolic composition and the in vitro antioxidant activity of teas from Camellia sinensis (green, black, white, oolong, red, and yellow teas) and Ilex paraguariensis (Yerba-mate), which are the most produced and consumed types of teas. However, evidence has shown that these approaches are not as suitable as multivariate statistical methods once they do not depict nor show association among all results and variables simultaneously, making it difficult to understand clearly the data structure and patterns. Then, the objective of this work is to review and explain some univariate/bivariate and multivariate statistical techniques used to assess the association between phenolic compounds and the in vitro antioxidant activity of green, white, black, red, yellow, oolong and Yerba-mate teas. Moreover, this paper provides an overview on some assays used to estimate the in vitro antioxidant capacity of teas.

Introduction: The antibodies have an important role in the serodiagnosis, constituting the most widely used biomarkers to detect and confirm various diseases. Objective: To investigate the reproducibility of anti-human immunodeficiency virus (HIV) antibodies reactivity, to assess the stability of the sera samples stored at -20ºC for two to eighteen years. Method: Sera were collected in the period 1988-2004 for routine anti-HIV antibodies diagnostic testing. The remaining samples stored at -20ºC, were analyzed in this study. Serum sample stability was assessed by enzyme-linked immunosorbent assay/enzyme immunoassay (ELISA/EIA), indirect immunofluorescence assay (IFA), and Western blot (WB) for detecting anti-HIV antibodies. The previously found results (1988-2004) and those obtained in 2006 were subjected to Kappa index analysis. Result: In the period 1988-to 2004, the degree of concordance of the ELISA/EIA, IFA and WB results were considered, good (k = 0.80), regular (k = 0.35), and good (k = 0.63), respectively. Conclusion: Regarding HIV serologic test, the serum samples were stable for 18 years in ELISA/EIA and for 4 years in IFA technique, however, for the WB methodology it was not possible to determine the time of stability of the anti-HIV antibodies. Introdução: Os anticorpos possuem papel fundamental no sorodiagnóstico e constituem os mais difundidos biomarcadores empregados na detecção e na confirmação de diversas doenças. Objetivo: Verificar a reprodutibilidade dos resultados de anticorpos contra o vírus da imunodeficiência humana (HIV) para avaliar a estabilidade dos soros armazenados a -20ºC durante período de dois a dezoito anos. Método: Os soros utilizados foram provenientes de amostras remanescentes da rotina diagnóstica para detecção de anticorpos anti-HIV, no período de 1988 a 2004, os quais estavam armazenados em freezer a -20ºC. A estabilidade das amostras de soro foi avaliada por meio de enzyme-linked immunosorbent assay/enzyme immunoassay (ELISA/EIA), imunofluorescência indireta (IFI) e Western blot (WB) para detecção de anticorpos anti-HIV, e os resultados dos testes realizados anteriormente (1988-2004) e os obtidos em 2006 foram submetidos à análise do índice Kappa. Resultado: No período de 1988-2004, os graus de concordância dos resultados do ELISA/EIA, IFI e WB foram considerados, respectivamente, bom (k = 0,80), regular (k = 0,35) e bom (k = 0,63). Conclusão: No que diz respeito à sorologia para HIV, as amostras de soro foram estáveis por 18 anos no ELISA/EIA e por quatro anos na técnica de IFI, no entanto, para a metodologia de WB, não foi possível determinar o tempo de estabilidade dos anticorpos anti-HIV.

Este estudo contextualiza a Enfermagem Psiquiátrica e a Saúde Mental no Brasil, considerando os principais marcos históricos, políticos e sociais que influenciam o cuidado de enfermagem nessa área. As reflexões apontam para mudanças no paradigma da atenção psicossocial considerando os aspectos legais que regulamentam as ações, os avanços e desafios vivenciados na prática da Enfermagem Psiquiátrica no contexto assistencial assim como na formação do enfermeiro. É fundamental formar profissionais capazes de atuar na gestão e assistência na perspectiva interdisciplinar e em rede. As conquistas almejadas para esta especialidade perpassam por uma comunidade científica crítica e organizada politicamente que conheça e atenda a realidade dos profissionais. Neste panorama, o Departamento Científico de Enfermagem Psiquiátrica e Saúde Mental, criado pela Associação Brasileira de Enfermagem se instala como uma estratégia para congregar e consolidar o trabalho dos especialistas, visando a excelência da Atenção Especializada em Psiquiatria e Saúde Mental. This study contextualizes the Psychiatric Nursing and Mental Health in Brazil, considering the major historic, political and social factors that influence nursing care in this area. Reflections pointed to changes in the paradigm of psychosocial care considering the legal aspects that regulate the actions, progress and challenges experienced in the practice of Psychiatric Nursing in healthcare settings as well as in nursing education. It is essential to train professionals to work in management and assistance in perspective and interdisciplinary care network. The achievements for this specialty rush on a scientific critical and organized politically and who knows answer the reality of professionals. In this view, the Scientific Department of Psychiatric Nursing and Mental Health, created by the Brazilian Association of Nursing was installed as a strategy to bring together and consolidate the work of experts, aiming to achieve the excellence of the Specialized Care in Psychiatry and Mental Health. Este estudio contextualiza la Enfermería Psiquiátrica y de Salud Mental en Brasil, teniendo en cuenta los principales factores históricos, políticos y sociales que influyen en la atención de enfermería en esta área. Reflexiones señalaron a los cambios en el paradigma de la atención psicosocial, considerando los aspectos legales que rigen las acciones, avances y desafíos experimentados en la práctica de la enfermería psiquiátrica en los centros de salud, así como en la formación de enfermería. Es fundamental la formación de profesionales que trabajen en la gestión y la asistencia en la creación de redes y la perspectiva interdisciplinaria. Los logros almejados para esta especialidad pasan por una comunidad científica crítica y organizada políticamente, que conozca y atienda la realidad de los profesionales. En este escenario, el Departamento Científico de Enfermería Psiquiátrica y de Salud Mental, creado por la Asociación Brasileña de Enfermería, se instala como una estrategia para reunir y consolidar el trabajo de los expertos, visando la excelencia de la Atención Especializada en Psiquiatría y Salud Mental.

SHU atypical (aHUS), that is, not associated with Escherichia coli Shiga toxinproducing, is seen in 5 to 10% of cases of Hemolytic Uremic Syndrome (HUS), and can occur at any age and may be sporadic or familial. The prognosis in these cases is reserved, with high mortality and morbidity in the acute phase of the disease, and about 50% of cases can develop chronic kidney disease. The increased knowledge of the pathogenesis of aHUS (overactivation of the alternative pathway of complement), was accompanied by the appearance of a drug, eculizumab, which acts as an inhibitor of membrane attack complex. Our goal is to report a case of infant with aHUS with excellent clinical and laboratory response with the use of eculizumab. 14 month old infant, previously healthy, male, presented anemia and thrombocytopenia at 12 months of age. He was treated with corticosteroids and forwarded to our service for high blood pressure. However, the scans showed nephrotic proteinuria with renal involvement and hypoalbuminemia with direct Coombs negative. He developed anemia, thrombocytopenia, worsening of renal function and hypertension. Renal biopsy showed thrombotic microangiopathy (TMA). On the non-hemolytic anemia, thrombocytopenia and acute renal failure with histological substrate MAT, was diagnosed of aHUS. The patient received eculizumab excellent clinical and laboratory response. This case shows the importance of early diagnosis and treatment of the aHUS. Eculizumab is effective and keeps long-term remission, avoiding invasive measures such as plasmapheresis, which resolves only part of the picture.

The purpose of this research was to study the occurrence of "caregiver role strain" in family members of patients hospitalized for the treatment of chronic diseases. Cross-sectional study involving 12 families, which were interviewed during the hospitalization period of the person index in a teaching hospital in the Midwest of Brazil, using the Calgary Family Assessment Model (CFAM). The NANDA-I taxonomy was applied to state the diagnosis, and it was necessary to make additions to the diagnosis components, which are highlighted in italics. All the families presented the studied diagnosis, and the following related factors were identified in more than 50% of them: liabilities related to the 24-hour care; the caregiver is female ; lack of adequate support, inadequate physical environment for care; and the defining characteristic of disturbed sleep pattern. The actions of health professionals should be directed so as to assist in the interaction and management of the patient’s daily life; contribute to alleviate the burden; facilitate the cooperation and reduce the stress factors that could trigger crises.

O câncer cervical é a segunda causa mais comum de câncer entre as mulheres no mundo, apesar de apresentar grande potencial de prevenção e cura quando diagnosticado e tratado precocemente, o que pode reduzir a taxa de mortalidade entre as acometidas. Como não há consenso entre as medidas terapêuticas nas lesões intraepiteliais escamosas cervicais de alto grau (HSIL), buscamos discutir a conduta frente às mulheres adultas que apresentam HSIL e o seguimento após o procedimento adotado. Foi realizada pesquisa eletrônica no Medline (através do PubMed), National Guidelines Clearinghouse, Google Acadêmico e Lilacs. Os consensos identificados foram avaliados segundo sua validade e recomendações. Em relação ao acompanhamento após o tratamento, o Europeu acompanha por citologia de 6/6 meses; o Australiano, citologia e colposcopia de 6/6 meses; o Americano preconiza a realização da captura híbrida em seis a 12 meses ou citologia em seis meses. Já o Projeto Diretrizes do Brasil recomenda que reavaliações clínicas e colpocitológicas devam ser realizadas a cada três ou quatro meses nos primeiros dois anos de seguimento. Estudos comparando o método "Ver e Tratar" com os três passos (histologia, colposcopia, biópsia) concluíram que este último é indicado em mulheres LSIL/ASCUS antes de se submeterem à exérese da zona de transformação (EZT), enquanto que o "Ver e Tratar" é indicado em mulheres com HSIL comprovada na citologia e sugestiva na colposcopia, pois apresenta como vantagens, o baixo custo e a resolução imediata. Todos os consensos são unânimes ao afirmar que frente à HSIL comprovada, retira-se a lesão por meio de ablação ou conização ou EZT. Cervical cancer is the second most common cancer among women worldwide, despite having great potential for prevention and cure when early diagnosed and treated, which can reduce the mortality rate among the affected. Since there is no consensus among the therapeutic measures in high grade cervical squamous intraepithelial lesions (HGSIL), we discuss its approach when dealing with adult women who have HGSIL and the follow-up after the adopted procedure. We performed electronic searches of MEDLINE (through PubMed), National Guidelines Clearinghouse, Google Scholar and Lilacs. The guidelines identified were evaluated according to their validity and recommendations. In relation to after-care, the European use cytology every 6 months, the Australians, cytology and colposcopy every 6 months, the Americans advocate the realization of hybrid capture in six to 12 months or cytology every 6 months. The Brazilian Guidelines Project, on its turn, recommends that clinical reassessments and Pap smear should be performed every three or four months during the first two years of follow-up. Studies comparing the method "See and Treat" with the three steps one (histology, colposcopy, biopsy) concluded that the latter is indicated for women ASCUS/LSIL before undergoing the excision of the transformation zone (ETZ), while the "View and Treat" is indicated in women with proven HGSIL in cytology and suggestive in colposcopy, because it presents advantages such as low cost and immediate solution. All the guidelines are unanimous in stating that when facing proven HGSIL, excision of the injury through ablation or conization or ETZ is indicated.

Mental suffering is present in every situation that involves the falling ill process. The prevalence of ordinary mental disturbances increases the demand of mental services, therefore it's made necessary to value this dimension as a health indicator in any level of attention. The goal of this study was to learn how the nurses who attend in basic units acknowledge actions of mental health. A qualitative descriptive exploratory study run in Botucatu-SP, driven by interviews, in the period of November and December of 2003 and with five nurses, which content led to two categories: the quotidian of basic units and mental health/ actions of mental health performed by the nurses. Data shows that despite the situations lived in the unit that involves the welcoming and listening to the patients in psychic suffering and in social isolation, there isn't any programmatic action focused on the clientele. The patients are guided to specialized services and the nurses recognize as actions of mental health only the control of psychiatric medications and orientations given from time to time, although, the nurses point to the need of specializing the whole staff to attend this type of clientele. We are able to understand that it's essential to the integrality of the basic attention the establishment of an interface with the actions of mental health. Mycoplasma arthritidis causes arthritis in rodents that resembles human rheumatoid arthritis. It produces a superantigen (MAM) that stimulates production of cytokines by making a bridge between lymphocyte T-cell receptor with the appropriate Vbeta chain, and H-2 1-Ealpha MHC class II molecules. Here we studied MAM-induced nitric oxide (NO) production in mouse peritoneal macrophages and found that it was: (1) time and concentration dependent, (2) possibly derived from inducible NOS synthase since it was reduced significantly by amino guanidine pretreatment, (3) restricted to H-2(K) (C3H/HePas and C3H/HeJ) and H-2(d) strains (BALB/c), (4) independent of TLR4 signaling since the coisogenic strains C3H/HePas and C3H/HeJ (TLR4 deficient) produced similar levels of NO following MAM stimulation, (5) potentiated by lipopolysaccharide, and (6) dependent on the presence of nonadherent peritoneal cells. Neutralization of interferon-gamma (IFNgamma in the peritoneal cell cultures with monoclonal antibodies abolished MAM-induced NO production. Addition of rIFNgamma to the adherent cells substituted the nonadherent cells for MAM-induced NO production. A macrophage cell line, J774A.1 (H-2(d)), also produced NO upon MAM stimulation but only when BALB/c spleen lymphocytes were added. Thus, in murine macrophages, MAM induces NO production that is dependent on signaling through MHC class II molecules and IFNgamma but independent of TLR4 expression.

465 Background: Prospective studies with sunitinib in mRCC have shown median progression-free survival (mPFS) of 11 months (first line) and 8.3 months (second line). In order to identify patients with LTB with SU, we describe the clinical characteristics of patients with mRCC treated with SU with an mPFS of 15 months or more. Methods: This is a retrospective chart review of patients with mRCC treated with SU in two hospitals, Alexander Fleming Institute Buenos Aires in Argentina and Hospital Israelita Albert Einstein in Sao Paulo, Brazil. Inclusion criteria included patients treated with SU who had a PFS of at least 15 months. Results: Between September 1995 and August 2009, 29 cases were identified. Patient demographics were: median age of 56 years, 65% male, 96% with previous nephrectomy, Eastern Cooperative Oncology Group performance status (PS) of either 0 (52%) or 1 (48%), 93% had clear cell histology, 69% received prior systemic therapy, and 78% had ≤ 2 metastatic sites (mostly in the lungs, liver and bone). Patients were started on SU 50 mg 4 weeks on treatment/2 weeks off treatment (4/2) (n=26) or 37.5 mg 6 weeks continuous dosing (n=3). For those patients starting on 4/2, dose reduction was necessary in 59% of the patients to maintain SU therapy. Median duration of therapy was 23.7 months. During treatment, 24 patients (83%) developed hypertension. Response rates were as follows: complete response 7% (n=2), partial response 38% (n=11), stable disease 52% (n=15); data missing for one patient. Conclusions: LTB is seen in patients who are young, have good performance status, and either 1 or 2 metastatic sites. Dose reductions are common in order to maintain treatment while benefiting from SU. Treatment with SU as either first- or second-line therapy did not appear to influence outcome. Hypertension is a common finding and serves as a predictive marker during treatment, but study limitations preclude the identification of pre-treatment predictive factors.

Introduction: Epstein-Barr virus (EBV) may serve as a target in therapeutic treatments, thus reliable diagnostic results are necessary. Objective: The aim of this study was to evaluate the accuracy of EBV detection by in situ hybridization (ISH) using five commercial probes in formalin-fixed and paraffin-embedded samples of nodular sclerosis Hodgkin's lymphoma (HL), and to compare the results with immunohistochemistry (IHC) and polymerase chain reaction (PCR). Material and method: Thirty samples were selected, 28 were lymph nodes, one bone marrow and one mediastinum. The following parameters were analyzed: signal intensity; proportionality of positive cells; quality of the reaction according to comfort for evaluation, sign quality and homogeneity of labeled cells; background reaction; morphology; presence of artifacts; and positivity in other non-neoplastic cells. All samples were analyzed for EBV detection using the five probes, IHC for latent membrane protein type 1 (LMP1) and PCR for Epstein Barr virus nuclear antigen 1 (EBNA1). Statistical analyses were performed with the R1 software; Fleiss' test and Cohen Kappa index of 5% were considered significant. Results: The detection by IHC-LMP1 was 26.7% (8/30) and 66.7% (20/30) by PCR-EBNA1. All probes detected EBV. Positivity was observed in 42/90 (46.7%), 38/90 (42.2%), 45/90 (50%), 27/90 (30%) and 61/90 (67.8%) for probes A, B, C, D and E, respectively. Discussion: All five probes demonstrated positivity. Conclusion: Probe E showed better rate (67.8%), sensitivity, specificity and accuracy (100%), a very good correlation among the different observers and with PCR, besides great cost-benefits relation.

Early prediction of IgA nephropathy progression: Proteinuria and AOPP are strong prognostic markers. Background Inflammation and oxidative stress have been incriminated in the pathogenesis of IgA nephropathy (IgAN). The aim of the present study was to assess whether markers reflecting these pathophysiologic processes, namely C-reactive protein (CRP) and advanced oxidation protein products (AOPP), would allow—in conjunction with clinical and histopathologic parameters—to predict disease progression. Methods Between 1994 and 1997, 120 adult patients with biopsy-proven IgAN were included in a prospective cohort study, and followed until the end of 2002 or start of dialysis. In every patient, we determined plasma levels of CRP and AOPP. These parameters were included, together with clinical data, in a multivariate Cox proportional hazard regression analysis, with halving of baseline creatinine clearance as the primary renal end point. Results A total of 51 patients reached the renal end point, including 30 who had to start dialysis. With multivariate analysis, the most potent independent risk factors of poor renal outcome were proteinuria ≥1 g/day [proportional hazard risk (HR) = 23.7, P = 0.0001], hypertension (HR = 8.13, P = 0.008), and AOPP plasma level (HR = 1.09 per 10 μmol/L, P = 0.042), whereas angiotensin II inhibitors were protective (HR = 0.19, P = 0.001). Conclusion Our data support the role of oxidative stress in the pathogenesis of IgAN and suggest that patients with proteinuria ≥1 g/day should be eligible for early implemented antioxidant and/or anti-inflammatory therapeutic strategies, with AOPP plasma level as a surrogate marker to evaluate their effects.
