WprowadzenieTrening słuchowy obejmuje ćwiczenia z wykorzystaniem dźwięków otoczenia oraz dźwięków mowy i ma na celu wypracowanie u osób z wadą słuchu zasobu doświadczeń akustycznych analogicznego do tego, jaki posiadają osoby słyszące. Sukces związany z leczeniem częściowej głuchoty poprzez zastosowanie implantu ślimakowego (CI) spowodował zwiększenie liczby pacjentów, którzy wymagają treningu słuchowego ukierunkowanego na rozpoznawanie dźwięków o średniej i wysokiej częstotliwości. Wobec braku pomocy dydaktycznych i rehabilitacyjnych dostosowanych do specyfiki trudności słuchowych tych pacjentów, w Klinice Rehabilitacji IFPS podjęto próbę opracowania materiałów do prowadzenia treningu słuchowego w ramach opieki pooperacyjnej.CelOcena przydatności oraz poziomu trudności treningu słuchowego z wykorzystaniem dźwięków otoczenia oraz dźwięków mowy o średniej i wysokiej częstotliwości przez pacjentów z częściową głuchotą korzystających z CI oraz logopedów.Materiał i metodyW badaniach uczestniczyło 29 dorosłych pacjentów z częściową głuchotą korzystających z CI i objętych rehabilitacją pooperacyjną. Pacjenci oraz logopedzi niezależnie od siebie dokonywali oceny trudności i przydatności zaproponowanego treningu słuchowego wypełniając ankietę z wykorzystaniem pięciostopniowej skali Likerta.WynikiAnaliza ankiet wykazała, że zarówno pacjenci z częściową głuchotą jak i logopedzi uważają, że trening słuchowy jest trudny dla tej grupy pacjentów, zwłaszcza ćwiczenia dotyczące identyfikacji dźwięków mowy o średniej i wysokiej częstotliwości. Zaproponowane ćwiczenia w ocenie pacjentów i logopedów sprawiały znacząco większe trudności kobietom niż mężczyznom. Pomimo tego logopedzi i pacjenci potwierdzili przydatność zastosowanych zestawów ćwiczeń w procesie rehabilitacji.WnioskiLeczenie operacyjne pacjentów z częściową głuchotą powinno być poparte odpowiednim treningiem słuchowym ukierunkowanym na odbiór i identyfikację dźwięków o średniej i wysokiej częstotliwości. Przeprowadzone badania stanowią uzasadnienie opracowywania materiałów celem wykorzystania ich w rehabilitacji słuchu pacjentów z częściową głuchotą.

Zastosowanie implantów ślimakowych oraz najnowszych aparatów słuchowych daje małym dzieciom z wadami słuchu możliwości odbioru sygnałów akustycznych w stopniu, który wcześniej był nieosiągalny. Urządzenia te umożliwiają małym pacjentom odbiór sygnałów (słyszenie), a właściwie organizowana i systematycznie prowadzona rehabilitacja pozwala rozwijać słuchanie, nabywanie języka i umiejętność mówienia. Istnieje wiele metod pracy z małymi dziećmi, które stosuje się obecnie. W niniejszym artykule przedstawione zostaną historia, założenia, zasady i praktyka Metody Audytywno-Werbalnej (ang. Auditory-Verbal Therapy, AVT). Jest ona oparta przede wszystkim na kanale słuchowym, a jej nadrzędnym celem jest, by dzieci z wadą słuchu wzrastały w normalnym środowisku (tam, gdzie żyją i uczą się), by uczynić je osobami niezależnymi i biorącymi udział w życiu otoczenia. Środkiem do tak określonego celu jest nauka poprzez słuchanie, bez korzystania z odczytywania mowy z ust, gestów, znaków alfabetu palcowego lub znaków języka migowego. Inspiracją do przedstawienia tych informacji był udział grupy specjalistów z Instytutu Fizjologii i Patologii Słuchu w jedynych w Polsce Międzynarodowych Warsztatach Naukowo-Szkoleniowych dla Terapeutów Mowy „Listening for Life. Auditory-Verbal Therapy: Principles into Practice”, prowadzonych przez prof. Warrena Estabrooksa oraz doświadczenia zdobyte w pracy z małymi dziećmi.

Telemedycyna to najnowsza forma opieki zdrowotnej, łącząca w sobie elementy telekomunikacji, informatyki oraz medycyny. Osiągnięcia techniczne w połączeniu z doświadczeniem specjalistów implikują powstawanie programów opieki nad pacjentami w różnym wieku i z różnego typu problemami zdrowotnymi realizowanych „na odległość”. Telerehabilitacja umożliwia dostarczenie odbiorcy usług rehabilitacyjnych z wykorzystaniem sieci połączeń telekomunikacyjnych i Internetu. W odniesieniu do osób z wadą słuchu, w tym korzystających z tradycyjnych aparatów słuchowych i/lub implantów słuchowych, działania o charakterze rehabilitacyjnym wpisują się także w programy określane mianem teleaudiologii i telezdrowia. W artykule przedstawiono najważniejsze doniesienia z literatury (artykułów, prac doktorskich, doniesień wstępnych, streszczeń i prezentacji) oraz raportów z realizacji programów unijnych, które podsumowują badania i działania praktyczne skierowane do pacjentów dorosłych korzystających z protez słuchowych w ramach szeroko rozumianej telerehabilitacji. Przedstawione zostały modele postępowania, prowadzący i odbiorcy programów, formy świadczenia usług oraz wrażenia uczestników terapii organizowanych w systemie „tele”. Wykorzystanie technologii w przygotowaniu i prowadzeniu opieki nad osobami z wadą słuchu stanowi istotną pomoc zwłaszcza w rejonach, gdzie brak specjalistów lub ograniczony dostęp do nich determinuje skuteczność stosowania określonego postępowania medycznego.

W artykule opisano cele rehabilitacji słuchowej prowadzonej z wykorzystaniem materiału językowego (głoski, sylaby, wyrazy, zdania, frazy) z osobami, które korzystają z implantów ślimakowych. Zaproponowano podział na grupy w zależności od etapu opanowania kompetencji lingwistycznej. Tekst jest próbą określenia założeń i wskazówek potrzebnych do prowadzenia efektywnej, wykorzystującej materiał językowy rehabilitacji słuchowej osób korzystających z implantów ślimakowych. W artykule pominięto zagadnienie ćwiczeń słuchowych wykorzystujących dźwięki otoczenia.

WstępZ dotychczasowych badań wynika, że nie ma związku między ocenami subiektywnymi a obiektywnym pomiarem korzyści z implantu ślimakowego (CI) u osób zaopatrzonych w ten typ protez słuchu bądź związek ten jest słaby.CelW pracy podjęto zagadnienie zależności między subiektywną oceną korzyści z implantu ślimakowego a obiektywną ich oceną dokonywaną w badaniach audiometrycznych i obserwacjach logopedycznych w szerszym kontekście, jaki tworzą: ocena psychologiczna dokonana na podstawie np. pomiaru kwestionariuszowego, jak i inne wybrane informacje zawarte w dokumentacji medycznej, dotyczące pacjenta z CI, np. posiadane choroby somatyczne czy psychiczne.Materiał i metodaDo analizy jakościowej wybrano 4 przypadki kliniczne osób z częściową głuchotą o początku prelingwalnym według kryterium subiektywnej oceny korzyści z CI dokonanej za pomocą kwestionariusza NCIQ. Dwie osoby, które ogółem wysoko oceniały te korzyści, oraz dwie osoby, które oceniały je jako bardzo niskie. W analizie jakościowej każdej z osób uwzględniono rezultaty wybranych badań audiometrycznych, informacje o ich funkcjonowaniu językowym i komunikacyjnym, a także psychologicznym, uwzględniając wyniki kwestionariuszy: SES, GHQ-28, Mini COPE.WynikiOkazało się, że oceny subiektywne i obiektywne, a także obserwacja logopedyczna i ocena psychologiczna tworzą spójny obraz w przypadku dwóch osób, a w odniesieniu do kolejnych dwóch osób (w tym jednej oceniającej wysoko, a jednej nisko korzyści z CI) ocena subiektywna i obiektywna są w różnym stopniu rozbieżne. Rozbieżność tę można wyjaśnić m.in. uwarunkowaniami psychologicznymi, co starano się pokazać w omówieniu rezultatów każdej z osób.WnioskiKlinicyści pracujący z osobami głuchymi i słabosłyszącymi, które otrzymały implant ślimakowy, powinni szczególnie wnikliwie analizować sytuacje rozbieżności oceny subiektywnej i obiektywnej korzyści z CI, poddając wieloaspektowej analizie jakościowej otrzymane rezultaty. Ważna rola przypada w niej konsultacji psychologicznej czy psychoterapeutycznej ze względu na znaczenie pozaudiologicznej motywacji do implantu ślimakowego i jej ewentualnych konsekwencji u osób już zaimplantowanych.

Purpose The present study investigated adults with partial deafness (PD) and asked them to rate the benefits of their cochlear implant (CI), their general level of satisfaction with it, and their level of psychological distress. Of particular interest was the role of gender. Methods The study comprised 71 participants (41 females) with PD who had been provided with a CI. The Nijmegen Cochlear Implant Questionnaire (NCIQ) was used to assess the benefits of their CI. Satisfaction with their CI was measured using a visual analog scale. The severity of mental distress was assessed with the General Health Questionnaire (GHQ-28). Results On various NCIQ scales, the average benefits of a CI were rated at 66%. Females gave a lower rating than males. The mental distress experienced by the group was significantly higher than in the general population. Females had more severe symptoms of anxiety and insomnia than males. There was a significant relationship between psychological distress and CI benefit, but only in females. Besides general distress, the most affected spheres were related to psychosocial functioning—“self-esteem”, “activity limitation”, and “social interaction”. Contrary to expectations, there was no relationship between mental distress and CI satisfaction. Conclusions The perceived benefits of a CI in subjects with PD relate mostly to the level of mental distress, although gender is an important factor. For females, their emotional state affects how beneficial their CI is perceived. Due to the higher levels of mental distress, females tend to need more psychological intervention and support.

OBJECTIVE The research question was: what coping strategies are used by the postlingually deafened adult cochlear implant (CI) users, in comparison to the hearing population, taking into account their gender and CI experience. MATERIAL AND METHODS In the assessment of the coping strategies we used the C.S. Carver's Brief COPE questionnaire in Polish adaptation. The questionnaire and information form have been sent to the adult patients implanted in the Institute of Physiology and Pathology of Hearing in the years 2005 and 2009. The response ratio was 59.5% (n = 78). The control group (n = 77) comprised hearing persons selected at random, allowing for age. RESULTS The stress coping strategies are different in the postlingually deafened adults and hearing ones. The postlingually deaf more seldom use the effective strategies, such as the active coping, planning and humour, and more often the denial. Also, they significantly more seldom turn to the self-blame and the substance use. People with longer CI experience significantly more often use the denial, more seldom they adopt the substance use and the self-distraction compared with those with shorter CI experience. Female postlingually deafened CI users are significantly less active in coping with stress and more seldom turn to the substance use, but more often, compared with the hearing women in the same age-group, use the denial. Male postlingually deafened CI users differ significantly from the hearing men only in more seldom turning to the self-blame and the substance use. CONCLUSIONS Postlingually deafened CI patients more often use the ineffective coping strategies. This tendency becomes stronger with the time lapse from the cochlear implantation and is more frequent in women, which corresponds with the data on both the considerably worse mental health of the postlingually deafened people compared to the general population and the deterioration of the mental, social and physical functioning with the lapse of time from the implantation. The standing availability of the psychological support for these patients is essential, as well as the awareness of their problems.

Objectives With more children receiving cochlear implants during infancy, there is a need for validated assessments of pre-verbal and early verbal auditory skills. The LittlEARS® Auditory Questionnaire is presented here as the first module of the LittlEARS® test battery. The LittlEARS® Auditory Questionnaire was developed and piloted to assess the auditory behaviour of normal hearing children and hearing impaired children who receive a cochlear implant or hearing aid prior to 24 months of age. This paper presents results from two studies: one validating the LittlEARS® Auditory Questionnaire on children with normal hearing who are German speaking and a second validating the norm curves found after adaptation and administration of the questionnaire to children with normal hearing in 15 different languages. Methods Scores from a group of 218 German and Austrian children with normal hearing between 5 days and 24 months of age were used to create a norm curve. The questionnaire was adapted from the German original into English and then 15 other languages to date. Regression curves were found based on parental responses from 3309 normal hearing infants and toddlers. Curves for each language were compared to the original German validation curve. Results The results of the first study were a norm curve which reflects the age-dependence of auditory behaviour, reliability and homogeneity as a measure of auditory behaviour, and calculations of expected and critical values as a function of age. Results of the second study show that the regression curves found for all the adapted languages are essentially equal to the German norm curve, as no statistically significant differences were found. Conclusions The LittlEARS® Auditory Questionnaire is a valid, language-independent tool for assessing the early auditory behaviour of infants and toddlers with normal hearing. The results of this study suggest that the LittlEARS® Auditory Questionnaire could also be very useful for documenting children's progress with their current amplification, providing evidence of the need for implantation, or highlighting the need for follow-up in other developmental areas.

To adapt the LittlEars questionnaire into Polish and to evaluate the psychometric properties of the Polish version of the questionnaire.A back-translation method was used to translate the LittlEars questionnaire into Polish. The translated version was first evaluated by means of an expert-appraisal method. After having improved the Polish version of LittlEars with the results obtained from that evaluation, various psychometric analyses were conducted. Study participants included 310 parents or other caregivers of children with normal hearing whose ages ranged between 0.5 and 24.0 months. Corrected item-total correlations were calculated to evaluate the extent to which the different questions distinguish levels of auditory development of the assessed children. Cronbach's alpha coefficient--to evaluate internal consistency across items--was also calculated. To obtain estimates of validity, correlations between item/total score and age were calculated. A non-linear regression model was derived to obtain normative data for expected and minimum values of total scores from the questionnaire according to age.Corrected item-total correlations ranged from 0.14 to 0.84. The Cronbach's alpha coefficient was 0.95, indicating that the measurements were highly reliable. The linear correlation between total scores and age was 0.90 (p.001). The regression analysis conducted to obtain normative data showed that 83% of the variance in the total scores can be explained by age.The results of psychometric analyses support the use of the Polish version of the LittlEars questionnaire as a sensitive and reliable tool to assess the development of auditory behavior in Polish children between 3 and 24 months of age.
