1. Oxidized Low Density Lipoprotein Receptor 1 3’UTR 188C>T Gene Polymorphism in Patients with Coronary Artery Bypass Grafting

Abstract

Aim: Coronary artery disease (CAD) is a pathological process characterized by atherosclerotic plaque accumulation in the epicardial arteries. Inflammation and high lipid levels play a role in pathological changes in atherosclerosis. Besides traditional risk factors, genetic factors such as single nucleotide polymorphism (SNPs) can be involved in disease process. In this study, we aimed to evaluate the effects of oxidized low density lipoprotein receptor 1 (OLR1) 3’UTR188C>T gene polymorphism, C-reactive protein (CRP), and lipid status in patients with coronary artery bypass grafting (CABG). Materials and Methods: The study population consisted of 109 CAD patients who had undergone CABG, and 127 healthy controls. The OLR1 3’UTR188C>T polymorphism was genotyped using PCR-RFLP technique. Serum CRP, high-density lipoprotein-cholesterol (HDL-C), and low-density lipoprotein-cholesterol (LDL-C) levels were measured with an automatic biochemistry analyzer. Results: The distribution of the OLR1 3’UTR188C>T genotypes and alleles did not differ significantly between CAD patients with CABG and controls. Serum CRP levels were increased in patients compared to the control group (pT polymorphism may not be involved in susceptibility to atherosclerosis. However, traditional risk factors in atherosclerosis such as smoking, alcohol consumption, family history, hypertension, diabetes mellitus, and circulating CRP levels were increased in our CABG population. The evaluation of OLR1 3’UTR188C>T and different OLR1 SNPs may be useful for their single and combined effects in atherosclerosis.

Abstract

Abstract This paper investigates the role of nuclear energy in promoting ecological sustainability in India, focusing on three ecological indicators: ecological footprint (EF), CO2 emissions (CO2), and load capacity factor (LF). In addition to nuclear energy, the study considers the influence of gas consumption and other drivers of ecological sustainability using data spanning from 1970 to 2018. The analysis also takes into account the impact of the 2008 global financial crisis on the model, employing the autoregressive distributed lag (ARDL) and frequency domain causality approaches to assess the relationships. Unlike previous studies, this research evaluates both the Environmental Kuznets Curve (EKC) and load capacity curve (LCC) hypotheses. The ARDL results support the validity of both the EKC and LCC hypotheses in the Indian context. Furthermore, the findings reveal that nuclear energy and human capital contribute positively to ecological quality, while gas consumption and economic growth have a negative impact on ecological sustainability. The study also highlights the increasing effect of the 2008 global financial crisis on ecological sustainability. Additionally, the causality analysis demonstrates that nuclear energy, human capital, gas consumption, and economic growth can serve as predictors of long-term ecological sustainability in India. Based on these findings, the research presents policy recommendations that can guide efforts towards achieving SDGs 7 and 13.

3. Understanding the hydrological response of a headwater-dominated catchment by analysis of distributed surface–subsurface interactions

Abstract

Abstract We computationally explore the relationship between surface–subsurface exchange and hydrological response in a headwater-dominated high elevation, mountainous catchment in East River Watershed, Colorado, USA. In order to isolate the effect of surface–subsurface exchange on the hydrological response, we compare three model variations that differ only in soil permeability. Traditional methods of hydrograph analysis that have been developed for headwater catchments may fail to properly characterize catchments, where catchment response is tightly coupled to headwater inflow. Analyzing the spatially distributed hydrological response of such catchments gives additional information on the catchment functioning. Thus, we compute hydrographs, hydrological indices, and spatio-temporal distributions of hydrological variables. The indices and distributions are then linked to the hydrograph at the outlet of the catchment. Our results show that changes in the surface–subsurface exchange fluxes trigger different flow regimes, connectivity dynamics, and runoff generation mechanisms inside the catchment, and hence, affect the distributed hydrological response. Further, changes in surface–subsurface exchange rates lead to a nonlinear change in the degree of connectivity—quantified through the number of disconnected clusters of ponding water—in the catchment. Although the runoff formation in the catchment changes significantly, these changes do not significantly alter the aggregated streamflow hydrograph. This hints at a crucial gap in our ability to infer catchment function from aggregated signatures. We show that while these changes in distributed hydrological response may not always be observable through aggregated hydrological signatures, they can be quantified through the use of indices of connectivity.

Abstract

Taylor and Francis Ltd, Aim: To determine the incidence and outcomes of respiratory syncytial virus (RSV)-related acute lower respiratory tract infection (ALRI) including morbidity, nosocomial infection and mortality among newborn infants who were admitted to the neonatal intensive care units (NICUs).Methods: A multicenter, prospective study was conducted in newborns who were hospitalized with community acquired or nosocomial RSV infection in 44 NICUs throughout Turkey. Newborns with ALRI were screened for RSV infection by Respi-Strip®-test. Main outcome measures were the incidence of RSV-associated admissions in the NICUs and morbidity, mortality and epidemics results related to these admissions.Findings: The incidence of RSV infection was 1.24% (n: 250) and RSV infection constituted 19.6% of all ALRI hospitalizations, 226 newborns (90.4%) had community-acquired whereas 24 (9.6%) patients had nosocomial RSV infection in the NICUs. Of the 250 newborns, 171 (68.4%) were full-term infants, 183 (73.2%) had a BW >2500 g. RSV-related mortality rate was 1.2%. Four NICUs reported seven outbreaks on different months, which could be eliminated by palivizumab prophylaxis in one NICU.Conclusion: RSV-associated ALRI both in preterm and term infants accounts an important percent of hospitalizations in the season, and may threat other high-risk patients in the NICU. © 2015 Taylor & Francis., Alan, S.; Neonatal Intensive Care Unit, Hitit University Corum Training and Research Hospital, Vedat Dolakay Cad. 5. Experience in Non-invasive Ventilation in Grade 3 Hepatic Encephalopathy

Abstract

Introduction:Hepatic encephalopathy (HE) is a common complication of acute and chronic liver failure, which can lead to significant morbidity and mortality. Grade 3-4 HE patients are typically managed with routine intubation and mechanical ventilation. However, recent research suggests that routine intubation may increase the risk of complications and death for the preservation of the airway. This study investigated the use of non-invasive ventilation (NIV) in patients with grade 3 encephalopathy associated with acute and chronic liver failure.Methods:This retrospective study included patients with grade 3 encephalopathy associated with liver failure who underwent NIV between January 2022 and March 2023 in a liver transplant intensive care unit. The patient demographic data, laboratory results, comorbidities, and outcomes were collected and analyzed. The results were compared to those of HE patients who were intubated as reported in the literature.Results:A total of 41 children and adults with grade 3 HE who received NIV were included in this study. Compared with HE patients who were intubated as reported in the literature, the NIV group had significantly lower rates of complications and mortality. Additionally, there were no additional complications observed in patients who received NIV without intubation, such as infections, cardiovascular disorders, or cognitive impairments.Conclusion:The use of NIV in grade 3 HE patients suggests that it is an effective alternative to intubation. These findings support the need for careful consideration when deciding to intubate HE patients and suggest that continuous support using NIV may provide potential benefits. Further studies are needed to investigate the optimal management of these patients to improve their outcomes.

Abstract

Skin sparing and nipple areola complex sparing subcutaneous mastectomies are both oncologically safe surgical procedures. Although autologuous breast reconstruction has been considered as a standard means of surgical approach, excellent cosmetic results have been obtained by implant based breast reconstruction. In this report, we present the result of titanium coated polypropylene mesh (Tiloop Bra; Pfm Medical, Cologne, Germany), an ultimate support material utilized in the reconstruction of a patient with breast cancer.

7. Characteristics of Patients in Follow-up After the Whipple Procedure in the Intensive Care Unit: Field-specific Intensive Care Experience

Abstract

Introduction:Pancreaticoduodenectomy (Whipple procedure) is an important surgical intervention in terms of adverse conditions that may occur after surgery. Our aim is to determine the change in the clinical course of the patients followed in the intensive care unit (ICU) after pancreaticoduodenectomy during the follow-up period.Methods:We included 37 patients followed up after the Whipple procedure from August 2021 to August 2022 in a tertiary ICU specific for liver-pancreas-biliary tract and liver transplantation.Results:The vital signs and laboratory values of the patients during the follow-up were examined. Statistically decreased arterial blood lactate levels and a significant increase in arterial blood partial oxygen pressures were seen upon admission to the ICU. The highest detected arterial lactate level was correlated with the percentage change in blood creatinine.Conclusion:Patients should be followed closely after the Whipple procedure. Fluid volume treatment and respiratory support play a significant role in patient recovery.

Abstract

Waste management has been a great problem globally for at least five decades. Some of the developed countries could initiated certain processes to benefit from the waste. In this way, countries could both got rid of waste and they could exploit from it and used waste in different ways such as heating, recycling, etc. The situation became even worse during the coronavirus disease 2019 (COVID-19) pandemic. The COVID-19 pandemic has altered global waste generation dynamics and thus became one of the most important issue for all countries for now and for future. Actually, healthcare waste is all the waste generated by healthcare facilities, medical laboratories and biomedical research facilities, as well as waste from minor or scattered sources. However, during the pandemic, masks, increased amounts of contaminated waste including gloves, other protective equipment, and along with non-contaminated materials, many types of additional medical and hazardous waste are produced. These contaminated wastes not only dirts the environment, but also may cause the pandemic spread. The inadequacies and inefficiencies of current waste management system to deal with the increased dependence on plastic could aggravate its mismanagement and leakage into the environment, thus triggering a new environmental crisis. Therefore, this study will highlights the possible effects of COVID-19 on the waste management and environment.

9. Single-port bilateral endoscopic thoracic sympathectomy for the treatment of hyperhidrosis

Abstract

INTRODUCTION: Hyperhidrosis is an excessive sweating disorder that affects patients' quality of life. Today, endoscopic thoracic sympathectomy offers a safe and effective solution of the disease. In this study, we aimed to investigate the effectiveness and outcomes of the endoscopic thoracic sympathectomy by single-port technique in the surgical treatment of hyperhidrosis. METHODS: Between 2013 and 2017, a total of 35 patients admitted to our clinic with palmar and/or axillary hyperhidrosis underwent bilateral endoscopic thoracic sympathectomy. These cases were evaluated retrospectively with respect to age and sex, surgical technique, operation time, intraoperative and postoperative complications, duration of hospitalization, patient satisfaction, visual analog scale at 24th hour, compensatory sweating, follow-up time and recurrence. RESULTS: The mean age of the patients was 22.7, 27 patients were male and 8 patients were female. There were palmar hyperhidrosis in 19 patients, both palmar and axillary hyperhidrosis in 10 and only axillary hyperhidrosis in 6. The average operative time was 14.1 minutes, and the length of hospital stay was 2.4 days. Visual analog scale pain score on postoperative 24th hours was 3.2. The mean follow-up period was 13 months, and during the follow-up period, compensatory sweating was observed in two of the patients (5.7 %). DISCUSSION AND CONCLUSION: Single-port thoracoscopic sympathectomy is an effective and reliable method for surgical treatment of palmar and axillary sweating.

Abstract

Pterion is an irregular H letter shaped sutural confluence in the temporal fossa formed by frontal, parietal bones, great wing of sphenoid bone and temporal squama. Pterion is classified in 4 types as follows: sphenoparietal, frontotemporal, epipteric and stellate. The pterion represents: anterior branch of the middle meningeal artery, middle cerebral artery, Broca’s motor speech area, insula and stem of the lateral cerebral sulcus. This pterion junction has been used as a common extra-cranial landmark for surgeons in microsurgical and surgical approaches pertaining to important pathologies of this region. In the present study, our aim was to determine pterion types, to estimate distances between pterion and some special landmarks by which means to contribute to the related literature by comparing the data with other studies focusing on various populations. Pterion types identified by observation and measurements were taken by steel Vernier caliper. This study was conducted with 75 adult skulls (both sides 150 pterion). Skulls were classified with regard to gender as: 47 male and 28 female. Pterion types observed in both genders were classified as: sphenoparietal type 82% (84.04% in male, 78.57% in female), frontotemporal type 4.66% (5.31% in male, 3.57% in female), epipteric type 10.66% (8.51% in male, 14.28% in female) and stellate type 2.66% (2.12% in male, 3.57% in female). These findings will be usefull for clinicians, anthropologists and forensics. El pterion es una confluencia sutural con forma de letra H irregular en la fosa temporal formada por los huesos frontales, parietales, el ala mayor del hueso esfenoides y la escama temporal. Pterion se clasifica en 4 tipos de la siguiente manera: Esfenoparietal, frontotemporal, epiptérico y estrellado. El pterion representa: la rama anterior de la arteria meníngea media, la arteria cerebral media, el área motora del habla de Broca, la ínsula y el vástago del surco cerebral lateral. Esta unión del pterión se ha utilizado como un hito extracraneal común para los cirujanos en enfoques microquirúrgicos y quirúrgicos relacionados con patologías importantes de esta región. En el presente estudio, nuestro objetivo es determinar los tipos de pterion, estimar las distancias entre el pterión y algunos puntos de referencia especiales para contribuir a la literatura relacionada mediante la comparación de los datos con otros estudios que se centran en diversas poblaciones. Los tipos de pterión identificados por observación y mediciones fueron tomados por un calibrador a Vernier de acero. Este estudio se realizó con 75 cráneos adultos (ambos lados 150 pterion). Los cráneos se clasifican en función del género como: 47 hombres y 28 mujeres. Los tipos de pterion observados en ambos sexos se clasifican en: tipo esfenoparietal 82% (84,04% en hombres, 78,57% en mujeres), tipo frontotemporal 4,66% (5,31% en hombres, 3,57% en mujeres), tipo epiptérico 10,66% (8,51% en hombres, 14,28% en mujeres) y tipo estrellado 2,66% (2,12% en hombres, 3,57% en mujeres). Estos hallazgos serán útiles para los clínicos, antropólogos y médicos forenses.

Abstract

The use of stem cells derived from adipose tissue as an autologous and self-replenishing source for a variety of differentiated cell phenotypes, provides a great deal of promise for reconstructive surgery. The secret of the human body, stem cells are reserved. Stem cells are undifferentiated cells found in the human body placed in any body tissue characteristics that differentiate and win ever known to cross the tissue instead of more than 200 diseases and thus improve and, rejuvenates the tissues. So far, the cord blood of newborn babies are used as a source of stem cells, bone marrow, and twenty years after tooth stem cells in human adipose tissue, scientists studied more than other sources of stem cells in adipose tissue and discovered that. Increase in number of in vitro studies on adult stem cells, depending on many variables is that the stem cells directly to the desired soybean optimization can be performed.. We will conclude by assessing potential avenues for developing this incredibly promising field. 14. Sugammadex Use in a Patient with Wolff-Parkinson-White (WPW) Syndrome

Abstract

Background: Wolff-Parkinson-White (WPW) syndrome is a disease associated with episodes of supraventricular tachycardia and ventricular pre-excitation or atrial fibrillation. WPW is characterized by an aberrant electrical conduction pathway between atria and ventricles. Case Report: The major anesthetic problem connected with WPW syndrome is the risk of tachyarrhythmias due to accessory pathway. Therefore, it has been proposed that the aim of anesthetic management should be the avoidance of tachyarrhythmia and sympathetic stimulation. Sugammadex was administered as a neuromuscular reversal agent in this case. To our knowledge, this is the first case report of sugammadex use in a patient with WPW. This report presents a case of general anesthesia management in a patient with WPW syndrome. Conclusion: We think that it is appropriate to use sugammadex to reverse rocuronium for the prevention of sudden hemodynamic changes in patients with WPW who underwent general anesthesia.

Abstract

Background Patients with hematological malignancies (HM) are at high risk of mortality from SARS-CoV-2 disease 2019 (COVID-19). A better understanding of risk factors for adverse outcomes may improve clinical management in these patients. We therefore studied baseline characteristics of HM patients developing COVID-19 and analyzed predictors of mortality. Methods The survey was supported by the Scientific Working Group Infection in Hematology of the European Hematology Association (EHA). Eligible for the analysis were adult patients with HM and laboratory-confirmed COVID-19 observed between March and December 2020. Results The study sample includes 3801 cases, represented by lymphoproliferative (mainly non-Hodgkin lymphoma n = 1084, myeloma n = 684 and chronic lymphoid leukemia n = 474) and myeloproliferative malignancies (mainly acute myeloid leukemia n = 497 and myelodysplastic syndromes n = 279). Severe/critical COVID-19 was observed in 63.8% of patients (n = 2425). Overall, 2778 (73.1%) of the patients were hospitalized, 689 (18.1%) of whom were admitted to intensive care units (ICUs). Overall, 1185 patients (31.2%) died. The primary cause of death was COVID-19 in 688 patients (58.1%), HM in 173 patients (14.6%), and a combination of both COVID-19 and progressing HM in 155 patients (13.1%). Highest mortality was observed in acute myeloid leukemia (199/497, 40%) and myelodysplastic syndromes (118/279, 42.3%). The mortality rate significantly decreased between the first COVID-19 wave (March–May 2020) and the second wave (October–December 2020) (581/1427, 40.7% vs. 439/1773, 24.8%, p value, EPICOVIDEHA has received funds from Optics COMMITTM (COVID-19 Unmet Medical Needs and Associated Research Extension) COVID-19 RFP program by GILEAD Science, United States (Project 2020-8223).

Abstract

Summary The manageable toxicity profile of obinutuzumab (GA101; G) alone or with chemotherapy in first‐line (1L; fit and non‐fit) and relapsed/refractory (R/R) patients with chronic lymphocytic leukaemia (CLL) was established in the primary analysis of the Phase IIIb GREEN trial (Clinicaltrials.gov: NCT01905943). The final analysis (cut‐off, 31 January 2019) is reported here. Patients received G (1000 mg) alone (G‐mono; fit and non‐fit patients) or with chemotherapy [fludarabine and cyclophosphamide (FC; fit patients); chlorambucil (non‐fit patients); bendamustine (any patient)]. Study endpoints were safety (primary) and efficacy (secondary). Subgroup analyses were performed on prognostic biomarkers in 1L CLL. Overall, 630 patients received 1L and 341 received R/R CLL treatment. At the final analysis, no new safety signals were observed [Grade ≥ 3 adverse events (AEs): 1L 82·7%, R/R 84·5%; serious AEs: 1L 58·1%, R/R 62·5%]. Neutropenia (1L 50·5%, R/R 53·4%) and thrombocytopenia (1L 14·6%, R/R 19·1%) were the most common Grade 3–5 AEs. G‐mono‐, G‐bendamustine and G‐FC‐treated patients with unmutated immunoglobulin heavy chain trended towards shorter progression‐free survival. Achievement of minimal residual disease negativity was greatest in 1L patients treated with G‐FC. In this final analysis of the GREEN trial, the safety profile of G was consistent with current risk management strategies. Abstract

BACKGROUND: The adjuvanted recombinant zoster vaccine (Shingrix) can prevent herpes zoster in older adults and autologous haemopoietic stem cell transplant recipients. We evaluated the safety and immunogenicity of this vaccine in adults with haematological malignancies receiving immunosuppressive cancer treatments. METHODS: In this phase 3, randomised, observer-blind, placebo-controlled study, done at 77 centres worldwide, we randomly assigned (1:1) patients with haematological malignancies aged 18 years and older to receive two doses of the adjuvanted recombinant zoster vaccine or placebo 1-2 months apart during or after immunosuppressive cancer treatments, and stratified participants according to their underlying diseases. The co-primary objectives of the study were the evaluation of safety and reactogenicity of the adjuvanted recombinant zoster vaccine compared with placebo from the first vaccination up to 30 days after last vaccination in all participants; evaluation of the proportion of participants with a vaccine response in terms of anti-glycoprotein E humoral immune response to the adjuvanted recombinant zoster vaccine at month 2 in all participants, excluding those with non-Hodgkin B-cell lymphoma and chronic lymphocytic leukaemia; and evaluation of the anti-glycoprotein E humoral immune responses to the vaccine compared with placebo at month 2 in all participants, excluding those with non-Hodgkin B-cell lymphoma and chronic lymphocytic leukaemia. We assessed immunogenicity in the per-protocol cohort for immunogenicity and safety in the total vaccinated cohort. The study is registered with ClinicalTrials.gov, number NCT01767467, and with the EU Clinical Trials Register, number 2012-003438-18. FINDINGS: Between March 1, 2013, and Sept 10, 2015, we randomly assigned 286 participants to adjuvanted recombinant zoster vaccine and 283 to placebo. 283 in the vaccine group and 279 in the placebo group were vaccinated. At month 2, 119 (80·4%, 95% CI 73·1-86·5) of 148 participants had a humoral vaccine response to adjuvanted recombinant zoster vaccine, compared with one (0·8%, 0·0-4·2) of 130 participants in the placebo group, and the adjusted geometric mean anti-glycoprotein E antibody concentration was 23 132·9 mIU/mL (95% CI 16 642·8-32 153·9) in the vaccine group and 777·6 mIU/mL (702·8-860·3) in the placebo group (adjusted geometric mean ratio 29·75, 21·09-41·96; p
