Testimony "to discuss the federally funded Money Follows the Person Grant. The Centers for Medicare and Medicaid Services (CMS) awarded a five year "Money Follows the Person" demonstration grant to SRS in 2007. This grant has now been extended to 2016. The purpose of the grant is to transition individuals out of institutions into the community, allowing their Medicaid funding to pay for community based services."

The Centers for Medicare and Medicaid Services (CMS) requires Medicaid programs to reimburse for professional services on behalf of their beneficiaries. This report will focus on three provider types: physicians, advanced registered nurse practitioners (ARNPs), and physician assistants (PAs) and the professional services they provide. Services are delivered in a variety of settings and include preventive care, well child services, and treatment for illnesses, chronic diseases, and traumatic injuries. This report targets cited professionals delivering services to Medicaid beneficiaries through Fee for Service Programs (FFS). All three provider types are able to bill Evaluation and Management Codes, or E & M codes, write prescriptions, and must enroll and bill independently. Mid-level practitioners, however, provide a more limited range of services when compared to physicians and are subject to a reduced payment, receiving 75 percent of maximum rates.

"Quality improvement is a formal approach to the analysis of performance and systematic efforts to improve it." The primary goal of this review of quality improvement is to measure the effectiveness of the Kansas Health Policy Authority (KHPA) in managing resources used to purchase and promote quality health care. The National Committee for Quality Assurance (NCQA) defines quality health care as "the extent to which patients get the care they need in a manner that most effectively protects or restores their health." KHPA's quality improvement efforts strive to systematically and deliberately assess, measure, and analyze data within and across programs through regular collection of core data and development of indicators focused on optimizing health outcomes and resource effectiveness.

A Local Education Agency (LEA) is defined by the U.S. Office of Special Education Program as "a school district, board of education, or other public authority under the supervision of a state educational agency having administrative control and direction of a public elementary or secondary schools in a city, county, township, school district, or political subdivision in a state." The Individuals with Disabilities Education Act (IDEA) guarantees every child a free and appropriate public education (FAPE). Kansas Medicaid will reimburse LEAs for medically necessary services required for a Medicaid eligible child to receive FAPE as documented in the child's individual educational program (IEP) or individual family service plan (IFSP).

Rehabilitative therapy services are optional Medicaid services which include physical therapy, occupational therapy, and speech and language therapy services. Rehabilitative therapy services are provided following an injury or illness to restore functional status. For example, rehabilitative therapy services may be provided following an illness in an attempt to restore strength and endurance. These services may also be provided after a traumatic injury such as a broken hip or a cerebrovascular accident (CVA). The objective in both examples would be to restore the individual to their pre-illness or pre-injury functional ability. In extreme cases the focus would be to assist the individual in performing basic activities of daily living (ADLs) such as feeding, dressing and bathing.

Kansas Medicaid maintains an out-of-state program for situations which require a Kansas Medicaid beneficiary to receive services in another state. The out-of-state program, described in this review, includes only those services provided to individuals enrolled in the fee for service program. The Managed Care contractors manage the out-of-state services provided to the HealthWave population.

The HealthWave program was developed to provide comprehensive, quality health coverage for low income children and families across the state of Kansas. Medical care is offered through a combination of fee-for-service (FFS) coverage and direct contracts with two physical health managed care organizations (MCOs), Children's Mercy Family Health Partners (CMFHP) and UniCare Health Plan of Kansas (UniCare). The majority of Medicaid beneficiaries, including all HealthWave 19 (HW 19) members, receive mental health care from Kansas Health Solutions (KHS) through the Prepaid Ambulatory Health Plan (PAHP) and substance abuse care from Value Options (VO) through the Prepaid Inpatient Health Plan (PIHP). HealthWave 21 (HW 21) participants (those who receive care through the Children's Health Insurance Program - CHIP), receive mental health services through a capitated managed care contract with Cenpatico Behavioral Health (CBH).

The primary purpose of prior authorization (PA) is to facilitate cost containment by ensuring fee for service (FFS) medical services provided to Medicaid/HealthWave beneficiaries are medically necessary and cost effective. This is accomplished by identifying, researching, and reviewing designated procedures or services, and implementing measures to promote performance and reimbursement of services considered to meet the specified criteria set forth by KHPA.

HealthConnect Kansas (HCK) is a primary care case management program established in 1994 to provide Medicaid beneficiaries increased access to quality care and continuity of service, while controlling costs for specialty care. Case management was instituted by contracting with Primary Care Case Managers (PCCMs) to provide primary care to HCK members and coordinate their specialty care.

The primary objectives of this review are to describe the role of Federally Qualified Health Centers (FQHCs) in the Medicaid program, to describe the role of Medicaid in supporting this essential piece of the health care safety net, and to review FQHC payment policy and oversight. Other important questions, such as the role of FQHCs in advancing the medical home, or innovations in combining mental and physical health services are addressed in other venues.

Medicaid is a partnership between the state and federal government that has a significant impact on the Kansas economy. In fiscal year (FY) 2006, the federal government spent nearly $304 billion dollars on the program nationally. The federal government contributes approximately $1.502 for every dollar of state Medicaid spending in Kansas. The rate of this match varies from state to state -- and is generally higher in poorer states. In Kansas, Medicaid accounts for 14.8 percent of the state budget (State General Fund only) and represents a significant portion of total spending on health care services. The only program for which the state spends more money is K-12 education.

"KHPA has been engaged for the past two years in a comprehensive effort to review and improve each major component of Medicaid and SCHIP. The overall purpose of the Medicaid Transformation Process is to provide a regular and transparent program reviews to monitor, assess, diagnose, and address policy issues in each major program area within Medicaid. The preparation of these reviews is designed to serve as the basis for KHPA budget initiatives in the Medicaid program on an ongoing basis. The following recommendations are based on findings from 14 program reviews completed in 2008; they address issues related to decreasing expenditures, addressing reimbursement, expanding coverage, and enhancing program oversight. An additional 12 reviews will be completed for 2009."

KHPA has been engaged for the past two years in a comprehensive effort to review and improve each major component of Medicaid and SCHIP. The agency completed fourteen program reviews as the first step in the KHPA Medicaid Transformation Plan, including fee-for-service Medicaid (HealthConnect) and HealthWave, two special populations (the aged and disabled), eight health care services, eligibility, and quality improvement. The eight health care services reviewed were dental, durable medical equipment (DME), home health, hospice, hospital, lab and radiology, pharmacy, and transportation. These reviews covered 77 percent of Medicaid and SCHIP medical care expenditures and 40 percent of the almost $2.5 billion cost of Medicaid and SCHIP.
