Guidelines recommend using antipsychotic monotherapy at the lowest effective dose, however high-dose and antipsychotic polypharmacy prescribing (APP) remain commonplace. The aim of this study was to determine the prevalence and patterns of high-dose antipsychotic prescribing and APP among mental health service users in New Zealand (NZ).A retrospective audit of service users discharged from inpatient (n=657), or registered with community (n=1560), mental health services at Auckland District Health Board was undertaken. Case notes were reviewed and data on demographics, antipsychotic routes and doses were collected. Outcomes measures included: frequency of total high-dose prescribing, high-dose monotherapy, APP, high-dose APP, and factors associated with these prescribing practices. Logistic regression models were used to examine the relationships between explanatory and outcome variables.Of the service users prescribed an antipsychotic (n = 2217), 14% were prescribed a high-dose antipsychotic. The frequency of high-dose monotherapy, APP, and high-dose APP was 3%, 26% and 11%, respectively. Being male, Māori, on compulsory treatment, having a schizophrenia diagnosis, or being prescribed polypharmacy were associated with high-dose antipsychotics. Olanzapine was most frequently prescribed in both high dosing (55%) and APP (40%).There is a high prevalence of high-dose prescribing and APP in this NZ setting.

ABSTRACT IntroductionPharmacist integration into general practice is gaining momentum internationally, with benefits noted in reducing medication errors, improving chronic disease management and alleviating general practitioner workforce shortages. Little is known about how general practice pharmacists are working in New Zealand. AimThis study characterised the current landscape of pharmacist integration into general practice in New Zealand. MethodsAn online questionnaire was developed, piloted and distributed to all pharmacists in New Zealand. ResultsThirty-six responses were analysed. Respondents were more likely to be female, have been pharmacists for at least 10 years and all but one held formal postgraduate clinical pharmacy qualifications. Seven pharmacists were working as pharmacist prescribers. Primary health organisations (PHOs) were the most common employer, with funding primarily derived from either PHOs or District Health Boards. Pharmacist integration into general practice appears to have progressed further in particular regions of New Zealand, with most respondents located in the North Island. Tasks performed by respondents included medication reviews, managing long-term conditions and medicines reconciliation. Increased job satisfaction compared with previous roles was reported by most respondents. Funding and a general lack of awareness about pharmacists’ professional scope were seen as barriers to further expansion of the role. DiscussionThis study describes the characteristics of pharmacists currently working in general practices in New Zealand and provides insights into key requirements for the role. Understanding the way practice pharmacists are currently employed and funded can inform general practices considering employing pharmacists.

Objective To explore pharmacist’s views on the shift in ethos, funding and service delivery model introduced through the New Zealand’s Community Pharmacy Services Agreement (CPSA). Methods A purposive sampling approach drew pharmacists from a matrix who were then contacted via telephone and invited to be interviewed. Semistructured interviews were conducted face-to-face with community pharmacists (n = 17) across urban and rural New Zealand. An interview schedule exploring 12 subject areas was used to facilitate discussion and determine pharmacist’s views and understanding of the CPSA. The interviews were recorded and transcribed verbatim and a general inductive approach was taken to identifying emergent themes. Key findings Key themes that emerged were: pharmacists supported the philosophy behind the CPSA, pharmacists understanding of the CPSA, implementing CPSA-related services, perceived impact on patient outcomes and future sustainability of the CPSA. Overall, pharmacists supported the alignment of funding with patient-centred services, but pharmacy owners reported difficulty understanding the funding model, resulting in uncertainty over income. Several pharmacists believed the quality of care offered had not changed, while others found their attitudes towards care had evolved. All pharmacists communicated an increase in their workload and many perceived the sustainability of the CPSA to be linked to its ability to financially sustain community pharmacies. Conclusions The majority of pharmacists believed in the philosophy of the CPSA, but expressed concerns over funding, workload and benefits for patients. Future research is required to determine generalisability of these findings, investigate patient perspectives and assess the effect of the CPSA on patient outcomes.

Background Atrial fibrillation (AF) is associated with increased risk of cardiovascular disease (CVD) complications including stroke. We investigated the assessment and management of cardiovascular risk among patients with AF aged 35–74years, by ethnic group, in a large cohort of people receiving a CVD risk assessment in primary care (PREDICT). Methods PREDICT was linked to national dispensing, hospitalisation and mortality records. AF was present if recorded in PREDICT or during a prior hospitalisation; medications were those dispensed ≤6months before or after a PREDICT assessment; the CHA 2 DS 2 -VASc score and a New Zealand (NZ) adjusted Framingham CVD risk were calculated. Data were linked to outcomes of stroke or major adverse cardiovascular event (MACE). Results 12,739 (2.8%) of 447,020 people aged 35–74years had AF. Māori, the indigenous population of NZ, had the highest proportion of AF, which by age group, was similar to that among Europeans 10years older. 77% were at high stroke risk, of whom 42% received anticoagulation; 54% were at high CVD risk, of whom 67% received both lipid- and blood pressure-lowering medication. Per category of predicted risk, stroke risk was overestimated and risk of MACE was underestimated. Conclusions The burden of AF and risk factors differed by ethnic group thus recommendations to screen for AF above a universal age threshold may introduce inequity in the detection and management of associated risk. The high burden of comorbidities at younger ages among many ethnic groups contributes to the poor performance of available risk assessment tools, further compounding potential inequity.

Objective To examine attitudes towards a new collaborative pharmacy-based model of care for management of warfarin treatment in the community. As background to the study, the New Zealand health authorities are encouraging greater clinical involvement of community pharmacists. Methods Fifteen community pharmacies in New Zealand took part in a community pharmacist-led anticoagulation management service (CPAMS). Participants (patients, general practitioners, practice nurses, pharmacists) were surveyed on their views on accessibility, convenience, confidence in the service, impact on warfarin control, impact on workloads, effect on relationships and whether the service should be further implemented. A small number from each group was interviewed on the same topics. Key findings Patients reported improved access, convenience, a preference for capillary testing, and the immediacy of the test result and dose changes. They indicated that they had a better understanding of their health problems. While sample sizes were small, the majority of general practitioners and practice nurses felt there were positive benefits for patients (convenience) and themselves (time saved) and expressed confidence in pharmacists' ability to provide the service. There were some concerns about potential loss of involvement in patient management. Pharmacists reported high levels of satisfaction with better use of their clinical knowledge in direct patient care and that their relationships with both patients and health professionals had improved. Conclusions The new model of care was highly valued by patients and supported by primary care practitioners. Wider implementation of CPAMS was strongly supported. Pharmacists and general practitioners involved in CPAMS reported a pre-existing collaborative relationship, and this appears to be important in effective implementation.

Publicly-funded atorvastatin required prior approval until September 2010 whereas simvastatin did not. Our aim was to examine if overall statin dispensing and atorvastatin dispensing among patients hospitalised for cardiovascular disease (CVD) differed systematically across sociodemographic groups during and after special authority criteria.National medication dispensing data were anonymously linked to patients hospitalised across New Zealand with CVD and discharged between 1/07/2009-31/12/2009 when special authority criteria applied and 1/09/2010-28/02/2011 after restrictions ceased. Statin dispensing at least once within six months post-discharge was analysed by sociodemographic characteristics.Overall statin use was the same (80%) among patients discharged during (n=14,094) and after (n=13,274) restrictions. With restrictions, atorvastatin dispensing was 32-33% less frequent among statin-users45 years and75 years than 65-74 year olds and 28-55% less among Māori, Pacific and Indian peoples than all others. Minimal relative differences occurred by sex or deprivation status. After restrictions were lifted, the proportion of statin-users dispensed atorvastatin increased around two-fold or more across all sociodemographic strata with three-four fold increases for patients55 years and for Māori, Pacific and Indian peoples.After funding restrictions ceased, disparities in atorvastatin dispensing appeared to reduce across age and ethnic groups among patients with CVD-related hospitalisations, but overall statin use was unchanged.

Objective To determine the alignment between New Zealand pharmacists’ views and the New Zealand Ten-Year Vision (TYV) document and explore factors associated with individuals’ alignment. Methods A postal survey of 1892 practicing pharmacists was conducted. The questionnaire included 37 attitude statements informed by analysis of 6 focus groups in which pharmacists discussed 24 statements representing 12 main vision areas. Responses are reported by proportion. Bivariate analysis compared differences in demographic characteristics and alignment with vision statements between 3 professional subgroups: community, hospital, and pharmacists working in District Health Boards or Primary Health Organizations. Results Nine hundred and eighty (54.6%) analyzable surveys were returned. Respondents broadly agreed with the 12 vision areas. There was a clear patient focus and strong support for extending roles to better serve patient care. Pharmacists believe that current practice and funding models need to change and want greater involvement in setting health policy. There are significant differences in agreement and level of agreement between professional subgroups. Notably, community pharmacists are more likely to put their relationship with the patient above that with the doctor, to believe that other professions would resist role extension, to support minor ailments schemes, and to indicate a shift in funding models was necessary to support innovative pharmacy practice. There are statistically significant and potentially important demographic differences between professional subgroups, including age, sex, formal postgraduate training, and experience working in health care in other countries. Further analysis is required to understand the interplay of these factors and how this may influence alignment with the TYV document. Conclusions Pharmacists’ responses indicated a high level of alignment with vision outlined in the TYV. Pharmacists appear receptive to practice and funding changes in order to facilitate greater contribution to patient care. Respondents demonstrated a clear desire to be involved in setting medicines-related health policy and feel underrepresented at this level.

Objective of the study To describe the dimensions of organisational culture within a selection of community pharmacies. Setting Community pharmacy in the New Zealand primary care sector which is partially government funded and currently undergoing major reform. Community pharmacy is under pressure to take on new roles, integrate within the wider primary care team and deliver the expectations of contemporary health policy. Method The mixed methods approach of concept mapping was undertaken with 10 representatives from six community pharmacies selected as case sites. The process was split into three parts (a) face to face brainstorming to generate statements describing culture, followed by (b) statement reduction, piloting and approval of statement list by participants, followed by (c) sorting the statements into ‘like’ groups. Multidimensional scaling analysis of participant sorting allows the development of discrete clusters of statements that describe aspects of organizational culture. Results A set of 105 statements were generated at the brainstorming meeting. Eight clusters of organisational culture resulted from participant sorting: leadership and staff management; valuing each other and the team; free thinking, fun and open to challenge; trusted behaviour; customer relations; focus on external integration; providing systematic advice; embracing innovation. Conclusion Community pharmacy is under pressure to take on new roles and deliver and there is some evidence organisational culture of pharmacy may be a barrier. Our paper outlines the development of a survey instrument for describing organisational culture through Concept mapping, a tool borrowed from social sciences. This tool can be used for exploration of aspects of culture that may be important in the change management process for improving the effectiveness of community pharmacy as expected by contemporary primary health care policy.

To examine if optimal clopidogrel therapy following percutaneous coronary intervention (PCI) differed systematically by sociodemographic characteristics during and after special authority (SA) restrictions, which required hospital-specialist approval for full funding.National health databases were anonymously linked for New Zealanders discharged following publicly-funded PCI between 1/07/2009-31/12/2009 when SA criteria applied and from 1/09/2010-28/02/2011 after funding restrictions ceased. The proportion of days that patients were dispensed clopidogrel in the two 6-month periods post-discharge was calculated (medicine possession ratio;MPR). Optimal clopidogrel-dispensing (defined as a MPR0.8) was analysed by sociodemographic characteristics.During the first discharge period, 74% (95% CI 72-75%) of patients (n=2416) had optimal dispensing compared with 81% (95% CI 79-82%) of discharges from 1/09/2010-28/02/2011 (n=2347). In both discharge periods, 2-3% of patients received no therapy. Minimal relative differences in optimal dispensing were noted by age, sex, or deprivation status in either discharge period. However, optimal clopidogrel-dispensing was 13-14% less likely among Maori and Pacific peoples relative to 'Other' patients during funding restrictions. Relative ethnic differences changed little once funding restrictions ended despite absolute increases in dispensing.Optimal clopidogrel coverage following PCI improved by 7% across sociodemographic groups after funding restrictions ceased, but ethnic disparities were unaltered.

Objective To describe patterns of statin use and predictors of poor maintenance over a 3-year period following an acute coronary syndrome (ACS). Methods National hospitalisation, mortality and pharmaceutical dispensing data were linked for all subjects aged 35–84 years discharged from a public hospital with an ACS in New Zealand in 2007. A Medication Possession Ratio (MPR; percentage of follow-up days patients were dispensed statins) was calculated for each patient. Adequate maintenance was defined by a MPR ≥80%. Results In 2007, 11 348 patients aged 35–84 years were discharged from hospital with ACS. Within 90 days of discharge, 83% had received a statin. Over the follow-up period, 66% were adequately maintained on a statin (MPR ≥80%): 69% in the first year, 67% in the second year and 66% in the third year. Patients taking statins prior to admission and those who underwent a coronary procedure were 20–50% more likely to have a MPR ≥80% over 3 years than others. In contrast, people aged 35–45 years and those of Maori or Pacific ethnicity were 13–25% less likely to have a MPR ≥80% than those aged 55–64 years and Europeans. Conclusions One-third of patients were not adequately maintained on statins over the 3-year period following ACS, but 82% of those on a statin prior to admission had an MPR ≥80% over 3 years of follow-up. These findings define achievable treatment levels and identify groups who may benefit from efforts to improve statin use.

Clozapine has been available since the early 1990s. Studies continue to demonstrate its superior efficacy in treatment-resistant schizophrenia. Despite this, numerous studies show under-utilisation, delayed access and reluctance by psychiatrists to prescribe clozapine. This retrospective cross-sectional study compared the prescribing of clozapine in two adult cohorts under the care of large public mental health services in Auckland (New Zealand) and Birmingham (United Kingdom) on 31 March 2007. Time from first presentation to clozapine initiation, prior antipsychotics trialled and antipsychotic co-prescribing were compared. Data included demographics, psychiatric diagnosis, co-morbid conditions, year of first presentation, admissions and pharmacological treatment (clozapine dose, start date, prior antipsychotics, co-prescribed antipsychotic). Overall, 664 people were prescribed clozapine (402 Auckland; 262 Birmingham); mean daily dose of 384 mg (Auckland) and 429 mg (Birmingham). 53 % presented after 1990 and the average duration of time before starting clozapine was significantly longer in the Birmingham cohort (6.5 vs. 5.3 years) but this reduced in both cohorts to a 1-year mean in those presenting within the last 3 years. The average number of antipsychotics trialled pre-clozapine for those presenting since 1990 was significantly higher in the Birmingham cohort (4.3 vs. 3.1) but in both cohorts this similarly reduced in those presenting within the last 3 years. Antipsychotic co-prescribing was significantly higher in the Birmingham cohort (22.9 vs. 10.7 %). There is evidence that access to clozapine has improved over time in both cohorts, with a reduction in the duration between presentation and initiation of clozapine and number of different antipsychotics trialled pre-clozapine. These are very positive findings in terms of optimising outcomes with clozapine and are possibly due to the impact of guideline recommendations, increasing clinician, consumer and carer knowledge, and experience with clozapine and funding changes.

Objectives Despite the introduction of new oral anticoagulants, vitamin K antagonists remain the mainstay of the prevention and treatment of thromboembolism. The advent of affordable point-of-care testing presents an opportunity for community pharmacists to provide anticoagulation management services, better utilizing their training, reducing the workload on medical practices and improving accessibility and convenience for patients. This study aimed to determine the effectiveness of anticoagulation management by community pharmacists. Methods All patients enrolled in a pilot programme for a community pharmacy anticoagulation management service using point-of-care international normalized ratio testing and computer-assisted dose adjustment were included in a follow-up study, including before–after comparison. Outcomes included time in therapeutic range (TTR), time above and below range, number and proportion of results outside efficacy and safety thresholds, and a comparison of care led by pharmacists and care led by a primary-care general practitioner (GP). Key findings A total of 693 patients were enrolled, predominantly males over 65 years of age with atrial fibrillation. The mean TTR was 78.6% (95% CI 49.3% to 100%). A subgroup analysis (n = 221) showed an increase in mean TTR from 61.8% under GP-led care to 78.5% under pharmacist-led care (P < 0.001), reflecting a reduction in the time above and, in particular, below the range. The mean TTR by pharmacy ranged from 71.4% to 84.1%. The median number of tests per month was not statistically different between GP- and pharmacist-led care. Conclusions Community-pharmacist-led anticoagulation care utilizing point-of-care testing and computerized decision support is safe and effective, resulting in significant improvements in TTR. Our results support wider adoption of this model of collaborative care.

Background Statins have numerous potential benefits relevant to abdominal surgery, and their use has been associated with a reduction in the systemic inflammatory response syndrome, wound infection, and anastomotic leak after colorectal surgery. However, this clinical evidence is limited to retrospective studies. The aim of this study was to prospectively investigate whether perioperative statin therapy can decrease the incidence of complications after major colorectal surgery. Study Design A prospective, double-blind, parallel-group, randomized controlled trial was conducted at 3 tertiary hospitals in New Zealand, between October 2011 and August 2013. Adult patients undergoing elective colorectal resection for any indication or reversal of Hartmann's procedure were randomized with a 1:1 patient allocation ratio to receive either 40 mg oral simvastatin or placebo once daily for 3 to 7 days preoperatively until 14 days postoperatively. The primary outcome was the overall incidence of complications for 30 days postoperatively. Secondary outcomes included the systemic and peritoneal cytokine response (interleukin [IL]-1α, IL-1β, IL-6, IL-8, IL-10, tumor necrosis factor [TNF]α) on postoperative day 1. Results There were 132 patients included in the study (65 simvastatin, 67 placebo). There were no significant differences between the 2 groups at baseline with regard to patient, operation, and disease characteristics. There were no significant differences between the 2 groups in the incidence, grade, and type of postoperative complications (simvastatin: 44 [68%] vs placebo: 50 [75%], odds ratio 0.71 [95% CI 0.33 to 1.52], p = 0.444). Plasma concentrations of IL-6, IL-8, and TNFα, and peritoneal concentrations of IL-6 and IL-8, were significantly lower in the simvastatin group postoperatively. Conclusions Perioperative simvastatin therapy in major colorectal surgery attenuates the early proinflammatory response to surgery, but there were no differences in postoperative complications.

Background: This study evaluates the effect of a program combing specialized medication packaging and telephonic medication therapy management on medication adherence, health care utilization, and costs among Medicaid patients. Research Design: A retrospective cohort design compared Medicaid participants who voluntarily enrolled in the program (n=1007) compared with those who did not (n=13,614). Main outcome measures were medication adherence at 12 months, hospital admissions and emergency department visits at 6 and 12 months, and total paid claim costs at 6 and 12 months. Multivariate regression models were used to adjust for the effect of age, sex, race, comorbidities, and 12-month preenrollment health care utilization. Results: Measures of medication adherence were significantly improved in the program cohort compared with the usual care cohort. At 6 months, adjusted all-cause hospitalization was marginally less in the program cohort compared with the usual care cohort [odds ratio=0.73, 95% confidence interval (CI), 0.54‐1.0, P=0.05]. No statistically significant differences were observed between the 2 cohorts for any of the other adjusted utilization endpoints at 6 or 12 months. Adjusted total cost at 6 and 12 months were higher in the program cohort (6-month cost ratio=1.76, 95% CI,1.65‐1.89; 12month cost ratio=1.84, 95% CI,1.72‐1.97), primarily because of an increase in prescription costs. Emergency department visits and hospitalization costs did not differ between groups. Conclusions: The program improved measures of medication adherence, but the effect on health care utilization and nonpharmacy costs at 6 and 12 months was not different from the usual care group. Reasons for these findings may reflect differences in the delivery of the specialized packaging and the medication therapy management program, health care behaviors in this Medicaid cohort, unadjusted confounding, or time required for the benefit of the intervention to manifest.

INTRODUCTION: Blood pressure–lowering (BPL) and lipid-lowering (LL) medications together reduce estimated absolute five-year cardiovascular disease (CVD) risk by >40%. International studies indicate that the proportion of people with CVD receiving pharmacotherapy increases with advancing age. AIM: To compare BPL and LL medications, by sociodemographic characteristics, for patients with known CVD in primary care settings. METHODS: The study population included patients aged 35–74 with known CVD assessed in primary care from July 2006 to October 2009 using a web-based computerised decision support system (PREDICT) for risk assessment and management. Clinical data linked anonymously to national sociodemographic and pharmaceutical dispensing databases. Differences in dispensing BPL and LL medications in six months before first PREDICT assessment was analysed according to age, sex, ethnicity and deprivation. RESULTS: Of 7622 people with CVD, 1625 < 55 years old, 2862 were women and 4609 lived in deprived areas (NZDep quintiles 4/5). The study population included 4249 European, 1556 Maori, 1151 Pacific and 329 Indian peoples. BPL medications were dispensed to 81%, LL medications to 73%, both BPL and LL medications to 67%, and 87% received either class of medication. Compared with people aged 65–75, people aged 35–44 were 30–40% less likely and those aged 45–54 were 10–15% less likely to be dispensed BPL, LL medications or both. There were minimal differences in likelihood of dispensing according to sex, ethnicity or deprivation. DISCUSSION: BPL and LL medications are under-utilised in patients with known CVD in New Zealand. Only two-thirds of patients in this cohort are on both. Younger patients are considerably less likely to be on recommended medications. Background: In New Zealand, a setting in which national guidelines recommend statins for all patients with coronary heart disease (CHD) and cost barriers are low, patterns of use of these drugs are unknown. We investigated dispensing rates after hospital discharge for acute CHD event. Design: Retrospective cohort study. Methods: Drug dispensing, hospital diagnosis, and mortality records were linked by unique identifier for all New Zealanders aged 35–84 years after discharge following acute CHD event in 2007. We defined the statin dispensing ratio (SDR) as the proportion of days that 15,506 patients aged 35–84 years were dispensed such agents during the 12 months post discharge. An SDR ≥0.8 (80% or more days covered) was considered optimal. Results: Overall, 59% of the cohort had an SDR ≥0.8. Of patients dispensed statins in the 3 months before admission ( n = 5506), almost all (99%; 5466) continued treatment during follow up and 82% had an SDR ≥0.8. In contrast, for patients not dispensed statins before admission ( n = 8014), only two–thirds started statins during follow up and only 44% had an SDR ≥0.8. Of all patients with low statin dispensing (SDR Conclusion: In a setting with few barriers to statin treatment, about 40% of patients had suboptimal statin dispensing during the year after hospital treatment for CHD. This study has identified four significant categories of suboptimal adherence that could inform quality improvement programmes.

Objectives: The aim of this study was to describe a metabolic risk assessment programme that was established by community mental health teams working within one locality of a District Health Board in Auckland. Conclusions: Physical health disparities between people with serious mental illness and the general population are of great concern. We have described a systematic way to engage with clients about the prevention of two important contributors to these disparities: cardiovascular disease and diabetes mellitus. Around 15% of all adult service users of the mental health service in the locality attended this metabolic risk assessment clinic in the first 12 months of its operation.

Objectives: The aim of this study is to understand the barriers and resulting solutions encountered by mental health service staff during the implementation of a metabolic risk assessment programme. Method: Semi-structured qualitative interviews were held with key programme staff. Thematic analysis was performed on the resulting data. Results: Barriers identified included an existing mindset that care of service users’ physical health is not part of Mental Health Service ‘core business’; the lack of processes that ensure consistent implementation of risk assessment; and lack of resources in terms of equipment and staff time/manpower. Solutions identified were categorized as: systems and process issues; staff training or education issues; issues requiring attention at an organizational level; and issues relating to service users, their family and communities. Conclusions: A valuable programme that contributes to the improved physical health of service users has been developed and implemented. However, a number of barriers to wider adoption were identified. Solutions to a number of these barriers were identified during implementation.

Objective. To develop a multi-constituent model of organizational effectiveness for community pharmacy. Design. Using Concept Systems w software, a project with 14 stakeholders included a three stage process: (i) face to face brainstorming to generate statements describing what constitutes an effective community pharmacy, followed by (ii) statement reduction and approval by participants, followed by (iii) sorting of the statements into themes with rating of each statement for importance. Setting. Primary care in a government-funded, national health care system. Participants. A multi-constituent group representing policy-makers and health care providers including; community pharmacy, professional pharmacy organizations, primary health care funders and policy-makers, general practitioners and general practice support organizations. Results. Statement clusters included: ‘has safe and effective workflows’, ‘contributes to the safe use of medicines’, ‘manages human resources and has leadership’, ‘has a community focus’, ‘is integrated within primary care’, ‘is a respected innovator’, ‘provides health promotion and preventative care’, ‘communicates and advocates’. These clusters fit into a quadrant model setting stakeholder focus against role development. The poles of stakeholder focus are ‘internal capacity’ and ‘social utility’. The poles of role development are labelled ‘traditional safety roles’ and ‘integration and innovation’. Conclusions. Organizational effectiveness in community pharmacy includes the internal and external focus of the organization and role development. Our preliminary model describes an effective community pharmacy and provides a platform for investigation of the factors that may influence the organizational effectiveness of individual community pharmacies now and into the future.

Objectives The aim was to determine New Zealand pharmacists' views on the range of services outlined in the Ten Year Vision for Pharmacists document and the need for accreditation to provide these services. Methods A national postal survey of practising pharmacists registered with the Pharmacy Council of New Zealand (n= 1892) was carried out, with two follow-ups. Key findings The response rate was 51.8% (n= 980 usable surveys). Findings indicated that the majority of pharmacists believe they should continue to undertake traditional clinical and technical roles (median 98.5%, range 92.7–99.3%). Less than one-third of respondents felt these activities required pharmacists to be accredited. A lower proportion, but still the majority, of respondents thought that pharmacy should undertake selected enhanced or collaborative roles (median 74.85%, range 64–92.5%). However, there was a greater emphasis on accreditation for these roles, with more than two-thirds of respondents suggesting a need for accreditation. Conclusions There is a high level of support for the retention of current clinical and technical roles. A lack of need for additional accreditation suggests that pharmacists believe their training is adequate. There is a positive, but more tempered view regarding enhanced or collaborative services. There is recognition of a greater need for accreditation for enhanced and collaborative services. This suggests a cautious optimism about new services and a perceived need for pharmacists to learn more about these programmes.

This study aimed to determine New Zealand pharmacists' awareness of, recall of consultation about, and potential barriers to the implementation of the Focus on the Future: Ten Year Vision for Pharmacists in New Zealand: 2004-2014 document.A national postal survey was carried out in New Zealand of practising pharmacists registered with the Pharmacy Council of New Zealand (n = 1892). The survey was conducted between September and December 2006.The response rate was 51.8% (n = 980 usable surveys). Approximately three-quarters (73.4%) reported being aware of the 10-year vision document and 40.9% recall being consulted. Fewer than one-third (29.8%) had read the document prior to completing the survey. Thirty-two variables describing potential barriers to implementing the 10-year vision were reduced, through factor analysis, to seven factors with significant eigenvalues (1.0). The factors describe the underlying themes of barriers identified in the survey, including pharmacist humanistic, integrated systems of care and teamwork, funder stakeholder relationships and remuneration, lack of appreciation of knowledge and skills, lack of research support, current expertise and continuing professional development, and lack of voice. The majority of barrier variables within the seven factors were rated as extremely or quite important.There was a high level of awareness and moderate level of recall of consultation on the document among respondents. Although 40% recall being consulted on the document, fewer than one-third reported having read the document prior to completing our survey. Pharmacists highlighted a significant number of potential barriers to the implementation of the 10-year vision. If the pharmacy profession in New Zealand is to move forward towards the state described in the 10-year vision document then these potential barriers need to be better understood and addressed.

Background The barriers to moving forward and meeting the expectations of policy makers and professional pharmacy bodies appear to relate to the organizational culture of community pharmacy. Despite the importance of cultural change for business transformation, organizational culture has largely gone unnoticed in community pharmacy practice research. Objectives To perform an organizational culture gap analysis in 6 New Zealand community pharmacies. Methods Mean scores from a cultural rating survey (n = 47) were calculated for 8 cultural clusters and mapped onto a typical and a beneficial pattern match (ladder diagram) for each case site. These ladder diagrams provide an understanding of the gap between the 2 ratings based on the gradient of the lines joining cultural clusters—the rungs of the ladder. Software can be used to generate a Pearson correlation describing the strength of the relationship between the typical and beneficial ratings. Results Eight cultural clusters were mapped: “leadership and staff management”; “valuing each other and the team”; “free-thinking, fun and, open to challenge”; “trusted behavior”; “customer relations”; “focus on external integration”; “provision of systematic advice”; and the “embracing of innovation.” Analysis suggested a high level of correlation between the means of the typical and beneficial ratings. Although the variance between average ratings might be quite small, the relative difference can still be meaningful to participants in the cultural setting. The diagrams suggest a requirement for external integration, the provision of systematic advice, and the embracing of innovation to become more typical in most pharmacies. Trusted behavior is the most typical and most beneficial cultural dimension in most pharmacies, whereas valuing each other and the team is the least beneficial. Conclusions Gaps in organizational culture have been identified through the use of a rating survey. The dimensions of focus on external integration, providing systematic advice, and embracing innovation require further exploration through interviews in case site pharmacies.

Thromboprophylaxis is highly effective, safe and cost-effective. However, data from international studies have shown that many at-risk hospitalised patients do not receive appropriate prophylaxis, leaving them at risk of venous thromboembolism (VTE) and its consequences.To assess whether patients readmitted with a VTE to the North Shore Hospital, Auckland, received appropriate thromboprophylaxis during their index admission.Discharge coding was used to identify all patients admitted for VTE-related events between 1 July 2005 to 30 June 2007. Further analysis identified those patients hospitalised for any cause in the 3 months prior to the VTE event. Data were collected on patient characteristics, risk factors for VTE and the thromboprophylaxis regimen prescribed. The seventh American College of Chest Physicians (ACCP) guidelines on the prevention of venous thromboembolism were used to assess VTE risk and to determine whether patients received appropriate prophylaxis.Of the 962 patients admitted with a VTE event, 243 (25%) had been hospitalised within the preceding 3 months. 108 patients were eligible for inclusion in the study; 58 were categorized as medical admissions and 50 as surgical. Using the ACCP-defined criteria, 29 (50%) medical and 38 (76%) surgical patients were identified as being at increased risk of VTE. Only one third of at-risk patients received ACCP-recommended prophylaxis during their index hospital admission. More surgical patients received recommended prophylaxis than medical patients, although this was not statistically significant (37% vs 24%, p=0.3, Fisher's exact test).A significant number of at-risk patients, who subsequently developed VTE after discharge from hospital, failed to receive appropriate thromboprophylaxis during their index admission. Implementing national and local strategies, to improve the use of thromboprophylaxis in hospitalised patients, may help to reduce the burden of VTE.

To assess the predictive value of general practice electronic prescribing records with respect to adherence to long-term medications as compared to claims-based pharmacy dispensing data.A total of 29772 electronic prescribing records relating to 2713 patients attending a New Zealand general medical practice were linked by national health identifier to 63 833 dispensing records used for community pharmacy reimbursement. Individual possession ratios-prescription possession ratio (PPR) for prescribing and medication possession ratio (MPR) for dispensing-were calculated for the 15-month period from 1 January 2006 to 30 March 2007 based on each data source for the common long-term medications simvastatin, metoprolol succinate, bendrofluazide, felodipine, cilazapril and metformin.Out of 646 patients prescribed at least one of the six medications by the practice during the 15-month period, 50% of patients maintained high adherence (MPRor = 80%) to all (out of the 6) medications that they were prescribed over the period, with rates of high adherence to individual medications ranging from 68 (felodopine) to 55% (metformin). In 93% of 4043 cases where there was a prescription in the general practice data, a subsequent dispensing record for the same patient and drug was present with a time-stamp no more than seven days later. PPR80% demonstrated a positive predictive value (PPV) of 81.4% (95%CI 78-85%) and negative predictive value (NPV) of 76.3% (95%CI 73-79%) for MPR80%.There is potential for general practices to identify substantial levels of long-term medication adherence problems through their electronic prescribing records. Significant further adherence problems could be detected if an e-pharmacy network allowed practices to match dispensing against prescriptions.

Objective To describe clozapine prescribing in a mental health service in Auckland, New Zealand and compare it with national and international treatment guidelines. Setting A large public mental health service for adults in Auckland, New Zealand. Method A retrospective cross-sectional study of all adult outpatients and stable inpatients being treated with clozapine on 31st March 2007 in one mental health service in Auckland, New Zealand. Data on patient characteristics, diagnosis, duration of illness, number of hospitalisations, legal status relating to their treatment, living situation, marital status and occupational activity were recorded from case notes. Data collected on clozapine included date of initiation, dose and duration of treatment. Prior antipsychotic use and information on all other psychotropic drugs prescribed was also collected. Data were entered into a custom-designed Microsoft Access database and analysed using SPSSv15.0. Main outcome measures Clozapine prescribing patterns and concordance with best practice recommendations for clozapine use. Results 402 adult mental health outpatients and stable inpatients were eligible for inclusion. The mean daily dose of clozapine was 383 (SD 166) mg. For those first presenting after universal government funding, the mean time between presentation and initiation of clozapine, was 2.8 (SD 1.9) years, compared to 5.7 (SD 3.3) years prior to funding. Of those presenting after universal government funding, approximately two-thirds (69.0%) had ≤2 trials with other antipsychotics prior to commencing clozapine; of whom the majority (62.0%) received only second-generation antipsychotics (SGA). Both the number of antipsychotic agents trialled and the time to clozapine initiation has fallen since government subsidy was introduced in 1999. Based on a analysis of annualised hospitalization rates, it appears that shortening the delay to receiving clozapine leads to fewer hospitalisations in this treatment-resistant population, although this did not achieve statistical significance in our study. Conclusions Contemporary management of patients with treatment resistant schizophrenia in New Zealand is broadly in line with national and international best practice guidelines. There is some evidence, based on hospitalisation rates, to support the assertion that shorter delays in accessing clozapine leads to better outcomes. This needs further evaluation using measures of clinical outcome including objective measures of functioning.

In a 2 year double blind controlled trial of cyclosporin against placebo in multiple sclerosis conducted at two centres there was a beneficial effect of the therapy upon the progression of the disease, relapse rate and relapse severity at one of the centres where the patients received a mean dose of 7.2 mg/kg/day. This beneficial effect was not seen in the other centre where a lower dose (mean 5 mg/kg/day) was given. Reduction in clinical progression was accompanied by decreased IgG synthesis in the central nervous system. Side effects included hypertension, renal insufficiency and anaemia and were of such severity to preclude the use of cyclosporin in a high enough dose to alter the course of the disease.

Background Adherence to antidepressant therapy remains a major issue worldwide. Most people with depression are treated in a general practice setting, but many stop taking antidepressants before completing a six-month course as recommended by guidelines. Objectives To determine antidepressant adherence rates as indicated in primary care prescribing data and pharmacy dispensing data; to demonstrate commonly occurring patterns related to non-adherence, using a prescription visualisation tool we have developed; and to determine whether prescribing data is a good predictor of dispensing based adherence. Methods We analysed general practice electronic prescribing data for the year ending 31 December 2006 and linked pharmacy dispensing records by National Health Index. We calculated medication adherence for patients starting antidepressants using a six-month evaluation period and a gap-based adherence measure. Patients with a gap of more than 15 days in antidepressant therapy were considered non-adherent. Using a prescription visualisation tool,we described common modes of non-adherence. Results Out of 2713 patients, 153 satisfied our inclusion criteria. Thirty-nine percent of patients showed poor adherence based on prescribing and 68% showed poor adherence on dispensing. Prescribing based non-adherence had a positive predictive value of 98% (95% CI 92%_99%) and negative predictive value of 51% (CI 47%_52%) for dispensing based non-adherence. Three broad categories of non-adherence were identified: 1) failure to return for re-prescription, 2) failure to maintain adherence despite initial attempts and 3) failure to return for re-prescription in a timely manner. Conclusions Prescribing data identifies substantial adherence issues in antidepressant therapy. Clinicians should consider adherence issues as part of the overall treatment regime and discuss such issues during consultations.

INTRODUCTION: Although people with serious mental illness (SMI) have a high prevalence of physical illness, health-related quality of life (HQoL) has not been sufficiently explored. AIM: To explore the self-reported HQoL of mental health service users in New Zealand. METHODS: Responses on the Medical Outcomes Study 36 Item Short Form (SF-36) measure of HQoL from 404 adult mental health service users in a metropolitan district health board area in New Zealand were analysed and compared to a representative sample of the general population. RESULTS: Mental health service users scored significantly lower on all eight domains of the SF-36 than the general population, the largest difference being in the role limitation — emotional domain. DISCUSSION: Being female, younger than 25, obese or overweight, or of New Zealand European/Other ethnicity were associated with poorer functioning on multiple HQoL domains. Future studies should seek to understand the factors contributing to perceptions of HQoL of mental health service users in New Zealand. To explore the feasibility and reliability of Clinical Coding Surveillance (CCS) for the routine monitoring of Adverse Drug Events (ADE) and describe the characteristics of harm identified through this approach in a large district health board (DHB).All hospital admissions at Waitemata DHB from 2015 to 2016 with an ADE-related ICD10-AM code of Y40-Y59, X40-X49 or T36-T50 were extracted from clinical coded data. The data was analysed using descriptive statistics, statistical process control and Pareto charts. Two clinicians assessed a random sample of 140 ADEs for their accuracy against what was clinically documented in medical records.A total of 11,999 ADEs were identified in 244,992 admissions (4.9 ADEs per 100 admissions). ADEs were more prevalent in older adults and associated with longer average length of stays and medicines such as analgesics, antibiotics, anticoagulants and diuretics. Only 2,164 (18%) of ADEs were classified as originating within hospital. Of ADEs originating outside of the hospital, the main causes were poisoning by psychotropics, anti-epileptics and anti-parkinsonism agents and non-opioid analgesics. Clinicians agreed that 91% of ADE positive admissions were accurately classified as per clinical documentation.CCS is a feasible and reliable approach for the routine monitoring of ADEs in hospitals.
