Purpose To evaluate whether visual target character and visibility affects spontaneous eye blink rate (SEBR) in primary eye gaze and silence. Methods Video recordings were made of young healthy adults who were either emmetropic (n = 32) or who wore spectacles for refractive error (range −4.75 D and +4.50 D (n = 31). Emmetropes had 5 min recordings made whilst seated and looking towards a distant whiteboard. For spectacle wearers, recordings were made whilst looking towards the whiteboard with a 35 mm sized cross, and repeated after spectacle removal. The average number of eye blinks over 5 min was assessed, and its intra-subject variability as the coefficient of variation (COV). Results Over 5 min without a distance target, an average SEBR of 10.4 blinks/min was observed in emmetropes with a of COV = 38.1%, and a significant increase in SEBR over the 5th minute to 13.6 blinks/min. Hyperopes being asked to look towards a distant target showed the essentially same blinking rate of 11.1/min with or without spectacle wear with the intra-subject variability (COV) being 21.3%. Myopic subjects showed a slightly higher SEBR if looking towards a target without their spectacles (12.4 vs. 11.0 blinks/min), with the COV being 18.8%. Conclusions The studies indicate that some form of visual target could be useful to promote constancy of spontaneous eye blink activity over time, but that a distance visual target (when provided) does not need to be seen clearly.

In Colombia, the cutaneous leishmaniasis (CL) is the most common manifestation across the army personnel. Hence, it is mandatory to determine the species associated with the disease as well as the association with the clinical traits. A total of 273 samples of male patients with CL were included in the study and clinical data of the patients was studied. PCR and sequencing analyses (Cytb and HSP70 genes) were performed to identify the species and the intra-specific genetic variability. A georeferenced database was constructed to identify the spatial distribution of Leishmania species isolated. The identification of five species of Leishmania that circulate in the areas where army personnel are deployed is described. Predominant infecting Leishmania species corresponds to L. braziliensis (61.1%), followed by Leishmania panamensis (33.5%), with a high distribution of both species at geographical and municipal level. The species L. guyanensis, L. mexicana and L. lainsoni were also detected at lower frequency. We also showed the identification of different genotypes within L. braziliensis and L. panamensis. In conclusion, we identified the Leishmania species circulating in the areas where Colombian army personnel are deployed, as well as the high intra-specific genetic variability of L. braziliensis and L. panamensis and how these genotypes are distributed at the geographic level., Author summary Colombia is one of the countries with the highest incidence of Cutaneous Leishmaniasis in the world and the army population is the most vulnerable population. Herein, we identified the infecting Leishmania species (L. braziliensis, L. panamensis, L. guyanensis, L. mexicana and L. lainsoni). We also showed the high intra-specific genetic variability of L. braziliensis and L. panamensis and how these genotypes are distributed at the geographic level.

Blastocystis is a common enteric protist colonizing probably more than 1 billion people with a large variety of non-human hosts. Remarkable genetic diversity has been observed, leading to the subdivision of the genus into multiple subtypes (ST), some of which are exclusively found in non-human hosts. The aim of this study was to determine the distribution of Blastocystis STs/18S alleles in symptomatic (abdominal pain, anal pruritus, diarrhea, headache, nauseas and/or vomit) and asymptomatic children from nine geographical regions of Colombia. A total of 2026 fecal samples were collected as part of a national survey to estimate the frequency of intestinal parasites in children. A set of 256 samples that were Blastocystis positive was finally selected. The samples were submitted to DNA extraction, Real Time PCR and sequencing using Blastocystis-specific primers targeting the small subunit rRNA gene for ST identification. DNA of Ascaris lumbricoides (16.4%), Trichuris trichiura (8.2%), hookworms (Necator americanus/Ancylostoma duodenale) (7.3%), Giardia duodenalis (23.1%), Entamoeba complex (82%), Entamoeba coli (55%), Hymenolepis nana (0.8%), Endolimax nana (33.2%) and Neobalantidium coli (2.7%) was detected in the Blastocystispositive samples. We detected ST1 (21.4%), ST2 (19.5%), ST3 (55.5%), ST4 (0.8%), ST6 (2%) and ST7 (0.8%); alleles 1, 2, 4, 81, 82 and 83 for ST1; alleles 9, 11, 12, 15, 67, 71 and 73 for ST2; alleles 34, 36, 38, 45, 49, 55, 134 and 128 for ST3; allele 42 for ST4; allele 122 for ST6, and allele 142 for ST7. Further studies implementing high-resolution molecular markers are necessary to understand the dynamics of Blastocystis transmission and the role of this Stramenopila in health and disease. © 2017 Ramírez et al. Determining the distribution of disease prevalence among heterogeneous populations at the national scale is fundamental for epidemiology and public health. Here, we use a combination of methods (spatial scan statistic, topological data analysis and epidemic profile) to study measurable differences in malaria intensity by regions and populations of Colombia. This study explores three main questions: What are the regions of Colombia where malaria is epidemic? What are the regions and populations in Colombia where malaria is endemic? What associations exist between epidemic outbreaks between regions in Colombia? Plasmodium falciparum is most prevalent in the Pacific Coast, some regions of the Amazon Basin, and some regions of the Magdalena Basin. Plasmodium vivax is the most prevalent parasite in Colombia, particularly in the Northern Amazon Basin, the Caribbean, and municipalities of Sucre, Antioquia and Cordoba. We find an acute peak of malarial infection at 25 years of age. Indigenous and Afrocolombian populations experience endemic malaria (with household transmission). We find that Plasmodium vivax decreased in the most important hotspots, often with moderate urbanization rate, and was re-introduced to locations with moderate but sustained deforestation. Infection by Plasmodium falciparum, on the other hand, steadily increased in incidence in locations where it was introduced in the 2009-2010 generalized epidemic. Our findings suggest that Colombia is entering an unstable transmission state, where rapid decreases in one location of the country are interconnected with rapid increases in other parts of the country. © 2018 Feged-Rivadeneira et al. OBJETIVO: Relatar tratamento de retinopatia por radiação em paciente submetido à radioterapia por linfoma em órbita direita com injeção intravítrea de bevacizumab (Avastin®). Paciente de 55 anos, diabético, com diagnóstico de linfoma MALT orbitário há três anos, tratado com radioterapia local (dose: 35Gy) há dois anos, com queixa de redução da acuidade visual do olho direito há quatro meses. Ao exame oftalmológico, apresentava alterações sugestivas de retinopatia por radiação, bem como espessura macular à tomografia de coerência óptica de 480 µm. Paciente foi submetido à injeção intravítrea (0,05 ml) de bevacizumab (Avastin®) no olho direito, apresentando redução do edema macular, bem como melhora discreta da acuidade visual. Neste caso, o tratamento da retinopatia por radiação com injeção intravítrea de bevacizumab (Avastin®) foi relativamente útil, com melhora discreta da acuidade visual, devido à regressão do edema macular.PURPOSE: To report a case of radiation retinopathy treatment with intravitreal injection of bevacizumab (Avastin®) in a patient undergoing radiotherapy for lymphoma in the right orbit. Patient of 55 years-old male, diabetic, diagnosed with an orbital MALT lymphoma three years ago, treated with local radiotherapy (dose: 35Gy) two years ago, complaining of reduced visual acuity of the right eye for about four months. During the ophthalmologic evaluation, he had an exam suggestive of radiation retinopathy. Macular thickness at the optical coherence tomography was 480 µm. Patient was referred to intravitreal injection (0.05 ml) of bevacizumab (Avastin®) in the right eye, showing reduction of macular edema and mild improvement of visual acuity. In this case, the treatment of radiation retinopathy with intravitreal injection of bevacizumab (Avastin®) was relatively useful, with mild improvement of visual acuity due to the regression of macular edema.

O objetivo deste relato é correlacionar achados à tomografia de coerência óptica e prognóstico visual de pacientes com commotio retina e de gravidades diferentes. Dois pacientes do sexo masculino, de 20 e 23 anos com baixa visual unilateral após trauma ocular contuso atribuível a edema de Berlin foram avaliados pela retinografia e tomografia de coerência óptica. A acuidade visual no olho afetado era de 20/25 no primeiro paciente e conta dedos a 2 metros no segundo. O exame oftalmológico revelou uveíte traumática e, na fundoscopia, evidenciaram-se opacificação retiniana moderada no primeiro caso e grave no segundo. A tomografia de coerência óptica confirmou discreta diminuição da depressão foveal no primeiro caso e desorganização das camadas retinianas no segundo. Houve resolução anatômica e funcional completa a tomografia de coerência óptica no primeiro paciente, enquanto o segundo evoluiu com baixa visual permanente e desorganização da arquitetura retiniana. A tomografia de coerência óptica é um exame complementar útil na avaliação do trauma retiniano, ajudando a entender sua fisiopatologia e predizer prognóstico a partir da análise anatômica da região acometida.The purpose of this case report was to correlate optical coherence tomography findings and visual outcomes of patients with different degrees of commotio retinae. A 20-year-old male and a 23-year-old male that presented with decreased vision due to Berlin's edema after blunt ocular trauma were evaluated by optical coherence tomography and retinography. The visual acuity in the affected eye was 20/25 in the first patient and counting fingers in the second one. The ophthalmic examination showed traumatic uveitis and fundoscopy revealed mild retinal opacification in the first case and severe opacification in the latter. The optical coherence tomography confirmed the reduction of foveal depression on the first case and the disarrangement of all retinal layers on the second. There has been complete functional and anatomical resolution by optical coherence tomography in the first patient, while the second evolved to permanent visual loss. Optical coherence tomography is a useful method in the evaluation of retinal trauma, helping to understand its physiopathology and to predict its prognosis through the anatomical analysis of the affected region.

A ceratopigmentação, ou tatuagem do estroma corneano, é método utilizado para tratar cosmeticamente lesões corneanas como leucoma e coloboma traumático de íris. Pode-se realizar a aplicação do pigmento pela micropunção estromal ou associada à ceratectomia ou ceratoplastia lamelar. Demonstramos um caso de tatuagem estromal corneana associada a ceratoplastia lamelar em paciente com leucoma corneano extenso, com irregularidade de superfície, que não tolerava lentes de contato e sem prognóstico visual. Houve também correção do estrabismo divergente no mesmo tempo cirúrgico. Após um ano de seguimento, o pigmento mantém-se impregnado no estroma corneano, proporcionando bom aspecto estético. Em conclusão, a técnica da tatuagem associada ao transplante lamelar da córnea constitui bom método para tratamento cosmético do leucoma central com irregularidades da superfície corneana.Stromal cornea tattooing has been used for esthetic treatment of corneal scars as leukoma and iris coloboma. The coloring agent can be applied through stromal micropuncture or associated with lamellar keratectomy or keratoplasty. We report a case of stromal tattooing associated with lamellar keratectomy in a patient's eye that had extensive leukoma and irregular corneal surface. Visual acuity in the operated eye was no light perception and the patient did not tolerate the use of cosmetic contact lens. Correction of divergent strabismus was also performed during the surgery. After one year follow-up, the ink is still impregnated in the corneal stroma, giving a good esthetic aspect. In conclusion, the technique of corneal tattooing associated with lamellar keratectomy is a good method for the cosmetic treatment of central leukoma with corneal irregularity.

Os fibro-histiocitomas representam 1% das massas orbitárias e podem ser histologicamente classificados em benignos, localmente agressivos e malignos - os últimos representando aproximadamente 11% do total. Nesse estudo, os autores descrevem um caso raro de fibro-histiocitoma maligno de pálpebra, ressaltando as características clínicas e histopatológicas desta neoplasia.Fibrous histiocytomas represent 1% of all orbital masses and may be classified into benign, locally aggressive and malignant - these representing nearly 11% of the total. In this study, the authors describe an unusual case of malignant fibrous histiocytoma of the eyelid, discussing clinical and histological features of this neoplasm.

Paciente de 22 anos, sexo masculino, notou mancha central no olho esquerdo durante um episódio da cefaléia de forte intensidade há aproximadamente 6 meses. Referia diagnóstico de enxaqueca com aura, apresentando crises recorrentes desde os 15 anos de idade. O exame oftalmológico revelou perda da camada de fibras nervosas da retina na região do feixe papilomacular do olho esquerdo. A tomografia de coerência óptica demonstrou importante redução da espessura macular, acompanhada de redução localizada da camada de fibras nervosas da retina no setor temporal do disco óptico no olho esquerdo. Avaliação sistêmica, exames de imagem e avaliação cardiovascular nada revelaram e a causa da perda da perda visual foi atribuída a possível enxaqueca retiniana, embora um evento isquêmico de outra natureza não pudesse ser descartado. Nosso caso é interessante porque demonstra que a tomografia de coerência óptica pode ser útil na detecção de perda neural localizada não apenas pela medida da camada de fibras nervosas da retina, mas também pela avaliação da espessura macular.A 22-year-old man developed a central scotoma in the left eye during an episode of severe headache 6 months previously. He had a diagnosis of migraine headaches with aura since age 15. Ophthalmic examination showed retinal nerve fiber layer thickness reduction on the papilomacular bundle in the left eye and unremarkable in the right. Optical coherence tomography documented severe macular thickness reduction and a mild and localized retinal nerve fiber layer loss on the temporal side of the optic disc in the left eye. Systemic investigation, neuroimaging and cardiovascular studies were non-revealing. The cause of his visual loss was possibly related to retinal migraine, although an ischemic episode of another origin could not be excluded. Our case is interesting because it draws attention to the fact that optical coherence tomography may be able to identify localized neural loss not only by measuring peripapillary retinal nerve fiber layer but also by means of macular thickness measurements.

As autoras relatam dois casos de discromatopsia hereditária e discutem a eficiência dos testes cromáticos no diagnóstico de uma discromatopsia. Os pacientes foram reprovados em diferentes concursos públicos federais por apresentarem diagnóstico de discromatopsia hereditária pelo teste de Ishihara. Submeteram-se a exame oftalmológico, com resultados dentro da normalidade. Procuraram novo parecer para melhor caracterização da sua discromatopsia. Não havia sintomas relacionados à deficiência. Os testes Panel D15 simples, D28 de Roth mostraram-se normais e o D15 dessaturado confirmou deutanomalia no caso 1. No segundo caso os testes de comparação mostraram alterações sem significado cromático. O diagnóstico de uma discromatopsia é muito complexo. As autoras destacam a importância de realização de testes de visão cromática complementares ao teste de Ishihara para diagnóstico de uma discromatopsia.The authors describe two cases of hereditary dyschromatopsia and discuss the efficiency of the color vision tests. The patients were disapproved in different federal public examinations because Ishihara's test diagnosed hereditary dyschromatopsia. Ophthalmological evaluation was normal. No symptoms related to dyschromatopsia were presented. Panels D15 and Roth D 28 were normal. Desaturated D 15 showed deuteranomaly in case one. In the second case the comparative color vision tests showed nonspecific disorder. The diagnosis of dyschromatopsia is complex. The authors recommend comparative color vision tests to complement the Ishihara test for a better understanding of the color deficiency.

Traumatic macular hole is a disease whose pathogenesis is not fully understood and the best treatment guideline is controversial. We report 2 cases of traumatic macular hole with different treatment approaches. In the first case, a 9-year-old boy presented with a traumatic macular hole secondary to blunt ocular trauma with a stone, and initial vision of 20/300. He underwent surgical repair and his final vision was 20/70 with hole closure after a 1 year follow-up. In the second case, a 20-year-old woman suffered a penetrating bullet wound on the left side of her forehead. The injury caused optic nerve head avulsion in the left eye with loss of light perception. The right eye had a traumatic macular hole and signs suggestive of sclopetaria chorioretinitis, with 20/60 vision. This case was initially observed and vision improved to 20/30 with reduction of the hole diameter. Vision and hole diameter remained stable after 8 months.O buraco macular traumático é doença cuja patogênese não é totalmente esclarecida e a melhor conduta terapêutica ainda é controversa. Relatamos 2 casos de buraco macular traumático para os quais adotamos condutas diferentes. No primeiro caso, um menino de 9 anos apresentou buraco macular traumático secundário a trauma ocular contuso com uma pedra, com visão inicial de 20/300. Foi submetido a tratamento cirúrgico e obteve visão final igual a 20/70 com buraco fechado após 1 ano de seguimento. No segundo caso, mulher de 20 anos sofreu traumatismo penetrante por projétil de arma de fogo na fronte, do lado esquerdo. O trauma causou avulsão do nervo óptico no olho esquerdo com perda de percepção luminososa neste olho. No olho direito apresentou buraco macular traumático e sinais sugestivos de coriorretinite esclopetária, com acuidade visual igual a 20/60. O caso foi inicialmente observado e a visão melhorou para 20/30 com diminuição do diâmetro do buraco. A visão e o diâmetro do buraco mantiveram-se estáveis por 8 meses.

X-linked juvenile retinoschisis (XLRS) is a recessively inherited vitreoretinal degeneration characterized by macular pathology and splitting of the neuroretinal layers that is associated with alterations in the XLRS1 gene. There have been no therapeutic interventions known to be effective for patients with X-linked juvenile retinoschisis, but some studies are trying to determine the importance of dorzolamide for the treatment of foveal lesions in this disease. The authors, using optical coherence tomography, describe findings in a patient with X-linked juvenile retinoschisis, before and after a topical use of dorzolamide. Besides the improvement in his visual acuity, further studies are required to elucidate the real prevalence of nonresponse to dorzolamide and the frequency with which there may be a recurrence of foveal cystic changes during continued treatment.A retinosquise juvenil ligada ao X (XLRS) é uma degeneração vítreo-retiniana hereditária e recessiva caracterizada por lesão macular e delaminação das camadas de fibras nervosas da retina, que está associada com alterações no gene XLRS1. Nenhuma intervenção terapêutica tem se mostrado efetiva em pacientes com retinosquise juvenil ligada ao X, mas alguns estudos estão tentando determinar a importância da dorzola mida no tratamento das lesões foveais desta doença. Os autores, usando a tomografia de coerência óptica, descrevem os achados em um paciente com retinosquise juvenil ligada ao X, antes e após o uso de dorzolamida tópica. Apesar da melhora na acuidade visual do paciente estudado, outros estudos são necessários para uma melhor elucidação da real prevalência dos pacientes não responsivos ao tratamento com dorzolamida, bem com a recorrência das lesões císticas a longo prazo.

O implante de lentes intra-oculares (LIOs) multifocais é um grande avanço na reabilitação de pacientes permitindo boa acuidade visual sem correção óptica para longe e perto. Contudo pacientes com alguns graus de astigmatismo corneano tem indicação limitada. Este artigo relata três pacientes que foram submetidas a facoemulsificação com implante de lentes intra-oculares multifocais (AcrySof® Restor®, Alcon Labs) após incisão relaxante limbar (IRL) no olho dominante e incisão no eixo mais curvo da topografia no olho contralateral. Não foi encontrado nenhuma análise relacionada a dominância ocular e relaxante limbar na literatura. Acredita-se que essa associação pode ampliar as indicações de implante das lentes intra-oculares multifocais em pacientes com astigmatismo corneano significativo, devendo-se confirmar esta possibilidade por meio de futuros estudos.The use of multifocal intraocular lenses (IOLs) is a great advance in the treatment of patients with cataract allowing near and distance uncorrected visual acuity recovery. However patients with some degrees of corneal astigmatism have some limitations. We present 3 cases that were indicated for phacoemulsitfication and multifocal intraocular lens implant (AcrySof® Restor®, Alcon Labs) after limbal relaxing incision (LRI) in the dominant eye and axis topographic incision in the fellow eye. There was no similar case report in the literature. It is believed that this association can extend the indications for multifocal intraocular lens implants in patients with significant corneal astigmatism and this possibility should be confirmed by future studies.

OBJETIVOS:Avaliar as indicações, os resultados e as complicações observadas nos pacientes submetidos ao implante de peso de ouro para correção do lagoftalmo paralítico. MÉTODOS: Vinte prontuários de pacientes com lagoftalmo secundário à paralisia facial de diversas etiologias, que foram submetidos à colocação do implante de ouro na pálpebra superior do lado afetado, foram examinados retrospectivamente. RESULTADOS: A causa mais freqüente de lagoftalmo paralítico foi pós-cirurgia de neurinoma do acústico (40%). Complicações precoces e tardias ocorreram em 40% dos implantes colocados. Quatro pacientes (20%) apresentaram reação inflamatória local nos primeiros meses de pós-operatório. Dois pacientes (10%) apresentaram afinamento da pele e do músculo orbicular sobre o peso de ouro, após 4 e 7 anos do implante, respectivamente. Um paciente (5%) apresentou deslocamento do peso de ouro após 3 anos de sua colocação e outro paciente (5%), extrusão do peso de ouro tardiamente, após 10 anos do implante. CONCLUSÕES: Nesta série, foi alto o índice de complicações com o implante de ouro (40%). As complicações foram divididas em precoces, possivelmente relacionadas à impureza do material, e complicações tardias, devido à evolução do quadro da paralisia facial que apresenta diminuição do tônus muscular.PURPOSE: To evaluate the indications, the results and the complications seen in the patients submitted to gold weight implantation to correct paralytic lagophthalmos. METHODS: Charts of 20 patients with lagophthalmos secondary to facial nerve palsy of diverse etiologies, which were submitted to gold weight implantation in the upper eyelid of the affected side, were retrospectively examined. RESULTS: The most frequent cause of paralytic lagophthalmos was acoustic neurinoma after surgery (40%). Early and late complications occurred in 40% of the implants. Four patients (20%) presented a local inflammatory reaction in the first 3 months after surgery. Two patients (10%) presented skin and orbicular muscle thinness over the gold weight 4 and 7 years after the implant, respectively. One patient (5%) presented gold weight displacement after 3 years and another patient (5%) had late gold weight extrusion after 10 years. CONCLUSIONS: In this series, the complication rate of gold weight implantation was high (40%). The complications were divided into early, possibly related to the material impurity and the late, due to the evolution of the facial nerve palsy that presented a decrease in muscle tonus.

Background Entamoeba histolytica, E. dispar and E. moshkovskii are the most frequent species described in human infection where E. histolytica is the only true pathogen. The epidemiology of this infection is complex due to the absence of a routine exam that allows a correct discrimination of the Entamoeba species complex. Therefore, molecular methods appear as the unique epidemiological tool to accomplish the species discrimination. Herein, we conducted a cross-sectional study to determine the frequency of Entamoeba species infections in a group of asymptomatic individuals from a rural area in central Colombia. Methodology/Principal Findings A total of 181 fecal samples from asymptomatic children under 16 years old from the hamlet La Vírgen, Cundinamarca (Colombia) that voluntarily accepted to participate in the study were collected. The fecal samples were examined by light microscopy and DNA-extracted, subsequently submitted to molecular discrimination of E. dispar/E. histolytica/E. moshkovskii infection based on a multiplex PCR assay targeting the 18S rRNA fragment. To confirm the species description, twenty samples were randomly submitted to DNA sequencing of the aforementioned fragment. By direct microscopic examination, frequency of the complex E. histolytica/E. dispar/E. moshkovskii was 18.8%(34/181). PCR showed a frequency of 49.1% (89/181), discriminated as 23.2%(42/181) that were positive for E. dispar, 25.4% (46/181) for E. moshkovskii and 0.55% (1/ 181) for E. histolytica. Also, mixed infections were detected between E. dispar and E. moshkovskii at 4.42% (8/181) of the samples. Molecular barcoding confirmed the diagnosis depicted by the multiplex PCR assay. Conclusions/Significance This is the first description of E. moshkovskii in Colombia and the second report in South- America to our knowledge. Our results suggest the need to unravel the true epidemiology of Entamoeba infections around the world, including the real pathogenic role that E. moshkovskii may have. Familial amyloidosis of the Finnish type (FAF) is an autosomal dominant form of systemic amyloidosis showing marked geographic clustering in Finland. The disease is caused by a point mutation, 654G-A, in the gelsolin gene. The Danish-subtype of FAF has been previously described in three families, the patients present clinical findings similar to FAF, and the mutation 654G-T in the gelsolin gene. Three members from two generations of the same family, with familial amyloidosis, were screened for mutations in the GSN gene. Genomic DNA was extracted from peripheral blood lymphocytes and the polymerase chain reaction (PCR) was carried out under standard conditions, using appropriate primers. Sequence analysis showed the presence of a G to T transition at nucleotide 654 of the gelsolin gene. This is the first report of gelsolin-related familial amyloidosis in a Brazilian family, and the result is particularly significant as this pedigree presents an unusual mutation, described previously in three families, with no known Finnish ancestors (Danish type).

Objective To analyze the variations in the angle basal sphenoid skulls of adult humans and their relationship to sex, age, ethnicity and cranial index. Methods The angles were measured in 160 skulls belonging to the Museum of the Universidade Federal de São Paulo Department of Morphology. We use two flexible rules and a goniometer, having as reference points for the first rule the posterior end of the ethmoidal crest and dorsum of the sella turcica, and for the second rule the anterior margin of the foramen magnum and clivus, measuring the angle at the intersection of two. Results The average angle was 115.41°, with no statistical correlation between the value of the angle and sex or age. A statistical correlation was noted between the value of the angle and ethnicity, and between the angle and the horizontal cranial index. Conclusions The distribution of the angle basal sphenoid was the same in sex, and there was correlation between the angle and ethnicity, being the proportion of non-white individuals with an angle >125° significantly higher than that of whites with an angle >125°. There was correlation between the angle and the cranial index, because skulls with higher cranial index tend to have higher basiesfenoidal angle too. Objetivo Analisar as variações do ângulo basiesfenoidal em crânios de humanos adultos e sua relação com sexo, idade, etnia e índice crânico horizontal. Métodos Os ângulos foram medidos em 160 crânios pertencentes ao Museu do Departamento de Morfologia da Universidade Federal de São Paulo. Utilizamos duas réguas milimetradas flexíveis e um goniômetro, tendo como pontos de referência para a primeira régua a extremidade posterior da crista etmoidal e o dorso da sela turca, e, para a segunda régua, a margem anterior do forame magno e o clivo, medindo o ângulo na intersecção das duas. Resultados A média dos ângulos foi de 115,41°, não havendo correlação estatística entre o valor do ângulo e o sexo ou a idade. Houve correlação estatística entre o valor do ângulo e a etnia, e entre o ângulo e o índice crânico horizontal. Conclusão A distribuição do ângulo basiesfenoidal foi a mesma entre os sexos, havendo correlação entre o ângulo e a etnia, e sendo a proporção de indivíduos não brancos com ângulo >125° significativamente maior que a de indivíduos brancos com ângulo >125°. Houve correlação entre o ângulo e o índice crânico horizontal, pois crânios com maior índice crânico horizontal tenderam a um ângulo basiesfenoidal maior.

Relatamos um caso inédito na literatura brasileira de fibrohistiocitoma conjuntival descoberto durante projeto comunitário de larga escala realizado pela equipe do Centro de Referência em Oftalmologia do Hospital das Clínicas da Universidade Federal de Goiás na região amazônica. Paciente do sexo feminino, leucoderma, de 38 anos de idade, apresentava lesão vascularizada no olho direito no canto medial da conjuntiva bulbar e carúncula com extensão para o limbo e invadindo cerca de 2 mm da córnea medial com configuração semelhante a um pterígio. O exame histopatológico revelou lesão constituída por células fusiformes, com arranjo estoriforme e células histiocitárias gigantes. A imunohistoquímica revelou forte positividade para vimentina e as células neoplásicas foram negativas para AE1/ AE3, HMB 45, proteína S 100. O diagnóstico histopatológico foi de fibrohistiocitoma. A evolução poderá demonstrar o crescimento tumoral e a possibilidade de lesão maligna. An inedited case in the Brazilian literature of conjunctival fibrous histiocytoma discovered during a wide scale community project accomplished by the team of the Center of Reference in Ophthalmology of the Hospital das Clínicas da Universidade Federal de Goiás in the Amazon region is reported. A 38 year-old caucasian woman presented with a vascularized lesion on the medial portion of the bulbar conjunctiva and caruncular region of the right eye with extension for the limbus and invading about 2 mm of the medial cornea resembling a pterygium. Pathologic findings revealed that the lesion was constituted by spindle-shaped cells, with storiform arrangement, and large histocytelike cells. The immunohistochemistry revealed strong positivity for vimentin and the neoplastic cells were negative for AE1/AE3, HMB 45, S 100 protein. The histopathologic diagnosis was fibrous histiocytoma. The evolution demonstrated the tumoral growth and the possibility of a malignant lesion.

Cistos vítreos são achados raros do segmento posterior ocular. Podem ocorrer em olhos com doenças oculares preexistentes ou em olhos aparentemente normais. Este estudo é um relato de caso de um paciente com retinose pigmentária e cisto vítreo, e descreve sua apresentação clínica e ultrassonográfica.Vitreous cyst is a rare condition of the posterior segment of the eye. It can occur in eyes with coexistent ocular diseases or in eyes that are otherwise normal. This study reports a case of vitreous cyst in a patient with retinitis pigmentosa and presents its clinical and ultrasonographic features.

Tay-Sachs disease is an autosomal recessive disorder of sphingolipid metabolism, caused by enzime hexosaminidase A deficiency that leads to an accumulation of GM2 in neurocytes which results in progressive loss of neurological function. O acúmulo de lipídios em células ganglionais da retina leva a uma aparência de mácula em cereja, característica do fundo de olho de pessoas acometidas. "Mácula em cereja" também pode ser vista em outras doenças neurometabólicas e em oclusão da artéria central da retina. Este trabalho relata o caso de um paciente com doença de Tay-Sachs em uma família com história de quatro óbitos por causas semelhantes sem diagnóstico.

Familial amyloidosis of the Finnish type (FAF) is an autosomal dominant form of systemic amyloidosis showing marked geographic clustering in Finland. The disease is caused by a point mutation, 654G-A, in the gelsolin gene. The Danish-subtype of FAF has been previously described in three families, the patients present clinical findings similar to FAF, and the mutation 654G-T in the gelsolin gene. Three members from two generations of the same family, with familial amyloidosis, were screened for mutations in the GSN gene. Genomic DNA was extracted from peripheral blood lymphocytes and the polymerase chain reaction (PCR) was carried out under standard conditions, using appropriate primers. Sequence analysis showed the presence of a G to T transition at nucleotide 654 of the gelsolin gene. Amiloidose familiar do tipo finlandes (FAF) é uma forma de amiloidose sistêmica autossômica dominante com grande concentração geográfica na Finlândia. É causada por uma mutação, 654G-A, no gene gelsolin. O subtipo dinamarquês da FAF foi previamente descrito em três famílias, com achados clínicos similares mas com a mutação 654G-T no gene gelsolin. Três membros de duas gerações da mesma família, com diagnóstico de amiloidose familiar, foram submetidos a screening de mutações no gene gelsolin. O DNA genômico foi extraído de linfócitos do sangue periférico, sendo realizada reação em cadeia de polimerase (PCR) em condições padronizadas. A análise do sequenciamento revelou uma transição de G para T no nucleotidio 654 do gene gelsolin. Este é o primeiro relato de uma amiloidose familiar relacionada ao gene gelsolin em uma família brasileira, que apresenta uma forma rara de mutação, descrita previamente em três famílias, sem ancestrais finlandeses (tipo dinamarquês).

OBJETIVO: Relatar tratamento de retinopatia por radiação em paciente submetido à radioterapia por linfoma em órbita direita com injeção intravítrea de bevacizumab (Avastin®). Paciente de 55 anos, diabético, com diagnóstico de linfoma MALT orbitário há três anos, tratado com radioterapia local (dose: 35Gy) há dois anos, com queixa de redução da acuidade visual do olho direito há quatro meses. Ao exame oftalmológico, apresentava alterações sugestivas de retinopatia por radiação, bem como espessura macular à tomografia de coerência óptica de 480 µm. Paciente foi submetido à injeção intravítrea (0,05 ml) de bevacizumab (Avastin®) no olho direito, apresentando redução do edema macular, bem como melhora discreta da acuidade visual. Neste caso, o tratamento da retinopatia por radiação com injeção intravítrea de bevacizumab (Avastin®) foi relativamente útil, com melhora discreta da acuidade visual, devido à regressão do edema macular. PURPOSE: To report a case of radiation retinopathy treatment with intravitreal injection of bevacizumab (Avastin®) in a patient undergoing radiotherapy for lymphoma in the right orbit. Patient of 55 years-old male, diabetic, diagnosed with an orbital MALT lymphoma three years ago, treated with local radiotherapy (dose: 35Gy) two years ago, complaining of reduced visual acuity of the right eye for about four months. During the ophthalmologic evaluation, he had an exam suggestive of radiation retinopathy. Macular thickness at the optical coherence tomography was 480 µm. Patient was referred to intravitreal injection (0.05 ml) of bevacizumab (Avastin®) in the right eye, showing reduction of macular edema and mild improvement of visual acuity. In this case, the treatment of radiation retinopathy with intravitreal injection of bevacizumab (Avastin®) was relatively useful, with mild improvement of visual acuity due to the regression of macular edema.

A síndrome de Brown enquadra-se no grupo de estrabismos restritivos e é causada pela limitação de movimento do tendão do oblíquo superior através da tróclea. Caracteriza-se por ortotropia ou hipotropia na primária do olhar, limitação ou ausência de elevação em adução, frequente depressão do olho em adução com anisotropia em V e dução passiva positiva na elevação em adução. Denomina-se síndrome de Brown inflamatória, o distúrbio secundário, que tem como principais causas inflamações locais na órbita e doenças inflamatórias como artrite reumatóide e tenossinovite. Apresentamos o caso de um paciente de 44 anos, do sexo masculino, com queixa de diplopia binocular vertical à dextroversão recorrente. Diagnosticado como síndrome de Brown inflamatória sem etiologia definida, confirmada por ressonância nuclear magnética e com resolução espontânea. Brown syndrome fits the group of restrictive strabismus and is caused by a movement limitation of the superior oblique tendon through the trochlea. It is characterized by parallelism in the primary gaze position, limitation or absence of elevation in adduction, frequent depression of the eye in adduction with anisotropy in V-pattern and positive passive duction in the elevation in adduction. It is called inflammatory Brown syndrome, a secondary disorder, which main causes are local inflammation in the orbit and inflammatory diseases such as rheumatoid arthritis and tenosynovitis. A case of a 44 year-old patient, male, complaining of binocular vertical diplopia due to recurring dextroversion is reported. It was diagnosed as inflammatory Brown syndrome of uncertain etiology, confirmed by magnetic nuclear resonance, and with spontaneous resolution.

The authors relate an uncommon case of pyogenic granuloma restricted to tarsal conjunctiva post retinopexy with scleral buckle. Os autores relatam um caso de granuloma piogênico subconjuntival pós retinopexia com introflexão escleral. É uma complicação pós-cirúrgica incomum neste tipo de procedimento.

If the ocular media are clear, indirect binocular ophthalmoscopy allows retinal detachment and retinal tear identification and treatment under direct visualization. However, if opacities are present preventing direct fundus examination, ultrasonography becomes the most important tool for evaluating the posterior segment. In addition, ultrasonography can be useful in retinal tear treatment by guiding cryotherapy. In this article we describe a rhegmatogenous retinal detachment treatment technique applied to a patient with corneal opacity. Cryopexy and circumferential and radial buckle positioning were guided by ultrasonography, resulting in retinal attachment during the 6-month follow-up period. Quando os meios oculares são transparentes, oftalmoscopia binocular indireta permite a identificação de descolamento de retina e roturas, bem como seu tratamento sob visibilização direta. Porém, em olhos que apresentam opacidades de meios impedindo o exame oftalmoscópico, a ultrassonografia constitui o exame mais importante do segmento posterior do olho. Além disso, o tratamento de roturas retinianas também pode ser auxiliado pelo uso desse equipamento, orientando a crioterapia. Neste trabalho será apresentada técnica de tratamento de descolamento de retina regmatogênico, no qual a criopexia e o posicionamento dos "buckles" episclerais circunferencial e radial foram guiados pelo ultrassom em paciente com leucoma corneano. O tratamento resultou em aplicação retiniana durante o seguimento em seis meses.

A ceratopigmentação, ou tatuagem do estroma corneano, é método utilizado para tratar cosmeticamente lesões corneanas como leucoma e coloboma traumático de íris. Pode-se realizar a aplicação do pigmento pela micropunção estromal ou associada à ceratectomia ou ceratoplastia lamelar. Demonstramos um caso de tatuagem estromal corneana associada a ceratoplastia lamelar em paciente com leucoma corneano extenso, com irregularidade de superfície, que não tolerava lentes de contato e sem prognóstico visual. Houve também correção do estrabismo divergente no mesmo tempo cirúrgico. Após um ano de seguimento, o pigmento mantém-se impregnado no estroma corneano, proporcionando bom aspecto estético. Em conclusão, a técnica da tatuagem associada ao transplante lamelar da córnea constitui bom método para tratamento cosmético do leucoma central com irregularidades da superfície corneana. Stromal cornea tattooing has been used for esthetic treatment of corneal scars as leukoma and iris coloboma. The coloring agent can be applied through stromal micropuncture or associated with lamellar keratectomy or keratoplasty. We report a case of stromal tattooing associated with lamellar keratectomy in a patient's eye that had extensive leukoma and irregular corneal surface. Visual acuity in the operated eye was no light perception and the patient did not tolerate the use of cosmetic contact lens. Correction of divergent strabismus was also performed during the surgery. After one year follow-up, the ink is still impregnated in the corneal stroma, giving a good esthetic aspect. In conclusion, the technique of corneal tattooing associated with lamellar keratectomy is a good method for the cosmetic treatment of central leukoma with corneal irregularity.

Os fibro-histiocitomas representam 1% das massas orbitárias e podem ser histologicamente classificados em benignos, localmente agressivos e malignos - os últimos representando aproximadamente 11% do total. Nesse estudo, os autores descrevem um caso raro de fibro-histiocitoma maligno de pálpebra, ressaltando as características clínicas e histopatológicas desta neoplasia. Fibrous histiocytomas represent 1% of all orbital masses and may be classified into benign, locally aggressive and malignant - these representing nearly 11% of the total. In this study, the authors describe an unusual case of malignant fibrous histiocytoma of the eyelid, discussing clinical and histological features of this neoplasm.

Paciente de 22 anos, sexo masculino, notou mancha central no olho esquerdo durante um episódio da cefaléia de forte intensidade há aproximadamente 6 meses. Referia diagnóstico de enxaqueca com aura, apresentando crises recorrentes desde os 15 anos de idade. O exame oftalmológico revelou perda da camada de fibras nervosas da retina na região do feixe papilomacular do olho esquerdo. A tomografia de coerência óptica demonstrou importante redução da espessura macular, acompanhada de redução localizada da camada de fibras nervosas da retina no setor temporal do disco óptico no olho esquerdo. Avaliação sistêmica, exames de imagem e avaliação cardiovascular nada revelaram e a causa da perda da perda visual foi atribuída a possível enxaqueca retiniana, embora um evento isquêmico de outra natureza não pudesse ser descartado. Nosso caso é interessante porque demonstra que a tomografia de coerência óptica pode ser útil na detecção de perda neural localizada não apenas pela medida da camada de fibras nervosas da retina, mas também pela avaliação da espessura macular. A 22-year-old man developed a central scotoma in the left eye during an episode of severe headache 6 months previously. He had a diagnosis of migraine headaches with aura since age 15. Ophthalmic examination showed retinal nerve fiber layer thickness reduction on the papilomacular bundle in the left eye and unremarkable in the right. Optical coherence tomography documented severe macular thickness reduction and a mild and localized retinal nerve fiber layer loss on the temporal side of the optic disc in the left eye. Systemic investigation, neuroimaging and cardiovascular studies were non-revealing. The cause of his visual loss was possibly related to retinal migraine, although an ischemic episode of another origin could not be excluded. Our case is interesting because it draws attention to the fact that optical coherence tomography may be able to identify localized neural loss not only by measuring peripapillary retinal nerve fiber layer but also by means of macular thickness measurements.

O manuseio anestésico de pacientes com doença pulmonar obstrutiva crônica grave é extensamente discutido devido ao elevado número de complicações destes pacientes, quando submetidos à procedimentos cirúrgicos de médio e grande porte. O objetivo deste estudo foi relatar um caso de paciente idoso portador de doença pulmonar obstrutiva crônica grave e coronariopatia, submetido a artroplastia de quadril sob anestesia espinhal e suporte intra-operatório de ventilação mecânica não-invasiva - bilevel positive airway pressure.. Paciente do sexo masculino, 81 anos, doença pulmonar obstrutiva crônica grave (GOLD 4), submetido à artroplastia de quadril devido à fratura de fêmur sob anestesia espinhal e suporte intra-operatório de ventilação mecânica não invasiva - bilevel positive airway pressure, sob parâmetros de pressão expiratória de 7 cmH2O, pressão inspiratória 15 cmH2O e fluxo de O2 de 3 L/min. Apresentou um episódio de broncoespasmo, revertido farmacologicamente, sem apresentar complicações no pós-operatório. A combinação de técnica anestésica regional com ventilação mecânica não-invasiva é de fácil aplicação e pode ser útil no intraoperatório destes pacientes de alto risco anestésico. A interação entre a equipe clínica, cirúrgica e de anestesia propiciou benefícios e reduz a morbimortalidade associada a procedimentos de grande porte em pacientes graves. Anesthetic management of patients with severe chronic obstructive pulmonary disease is extensively discussed, due to the high rates of complications in this subtype of patients submitted to medium and high complexity surgical procedures. The objective of this study is to report use of noninvasive positive pressure mechanical ventilation - bilevel positive airway pressure - and spinal anesthesia in a patient with severe chronic obstructive pulmonary disease during total hip arthroplasty. An 81 year old, male patient with severe chronic obstructive pulmonary disease (GOLD 4) was submitted to total hip arthroplasty due to a femoral bone fracture under spinal anestesia and noninvasive positive pressure mechanical ventilation-bilevel positive airway pressure with expiratory pressure of 7 cmH2O, inspiratory pressure of 15 cmH2O and O2 flow of 3 L/min. During the procedure, the patient had one episode of bronchospasm that was promptly reverted pharmacologically with no complications in the postoperative period. The combination of less invasive anesthetic and ventilation techniques is easy to apply and may be useful in the perioperative management of patients with high anesthetic morbidity. Interaction between clinical, surgical and anesthetic teams for these cases is very important to reduce the mortality associated with extensive procedures in severe patients.

Traumatic macular hole is a disease whose pathogenesis is not fully understood and the best treatment guideline is controversial. We report 2 cases of traumatic macular hole with different treatment approaches. In the first case, a 9-year-old boy presented with a traumatic macular hole secondary to blunt ocular trauma with a stone, and initial vision of 20/300. He underwent surgical repair and his final vision was 20/70 with hole closure after a 1 year follow-up. In the second case, a 20-year-old woman suffered a penetrating bullet wound on the left side of her forehead. The injury caused optic nerve head avulsion in the left eye with loss of light perception. The right eye had a traumatic macular hole and signs suggestive of sclopetaria chorioretinitis, with 20/60 vision. This case was initially observed and vision improved to 20/30 with reduction of the hole diameter. Vision and hole diameter remained stable after 8 months. O buraco macular traumático é doença cuja patogênese não é totalmente esclarecida e a melhor conduta terapêutica ainda é controversa. Relatamos 2 casos de buraco macular traumático para os quais adotamos condutas diferentes. No primeiro caso, um menino de 9 anos apresentou buraco macular traumático secundário a trauma ocular contuso com uma pedra, com visão inicial de 20/300. Foi submetido a tratamento cirúrgico e obteve visão final igual a 20/70 com buraco fechado após 1 ano de seguimento. No segundo caso, mulher de 20 anos sofreu traumatismo penetrante por projétil de arma de fogo na fronte, do lado esquerdo. O trauma causou avulsão do nervo óptico no olho esquerdo com perda de percepção luminososa neste olho. No olho direito apresentou buraco macular traumático e sinais sugestivos de coriorretinite esclopetária, com acuidade visual igual a 20/60. O caso foi inicialmente observado e a visão melhorou para 20/30 com diminuição do diâmetro do buraco. A visão e o diâmetro do buraco mantiveram-se estáveis por 8 meses.

As autoras relatam dois casos de discromatopsia hereditária e discutem a eficiência dos testes cromáticos no diagnóstico de uma discromatopsia. Os pacientes foram reprovados em diferentes concursos públicos federais por apresentarem diagnóstico de discromatopsia hereditária pelo teste de Ishihara. Submeteram-se a exame oftalmológico, com resultados dentro da normalidade. Procuraram novo parecer para melhor caracterização da sua discromatopsia. Não havia sintomas relacionados à deficiência. Os testes Panel D15 simples, D28 de Roth mostraram-se normais e o D15 dessaturado confirmou deutanomalia no caso 1. No segundo caso os testes de comparação mostraram alterações sem significado cromático. O diagnóstico de uma discromatopsia é muito complexo. As autoras destacam a importância de realização de testes de visão cromática complementares ao teste de Ishihara para diagnóstico de uma discromatopsia. The authors describe two cases of hereditary dyschromatopsia and discuss the efficiency of the color vision tests. The patients were disapproved in different federal public examinations because Ishihara's test diagnosed hereditary dyschromatopsia. Ophthalmological evaluation was normal. No symptoms related to dyschromatopsia were presented. Panels D15 and Roth D 28 were normal. Desaturated D 15 showed deuteranomaly in case one. In the second case the comparative color vision tests showed nonspecific disorder. The diagnosis of dyschromatopsia is complex. The authors recommend comparative color vision tests to complement the Ishihara test for a better understanding of the color deficiency.

X-linked juvenile retinoschisis (XLRS) is a recessively inherited vitreoretinal degeneration characterized by macular pathology and splitting of the neuroretinal layers that is associated with alterations in the XLRS1 gene. There have been no therapeutic interventions known to be effective for patients with X-linked juvenile retinoschisis, but some studies are trying to determine the importance of dorzolamide for the treatment of foveal lesions in this disease. The authors, using optical coherence tomography, describe findings in a patient with X-linked juvenile retinoschisis, before and after a topical use of dorzolamide. Besides the improvement in his visual acuity, further studies are required to elucidate the real prevalence of nonresponse to dorzolamide and the frequency with which there may be a recurrence of foveal cystic changes during continued treatment. A retinosquise juvenil ligada ao X (XLRS) é uma degeneração vítreo-retiniana hereditária e recessiva caracterizada por lesão macular e delaminação das camadas de fibras nervosas da retina, que está associada com alterações no gene XLRS1. Nenhuma intervenção terapêutica tem se mostrado efetiva em pacientes com retinosquise juvenil ligada ao X, mas alguns estudos estão tentando determinar a importância da dorzola mida no tratamento das lesões foveais desta doença. Os autores, usando a tomografia de coerência óptica, descrevem os achados em um paciente com retinosquise juvenil ligada ao X, antes e após o uso de dorzolamida tópica. Apesar da melhora na acuidade visual do paciente estudado, outros estudos são necessários para uma melhor elucidação da real prevalência dos pacientes não responsivos ao tratamento com dorzolamida, bem com a recorrência das lesões císticas a longo prazo.

O implante de lentes intra-oculares (LIOs) multifocais é um grande avanço na reabilitação de pacientes permitindo boa acuidade visual sem correção óptica para longe e perto. Contudo pacientes com alguns graus de astigmatismo corneano tem indicação limitada. Este artigo relata três pacientes que foram submetidas a facoemulsificação com implante de lentes intra-oculares multifocais (AcrySof® Restor®, Alcon Labs) após incisão relaxante limbar (IRL) no olho dominante e incisão no eixo mais curvo da topografia no olho contralateral. Não foi encontrado nenhuma análise relacionada a dominância ocular e relaxante limbar na literatura. Acredita-se que essa associação pode ampliar as indicações de implante das lentes intra-oculares multifocais em pacientes com astigmatismo corneano significativo, devendo-se confirmar esta possibilidade por meio de futuros estudos. The use of multifocal intraocular lenses (IOLs) is a great advance in the treatment of patients with cataract allowing near and distance uncorrected visual acuity recovery. However patients with some degrees of corneal astigmatism have some limitations. We present 3 cases that were indicated for phacoemulsitfication and multifocal intraocular lens implant (AcrySof® Restor®, Alcon Labs) after limbal relaxing incision (LRI) in the dominant eye and axis topographic incision in the fellow eye. There was no similar case report in the literature. It is believed that this association can extend the indications for multifocal intraocular lens implants in patients with significant corneal astigmatism and this possibility should be confirmed by future studies.

OBJETIVOS:Avaliar as indicações, os resultados e as complicações observadas nos pacientes submetidos ao implante de peso de ouro para correção do lagoftalmo paralítico. MÉTODOS: Vinte prontuários de pacientes com lagoftalmo secundário à paralisia facial de diversas etiologias, que foram submetidos à colocação do implante de ouro na pálpebra superior do lado afetado, foram examinados retrospectivamente. RESULTADOS: A causa mais freqüente de lagoftalmo paralítico foi pós-cirurgia de neurinoma do acústico (40%). Complicações precoces e tardias ocorreram em 40% dos implantes colocados. Quatro pacientes (20%) apresentaram reação inflamatória local nos primeiros meses de pós-operatório. Dois pacientes (10%) apresentaram afinamento da pele e do músculo orbicular sobre o peso de ouro, após 4 e 7 anos do implante, respectivamente. Um paciente (5%) apresentou deslocamento do peso de ouro após 3 anos de sua colocação e outro paciente (5%), extrusão do peso de ouro tardiamente, após 10 anos do implante. CONCLUSÕES: Nesta série, foi alto o índice de complicações com o implante de ouro (40%). As complicações foram divididas em precoces, possivelmente relacionadas à impureza do material, e complicações tardias, devido à evolução do quadro da paralisia facial que apresenta diminuição do tônus muscular. PURPOSE: To evaluate the indications, the results and the complications seen in the patients submitted to gold weight implantation to correct paralytic lagophthalmos. METHODS: Charts of 20 patients with lagophthalmos secondary to facial nerve palsy of diverse etiologies, which were submitted to gold weight implantation in the upper eyelid of the affected side, were retrospectively examined. RESULTS: The most frequent cause of paralytic lagophthalmos was acoustic neurinoma after surgery (40%). Early and late complications occurred in 40% of the implants. Four patients (20%) presented a local inflammatory reaction in the first 3 months after surgery. Two patients (10%) presented skin and orbicular muscle thinness over the gold weight 4 and 7 years after the implant, respectively. One patient (5%) presented gold weight displacement after 3 years and another patient (5%) had late gold weight extrusion after 10 years. CONCLUSIONS: In this series, the complication rate of gold weight implantation was high (40%). The complications were divided into early, possibly related to the material impurity and the late, due to the evolution of the facial nerve palsy that presented a decrease in muscle tonus.

The detection of significant levels of tryptase in human seminal plasma and follicular fluid and of tryptase-positive mast cells (MCs) in the wall of human Fallopian tubes lead us to hypothesize that tryptase may exert regulatory actions on human spermatozoa. Immunoelectronmicroscopy revealed proteinase-activated receptor 2 (PAR-2) in the membranes of the acrosomal region and midpiece of human spermatozoa. These PAR-2 were functional, as exposure of spermatozoa from healthy men (n = 12) with regular standard semen parameters to human recombinant tryptase significantly decreased motility in a dose- and time-dependent fashion. Motile spermatozoa (WHO a + b) were significantly decreased within 10 min of incubation with 1.000 ng/ml tryptase (P = 0.045). After 30 and 60 min, significant reduction of motility was also observed in the presence of lower tryptase concentrations (100 ng/ml, P = 0.037; 10 ng/ml, P = 0.046). The inhibitory effects of tryptase progressed throughout an observation period of 180 min. Furthermore, tryptase effects were reversible after washing procedures and could be inhibited by pretreatment with anti-tryptase antibody or anti-PAR-2 antiserum. Fil: Weidinger, S.. A síndrome de Brown enquadra-se no grupo de estrabismos restritivos e é causada pela limitação de movimento do tendão do oblíquo superior através da tróclea. Caracteriza-se por ortotropia ou hipotropia na primária do olhar, limitação ou ausência de elevação em adução, frequente depressão do olho em adução com anisotropia em V e dução passiva positiva na elevação em adução. Denomina-se síndrome de Brown inflamatória, o distúrbio secundário, que tem como principais causas inflamações locais na órbita e doenças inflamatórias como artrite reumatóide e tenossinovite. Apresentamos o caso de um paciente de 44 anos, do sexo masculino, com queixa de diplopia binocular vertical à dextroversão recorrente. Diagnosticado como síndrome de Brown inflamatória sem etiologia definida, confirmada por ressonância nuclear magnética e com resolução espontânea.Brown syndrome fits the group of restrictive strabismus and is caused by a movement limitation of the superior oblique tendon through the trochlea. It is characterized by parallelism in the primary gaze position, limitation or absence of elevation in adduction, frequent depression of the eye in adduction with anisotropy in V-pattern and positive passive duction in the elevation in adduction. It is called inflammatory Brown syndrome, a secondary disorder, which main causes are local inflammation in the orbit and inflammatory diseases such as rheumatoid arthritis and tenosynovitis. A case of a 44 year-old patient, male, complaining of binocular vertical diplopia due to recurring dextroversion is reported. It was diagnosed as inflammatory Brown syndrome of uncertain etiology, confirmed by magnetic nuclear resonance, and with spontaneous resolution.
