We investigated inpatients with and without Type 2 diabetes mellitus, aged over 60 yr, to compare their vitamin D status and calcium homeostatic parameters.We studied 140 patients consecutively admitted to our Internal Medicine Unit during the year 2010 (61 from November to April, 79 from May to October). The sample encompassed 70 patients with and 70 without diabetes. At admission we measured serum calcium (Ca), phosphate (P), sodium (Na), potassium (K), creatinine (Cr), alkaline phosphatase total activity (AP), albumin adjusted serum calcium (Caalb adj), 25 hydroxy-vitamin D (25OHD), PTH, and 24-h urinary Na/Cr (uNa/Cr), K/Cr (uK/Cr), Ca/Cr (uCa/Cr), P/Cr (uP/Cr) ratios, and calcium excretion (Ca ex).25OHD levels of patients with and without diabetes did not significantly differ. In patients without diabetes recruited from November to April, 25OHD levels were significantly lower than those from May to October, whilst patients with diabetes did not show a significant seasonal variation. PTH had opposite non-significant seasonal variations, and negatively correlated with 25OHD in both groups of patients. This correlation was lost after adjusting for age and body mass index in patients with diabetes. These inpatients had higher serum P and lower uP/Cr, according to lower PTH. Their serum glucose negatively correlated with uCa/Cr and Ca ex, contrary to inpatients with other diseases. Instead, uCa/Cr and Ca ex correlated with uNa/Cr only in patients without diabetes.Inpatients with diabetes did differ from those with other disorders for vitamin D status and calcium-phosphate homeostatic mechanism.

The paper reviews the laws that regulate the clinical scientific research with ionizing radiations in Italy. Although recent (all introduced after 1990), the laws are a maze of rules sometimes contradictory and unclear, with frequent cross-references which make them difficult to disentangle. The aim of the paper is to provide the researcher with a technical and legal guide to find his/her way in the labyrinth of rules, and to constitute a basis for a possible future rationalization of the regulations.The contents of the article n. 108 of the law by decree 230/1995 and of the Minister's Decree 21/11/1997 together with the ICRP 62 (introduced in Italy by the Minister's Decree 21/11/1997) are extensively reviewed. The authors stress the fact that these laws apply only to prospective, but not to retrospective studies. The procedure to obtain ministerial authorization of the research project is illustrated, together with the possibility of a bureaucratic shortcut whereby the ethical committee states that the project conforms to ICRP 62. Special attention is paid to the cases of contrast between the ICRP 62 and previously promulgated laws: dose thresholds in non-therapeutic research, research on pregnant women or in child-bearing age and the issue of the preliminary assessment of new radiopharmaceuticals on monkeys (recommended by ICRP 62 but strongly restricted by the law by decree 116/1992). As for studies with radiopharmaceuticals, the problem of the double authorization required by the Minister's Decree 21/11/1997 and by the memorandum 10/07/1997, n. 8 of the Ministry of Health is also discussed.The authors express the opinion that the Minister's Decree 21/11/1997, together with ICRP 62, render invalid all previous rules and regulations which contrast with them. The Minister's Decree 21/11/1997 and ICRP 62 are practical and exhaustive and offer the researchers and ethical committees precise and reliable guidelines. The paper aims to offer a contribution for the rationalization of the rules that regulate the field. To this regard, an accurate design of the future regulations, reflecting Euratom Directive 43/97 is expected by May 13, 2000. The authors' conclusion is that laws must regulate, not strangulate, clinical scientific research with ionizing radiations: incongruities, uncertainties and bureaucratic excesses must be corrected.

In two patients with achalasia we found intermittent, complete lower oesophageal sphincter (LES) relaxations at manometric evaluation. These patients had no weight loss, minimal oesophageal dilation, lower LES pressure and faster radionuclide oesophageal emptying when compared with other achalasia patients. Concurrent performing of radionuclide oesophageal emptying and manometry showed that the complete relaxations (CRs) were too short and functionally unsuccessful. Our findings suggest that these patients may be at an earlier disease stage and that intermittent CRs of LES may occasionally occur in achalasia.

It is well known that hypercholesterolemia is correlated with coronary atherosclerosis, but no definite information is available on its association with cerebrovascular atherosclerosis. We studied 10 young patients (age 3-32 years) with homozygous familial hypercholesterolemia (FH), together with 3 normal relatives as healthy controls. Extra- and intracranial Doppler examination, MRI and cerebral blood flow by SPECT and 99mTc-HM-PAO were performed on all. Six out of 10 patients already had signs and symptoms of coronary heart disease, but all patients were free from ischemic brain lesions, as small as detectable at MRI, and had normal cerebral blood flow. Two patients presented significant stenosis of the carotid arteries at Doppler examination. Young patients with homozygous FH have early and clinically evident coronary atherosclerosis, while overt disease in the cerebral district is delayed despite the extremely elevated plasma cholesterol concentration. This was also confirmed by the autopsy of two patients, who died after the study and whose cerebral arteries were totally free from atherosclerotic lesions. The age, at which flow-reducing atherosclerotic lesions develop in hypercholesterolemic patients, differs with regard to the arterial district involved.
