Proton resonance frequency shift (PRFS) MR thermometry is the most common method used in clinical thermal treatments because of its fast acquisition and high sensitivity to temperature. However, motion is the biggest obstacle in PRFS MR thermometry for monitoring thermal treatment in moving organs. This challenge arises because of the introduction of phase errors into the PRFS calculation through multiple methods, such as image misregistration, susceptibility changes in the magnetic field, and intraframe motion during MRI acquisition. Various approaches for motion correction have been developed for real-time, motion-robust, and volumetric MR thermometry. However, current technologies have inherent trade-offs among volume coverage, processing time, and temperature accuracy. These tradeoffs should be considered and chosen according to the thermal treatment application. In hyperthermia treatment, precise temperature measurements are of increased importance rather than the requirement for exceedingly high temporal resolution. In contrast, ablation procedures require robust temporal resolution to accurately capture a rapid temperature rise. This paper presents a comprehensive review of current cutting-edge MRI techniques for motion-robust MR thermometry, and recommends which techniques are better suited for each thermal treatment. We expect that this study will help discern the selection of motion-robust MR thermometry strategies and inspire the development of motion-robust volumetric MR thermometry for practical use in clinics., (© 2024 The Authors. MRI-guided microwave ablation (MWA) is a minimally invasive treatment for localized cancer. MR thermometry has been shown to be able to provide vital information for monitoring the procedure in real-time. However, MRI during active MWA can suffer from image quality degradation due to intermittent electromagnetic interference (EMI). A novel approach to correct for EMI-contaminated images is presented here to improve MR thermometry during clinical hepatic MWA. The method was applied to MR-thermometry images acquired during four MR-guided hepatic MWA treatments using a commercially available MRI-configured microwave generator system. During the treatments MR thermometry data acquisition was synchronized to respiratory cycle to minimize the impact of motion. EMI was detected and corrected using uncontaminated k-space data from nearby frames in k-space. Substantially improved temperature and thermal damage maps have been obtained and the treatment zone can be better visualized. Background: Magnetic resonance (MR) thermometry allows visualization of lesion formation in real-time during cardiac radiofrequency (RF) ablation. The present study was performed to evaluate the precision of MR thermometry without RF heating in patients exhibiting cardiac arrhythmia in a clinical setting. The evaluation relied on quantification of changes in temperature measurements caused by noise and physiological motion., Methods: Fourteen patients referred for cardiovascular magnetic resonance imaging underwent an extra sequence to test the temperature mapping stability during free-breathing acquisition. Phase images were acquired using a multi-slice, cardiac-triggered, single-shot echo planar imaging sequence. Temperature maps were calculated and displayed in real-time while the electrocardiogram (ECG) was recorded. The precision of temperature measurement was assessed by measuring the temporal standard deviation and temporal mean of consecutive temperature maps over a period of three minutes. The cardiac cycle was analyzed from ECG recordings to quantify the impact of arrhythmia events on the precision of temperature measurement. Finally, two retrospective strategies were tested to remove acquisition dynamics related either to arrhythmia events or sudden breathing motion., Results: ECG synchronization allowed categorization of inter-beat intervals (RR) into distinct beat morphologies. Five patients were in stable sinus rhythm, while nine patients showed irregular RR intervals due to ectopic beats. An average temporal standard deviation of temperature of 1.6°C was observed in patients under sinus rhythm with a frame rate corresponding to the heart rate of the patient. The temporal standard deviation rose to 2.5°C in patients with arrhythmia. The retrospective rejection strategies increased the temperature precision measurement while maintaining a sufficient frame rate., Conclusions: Our results indicated that real-time cardiac MR thermometry shows good precision in patients under clinical conditions, even in the presence of arrhythmia. Purpose: To perform multi-echo water/fat separated proton resonance frequency (PRF)-shift temperature mapping., Methods: State-of-the-art, iterative multi-echo water/fat separation algorithms produce high-quality water and fat images in the absence of heating but are not suitable for real-time imaging due to their long compute times and potential errors in heated regions. Existing fat-referenced PRF-shift temperature reconstruction methods partially address these limitations but do not address motion or large time-varying and spatially inhomogeneous B 0 shifts. We describe a model-based temperature reconstruction method that overcomes these limitations by fitting a library of separated water and fat images measured before heating directly to multi-echo data measured during heating, while accounting for the PRF shift with temperature., Results: Simulations in a mixed water/fat phantom with focal heating showed that the proposed algorithm reconstructed more accurate temperature maps in mixed tissues compared to a fat-referenced thermometry method. In a porcine phantom experiment with focused ultrasound heating at 1.5 Tesla, temperature maps were accurate to within 1 ∘ C of fiber optic probe temperature measurements and were calculated in 0.47 s per time point. Free-breathing breast and liver imaging experiments demonstrated motion and off-resonance compensation. Objectives: To develop, test and evaluate improved 2D and 3D protocols for proton resonance frequency shift magnetic resonance temperature imaging (MRTI) of laser interstitial thermal therapy (LITT). The objective was to develop improved MRTI protocols in terms of temperature measurement precision and volume coverage compared to the 2D MRTI protocol currently used with a commercially available LITT system., Methods: Four different 2D protocols and four different 3D protocols were investigated. The 2D protocols used multi-echo readouts to prolong the total MR sampling time and hence the MRTI precision, without prolonging the total acquisition time. The 3D protocols provided volumetric thermometry by acquiring a slab of 12 contiguous slices in the same acquisition time as the 2D protocols. The study only considered readily available pulse sequences (Cartesian 2D and 3D gradient recalled echo and echo planar imaging [EPI]) and methods (partial Fourier and parallel imaging) to ensure wide availability and rapid clinical implementation across vendors and field strengths. In vivo volunteer studies were performed to investigate and compare MRTI precision and image quality. Phantom experiments with LITT heating were performed to investigate and compare MRTI precision and accuracy. Different coil setups were used in the in vivo studies to assess precision differences between using local (such as flex and head coils) and non-local (i.e., body coil) receive coils. Studies were performed at both 1.5 T and 3 T., Results: The improved 2D protocols provide up to a factor of two improvement in the MRTI precision in the same acquisition time, compared to the currently used clinical protocol. The 3D echo planar imaging protocols provide comparable precision as the currently used 2D clinical protocol, but over a substantially larger field of view, without increasing the acquisition time. As expected, local receive coils perform substantially better than the body coil, and 3 T provides better MRTI accuracy and precision than 1.5 T. 3D data can be zero-filled interpolated in all three dimensions (as opposed to just two dimensions for 2D data), reducing partial volume effects and measuring higher maximum temperature rises., Conclusions: With the presented protocols substantially improved MRTI precision (for 2D imaging) or greatly improved field of view coverage (for 3D imaging) can be achieved in the same acquisition time as the currently used protocol. Most parameters that influence the magnetic resonance imaging (MRI) signal experience a temperature dependence. The fact that MRI can be used for non-invasive measurements of temperature and temperature change deep inside the human body has been known for over 30 years. Today, MR temperature imaging is widely used to monitor and evaluate thermal therapies such as radio frequency, microwave, laser, and focused ultrasound therapy. In this paper we cover the physical principles underlying the biological applications of MR temperature imaging and discuss practical considerations and remaining challenges. For biological tissue, the MR signal of interest comes mostly from hydrogen protons of water molecules but also from protons in, e.g., adipose tissue and various metabolites. Most of the discussed methods, such as those using the proton resonance frequency (PRF) shift, T 1 , T 2 , and diffusion only measure temperature change, but measurements of absolute temperatures are also possible using spectroscopic imaging methods (taking advantage of various metabolite signals as internal references) or various types of contrast agents. Currently, the PRF method is the most used clinically due to good sensitivity, excellent linearity with temperature, and because it is largely independent of tissue type. Because the PRF method does not work in adipose tissues, T 1 - and T 2 -based methods have recently gained interest for monitoring temperature change in areas with high fat content such as the breast and abdomen. Absolute temperature measurement methods using spectroscopic imaging and contrast agents often offer too low spatial and temporal resolution for accurate monitoring of ablative thermal procedures, but have shown great promise in monitoring the slower and usually less spatially localized temperature change observed during hyperthermia procedures. Much of the current research effort for ablative procedures is aimed at providing faster measurements, larger field-of-view coverage, simultaneous monitoring in aqueous and adipose tissues, and more motion-insensitive acquisitions for better precision measurements in organs such as the heart, liver, and kidneys. For hyperthermia applications, larger coverage, motion insensitivity, and simultaneous aqueous and adipose monitoring are also important, but great effort is also aimed at solving the problem of long-term field drift which gets interpreted as temperature change when using the PRF method., (Copyright © 2019 Elsevier B.V. Purpose: To explore the temperature-induced signal change in two-dimensional multi-spectral imaging (2DMSI) for fast thermometry near metallic hardware to enable MR-guided focused ultrasound surgery (MRgFUS) in patients with implanted metallic hardware., Method: 2DMSI was optimized for temperature sensitivity and applied to monitor focus ultrasound surgery (FUS) sonications near metallic hardware in phantoms and ex vivo porcine muscle tissue. Further, we evaluated its temperature sensitivity for in vivo muscle in patients without metallic hardware. In addition, we performed a comparison of temperature sensitivity between 2DMSI and conventional proton-resonance-frequency-shift (PRFS) thermometry at different distances from metal devices and different signal-to-noise ratios (SNR)., Results: 2DMSI thermometry enabled visualization of short ultrasound sonications near metallic hardware. Calibration using in vivo muscle yielded a constant temperature sensitivity for temperatures below 43 °C. For an off-resonance coverage of ± 6 kHz, we achieved a temperature sensitivity of 1.45%/K, resulting in a minimum detectable temperature change of ∼2.5 K for an SNR of 100 with a temporal resolution of 6 s per frame., Conclusion: The proposed 2DMSI thermometry has the potential to allow MR-guided FUS treatments of patients with metallic hardware and therefore expand its reach to a larger patient population. Background: Noninvasive magnetic resonance thermometry (MRT) at low-field using proton resonance frequency shift (PRFS) is a promising technique for monitoring ablation temperature, since low-field MR scanners with open-configuration are more suitable for interventional procedures than closed systems. In this study, phase-drift correction PRFS with first-order polynomial fitting method was proposed to investigate the feasibility and accuracy of quantitative MR thermography during hyperthermia procedures in a 0.35 T open MR scanner., Methods: Unheated phantom and ex vivo porcine liver experiments were performed to evaluate the optimal polynomial order for phase-drift correction PRFS. The temperature estimation approach was tested in brain temperature experiments of three healthy volunteers at room temperature, and in ex vivo porcine liver microwave ablation experiments. The output power of the microwave generator was set at 40 W for 330 s. In the unheated experiments, the temperature root mean square error (RMSE) in the inner region of interest was calculated to assess the best-fitting order for polynomial fit. For ablation experiments, relative temperature difference profile measured by the phase-drift correction PRFS was compared with the temperature changes recorded by fiber optic temperature probe around the microwave ablation antenna within the target thermal region., Results: The phase-drift correction PRFS using first-order polynomial fitting could achieve the smallest temperature RMSE in unheated phantom, ex vivo porcine liver and in vivo human brain experiments. In the ex vivo porcine liver microwave ablation procedure, the temperature error between MRT and fiber optic probe of all but six temperature points were less than 2 °C. Overall, the RMSE of all temperature points was 1.49 °C., Conclusions: Both in vivo and ex vivo experiments showed that MR thermometry based on the phase-drift correction PRFS with first-order polynomial fitting could be applied to monitor temperature changes during microwave ablation in a low-field open-configuration whole-body MR scanner.

This technical note describes an effective method for monitoring temperature based on the minimum curvature surface fitting (MCSF) referenceless proton resonance frequency shift (PRFs) at 0.35 T. Experimental tests on phantom, ex vivo porcine livers and human brain were conducted. A comparison study between the proposed method and a fiber optic temperature probe was conducted for microwave (MW) ablation. Experimental results showed that temperature root mean square error (RMSE) calculated by MCSF model were lower than those computed by high-order polynomial fitting. For ablation experiments, the temperature errors between temperature probe and MCSF method were all less than 2∘C.

Purpose: To investigate the temperature dependence of the resonance frequency of lipid-dissolved xenon (LDX) and to assess the accuracy of LDX-based MR thermometry., Methods: The chemical shift temperature dependence of water protons, methylene protons, and LDX was measured from samples containing tissues with varying fat contents using a high-resolution NMR spectrometer. LDX results were then used to acquire relative and absolute temperature maps in vivo and the results were compared with PRF-based MR thermometry., Results: The temperature dependence of proton resonance frequency (PRF) is strongly affected by the specific distribution of water and fat. A redistribution of water and fat compartments can reduce the apparent temperature dependence of the water chemical shift from -0.01 ppm/°C to -0.006 ppm, whereas the LDX chemical shift shows a consistent temperature dependence of -0.21 ppm/°C. The use of the methylene protons resonance frequency as internal reference improves the accuracy of LDX-based MR thermometry, but degrades that of PRF-based MR thermometry, as microscopic susceptibility gradients affected lipid and water spins differently., Conclusion: The LDX resonance frequency, with its higher temperature dependence, provides more accurate and precise temperature measurements, both in vitro and in vivo. More importantly, the resonance frequency of nearby methylene protons can be used to extract absolute temperature information. Purpose: Dual-pathway sequences have been proposed to help improve the temperature-to-noise ratio (TNR) in MR thermometry. The present work establishes how much of an improvement these so-called "PSIF-FISP" sequences may bring in various organs and tissues., Methods: Simulations and TNR calculations were validated against analytical equations, phantom, abdomen, and brain scans. Relative TNRs for PSIF-FISP, as compared to a dual-FISP reference standard, were calculated for flip angle (FA) = 1 to 85 º and repetition time (TR) = 6 to 60 ms, for gray matter, white matter, cervix, endometrium, myometrium, prostate, kidney medulla and cortex, bone marrow, pancreas, spleen, muscle, and liver tissues., Results: PSIF-FISP was TNR superior in the kidney, pelvis, spleen, or gray matter at most tested TR and FA settings, and benefits increased at shorter TRs. PSIF-FISP was TNR superior in other tissues, e.g., liver, muscle, pancreas, for only short TR settings (20 ms or less). The TNR benefits of PSIF-FISP increased slightly with FA, and strongly with decreasing TR. Up to two- to three-fold reductions in TR with 20% TNR gains were achievable. In any given tissue, TNR performance is expected to further improve with heating, due to changes in relaxation rates., Conclusion: Dual-pathway PSIF-FISP can improve TNR and acquisition speed over standard gradient-recalled echo sequences, but optimal acquisition parameters are tissue dependent. Purpose: The lack of a technique for MR thermometry near metal excludes a growing patient population from promising treatments such as MR-guided focused ultrasound therapy. Here we explore the feasibility of multispectral imaging (MSI) for noninvasive temperature measurement in the presence of strong field inhomogeneities by exploiting the temperature dependency of the T 1 relaxation time., Methods: A two-dimensional inversion-recovery-prepared MSI pulse sequence (2DMSI) was implemented for artifact-reduced T 1 mapping near metal. A series of T 1 maps was acquired in a metallic implant phantom while increasing the phantom temperature. The measured change in T 1 was analyzed with respect to the phantom temperature. For comparison, proton resonance frequency shift (PRFS) thermometry was performed., Results: 2DMSI achieved artifact-reduced, single-slice T 1 mapping in the presence of strong off-resonance with a spatial resolution of 1.9 mm in-plane and a temporal resolution of 5 min. The maps enabled temperature measurements over a range of 30°C with an uncertainty below 1.4°C. The quality of the resulting temperature maps was independent of the distance from the metal, whereas the PRFS-based temperature measurements were increasingly impaired with increasing off-resonance., Conclusion: We demonstrated the ability to noninvasively measure temperature near metal using MSI and the T 1 temperature sensitivity. Purpose: Introduce a novel compressed sensing reconstruction method to accelerate proton resonance frequency shift temperature imaging for MRI-induced radiofrequency heating evaluation., Methods: A compressed sensing approach that exploits sparsity of the complex difference between postheating and baseline images is proposed to accelerate proton resonance frequency temperature mapping. The method exploits the intra-image and inter-image correlations to promote sparsity and remove shared aliasing artifacts. Validations were performed on simulations and retrospectively undersampled data acquired in ex vivo and in vivo studies by comparing performance with previously published techniques., Results: The proposed complex difference constrained compressed sensing reconstruction method improved the reconstruction of smooth and local proton resonance frequency temperature change images compared to various available reconstruction methods in a simulation study, a retrospective study with heating of a human forearm in vivo, and a retrospective study with heating of a sample of beef ex vivo., Conclusion: Complex difference based compressed sensing with utilization of a fully sampled baseline image improves the reconstruction accuracy for accelerated proton resonance frequency thermometry. An MR thermometry method is proposed for measuring in vivo small temperature changes engendered by external RF heat sources. The method relies on reproducible and stable respiration and therefore currently applies to ventilated animals whose breathing is carefully controlled. It first consists in characterizing the stability of the main magnetic field as well as the variations induced by breathing during a first monitoring stage. Second, RF heating is applied while the phase and thus temperature evolutions are continuously measured, the corrections due to breathing and field drift being made thanks to the data accumulated during the first period. The RF heat source is finally stopped and the temperature rise likewise is continuously monitored during a third and last stage to observe the animal cooling down and to validate the assumptions made for correcting for the main field variation and the physiological noise. Experiments were performed with a clinical 7 T scanner on an anesthetized baboon and with a dedicated RF heating setup. Purpose: The present study aimed to evaluate magnetic resonance (MR) thermometry using proton resonance frequency shift (PRFS) during laser-induced thermotherapy (LITT), and to compare the results of using different sequences at a field strength of 7-Tesla to identify the optimal for use in ablation so that the surrounding healthy tissues may be protected from damaging in real time. Materials and methods: LITT was applied to agarose gel phantoms and ex-vivo porcine brains. We reconstructed both magnitude and phase images to perform MR thermometry based on PRFS methods. We tested four different sequences: a gradient-echo (GRE), a segmented gradient-echo echoplanar imaging (EPI-GRE), a fast-low angle shot (FLASH), and a true fast imaging with steady precession (TRUFI). Temperature was monitored and verified using a fiber-optic thermometry device. Results: All sequences showed good linear correlations (R = 0.97–0.99) between the measured temperature and the calculated MR-thermometry measurements. The phantom/porcine brain experiments revealed the temperature precisions at 1.53/0.69 °C (GRE), 0.61/0.43 °C (EPI-GRE), 1.64/1.32 °C (FLASH), and 0.58/1.52 °C (TRUFI), respectively. Furthermore, we performed a Bland–Altman analysis and the temperature accuracies were found to be − 1.32/− 0.60 °C (GRE), 0.42/− 0.33 °C (EPI-GRE), − 1.28/− 0.98 °C (FLASH), and 0.14/0.46 °C (TRUFI) in the phantom/porcine brain experiments, respectively. Conclusions: Our experiments recommend that EPI-GRE sequence be the best of the all sequences for MR temperature imaging with PRFS in the LITT on 7 T magnetic resonance imaging (MRI) systems because of its relatively higher precision and accuracy. Magnetic resonance imaging (MRI) has become a popular modality in guiding minimally invasive thermal therapies, due to its advanced, nonionizing, imaging capabilities and its ability to record changes in temperature. A variety of MR thermometry techniques have been developed over the years, and proton resonance frequency (PRF) shift thermometry is the current clinical gold standard to treat a variety of cancers. It is used extensively to guide hyperthermic thermal ablation techniques such as high‐intensity focused ultrasound (HIFU) and laser‐induced thermal therapy (LITT). Essential attributes of PRF shift thermometry include excellent linearity with temperature, good sensitivity, and independence from tissue type. This noninvasive temperature mapping method gives accurate quantitative measures of the temperature evolution inside biological tissues. In this review, the current status and new developments in the fields of MR‐guided HIFU and LITT are presented with an emphasis on breast, prostate, bone, uterine, and brain treatments. Purpose: To investigate k-space subsampling strategies to achieve fast, large field-of-view (FOV) temperature monitoring using segmented echo planar imaging (EPI) proton resonance frequency shift thermometry for MR guided high intensity focused ultrasound (MRgHIFU) applications. Methods: Five different k-space sampling approaches were investigated, varying sample spacing (equally vs nonequally spaced within the echo train), sampling density (variable sampling density in zero, one, and two dimensions), and utilizing sequential or centric sampling. Three of the schemes utilized sequential sampling with the sampling density varied in zero, one, and two dimensions, to investigate sampling the k-space center more frequently. Two of the schemes utilized centric sampling to acquire the k-space center with a longer echo time for improved phase measurements, and vary the sampling density in zero and two dimensions, respectively. Phantom experiments and a theoretical point spread function analysis were performed to investigate their performance. Variable density sampling in zero and two dimensions was also implemented in a non-EPI GRE pulse sequence for comparison. All subsampled data were reconstructed with a previously described temporally constrained reconstruction (TCR) algorithm. Results: The accuracy of each sampling strategy in measuring the temperature rise in the HIFU focal spot was measured in terms of the root-mean-square-error (RMSE) compared to fully sampled "truth." For the schemes utilizing sequential sampling, the accuracy was found to improve with the dimensionality of the variable density sampling, giving values of 0.65 °C, 0.49 °C, and 0.35 °C for density variation in zero, one, and two dimensions, respectively. The schemes utilizing centric sampling were found to underestimate the temperature rise, with RMSE values of 1.05 °C and 1.31 °C, for variable density sampling in zero and two dimensions, respectively. Similar subsampling schemes with variable density sampling implemented in zero and two dimensions in a non-EPI GRE pulse sequence both resulted in accurate temperature measurements (RMSE of 0.70 °C and 0.63 °C, respectively). With sequential sampling in the described EPI implementation, temperature monitoring over a 192 x 144 x 135 mm3 FOV with a temporal resolution of 3.6 s was achieved, while keeping the RMSE compared to fully sampled "truth" below 0.35 °C. Conclusions: When segmented EPI readouts are used in conjunction with k-space subsampling for MR thermometry applications, sampling schemes with sequential sampling, with or without variable density sampling, obtain accurate phase and temperature measurements when using a TCR reconstruction algorithm. Object: To develop an ultrafast MRI-based temperature monitoring method for application during rapid ultrasound exposures in moving organs. Materials and methods: A slice selective 90° − 180° pair of RF pulses was used to solicit an echo from a column, which was then sampled with a train of gradient echoes. In a gel phantom, phase changes of each echo were compared to standard gradient-echo thermometry, and temperature monitoring was tested during focused ultrasound sonications. Signal-to-noise ratio (SNR) performance was evaluated in vivo in a rabbit brain, and feasibility was tested in a human heart. Results: The correlation between each echo in the acquisition and MRI-based temperature measurements was good ( R = 0.98 ± 0.03). A temperature sampling rate of 19 Hz was achieved at 3T in the gel phantom. It was possible to acquire the water frequency in the beating heart muscle with 5-Hz sampling rate during a breath hold. Conclusion: Ultrafast thermometry via phase or frequency monitoring along single columns was demonstrated. With a temporal resolution around 50 ms, it may be possible to monitor focal heating produced by short ultrasound pulses. [ABSTRACT FROM AUTHOR]

Purpose: A review of MRI temperature imaging methods based on intermolecular multiple quantum coherences (iMQCs) is presented. Temperature imaging based on iMQCs can provide absolute temperature maps that circumvent the artefacts that other proton frequency shift techniques suffer from such as distortions to the detected temperature due to susceptibility changes and magnetic field inhomogeneities. Thermometry based on iMQCs is promising in high-fat tissues such as the breast, since it relies on the fat signal as an internal reference. This review covers the theoretical background of iMQCs, and the necessary adaptations for temperature imaging using iMQCs. Materials and methods: Data is presented from several papers on iMQC temperature imaging. These studies were done at 7T in both phantoms and in vivo. Results from phantoms of cream (homogeneous mixture of water and fat) are presented as well as in vivo temperature maps in obese mice. Results: Thermometry based on iMQCs offers the potential to provide temperature maps which are free of artefacts due to susceptibility and magnetic field inhomogeneities, and detect temperature on an absolute scale. Conclusions: The data presented in the papers reviewed highlights the promise of iMQC-based temperature imaging in fatty tissues such as the breast. The change in susceptibility of fat with temperature makes standard proton frequency shift methods (even with fat suppression) challenging and iMQC-based imaging offers an alternative approach. Purpose: MR thermometry based on the proton resonance frequency shift (PRFS) is the most commonly used method for the monitoring of thermal therapies. As the chemical shift of water protons is temperature dependent, the local temperature variation (relative to an initial baseline) may be calculated from time-dependent phase changes in gradient-echo (GRE) MR images. Dynamic phase shift in GRE images is also produced by time-dependent changes in the magnetic bulk susceptibility of tissue. Gas bubbles (known as “white cavitation”) are frequently visualized near the RF electrode in ultrasonography-guided radio frequency ablation (RFA). This study aimed to investigate RFA-induced cavitation’s effects by using simultaneous ultrasonography and MRI, to both visualize the cavitation and quantify the subsequent magnetic susceptibility-mediated errors in concurrent PRFS MR-thermometry (MRT) as well as to propose a first-order correction for the latter errors. Methods: RF heating in saline gels and in ex vivo tissues was performed with MR-compatible bipolar and monopolar electrodes inside a 1.5 T MR clinical scanner. Ultrasonography simultaneous to PRFS MRT was achieved using a MR-compatible phased-array ultrasonic transducer. PRFS MRT was performed interleaved in three orthogonal planes and compared to measurements from fluoroptic sensors, under low and, respectively, high RFA power levels. Control experiments were performed to isolate the main source of errors in standard PRFS thermometry. Results: Ultrasonography, MRI and digital camera pictures clearly demonstrated generation of bubbles every time when operating the radio frequency equipment at therapeutic powers (>=30 W). Simultaneous bimodal (ultrasonography and MRI) monitoring of high power RF heating demonstrated a correlation between the onset of the PRFS-thermometry errors and the appearance of bubbles around the applicator. In an ex vivo study using a bipolar RF electrode under low power level (5 W), the MR measured temperature curves accurately matched the reference fluoroptic data. In similar ex vivo studies when applying higher RFA power levels (30 W), the correlation plots of MR thermometry versus fluoroptic data showed large errors in PRFS-derived temperature (up to 45 °C absolute deviation, positive or negative) depending not only on fluoroptic tip position but also on the RF electrode orientation relative to the B0 axis. Regions with apparent decrease in the PRFS-derived temperature maps as much as 30 °C below the initial baseline were visualized during RFA high power application. Ex vivo data were corrected assuming a Gaussian dynamic source of susceptibility, centered in the anode/cathode gap of the RF bipolar electrode. After correction, the temperature maps recovered the revolution symmetry pattern predicted by theory and matched the fluoroptic data within 4.5 °C mean offset. Conclusions: RFA induces dynamic changes in magnetic bulk susceptibility in biological tissue, resulting in large and spatially dependent errors of phase-subtraction-only PRFS MRT and unexploitable thermal dose maps. These thermometry artifacts were strongly correlated with the appearance of transient cavitation. A first-order dynamic model of susceptibility provided a useful method for minimizing these artifacts in phantom and ex vivo experiments. [ABSTRACT FROM AUTHOR]

Abstract: A model-based proton resonance frequency shift (PRFS) thermometry method was developed to significantly reduce the temperature quantification errors encountered in the conventional phase mapping method and the spatiotemporal limitations of the spectroscopic thermometry method. Spectral data acquired using multi-echo gradient echo (GRE) is fit into a two-component signal model containing temperature information and fat is used as the internal reference. The noniterative extended Prony algorithm is used for the signal fitting and frequency estimate. Monte Carlo simulations demonstrate the advantages of the method for optimal water-fat separation and temperature estimation accuracy. Phantom experiments demonstrate that the model-based method effectively reduces the interscan motion effects and frequency disturbances due to the main field drift. The thermometry result of ex vivo goose liver experiment with high intensity focused ultrasound (HIFU) heating was also presented in the paper to indicate the feasibility of the model-based method in real tissue. The Larmor frequency of water protons has reliably linear temperature dependence. Since this frequency shift is easily measurable using relatively simple MRI techniques, a remarkable opportunity arises for uniquely non-invasive and accurate temperature evaluation, deep within any water-containing object. Major applications are appearing in the field of image-guided surgery. The cutting-edge papers collected in this Special Issue demonstrate both the versatility and the power of MRI thermometry.
