The main purpose of this pictorial review is to highlight the important MR imaging findings of various conditions involving the cavernous sinus in addition to brief description of normal anatomy. The pathological conditions that can involve the cavernous sinus can be categorized into infective, inflammatory, granulomatous, vascular and neoplastic causes. Imaging, especially with MRI, plays an important role not only in detection but also in definition of disease extent and in characterization of the pathology. This article discusses a rare case of acute necrotizing encephalopathy (ANE) in a 20-year-old laborer with no prior health issues. The patient presented with a high-grade fever, seizures, and altered behavior. Imaging revealed multiple extensive hemorrhagic lesions in the brain, and a diagnosis of ANE, likely post-viral, was made. Despite treatment, the patient experienced cardiopulmonary arrest and died. ANE is a rare neurological complication associated with viral infections, characterized by altered mental status and seizures. It has a rapid and severe clinical course, often leading to disability or death. The overall prognosis is poor, with a mortality rate of over 40%. [Extracted from the article]
