OBJECTIVE. The purpose of this article is to review the appearance of various abnormalities that affect the lumbar intervertebral disk and diskovertebral segment through anatomic-pathologic correlation in cadavers. CONCLUSION. Familiarity with the pathologic conditions in and around the intervertebral disk is important in recognizing such conditions as a potential source of symptoms. Background: To evaluate the associations between magnetic resonance imaging (MRI) findings and pain, disability and quality of life before surgery and up to 5 years after lumbar microdiscectomy., Materials and Methods: Sixty-one patients who underwent one-level lumbar microdiscectomy by the same surgeon participated in this analytic, observational, prospective study. Lumbar spine MRI was performed preoperatively and 5 years postoperatively. Pain, disability and quality of life were measured with VAS, ODI, Roland Morris and SF-36 pre- and up to 5 years postoperatively. Subsequently associations between radiological findings and clinical outcomes were recorded., Results: Before surgery patients with disc extrusion or sequestration, with increased thecal sac compression (d > 2/3), with Modic changes (MC) 2 and 3 on the operated level and Pfirrmann grades IV and V on the operated and both adjacent discs presented the worst preoperative clinical outcomes. MC preoperatively were not related with postoperative results, in contrast with the type of disc herniation and thecal sac compression. Preoperative Pfirrmann grade IV and V on the operated and both adjacent discs and postoperative MC 2 and 3 on the operated level were related to poor clinical outcomes 36-60 months post-discectomy., Conclusions: Extrusion or sequestration of the operated disc, increased compression of thecal sac, MC 2 and 3 on the operated level and Pfirrmann grades IV and V on the operated and adjacent discs were associated with the worst clinical outcomes. Background: Lumbar microdiscectomy is a common procedure with satisfactory results; however, postoperative events like progressive adjacent level degeneration and perineural fibrosis can contribute to long-term pain. The purpose of the study was to evaluate MRI changes 5 years after lumbar microdiscectomy and assess their association with clinical parameters., Materials and Methods: A prospective study enrolling 61 patients who underwent microdiscectomy. Changes between preoperative and postoperative MRI findings were recorded, and these findings were tested for associations with demographic, clinical and perioperative parameters. The measured imaging parameters were degeneration of the operated and adjacent discs and endplates, morphology of the disc herniation, facet joints arthritis and the presence of postoperative perineural fibrosis., Results: Statistically significant differences were found between preoperative and postoperative morphology of the operated disc, facet joints arthritis and degeneration of the operated and caudal adjacent disc. There were no differences between preoperative and postoperative disc degeneration of the superior adjacent disc and in degeneration of the operated and adjacent endplates. Postoperatively perineural fibrosis was common; however, thecal sac compression and nerve root impingement were reduced. Age at the time of surgery was the only parameter associated with postoperative changes., Conclusion: Five years after microdiscectomy, several postoperative MRI changes including operated disc's morphology, facet joints arthritis and degeneration of the operated and caudal adjacent disc were shown. Objective: The purpose of this study was to describe the normal anatomy and abnormalities of the lumbar intervertebral disk with sonography in cadaveric specimens and to correlate the sonographic findings with pathologic findings., Methods: Sonographic imaging with both 4.5- and 10-MHz linear array transducers was performed on 35 lumbar intervertebral disks in 13 human cadaveric spines. The cadaveric specimens were sectioned for anatomic and histopathologic evaluation. Findings on anatomic sections were correlated with the findings on corresponding sonographic images with respect to the sonographic appearance of disk components in 30 intervertebral disks., Results: High-resolution sonography with a 10-MHz frequency transducer enabled distinction of the nucleus pulposus from the annulus fibrosus and assessment of the echogenic characteristics of these structures. Sonography showed numerous fine linear echoes in the outer portion of the intervertebral disk in 26 (87%) of 30 specimens, which corresponded to the normal concentric arrangement of the fibers in the periphery of the annulus fibrosus. Amorphous areas of low echogenicity in the inner portion of the annulus fibrosus (n = 14, 47%) correlated with degenerative changes of the disk on corresponding microscopic sections. The nucleus pulposus appeared relatively isoechoic (n = 5, 17%) or hyperechoic (n = 4, 13%) to the annulus fibrosus. In degenerative disks (n = 21, 70%), the nucleus pulposus showed decreased echogenicity, and differentiation between the nucleus pulposus and annulus fibrosus was difficult., Conclusions: High-resolution sonography is a simple imaging method that can show the normal lumbar intervertebral disk and degenerative changes in appropriate subjects. Purpose: The purpose of this work was to describe the MR appearance of cartilaginous endplates (CEPs) with close anatomic correlation in cadavers derived from elderly subjects., Method: High-resolution MRI was performed on five cadaveric lumbar spines, and a total of 48 CEPs were studied with T1-weighted spin echo, T2-weighted fast spin echo, and fat-suppressed 3D spoiled GRASS gradient echo (SPGR) MR images. All specimens underwent anatomic sectioning, and gross anatomic findings were correlated with those of MRI., Results: Conventional MR images allowed gross morphologic evaluation of the integrity of the CEPs and demonstrated cartilaginous nodes. In all specimens, fat-suppressed 3D-SPGR images invariably improved visualization of the fine anatomic structures at the diskovertebral junction (p < 0.01). Various morphologic abnormalities of the CEPs demonstrated on MR images, including thinning, irregularity, erosions, cartilaginous defects, and Schmorl nodes, were confirmed on anatomic inspection., Conclusion: Results in our study indicate that MRI may delineate the normal anatomy of CEPs and demonstrate morphologic changes occurring at the diskovertebral junction. Dedicated high-resolution technique and fat-suppressed 3D-SPGR images may significantly improve the diagnostic capabilities of MRI of this particular anatomic region. more...

Vertebral body compression fractures can cause chronic pain and may result in progressive kyphosis. Although vertebroplasty has been used to treat pain, it does not attempt to restore vertebral body height and eliminate spinal deformity. Percutaneous balloon kyphoplasty is a novel technique, which involves the introduction of inflatable bone tamps into the fractured vertebral body for elevation of the endplates, prior to fixation of the fracture with bone cement. Our initial experience with this minimally invasive procedure indicates that percutaneous balloon kyphoplasty can be efficacious in the treatment of painful, osteoporotic vertebral compression fractures. more...
