Background: The large number of articulating joints within the spinal column provides an abundance of options to control its movement. However, the ability of individuals to consciously manipulate these movement options is poorly understood., Objectives: To determine if short-term training can improve the ability to consciously dissociate motion between the pelvis and thorax during repetitive pelvic tilting movements., Design: Cross-over design with young healthy individuals., Method: Seventeen participants performed trials consisting of 35 continuous lift/lowers followed by 35 continuous anterior/posterior pelvic tilts while spine kinematics were recorded. Participants then underwent a 20-min training protocol designed to improve the control of pelvic motion and in particular the dissociation of pelvic and trunk motion. Post-training, the continuous pelvic tilt and lift/lower trials were repeated. Thorax-pelvis movement coordination was analyzed via vector coding and lumbar spine local dynamic stability was analyzed via Lyapunov exponents. Participants were grouped as being either high or low skill movers based on their ability to perform the pre-training pelvic tilt movements., Results: The low skill movement group demonstrated statistically significant increases in the time spent using in-phase pelvic dominant (p = 0.028) and anti-phase pelvic dominant (p = 0.043) coordination patterns during the pelvic tilt movements after the completion of the training protocol. The high skill movement group showed no differences in their movement patterns post-training., Conclusions: Short-term training, targeted to improve the ability to dissociate pelvic from thorax motion, had a beneficial effect on the group of individuals who initially lacked skill performing the pelvic tilting task., (Copyright © 2024 Elsevier Ltd. Background: Sacroiliac joint (SIJ) motion has been documented using invasive and noninvasive kinematic techniques. No study has explored SIJ angular positions in functional postures using noninvasive techniques. The purpose of this study was to quantify SIJ positioning among different seated postures in a healthy population., Methods: Twelve female and 11 male healthy young participants participated. Left and right anterior and posterior superior iliac spines were manually digitized during standing, neutral sitting and four different seated postures. Rigid bodies recorded the kinematics of the lumbar spine. Angles calculated included transverse sacroiliac angle, innominate sagittal angle, sacral tilt, lumbar flexion-extension, lumbar lateral bend and lumbar axial twist., Findings: The observed range of angular positions was approximately 3 to 4 degrees across the SIJ-related angles. The main effect of seated posture was observed for all angles measured. The main effect of sex was observed for all angles except lumbar lateral bending. Females consistently experienced more posterior sacral tilt than males. Interaction effects between sex and posture were only observed at the right-transverse sacroiliac angle and sacral tilt. Previous sitting posture affected the subsequent neutral sitting posture for the right-transverse sacroiliac angle and lumbar spine angle., Interpretation: SIJ angular position differences among the seated postures were similar in magnitude to motions previously reported in participants undergoing prone passive hip abduction and external rotation. Sex differences, including greater sacral posterior tilt observed in females, likely reflect underlying morphological and physiological differences. Future studies should explore SIJ positioning during functional tasks in pathological populations to help elucidate the underlying causes of SIJ pain and inform treatment strategies.

Auditory metronomes have been used to preserve movement consistency when examining local dynamic stability (LDS) and coordination variability (CV) of lumbar spine motion during repetitive movements. However, the potential influence of the metronome itself on these outcome measures has rarely been considered. Therefore, this study investigated the influence of different metronome paces (i.e., lifting speeds) on measures of lumbar spine LDS and thorax-pelvis CV during a repetitive lifting/lowering task in comparison to self-paced movements. Ten participants completed 5 repetitive lift/lower trials, where participants completed 35 consecutive repetitions (analysis on last 30 repetitions) at a self-selected pace for the first and last trial, and were paced by a 10 lift/min, 15 lift/min, and 20 lift/min metronome, in randomized order, for the remaining three trials. The average self-paced lift/lower speed before and after experiencing the three different metronome paced speeds was 16.2 (±1.02) and 17.2 (±0.73) lifts/min, respectively, and the most-preferred metronome pace trial was 15 lifts/min. Thorax-pelvis CV during the self-paced trials were similar (p > 0.05) to the 15 lift/min metronome paced trials, while greater thorax-pelvis CV was observed for the 10 lift/min compared to the 15 lift/min and 20 lift/min and second self-paced trial (all p < 0.026). This movement speed effect was not observed for lumbar spine LDS; however, more-dynamically stable movements were observed during all metronome paced trials in comparison to the self-paced trials. The association between low back pain and lumbar spine local dynamic stability (LDS) appears to be modulated by if and how someone catastrophizes about pain, suggesting that the cognitive perceptions of pain may influence an individual's ability to control lumbar spine motion. Previous work also demonstrates that directing cognitive resources and attentional focus can influence movement performance. Therefore, we aimed to examine whether distracting attentional focus would influence lumbar spine LDS during repetitive flexion-extension movements. Sixteen participants performed repetitive spine flexion-extension movements under two baseline conditions (pre- and post-), and while attentional focus was distracted by either an external sensory stimulus or a cognitive-motor dual-task, both targeted at the hands. Lumbar spine LDS was examined over 30 continuous movement repetitions using maximum Lyapunov exponents. In comparison to both Baseline and Post-Baseline trials, the perceived mental workload was significantly elevated during the cognitive-motor dual-task trial but not the external sensory stimulus trial. The only statistically significant effect on LDS occurred in the Post-Baseline trial, where LDS was higher than in the cognitive-motor dual-task. In combination with previous work, these findings suggest that distracting attentional focus during repetitive lumbar spine flexion-extension movements does not have a negative influence on lumbar spine LDS.

This study investigated feasibility of imaging lumbopelvic musculature and geometry in tandem using upright magnetic resonance imaging (MRI) in asymptomatic adults, and explored the effect of pelvic retroversion on lumbopelvic musculature and geometry. Six asymptomatic volunteers were imaged (0.5 T upright MRI) in 4 postures: standing, standing pelvic retroversion, standing 30° flexion, and supine. Measures included muscle morphometry [cross-sectional area (CSA), circularity, radius, and angle] of the gluteus and iliopsoas, and pelvic geometry [pelvic tilt (PT), pelvic incidence (PI), sacral slope (SS), L3-S1 lumbar lordosis (LL)] L3-coccyx. With four volunteers repeating postures, and three raters assessing repeatability, there was generally good repeatability [ICC(3,1) 0.80-0.97]. Retroversion had level dependent effects on muscle measures, for example gluteus CSA and circularity increased (up to 22%). Retroversion increased PT, decreased SS, and decreased L3-S1 LL, but did not affect PI. Gluteus CSA and circularity also had level-specific correlations with PT, SS, and L3-S1 LL. Overall, upright MRI of the lumbopelvic musculature is feasible with good reproducibility, and the morphometry of the involved muscles significantly changes with posture. Prolonged or repetitive spine flexion induces creep deformation of posterior spine tissues allowing for increased intervertebral motion beyond 'normal' limits, which may influence sub-regional (intersegmental) spine motion during subsequent manual lifting tasks. Using spine skin-surface kinematics, intersegmental lumbar spine motion was recorded over 20 minutes of prolonged static spine flexion and a subsequent manual lifting task (2 lifts every 3 minutes, 30 minutes total) in 14 participants. Results demonstrated that mid to lower lumbar intersegmental levels (i.e. L 2 /L 3 to L 4 /L 5 ) experienced the greatest overall creep deformation and range of motion during both prolonged flexion and manual lifting; however, overall range of motion during manual lifting was unaffected. Additionally, creep deformation did not completely recover within 30 minutes. Future work should continue to investigate the influence of this residual creep, as well as how overall creep deformation impacts spine neuromuscular control and stability, and ultimately the development of low back disorders. Practitioner summary: Mid to lower lumbar spine levels (i.e. L 2 /L 3 to L 4 /L 5 ) experienced the greatest creep deformation and range of motion during both prolonged flexion and manual lifting. Repeated lifting following prolonged flexion may limit creep recovery; however, overall lifting kinematic motion remained unchanged.

Background: Walking is often considered a beneficial management strategy for certain populations of low back pain patients. However, little is known about how simple challenges that people often encounter, such as carrying loads in the hands, affect the low back during walking., Research Question: How do variations in hand loading affect arm swing, lumbar spine range of motion (ROM), and lumbar spine local dynamic stability (LDS) during walking?, Methods: Sixteen young healthy participants (8 female) performed nine treadmill walking trials, each at 1.25 m/s for 3 consecutive minutes. Conditions manipulated the magnitude of hand loads (unloaded, low, high) and location of hand loads (directly in hands, in bags). Kinematic markers were used to measure sagittal plane arm swing, 3D lumbar spine ROM, and lumbar spine LDS during each trial., Results: Arm swing was significantly (p < 0.001) reduced as load increased directly in the hands; however, when held in bags load magnitude had no effect. Further, arm swing was significantly (p < 0.0001) lower when loads were held in bags. Lumbar flexion/extension ROM was greatest with the low load compared to both unloaded (p = 0.012) and high load (p = 0.0717) conditions, and was also greater (p < 0.0001) with loads held directly in the hands compared to loads in bags. Despite these changes in lumbar spine ROM, lumbar spine LDS was not significantly affected by any of the variations in hand loading., Significance: The greater lumbar spine cyclic motion, elicited by low hand loads held directly in the hands during walking, may be beneficial to the health of the low back. No changes in lumbar LDS were found, thereby suggesting that the small, likely beneficial, increases in lumbar spine ROM are well controlled by the motor control system and do not create an increased risk of injury., (Copyright © 2018 Elsevier B.V. Given the appropriate cues, kinematic factors associated with low back injury risk and pain, such as spine flexion, can be avoided. Recent research has demonstrated the potential for tactile sensory information to change movement. In this study an athletic strapping tape was applied bilaterally along the lumbar extensor muscles to provide continuous tactile feedback information during a repeated lifting and lowering task. The presence of the tape resulted in a statistically significant reduction in lumbar spine flexion when compared to a baseline condition in which no tape was present. This reduction was further increased with the explicit instruction to pay attention to the sensations elicited by the tape. In both cases, the reduction in lumbar spine flexion was compensated for by increases in hip and knee flexion. When the tape was then removed and participants were instructed to continue lifting as if it was still present, the reduction in lumbar flexion and increases in hip and knee flexion were retained. Thus this study provides evidence that tactile cues can provide vital feedback information that can cue human lumbar spine movement to reduce kinematic factors associated with injury risk and pain., (Copyright © 2018 Elsevier B.V. A novel method of recording intersegmental spine kinematics was developed. The method was required to: (1) have similar accuracy and precision as current methods that record gross spine kinematics; (2) be reasonably insensitive to errors associated with marker detection or misplacement; and (3) be reasonably insensitive to skin movement artefacts. Four healthy participants performed trunk flexion, lateral bending, and axial twist movements; data were collected using the intersegmental method as well as electromagnetic sensors. Comparing methods, gross angular kinematic differences were within 1 SD during flexion and lateral bend, while axial twist resulted in the largest differences. To test sensitivity to marker error, random error was added to marker positions. The most proximal and distal intersegmental units were the most sensitive to marker error. Adding additional markers at the ends or interpolating padded markers reduced this sensitivity. The influence of skin movement artefact was investigated by digitizing locations of the skin with respect to the spinous processes in both neutral and fully flexed postures. In the lumbar region, large skin artefacts had minimal effect on intersegmental angles. The greatest strength of this method is the ability to dynamically record intersegmental spine kinematics.

Study Design: Basic science study of the relationship between the structural properties of the spine and its surrounding musculature., Objective: To determine whether an increase in spine stiffness causes an inverse compensatory change in the passive stiffness of the adjacent paraspinal muscles., Summary of Background Data: Intervertebral disc degeneration causes an increase in multifidus passive stiffness; this was hypothesized to compensate for a decrease in spine stiffness associated with disc degeneration. Mice lacking equilibrative nucleoside transporter 1 (ENT1) develop progressive ectopic calcification of the fibrous connective tissues of the spine, which affects the lumbar spine by 6 months of age and likely creates a mechanically stiffer spine., Methods: Experiments were conducted on four groups of mice (n = 8 mice/group): wild-type (WT) and ENT1 knockout (KO) at 2 or 8 months of age. Lumbar spines were removed and tested in cyclic axial compression to determine neutral zone length and stiffness. Single muscle fibers and bundles of fibers were isolated from lumbar multifidus and erector spinae, as well as tibialis anterior (a non-spine-related control) and tested to determine elastic modulus (passive stiffness)., Results: At 2 months of age, neither spine nor muscle stiffness was different between KO and WT. At 8 months of age, compared with WT the lumbar spines of ENT1 KO mice had a stiffer and shorter neutral zone, and the paraspinal muscle fibers were less stiff; however, fiber bundles were not different. In addition, tibialis anterior was not different between KO and WT., Conclusion: This work has confirmed that calcification of spinal connective tissues in the ENT1 KO mouse results in a stiffened spine, whereas the concurrent decrease in muscle fiber elastic modulus in the adjacent paraspinal muscles suggests a direct compensatory relationship between the stiffness of the spine and the muscles that are attached to it., Level of Evidence: N/A.
