Background/aims: The aim of the study was evaluation of the morphology of the blood vessels, blood flow velocity and direction with Doppler ultrasound (D-US) and correlation with the relative liver parenchymal perfusion assessed by hepatic radionuclide angiography (HRA)., Methodology: Real-time, D-US and HRA were performed in 108 patients., Results: In patients with portal venous aneurysm, hepatopetal blood flow was increased, while portal perfusion did not differ from controls. In portal hypertensive patients, D-US detected dilatation of the portal system veins, with decreased blood flow. In comparison to the portal perfusion in controls and portal venous aneurysm, values were significantly (p < 0.01) lower in chronic active hepatitis and liver cirrhosis and differed between themselves (p < 0.01). In the groups of cirrhotic patients with esophageal varices, sclerosed esophageal varices, recanalized umbilical vein, partial portal thrombosis and cavernous portal vein with hepatofugal, hyperkinetic or slow blood flow, and very low velocities beside the thrombi, portal perfusion was lower (p < 0.01) than in controls, portal venous aneurysm, chronic active hepatitis and liver cirrhosis without collaterals. In complete thrombosis, minimal collateral flow was found with D-US, while HRA proved no portal supply., Conclusions: D-US and HRA are complementary for the estimation of various liver vascular disorders.

The aim of the study is evaluation of hepatocellular function, as well as morphology and patency of the biliary three of the liver transplants by dynamic hepatobiliary scintigraphy. The study was performed in 10 controls and 10 patients after orthotopic transplantation (up to two years). Sixty minutes dynamic acquisition (1 frame/min) was performed with scintillation camera after injection of 360 MBq 99mTc-diethyl- IDA. Hepatobiliary scintigrams were analysed for morphology, and parenchymal and hepatobiliary TA curves were generated and analysed as regard to the time to maximal acitivity (Tmax) and the time to half of maximum acitivity (T1/2). Uptake of the radiopharmaceutical was slightly but not significantly delayed (Tmax=18.5 +/- 2.9 min) in comparison to the controls (Tmax=14.2 +/- 3.4min), while excretion was significantly prolonged (T1/2=59.5 +/- 12.1 min) than physiological (Tmax=34.2 +/- 4.1min). Intrahepatic bile flow was nonsignificantly prolonged (Tmax=31.3 +/- 3.7 min) in comparison to the controls (Tmax=25.7 +/- 3.5 min) while extrahepatic one is high significantly prolonged (T1/2=89.0 +/- 14.3 min) than physiological (T1/2 =45.0 +/- 7.2 min). Biliary phase of hepatobiliary scintigraphy showed increased accumulation of radiopharmaceutical in the left (n=1) or right (n=2) hepatic duct. Radionuclide methods are noninvasive, and apear to be sensitive and valuable for the monitoring of liver transplants.

Unlabelled: Aim of the study was to determine the time needed for establishing maximal labelled red blood cell concentration in hemangioma by scintigraphic semiquantitative blood pool estimation as a function of time., Patients and Methods: Eleven patients (eight females and three males) with total number of fourteen liver hemangiomas have been examined. Nine of them had solitary lesions and remaininig two patients had two and three liver hemangiomas, respectively. All patients underwent blood pool scintigraphy 40, 60, 120 and 180 minutes after in vivo labelling of autologous red blood cells using 740 MBq of 99mTc. After correction for radioactive decay of 99mTc and back-ground correction blood pool indexes, as hemangioma/heart and liver/heart counting rate ratios, have been calculated., Results: Mean blood pool indexes obtained 40-180 minutes after in vivo red blood cell labelling did not differ significantly (p > 0.05) neither in hemangioma (0.84-0.86) nor in liver tissue (0.55-0.58). In every acquisition time hemangioma blood pool index was substantialy higher than that of liver tissue (p < 0.01)., Conclusion: The results of the study indicates that labelled red blood cells concentration reaches its plateau before 40th minute following in vivo labelling. The concentration difference between hemangioma and liver tissue also does not differ significantly after 40th minute of blood pool examination. In most patients blood pool scintigrams taken 60, 120 and 180 minutes following in vivo RBC labelling do not contribute to diagnostic value of the method.

Background: The aim of the study was the evaluation of the perfusion, morphology and biliary three patency of liver transplants employing two radionuclide methods., Material and Methods: The study was performed on 10 controls and 10 patients after an orthotopic transplantation (up to two years). "First pass" dynamic acquisition was performed with a scintillation camera, after a bolus injection of 360 MBq 99mTc-diethyl- IDA, (60 frames/60s), continued by a 59 minute (1 frame/min) slower dynamic study. From the liver and kidney activity during the "first pass" study, the hepatic perfusion index (HPI) was calculated using slope-analysis. Hepatobiliary scintigrams obtained during second phase of the study were analyzed for morphology, and parenchymal and hepatobiliary TA curves were generated and analyzed according to the time to maximal activity (Tmax) and the time to half of maximum activity (T/2)., Results: In comparison to the controls (HPI, X = 0.64.5 +/- 0.05%) portal perfusion had slightly (X = 0.68 +/- 0.04%), but not significantly (p > 0.05) increased. In 3 patients, the biliary phase of hepatobiliary scintigraphy showed an increased accumulation of the radiopharmaceutical in the left (n = 1) or right (n = 2) hepatic duct. The uptake of the radiopharmaceutical (Tmax, X = 18.5 +/- 2.9 min) was slightly, but not significantly (p > 0.05) delayed in comparison to the controls (X = 14.2 +/- 3.4 min), while excretion was significantly (p < 0.05) prolonged (X = 59.5 +/- 12.1 min) (X = 34.2 +/- 4.1 min). Intrahepatic bile flow was insignificantly (p > 0.05) prolonged (X = 31.3 +/- 3.7 min) in comparison to the controls (X = 25.7 +/- 3.5 min) while extrahepatic bile flow was high, significantly (p < 0.01) prolonged (X = 89.0 +/- 14.3 min) than physiological one (X = 45.0 +/- 7.2 min)., Conclusions: Radionuclide methods are noninvasive, sensitive and valuable in monitoring liver transplants.

The correlation between chemical formulation and biodistribution of 99mTc-phytate solutions was examined. On the basis of radiochemical analyses and biological experiments, the following optimum parameters were found: weight ratio of the anhydrous sodium phytate to SnCl2 18:1; concentration of phytate 5-6 g/l, pH of the injection solution 6-7. Maximum uptakes in rat organs were in the liver 95.8 +/- 3.26 at 15-20 min p.i., and in the spleen 3.98 +/- 0.79% 3 hours p.i. Two new methods for radiochemical analysis enabling the separation of three peaks were used: ITLC-SG Gelman chromatography and molecular sieving through Sephadex G-25. The bench-life of the injection solution was also checked during a 5-h period, as well as its shelf-life which was at least 6 months. Clinical examinations showed excellent visualization of the normal liver with discrete radiocolloid uptake in the spleen and no uptake in other tissues. With SPECT a more precise and clear localization of liver lesions was achieved especially due to the low uptake of 99mTc-phytate in the spleen.
